Introduction: Currently, the impact of body mass index (BMI) on the outcomes of laparoscopic liver resections (LLR) is poorly defined. This study attempts to evaluate the impact of BMI on the peri-operative outcomes following laparoscopic left lateral sectionectomy (L-LLS). Methods: A retrospective analysis of 2183 patients who underwent pure L-LLS at 59 international centers between 2004 and 2021 was performed. Associations between BMI and selected peri-operative outcomes were analyzed using restricted cubic splines. Results: A BMI of >27kg/m2 was associated with increased in blood loss (Mean difference (MD) 21 mls, 95% CI 5–36), open conversions (Relative risk (RR) 1.13, 95% CI 1.03–1.25), operative time (MD 11 min, 95% CI 6–16), use of Pringles maneuver (RR 1.15, 95% CI 1.06–1.26) and reductions in length of stay (MD -0.2 days, 95% CI -0.3 to −0.1). The magnitude of these differences increased with each unit increase in BMI. However, there was a “U” shaped association between BMI and morbidity with the highest complication rates observed in underweight and obese patients. Conclusion: Increasing BMI resulted in increasing difficulty of L-LLS. Consideration should be given to its incorporation in future difficulty scoring systems in laparoscopic liver resections.

The evidence regarding programmed cell death 1 (PD-1) inhibitors on pancreatic ductal adenocarcinoma (PDAC) with metastases remains controversial. This study aimed to assess the efficacy and safety of Nab-paclitaxel plus S1 (NPS) with or without Sintilimab, a PD-1 inhibitor, in patients with PDAC with only hepatic metastases (mPDAC). Untreated mPDAC patients who received NPS with (the combination group) or without Sintilimab (the NPS group) were retrospectively studied. Surgery was considered when the pancreatic tumor became resectable or borderline resectable on radiological examinations, and with complete metabolic response of liver metastases. Between October 2017 and February 2020, 32 patients were in the combination group and 34 patients in the NPS group. Successful salvage resection was achieved in 17 (25.8%) patients after tumor-downstaging (combination 12 vs. NPS 5, P = 0.03). The median overall survival (OS) was 16.8 months in the combination group and 10.0 months in the NPS group (P = 0.002). Remarkable OS benefit was observed in patients with decline in CA19-9 of ≥ 50% (16.0 vs. 6.5, P = 0.003), reduction in 18F-fluorodeoxyglucose uptake of primary tumor of ≥ 50% (16.5 vs. 10.0, P

The technical complexity of robotic pancreaticoduodenectomy (RPD) and lack of technical surgical standardization have slowed its widespread application. RPD is only routinely performed in a few highly specialized centers. This study describes in detail the standard steps and core techniques of an experienced robotic center in China. We took advantage of our single experience to provide a step-by-step technique and surgical video of our RPD standardized procedure. We divided RPD into 18 key steps. Demographics and perioperative outcomes of consecutive 20 patients who underwent the RPD standardized procedure were analyzed. For the 20 consecutive patients, the mean operative time was 253.6 min, and the median estimated blood loss was 210.0 mL. One patient required conversion to laparotomy due to the need for PV reconstruction. One patient had grade 3 complication. The median postoperative hospital stay was 11.0 days. No 90-day mortality was observed. By simplifying and optimizing the surgical techniques, the RPD procedure can be standardized and modeled to improve feasibility and repeatability.

Background Malignant cerebral edema (MCE) is a common and feared complication after endovascular thrombectomy (EVT) in acute ischemic stroke (AIS). This study aimed to establish a nomogram to predict MCE in anterior circulation large vessel occlusion stroke (LVOS) patients receiving EVT. Methods In this retrospective cohort study, 381 patients with anterior circulation LVOS receiving EVT were screened from 636 hospitalized patients with LVOS in two stroke medical centers. Clinical baseline data and imaging data were collected within 2–5 days of admission to the hospital. The patients were divided into two groups based on whether MCE occurred after EVT. Multivariate logistic regression analysis was used to evaluate the independent risk factors for MCE and to establish a nomogram. Results Sixty-six patients out of 381 (17.32%) developed MCE. The independent risk factors for MCE included admission NIHSS ≥ 16 (OR, 1.851; 95% CI: 1.029–3.329; P = 0.038), ASPECT score (OR, 0.621; 95% CI: 0.519–0.744; P

With the advances in surgical techniques and perioperative management, the surgical indications for pancreaticoduodenectomy have been extended to elderly patients. Whether robotic pancreaticoduodenectomy (RPD) is superior to open pancreaticoduodenectomy (OPD) in older patients remains uncertain. Thus, this study aimed to compare perioperative outcomes between RPD and OPD in elderly patients.The demographics and perioperative outcomes of a consecutive series of elderly patients (aged ≥75 years) who underwent RPD or OPD at seven pancreatic centers in China between July 2011 and July 2020 were retrospectively analyzed.Of the 302 patients included in this study, 169 underwent RPD and 133 underwent OPD. The RPD group had a shorter operative time (OT) (264.3 vs. 278.2 min, P = 0.01) and less estimated blood loss (EBL) (100 (50 150) vs. 200 (150 300) mL, P 0.001) than the OPD group. RPDs in 3 (1.8%) patients were converted to OPD. The postoperative length of stay (LOS) after RPD was significantly shorter than that after OPD (13.0 vs. 17.0 days, P 0.001). No significant differences were found in the rates of clinically relevant postoperative pancreatic fistula, bile leakage, delayed gastric emptying, postoperative pancreatectomy hemorrhage, major morbidity, reoperation, 90-day readmission, or 90-day mortality between the two groups (P 0.05). The multivariate logistic regression analysis revealed that type 2 diabetes, chronic obstructive pulmonary disease, postoperative hemorrhage, and cardiac events were independent risk factors for postoperative 90-day mortality.This study demonstrated that RPD was comparable to OPD in terms of safety and feasibility in elderly patients with shorter OT, lower EBL, and shorter postoperative LOS. Surgical approach was not an independent risk factor for 90-day mortality.

Background: Despite the rapid advances that minimally invasive liver resection has gained in recent decades, open conversion is still inevitable in some circumstances. In this study, we aimed to determine the risk factors for open conversion after minimally invasive left lateral sectionectomy, and its impact on perioperative outcomes. Methods: This is a post hoc analysis of 2,445 of 2,678 patients who underwent minimally invasive left lateral sectionectomy at 45 international centers between 2004 and 2020. Factors related to open conversion were analyzed via univariate and multivariate analyses. One-to-one propensity score matching was used to analyze outcomes after open conversion versus non-converted cases. Results: The open conversion rate was 69/2,445 (2.8%). On multivariate analyses, male gender (3.6% vs 1.8%, P = .011), presence of clinically significant portal hypertension (6.1% vs 2.6%, P = .009), and larger tumor size (50 mm vs 32 mm, P < .001) were identified as independent factors associated with open conversion. The most common reason for conversion was bleeding in 27/69 (39.1%) of cases. After propensity score matching (65 open conversion vs 65 completed via minimally invasive liver resection), the open conversion group was associated with increased operation time, blood transfusion rate, blood loss, and postoperative stay compared with cases completed via the minimally invasive approach. Conclusion: Male sex, portal hypertension, and larger tumor size were predictive factors of open conversion after minimally invasive left lateral sectionectomy. Open conversion was associated with inferior perioperative outcomes compared with non-converted cases.

A novel technique of single-layer continuous suturing (SCS) for pancreaticojejunostomy (PJ) during robotic pancreaticoduodenectomy (RPD), a technically straightforward procedure, has been shown to produce promising results in a previous study. The present RCT aims to show that SCS during RPD does not increase the incidence of clinically relevant postoperative pancreatic fistula (CR-POPF) when compared with modified Blumgart anastomosis (MBA). Between January 2019 and September 2019, consecutive patients (ASA score ≤ 2) who underwent RPD were enrolled and randomized to the SCS or the MBA group. The primary endpoint was the rate of CR-POPF. A noninferiority margin of 10% was chosen. Of the 186 patients, 4 were excluded because PJ was not performed. The remaining 182 patients were randomized to the SCS group (n = 89) or MBA group (n = 93). CR-POPF rate was not inferior in the SCS group [SCS: 6.7%, MBA: 11.8%; 95% confidence interval (− 0.76, − 0.06), P = 0.0002]. PJ duration was significantly lower in the SCS group (P

Robotic pancreaticoduodenectomy (RPD) has gradually been accepted as it has overcome some of the limitations of laparoscopic surgery. Outcomes following RPD in elderly patients are still uncertain. This study aimed to evaluate the safety and feasibility of RPD in elderly patients.The demographics and perioperative outcomes of a consecutive series of patients who underwent RPD between January 2018 and September 2019, were retrospectively analyzed. Patients were divided into 2 groups: elderly patients (≥75 years) and younger patients (75 years).Of 431 patients who were included in this study, 77 were elderly patients and 354 were younger patients. Elderly patients had a significantly higher ASA score than younger patients (P 0.001). There were no significant differences in operative time, estimated blood loss and blood transfusion rate between groups (P 0.05). Elderly patients had significantly higher morbidity and longer postoperative hospital stay than younger patients (49.3% vs. 31.1%, P = 0.002; 22.8 vs. 13.3 days, P 0.001, respectively). However, the reoperation, 90-day readmission and mortality rates were comparable in the two groups (P 0.05). Multivariate analysis demonstrated that a higher ASA score was the only independent factor for postoperative morbidity (OR 2.02, 95% CI 1.06-3.88, P = 0.03), while old age was not (OR 0.81, 95% CI 0.36-1.81, P = 0.80).This study demonstrated that RDP was safe and feasible in elderly patients. Age should not be a contraindication to RPD. Elderly patients with careful patient selection should be considered for RPD.

Sleeve gastrectomy (SG) has been proven effective in the treatment of obesity and type 2 diabetes. We hypothesized that SGLT3 may play an important role in the mechanism of glucose control and weight loss after SG.Daily body weight and food intake were measured in SG and sham-operated mice. Glucose tolerance test, SGLT3 agonist (αMG), and SGLT1 inhibitor (phlorizin) perfusion experiments were used to detect changes in intestinal SGLT3 and SGLT1 activity following SG. Expression of SGLT3a and SGLT1 was assessed at 2 weeks, 1 month after surgery by quantitative PCR and fluorescence immunoassay. Hematoxylin and eosin staining was used to detect morphological changes in the villi. SGLT3 and SGLT1 expression was measured after stimulation of human intestinal epithelial cells (HIEC).Both the body weight and daily food intake of the SG-treated mice decreased within 30 days after surgery. Oral glucose absorption was significantly reduced at 30 days. The intestinal stimulation proved that SG can improve glucose metabolism, which can be reversed by αMG and enhanced by phlorizin. Villus height and surface area of the intestine in SG mice decreased after surgery. mRNA expression of SGLT3a and SGLT1 decreased at 2 weeks and 1 month after SG, immunofluorescence also confirmed these changes. HIEC stimulation confirmed that αMG could increase the expression of SGLT3 and SGLT1, but the expression of SGLT1 was down regulated when phlorizin was added to the medium.The results suggest that reducing SGLT3 expression might contribute to lowering blood glucose and controlling body weight after SG.

Pancreatoduodenectomy is the only potentially curative treatment for distal cholangiocarcinoma (DCC). In this study, we sought to compare the perioperative and oncological outcomes of robotic pancreaticoduodenectomy (RPD) and open pancreaticoduodenectomy (OPD) based on a multicenter propensity score-matched study.Consecutive patients with DCC who underwent RPD or OPD from five centers in China between January 2014 and June 2019 were included. A 1:1 propensity score matching (PSM) was performed. Univariable and multivariable Cox regression analyses were used to identify independent prognosis factors for overall survival (OS) and recurrence-free survival (RFS) of these patients.A total of 217 patients and 228 patients underwent RPD and OPD, respectively. After PSM, 180 patients in each group were enrolled. There were no significant differences in operative time, lymph node harvest, intraoperative transfusion, vascular resection, R0 resection, postoperative major morbidity, reoperation, 90-day mortality, and long-term survival between the two groups before and after PSM. Whereas, compared with the OPD group, the RPD group had significantly lower estimated blood loss (150.0 ml vs. 250.0 ml; P 0.001), and a shorter postoperative length of stay (LOS) (12.0 days vs. 15.0 days; P 0.001). Multivariable analysis showed carbohydrate antigen 19-9 (CA19-9), R0 resection, N stage, perineural invasion, and tumor differentiation significantly associated with OS and RFS of these patients.RPD was comparable to OPD in feasibility and safety. For patients with DCC, RPD resulted in similar oncologic and survival outcomes as OPD.

BACKGROUND The impact of resection margin status on long-term survival after pancreaticoduodenectomy (PD) for patients with pancreatic head carcinoma remains controversial and depends on the method used in the histopathological study of the resected specimens. This study aimed to examine the impact of resection margin status on the long-term overall survival of patients with pancreatic head carcinoma after PD using the tumor node metastasis standard. METHODS Consecutive patients with pancreatic head carcinoma who underwent PD at the Chinese People's Liberation Army General Hospital between May 2010 and May 2016 were included. The impact of resection margin status on long-term survival was retrospectively analyzed. RESULTS Among the 124 patients, R0 resection was achieved in 85 patients (68.5%), R1 resection in 38 patients (30.7%) and R2 resection in 1 patient (0.8%). The 1- and 3-year overall survival (OS) rates were significantly higher for the patients who underwent R0 resection than the rates for those who underwent R1 resection (1-year OS rates: 69.4% vs 53.0%; 3-year OS rates: 26.9% vs 11.7%). Multivariate analysis showed that resection margin status and venous invasion were significant risk factors for OS. CONCLUSION Resection margin was an independent risk factor for OS for patients with pancreatic head carcinoma after PD. R0 resection was associated with significantly better OS after surgery.

Radical antegrade modular pancreatosplenectomy (RAMPS), a new surgical approach for pancreatic ductal adenocarcinoma of the body and tail, has become increasingly accepted and performed in recent years. Robotic surgery has advantages over open and laparoscopic surgeries in terms of surgical vision and instrument flexibility. However, the lack of comprehension of the learning curve has limited its generalization. This study aimed to evaluate the learning curve of robotic posterior RAMPS.Patients who underwent robotic posterior RAMPS between February 2017 and April 2021 at our institution were included in this study. Data on patient characteristics, perioperative outcomes, and pathological outcomes were summarized and analyzed. The cumulative sum (CUSUM) method was used to assess the learning curve and inflection points based on operation time and estimated blood loss.One hundred consecutive patients who underwent robotic posterior RAMPS were enrolled. The median operation time was 235.0 (interquartile range [IQR], 210.0-270.0) min, and the estimated blood loss was 210.0 (IQR, 165.0-245.0) mL. The grade 3/4 Clavien-Dindo complication rate was 8% (8/100). According to the CUSUM plot, the inflection points of the learning curve were 25 and 65 cases, dividing the case series into the learning (1-25 cases), plateau (26-65 cases), and maturation (66-100 cases) phases. The operation time was relatively high in the learning phase, reached a plateau between 25 and 65 cases (270.0 min vs. 220.0 min, p 0.01), and decreased significantly in the maturation phase (p 0.01). Estimated blood loss improved in the maturation phase compared to the learning phase (150.0 vs. 245.0 mL, p 0.01). No significant differences in conversion rate, complications, or mortality were observed among the three phases.The inflection points of the learning and plateau phases were the 25th and 65th cases, respectively. Robotic RAMPS is safe and feasible even in the learning phase.

Patients with distal cholangiocarcinoma (DCC) are prone to relapse even after radical pancreaticoduodenectomy. In this study, we sought to create an online nomogram calculator to accurately predict the recurrence risk of DCC.A total of 184 patients were included. Multivariate Cox regression analysis was used to identify independent prognosis factors for recurrence-free survival and overall survival. A nomogram was constructed according to the prognostic factors in the training cohort and then tested in the validation cohort.Multivariate Cox analysis showed preoperative carbohydrate antigen 19-9 (p 0.001), maximum tumor size (p = 0.076), perineural invasion (p = 0.044), and N stage (p = 0.076) were independent prognostic factors for DCC relapse. We then constructed a nomogram with these four factors. The consistency index (C-index) of the nomogram in the training and validation cohorts were 0.703 and 0.665, respectively. Time-dependent receiver operating characteristic and decision curve analyses revealed that the nomogram provided higher diagnostic power and net benefit compared with other staging systems.In this study, we developed an online nomogram calculator that can accurately predict the recurrence risk of DCC and identify patients with a high risk of recurrence in a simple and convenient manner.

OBJECTIVES This study aimed to perform a multicenter comparison between robotic pancreaticoduodenectomy (RPD) and open pancreaticoduodenectomy (OPD). BACKGROUND Previous comparisons of RPD versus OPD have only been carried out in small, single-center studies of variable quality. METHODS Consecutive patients who underwent RPD (n = 1032) or OPD (n = 1154) at 7 centers in China between July 2012 and July 2020 were included. A 1:1 propensity score matching (PSM) was performed. RESULTS After PSM, 982 patients in each group were enrolled. The RPD group had significantly lower estimated blood loss (EBL) (190.0 vs 260.0 mL; P < 0.001), and a shorter postoperative 1length of hospital stay (LOS) (12.0 (9.0-16.0) days vs 14.5 (11.0-19.0) days; P < 0.001) than the OPD group. There were no significant differences in operative time, major morbidity including clinically relevant postoperative pancreatic fistula (CR-POPF), bile leakage, delayed gastric emptying, postoperative pancreatectomy hemorrhage (PPH), reoperation, readmission or 90-day mortality rates. Multivariable analysis showed R0 resection, CR-POPF, PPH and reoperation to be independent risk factors for 90-day mortality. Subgroup analysis on patients with pancreatic ductal adenocarcinoma (PDAC) (n = 326 in each subgroup) showed RPD had advantages over OPD in EBL and postoperative LOS. There were no significant differences in median disease-free survival (15.2 vs 14.3 months, P = 0.94) or median overall survival (24.2 vs 24.1 months, P = 0.88) between the 2 subgroups. CONCLUSIONS RPD was comparable to OPD in feasibility and safety. For patients with PDAC, RPD resulted in similar oncologic and survival outcomes as OPD.

Single-port (SP) robotic systems have the advantages of multi-jointed wristed instruments and a fully wristed three-dimensional high-definition camera. SP robotic pancreatic surgery (SPRPS) is rarely reported because of its complexity and technical challenges. This study aimed to evaluate the feasibility of SPRPS using the new da Vinci SP system.The demographics and short-term outcomes of a consecutive series of patients who underwent SP robotic pancreatic enucleation, distal pancreatectomy, and pancreaticoduodenectomy between December 24, 2021, and February 23, 2022, were analyzed.Twenty-three patients (eight men and 15 women; mean age, 39.7 years) were included in the study; 11 underwent SP robotic pancreatic enucleation, 11 underwent SP robotic distal pancreatectomy, and one underwent SP robotic pancreaticoduodenectomy. The mean operative time was 156.5 min, and the median estimated blood loss was 40.0 (interquartile range, 20.0-50.0) mL. No patient required conversion to laparotomy or blood transfusions. One patient in each of the three groups had grade B postoperative pancreatic fistula (POPF) and abdominal infection; they all recovered after conservative treatment. No patient had postoperative complications of grade C POPF, pancreatectomy hemorrhage, or major complications (Clavien grade ≥3). The mean length of postoperative length of hospital stay was 4.0 days. Pathological examination showed that all tumors were benign, and the mean largest tumor diameter was 2.8 cm.SPRPS using the da Vinci SP system is safe and feasible in selected patients. Further research and more surgical experience are needed to determine the indications for SPRPS.

Robotic surgery has been increasingly applied in pancreatic surgery and showed many advantages over conventional open surgery. The robotic pancreaticoduodenectomy (RPD) is a surgical option for primary nonampullary duodenal adenocarcinoma (PNDA). However, whether RPD is superior to open pancreaticoduodenectomy (OPD) for PNDA has not been reported. The comparative study was designed to analyze the short- and long-term outcomes of RPD versus OPD on patients with PNDA.Demographics, perioperative, and survival outcomes among patients who underwent RPD (n = 49) versus OPD (n = 43) for PNDAs between January 2013 and March 2018 were collected and analyzed RESULTS: Demographic characteristics were comparable between the RPD group and the OPD group. The RPD group demonstrated a decreased estimated blood loss (100 vs. 200 ml, p 0.001), time to oral intake (4.0 vs. 4.0 days, p = 0.04), and postoperative hospital stay (12.9 vs. 15.0 days, p = 0.01) compared with the OPD group. However, no differences were observed between the two groups in terms of operative time and the rates of major complications, grade B and C POPF, PPH, grade B and C DGE, biliary fistular, reoperation, and 90-day readmission. No patient died within 90 days. There were no significant differences in tumor size, differentiation, TNM stage, number of harvested lymph nodes, and the rates of nerve invasion, lymph node invasion, R0 resection, and the median overall survival between the two groups (p 0.05) CONCLUSIONS: RPD is a safe, feasible, and effective treatment for PNDA compared with OPD and can be used as an alternative for surgeons in the treatment of PNDA. Further multicenter randomized controlled trials are needed to evaluate the effectiveness of RPD in patients with PNDA.

Background Robotic resection of benign nonadrenal retroperitoneal tumors (BNRTs) is considered safety and feasibility. However, whether robotic BNRT resection (RBR) is superior to open BNRT resection (OBR) has not been reported. The comparative study was designed to analyze the short outcomes of RBR versus OBR on patients with BNRTs. Methods Demographics and perioperative outcomes among patients who underwent RBR (n = 30) versus OBR (n = 30) for BNRTs between January 2015 and September 2018 were reviewed. A 1:1 propensity score matched analysis was performed between both groups. Results There were no significant differences in the operative time, blood transfusion rate, and morbidity rate between the RBR and OBR groups. No patients underwent RBR required conversion to laparotomy. Robotic approach reduced estimated blood loss (EBL) (50 vs. 100 ml, p = 0.00) and postoperative hospital stay (PHS) significantly (4.6 vs. 7.9 d, p = 0.00) when compared with OBR. In patients with tumors adherent to major vessels, RBR also reduced EBL and PHS significantly (50 vs. 250 ml, p = 0.02; 4.4 vs. 9.3 d, p = 0.00), which were similar to the results of the patients with tumors larger than 5 cm (50 vs. 200 ml, p = 0.00; 4.9 vs. 7.5 d, p = 0.01). Conclusions When compared with OBR, RBR was associated with less EBL, and shorter PHS in selected patients even for tumors which are large or adherent to major vessels.

Background Suturing the proximal pancreatic stump and performing pancreaticoenterostomy for the distal pancreatic stump following central pancreatectomy is a conventional procedure. This reconstruction after resection of the pathological pancreatic lesion brings changes in anatomy and physiology. In this study, an innovative one-stage robotic end-to-end pancreatic anastomosis was reported to replace the conventional pancreaticoenterostomy following central pancreatectomy. Materials and methods The clinical data of 11 consecutive patients who underwent robotic central pancreatectomy with end-to-end pancreatic anastomosis between August 2017 and December 2017 were analyzed retrospectively. Results All operations were completed successfully without any conversion to open surgery. Nine patients had benign tumors, one had a mass-forming chronic pancreatitis, and one had an isolated pancreatic metastasis from a renal cancer. The mean gap left after central pancreatectomy was 4.3 ± 1.0 cm. The median operative time was 121 (range, 105 to 199) min. The median blood loss was 50 (range, 20 to 100) ml. Seven (63.6%) patients developed complications which included Clavien–Dindo Grade I complications in five patients, a Grade II complication in one patient, and a Grade IIIa complication in one patient. Seven patients developed a Grade B postoperative pancreatic fistula, and two patients a biochemical leak. There was no Grade C or worse pancreatic fistula. Magnetic resonance cholangiopancreatography at postoperative 6 months showed no stricture in any of the main pancreatic ducts. Three patients had an asymptomatic and small pancreatic pseudocyst. Conclusion Robotic central pancreatectomy with end-to-end pancreatic anastomosis was safe and feasible. It restores the normal anatomy of the pancreas. With its good short-and long-term outcomes, it could be an alternative reconstructive method to pancreaticoenterostomy following central pancreatectomy.

Rapid advances of big data and artificial intelligence (AI) techniques have pushed the envelope of traditional surgery to intelligent surgery, which revolutionizes the traditional medicine model dominated by the experiences of interveners and evidence-based medicine. Intelligent surgery requires to judiciously integrate the clinical experience of interveners and medical evidences with the new AI techniques, so as to achieve the best therapeutic effect for patients through effective disease risk control. This paper puts forward a new theory of prognostic control surgery in the long-term clinical practice of hepato-pancreato-biliary surgery. Our theory takes the optimal prognosis of patients as the goal, and pre-controls the disease risk through the best combination of the optimal intervener, intervention methods and intervention timing to achieve the maximum clinical benefits of patients with minimal medical trauma. This theory also takes full advantage of information technologies such as neural networks, deep learning, big data and imaging system, and uses high-dimensional network decision making to replace the traditional two-dimensional decision making tree model. A main technical challenge in the theory is how to effectively exploit the AI techniques in to improve the production efficiency of large hospitals, enhance the service level of primary medical care, and improve the diagnosis, prediction, treatment of diseases. Prognostic control surgery advocates the exploitation of emerging information technology methods, such as medical image recognition and prediction, 3D reconstruction based surgical planning, intraoperative navigation, and remote intelligent robotic surgical system, to promote the rapid and balanced surgical diagnosis and treatment. Guided by the theory of prognosis control surgery, our team successfully established minimally invasive anatomic hepatectomy, individualized pancreatic surgical approaches, modular hepatectomy, single-layer continuous suture of pancreaticojejunostomy, and end-to-end pancreatic anastomosis and advocated en-bloc resection of pancreatic cancer. This paper also provides the details of four core surgical strategies proposed in our prognosis control surgery, namely, blood control technique, surgical approach selection, resection technique and reconstruction technique, by which to achieve the minimally invasive surgery regularization and the complex surgery simplification.

Robotic total pancreatectomy (RTP), although considered safe and feasible, has rarely been reported. This study aimed to evaluate whether RTP has advantages over open TP (OTP). Demographics and perioperative outcomes among patients who underwent RTP (n=14) versus OTP (n=15) between May 2015 and September 2020 were retrospectively analyzed. RTP reduced the operative time (307.2 vs. 382.0 min, p=0.01) and estimated blood loss (EBL) (200 vs. 700 ml, p=0.002) compared to those of OTP. The patients in the RTP group got out of their beds and stood, received their first liquid, and took oral diets earlier (2.0 vs. 3.0 days, p=0.002; 2.0 vs. 4.0 days, p=0.009; 3.0 vs. 5.0 days, p=0.006) and experienced a shorter postoperative hospital stay (PHS) (9.0 vs. 12.0 days, p=0.03). There were no significant differences in the rates of spleen preservation, splenic vessel preservation, bile leakage, delayed gastric emptying, morbidity, or the number of lymph nodes harvest between the two groups. This study demonstrates that RTP is safe and feasible in selected patients with different indications in experienced robotic center. RTP was associated with a shorter operative time, lower EBL, and shorter PHS than OTP.

BACKGROUND Intraductal papillary mucinous neoplasm (IPMN) of the pancreas is a mucin-producing tumor that develops from the epithelial lining of the main pancreatic duct or branch pancreatic ducts. Here, we assessed the feasibility and safety of various robotic pancreatectomy approaches to treating IPMN, and short- and long-term outcomes of robotic IPMN resection. METHODS Data from patients who underwent robotic pancreatectomy for IPMN between 2012 and 2019 at our hospital were retrospectively analyzed. The survival outcomes for patients were compared using the Kaplan-Meier method and Cox regression analysis. RESULTS Of the 174 patients who underwent robotic pancreatectomy, 120 and 54 patients had benign or malignant tumors, respectively. Radical antegrade modular pancreatosplenectomy (RAMPS) was first introduced in 2003. It has been accepted as an alternative technique for pancreatic cancer of the body and tail. However, robotic RAMPS is not yet popular because of its technical difficulty and lack of standardized technique. This study describes in detail the standard steps of robotic RAMPS using the flip-up approach with the benefit of a robotic view when treating pancreatic cancer of the body and tail. We took advantage of our single-center experience to provide a step-by-step technique of robotic RAMPS procedure using the da Vinci Si system. We divided the procedure into 11 key steps. The surgical steps are optimized to achieve margin-negative curative resection and sufficient regional lymphadenectomy. The artery-first approach is usually used to determine tumor resectability early before performing an irreversible operative step. We also determine the borders of surgical resection and divide the splenic artery after dividing the pancreatic neck and the splenic vein, which facilitates a complete lymphadenectomy around the celiac axis with a bottom-up view. Robotic RAMPS using the flip-up approach is safe and feasible in performing curative resection for well-selected pancreatic cancer of the body and tail. A randomized controlled trial comparing open and robotic RAMPS is needed in the future.

The robotic system has overcome some laparoscopic shortcomings, such as stereoscopic 3D-magnified vision, a stable camera and flexible and tremor filtering manipulation. Therefore, robotic surgery has made many steps forward gaining more diffusion in the field of general surgery and this trend was associated with a decrease in laparoscopic surgery. Although the safety and effectiveness of robotic surgical systems in most procedures has been confirmed, some difficult procedures have not been widely performed but rather limited to a few highly specialized centers. This study describes the worldwide diffusion of robotic surgery in their respective fields in terms of hepatectomy, gastrectomy, esophagectomy, colectomy and pancreatectomy, respectively. The worldwide diffusion of robotic surgery is uneven, which may be related to the local economic strength, government health care policies, and surgeons' preference for minimally invasive surgery of various countries and regions. In the future, with the gradual decrease of cost of robotic surgical systems, and the standardization of surgical procedures, as well as the coverage of medical insurance, we believe that robotic surgery will become the gold standard procedure in general surgery.

Objective Robotic hepatectomy has been continuously improving and shown to be safe and reliable. The learning curve of robotic hemihepatectomy is required which enable beginners to benefit from previous experience. The aim of this study was to assess the learning curve of robotic left (RLH) and right hemihepatectomy (RRH) in terms of operative time (OT) to determine which procedure has an easier learning curve for beginners. Methods Data records for each 100 consecutive patients who underwent RLH and RRH between July 2012 and May 2019 were collected prospectively and analyzed retrospectively. The data included demographics, OT, estimated blood loss (EBL), postoperative hospital stay (PHS), and rates of morbidity and mortality. The cumulative sum method was used to evaluate the learning curve of OT. Results All patients underwent the RRH and RLH procedure performed by the same surgical team. RRH and RLH learning curve consisted of two phases: the first and second phase. The first phase of RRH included 45 patients, while RLH outcomes were optimized after 35 cases were completed. Compared with the first phase, the mean OT and the median blood loss were decreased significantly in the second phase in both learning curves. No significant decrease in the rates of morbidity and conversion to laparotomy or PHS was observed. Conclusions This study demonstrated the safety and feasibility of RLH and RRH. The surgeons who previously lacked robotic experience are able to overcome the learning curve for RLH faster than RRH.

Background The deep location, narrow operative space and proximity to major vessels make minimally invasive resection of nonadrenal retroperitoneal tumors (NTRs) challenging and rarely reported. This study aimed to evaluate the safety and feasibility of robotic resection of benign nonadrenal retroperitoneal tumors. Methods The demographics and perioperative outcomes of a consecutive series of patients who underwent robotic NTR resection between January 1, 2015, and August 30, 2017, were analyzed. Results Twenty-five patients (9 men and 16 women; mean age, 50.5 years) were included in the study. The mean largest tumor diameter was 4.9 cm, 11 had a tumor larger than 5 cm and 13 had a tumor adherent to major vessels. The mean operative time was 139.0 min, and the median estimated blood loss was 50.0 mL (interquartile range, 30–80 mL). No patients required conversion to laparotomy or blood transfusion. Postoperative morbidities occurred in 3 patients (12%), and all recovered after conservative treatment. The mean postoperative hospital stay was 4.8 days. On histopathological examination all the tumors were benign: lymphangioma (8, 32%), schwannoma (5, 20%), lymphatic cyst (4, 16%), paraganglioma (2, 8%), bronchogenic cyst (2, 8%), mucinous cystadenoma (1, 4%), neurofibroma (1, 4%), ganglioneuroma (1, 4%), and Castleman disease (1, 4%). Neither large tumor size nor adherence to major vessels affected the perioperative outcomes. Conclusions Robotic resection of NRTs is safe and feasible even for tumors which were large or adherent to major vessels.

Background Robotic distal pancreatectomy (RDP) is considered a safe and feasible alternative to laparoscopic distal pancreatectomy (LDP). However, previous studies have some limitations including small sample size and selection bias. This study aimed to evaluate whether the robotic approach has advantages over laparoscopic surgery in distal pancreatectomy. Methods Demographics and perioperative outcomes among patients undergoing RDP (n = 102) and LDP (n = 102) between January 2011 and December 2015 were reviewed. A 1:1 propensity score matched analysis was performed between both groups. Results Both groups displayed no significant differences in perioperative outcomes including operative time, blood loss, transfusion rate, and rates of overall morbidities and pancreatic fistula. Robotic approach reduced the rate of conversion to laparotomy (2.9% vs 9.8%, P = 0.045), especially in patients with large tumors (0% vs 22.2%, P = 0.042). RDP improved spleen (SP) and splenic vessels preservation (SVP) rates in patients with moderate tumors (60.0% vs 35.5%, P = 0.047; 37.1% vs 12.9%, P = 0.025), especially in patients without malignancy (95.5% vs 52.4%, P = 0.001; 59.1% vs 19.0%, P = 0.007). RDP also reduced postoperative hospital stay (PHS) significantly (7.67% vs 8.58, P = 0.032). Conclusions RDP is associated with less rate of conversion to laparotomy, shorter PHS, and improved SP and SVP rates in selected patients than LDP.

Effects of adjuvant transcatheter arterial chemoembolization (TACE) for intrahepatic cholangiocarcinoma (ICC) radical surgery have never been evaluated. A retrospective analysis was conducted on 125 ICC patients who had undergone operations with curative intent in Shanghai Eastern Hepatobiliary Surgery Hospital from July 2002 to December 2003. Of these patients, 53 underwent adjuvant TACE (TACE group) and 72 did not (non-TACE group). Adjuvant TACE was performed one time 1.5–2.0 months after the operation. Follow-up was performed at a median of 18 months (range 3–96 months). There was no significant recurrence-free survival (RFS) difference between the TACE and non-TACE groups (P = 0.659). The 1-, 3-, and 5-year overall survival (OS) rates were 69.8, 37.7, and 28.3%, respectively, for the TACE group and 54.2, 25.0, and 20.8%, respectively, for the non-TACE group (P = 0.045). Hepatic lymphangiomas, malformations of the liver lymphatic system, are extremely rare conditions in adults. A 41-year-old man presented with right upper abdominal pain for 6 months was introduced in this report. Ultrasound (US) and computed tomography (CT) scan demonstrated a giant cystictumor with a pedunculatedextrahepatic growth pattern. Due to diagnostic uncertainty, a partial hepatectomy was performed and pathological results confirmed the diagnosis of solitary hepatic lymphangioma. In this article, we reviewed the clinical and pathology features, preoperative diagnostic challenges, and treatments of hepaticlymphangiomas.

Purpose To evaluate the effectiveness and safety of carotid artery stent (CAS) placement for treatment of long segment stenosis in patients with Takayasu arteritis. Materials and Methods Between January 2002 and February 2012, all patients with Takayasu arteritis found to have long segment (≥80mm) carotid artery stenoses at a single institution were retrospectively analyzed. Five patients treated by CAS placement with either long or multiple self-expandable stents were included. All patients had focal neurologic symptoms, including three strokes and two transient ischemic attacks (TIAs). Six self-expanding stents were used in five patients. The mean follow-up period was 19.2 months (range, 6–30 mo); all patients had clinical evaluation, laboratory examination, and vascular imaging follow-up. Results Improvement in clinical symptoms was shown after successful angioplasty. There were no perioperative or in-hospital deaths. Four patients exhibited persistent relief, and repeated angiography or computed tomography (CT) angiography showed normal flow. One patient stopped taking her medications after CAS placement and became symptomatic 8 months later as a result of a severe in-stent stenosis. Conclusions CAS placement was shown to be a feasible option for treating long segment (≥80mm) stenosis of carotid arteries in patients with Takayasu arteritis with encouraging results.

Peutz–Jeghers syndrome is a rare autosomal dominantly inherited disease characterized by mucocutaneous pigmentations and gastrointestinal polyps. The polyps are located predominantly in the small intestine and usually cause intussusceptions. Adult intussusception caused by Peutz–Jeghers syndrome occurs very rarely. The purpose of this study was to analyze the clinical characteristics, preoperative diagnosis, and surgical management of Peutz–Jeghers syndrome associated with acute intussusception in adult patients. Consecutive patients with the postoperative diagnosis of acute intussusception caused by Peutz–Jeghers syndrome from 1995 to 2010 were reviewed retrospectively for this study. Data concerning clinical considerations, morphological examinations, and surgical procedure were analyzed. Different clinical manifestations were presented in patients with intussusception due to Peutz–Jeghers syndrome. Computed tomography associated or not with ultrasonography may be the most accurate examination for acute intussusceptions caused by Peutz–Jeghers syndrome. Surgical intervention is the first choice regimen in acute intussusceptions caused by Peutz–Jeghers syndrome. Prophylaxis and polypectomy of the entire small bowel is a worthy way in Peutz–Jeghers syndrome patients to reduce the frequency of laparotomies.
