BACKGROUND: The supplementary motor area (SMA) is implicated in stereotypic multi-limb movements such as walking with arm swing. Gait difficulties in Parkinson's Disease (PD) include reduced arm swing, which is associated with reduced SMA activity.OBJECTIVE: To test whether enhanced arm swing improves Parkinsonian gait and explore the role of the SMA in such an improvement.METHODS: Cortical activity and gait characteristics were assessed by ambulant EEG, accelerometers and video recordings in 27 PD patients with self-reported gait difficulties and 35 healthy participants when walking normally. Within these two groups, 19 PD patients additionally walked with enhanced arm swing and 30 healthy participants walked without arm swing. Power changes across the EEG frequency spectrum were assessed by Event Related Spectral Perturbation analysis of recordings from Fz over the putative SMA and gait analysis was performed.RESULTS: Baseline PD gait, characterized by reduced arm swing among other features, exhibited reduced within-step Event Related Desynchronization (ERD)/Synchronization (ERS) alternation (Fz; 20-50Hz), accompanied by a reduced step length and walking speed. All became similar to normal gait when patients walked with enhanced arm swing. When healthy controls walked without arm swing, their alternating ERD-ERS pattern decreased, mimicking baseline PD gait.CONCLUSION: Enhanced arm swing may serve as a driving force to overcome impaired gait control in PD patients by restoring reduced ERD-ERS alternation over the putative SMA. Accompanied by increased step length and walking speed, this provides a neural underpinning of arm swing as an effective rehabilitation concept for improving Parkinsonian gait.

The evidence of the responsiveness of dopaminergic medication on gait in patients with Parkinson’s disease is contradicting. This could be due to differences in complexity of the context gait was in performed. This study analysed the effect of dopaminergic medication on arm swing, an important movement during walking, in different contexts. Forty-five patients with Parkinson’s disease were measured when walking at preferred speed, fast speed, and dual-tasking conditions in both OFF and ON medication states. At preferred, and even more at fast speed, arm swing improved with medication. However, during dual-tasking, there were only small or even negative effects of medication on arm swing. Assuming that dual-task walking most closely reflects real-life situations, the results suggest that the effect of dopaminergic medication on mobility-relevant movements, such as arm swing, might be small in everyday conditions. This should motivate further studies to look at medication effects on mobility in Parkinson’s disease, as it could have highly relevant implications for Parkinson’s disease treatment and counselling.

Background The Hawthorne Effect occurs when participants alter their behavior when they are aware that they are being examined. The effect has been reported in many experiments, including gait analysis, and is considered an important source of bias that might impact both clinical and research results. Cognitive distraction is one potential solution to reducing the Hawthorne effect during gait analysis, but it is challenging in children, and can, in itself, alter gait. This study investigated the carryover effect of an alternative low-immersion avatar-based intervention on gait and subjective feelings in typically developing children. Research Question Will a low-immersion avatar-based intervention change feelings and indicators of temporospatial and kinematic outcomes in children in a laboratory setting, potentially reducing the Hawthorne Effect? Methods Typically developing children aged 5–13 participated in a standard laboratory gait analysis before experiencing a game in which they viewed their motion on monitors around the lab as that of a cartoon avatar in a 3D virtual environment. Following this intervention, standard walking trials were repeated. In addition, participants completed a survey of their feelings about the study both before onset and after completion. Results Thirty-one children participated in the study, 16 females and 15 males, mean aged 9.1 years. Arm swing, proposed as a measure of how relaxed and natural gait was, increased significantly following the intervention, while temporospatial parameters did not. The effect was more pronounced in females and younger children. Participants felt significantly happier, more excited, less scared, and less sad after the intervention. Changes in feelings were not closely associated with changes in gait. Significance This study suggests that gamification may reduce the Hawthorne effect and potentially produce more natural gait in children. The game intervention had a carryover effect, producing changes in gait even after the intervention was removed.

The spike attack is one of the most important skills in beach volleyball, and as an overarm movement, places significant load on the shoulder joint that could lead to increased risk of injury. An analysis of the different techniques used by elite athletes could provide important insights regarding the etiology of these injuries. This study aimed to assess the spike attack arm swing techniques of Olympics and World Championships (1996-2019) beach volleyball winners and to investigate possible gender differences. Abstract Human gait benefits from arm swing, as it enhances efficient lower limb muscle activation in healthy participants as well as patients suffering from neurological impairment. The underlying neuronal mechanisms of such coupling between upper and lower limbs remain poorly understood. The aim of the present study was to examine this coupling by intermuscular coherence analysis during gait. Additionally, directed connectivity analysis of this coupling enabled assessment of whether gait‐related arm swing indeed drives lower limb muscles. To that end, electromyography recordings were obtained from four lower limb muscles and two upper limb muscles bilaterally, during gait, of 20 healthy participants (mean (SD) age 67 (6.8) years). Intermuscular coherence analysis revealed functional coupling between upper and lower limb muscles in the alpha and beta/gamma band during muscle specific periods of the gait cycle. These effects in the alpha and beta/gamma bands indicate involvement of subcortical and cortical sources, respectively, that commonly drive the rhythmic four‐limb gait pattern in an efficiently coordinated fashion. Directed connectivity analysis revealed that upper limb muscles drive and shape lower limb muscle activity during gait via subcortical and cortical pathways and to a lesser extent vice versa. This indicates that gait‐related arm swing reflects the recruitment of neuronal support for optimizing the cyclic movement pattern of the lower limbs. The high portability of vibrotactile feedback systems makes them suited to wearable applications, which improves their usability for rehabilitation applications encompassing a variety of environments and scenarios. A number of works have explored the relationship between arm movement and gait parameters such as gait variations and age on the arm swing. However, the inter-limb coupling scheme, i.e. the effects of the specific side (left or right) and direction (forward or backward) of arm swing variation on gait and balance parameters have not yet been evaluated. The study of these effects can enable us to devise arm movement based gait training protocols that may be beneficial for stroke survivors. We have developed a vibrotactile biofeedback system worn on the upper limb for post-stroke gait rehabilitation training. Using this system, we have carried out a study with ten healthy subjects and one stroke survivor to determine the effects of arm swing variation on gait and balance parameters. The healthy subject experiments revealed that increase in arm swing significantly increases the stride length while bringing about a statistically non-significant increase in the gait velocity. The study also revealed that the protocols involving variation of forward arm swing appear to have greater efficacy in modifying the gait symmetry ratio. Furthermore, the variations in arm swing and the resulting gait modifications do not produce any significant difference in the balance parameters. The results from the pilot test with one stroke survivor also show that increasing the arm swing increases the stride length and velocity. These findings suggest that arm swing variation using vibrotactile bracelets has effects on gait parameters that may be utilized for gait training of stroke survivors.

BACKGROUND: The supplementary motor area (SMA) is implicated in both motor initiation and stereotypic multi-limb movements such as walking with arm swing. Gait in Parkinson's disease exhibits starting difficulties and reduced arm swing, consistent with reduced SMA activity.OBJECTIVE: We tested whether enhanced arm swing could improve Parkinson gait initiation and assessed whether increased SMA activity during preparation might facilitate such improvement.METHODS: Effects of instructed arm swing on cortical activity, muscle activity and kinematics were assessed by ambulant EEG, EMG, accelerometers and video in 17 Parkinson patients and 19 controls. At baseline, all participants repeatedly started walking after a simple auditory cue. Next, patients started walking at this cue, which now meant starting with enhanced arm swing. EEG changes over the putative SMA and leg motor cortex were assessed by event related spectral perturbation (ERSP) analysis of recordings at Fz and Cz.RESULTS: Over the putative SMA location (Fz), natural PD gait initiation showed enhanced alpha/theta synchronization around the auditory cue, and reduced alpha/beta desynchronization during gait preparation and movement onset, compared to controls. Leg muscle activity in patients was reduced during preparation and movement onset, while the latter was delayed compared to controls. When starting with enhanced arm swing, these group differences virtually disappeared.CONCLUSION: Instructed arm swing improves Parkinson gait initiation. ERSP normalization around the cue indicates that the attributed information may serve as a semi-internal cue, recruiting an internalized motor program to overcome initiation difficulties.

However, controversy remains in whether to incorporate an arm swing during the CMJ (CMJ AS) or keep the hands placed on the hips (CMJ NAS). Incorporating the arms provides a higher degree of sport-specificity that may yield improved reliability, especially in skilled jumpers. By contrast, the hands-on-hips approach isolates lower extremity force production and eliminates potential arm-swing variation. Therefore, the purpose of this study was to establish the reliability of CMJ typical (CMJ-TYP), CMJ concentric alternative (CMJ-Conc-ALT), and CMJ eccentric alternative (CMJ-Ecc-ALT) variables obtained during the CMJ AS and CMJ NAS. Twenty-two (men = 14, women = 8) NCAA Division 1 collegiate basketball players performed 3 CMJ AS and 3 CMJ NAS on a force plate, in a randomized order. To assess the test-retest reliability, participants returned 1 week later to perform 3 additional CMJ AS and 3 CMJ NAS. Background/aims Both upper and lower limbs interact through neural coupling. Such interconnection leads to rhythmic interlimb coordination, which affects the central pattern generator for the lower limbs. The aim of this study was to investigate the effects of repetitive intensive arm swing indirect gait training on muscle activity and gait parameters in children with cerebral palsy. Methods A total of 9 children with cerebral palsy were recruited for 20 sessions of repetitive intensive arm swing indirect gait training. They were tested before and after completion of this training using surface electromyography, spatiotemporal gait parameters assessments and clinical tests. A paired t-test was used to investigate differences in participants' vasti and hamstring activity, spatiotemporal gait parameters, and clinical test results before and after the training. Results Participants' vasti muscle activity increased significantly after the repetitive intensive arm swing indirect gait training, but there was no significant change in their hamstring muscles. However, spatiotemporal gait parameters and clinical motor function improved significantly. Conclusions Repetitive intensive arm swing indirect gait training may be suitable as an effective exercise in gait training programmes for children with cerebral palsy.

The Timed Up & Go (TUG) test is a simple test for gait and balance that requires no special equipment and can be part of a routine clinical examination. Combined with the development of motion capture technologies, the possibilities of assessing individual TUG sub-components (i.e. sit-to-stand, gait, turn, turn-to-sit) are increasing. The clinical evaluation of an instrumented TUG requires reliable values. We analysed the intra-session repeatability of the iTUG sit-to-stand, gait and turn parameters in three conditions: (1) single, (2) cognitive dual-tasks, and (3) manual dual-tasks in older adults and Parkinson's disease (PD) patients. The repeatability coefficient (RC) was calculated for each of the 18 parameters. The repeatability varied across subject groups, the performed tasks, and the TUG subcomponent. The gait subcomponent had 6 non-repeatable spatio-temporal parameters and 2 non-repeatable parameters for the arm swing. The parameters of the turn subcomponent can be considered as non-repeatable in both groups under the manual dual-task condition and in HC under the single-task condition. When comparing PD to HC, the repeatability of the majority of the single-task parameters was higher in PD whereas lower under dual-tasks. In PD, the major part of gait parameters had a higher repeatability under single-tasks than dual-tasks. In contrast, HC exhibited better repeatability of dual-tasks than single-tasks. Repeatability can be used to assist researchers and clinicians to select adequate parameters with respect to the purpose of motion assessment.

Purpose: The study attempts to understand the effect of lumbar stability exercise of transcranial direct current stimulation on muscle activity and jump performance in soccer player. Methods: Transcranial direct current stimulation combined lumbar stability exercise have been implemented on 15 trainees and general lumbar stability exercise have been implemented on another 15 trainees for 30 minutes at 5 times per week during 8 weeks, and elector spinae, rectus abdominis, external abdominal obilique have been measured using to evaluate the muscle activity of the trunk. Fall-induced injuries can stem from a disruption in the postural control system and place a financial burden on the healthcare system. Most gait research focused on lower extremities and neglected the contribution of arm swing, which have been shown to affect the movement of the center of mass when walking. This study evaluated the effect of arm swing on postural control and stability during regular and rocky surface walking. Fifteen healthy young adults (age = 23.4 ± 2.8) walked on these two surfaces with three arm motions (normal, held, and active) using the CAREN Extended-System (Motek Medical, Amsterdam, NL). Mean, standard deviation and maximal values of trunk linear and angular velocity were calculated in all three axes. Moreover, step length, time and width mean and coefficient of variation as well as margin of stability mean and standard deviation were calculated. Active arm swing increased trunk linear and angular velocity variability and peak values compared to normal and held arm conditions. Active arm swing also increased participants’ step length and step time, as well as the variability of margin of stability. Similarly, rocky surface walking increased trunk kinematics variability and peak values compared to regular surface walking. Furthermore, rocky surface increased the average step width while reducing the average step time. Though this surface type increased the coefficient of variation of all spatiotemporal parameters, rocky surface also led to increased margin of stability mean and variation. The spatiotemporal adaptations showed the use of “cautious” gait to mitigate the destabilizing effects of both the active arm swing and rocky surface walking and, ultimately, maintain dynamic stability.

This research develops a walking promotion device. The device is worn by user’s forearm and leads to increase arm swing. The device’s motion principle is based on parametric excitation. The Authors manufactured the prototype named “linear motion device”, and evaluated amplification of arm swing in previous paper. The device could increase the arm swing by 10°. In this paper, the authors experimented whether the device can increase the stride and walking speed. First, the participants wore the device on the right forearm. Afterwards, the authors measured at left thigh angle for stride and gait cycle. Comparing two states of disable with enable, the stride and walking speed were not increased. Second, they wore the device on the both forearms. As a result, the stride was increased 0.13 m, and the walking speed was increased 0.20 m/s. Therefore, the effectiveness of this device in walking promotion was confirmed.

BackgroundWalking is characterized by stable antiphase relations between upper and lower limb movements. Such bilateral rhythmic movement patterns are neuronally generated at levels of the spinal cord and brain stem, that are strongly interconnected with cortical circuitries, including the Supplementary Motor Area (SMA).ObjectiveTo explore cerebral activity associated with multi-limb phase relations in human gait by manipulating mutual attunement of the upper and lower limb antiphase patterns.MethodsCortical activity and gait were assessed by ambulant EEG, accelerometers and videorecordings in 35 healthy participants walking normally and 19 healthy participants walking in amble gait, where upper limbs moved in-phase with the lower limbs. Power changes across the EEG frequency spectrum were assessed by Event Related Spectral Perturbation analysis and gait analysis was performed.ResultsAmble gait was associated with enhanced Event Related Desynchronization (ERD) prior to and during especially the left swing phase and reduced Event Related Synchronization (ERS) at final swing phases. ERD enhancement was most pronounced over the putative right premotor, right primary motor and right parietal cortex, indicating involvement of higher-order organization and somatosensory guidance in the production of this more complex gait pattern, with an apparent right hemisphere dominance. The diminished within-step ERD/ERS pattern in amble gait, also over the SMA, suggests that this gait pattern is more stride driven instead of step driven.ConclusionIncreased four-limb phase complexity recruits distributed networks upstream of the primary motor cortex, primarily lateralized in the right hemisphere. Similar parietal-premotor involvement has been described to compensate impaired SMA function in Parkinson’s disease bimanual antiphase movement, indicating a role as cortical support regions.

Background The shoulder joint is immobilized in various positions after injury or reconstructive operative intervention. It is not clear how these immobilization positions in the shoulder joint affect gait. Research question Does the immobilized shoulder joint in different positions following shoulder surgery or injury affect gait? Methods A total of 38 healthy individuals with a mean age of 25.94 years and BMI of 25.66 kg/m2, underwent gait analysis in 4 different immobilized positions and normal gait. Gait parameters were evaluated using the GAITRite electronic walkway, and to determine symmetry, the bilateral spatiotemporal gait parameters were calculated using the Symmetry Index. Repeated-measures one way analysis of variance was used to compare the walking parameters in different positions. Results Velocity, step length and stride length were significantly decreased, and step width and single support time were increased in some immobilized positions (p Significance This cross-sectional simulation study may be important in demonstrating the clinical changes of gait in injuries, pathologies, and postoperative rehabilitation that require the immobilization of the shoulder joint. Background Reduction of arm swing during gait is an early and common symptom in Parkinson's disease (PD). By using the technology of a mobile phone, acceleration of arm swing can be converted into a closed-loop musical feedback (musification) to improve gait. Objectives To assess arm swing in healthy subjects and the effects of musification on arm swing amplitude and other gait parameters in patients with PD. Methods Gait kinematics were analyzed in 30 patients during a 320 m walk in 3 different conditions comprising (1) normal walking; (2) focused swinging of the more affected arm; and (3) with musification of arm swing provided by the iPhone application CuraSwing. The acceleration of arm swing was converted into musical feedback. Arm swing range of motion and further gait kinematics were analyzed. In addition, arm swing in patients was compared to 32 healthy subjects walking at normal, slow, and fast speeds. Results Musification led to a large and bilateral increase of arm swing range of motion in patients. The increase was greater on the more affected side of the patient (+529.5% compared to baseline). In addition, symmetry of arm swing, sternum rotation, and stride length increased. With musical feedback patients with PD reached arm swing movements within or above the range of healthy subjects. Conclusions Musification has an immediate effect on arm swing and other gait kinematics in PD. The results suggest that closed-loop musical feedback is an effective technique to improve walking in patients with PD.

Persons with upper limb loss (ULL) experience a high prevalence of falls, with the majority of falls occurring when walking. This issue may be related to altered arm dynamics, which play an important role in proactive mechanisms of locomotor stability. This study investigated effects of ULL and prosthesis use on proactive stability mechanisms, particularly if matching the mass and inertia of the impaired limb to the sound limb would enhance locomotor stability. Gait data were collected on adults with unilateral ULL during level walking while: (1) not wearing a prosthesis, (2) wearing their customary prosthesis, (3) wearing a mock prosthesis that matched the sound limb mass and inertia. Main and interaction effects of limb side and condition on trunk rotations, arm swing, step width, free vertical moment, and margin-of-stability were analyzed. Across conditions, arm swing, free vertical moment, and margin-of-stability were 2.27, 1.13, and 1.20 times greater, respectively, on the sound limb side than the impaired limb side. Persons with ULL display asymmetry in proactive mechanisms of locomotor stability with potentially greater medial-lateral stability on the sound limb side irrespective of prosthesis use, but heavier prostheses reduced the walking base of support. This bias may enhance fall risk on the impaired side if the prosthetic limb is used inappropriately to regain balance following a disturbance. Research is warranted to explore the consequences of this asymmetry on perturbation response.

J Strength Cond Res XX(X): 000-000, 2019-The purpose of this study was compare the effect of internal and external focus of attention instructions on force-time characteristics of the countermovement jump (CMJ) in collegiate student-athletes. Forty-three resistance-trained men ((Equation is included in full-text article.)± SD; age = 20 ± 1.5 years) on an NCAA Division I baseball team volunteered to participate in this study. Each participant performed a total of 16 CMJs on a force platform while holding a wooden dowel on their shoulders to eliminate arm swing. Force and power parameters such as jump height (JH), peak velocity (PV), and mean concentric velocity (MCV) were calculated from force-time and position data. Paired-sample t-tests and Cohen's d effect sizes were used to examine differences between conditions. When subjects were instructed using an external focus, they demonstrated significantly (p < 0.05) greater JH, PV, and MCV compared with jumps performed with the internal focus (external JH = 48.0 ± 5.6 cm, internal JH = 46.4 ± 5.4 cm; external PV = 3.6 ± 0.3 m·s, internal PV = 3.5 ± 0.3 m·s; and external MCV = 2.31 ± 0.22 m·s, internal MCV = 2.25 ± 0.23 m·s). It is interesting to note that there was superior recall of the internal instructions during the manipulation checks that may suggest that the subjects consciously processed these instructions to a greater extent and reduced internal condition performance. These results indicate that instructions can alter the efficiency and performance of a skill. According to the literature and this study, if an optimum performance metric is desired, external focus of attention instructions should be used.

In this context, the aim of this study was to determine the effects of an increased arm swing frequency or amplitude on the gait parameters in people with PD and healthy older adults. Seventeen individuals with PD and 19 older people were invited to walk on a 10 m pathway under three experimental conditions: (i) usual walking (no arm swing instructions); (ii) an increased arm swing amplitude; and (iii) an increased arm swing frequency. Both groups had an increased stride speed, vertical center of mass and arm swing accelerations and decreased double support time under the increased arm swing amplitude and frequency conditions. People with PD were able to modulate the gait parameters according to the experimental conditions, but at a smaller magnitude than the older individuals. These results indicate that bradykinesia and hypometria of gait can be positively overcome by increasing the amplitude and frequency of arm swing. Arm movements should be included in gait rehabilitation protocols for PD. Purpose: This study is aimed to preliminary investigate whether transcranial alternating current stimulation (tACS) could affect explosive power considering genetic background in sport subjects.Methods: Seventeen healthy sports volunteers with at least 3 years of sports activities participated in the experiment. After 2 weeks of familiarization performed without any stimulation, each participant received either 50 Hz-tACS or sham-tACS. Before and after stimulation, subjects performed the following tests: (1) the squat jump with the hands on the hips (SJ); (2) countermovement jump with the hands on the hips (CMJ); (3) countermovement jump with arm swing (CMJ-AS); (4) 15-s Bosco’s test; (5) seated backward overhead medicine ball throw (SBOMBT); (6) seated chest pass throw (SCPT) with a 3-kg rubber medicine ball; and (7) hand-grip test. Additionally, saliva samples were collected from each participant. Genotyping analysis was carried out by polymerase chain reaction (PCR).Results: No significant differences were found in sport performance of subjects after 50 Hz-tACS. Additionally, we did not find any influence of genetic background on tACS-related effect on physical performance. These results suggest that tACS at gamma frequency is not able to induce an after-effect modulating sport performance. Further investigations with larger sample size are needed in order to understand the potential role of non-invasive brain stimulation techniques (NIBS) in motor performances.Conclusions: Gamma-tACS applied before the physical performance fails to improve explosive power in sport subjects.

The arm elevation strategy assists in recovering stability during slips in healthy young and elderly individuals. However, in people with Parkinson’s Disease, one of the main motor symptoms affecting the upper limbs is reduced arm swing which intensifies throughout the course of the disease before becoming absent. This holds direct implications for these individuals when encountering slips as the arm elevation strategy is an integral component in the interlimb slip response to restore stability. Arm swing’s effect in recovering from slips in people with Parkinson’s Disease though remains unexamined. Twenty people with Parkinson’s Disease (63.78 ± 8.97 years) walked with restricted and unrestricted arm swing conditions on a dual-belt treadmill where slips were induced on the least and most affected sides. Data were collected on the CAREN Extended System (Motek Medical, Amsterdam, NL). The Margin of Stability, linear and angular trunk velocities, as well as step length, time, and width were calculated. Data were examined during the slipped step and recovery step. The restricted arm swing condition, compared to unrestricted, caused a faster step time during the slipped step. Compared to the most affected leg, the least affected had a wider step width during the slipped step. During the recovery step, the least affected leg had a larger anteroposterior Margin of Stability and longer step time than the most affected. No differences between our arm swing conditions suggests that the normal arm swing in our participants was not more effective at restoring stability after an induced slip compared to when their arm motion was restricted. This may be due to the arm elevation strategy being ineffective in counteracting the slip’s backward destabilization in these individuals. Differences between the legs revealed that our participants were asymmetrically impaired in their slip recovery response.

BACKGROUND: Human bipedal gait benefits from arm swing, as it drives and shapes lower limb muscle activity in healthy participants as well as patients suffering from neurological impairment. Also during gait initiation, arm swing instructions were found to facilitate leg muscle recruitment.RESEARCH QUESTION: The aim of the present study is to exploit the directional decomposition of coherence to examine to what extent forward and backward arm swing contribute to leg muscle recruitment during gait initiation.METHODS: Ambulant electromyography (EMG) from shoulder muscles (deltoideus anterior and posterior) and upper leg muscles (biceps femoris and rectus femoris) was analysed during gait initiation in nineteen healthy participants (median age of 67 ± 12 (IQR) years). To assess to what extent either deltoideus anterior or posterior muscles were able to drive upper leg muscle activity during distinct stages of the gait initiation process, time dependent intermuscular coherence was decomposed into directional components based on their time lag (i.e. forward, reverse and zero-lag).RESULTS: Coherence from the forward directed components, representing shoulder muscle signals leading leg muscle signals, revealed that deltoideus anterior (i.e. forward arm swing) and deltoideus posterior (i.e. backward arm swing) equally drive upper leg muscle activity during the gait initiation process.SIGNIFICANCE: The presently demonstrated time dependent directional intermuscular coherence analysis could be of use for future studies examining directional coupling between muscles or brain areas relative to certain gait (or other time) events. In the present study, this analysis provided neural underpinning that both forward and backward arm swing can provide neuronal support for leg muscle recruitment during gait initiation and can therefore both serve as an effective gait rehabilitation method in patients with gait initiation difficulties.

Background While walking, people swing their arms in a specific pattern. This specific arm swing pattern during walking has shown to have a beneficial effect on gait as it reduces walking energy cost and optimizes balance. In several patient populations the arm movements can be directly affected (e.g. in patients with acquired brain injury (ABI)), which in turn has a negative effect on their gait pattern, balance and energy cost of walking. Main text In December 2019, Kahn et al. published a paper in JNER concerning the quantification of upper limb associated reactions (ARs) during walking in people with ABI. ARs are defined as “an effort-dependent phenomenon causing an involuntary increase in upper limb muscle tone, with awkward and uncomfortable postures”. These upper limb ARs appear often in patients with ABI and can have an important effect on their gait. The authors calculated kinematic measures using three-dimensional gait analysis relating to range of motion, variability and mean position over the gait cycle for the different upper limb joints (shoulder, elbow, wrist) during self-selected steady-state walking. Based on differences they found between an ABI cohort and healthy control cohort, the authors concluded that they were able to quantify ARs during walking in this population. This calculation, however, is not specific for upper limb ARs. In fact, the authors calculated general measures of arm posture (e.g. mean position over the gait cycle) or arm movement (e.g. range of motion and variability) during gait. Previous research has already indicated that other factors than ARs can influence the posture or movement of the arm during gait in patients with brain injury, such as voluntary compensations for gait instability and contractures or spasticity of upper arm muscles. Yet, it is not possible to disentangle the different causes of the altered arm posture during steady-state walking based on the proposed measures. Conclusion The kinematic arm measures proposed by Kahn et al. (J Neuroeng Rehabil 16(1):160, 2019) are not a direct measure of ARs, but provide a quantification of overall deviation of arm posture or movement during gait. Depending on the specific study design these measures may provide insights in ARs.

Parkour speed-runs require performers (known as Traceurs) to negotiate obstacles with divergent properties such as angles, inclinations, sizes, surfaces, and textures in the quickest way possible. The quicker the run, the higher the performer is ranked. Performance in Parkour speed-runs may be regulated through Parkour Traceurs’ basic movement skill capacities given the physical requirements of the event. This study examined what functional movement skills correlate with Parkour speed-run performance. Nineteen male Parkour Traceurs undertook a physical testing battery inclusive of: agility T-test, maximal grip strength test, and maximal vertical and horizontal jumps across several jump modalities. For the speed-run, Parkour Traceurs navigated an indoor Parkour installation. Pearson’s correlation analyses (r) revealed that agility T-test performance showed a significant negative correlation with Parkour speed-run performance, whereas standing long jump and counter movement jump (with and without arm swing) were significantly positively correlated with Parkour speed-run performance. Concurrent with the intrinsically-linked building blocks in the Athletic Skills Model, the data from the present study suggest that performance in Parkour-speed-runs are underpinned by functional movement skills (jumping, running; arm swinging) and conditions of movement (agility), all of which encapsulate elements of basic motor properties (speed; strength). From a practical perspective, the agility T-test, standing long jump, and counter movement jump without and without arm swing can form a basic battery to evaluate the physical effects of Parkour speed-run interventions on functional movement skills.

Falls are common in daily life, often caused by trips and slips and, particularly in older adults, with serious consequences. Although arm movements play an important role in balance control, there is limited research into the role of arm movements during balance recovery after tripping in older adults. We investigated how older adults use their arms to recover from a trip and the difference in the effects of arm movements between fallers (n=5) and non-fallers (n=11).Sixteen older males and females (69.7±2.3 years) walked along a walkway and were occasionally tripped over suddenly appearing obstacles. We analysed the first trip using a biomechanical model based on full-body kinematics and force-plate data to calculate whole body orientation during the trip and recovery phase. With this model, we simulated the effects of arm movements at foot-obstacle impact and during trip recovery on body orientation.Apart from an increase in sagittal plane forward body rotation at touchdown in fallers, we found no significant differences between fallers and non-fallers in the effects of arm movements on trip recovery. Like earlier studies in young adults, we found that arm movements during the recovery phase had most favourable effects in the transverse plane: by delaying the transfer of angular momentum of the arms to the body, older adults rotated the tripped side more forward thereby allowing for a larger recovery step. Older adults that are prone to falling might improve their balance recovery after tripping by learning to prolong ongoing arm movements.

The present study examined if elite youth male association football (soccer) players aged 8–19 y (n = 2,875) from the English talent development system, who ultimately achieved professional status differed in stature, body mass, and physical performance (20-m sprint speed, slalom agility speed, vertical counter-movement jump with arm swing jump height, multistage fitness test distance) compared with their non-professional peers. The study also examined the longitudinal pattern of development of stature, body mass, and physical performance, and if this was different between future professionals and non-professionals, while considering the effects of playing position. Multilevel modeling of the 8,898 individual (player-occasion) data points suggested that from age 12.0, the future professionals performed better in a vertical counter-movement jump with arm swing test and slalom agility test than future non-professionals, and improved at a faster rate, so that by age 18.0 the differences in vertical counter-movement jump with arm swing and slalom agility performance were 1.7 cm (p < 0.001, d = 0.3) and 0.14 s (p < 0.001, d = 0.5), respectively. In addition, future professionals were faster (by 0.02–0.04 s on the 20-m sprint, p < 0.001, d = 0.2) and ran further in the multistage fitness test (by 47 m, p = 0.014, d = 0.2) than future non-professionals throughout their development, but there were no differences in stature or body mass during development between the groups. Whereas, multistage fitness test performance improved linearly with age, the development of all other physical characteristics was non-linear. There were inter-individual differences in the development of all characteristics, and there were differences between playing positions in the development of all characteristics. Thus, in summary, future professionals jump higher, are more agile, faster, and more endurance fit than future non-professionals as they age, and the pattern of development is different in professionals and non-professionals for vertical jumping and slalom agility performance.

Introduction: Fall rates in people with Parkinson's Disease range between 35 and 68% with the majority of falls occurring while walking. Initial evidence suggests that when walking without arm swing, people with Parkinson's Disease adapt their stepping foot placement as a means to preserve dynamic stability. However, it remains unexamined what arm swing's effect has on dynamic stability when walking on destabilizing surfaces. Methods: Twenty people with Parkinson's Disease (63.78 ± 8.97 years) walked with restricted and unrestricted arm swing on unperturbed, rocky, rolling-hills, and mediolateral translational surfaces. Data were collected on a split-belt treadmill CAREN Extended-System (Motek Medical, Amsterdam, NL). Bilateral averages and coefficient of variations for step time, length, and width; and mediolateral margin of stability were calculated. Results: Results were examined in three separate analyses that included arm conditions during each of the destabilizing surfaces compared to unperturbed walking (arm-rolling hills, arm-rocky, and arm-mediolateral). Compared to unrestricted arm swing, restricted arm swing reduced average step length (arm-rolling hills) and time (arm-rocky), and increased COV step time (arm-rolling hills). The arm-rolling hills analysis revealed that the most affected leg had a shorter step length than the least affected. The destabilizing surface effects revealed that during the arm-rolling hills and arm-rocky analyses, step time decreased, step width increased, and the COV for step time, length and width increased. No main effects occurred for the arm-mediolateral analysis. Conclusion: Results indicate that foot placement in response to restricted arm swing, in people with Parkinson's Disease, depends on the encountered destabilizing surface. The arm-rolling hills analysis revealed that participants appropriately reduced step length as compensation to their restricted arm swing. However, the arm-rocky analysis revealed that individuals prioritized forward progression over dynamic stability as they decreased average step time. Additionally, the increased spatiotemporal variability in response to the rocky and rolling hills conditions indicate partial foot placement adaptation to maintain an already existing level of global dynamic stability as no changes in the Margin of Stability occurred. Adaptation is further corroborated by the decreased step time and increased step width. These responses reflect attempts to pass the destabilizing terrains faster while increasing their base of support.

In patients with Parkinson&rsquo, s disease (PD), arm swing changes are common, even in the early stages, and these changes are usually evaluated subjectively by an expert. In this article, hypothesize that arm swing changes can be detected using a low-cost, cloud-based, wearable, sensor system that incorporates triaxial accelerometers. The aim of this work is to develop a low-cost, assistive diagnostic tool for use in quantifying the arm swing kinematics of patients with PD. Ten patients with PD and 11 age-matched, healthy subjects are included in the study. Four feature extraction techniques were applied: (i) Asymmetry estimation based on root mean square (RMS) differences between arm movements, (ii) posterior&ndash, anterior phase and cycle regularity through autocorrelation, (iii) tremor energy, established using Fourier transform analysis, and (iv) signal complexity through the fractal dimension by wavelet analysis. The PD group showed significant (p &lt, 0.05) reductions in arm swing RMS values, higher arm swing asymmetry, higher anterior&ndash, posterior phase regularities, greater &ldquo, high energy frequency&rdquo, signals, and higher complexity in their XZ plane signals. Therefore, the novel, portable system provides a reliable means to support clinical practice in PD assessment.

In this paper, we describe and show the results from an affordable integrated system for Parkinson’s disease (PD) diagnostic and follow-up, using RGBD cameras and accelerometers. We validate the system with 26 PD patients and 34 control subjects, in order to analyze the clinical relevance of the system and how both technologies are complimentary in the process of diagnosis and follow-up the progression of the disease.

The symptoms of (PD) often begins on one side of the body before separating bilaterally. Early symptoms of PD are rest tremors, bradykinesia and rigidity. Common signs are a decrease in arm swing, shuffling foot movement and slowness arising from a chair. Rigidity of muscles commonly affects a patient limbs, neck and shoulder PD is currently diagnosed after the neuro degenerative process has started. To optimize and improve PD quality of care, it should be diagnosed early in its onset once symptoms are not yet evident. The current gold standard for patient assessment of PD is the completion of symptom diaries. These diaries are subjective and usually do not accurately reflect what is taking place throughout the day. It is exceedingly difficult for an untrained observer such as family members to provide accurate description of movement characteristic in PD. An alternative way to exercise the abnormal movement in PD is to use a wearable technology system which could aid early detection of the disease and help clinicians manage medication when symptoms fluctuate throughout the day. This paper will evaluate the effectiveness of a wearable multi-sensor kinematic system capable of detecting the tell-tale signs of PD when performing tailored exercise routines. Each exercise routine will examine movement for potential PD symptoms and report finding for deeper clinician analysis.

Introduction: Falling during walking is a common occurrence in people with Parkinson's disease and is closely associated with severe social and medical consequences. Recent evidence demonstrates that arm swing affects dynamic balance in healthy young adults; however, it remains unexamined what its effect is in people with Parkinson's disease, particularly when combined with a secondary dual task. Methods: Twenty people with Parkinson's disease (63.78 ± 8.97) walked with two arm swing conditions (absent and normal) with and without a secondary dual task. Data were collected on a split-belt treadmill CAREN Extended-System (Motek Medical, Amsterdam, NL). Average and standard deviations for trunk linear and angular velocity were calculated along with their instantaneous values (during foot strikes) in all three axes. Averages and coefficient of variations for step length, time, and width; margin of stability; and harmonic ratios were also calculated. Results: Compared with normal arm swing, absent arm swing reduced the least affected leg's average step length and increased its step length coefficient of variation while increasing step time coefficient of variation in the most affected leg. Further, absent arm swing reduced trunk anteroposterior instantaneous angular velocity (least affected leg) and reduced anteroposterior instantaneous linear velocity (bilaterally). For the vertical axis, absent arm swing increased the trunk's average angular velocity but reduced its instantaneous linear velocity and angular velocity standard deviation (least affected leg). Additionally, the margin of stability increased when the arms were absent (least affected leg). Alternatively, dual tasking reduced average step time (most affected leg) and increased the step width coefficient of variation (bilaterally). Additionally, dual tasking increased the mediolateral average angular velocity, instantaneous linear velocity standard deviation (bilaterally), and instantaneous angular velocity standard deviation (least affected leg). For the vertical axis, dual tasking increased average linear and angular velocity standard deviation as well as instantaneous angular velocity standard deviation (bilaterally). Conclusion: Findings suggest that participants attempted to control extraneous trunk movement (due to absent arm swing) through compensatory responses in both lower and upper extremities. However, participants appeared to predominately compensate on their least affected side. Contrastingly, modifying mediolateral foot placement appeared to be the main means of maintaining walking stability while dual tasking.

[Purpose] Arm swing is seldom considered while designing clinical rehabilitation protocols for hemiplegic patients with upper or lower extremity disabilities, likely due to the unclear role that arm swinging plays in the ability to walk. We, therefore, aimed to investigate the effect of arm swinging on walking abilities. [Participants and Methods] The study enrolled 20 healthy adults who performed a 10 m walking test with normal gait, single-arm restricted gait, both-arms restricted gait, and maximum arm-swing gait with one arm fixed in the Wernicke-Mann's position. The walking time, number of steps taken, and pelvic fluctuation were measured for the four gaits. A fixed-trunk type arm sling was used for maintaining the Wernicke-Mann's position. [Results] Velocity and stride length decreased significantly while walking with the single-arm restricted gait and both-arms restricted gait in comparison to normal gait. The maximum arm-swing gait showed no significant differences from normal gait in terms of cadence, velocity, and stride. Pelvic fluctuations also had no significant differences among all gaits. [Conclusion] Restricting movement of one or both arms limited the walking speed and stride; however, in Wernicke-Mann's limb position, if the arm is intentionally swung, the walking speed and stride resembled that of normal gait.

People who work in offices for a long time tend to have poor posture and a decreased range of motion throughout the body. This could lead to chronic neck and shoulder pain if not corrected. Shuai Shou Gong, a version of Arm Swing Exercise (ASE), has been developed and practiced by some Chinese people for over one thousand years to maintain physical well-being. Its beneficial effects on posture and range of motion had not yet been verified. The purpose of this study was to preliminarily examine the immediate effects of ASE on shoulder range of motion (ROM) and occiput-wall distance (OWD) in young adults. The before-after study design was used in fifteen healthy office workers (6 males, 9 females), aged 20-40 years, who participated in the study. The shoulder ROM and OWD of the participants were measured before and immediately after a 10-minute session of supervised ASE. A paired-sample t-test was used to estimate the mean changes in the outcome. Results in this study revealed that shoulder ROM was increased in all directions (P < 0.05) while the OWD was decreased (P < 0.05) after the 10-minute session. However, a postural assessment method using a mobile application (PostureScreen) could not demonstrate a significant improvement. In conclusion, ASE have provided immediate improvements on shoulder ROM and forward head posture as indicated by a decreased OWD. Further research using a randomized controlled trial is needed.

Background Arm swing changes are common even in the early stages of Parkinson's disease (PD). We hypothesized that arm swing changes decrease with age and can be detected using a low-cost, RGB-D depth-sensing camera. Objective This study aimed to assess the differences in arm swing between PD patients and healthy participants and to investigate the possible effects of aging on these differences. Methods Twenty-five PD patients (aged 45-87 years) and 25 age-matched, healthy subjects (aged 46-88 years) were included. Clinical variables were evaluated using a descriptive analysis. No spatiotemporal variables were normally distributed; therefore, we used a Mann-Whitney U test to compare the continuous variables between groups and to perform age-stratified analysis. A receiver operating characteristic analysis was generated to evaluate the discrimination activity of arm swing asymmetry (ASA). Results The PD group showed significant reductions in arm swing magnitude (left, p = 0.002; right, p = 0.006) and arm swing speed (left, p = 0.002; right, p = 0.004) and significantly greater ASA (p 67 years. Conclusions The camera detected differences in ASA, arm swing speed, and arm swing magnitude between PD patients and healthy individuals. Analysis of arm swing variables should be stratified by age, and the validity of the analysis may be questionable in patients aged >67 years.

Previous studies have investigated how additional arm weights affect gait. Although light weights (0.45 kg) seemed to elicit performance improvements in Parkinsonian patients, it was not studied how light weights affect gait parameters in healthy individuals. It is important to understand normal responses in a healthy population so that clinical effects might be better understood. Therefore, the purpose of this study was to investigate the effects of arm weights on arm swing amplitude, gait performance, and muscle activity in healthy people. Twenty-two subjects walked overground at their preferred speed under different weight carriage conditions (C1: no weight; C2: unilateral arm weight; C3: bilateral arm weights; C4: waist weights). Gait speed increased in C2 (p = 0.018) and C4 (p = 0.013) when compared with C1(C1: 1.21 ± 0.08; C2: 1.25 ± 0.11; C3: 1.24 ± 0.11; C4: 1.25 ± 0.11 m/s) with an increase in cadence during C2 (p 0.001), C3 (p = 0.008), and C4 (p 0.001) (C1: 105.5 ± 5.2; C2: 108.5 ± 5.6; C3: 107.9 ± 5.6; C4: 108.5 ± 5.3 steps/min) and in tibialis anterior electromyographic activity on the unweighted side in C2 (p = 0.048) (C1: 21.05 ± 4.59; C2: 25.10 ± 6.10; C3: 23.93 ± 4.75; C4: 24.33 ± 6.32 μV). The results indicate that an additional sensory input with the application of the weights may result in an overcompensation with the whole body and facilitate faster walking speed when applied on one arm or around the waist. The locations of the weights and amount of the weights may elicit different responses. Various strategies of adding weights should be further investigated as a potential intervention to improve performance in individuals with various gait impairments. Although there is evidence for benefits of this intervention in Parkinsonian patients, further study is warranted in other patient populations, such as stroke patients, who might benefit from this intervention to improve gait performance.

This study aimed to identify the effects of rhythmic arm swing during robot-assisted walking training on balance, gait, motor function, and activities of daily living among patients with subacute stroke. Twenty patients with subacute stroke were recruited, and thereafter randomly allocated to either the experimental group that performed the robot-assisted walking training with rhythmic arm swing, or the control group that performed the training in arm fixation. In total, 30 training sessions were carried out. The outcome measures included the 10-m walk test, Berg balance scale, timed up-and-go test, fall index that was measured using the Tetrax system, motor function test of Fugl-Meyer assessment, and modified Barthel index. Normal walking includes coordinated and controlled movement of the legs and arms. However, patients following stroke often present with inappropriate motor control which limits coordinated movement patterns of the affected limbs. This study aimed to compare the effects of rhythmic arm swing and arm fixation during treadmill walking in patients with poststroke hemiparesis. We used an alternating study design with multiple baselines across subjects. Three patients with chronic stroke participated in this study. During treadmill walking, rhythmic arm swing and arm fixation conditions were alternately applied. Outcome measures included the 10-meter walk test (10MWT) and energy expenditure index (EEI). In the intervention phase, all subjects showed significantly greater improvements in the 10MWT and EEI scores for rhythmic arm swing condition compared to those for arm fixation condition (p 0.05). 10MWT improvement rates: Subject 1-34.81% vs. 15.75%; Subject 2-40.00% vs. 17.95%; and Subject 3-38.08% vs. 21.85%; and EEI improvements: Subject 1-23.19% vs. 14.08%; Subject 2-26.15% vs. 20.43%; and Subject 3-22.99% vs. 14.49%. These findings suggest that rhythmic arm swing is clinically feasible as a more favorable option to enhance the effects of treadmill walking training. However, larger studies with a different study design are needed to be able to make any judgment about the usefulness of the treatment.

[Purpose] Interlimb coordination can be affected by the symptoms associated with Parkinson’s disease and may result in an increased risk of falls. The purpose of the current study was to compare changes in interlimb coordination in individuals with Parkinson’s disease to healthy older adults while systematically manipulating walking speed. [Subjects and Methods] Participants walked on a treadmill while systematically increasing and decreasing the walking speed between 0.22 and 1.30 m/s. Kinematic data were collected by means of a three dimensional motion capture system. Dependent variables included the phase relation between arm and leg movements as well as between pelvic and thoracic rotation. [Results] Compared to healthy controls, an increased variability in relative phase between left and right arm swing, and smaller amplitude with arm, leg as well as less variability for the phase relation between thoracic and pelvic rotations were shown in individuals with Parkinson’s disease. [Conclusion] The increased variability of phase relation between left and right arm swing may be related to the reduced out-of-phase forcing of the arm movements at the shoulders as a result of axial rigidity in Parkinson’s disease. It deserves further investigation whether the improvement of the coordination between arms could result in the normalization of parkinsonian gait.

The purpose of this pilot study was to assess the accuracy of steps and heart rate measurement of wrist fitness trackers at different prices. Four healthy college students voluntarily tested three wrist fitness bands and a sports watch (Xiaomi Mi Band, Fitbit Charge HRm, Fitbit Surge, and sports watch Polar M400). Subjects performed two sets of 10 series of 100 steps wearing the fitness trackers on an indoor track in two situations: walking and jogging. In the walking situation, the subjects wore a winter coat and gloves. The variables measured were the number of steps, the heart rate, and the level of error. The steps error percentage for all four devices was lower than 8%. The Fitbit Surge registered significantly more steps in the walking situation (p < 0.001). No significant differences were found in the steps measurements in the jogging situation (p = 0.138). In the jogging situation, significantly lower values in the heart rate measurements for the Xiaomi Mi Band, Fitbit Charger HR, and Fitbit Surge were found (p < 0.001). The results showed that the wearable fitness trackers were relatively accurate for tracking steps (on average, there was a level of error of 2–6%). The assessment of the steps was more accurate in the jogging situation (higher and faster arm swing) than in the walking situation, which involved wearing coats and gloves. The results showed that the wearable fitness trackers that were tested underestimate the heart rate with a level of error of approximately 6–11%. The step error was lower in the walking situation (less mobility of the devices). The price of the devices that were tested did not affect the accuracy of the steps and heart rate assessment. Further studies with a larger sample and more type of devices are needed to confirm these results.

[Purpose] The gait characteristics, such as short step, decreased arm swing and stooped posture, in Parkinson disease (PD) are well established. Pharmacological and non-pharmacological treatments have been attempted to improve the gait in patients with PD. Among the non-pharmacological approaches to improve gait, spinal orthosis can be applied for patients with spinal deformity. The purpose of our study was to investigate the effect of spinal kypho-orthosis to gait in PD by using a three-dimensional analysis. [Participants and Methods] Twenty-six patients with PD with gait disturbance with forward bend posture >15°. All participants were instructed to walk along a 6-m track and turn 180° and come back to the starting point under three-dimensional motion capture. The participants performed the test again with spinal kypho-orthosis. Gait parameters during examination is compared with and without spinal kypho-orthosis. The degree of forward flexion after kypho-orthosis in patients with PD was compared. [Results] Wearing the spinal kypho-orthosis significantly improved turning performance but did not affect locomotion. The severity of forward bend posture is mildly improved after the application of spinal kypho-orthosis. [Conclusion] Spinal kypho-orthosis has a short-term effect for gait performance, particularly during turning and erect posture. Spinal kypho-orthosis can be potentially used for management of turning deficits in PD.

Background Direction changes while walking are more likely to cause a hip fracture than is falling while walking in a straight line. Trunk stability is an important contributor to safe and effective walking, and arm movements influence trunk movement while walking. However, the difference in the trunk stability during semicircular turns performed by elderly women with a light bag has not been examined. Objective To investigate the effects of carrying a bag on trunk stability during semicircular turns in elderly women. Methods We enrolled 15 community-dwelling elderly women capable of independent walking. Participants walked with and without a bag at a self-selected speed along a marked path, which included semicircular turns, while fitted with an accelerometer attached over the L3 spinous process. Results Gait velocity was faster during semicircular turning with a bag versus without a bag. The normalized medial-lateral center of mass acceleration was lower during semicircular turning with a bag versus without a bag. Conclusions We suggest that a light additional arm load and increased arm swing contributes to trunk stability and efficient walking during semicircular turning by elderly women.
