Purpose The purpose of this study was to compare the dose–volume parameters and regression scatter plots of the iteratively improved RapidPlan (RP) models, specific knowledge‐based planning (KBP) models, in volumetric‐modulated arc therapy (VMAT) for prostate cancer over three periods. Methods A RP1 model was created from 47 clinical intensity‐modulated radiation therapy (IMRT)/VMAT plans. A RP2 model was created to exceed dosimetric goals which set as the mean values +1SD of the dose–volume parameters of RP1 (50 consecutive new clinical VMAT plans). A RP3 model was created with more strict dose constraints for organs at risks (OARs) than RP1 and RP2 models (50 consecutive anew clinical VMAT plans). To evaluate the feasibility of the use of iterative cone-beam computed tomography (CBCT) for dose calculation in the head and neck region.This study includes phantom and clinical studies. All acquired CBCT images were reconstructed with Feldkamp-Davis-Kress algorithm-based CBCT (FDK-CBCT) and iterative CBCT (iCBCT) algorithm. The Hounsfield unit (HU) consistency between the head and body phantoms was determined in both reconstruction techniques. Volumetric modulated arc therapy (VMAT) plans were generated for 16 head and neck patients on a planning CT scan, and the doses were recalculated on FDK-CBCT and iCBCT with Anisotropic Analytical Algorithm (AAA) and Acuros XB (AXB). As a comparison of the accuracy of dose calculations, the absolute dosimetric difference and 1%/1 mm gamma passing rate analysis were analyzed.The difference in the mean HU values between the head and body phantoms was larger for FDK-CBCT (max value: 449.1 HU) than iCBCT (260.0 HU). We aimed to evaluate the impact of rotational setup errors on the doses received during postoperative volumetric-modulated arc therapy (VMAT) for cervical cancer. Overall, 121 cone-beam computed tomography (CBCT) sets from 20 patients were rigidly registered to reference computed tomography (CT) sets based on bony landmarks. The rotational setup errors (pitch, yaw and roll) were calculated. Then, 121 CT sets involving rotational setup errors were created, and the dose distribution in these CT sets were recalculated. The recalculated dosimetric parameters for the clinical target volume (CTV) and organs at risk (OAR) were compared to the reference values, and the correlation coefficients between the dosimetric parameter differences and rotational setup errors were calculated. Only the pitch setup error was moderately correlated with CTV coverage (r ≥ 0.40) and strongly correlated with V45 for the bladder (r ≥ 0.91) and V40 for the rectum, small bowel and bone marrow (r ≥ 0.91). The maximum dosimetric difference in a single fraction and overall fractions was −1.59% and −0.69% in D98 for the CTV, 11.72% and 5.17% in V45 for the bladder and −8.03% and −4.68% in V40 for the rectum, respectively. In conclusion, rotational setup errors only slightly impact dose coverage during postoperative cervical cancer VMAT. However, the pitch setup error occasionally affected the doses received by the bladder or the rectum in the overall fraction when the error was systematic. Thus, rotational setup errors should be corrected by adjusting six-degree-of-freedom (DOF) couches to reduce dosimetric differences in the OARs.

Purpose This study investigated the intra‐fractional motion (IM) of patients immobilized using the QFix Encompass Immobilization System during HyperArc (HA) treatment. Method HA treatment was performed on 89 patients immobilized using the Encompass. The IM during treatment (including megavoltage (MV) registration) was analyzed for six degrees of freedom including three axes of translation (anterior‐posterior, superior‐inferior (SI) and left‐right (LR)) and three axes of rotation (pitch, roll, and yaw). Then, the no corrected IM (IMNC) was retrospectively simulated (excluding MV registration) in three directions (SI, LR, and yaw). Finally, the correlation between the treatment time and the IM of the 3D vector was assessed. Results The average IM in terms of the absolute displacement were 0.3 mm (SI), 0.3 mm (LR) and 0.2° (yaw) for Stereotactic radiosurgery (SRS), and 0.3 mm (SI), 0.2 mm (LR), and 0.2° (yaw) for stereotactic radiotherapy (SRT). The absolute maximum values of IM were 1 mm along the SI and LR axes and >1° along the yaw axis. In the correlation between the treatment time and the IM, the r‐values were −0.025 and 0.027 for SRS and SRT respectively, along the axes of translation. For the axes of rotation, the r‐values were 0.012 and 0.206 for SRS and SRT, respectively. Conclusion Encompass provided patient immobilization with adequate accuracy during HA treatment. The absolute maximum displacement IM was less than IMNC along the translational/rotational axes, and no statistically significant relationship between the treatment time and the IM was observed.

The immobilization of patients with a bite block (BB) carries the risk of interpersonal infection, particularly in the context of pandemics such as COVID-19. Here, we compared the intra-fractional patient setup error (intra-SE) with and without a BB during fractionated intracranial stereotactic irradiation (STI). Fifteen patients with brain metastases were immobilized using a BB without a medical mask, while 15 patients were immobilized without using a BB and with a medical mask. The intra-SEs in six directions (anterior–posterior (AP), superior–inferior (SI), left–right (LR), pitch, roll, and yaw) were calculated by using cone-beam computed tomography images acquired before and after the treatments. We analyzed a total of 53 and 67 treatment sessions for the with- and without-BB groups, respectively. AIM: This study evaluated a convolutional neural network (CNN) for automatically delineating the liver on contrast-enhanced or non-contrast-enhanced CT, making comparisons with a commercial automated technique (MIM Maestro®). BACKGROUND: Intensity-modulated radiation therapy requires careful labor-intensive planning involving delineation of the target and organs on CT or MR images to ensure delivery of the effective dose to the target while avoiding organs at risk. MATERIALS AND METHODS: Contrast-enhanced planning CT images from 101 pancreatic cancer cases and accompanying mask images showing manually-delineated liver contours were used to train the CNN to segment the liver. The trained CNN then performed liver segmentation on a further 20 contrast-enhanced and 15 non-contrastenhanced CT image sets, producing three-dimensional mask images of the liver. Dosimetric evaluation and variation assessment were performed with two knowledge-based planning (KBP) models created at different periods for volumetric-modulated arc therapy (VMAT) for prostate cancer at five institutes. The first and second models (F- and S-models) for KBP were created before April 2017 and April 2019, respectively. The S-model was created using feedback plans from the F-model. Dose evaluation was compared between the two models using the same two computed tomography (CT) datasets and structures. The evaluation metrics were the dose received by 95.0% and 2.0% of the planning target volume (PTV); dose-volume parameters to the rectum and bladder as V90, V80, and V50; and monitor unit (MU). Dosimetric variation was compared by exporting estimated dose-volume histograms for each model to the Model Analytics website and assessing the organ at risk volume. There were no dosimetric differences between the two models for PTV. The V50 of the rectum in the S-model had improved compared to that of the F-model (case I: 49.3 ± 15.6 and 43.5 ± 15.2 [p = 0.08]; case II: 42.5 ± 16.9 and 36.0 ± 15.6 [p = 0.138]). The differences in other parameters were within ± 1.8% between the rectum and the bladder. The MU was slightly higher in the S-model than in the F-model, and dosimetric variation was reduced to the rectum and bladder among all the institutes. The polished S-model for KBP could be used for standardization of the plan quality and sharing of KBP models in VMAT for prostate cancer.

Background/aim To evaluate treatment schedules involving concurrent chemoradiotherapy in stage III non-small cell lung cancer (NSCLC) using the tumor control probability (TCP) and normal tissue complication probability (NTCP) parameters. Patients and methods The standard schedules were compared with two types of schedules, the dose escalation and the short-term schedules. Standard schedules were 60-74 Gy in 30-37 fractions. The dose escalation schedules with hypofractionation and hyperfractionation were 69 Gy in 30 fractions and 69.6 Gy in 58 fractions, respectively, twice per day (b.i.d). The short-term schedules were concomitant boost, 64 Gy in 40 fractions b.i.d. and the accelerated radiotherapy schedule, 57.6 Gy in 36 fractions, three fractions per day (t.i.d). Results The average TCP for the short-term schedules was more than 16% in two tumor models; however, the TCP for standard and dose escalation schedules was less than 5%. In each organ, the increase in NTCP for the short-term schedule compared with standard schedules was less than 15%. Conclusion The short-term schedules had an advantage over standard schedules for NSCLC.

Purpose: This study aimed to investigate the influence of cleaned-up knowledge-based treatment planning (KBP) models on the plan quality for volumetric-modulated arc therapy (VMAT) of prostate cancer. Materials and Methods: Thirty prostate cancer VMAT plans were enrolled and evaluated according to four KBP modeling methods as follows: (1) model not cleaned – trained by fifty other clinical plans (KBPORIG); (2) cases cleaned by removing plans that did not meet all clinical goals of the dosimetric parameters, derived from dose–volume histogram (DVH) (KBPC-DVH); (3) cases cleaned outside the range of ±1 standard deviation through the principal component analysis regression plots (KBPC-REG); and (4) cases cleaned using both methods (2) and (3) (KBPC-ALL). Rectal and bladder structures in the training models numbered 34 and 48 for KBPC-DVH, 37 and 33 for KBPC-REG, and 26 and 33 for KBPC-ALL, respectively. The dosimetric parameters for each model with one-time auto-optimization were compared. Results: All KBP models improved target dose coverage and conformity and provided comparable sparing of organs at risks (rectal and bladder walls). There were no significant differences in plan quality among the KBP models. Nevertheless, only the KBPC-ALL model generated no cases of >1% V78 Gy (prescribed dose) to the rectal wall, whereas the KBPORIG, KBPC-DVH, and KBPC-REG models included two, four, and three cases, respectively, which were difficult to overcome with KBP because the planning target volume (PTV) and rectum regions overlapped. Conclusions: The cleaned-up KBP model based on DVH and regression plots improved plan quality in the PTV–rectum overlap region.

Virtual monochromatic images (VMIs) at a lower energy level can improve image quality but the computed tomography (CT) number of iodine contained in the contrast-enhanced agent is dramatically increased. We assessed the effect of the use of contrast-enhanced agent on the dose distributions in volumetric modulated arc therapy (VMAT) planning for head and neck cancer (HNC). Based on the VMIs at 40 keV (VMI40keV ), 60 keV(VMI60keV ), and 77 keV (VMI77keV ) of a tissue characterization phantom, lookup tables (LUTs) were created. VMAT plans were generated for 15 HNC patients based on contrast-enhanced- (CE-) VMIs at 40-, 60-, and 77 keV using the corresponding LUTs, and the doses were recalculated based on the noncontrast-enhanced- (nCE-) VMIs. For all structures, the difference in CT numbers owing to the contrast-enhanced agent was prominent as the energy level of the VMI decreased, and the mean differences in CT number between CE- and nCE-VMI was the largest for the clinical target volume (CTV) (125.3, 55.9, and 33.1 HU for VMI40keV , VMI60keV , and VMI77keV, respectively). Purpose In stereotactic radiosurgery (SRS) with single‐isocentric treatments for brain metastases, rotational setup errors may cause considerable dosimetric effects. We assessed the dosimetric effects on HyperArc plans for single and multiple metastases. Methods For 29 patients (1–8 brain metastases), HyperArc plans with a prescription dose of 20–24 Gy for a dose that covers 95% (D95%) of the planning target volume (PTV) were retrospectively generated (Ref‐plan). Subsequently, the computed tomography (CT) used for the Ref‐plan and cone‐beam CT acquired during treatments (Rot‐CT) were registered. The HyperArc plans involving rotational setup errors (Rot‐plan) were generated by re‐calculating doses based on the Rot‐CT. The dosimetric parameters between the two plans were compared. Results The dosimetric parameters [D99%, D95%, D1%, homogeneity index, and conformity index (CI)] for the single‐metastasis cases were comparable (P > 0.05), whereas the D95% for each PTV of the Rot‐plan decreased 10.8% on average, and the CI of the Rot‐plan was also significantly lower than that of the Ref‐plan (Ref‐plan vs Rot‐plan, 0.93 ± 0.02 vs 0.75 ± 0.14, P

Purpose The aim of this study was to investigate whether additional manual objectives are necessary for the RapidPlan (RP) with a single optimization. We conducted multi-institutional comparisons of plan quality for head and neck cancer (HNC) using the models created at each institute. Methods The ability of RP to produce acceptable plans for dose requirements was evaluated in two types of oropharynx cancers at five institutes in Japan. Volumetric modulated arc therapy plans created without (RP plan) and with additional manual objectives (M-RP plan) were compared in terms of planning target volume (PTV), brainstem, spinal cord and parotid glands in dosimetric parameters. Results There were no major dosimetric PTV differences between RP and M-RP plans. For the brainstem and spinal cord in the RP plans, only 40% and 30% of the plans achieved the dose requirements, while the M-RP plans with upper objective added to volume 0% at all institutes achieved them for 90% of the plans. For the L-parotid gland, there was no difference in the RP and M-RP plans (both were 40%) in achieving the acceptable criteria. For the R-parotid gland, 60% and 80% of the RP and M-RP plans achieved the constraint criteria, and in terms of the achievement rate, the RP plans were relatively high. Conclusions M-RP plans did not require reoptimization; only an upper objective was needed for the brainstem and spinal cord, while the parotid gland dose was reduced in both RP plans with the auto generated line objectives alone.

Extracorporeal membrane oxygenation (ECMO) therapy might be controversial when patients with advanced malignant disease develop heart or lung failure refractory to conventional management. Especially as for the hematological malignancy patients, the induction of ECMO therapy must be considered carefully, since it is often associated with bleeding complications or infectious diseases. Here, we report a case of life-threatening airway obstruction requiring ECMO. The trachea of the patient was narrowed by an anterior mediastinal tumor too highly to ventilate both lungs, so she had to be connected to venovenous-ECMO (VV-ECMO) before pathological examination and radical treatment were planned. During the intensive care with ECMO, she was diagnosed with malignant lymphoma and the chemotherapy was started. The chemotherapy brought such an immediate result that the trachea regained its patency and ECMO was disconnected 9 days after the initiation of the chemotherapy. Then, the patient was able to leave the intensive care unit with no sequelae. Central airway obstruction is a life-threatening situation, in which prompt decisions are essential. On the other hand, if the airway is impaired by hematological malignancy, it might be generally challenging to consider ECMO as a bridge until the chemotherapy takes an effect. The prognosis of hematological malignancy differs according to its subtype or stage, so it may be reasonable to take ECMO therapy into consideration for some population.

Purpose To evaluate the utility of the use of iterative cone-beam computed tomography (CBCT) for machine log file-based dose verification during volumetric modulated arc therapy (VMAT) for prostate cancer patients. Methods All CBCT acquisition data were used to reconstruct images with the Feldkamp-Davis-Kress algorithm (FDK-CBCT) and the novel iterative algorithm (iCBCT). The Hounsfield unit (HU)-electron density curves for CBCT images were created using the Advanced Electron Density Phantom. The I'mRT and anthropomorphic phantoms were irradiated with VMAT after CBCT registration. Subsequently, fourteen prostate cancer patients received VMAT after CBCT registration. Machine log files and both CBCT images were exported to the PerFRACTION software, and a 3D patient dose was reconstructed. Mean dose for planning target volume (PTV), the bladder, and rectum and the 3D gamma analysis were evaluated. Results For the phantom studies, the variation of HU values was observed at the central position surrounding the bones in FDK-CBCT. There were almost no changes in the difference of doses at the isocenter between measurement and reconstructed dose for planning CT (pCT), FDK-CBCT, and iCBCT. Mean dose differences of PTV, rectum, and bladder between iCBCT and pCT were approximately 2% lower than those between FDK-CBCT and pCT. For the clinical study, average gamma analysis for 2%/2 mm was 98.22% ± 1.07 and 98.81% ± 1.25% in FDK-CBCT and iCBCT, respectively. Conclusions A similar machine log file-based dose verification accuracy is obtained for FDK-CBCT and iCBCT during VMAT for prostate cancer patients.

This study applied a “shell bolus,” an immobilizing thermoplastic shell locally thickened with extra layers over the radiation target, during postmastectomy radiation therapy (PMRT). We performed ion chamber and film measurements for a solid water phantom for thermoplastic sheets and a gel bolus for dosimetric characterization using a 6-MV X-ray flattening-filter-free (FFF) beam. The air gaps between the body surface for the gel and shell bolus were measured using computed tomography (CT) images in patients who underwent PMRT. This included seven and 13 patients treated with the gel and shell boluses, respectively. A comparison of the dose differences between a 10-mm gel bolus and a 9.6-mm-thick thermoplastic sheet at the surface and 5 cm below the surface showed a 4.2% higher surface dose and 0.5% lower dose at 5-cm depth for the thermoplastic sheet compared to those for the gel bolus. To construct a deep convolutional neural network that generates virtual monochromatic images (VMIs) from single-energy computed tomography (SECT) images for improved pancreatic cancer imaging quality.Fifty patients with pancreatic cancer underwent a dual-energy CT simulation and VMIs at 77 and 60 keV were reconstructed. A 2D deep densely connected convolutional neural network was modeled to learn the relationship between the VMIs at 77 (input) and 60 keV (ground-truth). Subsequently, VMIs were generated for 20 patients from SECT images using the trained deep learning model.The contrast-to-noise ratio was significantly improved (p 0.001) in the generated VMIs (4.1 ± 1.8) compared to the SECT images (2.8 ± 1.1). The mean overall image quality (4.1 ± 0.6) and tumor enhancement (3.6 ± 0.6) in the generated VMIs assessed on a five-point scale were significantly higher (p 0.001) than that in the SECT images (3.2 ± 0.4 and 2.8 ± 0.4 for overall image quality and tumor enhancement, respectively).The quality of the SECT image was significantly improved both objectively and subjectively using the proposed deep learning model for pancreatic tumors in radiotherapy.

Purpose: To evaluate customizing a knowledge-based planning (KBP) model using dosimetric analysis for volumetric modulated arc therapy for pancreatic cancer. Materials and Methods: The first model (M1) using 56 plans and the second model (M2) using 31 plans were created in the first 7 months of the study. The ratios of volume of both kidneys overlapping the expanded planning target volume to the total volume of both kidneys (Voverlap/Vwhole) were calculated in all cases to customize M1. Regression lines were derived from Voverlap/Vwhole and mean dose to both kidneys. The third model (M3) was created using 30 plans which data put them below the regression line. For validation, KBP was performed with the three models on 21 patients. Results: V18 of the left kidney for M1 plans was 7.3% greater than for clinical plans. Dmean of the left kidney for M2 plans was 2.2% greater than for clinical plans. There was no significant difference between all kidney doses in M3 and clinical plans. Dmean of the left kidney for M2 plans was 2.2% greater than for clinical plans. Dmean to both kidneys did not differ significantly between the three models in validation plans with Voverlap/Vwhole lower than average. In plans with larger than average volumes, the Dmean of validation plans created by M3 was significantly lower for both kidneys by 1.7 and 0.9 Gy than with M1 and M2, respectively. Conclusions: Selecting plans to register in a model by analyzing dosimetry and geometry is an effective means of improving the KBP model.

Background Total scalp irradiation presents technical and dosimetric challenges. While reports suggest that HyperArc, a new stereotactic radiosurgery planning technique applied to non-coplanar volumetric-modulated arc therapy (VMAT) technique, is associated with high conformity and rapid dose fall-off, the performance of HyperArc for total scalp irradiation has not been explored. The current study aimed to compare the dosimetric performance of HyperArc plans with those of non-coplanar VMAT plans in angiosarcoma of the scalp. Methods Ten patients with angiosarcoma of the scalp were included in this study. The performance of three different plans administered using TrueBeam Edge were compared: non-coplanar VMAT using flattening filter (FF) beams (VMAT-FF), HyperArc using FF beams (HyperArc-FF), and HyperArc using flattening filter free (FFF) beams (HyperArc-FFF). The dose distribution, dosimetric parameters, and dosimetric accuracy for each of these plans were evaluated. Results The three plans showed no statistically significant differences in target volume coverage, conformity, and homogeneity. The HyperArc-FF and HyperArc-FFF plans provided significantly lower mean brain doses (12.63 ± 3.31 Gy and 12.71 ± 3.40 Gy) than did the VMAT-FF plans (17.11 ± 5.25 Gy). There were almost no differences in sparing the organs at risk between the HyperArc-FF and HyperArc-FFF plans. The HyperArc-FF and HyperArc-FFF plans provided a shorter beam-on time than did the VMAT-FF plan. The 3%/2 mm gamma test pass rates were above 95% for all three plans. Conclusions Our results suggest that the HyperArc plan can be potentially used for radiation therapy of target regions with large and complicated shapes, such as the scalp, and that there are no advantages of using FFF beams.

BACKGROUND: The aim of this study was to clarify factors predicting the performance of knowledge-based planning (KBP) models in volume modulated arc therapy for prostate cancer in terms of sparing the organ at risk (OAR). MATERIALS AND METHODS: In three institutions, each KBP model was trained by more than 20 library plans (LP) per model. To validate the characterization of each KBP model, 45 validation plans (VP) were calculated by the KBP system. The ratios of overlap between the OAR volume and the planning target volume (PTV) to the whole organ volume (V(overlap)/V(whole)) were analyzed for each LP and VP. Regression lines between dose–volume parameters (V(90), V(75), and V(50)) and V(overlap)/V(whole) were evaluated. The mean OAR dose, V(90), V(75), and V(50) of LP did not necessarily match those of VP. RESULTS: In both the rectum and bladder, the dose–volume parameters for VP were strongly correlated with V(overlap)/V(whole) at institutes A, B, and C (R > 0.74, 0.85, and 0.56, respectively). Except in the rectum at institute B, the slopes of the regression lines for LP corresponded to those for VP. For dose–volume parameters for the rectum, the ratios of slopes of the regression lines in VP to those in LP ranged 0.51–1.26. In the bladder, most ratios were less than 1.0 (mean: 0.77). CONCLUSION: For each OAR, each model made distinct dosimetric characterizations in terms of V(overlap)/V(whole). The relationship between dose–volume parameters and V(overlap)/V(whole) of OARs in LP predicts the KBP models’ performance sparing OARs.

The STE20-like serine/threonine kinases MST1 and MST2 (MST1/2) are mammalian homologs of Hippo in flies. MST1/2 regulate organ size by suppressing the transcription factor YAP, which promotes proliferation. MST1 is predominantly expressed in immune cells, where it plays distinct roles. Here, we review the functions of MST1/2 in immune cells, uncovered by a series of recent studies, and discuss the connection between MST1/2 function and immune responses. MST1/2 regulate lymphocyte development, trafficking, survival, and antigen recognition by naive T cells. MST1/2 also regulate the function of regulatory T cells and effector T cell differentiation, thus acting to balance immune activation and tolerance. Interestingly, MST1/2 elicit these functions not by the "canonical" Hippo pathway, but by the non-canonical Hippo pathway or alternative pathways. In these pathways, MST1/2 regulates cellular processes relating to immune response, such as chemotaxis, cell adhesion, immunological synapse, gene transcriptions. Recent advances in our understanding of the molecular mechanisms of these processes have revealed important roles of MST1/2 in regulating cytoskeleton remodeling, integrin activation, and vesicular transport in lymphocytes. We discuss the significance of the MST1/2 signaling in lymphocytes in the regulation of organelle dynamics.

Background: The aim of this study was to investigate the performance of the RapidPlan knowledge-based treatment planning system using models including registered pseudo-structures, and to determine how many structures are required for automatic optimization of volumetric modulated arc therapy (VMAT) for postoperative uterine cervical cancer. Methods: Pseudo-structures were retrospectively contoured for patients who had completed treatment at one of five institutions. For 22 patients, RPs were generated with a single optimization for models with two (RP_2), four (RP_4), or five (RP_5) registered structures, and the dosimetric parameters of these models were compared with a clinical plan with several optimizations. The total times for pseudo-structure creation and optimization were also measured.Results: Most dosimetric parameters showed no major differences between each RP. In particular, the rectum Dmax, V50Gy, and V40Gy with RP_2, RP_4, and RP_5 were not significantly different, and were lower than those of the clinical plan. In addition, the average proportions of plans achieving acceptable criteria for all dosimetric parameters were 98%, 99%, 98%, and 98% for the clinical plan, RP_2, RP_4, and RP_5, respectively. The average times for the creation and optimization of pseudo-structures were 105, 17, 21, and 29 minutes, for the clinical plan, RP_2, RP_4, and RP_5, respectively. Conclusions: The RapidPlan model with two registered pseudo-structures could generate clinically acceptable plans while saving time. This modeling approach using pseudo-structures could possibility be used for the VMAT planning process.

Background: The aim of the study was to evaluate analysis criteria for the identification of the presence of rectal gas during volumetric modulated arc therapy (VMAT) for prostate cancer patients by using electronic portal imaging device (EPID)-based in vivo dosimetry (IVD). Materials and methods: All measurements were performed by determining the cumulative EPID images in an integrated acquisition mode and analyzed using PerFRACTION commercial software. Systematic setup errors were simulated by moving the anthropomorphic phantom in each translational and rotational direction. The inhomogeneity regions were also simulated by the I’mRT phantom attached to the Quasar phantom. The presence of small and large air cavities (12 and 48 cm 3 ) was controlled by moving the Quasar phantom in several timings during VMAT. Sixteen prostate cancer patients received EPID-based IVD during VMAT. Results: In the phantom study, no systematic setup error was detected in the range that can happen in clinical ( < 5-mm and < 3 degree). The pass rate of 2% dose difference (DD2%) in small and large air cavities was 98.74% and 79.05%, respectively, in the appearance of the air cavity after irradiation three quarter times. In the clinical study, some fractions caused a sharp decline in the DD2% pass rate. The proportion for DD2% < 90% was 13.4% of all fractions. Rectal gas was confirmed in 11.0% of fractions by acquiring kilo-voltage X-ray images after the treatment. Conclusions: Our results suggest that analysis criteria of 2% dose difference in EPID-based IVD was a suitable method for identification of rectal gas during VMAT for prostate cancer patients.

Summary Extracellular signal-regulated kinase (ERK) plays critical roles in T cell development in the thymus. Nevertheless, the dynamics of ERK activity and the role of ERK in regulating thymocyte motility remain largely unknown due to technical limitations. To visualize ERK activity in thymocytes, we here developed knockin reporter mice expressing a Förster/fluorescence resonance energy transfer (FRET)-based biosensor for ERK from the ROSA26 locus. Live imaging of thymocytes isolated from the reporter mice revealed that ERK regulates thymocyte motility in a subtype-specific manner. Negative correlation between ERK activity and motility was observed in CD4/CD8 double-positive thymocytes and CD8 single-positive thymocytes, but not in CD4 single-positive thymocytes. Interestingly, however, the temporal deviations of ERK activity from the average correlate with the motility of CD4 single-positive thymocytes. Metformin, a widely used medicine for diabetes mellitus, is well known to cause lactic acidosis, which may require intensive care, including hemodialysis, especially in severe cases because of the high mortality rate. Because metformin is effectively removed by renal replacement therapy, early initiation of hemodialysis is crucial for metformin overdose. We report a case of a 38-year-old male who was brought to our emergency department (ED) with altered mental status. He subsequently developed severe lactic acidosis with a peak lactate level of 33.4 mmol/L. No clear etiology was identified for these critical conditions at the ED. Although metformin intoxication could not be confirmed at this time, we decided to start hemodialysis immediately. Soon after hemodialysis, blood pH and lactate levels dramatically improved. The patient fully recovered after 9 days of intensive care unit (ICU) stay. Later on, his serum metformin concentration was revealed to be 146 μg/mL and 26 μg/mL at ICU admission and after dialysis at day 3, respectively (therapeutic range, 1–2 μg/mL). Early initiation of hemodialysis might be beneficial for diabetic patients with unexplained severe lactic acidosis despite uncertainties in history of overdose or serum concentration of metformin.

Purpose: The aim of this study is to compare the interfractional setup reproducibility of two types of patient immobilization devices for prostate cancer receiving image-guided radiation therapy (IGRT). Materials and Methods: The MOLDCARE (MC) involves hydraulic fixation, whereas the BlueBAG (BB) and Vac-Lock (VL) involve vacuum fixation. For 72 patients, each immobilization device was individually customized during computed tomography (CT) simulation. Before the treatment, bony registration was performed using orthogonal kV images and digitally reconstructed radiographs. The shift of the treatment couch was recorded as a benchmark in the first session. In subsequent sessions, the shifts from the benchmark were measured and analyzed. Soft-tissue registration was performed weekly by cone-beam CT and CT images, and the shifts were measured and analyzed. Results: In the superior-inferior and left-right directions, there were nearly no changes in the overall mean among the immobilization devices. In the anterior-posterior (AP) direction, the overall mean for the MC, BB, and VL were 0.34 ± 1.33, −0.47 ± 1.27, and −1.82 ± 1.65 mm, respectively. The mean shifts along the AP direction were approximately 1 mm more in patients immobilized on the BB and 2.5 mm more in those on the VL, compared to those on the MC, after the twentieth treatment. No significant changes were observed among the patients immobilized on those devices, respectively, in soft-tissue registration. Conclusion: It can be concluded that the settling of the vacuum fixation was caused by air leakage in the latter-half treatment, and the immobilization device type has no effect on the treatment-position reproducibility in IGRT.

A 35-year-old woman intentionally took 40,000 mg of lithium carbonate, and she was transferred to our hospital with nausea, vomiting, and diarrhea. She was diagnosed as having bipolar disorder 10 years ago and was receiving oral lithium therapy. Blood test results on arrival were remarkable for a negative anion gap of -2.1 and later, the serum lithium level turned out to be as high as 15.4 mEq/L. Intubation was required because of disrupted consciousness, and continuous hemodiafiltration (CHDF) was immediately started in the intensive care unit to obtain constant removal of lithium. After adding intermittent hemodialysis (IHD) twice during the daytime to accelerate the lithium clearance, CHDF became unnecessary on day 4, and she was extubated on day 6 with complete recovery of consciousness. Close monitoring of the patient data showed recovery of the decreased anion gap as indicator of the serum lithium level reduction. On day 36, she was discharged without any complication and sequela. The current case highlighted the effective use of CHDF between IHD sessions to prevent the rebound elevation of lithium and the role of the anion gap as a surrogate marker of serum lithium concentration during the treatment.

The aim of this study was to investigate the accuracy and efficacy of two commonly used commercial immobilization systems for stereotactic body radiation therapy (SBRT) in lung cancer. This retrospective study assessed the efficacy and setup accuracy of two immobilization systems: the Elekta Body Frame (EBF) and the Civco Body Pro‐Lok (CBP) in 80 patients evenly divided for each system. A cone beam CT (CBCT) was used before each treatment fraction for setup correction in both devices. Analyzed shifts were applied for setup correction and CBCT was repeated. If a large shift (>5 mm) occurred in any direction, an additional CBCT was employed for verification after localization. The efficacy of patient setup was analyzed for 105 sessions (48 with the EBF, 57 with the CBP). Result indicates that the CBCT was repeated at the 1st treatment session in 22.5% and 47.5% of the EBF and CBP cases, respectively. The systematic errors {left–right (LR), anterior–posterior (AP), cranio‐caudal (CC), and 3D vector shift: (LR 2 + AP 2 + CC 2)1/2 (mm)}, were {0.5 ± 3.7, 2.3 ± 2.5, 0.7 ± 3.5, 7.1 ± 3.1} mm and {0.4 ± 3.6, 0.7 ± 4.0, 0.0 ± 5.5, 9.2 ± 4.2} mm, and the random setup errors were {5.1, 3.0, 3.5, 3.9} mm and {4.6, 4.8, 5.4, 5.3} mm for the EBF and the CBP, respectively. The 3D vector shift was significantly larger for the CBP (P < 0.01). The setup time was slightly longer for the EBF (EBF: 15.1 min, CBP: 13.7 min), but the difference was not statistically significant. It is concluded that adequate accuracy in SBRT can be achieved with either system if image guidance is used. However, patient comfort could dictate the use of CBP system with slightly reduced accuracy.

We investigated whether methods conventionally used to evaluate patient-specific QA in volumetric-modulated arc therapy (VMAT) for intracranial tumors detect clinically relevant dosimetric errors. OBJECTIVES The objective of this study was to assess the objective and subjective qualities of the contrast-enhanced 4-dimensional dual-energy computed tomography using adaptive statistical iterative reconstruction (ASiR) and ASiR-V. METHODS The virtual monochromatic images at 60 keV were reconstructed using filtered back projection, ASiR, and ASiR-V (10%-100%) for 14 patients with pancreatic cancer. The contrast-to-noise ratio (CNR) was calculated, and the subjective measurements were compared based on a 5-point score scale. RESULTS The ASiR-V yielded a significantly higher CNR than ASiR (P < 0.05). The subjective image quality (peak) was significantly improved (P < 0.01) with ASiR (50%) (3.8, 3.5, and 4.0; overall image quality, tumor delineation, and noise, respectively) and with ASiR-V (50%) (3.9, 3.5, and 4.2, respectively) compared with the filtered back projection (3.2, 3.2, and 3.0, respectively). CONCLUSIONS The ASiR-V yielded higher CNR than ASiR and provided the highest subjective scores regarding the overall image quality.

Purpose This study aimed to generate a functional image of the liver using dual-energy computed tomography (DECT) and a functional-image-based stereotactic body radiation therapy plan to minimize the dose to the volume of the functional liver (Vfl). Material and methods A normalized iodine density (NID) map was generated for fifteen patients with liver tumors. The volume of liver with an NID Results FLR showed a significantly strong correlation with FIB-4 (r = −0.71, p 0.05). For Vfl, F-VMAT plans achieved lower V5Gy (122.4 ± 31.7 vs 181.1 ± 57.3 cc), V10Gy (44.4 ± 22.2 vs 98.2 ± 33.3 cc), V15Gy (22.6 ± 20.3 vs 49.8 ± 33.7 cc), V20Gy (11.6 ± 14.1 vs 24.9 ± 25.1 cc), and Dmean (3.9 ± 2.3 vs 5.8 ± 3.0 Gy) values than the C-VMAT plans (p Conclusions The functional image derived from DECT was successfully used, allowing for a reduction in the dose to the Vfl without compromising target coverage.

PURPOSE The aim of this study was to develop a deep learning (DL) method for generating virtual noncontrast (VNC) computed tomography (CT) images from contrast-enhanced (CE) CT images (VNCDL ) and to evaluate its performance in dose calculations for head and neck radiotherapy in comparison with VNC images derived from a dual-energy CT (DECT) scanner (VNCDECT ). METHODS This retrospective study included data for 61 patients who underwent head and neck radiotherapy. All planning CT images were obtained with a single-source DECT scanner (80 and 140 kVp) with rapid kVp switching. The DL-based method used a pair of virtual monochromatic images (VMIs) at 70 keV with and without contrast materials. VMIs without contrast materials were used as reference true noncontrast (TNC) images. Deformable image registration was used between the TNC and CE images. We used the data of 45 patients, chosen randomly, for training (7922 paired images), and data from the other 16 patients as test data. We generated the VNCDL images with a densely connected convolutional network. As the VNCDECT images, we used VMIs with the iodine signal suppressed, reconstructed from the CE images of the 16 test patients. The CT numbers of the tumor, common carotid artery, internal jugular vein, muscle, fat, bone marrow, cortical bone, and mandible of each VNC image were compared with those of the TNC image. The dose of the reference TNC plan was recalculated using the CE, VNCDL , and VNCDECT images. Difference maps of the dose distributions and dose-volume histograms were evaluated. RESULTS The mean prediction time for the VNCDL images was 3.4 s per patient, and the mean number of slices was 204. The absolute differences in CT numbers of the VNCDL images were significantly smaller than those of the VNCDECT images for the bone marrow (8.0 ± 6.5 vs 175.1 ± 40.9 HU; P 1.0% were observed in bone regions. We assessed the effect of collimator angle on the dosimetric parameters for targets and organs at risk (OARs) for collimator-optimized HA (CO-HA) and non-CO-HA (nCO-HA) plans. The nCO-HA and CO-HA plans were retrospectively generated for 26 patients (1 to 8 brain metastases). The dosimetric parameters for planning target volume (homogeneity index [HI]; conformity index [CI]; gradient index [GI]) and for OARs were compared. The modulation complexity score for volumetric modulated arc therapy (MCSV) and monitor units (MUs) were calculated. Doses were measured using the electronic portal imaging device and compared with the expected doses. Dosimetric parameters of the HI, CI, and GI for single (n = 12) and multiple (n = 14) metastases cases were comparable (p > 0.05). For multiple metastases cases, the CO-HA plan provided lower V4Gy, V12Gy, V14Gy, V16Gy for brain tissue compared to the nCO-HA plan (p 0.05). For multiple metastases cases, the CO-HA plan resulted in less complex multileaf collimator (MLC) patterns (MCSV = 0.19 ± 0.04, p 0.05). The collimator optimization in the HA planning reduced doses to brain tissues and improved the treatment efficacy.

Objective: To evaluate and compare the dosimetric plan quality for noncoplanar volumetric arc therapy of single and multiple brain metastases using the linear accelerator-based radiosurgery system HyperArc and a robotic radiosurgery system. Methods: 31 tumors from 24 patients were treated by stereotactic radiosurgery using the CyberKnife system. CT images, structure sets, and dose files were transferred to the Eclipse treatment planning system for the HyperArc system. Dosimetric parameters for both plans were compared. The beam-on time was calculated from the total monitor unit and dose rate. Results: For normal brain tissue, the received volume doses were significantly lower for HyperArc than for CyberKnife_G4 and strongly correlated with the planning target volume (PTV) for cases of single brain metastasis. In addition, the difference in volume dose between CyberKnife_G4 and HyperArc was proportional to the PTV. For multiple brain metastases, no significant difference was observed between the two stereotactic radiosurgery systems, except for high-dose region in the normal tissue. In low dose for brain minus PTV, when the maximum distance among each target was above 8.0 cm, HyperArc delivered higher dose than CyberKnife_G4. The mean ± SDs for the beam-on time were 15.8 ± 5.3 and 5.6 ± 0.8 min for CyberKnife_G4 and HyperArc, respectively (p < .01). Conclusion: HyperArc is best suited for larger targets in single brain metastasis and for smaller inter tumor tumor distances in multiple brain metastases. Advances in knowledge: The performance of HyperArc in comparison with CyberKnife_G4 was depended on defined margin and tumor distances.

This study aimed to obtain details regarding treatment planning techniques for lung stereotactic body radiation therapy (SBRT) employed at each institution in Japan by using a questionnaire survey. An Internet questionnaire survey on SBRT procedures performed in 2016 was conducted by the QA/QC committee of the Japan Society of Medical Physics from April to June 2017. The questionnaire assessed two aspects: the environment for SBRT at each institution and the treatment planning techniques with and without respiratory motion management techniques (RMMT). Of the 309 evaluated responses, 218 institutions had performed SBRT. A total of 186 institutions performed SBRT without RMMT and 139 institutions performed SBRT with RMMT. When respiratory motion was ≥10 mm, 69 institutions applied RMMT. The leading RMMT were breath holding (77 institutions), respiratory gating (49 institutions) and real-time tumor tracking (11 institutions). The most frequently used irradiation technique was 3D conformal radiotherapy, which was used in 145 institutions without RMMT and 119 institutions with RMMT. Computed tomography (CT) images acquired under free breathing were mostly used for dose calculation for patients treated without RMMT. The aim of this study was to compare three strategies for intensity-modulated radiotherapy (IMRT) for 20 head-and-neck cancer patients. For simultaneous integrated boost (SIB), doses were 66 and 54 Gy in 30 fractions for PTVboost and PTVelective, respectively. Two-phase IMRT delivered 50 Gy in 25 fractions to PTVelective in the First Plan, and 20 Gy in 10 fractions to PTVboost in the Second Plan. Sequential SIB (SEQ-SIB) delivered 55 Gy and 50 Gy in 25 fractions, respectively, to PTVboost and PTVelective using SIB in the First Plan and 11 Gy in 5 fractions to PTVboost in the Second Plan. Conformity indexes (CIs) (mean ± SD) for PTVboost and PTVelective were 1.09 ± 0.05 and 1.34 ± 0.12 for SIB, 1.39 ± 0.14 and 1.80 ± 0.28 for two-phase IMRT, and 1.14 ± 0.07 and 1.60 ± 0.18 for SEQ-SIB, respectively. CI was significantly highest for two-phase IMRT. Maximum doses (Dmax) to the spinal cord were 42.1 ± 1.5 Gy for SIB, 43.9 ± 1.0 Gy for two-phase IMRT and 40.3 ± 1.8 Gy for SEQ-SIB. Brainstem Dmax were 50.1 ± 2.2 Gy for SIB, 50.5 ± 4.6 Gy for two-phase IMRT and 47.4 ± 3.6 Gy for SEQ-SIB. Spinal cord Dmax for the three techniques was significantly different, and brainstem Dmax was significantly lower for SEQ-SIB. The compromised conformity of two-phase IMRT can result in higher doses to organs at risk (OARs). Lower OAR doses in SEQ-SIB made SEQ-SIB an alternative to SIB, which applies unconventional doses per fraction.

Purpose To investigate whether a novel iterative cone-beam computed tomography (CBCT) reconstruction algorithm reduces metal artifacts in head and neck patient images. Method An anthropomorphic phantom and 35 patients with dental metal prostheses or implants were analyzed. All CBCT images were acquired using a TrueBeam linear accelerator and reconstructed with a Feldkamp–Davis–Kress algorithm-based CBCT (FDK-CBCT) and an iterative CBCT algorithm. The mean Hounsfield unit (HU) and standard deviation values were measured on the tongue near the metal materials and the unaffected region as reference values. The artifact index (AI) was calculated. For objective image analysis, the HU value and AI were compared between FDK-CBCT and iterative CBCT images in phantom and clinical studies. Subjective image analyses of metal artifact scores and soft tissue visualizations were conducted using a five-point scale by two reviewers in the clinical study. Results The HU value and AI showed significant artifact reduction for the iterative CBCT than for the FDK-CBCT images (phantom study: 389.8 vs.−10.3 for HU value, 322.9 vs. 96.2 for AI, FDK-CBCT vs. iterative CBCT, respectively; clinical study: 210.3 vs. 69.0 for HU value, 149.6 vs. 70.7 for AI). The subjective scores in the clinical patient study were improved in the iterative CBCT images (metal artifact score: 1.1 vs. 2.9, FDK-CBCT vs. iterative CBCT, respectively; soft tissue visualization: 1.8 vs. 3.6). Conclusions The iterative CBCT reconstruction algorithm substantially reduced metal artifacts caused by dental metal prostheses and improved soft tissue visualization compared to FDK-CBCT in phantom and clinical studies.

OBJECTIVE: To develop a mathematical model for cardiac dose estimation for patients who have been treated for left-sided breast cancer without CT data. METHODS: After obtaining institutional review board approvals, 147 patients with left-sided breast cancer were selected that were treated supine with opposed tangents. The heart blocks from the tangential fields were removed and dose calculations were performed with 6 MV beams using an advanced algorithm. This study was performed with CT data with DRR to represent a radiographic simulator image of yesteryear treatments. The beam’s eye-view images showing delineated breast, lung and heart were created to represent views on radiograph. The maximum heart distance (MHD) was recorded and irradiated heart area (AREA) was computed by combination of triangles and rectangles. Based on accurate 3D dose calculation, mean dose (D(mean)) and V(10) to V(50) of heart were analyzed with respect to MHD and AREA for dosimetric parameters using linear and quadratic fit. RESULTS: The treatment parameters calculated by MHD and segments using 2D radiographs were within 2% of the actual dosimetric parameters computed from the 3D planning system. The MHD and AREA vs D(mean), V(10), V(20), V(30) and V(50) showed very good correlation with linear model (R(2) > 0.91); however the correlation was significantly better with quadratic model (R(2) > 0.92). PURPOSE A contrast-enhancing agent is imperative for the accurate target delineation of pancreatic tumors. This study demonstrates the potential use of treatment planning for patients with pancreatic tumors based on the water density image (WDI) generated by dual-energy computed tomography (DECT). METHODS Tissue characterization and multi-energy phantom scanning were performed through DECT and the physical characteristics of the WDI and a virtual monochromatic image (VMI) were assessed. The measured and the corresponding theoretical electron density relative to water (RED) and mass density (MD) were compared. Treatment plans based on the WDI (TPWDI ) and VMI (TPVMI ) were compared for 22 pancreatic cancer patients who underwent contrast-enhanced DECT scan. RESULTS The total absolute difference in the HU value between the conventional 120 kVp images and the VMI was the smallest at the energy level of 77 keV (3.3 HU), and the VMI at 77 keV was used for subsequent analysis. OBJECTIVE The objective of this study was to assess the accuracy of the quantitative measurements obtained using dual-energy computed tomography with metal artifact reduction software (MARS). METHODS Dual-energy computed tomography scans (fast kV-switching) are performed on a phantom, by varying the number of metal rods (Ti and Pb) and reference iodine materials. Objective and subjective image analyses are performed on retroreconstructed virtual monochromatic images (VMIs) (VMI at 70 keV). RESULTS The maximum artifact indices for VMI-Ti and VMI-Pb (5 metal rods) with MARS (without MARS) were 17.4 (166.7) and 34.6 (810.6), respectively; MARS significantly improved the mean subjective 5-point score (P < 0.05). The maximum differences between the measured Hounsfield unit and theoretical values for 5 mg/mL iodine and 2-mm core rods were -42.2% and -68.5%, for VMI-Ti and VMI-Pb (5 metal rods), respectively, and the corresponding differences in the iodine concentration were -64.7% and -73.0%, respectively. CONCLUSIONS Metal artifact reduction software improved the objective and subjective image quality; however, the quantitative values were underestimated.

Background The aim of this study was to evaluate the performance of a commercial knowledge-based planning system, in volumetric modulated arc therapy for prostate cancer at multiple radiation therapy departments. Methods In each institute, > 20 cases were assessed. For the knowledge-based planning, the estimated dose (ED) based on geometric and dosimetric information of plans was generated in the model. Lower and upper limits of estimated dose were saved as dose volume histograms for each organ at risk. To verify whether the models performed correctly, KBP was compared with manual optimization planning in two cases. The relationships between the EDs in the models and the ratio of the OAR volumes overlapping volume with PTV to the whole organ volume (Voverlap/Vwhole) were investigated. Results There were no significant dosimetric differences in OARs and PTV between manual optimization planning and knowledge-based planning. In knowledge-based planning, the difference in the volume ratio of receiving 90% and 50% of the prescribed dose (V90 and V50) between institutes were more than 5.0% and 10.0%, respectively. The calculated doses with knowledge-based planning were between the upper and lower limits of ED or slightly under the lower limit of ED. The relationships between the lower limit of ED and Voverlap/Vwhole were different among the models. In the V90 and V50 for the rectum, the maximum differences between the lower limit of ED among institutes were 8.2% and 53.5% when Voverlap/Vwhole for the rectum was 10%. In the V90 and V50 for the bladder, the maximum differences of the lower limit of ED among institutes were 15.1% and 33.1% when Voverlap/Vwhole for the bladder was 10%. Conclusion Organs’ upper and lower limits of ED in the models correlated closely with the Voverlap/Vwhole. It is important to determine whether the models in KBP match a different institute’s plan design before the models can be shared.
