This research builds a healthcare analytics adoption model, drawing insights from institutional theory literature to identify the key institutional drivers of managerial beliefs and subsequent healthcare analytics adoption in U.S. hospitals. A sample of 223 healthcare employees completed a survey to test how institutional forces in the business environment influence managerial beliefs and adoption of healthcare analytics at any given point in time. The study results indicate that mimetic pressure positively influences the level of adoption of healthcare analytics and coercive pressure positively influences managerial beliefs. The findings also show that though healthcare organizations are embedded in organizational networks, normative pressure has no significant role in shaping managerial beliefs in healthcare analytics adoption., The 2010 passage of the Affordable Care Act provided strong motivation for healthcare organizations to improve operational efficiency and enhance organizational performance. In this paper, a new method for steady-state screening of buses to identify possible fault-induced delayed-voltage recovery problems is presented. The method is particularly useful for 1) prioritizing substation relaying and breaker maintenance, 2) evaluating impacts of system improvement alternatives, 3) identifying most-effective locations for undervoltage load shedding, and 4) locating voltage (reactive) support equipment. The method is based on a single index value for each bus that is determined by considering the amount of load impacted by a fault at that bus using a weighted summation. The steady-state screening algorithm has been validated using dynamic simulations and has been used to successfully identify as a potentially severe contingency the location of an actual event that occurred on the Southern Eiectric System in the summer of 1999. In this paper, a new relay algorithm is proposed for under-voltage load shedding (UVLS) based on rate of voltage recovery and predicted time to recover above a preset voltage threshold. Simulation experience in the Southern Electric System has shown that conventional UVLS relay logic is ill-suited to the task of mitigating delayed voltage recovery and fast voltage collapse situations while maintaining compliance with existing NERC Reliability Standards. The proposed scheme continuously monitors both voltage magnitude and rate-of-change of voltage magnitude and uses these values to determine a projected voltage recovery time following a contingency event. If the projected recovery time is such that resultant system conditions are unacceptable (e.g., major load loss, loss of generation, or voltage collapse), the relay will operate to shed load at the distribution feeder level. This unique scheme has been implemented in a commercially-available programmable relay and is presently installed and active in a number of substations in Georgia Power Company. Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p p Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.

High temperature stress imposed during the cell division stage of maize (Zea mays L.) kernel development adversely affects growth and mature mass. However, the processes affected by heat stress are not known. The goal of this study was to determine the mechanisms by which heat stress affects early zein accumulation in maize kernels. Intact ears of growth chamber-grown plants were subjected to heat stress (continuous 35[degrees]C) for 2 or 4 d, starting at 5 d after pollination (DAP). Both the 27 kDa and cluster 1 zeins of subfamily 4 (ZSF4C1) zein mRNA steady-state levels were significantly delayed by 4 d of heal stress (DHS), but were not affected by 2 DHS. Similarly, transcription rates of both zeins were reduced in the endosperm of kernels exposed to 4 DHS treatment up to 17 DAP. The 2 DHS treatment significantly delayed endosperm endoreduplication, up to 17 DAP, whereas 4 DHS significantly repressed it. The lack of coordinate changes among mRNA steady-state levels, transcription rates, and endoreduplication during heat stress indicates that the effects of heat stress on zein transcription rates may not be directly related to alterations in endoreduplication. Instead, zein transcription is most likely affected by a delay in endosperm development.

Heat stress is a major factor limiting crop yield in many agricultural regions. During the early stage of kernel development, heat stress is particularly detrimental to subsequent dry matter accumulation since it causes disruption of cell division, sugar metabolism, and starch biosynthesis in the endosperm. The effects of heat stress on protein accumulation, however, is less well understood. Therefore, the objective of this study was to determine the mechanisms by which heat stress decreases protein accumulation and alters composition of developing maize (Zea mays L.) kernels. In this study, maize ears were heat stressed for 2 and 4 d at continuous 35°C starting at 5 d after pollination (DAP). Endosperms were analyzed for the relative proportion of each Osborne protein solubility class fraction and for individual zein proteins. The 2- and 4-d heat stress (DHS) treatment caused a 20 and 48% reduction in kernel final dry weight, respectively, and protein content was similarly reduced. Specifically, zein content was reduced by an average of 53%, but zein composition was only mildly affected. The concentrations of glutelin and albumin plus globulin tended to increase throughout most of 4-DHS kernel development. L-[[sup.35]S]-methionine incorporation in the zein fraction was delayed by the 4 DHS when compared to control and 2-DHS treated kernels. Therefore, we concluded that heat stress during early stages of endosperm development repressed zein accumulation at the synthesis level. In contrast, later in development, zein accumulation appeared to be repressed mainly by protein degradation, which appears to be a part of the natural progression of kernel development since there was no apparent significant effect of heat stress treatments on this process. It has always been clear that one of the main driving forces behind the space race was Cold War propaganda. But just what impact did it have on 'the minds of men everywhere'? This paper presents a survey of the impact of space exploration on popular culture in Britain in the 1950s and 1960s, concentrating chiefly on the cinema. Before the launch of Sputnik in 1957, although space was present in popular culture, it was generally aimed at a specialised audience. In the 1960s, interest in space became widespread and reached unexpected corners of popular culture such as pop music. This popular success had dangerous consequences, however: when public interest waned in the early 1970s, the immense cost of the Moon landings could no longer be justified on propaganda grounds and the project was massively reduced.

Consider a thin wing in frictionless flow and suppose the plan form of the wing and also the total lift to be given. The drag of the wing will then depend on the way in which the lift is distributed over its surface. In a previous paper, it was shown that the minimum drag occurs when the superposition of the induced disturbance fields in forward and reversed motion results in a constant value of the induced downwash at all points of the wing surface. Similar problems involving the ideal distribution of thickness over the surface were found to lead to similar conditions governing the distribution of pressure in the superimposed or 'combined' flow field. The present paper describes a method for determining mathematically the combined disturbance field, and in certain cases the minimum drag, of wings at supersonic speeds. The simplest analytic example is provided by the wing of elliptic plan form, which achieves its minimum drag when the lift is distributed uniformly over the surface. With a symmetrical distribution of thickness, the requirement of minimum drag for a given total volume is found to lead to profiles of constant curvature.

Argentina's current economic crisis is used as a backdrop to illustrate some fundamental economic principles, notably convertibility. The country set up a currency board ten years ago to peg the Argentinian peso to the United States dollar. This was viewed as an anti-inflationary device and a way to gain stability and respectability and end its reputation for hyper-inflation. However, the economy's underlying structural weakness reappeared as rising unemployment and shrinking real gross domestic product.

The studies reported here examined the validity of SME trainability judgments from two perspectives. We found that (1) different SME groups' judgments related differently to actual learning changes in KSAOs covered in a training program and (2) sources of variability thought to be unrelated to the job influenced trainability ratings. Implications for the selection of SMEs in job analysis applications intended to determine the trainability of various job components are outlined. The Group of 8 has proposed debt relief in the Cologne agreement of 1999. Developing economies increased their debts in the late 1970s, but rising interest rates increased borrowing costs and crises arose. Heavily Indebted Poor Countries (HIPCs) are the targets of debt relief programs. Some 41 countries need special assistance, according to a 1996 report. These countries have low life expectancy rates and many people lack basic amenities such as clean water. Divisions between rich and poor countries have widened, and it is argued that rich countries are damaged by this inequality as well as poor countries.

Demand is rising steadily but slowly, and is set to reach around 52 million units in 2000, from around 45 million units in 1995. Car manufacturers are being forced to make a wide range of special offers in order to eliminate surplus stock, and this is in turn having a negative impact on their profit margins. It is clear that manufacturers must reduce costs, and mergers are regarded as a way to bring greater economies of scale and rationalize some production procedures.

Introduction Proinflammatory cytokine interleukin-17A (IL-17A) is overexpressed in a subset of patients with lung cancer. We hypothesized that IL-17A promotes a protumorigenic inflammatory phenotype and inhibits antitumor immune responses. Methods We generated bitransgenic mice expressing a conditional IL-17A allele along with conditional Kras G12D and performed immune phenotyping of mouse lungs, a survival analysis, and treatment studies with antibodies either blocking programmed cell death 1 (PD-1) or IL-6 or depleting neutrophils. To support the preclinical findings, we analyzed human gene expression data sets and immune profiled patient lung tumors. Results Tumors in IL-17:Kras G12D mice grew more rapidly, resulting in a significantly shorter survival as compared with that of Kras G12D mice. IL-6, granulocyte colony-stimulating factor (G-CSF), milk fat globule-EGF factor 8 protein, and C-X-C motif chemokine ligand 1 were increased in the lungs of IL17:Kras mice. Time course analysis revealed that levels of tumor-associated neutrophils were significantly increased, and lymphocyte recruitment was significantly reduced in IL17:Kras G12D mice as compared with in Kras G12D mice. In therapeutic studies PD-1 blockade was not effective in treating IL-17:Kras G12D tumors. In contrast, blocking IL-6 or depleting neutrophils with an anti–Ly-6G antibody in the IL17:Kras G12D tumors resulted in a clinical response associated with T-cell activation. In tumors from patients with lung cancer with KRAS mutation we found a correlation between higher levels of IL-17A and colony- stimulating factor 3 and a significant correlation among high neutrophil and lower T-cell numbers. Conclusions Here we have shown that an increase in a single cytokine, IL-17A, without additional mutations can promote lung cancer growth by promoting inflammation, which contributes to resistance to PD-1 blockade and sensitizes tumors to cytokine and neutrophil depletion.

Importance The patterns of disease recurrence after resection of pancreatic ductal adenocarcinoma with adjuvant chemotherapy remain unclear. Objective To define patterns of recurrence after adjuvant chemotherapy and the association with survival. Design, Setting, and Participants Prospectively collected data from the phase 3 European Study Group for Pancreatic Cancer 4 adjuvant clinical trial, an international multicenter study. The study included 730 patients who had resection and adjuvant chemotherapy for pancreatic cancer. Data were analyzed between July 2017 and May 2019. Interventions Randomization to adjuvant gemcitabine or gemcitabine plus capecitabine. Main Outcomes and Measures Overall survival, recurrence, and sites of recurrence. Results Of the 730 patients, median age was 65 years (range 37-81 years), 414 were men (57%), and 316 were women (43%). The median follow-up time from randomization was 43.2 months (95% CI, 39.7-45.5 months), with overall survival from time of surgery of 27.9 months (95% CI, 24.8-29.9 months) with gemcitabine and 30.2 months (95% CI, 25.8-33.5 months) with the combination (HR, 0.81; 95% CI, 0.68-0.98;P = .03). The 5-year survival estimates were 17.1% (95% CI, 11.6%-23.5%) and 28.0% (22.0%-34.3%), respectively. Recurrence occurred in 479 patients (65.6%); another 78 patients (10.7%) died without recurrence. Local recurrence occurred at a median of 11.63 months (95% CI, 10.05-12.19 months), significantly different from those with distant recurrence with a median of 9.49 months (95% CI, 8.44-10.71 months) (HR, 1.21; 95% CI, 1.01-1.45;P = .04). Following recurrence, the median survival was 9.36 months (95% CI, 8.08-10.48 months) for local recurrence and 8.94 months (95% CI, 7.82-11.17 months) with distant recurrence (HR, 0.89; 95% CI, 0.73-1.09;P = .27). The median overall survival of patients with distant-only recurrence (23.03 months; 95% CI, 19.55-25.85 months) or local with distant recurrence (23.82 months; 95% CI, 17.48-28.32 months) was not significantly different from those with only local recurrence (24.83 months; 95% CI, 22.96-27.63 months) (P = .85 andP = .35, respectively). Gemcitabine plus capecitabine had a 21% reduction of death following recurrence compared with monotherapy (HR, 0.79; 95% CI, 0.64-0.98;P = .03). Conclusions and Relevance There were no significant differences between the time to recurrence and subsequent and overall survival between local and distant recurrence. Pancreatic cancer behaves as a systemic disease requiring effective systemic therapy after resection. The General Agreement on Tariffs and Trade (GATT), known as the World Trade Organisation (WTO) since Jan 1995, celebrated its 50th anniversary in May 1998. GATT proved very successful, with average tariff levels being cut from 40% to 4% in the period 1950 to 1996, while the volume of world merchandise trade rose by 1,500%. WTO has differed from GATT in a number of ways. It has much more extensive powers than GATT to arbitrate trade conflicts, and is able to impose sanctions on member countries that do not comply with the rulings of its dispute panels.

The role of the state has expanded in developed economies since 1900, and there was a large increase in subsidies and transfers from 1960 to 1990. There is concern about the increasing costs of pensions as the population ages, but this problem is less acute in the UK than in many economies. The government sought to reduce state spending from 1979 to 1990 but this was difficult due to demand for education and health services. Transfers and subsidies have also grown. The Labor government aims to spend funds more efficiently rather than increase spending. The role of the state is greater in developed than in developing economies.

The biggest 300 United Kingdom companies increased their spending on research and development (R and D) by less than the increase in their sales in 1996, according to the Department of Trade and Industry. There is concern that UK companies are not investing enough in R and D, since such investment contributes to growth and profits. A case study of an Italian company, Candy Elettrodomestici, highlights the UK's low labor costs which may deter investment. UK labor costs are lower than those of Italy, but productivity is also lower.

Due to the limited availability of telemetered data on distribution circuits, an integral part of any practical distribution circuit state estimator is a load modeling procedure that provides estimates of load demands at all circuit nodes. Traditional load modeling techniques used for distribution power flows are designed for system peak studies, and are not suitable for providing real-time statistical load estimates. This paper presents a real-time load modeling technique which incorporates the use of customer class curves and provides a measure of the uncertainty (statistics) in the estimates. It also incorporates the possibility of multiple line flow measurements and makes use of customer billing information. The load modeling technique is combined with a probabilistic distribution circuit state estimation algorithm. Results of the combined load modeling and state estimation algorithm are provided on an actual 934-bus distribution circuit illustrating its feasibility. There has been considerable debate recently about the relative merits of the approaches to job creation adopted in continental Europe and in the UK and the US. The US/UK model emphasises the need for considerable labour market flexibility, thus bringing reduced job security and a limited role for the trade unions. In contrast, the continental European model stresses job security and extensive worker participation. The UK has certainly seen improvements in labour costs, productivity and unemployment, but it remains uncertain whether flexibility will boost competitiveness in the long term.

Past work on distribution circuit state estimation has focused on the adoption of a transmission state estimator approach, without necessarily accounting for the specific requirements of a distribution circuit-based analysis. On distribution circuits, typically, there are very few available real-time measurements, and thus, researchers have treated customer load demand estimates as pseudo. measurements in a weighted-least-squares formulation. This can lead to convergence problems, and also, the approach effectively assumes that all bus load demands are normally distributed (gaussian) which may not be valid on distribution circuits. This paper presents an alternative approach to distribution circuit state estimation using a probabilistic extension of the radial load flow algorithm while accounting for real-time measurements as solution constraints. The algorithm which takes advantage of the radial nature of distribution circuits also accounts for other issues specific to distribution circuits. Namely, the algorithm accounts for non-normally distributed loads, incorporates the concept of load diversity (load correlation) and can interact with a load allocation routine. The effectiveness of the algorithm is illustrated through comparisons made with Monte Carlo simulations. There are several ways of measuring the UK's economic performance and competitiveness relative to other countries. The traditional method is to use per capita gross domestic product at purchasing power parity: this 'prosperity league' shows that the UK has fallen from 13th place in 1979 to 18th place in 1994. However, the index of competitiveness gives a better guide to future economic performance: the World Economic Forum's index attaches great importance to public finances and labour market flexibility, and therefore ranks the UK 15th in 1996, up vs 18th in 1995.

Previous research has shown that kernel sink capacity, the potential of maize (Zea mays L.) kernels to achieve maximal mass, is established during the early stages of kernel development and is a function of the number of endosperm cells and starch granules formed. Kernel growth was characterized in four disparate inbred lines (which differed in relative maturity and kernel size) and their reciprocal crosses. Relative maturity affected the pattern of kernel development (i.e., when endosperm cell division and starch granule formation was initiated) but not the maximum number of endosperm or starch granules formed nor the mature kernel mass obtained. Kernel growth rate and mature kernel mass of the large kernel inbreds were nearly twofold higher than that of the small kernel inbreds, regardless of relative maturity. Comparison of [F.sub.1] kernels from reciprocal crosses revealed a strong maternal influence on mature kernel mass. However, in reciprocal crosses between A619 (large kernel) and W64A (small kernel), the maternal effects on mature kernel mass were due to changes in the number of endosperm cells formed, whereas in reciprocal crosses between Mo17 (large kernel) and B73 (small kernel), it was due to modulation of the number of starch granules. The results confirm that the number of endosperm cells and/or starch granules are good indicators of kernel sink capacity in maize. Moreover, the data show a strong maternal influence on kernel sink capacity and mature kernel mass. However, whether the number of endosperm cells or the number of starch granules is the more important determinant of mature kernel mass is genotype dependent.

Faults on distribution circuits are normally detected by simple overcurrent relays. Faults through a high impedance such as dry earth or asphalt do not have sufficient current to operate overcurrent relays and must be cleared manually. Such high impedance faults may pose a hazard to the public. Equipment is now commercially available to detect a high percentage of high impedance faults, but use of this equipment introduces operational issues which affect applications. This paper discusses technical as well as non-technical issues associated with applying high impedance fault detectors. Recognizing all the issues is important to the effective implementation of these new fault detecting techniques.
