2. Advanced harmonization techniques result in accurate establishment of in vitro-in vivo correlations for oxybenzone from four complex dermal formulations with reapplication.

Due to high variability during clinical pharmacokinetic (PK) evaluation, the prediction of in vivo exposure from in vitro absorption testing of topical semisolid and liquid dermal products has historically proven difficult. Since absorption from unoccluded formulations can be influenced by environmental factors such as temperature and humidity, maximal effort must be placed on the harmonization of experimental parameters between in vitro and in vivo testing conditions to establish accurate in vitro/in vivo correlations (IVIVC). Using four different sunscreen formulations as a model, we performed in vitro permeation testing (IVPT) studies with excised human skin and maintained strict harmonization techniques to control application time, occlusion, temperature, and humidity during in vivo human serum PK evaluation. The goal was to investigate if increased control over experimental parameters would result in decreased inter-subject variability of common topical formulations leading to acceptable IVIVC establishment. Using a deconvolution-based approach, excellent point-to-point (Level A correlation) IVIVC for the entire 12-h study duration was achieved for all four sunscreen formulations with < 10% prediction error of both area under the curve (AUC) and peak concentration (C
max ) estimation. The low variability of in vivo absorption data presents a proof-of-concept protocol design for testing of complex semisolid and liquid topical formulations applied over a large surface area with reapplication in a reliable manner. In der eigenen Häuslichkeit versorgt zu werden, ist für viele pflegebedürftige Menschen von großer Bedeutung. Neben Angehörigen gewährleisten ambulante Pflegedienste die häusliche Versorgung. In Deutschland wird ein Viertel der zu Hause betreuten pflegebedürftigen Menschen von ambulanten Pflegediensten versorgt. Das Thema der Qualität der ambulanten Pflege ist zunehmend Gegenstand pflegefachlicher und politischer Diskussionen. Eine Möglichkeit zur Messung von Versorgungsqualität ist die Nutzung von Routinedaten von Kranken- bzw. Pflegekassen. Für die ambulante Pflege gab es hierzu bislang keine Überlegungen. Im Rahmen der Forschungsarbeit wurde daher die generelle Nutzbarkeit von Routinedaten für die ambulante Pflege untersucht. Im Ergebnis zeigt sich, dass eine Reihe von qualitätsrelevanten Versorgungsaspekten in der ambulanten Pflege anhand von Routinedaten operationalisierbar ist. Die Mehrzahl der relevanten Versorgungsaspekte ist jedoch nicht über Routinedaten abbildbar. Hierfür wären unter anderem grundlegende Anpassungen der gesetzlichen Rahmenbedingungen erforderlich.

This book intends to investigate the broad spectrum of genetic changes in immunological processes involved in cutaneous diseases. One of the main goals of immunogenetic studies is finding susceptibility genes for complex diseases. This can provide an insight into the pathogenesis of the condition in a way that is not easily achievable through other kinds of studies. Thus they are a rational initial step for generating hypotheses about disease pathogenesis. This may especially benefit dermatology, a field notorious for having too many diseases with unknown etiologies.Immunogenetic investigations have made targeted treatment strategies possible for diseases such as psoriasis and pemphigus. Even though these strategies have revolutionized the management of chronic dermatological conditions such as psoriasis, still there are a lot of unanswered questions. For instance, psoriasis patients respond very differently to each of the commercially available biological agents. This diversity could be partially explained by the differences in the sets of genes responsible for disease induction in each individual. Thus whole genome sequencing strategies, if feasible at individual levels, might help in tailoring these targeted treatments based on specific genetic backgrounds.Our intention in preparing this book was to explore the broad spectrum of the genetic aspects of immunological processes involved in cutaneous diseases. We have tried to cover most areas of dermatology where enough studies were available to gather a chapter. Still, there is a substantial lack of knowledge on the immunogenetics of many dermatological conditions. We hope that this book would encourage the investigators to fill these gaps of knowledge.

It is indeed a pleasure to write a foreword for this spectacular book which represents a landmark in the history of ocular pathology. Not since the three volumes of the Henke-Lubarsch handbook appeared forty years ago has anything like this come on the market. This book represents a compre hensive, thorough and up-to-date clinically oriented textbook on ocular pathology. We are all indebted to the senior author, who is now professor of ophthal mology at the University of Erlangen, who presents us here with the fruit of decades of industrious endeavors. We also have to be grateful to the American ophthalmic pathologist, Dr. David Apple, who, as one of the main collaborators of the German edition, has now provided us with the English translation. The book has a twofold purpose: First, it is meant to be a source of instruction for ophthalmologists and pathologists. For that purpose it is beautifully illustrated both by clinical pictures and by excellent photomicro graphs and electromicroscopic pictures. Most valuable from a didactic point of view are the colored schematic drawings and the many tables. These two features are practically unique and should help any neophyte in grasp ing the principles of ocular pathology.

The past 10 years have seen a remarkable change in the approach to cardiac arrhythmias, from a position of confidence and a feeling of well-being about pharmacological treatment to a situation in which there is now marked uncertainty and general apprehension about the role of antiarrhythmic drugs. Until relatively recently the prevailing concept in antiarrhythmic therapy was that arrhythmias could be controlled by drugs which slowed conduction or suppressed automaticity, goals well served by the sodium channel-blocking drugs and glycosides. Drug re search was based largely on the development of agents mimicking those already available, but with greater efficacy, fewer side effects or a more favourable phar macokinetic profile. The CAST trial stands out as a landmark in the evolution of arrhytmia manage ment; rarely has a single trial had such a profound impact not only on clinical prac tice, but also on the whole approach of those involved in the research, development and regulation of antiarrhythmic drugs. The results of the CAST trial, designed to redress the shortcomings of earlier trials which had failed to demonstrate the anticipated improvement in mortality post-myocardial infarction with the use of class I agents, are well known. The CAST and CAST II showed an increase in mor tality associated with the active agent (encainide, flecainide or morizicine) com pared to placebo treatment. They firmly established the potential danger in the use of class I drugs.

Tis book originated as a small reference manual that I created to serve as an educational supplement for the dermatology residents a-t Loui siana State University Health Sciences Center. Deeming the compiled information to be useful for all dermatologists, I decided to expand the text and publish it. Every major category of the patient evaluation, from the chief complaint to the diagnosis, is addressed with regard to the dermatological diferential diagnosis. Te establishment of a precise diferential diagnosis for a gi- ven cu taneous problem is the fundamental challenge that the dermatologist faces with every patient. Tis unique exercise is very intellectual; in a short period of time the clinician must select from a list of perhaps several hundred diseases a few possibilities that match the clinic-al pre sentation. Tis is performed while also negotiating the patient - interac tion, examining the patient, and beginning to formulate a pla- n of ac tion. Profciency in the formulation of a diferential diagnosis that is brief and simultaneously thorough allows for consideration of- all pos sibilities, proper evaluation, and, hopefully, rapid diagnosis. We hope to provide the target readers (dermatologists and dermatologists- -in-train ing) with some assistance in carrying out this frequently complicated task. For the confrontation with an atypical presentation of a common disease or the classic presentation of an uncommon disease, the reader will hopefully fnd this book very useful.

Seit 1985 wird der deutsche Arzneimittelmarkt im Arzneiverordnungs-Report analysiert. Datengrundlage ist die Vollerfassung von 400 Millionen Rezepten für die Patienten der gesetzlichen Krankenversicherung. Die Arzneimittelausgaben nahmen 2004 erstmals seit 12 Jahren um 2,5 Mrd. € auf 21,7 Mrd. € durch das GKV-Modernisierungsgesetz ab. Wesentliche Elemente der Kostensenkung waren der Ausschluss rezeptfreier Arzneimittel aus der vertragsärztlichen Versorgung, erhöhte Rabatte der Arzneimittelhersteller an die Krankenkassen, erhöhte Patientenzuzahlungen und die Einführung einer Praxisgebühr. Aktuelle Verordnungstrends werden an 3000 Arzneimitteln in 46 Indikationsgruppen dargestellt. Besonders eingegangen wird auf: Bewertung von 33 Neueinführungen des Jahres 2004, Einsparerfolge bei Generika und umstrittenen Arzneimitteln, Verordnungsprofil rezeptfreier Arzneimittel. Als Schwerpunkthema werden die Arzneiverordnungen pharmazeutischer Hersteller und ihr Produktsortiment im Kindesalter dargestellt. Immer noch problematisch ist hier die Anwendung von Arzneimitteln außerhalb der Zulassung (Off label). Noch häufiger werden bei Kindern Arzneimittel mit umstrittener Wirkung verordnet. This book is the third in aseries of publications devoted to the biogeographieal and ecological research in the Southern Hemisphere, published in the'Monographiae Biologicae'. After dealing with Australia (vol. VIII) and Southern Africa (Vol. XIV) it was thought essential to include Antarctiea in this series. Ever since the expedition of the'Belgiea'made the first suc cessful wintering within the antarctie circle in 1898 and brought back a very rieh harvest of scientific data, Belgium kept a vivid interest in Antarctiea and took an active part in the modern and international exploration of this vast continent. As part of their programs for the International Geophysieal Year (I. G. Y.) twelve nations established permanent or semi-permanent bases on the Antarctie Continent or on subantarctie islands. Thus a new era of vast and free international scientific collaboration in the Antarctie was opened and it culminated in the formulation and the signing of the Antarctic Treaty (Washington 1959). It was recognized and accepted that'Antarctiea'shall be used for peaceful purposes only and'Freedom of scientific investigation in Antarctiea and coopera tion toward that end, as applied during the I. G. Y., shall continue..'In order to organize this collaboration e. g. by full exchange of programs and resuIts a'Special Committee on Antarctie Research'(S. C. A. R.) was founded in 1957.
