Leading an empowered organisation (LEO) is a 3-day course that has been delivered to over 40000 front-line managers in the NHS, among them many district nurses. The aim of this review was to identify and assess all published literature about LEO; to critically appraise such evidence relating to the impact of LEO on participants' leadership behaviour and to critically appraise any evidence of the need for follow-up activities to sustain participants learning. Because of the limited quantity and quality of research literature (n=6) it was considered legitimate to also include discussion papers (n=2) and descriptive papers (n=4) in the review. All papers were reviewed using a framework devised by the author broadly based on the critical appraisal checklist for qualitative research (Greenhalgh and Donald, 2000). From the available evidence it can be concluded that the LEO programme does have a positive impact on the leadership behaviour of participants, and that follow-up activities are valuable to consolidate and sustain participants' learning. Public health services will need to cope with additional demands due to an ageing society and the increasing prevalence of chronic conditions. Lower-limb ulceration is a long-term, life-changing condition and leg ulcer management can be challenging for nursing staff. The Lindsay Leg Club model is a unique partnership between community nurses, members and the local community, which provides quality of care and empowerment for patients with leg ulcers, while also supporting and educating nursing staff. The Leg Club model works in accord with core themes of Government and NHS policy. Patient feedback on the Leg Club model is positive and the Leg Clubs provide a service to members which is well accepted by patients, yet is more economically efficient than the traditional district nursing practice of home visits. The introduction of social enterprises into healthcare has brought about opportunities and challenges for providers and clinicians. Dudley Lymphoedema Service, a team originally of five staff members, has undergone radical changes. The team has successfully gone through the Department of Health's right to request programme to move from a primary care trust service to become a social enterprise, LymphCare UK, which is a community interest company. There are only approximately 40 NHS services throughout the country that have taken this option as part of government changes and plans to put clinicians at the helm of services. Becoming a social enterprise has led to the service becoming bespoke, more responsive, flexible and innovative. Stepping out of the NHS has not been an easy journey but is one that the team is confident will improve outcomes for the organisation and the patients it serves. INSETS: Box 1.
