This article discusses the benefits of upgrading the pump seal casing in a US pulp and paper mill. Vertical pumps often rely on packing instead of mechanical seals, but packing has disadvantages such as the need for continuous adjustment and fluid leakage. The mill decided to upgrade to a seal casing design to address reliability and environmental concerns. The seal casing design provides advantages such as minimal maintenance, zero leakage above the baseplate, and improved safety. Change has become the norm and with the digital revolution we are seeing the demise of the printed word. Dentistry is a pictorial subject, but will pictures in a textbook or journal be surpassed by accessing digital ones on a screen? This article reviews the rapidly changing access and reading of dental publications. An argument is that the ease of access to electronic material, such as video and podcasts, may make traditional paper-based publications obsolete. Already dentistry is seeing such changes take place. With such revolutions come challenges. Misinformation is much easier to generate, and the evidence base may be diluted. Furthermore artificial intelligence is now able to produce text and papers that will make it more difficult to critically review the dental content on offer. The future will bring many challenges and this article provides an opinion on what dentistry might expect. Healthcare policies promote technology use as a means to modernise healthcare and support seamless, person-centred care. However, despite information technology (IT) use being common practice in clinical settings, its use in patients' homes is still developing. This study explored patients' perspectives on the use of IT and electronic health records (EHR) in their home environment. Semi structured interviews were conducted with housebound patients who received regular care from the district nursing team, and thematic data analysis was undertaken. Participants reported variable knowledge and experiences with mobile working and EHR. Most were positive and identified clear benefits for clinicians. However, few participants reported benefits to themselves. Contrary to popular belief, IT use is expected by older patients and, while barriers were identified, the overall opinion was positive. Prompt application of appropriate compression therapy is essential for effective treatment of lymphoedema. However, it is accepted that prior to the application of compression to the lower limbs, either with bandaging or compression garments, patients should demonstrate a satisfactory vascular status, as assessed via axillary brachial pressure index (ABPI). Unfortunately, the presence of peripheral oedema may render a reading impossible or grossly inaccurate. Relying solely on ABPI assessment is potentially harmful to patients, who may be denied appropriate treatment or experience complications and deterioration of their condition due to delayed treatment. The British Lymphology Society recognises a need to focus more on clinical assessment skills to determine vascular status, rather than relying on ABPI alone. Thus, the Society has developed guidance and a practical tool to support clinical decision-making and enhance practitioner confidence in the safe application of compression therapy in the absence of ABPI. Background: The documentation of patient data on health records is a vital component of the care process. Accurate and complete recording of this data is a necessary practice. The adoption of electronic health records to improve the quality of nursing documentation is on the rise. Objectives: This study compares the accuracy and completeness of pressure ulcer data documentation between electronic and paper records. Design: A descriptive, comparative design with a retrospective review of patient records. Settings and sample: Two hospitals were chosen purposefully, one using electronic recording of patient data and the other using paper records. Methods: In the first phase, all hospitalised patients aged 18 years and over were inspected for pressure ulcers. In the second phase, the files of patients with pressure ulcers were audited. Results: Of the 52 patients with ulcers found in the hospital that used an electronic system, 43 of their records documented the pressure ulcers (83%). Of the 55 patients with pressure ulcers in the hospital using paper records, 39 files had corresponding documentation of the presence of a pressure ulcer (71%). Conclusion: In terms of accuracy and completeness, more comprehensive documentation practice was found on the electronic health records compared with paper records. Aim: this article proposes a radical new approach to the monitoring and governance of services, and the education and training of nurses to meet ‘failure to rescue’ requirements. Background: healthcare policy in the UK that seeks to ensure safe and effective services for the acutely ill has largely failed, resulting in adult patients dying unnecessarily. Despite grand rhetoric, UK governments have distanced themselves from implementation, resulting in patchy localised developments and creating inconsistent service responses. Design and data sources: this article draws on a review of research and UK policy literature and best international practice to propose a new national framework approach that combines competency development, governance and performance monitoring to address ‘failure to rescue’ shortcomings. Implications for nursing: paramount is a nationally agreed and fit-for-purpose competency tool using simulation to assess staff in order to improve recognition of the deteriorating patient. Service improvements should include increased investment in telemedicine; service performance should be communicated through publicly available ratings and overseen by patient panels; and governance will require strengthening through enhanced Care Quality Commission/Monitor input, which should be linked to the maintenance of foundation trust status. Conclusion: health professional bodies, regulators, providers and the Government must work together to strengthen the safety and effectiveness of acute services. This literature review aimed to assess the reliability and validity of the quantified behavioural test (QbTest) to measure the symptoms of attention deficit hyperactivity disorder (ADHD), determining the extent to which results of the selected studies were accurate and generalisable. A literature review was undertaken, with each paper appraised using the Critical Appraisal Skills Programme randomised controlled trial checklist. Thematic analysis was also implemented to identify key themes and relationships between data sets. Three papers concluded that the QbTest increases clinical efficiency without compromising diagnostic accuracy, while others identified limitations relating to its ability to identify symptoms of impulsivity correctly. Additional issues were identified relating to external validity, generalisability and the extent to which the QbTest could differentiate ADHD from other conditions. It is concluded that the QbTest is an unreliable means of assessing ADHD in both children and adults, particularly when used as a stand-alone assessment tool. Thematic analysis highlighted concerns around diagnostic accuracy and the QbTest's inability to differentiate symptoms of ADHD from other conditions – fundamental flaws affecting the overarching fidelity of QbTesting.
