BACKGROUND: Limbal stem cell deficiency is a complicated pathology of ocular surface, which is not always helped by conservative methods of treatment and surgery is limited by available sources of tissue. Consequently, searching for new effective methods of its treatment is now gaining popularity. The most promising approach is transplantation of tissue-engineered constructs consisting of cultured limbal stem cells (LSCs) and variety of biopolymer carriers. AIM: This study was performed to obtain and characterize a tissue-engineered constructs consisting of cultured LSCs and collagen membrane. MATERIALS AND METODS: The study was performed at the Krasnov Research Institute at the Krasnov Research Institute of Eye Diseases and Koltzov Institute of Developmental Biology in cooperation with IMTEK Ltd. with a series of experiments. Two Chinchilla rabbits with an average weight of 3.5 kg and age of 6 months have been involved in this trial. LSCs were isolated and cultured in vitro from the healthy eye of rabbits using a method modified by the authors. The abtained cells were then cultured for 14 days and transplanted to the collagen membrane, which was then examined using immunohistochemical analysis. RESULTS: The cells isolated from the biopsy were a mixture of fibroblast-type cells and cells with characteristics of LSC. They maintained high survivability, proliferativity, phenotype and stemness on the collagen carrier according to immunofluorescent study. CONCLUSIONS: Thus, the abstained tissue-engineered constructs could be used for further transplantation to the affected eye with limbal stem cell deficiency under experimental conditions.

INTRODUCTION: Certain mineral elements dismetabolism is known to play an important part in the pathogenesis of the corneal degenerative and dystrophic diseases, as metalloenzymes participate in connective tissues metabolism affecting their properties. Thus, in keratoconus corneal tissue is depleted in iron, copper and zinc, what could be the underlying cause of cornea biomechanical properties impairment. Keratoconus modeling is complicated and practically not reproducible in animals, therefore cell models elaboration is very much in demand. Definite mineral elements content must be reduced in order to simulate pathological changes specific for keratoconic corneas. PURPOSE: The aim of this study was an elaboration of a cell model suitable for keratoconus pathogenesis research. This goal achieving involved solving the following tasks: 1) to develop a tissue-engineered system that mimic healthy corneal stroma; 2) to develop a technique for selective depletion of the nutrient medium by mineral elements involved in keratoconus pathogenesis; 3) to assess the possibility of the designed tissue-engineered system growth in the depleted nutrient medium. MATERIAL AND METHODS: The study was carried out with the primary culture of human keratocytes, which were used to build tissue-engineered systems of three types: on silicone, on a membrane, without a carrier. Cell cultures morphology was evaluated by light and electron microscopy. The nutrient media were zinc depleted via decationization of fetal bovine serum using two types of ion-exchange resins; mineral elements concentrations were evaluated by means of inductively coupled plasma mass spectrometry. RESULTS: The tissue system engineered without a carrier in form of cell sheet was chosen as the most convenient model. The decationization of the serum by means of Chelex 100 resin was shown to be a successful method for tenfold zinc concentration reduction in the nutrient medium. Keratocytes cultivation in the form of cell sheet on a zinc-depleted medium was successfully approved. CONCLUSION: Elaborated tissue-engineered system could be considered as a model of the corneal stroma under specific for keratoconus conditions of zinc depletion.

AIM:to evaluate the effectiveness of the platelet-rich plasma lysate (PRP lysate) use in the treatment of persistent epithelial defects (PED) after keratoplasty. MATERIALS AND METHODS:In the study, 60 patients with PED after keratoplasty were included. The 1st group (24 cases)included patients after keratoplasty with low risk of rejection, and the 2nd group 36 cases after keratoplasty with highrisk of rejection. Each group was divided into two subgroupscontrol subgroups 1a (cases 10) and 2a (cases 16), where patients received only standard postoperative therapy, and the main subgroups 1b (cases 14) and 2b (cases 20), in which PRP lysate was prescribed against the background of standard therapy, starting from the Day 15 post-op. As the criterion for effective treatment, complete persistent epithelialization after keratoplasty was considered. RESULTS:The effectiveness of the use of PRP lysate in the subgroup 1b was 85.7%, while complete epithelialization in the control subgroup 1a was recorded in 70%; in the subgroup 2b, complete epithelialization was observed in 55%, in the control subgroup 2ain 43.75%. CONCLUSION:The use of PRP lysate in the treatment of PED after corneal transplantation as an adjuvant therapy is effective and safe in both high and low risk keratoplasty. In the examined category of patients, treatment with blood derivatives increases the frequency and rate of complete epithelialization.

The problem of the restoration of the epithelial layer after various modifications of keratoplasty is of great fundamental interest. In this regard, new methods of induction of regeneration are being developed; one of the promising approaches in this area is the use of autologous blood derivatives with a high regenerative potential. Objective: to compare the effect of 3 blood derivatives serum, platelet-rich plasma and plasma rich growth factors on the culture of corneal epithelial cells. The study was carried out on cells of the epithelium of the human cornea of passage 3. To confirm corneal affiliation, cells were typed for characteristic cytokeratins. The dynamics of migration was assessed in the test for wound healing of the monolayer. Proliferation was assessed by the results of the formazan test. Plasma rich growth factors had the greatest stimulating effect on cell proliferation. There were no significant differences between groups in the rate of wound healing of the monolayer. It was found that, in comparison with the control, all stimulants shift the morphological phenotype of cells to a more mature side. As a result of the study, it was shown that all 3 types of tested blood derivatives are promoters of corneal re-epithelialization. The use of drugs obtained from blood can positively influence the processes of epithelialization in persistent epithelial corneal defects, which requires further study.

A new form of open-angle glaucoma has been identified, in which calcification and silicification of the trabecular meshwork is a potentially significant component of outflow obstruction. It is noted that the mineralization of this area is promoted by various disturbances in the acid-base balance in the tissue. The role of melanosomal enzymes in the initiation of the formation of mineral calcium phosphate in trabecular tissue in open-angle glaucoma is considered.

Mesenchymal stromal cells (MSCs) have shown a high potential for cartilage repair. Collagen-based scaffolds are used to deliver and retain cells at the site of cartilage damage. The aim of the work was a comparative analysis of the capacity of the MSCs from human adipose tissue to differentiate into chondrocytes in vitro and to stimulate the regeneration of articular cartilage in an experimental model of rabbit knee osteoarthrosis when cultured on microheterogenic collagen-based hydrogel (МCH) and the microparticles of decellularized porcine articular cartilage (DPC). The morphology of samples was evaluated using scanning electron microscopy and histological staining methods. On the surface of the DPC, the cells were distributed more uniformly than on the MCH surface. On day 28, the cells cultured on the DPC produced glycosaminoglycans more intensely compared to the MCH with the synthesis of collagen type II. However, in the experimental model of osteoarthrosis, the stimulation of the cartilage regeneration was more effective when the MSCs were administered to the MCH carrier. The present study demonstrates the way to regulate the action of the MSCs in the area of cartilage regeneration: the MCH is more conducive to stimulating cartilage repair by the MSCs, while the DPC is an inducer for a formation of a cartilage-like tissue by the MSCs in vitro.

Severe lesions of corneal endothelium usually lead to progressing corneal oedema, sharp decrease in visual acuity and as a result to the need for transplanting donor cornea that can provoke relevant sequelae, thus an active search for alternative impacts to the affected cell layer is underway allowing for keeping out of operative intervention. The purpose of this research is to evaluate the clinical efficacy of a method of personalized cell therapy (aqueous chamber administration of suspension consists of activated autologous leukocytes in serum) aimed to treatment of early postoperative bullous keratopathy and severe form of herpetic keratoiridocyclitis. The results of clinical application of personalized cell therapy in 210 patients with postoperative bullous keratopathy (60) and herpetic keratoiridocyclitis (150) - divided into 2 subgroups: 75 patients received personalized cell therapy (experimental subgroup), the other 75 patients were injected intracamerally with a solution of poludan preparation (control subgroup), are presented in the article. The pronounced therapeutic effect in the treatment of early postoperative bullous keratopathy was achieved in 44 % of cases, with an increase in visual acuity of 0,49±0,27. Straightening of the descemet membrane's folds, significant dehydration of corneal stroma and complete restoration of corneal transparency, a reduction in the thickness of the cornea from 801±112 pm to 582±55 pm have been achieved. In the group of herpetic keratoiridocyclitis treatment, recovery was achieved in 85 % - complete resorption of corneal infiltration and edema and restoration of corneal transparency. The increase in visual acuity in this group was 0,6±0,3. The normalization of the corneal structure after personalized cell-based therapy is also evidenced by in vivo confocal microscopy data. The correlation of the clinical effect with the concentration in the cellular preparation of TNF-α, IL-6 and IL-1 was established. Personalized cell therapy is effective method of curative action on damaged corneal endothelium that does not require keratoplasty.

Background: One of the main reasons of failure in surgical treatment of primary acquired nasolacrimal duct obstruction is excessive postoperative scarring of the dacryostomy. Despite the variety of procedures designed to prevent this, conflicting evidence of their efficacy and safety provide incentive for further research of antifibrotic therapeutics for adjunctive use in dacryocystorhinostomy. Aims: To evaluate the antifibrotic effect of pirfenidone on human nasal mucosal fibroblast cell culture. Materials and methods: Human nasal mucosal fibroblast cell cultures were established using samples obtained from 3 consecutive patients undergoing endonasal endoscopic dacryocystorhinostomy. Cell viability following treatment with pirfenidone was evaluated using MTS-assay. Induced inhibition of cell proliferation and migration was determined using scratch wound assay. Results: In this study pirfenidone exhibited a significant dose-dependent inhibiting effect on fibroblast proliferation with insignificant cell toxicity. Cell viability following 48 hours of incubation with various pirfenidone concentrations did not drop below 80%. The recovery of the fibroblast monolayer assessed after 24 hours of incubation was 84.88 and 8.26% in the control group, at a drug concentration of 0.15 mg/ml. Cell proliferation and migration was severely inhibited in cell culture specimens treated with pirfenidone compared to controls. The difference between groups was statistically significant (p=0,001). Conclusions : In our study pirfenidone demonstrated a pronounced antifibrotic effect. It is unlikely that inhibition of proliferation and migration of human nasal mucosal fibroblasts is mediated by cell toxicity of this medication as it was evaluated as low. Nonetheless an in vitro analysis is insufficient to judge pirfenidone’s efficacy and safety in preventing cicatrix formation following dacrycystorhinostomy.

Corneal transplantation is the most successful and frequently performed allotransplantation procedure. Benign outcome of penetrating and posterior keratoplasty depends on viability of the corneal tissue and presence of the proper endothelial layer, which retaining is one of the main objectives of preservation of donor material. Methods designed to reach this goal may be classified according to storage duration as short-term (storage in a moist chamber), medium-term (hypothermia), long-term (organ cultivation) and unlimited (cryopreservation). While there are reports of successful application of cryo-conserved corneas for penetrating keratoplasty, its use for this type of transplantation is limited due to complexity of the method and very high requirements for the quality of donor material. However, being a method of unlimited storage, it still attracts the attention of researchers. Taking into account the ongoing development of innovative keratoplasty technologies and the tendency for predominant use of layered grafts that in some cases do not need viable endothelium, methods of preservation of such material for anterior lamellar and intralamellar keratoplasties become relevant. In this context, cryopreservation without the use of cryoprotective agents is promising because it allows simple and fast accumulation of large amount of material with long storage time. Further research on the development of preservation methods of the corneal endothelial layer at sub-zero temperatures may allow storing the material by cryopreservation that will be suitable for penetrating and posterior keratoplasty.Пересадка роговицы является наиболее успешной и часто выполняемой процедурой аллотрансплантации. Благоприятный исход сквозных и задних послойных кератопластик зависит от жизнеспособности ткани роговицы и наличия полноценного эндотелиального слоя, поэтому именно его сохранение является одной из основных целей консервации донорского материала. Используемые для этого методы классифицируются в зависимости от продолжительности хранения как краткосрочные, среднесрочные, долгосрочные и с неограниченным сроком. Это, соответственно, хранение во 'влажной камере', гипотермический метод, метод органного культивирования и криоконсервация. Несмотря на то что есть сообщения об успешных результатах применения криоконсервированной роговицы для сквозной кератопластики, сложность метода и очень высокие требования к качеству донорского материала ограничивают ее использование для этого типа трансплантации. Тем не менее криоконсервация как метод хранения с неограниченным сроком до сих пор привлекает внимание исследователей. Учитывая развитие инновационных технологий трансплантации роговицы, тенденцию к преимущественному использованию послойных кератопластик, для ряда из которых необязательно наличие жизнеспособного эндотелия, в современной хирургии приобретают актуальность и методы сохранения материала, подходящего для этого типа оперативных вмешательств. В данном аспекте перспективным является способ криоконсервации без применения криопротекторов, позволяющий относительно просто и быстро создать большой пул материала с длительным сроком хранения. Дальнейшие исследования по разработке методов сохранения эндотелиального слоя роговицы при минусовых температурах, возможно, в будущем позволят сохранять методом криоконсервации материал, пригодный для сквозных и эндотелиальных кератопластик.

To evaluate cytokine levels in the aqueous humor (AH) of patients with Fuchs endothelial corneal dystrophy (FECD) and bullous keratopathy (BK).The study included 74 patients (74 eyes). The first group consisted of 31 patients (72.7±9.2 years) with FECD; the second group included 35 patients (72.4±9.1 years) with BK. The control group comprised 8 patients (74.3±4.1 years) with immature cataract. Before surgery, patients underwent pachymetry of the central cornea (RTvue-100 OCT, Optovue, USA). Patients of groups 1 and 2 underwent endothelial keratoplasty (DSAEK or DMEK), or penetrating corneal transplantation. Patients of the control group underwent phacoemulsification with implantation of intraocular lens. The initial stage of the surgery involved AH sample collection for evaluation of cytokine levels (IL-1β, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12(p70), IL-13, IL-17, G-CSF, GM-CSF, IFNγ, MCP-1, MIP-1β and TNF-α) by fluorescent flow cytometry using the Bio-Plex Pro Human Cytokine Panel, 17-plex (Bio-Rad, USA).Multiplex analysis of the AH content did not show any statistically significant differences in cytokine levels between decompensated FECD and BK eyes. The levels of IL-6, IL-8, GM-CSF, IFNγ, MCP-1, MIP-1β were significantly elevated in FECD and BK eyes compared with healthy control. An insignificant deviation of IL-4 and IL-13 levels was detected in FECD and BK eyes compared with healthy controls. There were no significant differences in IL-1β and TNF-α (indicators of acute inflammation) between the study groups.The obtained data confirm that FECD and BK are associated with disruption of ocular immune privilege that leads to chronic local inflammation, which in turn causes remodeling of the corneal tissues resulting in fibrosis.Изучить содержание эндогенных биологически активных веществ во внутриглазной жидкости (ВГЖ) у пациентов с эндотелиальной дистрофией роговицы Фукса (ДФ) и буллезной кератопатией (БК).В исследование включены 74 пациента (74 глаза). 1-ю группу составили 31 пациент (72,7±9,2 года) с ДФ. 2-я группа представлена 35 больными (72,4±9,1 года) с БК. В контрольную группу включены 8 больных (74,3±4,1 года) с незрелой катарактой. До операции пациентам выполняли пахиметрию центральной зоны роговицы (RTvue-100 OCT, Optovue, США). Пациентам 1-й и 2-й группы проводили эндотелиальную кератопластику (DSAEK или DMEK), или сквозную пересадку роговицы. Пациентам контрольной группы выполняли факоэмульсификацию с имплантацией интраокулярной линзы. На начальном этапе операции были взяты образцы ВГЖ для определения концентрации цитокинов (IL-1β, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12(p70), IL-13, IL-17, G-CSF, GM-CSF, IFNγ, MCP-1, MIP-1β и TNF-α) методом флуоресцентной проточной цитометрии с использованием Bio-Plex Pro Human Cytokine Panel, 17-plex (Bio-Rad, США).Мультиплексный анализ состава ВГЖ не выявил статистически значимых различий концентраций цитокинов в ВГЖ у пациентов с ДФ в фазе декомпенсации и БК. Обнаружено достоверное повышение уровня IL-6, IL-8, GM-CSF, IFNγ, MCP-1, MIP-1β в ВГЖ у пациентов 1-й и 2-й групп по сравнению с показателями контрольной группы. Отмечено незначительное изменение содержания IL-4 и IL-13 у пациентов с ДФ и БК по сравнению с контролем. Сравнение концентрации показателей острой воспалительной реакции (IL-1β и TNF-α) в ВГЖ не выявило статистически значимых различий между группами.Полученные данные подтверждают, что в основе ДФ и БК лежит нарушение механизма «иммунной привилегии» глаза, приводящее к развитию хронической локальной воспалительной реакции, обусловливающей в том числе ремоделирование роговичной ткани с исходом в фиброз.

The article reviews modern methods of treatment of persistent corneal epithelial defects and considers the factors involved in the development of this pathology, including the limbal stem cell deficiency, which is likely to play the main role. The most promising treatment methods are described, particularly the use of blood derivatives and cell therapy.Данный обзор посвящен современным методам терапии персистирующих эпителиальных дефектов роговицы. Рассмотрены основные факторы, влияющие на развитие этой патологии, обсуждается влияние лимбальной недостаточности как одного из наиболее вероятных факторов развития. Описаны наиболее перспективные методы лечения, а именно применение различных дериватов крови и клеточной терапии.

To analyze the chemical composition of the sclera and trabecular meshwork in patients with primary open-angle glaucoma (POAG) and to assess the impact of corneal-compensated intraocular pressure (IOP) on the chemistry of the drainage area.Biopsy specimens of the trabecular meshwork (89 specimens) and sclera (41 specimens) obtained from patients with POAG on maximal medical therapy were analyzed to determine the content of certain chemical elements - carbon (C), nitrogen (N), oxygen (O), aluminum (Al), calcium (Ca), chlorine (Cl), potassium (K), magnesium (Mg), sodium (Na), phosphorus (P), silicon (Si) and sulfur (S). The elements were selected based on chemical structure target tissue and sensitivity of the method used for analysis. Visualization was performed using theThe IOP of patients with POAG was found to have positive correlation with the total amount of S, P, and Ca in the trabecular meshwork. The accumulation of sulfur-containing compounds associated with pigment granules in trabecular meshwork's tissue was identified. A correlation was determined between the organic and inorganic (mineral) phosphorus content in the trabeculae, and the IOP values. The organic component of phosphorus was better represented than the mineral component in patients with increased IOP.In patients with POAG, an increase in the IOP level causes the amount of S associated with pigment granules and the proportion of organic P to increase in the trabecular meshwork, which should be taken into account in the further search for drug therapy that would potentially affect pathologically altered tissue.Изучить химический состав склеры и трабекулярной ткани глаза при первичной открытоугольной глаукоме (ПОУГ). Оценить взаимосвязь роговично-компенсированного внутриглазного давления (ВГД) и химического состава дренажной зоны.Было определено содержание ряда химических элементов: углерода (C), азота (N), кислорода (O), алюминия (Al), кальция (Ca), хлора (Cl), калия (K), магния (Mg), натрия (Na), фосфора (P), кремния (Si), серы (S) в 89 биоптатах трабекулярной ткани и в 41 биоптате склеры у пациентов с ПОУГ, использующих максимальную местную гипотензивную терапию. Выбор анализируемых элементов был обусловлен химическим строением анализируемой ткани и чувствительностью инструмента, используемого для анализа. Визуализацию проводили с помощью сканирующего электронного микроскопа EVO LS 10 (Zeiss, Германия), исследование химического состава — с использованием энергодисперсионного спектрометра Oxford-X-MAX-50 (Oxford, Великобритания). В качестве статистических критериев оценки использовали тест Спирмена.Обнаружены прямые корреляции между уровнем ВГД у пациентов с ПОУГ и общим содержанием S, P, Ca в трабекуле. Показана аккумуляция ассоциированных с пигментными гранулами серосодержащих соединений в трабекулярной ткани при ПОУГ. Прослежена связь между органической и неорганической (минеральной) формами нахождения P в трабекуле при ПОУГ в зависимости от уровня офтальмотонуса. Для P установлено преобладание органического компонента над минеральным при увеличении уровня ВГД у пациентов с открытоугольной глаукомой.Заключение. У пациентов с ПОУГ при повышении уровня ВГД в ткани трабекулы увеличивается количество S, ассоциированной с пигментными гранулами, и возрастает доля органического P, что следует учитывать при дальнейшем поиске медикаментозной терапии, потенциально воздействующей на патологически измененную ткань.

The study is based on the hypothesis that high taxonomic diversity of bacteria detectable on the eye surface by molecular genetic methods is attributed to the high level of its contamination by skin microflora. Such contamination would make it problematic to identify the fractions of real ocular surface microbiome, which remains behind the one-percent cut-off threshold adopted in the metagenomic analysis. Hard contact lenses for long-wearing act as a physical filter preventing DNA contamination from random microorganisms, and at the same time providing adhesion to the living cells of bacteria and fungi. To confirm this assumption, a detailed analysis of references was carried out, supplemented by original laboratory research.The analysis included 16 hard contact lenses obtained from 11 patients with impaired refraction (myopia). Это затрудняет выявление фракций реального микробиома глазной поверхности, которые остаются за принятым в метагеномном анализе однопроцентным порогом отсечения. В роли физического фильтра для предотвращения захвата остатков ДНК случайных микроорганизмов, одновременно обеспечивающего адгезию живых клеток бактерий и грибов, могут выступать контактные линзы длительного ношения. Для подтверждения этого предположения был проведен детальный анализ литературы, дополненный собственными лабораторными исследованиями.Всего в анализ вошло 16 жестких контактных линз, полученных от 11 пациентов с миопией. Также у 42 пациентов были получены соскобы слизистой оболочки конъюнктивы. Corneal transplantation is the most common and successful type of allotransplantation surgery. Post-transplant immune response in keratoplasty is less pronounced than that in other transplantation procedures, which is accounted for by anatomical features of the cornea and, also, its low antigenic potential and active immunosuppression. However, the immune privilege of the cornea can be violated by neovascularization, inflammation, or trauma. Patients who require keratoplasty to restore their sight and whose immune privilege is disturbed, fall into a high-risk group and are likely to demonstrate tissue incompatibility and non-transparent engraftment. Two approaches exist as to how graft rejection can be prevented. One of them involves induction of donor-specific tolerance, the other - non-specific suppression of the recipient's immune response. To avoid tissue incompatibility, measures can be taken to restore the immune privilege of the cornea as well as to induce antigen-specific tolerance, which is considered a promising, thought yet experimental, area of modern transplantology. In clinical practice, one pays most attention to improvement of non-specific immune suppression methods based on interfering in the metabolism of immunocompetent cells. Thus, timely prescriptions and proper immunosuppressive tactics with account to possible risk factors determine the outcome in high-risk patients undergoing corneal transplantation surgery.Пересадка роговицы является наиболее успешной и часто выполняемой процедурой аллотрансплантации. Пациенты с нарушенной иммунной привилегией, которым для восстановления зрения необходима кератопластика, относятся к группе 'высокого риска' с большой вероятностью развития реакции тканевой несовместимости и непрозрачного приживления трансплантата. Существуют два принципиально отличающихся подхода к подавлению реакции отторжения трансплантата - создание специфической толерантности реципиента к трансплантату и неспецифическое подавление иммунного ответа организма на чужеродные антигены. Профилактика реакции тканевой несовместимости путем восстановления иммунной привилегии роговицы и создания антигенспецифической толерантности - перспективное направление современной трансплантологии. Однако в настоящее время подобные исследования не вышли за рамки эксперимента, а в клинической практике на первый план выходит совершенствование методов неспецифической иммуносупрессивной терапии, основой которой является подавление метаболизма иммунокомпетентных клеток. Таким образом, своевременное назначение и правильный выбор тактики иммуносупрессии с учетом имеющихся факторов риска имеют решающее значение в исходе трансплантации роговицы в группе больных 'высокого риска'.

Fibrosis is the most important pathologic condition involved in undesirable outcomes of dacryocystorhinostomy. A number of biochemical factors are currently known to have an effect on wound healing by promoting excessive scarring. Isoforms of transforming growth factor β (TGF-β1) are considered the 'main' pro-fibrotic factor, but wound healing is also affected by other cytokines such as connective tissue growth factor (CTGF), which stimulates fibrosis, and fibroblast growth factor (FGF-2), which acts as antagonist to it.To investigate correlations between endoscopic endonasal dacryocystorhinostomy outcomes and certain mediators of fibrosis.The study included 45 cases of endoscopic endonasal dacryocystorhinostomy. The patients were grouped according to surgery outcome: patients with unsuccessful surgical treatment were assigned to group 1 (n=10); patients with successful surgical treatment - to group 2 (n=34). One patient was excluded from the study. Full-layer biopsy specimen were taken from patients' nasal mucosa before the surgery. TGF-β1, TGF-β2, TGF-β3, CTGF, FGF-2 concentrations were evaluated using ELISA and normalized by total protein concentration.Surgical failure was observed in 10 cases (22.72%). CTGF concentration was significantly correlated with negative outcome (p0.05) and was elevated in most specimen obtained from group 1. No significant correlation was noted between the concentrations of other evaluated cytokines in nasal mucosa specimens and the surgical outcome.The study found a correlation between CTGF concentration in nasal mucosa and dacryocystorhinostomy outcome, which supports the hypothesis suggested by several authors linking dacryocystorhinostomy failure with chronic inflammation in nasal mucosa.Введение. Фиброз является наиболее важным патологическим состоянием, приводящим к неудачному результату оперативного вмешательства при дакриоцисториностомии. В настоящее время известно множество биохимических факторов, оказывающих влияние на течение процесса заживления, активность которых может являться причиной избыточного рубцевания. 'Центральными' медиаторами фиброза считаются изоформы трансформирующего фактора роста (TGF-β), но в процессе заживления ран также участвуют и другие цитокины, такие как фактор роста соединительной ткани (CTGF), обладающий профибротическим действием, и фактор роста фибробластов (FGF-2), обладающий антифибротическим действием. Цель исследования - изучить корреляцию результата эндоназальной эндоскопической дакриоцисториностомии с концентрацией некоторых медиаторов фиброза. Материал и методы. В исследование было включено 45 случаев эндоназальной эндоскопической дакриоцисториностомии. В соответствии с результатом оперативного вмешательства пациентов разделили на две группы: в 1-ю группу (n=10) вошли пациенты с неблагоприятным результатом операции; во 2-группу (n=34) - пациенты с благоприятным результатом. Из исследования исключен 1 пациент. До начала оперативного вмешательства осуществляли забор полнослойных биоптатов слизистой оболочки полости носа. Исследование концентрации цитокинов TGF-β1, TGF-β2, TGF-β3, CTGF, FGF-2 выполняли методом твердофазного иммуноферментного анализа (ELISA) с последующей нормировкой показателей по содержанию общего белка. Результаты. Неблагоприятный результат оперативного вмешательства наблюдали в 10 (22,72%) случаях. Концентрация цитокина CTGF повышена у большинства пациентов 1-й группы и статистически значимо коррелировала с неблагоприятным результатом хирургического вмешательства (p0,05). Корреляции между результатом оперативного вмешательства и содержанием других исследованных цитокинов в образцах слизистой оболочки полости носа не обнаружено. Заключение. В настоящем исследовании выявлена корреляция между концентрацией цитокина CTGF в слизистой оболочке полости носа и результатом эндоназальной эндоскопической дакриоцисториностомии; это поддерживает выдвинутую рядом авторов гипотезу, что хроническое воспаление в полости носа обусловливает неблагоприятный результат оперативного вмешательства.

Mesenchymal stromal cells from deciduous teeth in 2D- and 3D-cultures on culture plastic, silicate glass, porous polystyrene, and experimental polylactoglycolide matrices were visualized by scanning electron microscopy with lanthanide contrasting. Supravital staining of cell cultures with a lanthanide-based dye (neodymium chloride) preserved normal cell morphology and allowed assessment of the matrix properties of the carriers. The developed approach can be used for the development of biomaterials for tissue engineering.

Considering the demand for investigating spatial cell arrangement within a scaffold, yet unsatisfied, we aimed at developing a new method of sample preparation for SEM that would, besides other things, allow visualization of cellular ultrastructure and deliver from toxic chemicals and artifacts. A combination of BSE imaging in a low-vacuum mode with supravital lanthanoid staining has yielded highly informative images that contain many of the subsurface cell structures, particularly, intercellular contacts, cell membranes, nuclei with nucleoli, mitochondria, endoplasmic reticulum, and cytoskeleton. Moreover, the developed approach provides an idea of three-dimensional arrangement of cellular elements with a greater depth of focus than that of optical methods, which is drastically important for fulfilling the needs of tissue engineering. Being fast and simple, the method is able to change the whole concept of using SEM in biomedical studies.

To study the effectiveness of polishing Bowman's membrane with a diamond burr (DBPBM) in the treatment of recurrent corneal erosion syndrome (RCES).The study included 22 patients (22 eyes) with RCES aged 20-56 years who underwent our original modification of DBPBM. All patients had keratotopography examined, endothelial cell density measured, and optical coherence tomography (OCT) of the cornea taken before and after the procedure. Additionally, conjunctival scraping, blood test for types I and II herpes (fluorescent antibody method) were performed before the surgery. Post-operative treatment included 1-2 weeks of anti-inflammatory and anti-bacterial steroid therapy, and 6 months of artificial tear Cationorm usage.Rehabilitation of all patients after the procedure went quickly, and no relapses were noted during the follow-up. Complete epithelialization of the area of surgical intervention under soft contact lens occurred over 2-3 days. Epithelial thickness was measured with OCT including epithelial mapping; after the surgery, it was almost even over the entire corneal surface. No complications such as induced post-operative refractive error or corneal haze were registered during the course of the study. Disruption of basal epithelial layer and epithelial basal membrane in the area of erosion relapse detected with scanning electron microscopy confirmed the need for their removal in the course of the surgery.Conservative treatment of RCES rarely yields results. Removal of inadequate epithelium and following DBPBM using our original method is a highly effective and accessible procedure for RCES treatment with low risk of complications and relapse.Цель - изучить эффективность абразивной шлифовки боуменовой мембраны алмазным бором (АШБМАБ) в лечении синдрома рецидивирующей эрозии роговицы (РЭР). Материал и методы. В исследовании принимали участие 22 пациента (22 глаза) с РЭР в возрасте 20-56 лет, которым была проведена АШБМАБ в нашей модификации. Всем пациентам до и после оперативного вмешательства проводили кератотопографию, оптическую когерентную томографию (ОКТ) роговицы, определение плотности эндотелиальных клеток. Кроме того, исследовали соскоб с конъюнктивы и кровь на вирус простого герпеса 1-го и 2-го типов (МФА) до операции. Послеоперационное лечение включало применение стероидных противовоспалительных и антибактериальных средств в течение 1-2 нед, препарата искусственной слезы катионорм в течение 6 мес. Результаты. У всех пациентов отмечались быстрая реабилитация после процедуры и отсутствие рецидивов в период послеоперационного наблюдения. Полная эпителизация зоны вмешательства под мягкой контактной линзой наступала в течение 2-3 дней. Толщину эпителия оценивали при помощи ОКТ с составлением эпителиальной карты, после вмешательства она была практически одинаковой на всей поверхности роговицы. Такие осложнения, как индуцированные послеоперационные нарушения рефракции и появление хейза роговицы, не были выявлены в ходе исследования. Неполноценность базального слоя эпителия и эпителиальной базальной мембраны в области рецидивов эрозии, выявленная при сканирующей электронной микроскопии, подтверждала необходимость их тщательного удаления в ходе хирургического вмешательства. Заключение. Консервативное лечение РЭР нередко оказывается безуспешным. Удаление неполноценного эпителия и последующее проведение АШБМАБ в нашей модификации - высокоэффективная и доступная процедура в лечении синдрома РЭР, имеющая низкие риски возникновения осложнений и рецидивов.

Scarring is one of the main causes of surgical failure in a number of eye diseases, dacryologic conditions in particular. The process of wound healing, including postoperative wound healing, goes through several stages mediated by various biochemical factors, such as growth factors and pro- and anti-inflammatory cytokines. The balance between the latter directly influences the wound healing. However, current data on the effect of these factors on postoperative outcomes are few and contradictory. Thus, in dacryology as well as in other areas of ophthalmology, the role of cytokines and growth factors in healing of surgical wounds is being intensively researched.Одной из основных причин неудачного исхода хирургического лечения ряда заболеваний в офтальмологии, в частности в дакриологии, является рубцевание. Процесс заживления ран, в том числе и послеоперационных, происходит в несколько этапов, находящихся под контролем биохимических факторов, таких как факторы роста, про- и противовоспалительные цитокины. Баланс этих медиаторов оказывает непосредственное влияние на исход раневого процесса. Тем не менее имеющиеся данные о влиянии этих факторов на исход оперативного вмешательства немногочисленны и противоречивы. В настоящее время большое внимание уделяется изучению роли цитокинов и факторов роста в заживлении ран после оперативных вмешательств как в дакриологии, так и других областях офтальмологии.

Despite of common opinion that mitotic processes in adult human corneal endothelium are absent the accumulated data show potential of these cells for proliferation. Mechanisms inhibiting endothelial corneal cells mitosis are illustrated. The data on possible ways of proliferation activation and cell division in vitro and in vivo is provided. A brief review of cell technologies applied in contemporary ophthalmology is given including method of personalized cell therapy, a way of effective medical impact on damaged corneal endothelium in early postoperative bullous keratopathy.

Transplantation of donated cornea is a radical and pathogenetically oriented measure of rehabilitation for patients with corneal blindness. Unlike other transplantation surgeries, keratoplasty is usually done without tissue typing and systemic immunosuppression. Even so, in the absence of risk factors, grafts can remain clear in as many as 90% of cases. The phenomenon is explained by immune privilege of the cornea. The hypothesis of immune privilege comprises several interrelated mechanisms that ensure long-lasting transparency of the graft. These include transfer limits of immunogenic stimuli to peripheral lymphoid tissues, deviation of the immune response, and neutralization of immune effector elements within the host-graft interface by immunosuppressive ocular environment. A change in any of the said components leads to loss of the immune-privilege status of the cornea, thus, significantly increasing the risk of graft rejection. We know several stages of the response activation cascade in graft rejection that can be influenced therapeutically, namely, antigen absorption, processing, and presentation as well as T-cell expansion and cellular inflow to the eye. The first two stages take place in the eye itself and are susceptible to instillation drugs and gene therapy. New highly selective medications aimed at specific signals from the immune cells and their pathways may be able to help restore the immune privilege of the cornea and improve the results of optical and reconstructive surgeries in high-risk patients.Пересадка донорской роговицы является радикальным патогенетически ориентированным способом реабилитации пациентов с роговичной слепотой. В отличие от других форм трансплантации, кератопластика, как правило, выполняется без применения тканевого типирования и системной иммуносупрессии. При этом частота прозрачного приживления роговичного трансплантата в отсутствие факторов риска может достигать 90%. Этот феномен объясняется особым иммунопривилегированным состоянием роговицы. Гипотеза иммунной привилегии роговицы объединяет несколько взаимозависимых механизмов, которые способствуют сохранению прозрачности трансплантата на протяжении длительного периода. Эти механизмы включают ограничение передачи иммуногенных стимулов периферическим лимфоидным тканям, наличие процессов девиации иммунного ответа и нейтрализацию иммунных эффекторных элементов в интерфейсе хозяин - трансплантат за счет иммуносупрессивного микроокружения глаза. Изменение статуса любого из трех компонентов приводит к потере иммунопривилегированного статуса роговицы, что существенно повышает риск возникновения реакции отторжения кератотрансплантата. Известно несколько этапов каскадной активации иммунных реакций при отторжении роговичного трансплантата, которые могут поддаваться терапевтическому воздействию. Эти этапы включают поглощение антигена, обработку антигена, презентацию антигена, T-клеточную экспансию, приток эффекторных клеток в глаз. Два первых этапа проходят в глазу и подвержены влиянию инстилляционных препаратов или генной терапии. Новые высокоселективные препараты, воздействующие на иммунные клеточные сигналы и пути их передачи, могут способствовать восстановлению иммунопривилегированности роговицы и повышению результатов оптико-реконструктивных операций у пациентов в группе 'высокого риска'.

Cumulative biomicroscopic evidence is usually sufficient for the diagnosis of recurrent corneal erosion or bullous keratopathy, however, exploration of the disease pathogenesis requires subcellular-level visualization of corneal structure. In the current study, lanthanoid staining and scanning electron microscopy were employed to visualize quite a number of structures responsible for epithelium organization. In particular, the study proves possible the use of Ca/Nd isomorphous substitution at Ca2+ sites of cytoadherence proteins for visualization of corresponding cellular structures.To assess the value of information provided by scanning electron microscopy of corneal epithelium that involves lanthanoid staining based on the Ca/Nd isomorphous substitution in Ca-dependent molecular systems.Anterior corneal epithelial scrapes were obtained from patients with recurrent corneal erosion or bullous keratopathy and cadaver eyes with no signs of any ophthalmic disease. Samples were then studied under a scanning electron microscope (Zeiss EVO LS10, BSE, EP - 79 Pa, 20-28 kV, Ln-staining with the BioREE assay kit).In all cases, lanthanoid staining of biopsy material provided high-contrast SEM images with well-recognizable structural and ultrastructural elements associated with Ca2+ sites of cytoadherence proteins.Lanthanoid staining of biopsy material and subsequent SEM enabled detailed visualization of structural features of the corneal epithelium in various pathologies. Due to the Ca/Nd isomorphism we were able to evaluate structural position of the majority of protein molecules engaged in Ca-dependant processes and, consequently, in cytoadherence. Basing on the neodymium distribution within the basal membrane, we have described local effects of different substances on the lamina densa in the projection of basal layer cell borders that occur after unidirectional ultrafiltration. The results confirm the failure of the junctional adhesion complex in recurrent corneal erosion.Совокупность признаков, выделяемых биомикроскопически, как правило, позволяет диагностировать рецидивирующую эрозию роговицы и буллезную кератопатию, однако для раскрытия патогенеза заболевания необходима визуализация структуры на субклеточном уровне. В работе предпринимается попытка одновременной визуализации различных структур, отвечающих за организацию эпителиальной ткани, с использованием лантаноидного контрастирования и последующим проведением сканирующей электронной микроскопии (СЭМ). В частности, доказывается применимость Ca/Nd изоморфного замещения Ca2+-сайтов в белках клеточной адгезии для визуализации соответствующих им структур. Цель - оценка информативности изображений, полученных посредством визуализации структуры эпителия роговицы методом СЭМ с лантаноидным контрастированием на основе Ca/Nd изоморфного замещения в Сa-зависимых молекулярных системах. Материал и методы. Соскобы переднего эпителия роговицы, полученные у больных, страдающих рецидивирующей эрозией роговицы или буллезной кератопатией, а также с кадаверных глаз без признаков офтальмологических заболеваний изучали на сканирующем электронном микроскопе (Zeiss EVO LS10, BSE, EP - 70 Па, 20-28 кВ, лантаноидное контрастирование набором реактивов BioREE). The results of vital confocal microscopy (VCM) of 5 patients with early postoperative bullous keratopathy after cataract phakoemulsification (PE) treated with author's technique of personalized cell therapy (PCT, intracameral injection of activated patient's leukocytes in autologous serum) are presented. In 3 of all presented cases endothelial loss after PE was more than 50%. Before PCT using of VCM was not possible due to corneal edema. After 2-3 PCT sessions corneal clarity recovery was observed and signs of regenerative processes in cornea were captured by VCM: activation of keratocytes (hyperreflective nuclei) and endothelial cells (multinucleated cells), descemet membrane folds smoothening, normalization of epithelium. Results of VCM correlate with biomicroscopic signs of corneal edema resolution, corneal clarity recovery, improvement of visual acuity after PCT.
