Introdução: O transplante de córnea é o transplante mais frequentemente realizado mundialmente. Na última década, a queratoplastia penetrante (QP) tem sido ultrapassada pelos procedimentos lamelares (QL) em vários países desenvolvidos, dado serem menos invasivos, com menor risco de rejeição e melhores resultados funcionais. Com este trabalho, pretendeu-se concretizar a casuística deste tipo cirurgia realizada no nosso centro, avaliar a tendência das diferentes técnicas desde 2011 até 2021 e secundariamente avaliar o impacto da pandemia COVID-19 no perfil de transplantação de córnea. Material e Métodos: Análise retrospetiva de todos os doentes submetidos a transplante de córnea no Centro Hospitalar Universitário de São João (CHUSJ) entre Janeiro de 2011 e Dezembro de 2021. Foram colhidas e analisadas variáveis demográficas, indicação cirúrgica e tipo de transplante realizado. Resultados: Foram incluídos 1392 olhos. Em 2011, 99,3% dos transplantes realizados foram QP. A percentagem de QL foi aumentando progressivamente, especialmente depois de 2015, sobretudo à custa dos lamelares posteriores no contexto de doença endotelial. Em 2011, todos os casos de patologia endotelial foram tratados com QP. Em 2016, a QL ultrapassou a QP no tratamento dessa condição, sendo que 37,8% dos procedimentos realizados foram Descemet stripping automated endothelial keratoplasty (DSAEK) e 21,6% foram Descemet membrane endothelial keratoplasty (DMEK). Em 2021, 52,6% das patologias endoteliais foram tratadas com DSAEK e 26,3% com DMEK. O ano de 2020 limitou o número total de transplantes de córnea realizados (tanto QP e QL) para o valor mínimo da última década (n total de 85), considerando os constrangimentos da pandemia COVID-19. Contudo, em 2021, observou-se um aumento de 58% do número de transplantes face ao ano anterior. Conclusão: Na última década, tem-se verificado uma evolução do número de QL no CHUSJ em detrimento da QP, tendo esta sido ultrapassada a partir de 2016 no que concerne ao tratamento das patologias endoteliais com as técnicas lamelares posteriores. INTRODUCTION: Our objective was to evaluate the 5-year clinical outcomes of a case series after Descemet membrane endothelial keratoplasty (DMEK) in a tertiary hospital center. METHODS: Retrospective, single-center, observational cohort revision of the DMEK surgeries performed between August 2016 and August 2022. Main outcome parameters (survival graft, best-corrected visual acuity (BCVA) in logMAR scale, and central endothelial cell density (ECD) were recorded. Intra, postoperative complica- tions, and the need for subsequent keratoplasties were considered secondary outcomes. RESULTS: A total of 69 DMEKs, in 56 patients, were performed in our center between Au- gust 2016 and August 2022. The mean age of the patients at surgery was 65.91 ± 11.82 years. A percentage of 62.5% were female while 37.5% male. The leading indication for surgery was Fuchs dystrophy, followed by pseudophakic bullous keratopathy, corneal decompensation from previ- ous keratoplasties, and bullous keratopathy associated with phakic lens. Mean BCVA improved from 0.75 ± 0.26 (LogMAR) to 0.24 ± 0.23 at 1 year follow-up (N=57, p, INTRODUÇÃO: OO nosso objetivo foi valiar os resultados clínicos de uma série de casos de Descemet membrane endothelial keratoplasty (DMEK) realizados num centro hospitalar terciário durante 5 anos. MÉTODOS: Estudo de coorte retrospetivo de um único centro hospitalar. Foi efetuada a revisão dos registos clínicos de cirurgias DMEK realizadas entre Agosto de 2016 e Agosto de 2022. Os outcomes primários foram a sobrevida do enxerto, a melhor acuidade visual corrigida (MAVC) na escala logMAR, e a densidade central de células endoteliais (ECD). Como outcomes secundários foram consideradas as complicações intra e pós-operatórias, bem como a necessidade de querato- plastias subsequentes. RESULTADOS: No período compreendido entre Agosto de 2016 e Agosto de 2022 foram re- alizados no nosso centro um total de 69 DMEKs, em 56 doentes. A idade média dos doentes à data da cirurgia foi de 65,9 ± 11,8 anos. Dos pacientes, 62,5% eram do sexo feminino e 37,5% do sexo masculino. INTRODUCTION: The COVID-19 pandemic introduced significant changes in our daily habits, including the widespread use of face masks. Numerous reports pointed to an increase in perceived dry eye symptoms, due to an unnatural airflow towards the ocular surface during expiration. The purpose of this work was to assess the change in dry eye symptoms in healthcare profes- sionals with the frequent use of face masks and to measure the effect of a half-workday period of mask wearing in noninvasive ocular surface parameters. METHODS: Ocular Surface Disease Index (OSDI) questionnaire was used to assess symp- toms of dry eye disease. In each answer, the participants were asked to self-report the symptoms before and after the generalized use of face masks. In addition, all volunteers underwent a nonin- vasive ocular surface workup by means of TearCheck® (ESW Vision, Linz, Austria) in the same day at 2 different time points: 1 - in the beginning of the work shift before wearing face mask; 2 - after 6 hours of continuous face mask use. Eye redness score (range 1-4), non-invasive break-up time (NIBUT), tear meniscus thickness and tear film stability evaluation (TFSE®) variation between the two measurements was calculated. RESULTS: Forty eyes from 20 subjects were included. A percentage of 80% (n=16) of the par- ticipants were female and the mean age of the sample was 47.15 ± 12.99 years old. Vários relatos apontaram para um aumento dos sintomas de olho seco, causado pelo fluxo anormal de ar em direção à superfície ocular durante a expiração. O objetivo deste trabalho foi avaliar a mudança nos sintomas de olho seco em profissionais de saúde com o uso frequente de máscaras faciais e medir o efeito da utilização de máscara durante meio período de trabalho na superfície ocular. MÉTODOS: O questionário Ocular Surface Disease Index (OSDI) foi usado para avaliar os sin- tomas da doença do olho seco, antes e após o uso generalizado de máscaras faciais. Além disso, to- dos os voluntários foram submetidos a uma avaliação não invasiva da superfície ocular através do equipamento TearCheck® (ESW Vision, Linz, Áustria) no mesmo dia em 2 momentos diferentes: 1 - no início do turno de trabalho antes do uso da máscara facial; 2 - após 6 horas de uso contínuo da máscara facial. INTRODUCTION: Our purpose was to compare best corrected visual acuity (BCVA), endothelial cell density (ECD) and postoperative complications in adult patients with corneal endothelial disorders who were submitted to descemet stripping automated endothelial keratoplasty (DSAEK) or descemet membrane endothelial keratoplasty (DMEK). METHODS: Retrospective, single-centre, observational cohort study. Fifty one eyes from 51 patients with corneal endothelial disorders who were submitted to either a traditional DSAEK (n=23 patients) or a DMEK (n=28 patients) at Centro Hospitalar Universitário S. João (Porto, Portugal), and followed for at least one year after the procedure in our department were included. Patients without at least one ECD determination after transplantation and those who experienced primary graft failure were excluded. Patient demographics, BCVA with the logMAR scale before and one year after grafting, indication for transplantation, and postoperative complications were recorded. Specular microscopy with ECD determination (in cells/mm2) was performed on all donor corneas before grafting and regularly after transplantation, as part of our patient’s usual follow-up. RESULTS: Patients’ demographics, indications for transplantation and BCVA before graft- ing were similar in both groups. BCVA 1-year after transplantation was better in the DMEK group (0.26 ± 0.19 vs 0.47 ± 0.29 in the DSAEK group; p=0.003). ECD in donor corneas before grafting was similar in both groups (p=0.986). Graft ECD after transplantation was higher in the DMEK group at up to 5 months (p, INTRODUÇÃO: O nosso objetivo foi comparar melhores acuidades visuais corrigidas (MAVC) e densidade das células endoteliais corneanas (DCE) em adultos com patologias endoteliais corneanas que foram submetidos a descemet stripping automated endothelial keratoplasty (DSAEK) ou a descemet membrane endothelial keratoplasty (DMEK). MÉTODOS: Estudo retrospetivo observacional de centro único. Foram incluídos 51 olhos de 51 doentes com patologias endoteliais corneanas que foram submetidos ou a DSAEK tradicional (n=23 olhos) ou a DMEK (n=28 olhos), no Centro Hospitalar Universitário S. João (Porto, Portugal), com seguimento de pelo menos 1 ano após o procedimento. Doentes sem pelo menos uma determinação da DCE após o procedimento e doentes com falência primária do enxerto foram excluídos. Foram colhidos os dados demográficos dos doentes, as suas MAVC antes e após transplante (escala logMAR), as indicações cirúrgicas para transplante e as complicações pós-operatórias. Foi realizada microscopia especular com determinação da DCE (em celúlas/mm2) em todas as córneas dadoras e nos enxertos transplantados, como parte do nosso seguimento habitual destes doentes. RESULTADOS: Dados demográficos, indicações para transplante e MAVC prévia ao transplante foram similares em ambos os grupos. The mean age of patients was 63.7 years and 46.5% were men. In culture were isolated 22 (51.2%) filamentous fungi and 21 (48.8%) yeast. Candida species (n = 20, 46.5%), Fusarium species (n = 10, 23.4%) and Aspergillus species (n = 4, 9.3%) were the most common isolated species. Important risk factors were contact lens use (n = 24, 55.8%), long-term users of topical corticosteroids (n = 19, 44.2%) and previous keratitis (n = 19, 44.2%). Yeast isolates had a statistically significant higher prevalence in long-term users of topical corticosteroids compared to filamentous ones (p = 0.043). Twenty-four cases (55.8%) required surgical intervention, of which 23 cases underwent therapeutic penetrating keratoplasty. Ocular complications, such as evisceration was noted in 12 patients (27.9%) and endophthalmitis in 5 (11.6%). No statistically significant changes of best corrected visual acuity (BCVA) were found after treatment (p = 0.687).Conclusion: Most patients with fungal keratitis have associated risk factors. Filamentous and yeast species have equally prevalent etiologies. Best-corrected visual acuity (BCVA), keratometry values, thinnest corneal thickness (PachyMin) and topometric indexes were analysed preoperatively and at 6-month, 12-month, 18-month and 24-month postoperative. Progression was assessed by increase ≥ 1.00D in maximum keratometry (Kmax); increase ≥ 1.00D in corneal astigmatism; decrease ≥ 2% in PachyMin; increase ≥ 0.42 in D-index.Results: There were no complications during or after TE-ACXL. No significant differences (Δ) were observed between baseline and 12-month or 24-month postoperative: ∆BCVA (− 0.08 ± 0.25, p=0.190; − 0.04 ± 0.17, p=0.588), ∆Kmax (− 0.08 ± 1.32, p=0.792; − 1.04 ± 1.89, p=0.135), ∆Astigmatism (− 0.15 ± 0.89, p=0.485; − 0.24 ± 1.38, p=0.609), ∆PachyMin (− 0.56 ± 15.70, p=0.882; 0.56 ± 18.74, p=0.931), ∆Index Surface Variation (∆ISV) (− 2.11 ± 10.27, p=0.395; − 4.67 ± 17.32, p=0.442), ∆Index Vertical Asymmetry (∆IVA) (− 0.05 ± 0.17, p=0.208; − 0.08 ± 0.26, p=0.397), ∆Index Height Decentration (∆IHD) (0.00 ± 0.02, p=0.368; − 0.01 ± 0.04, p=0.484), ∆KI (0.00 ± 0.05, p=0.851; 0.01 ± 0.06, p=0.877) and ∆D-index (0.15 ± 1.14, p=0.572; 0.06 ± 1.36, p=0.892). Eleven to 33% of patients had disease progression at 24-month postoperative according to the parameters used to determine progression.Conclusion: Although some patients maintain disease progression, TE-ACXL seems to be a safe and effective treatment for keratoconus over the 2-year follow-up period. Objetivo: As técnicas de Queratoplastia Lamelar (KL) permitem a substituição seletiva das camadas de córnea alteradas. A KL associa-se a menos complicações, menor taxa de rejeição e melhores resultados funcionais. O Centro Hospitalar de São João (CHSJ) iniciou a realização de KL em 2009. O objetivo deste trabalho é avaliar a evolução das técnicas cirúrgicas e principais indicações para transplante de Córnea no CHSJ nos últimos 8 anos. Material e Métodos: Análise retrospetiva de todos os pacientes submetidos a transplante de Córnea no CHSJ entre 2011 e 2018. Foram analisadas variáveis demográficas, indicação cirúrgica e tipo de transplante realizado. Resultados: Foram incluídos 1075 olhos nesta análise. Em 2011, 99,3% dos transplantes realizados foram Queratoplastias Penetrantes (KP). A percentagem de KL foi aumentando progressivamente, especialmente depois de 2015. Em 2017 realizaram-se 25% de KL, e, em 2018, 40,9% dos transplantes realizados foram KL. Em 2018, 23,9% foram Descemet’s stripping automated endothelial keratoplasty (DSAEK), 11,4% Descemet membrane endothelial keratoplasty (DMEK) e 5,7% Deep anterior lamellar keratoplasty (DALK)). As principais indicações cirúrgicas foram a patologia endotelial, queratocone e falência de enxerto prévio. Relativamente às ténicas de KL, as principais indicações cirúrgicas foram a patologia endotelial e queratocone, verificando-se um aumento de transplantes por falência de enxerto prévio. Conclusões: O uso de KL no CHSJ tem aumentado progressivamente em detrimento de KP, principalmente desde 2015. Tendo em conta os bons resultados funcionais e baixo número de complicações, a KL deve ser a primeira opção para o tratamento de patologia lamelar seletiva da córnea., Introduction: Lamellar Keratoplasty (LK) allows the selective replacement of altered corneal layers. It is associated with fewer complications, lower rejection rate and better functional results, when compared with penetrant keratoplasty (PK). LK started to be performed in the Centro Hospitalar São João (CHSJ) in 2009, with significant evolution in the number and techniques performed recently. The objective of this study was to evaluate the evolution of surgical techniques and the main indications for corneal transplantation in CHSJ in the last 8 years. Material and methods: Retrospective analysis of the patients undergoing corneal transplantation in CHSJ between 2011 and 2018. Demographic variables, surgical indication and type of transplantation were analyzed. Results: 1075 eyes were included in this analysis. In 2011, 99.3% of the transplants performed were PK. The percentage of LK increased progressively, especially after 2015. In 2017, 25% of the transplants performed were LK, while in 2018 the percentage was 40.9%. In 2018, 23.9% of the transplants were Descemet's stripping automated endothelial keratoplasty (DSAEK), 11.4% were Descemet membrane endothelial keratoplasty (DMEK) and 5.7% were deep anterior lamellar keratoplasty (DALK)). The main surgical indications were endothelial pathology, keratoconus and previous graft failure. Regarding LK techniques, the main surgical indications were endothelial pathology and keratoconus, with a progressive increase of transplants due to previous graft failure. Conclusions: LK techniques performed in CHSJ have increased progressively, especially since 2015. Given the good functional results and low complication rate, LK should be the first option for the treatment of selective lamellar corneal pathology.

Aim: To evaluate the effect of intravitreal anti-vascular endothelial growth factor (anti-VEGF) agents on the development of sustained ocular hypertension (OHT) in patients with neovascular age-related macular degeneration (AMD). Methods: A retrospective study of patients with neo- vascular AMD who received unilateral intravitreal anti-VEGF treatment was performed. Fellow non-injected eyes were used as controls. Initial intraocular pressure (IOP) was measured before the first injection and final IOP was measured 3-4 weeks after the last injection. Eyes that re- quired IOP lowering treatment during follow-up were identified. Results: Seventy-two patients were eligible for the study. Sustained ocular hypertension (OHT) developed in 3 (4.2%) of the injected eyes and in 1 (1.4%) of controls. There was no statistically significant difference be- tween final IOP in the study and control groups. In the study group, patients who received >20 anti-VEGF injections showed a significantly larger variation of IOP during follow-up (p=0.034) Conclusion: Eyes receiving a higher number of injections suffer a significantly larger IOP varia- tion than eyes treated with fewer injections, but larger studies are needed to conclude if treatment with intravitreal anti-VEGF agents increases the risk of developing sustained OHT., Revista Sociedade Portuguesa de Oftalmologia, Vol. 38 N.º 4 (2014)
