Despite advances in ex vivo expansion of cord blood‐derived hematopoietic stem/progenitor cells (CB‐HSPC), challenges still remain regarding the ability to obtain, from a single unit, sufficient numbers of cells to treat an adolescent or adult patient. We and others have shown that CB‐HSPC can be expanded ex vivo in two‐dimensional (2D) cultures, but the absolute percentage of the more primitive stem cells decreases with time. During development, the fetal liver is the main site of HSPC expansion. Therefore, here we investigated, in vitro, the outcome of interactions of primitive HSPC with surrogate fetal liver environments. We compared bioengineered liver constructs made from a natural three‐dimensional‐liver‐extracellular‐matrix (3D‐ECM) seeded with hepatoblasts, fetal liver‐derived (LvSt), or bone marrow‐derived stromal cells, to their respective 2D culture counterparts. We showed that the inclusion of cellular components within the 3D‐ECM scaffolds was necessary for maintenance of HSPC viability in culture, and that irrespective of the microenvironment used, the 3D‐ECM structures led to the maintenance of a more primitive subpopulation of HSPC, as determined by flow cytometry and colony forming assays. In addition, we showed that the timing and extent of expansion depends upon the biological component used, with LvSt providing the optimal balance between preservation of primitive CB HSPC and cellular differentiation. The author describes the Castel Sant'Angelo in Rome, Italy. The building was originally erected by the Roman Emperor Hadrian in the 130s as his own mausoleum and as a family vault that was used until 217 A.D. The accompanying access bridge, now known as the Ponte Sant'Angelo, was for a time the only bridge that crossed from Rome to the Vatican and is now decorated with sculptures by Bernini.

The Minnesota Multiphasic Personality Inventory was administered to 47 male patients on the Renal Dialysis Unit at the Seattle VA Hospital. The patients were divided into three survival groups for comparison. Group A died within 1 year of initiating dialysis. Group B had been alive on dialysis between 3 and 7 years, while group C had been alive between 7 and 10 years at the time of data analysis. Group A differed significantly from group B on the Hs, D, and Hy scales, and from group C on the F, Hs, D, and Pt scales. Interpretation of the mean MMPI profiles implies that persons in group A are characterized by feelings of helplessness, high levels of depression, anxiety, and preoccupation with somatic difficulties, whereas those in groups B and C tended to be dependent, have mild levels of depression, and have a sense of hopefulness about the future. The authors suggest that psychiatric intervention might increase the longevity of those patients judged to be moderately to severely depressed with somatic preoccupations.

In a selected subgroup of 50 survivors of cardiac arrest, the impact of surgical myocardial revascularization on inducible arrhythmias, arrhythmia recurrence and long-term survival was examined. The effects of several clinical, angiographic and electrophysiologic variables on arrhythmia recurrence and survival were also analyzed. All patients had a prehospital cardiac arrest and severe operable coronary artery disease and underwent myocardial revascularization.

A young woman with palpitation and syncope was found to have ventricular tachyarrhythmia and a congenital long QT interval. The QT interval was shortened and the arrhythmia suppressed by propranolol. Electrograms recorded at various sites in both ventricles revealed a distinct diastolic slow wave that followed the T wave and measured 1.1 mV. Epinephrine infusion and emotion augumented this diastolic wave and induced ventricular ectopic complexes arising from this potential. Similar endocardial recordings in eight patients without a long QT interval showed diastolic slow waves that never exceeded 0.28 mV. In normal canine myocardium, afterdepolarizations can be induced by norepinephrine and blocked by propranolol. These findings suggest that the long QT syndrome is associated with abnormally large afterdepolarizations in ventricular myocardial cells, which are enhanced by beta-adrenergic stimulation to attain threshold and produce firing.

This study investigated whether data available after the initial electrophysiologic study in patients with sustained ventricular tachyarrhythmia could identify those patients in whom serial drug testing is likely to be efficacious. One hundred six patients with inducible sustained ventricular tachyarrhythmia, whose initial study included short-term drug testing with intravenous procainamide, were evaluated. The baseline arrhythmia induced (in the absence of all antiarrhythmic drugs) was monomorphic tachycardia with a cycle length >200 ms in 81 patients and ventricular flutter or fibrillation in the remaining 25 patients. After intravenous infusion of procainamide (1,250 ± 300 mg), a ventricular tachyarrhythmia could still be induced in 80 patients during testing with up to three extrastimuli. Serial drug testing with one to four trials of oral conventional and investigational agents was then undertaken.

Balanitis xerotica obliterans (post-operationem) is the term which Stühmer1 used in 1928 to describe an atrophic, shrinking process involving the glans and prepuce and frequently leading to urethral stenosis. On the basis of his first 5 cases Stühmer concluded that, although there was a close relation among various sclerosing processes of the male genitals described in the literature under the titles of leukoplakia and kraurosis, the condition in his cases was singular in that it developed in men in the second and third decades and followed circumcision for phimosis. A type of cutaneous cancer which has received scant mention is the metastatic form, in cases of which metastases from tumors in other organs result from circulating emboli or contiguous implantation from lesions which are directly under the integument. In this communication we shall describe a rare example of extension into, and involvement of, the skin from a mammary cancer.The skin may be involved by mere extension of underlying cancer, and ulceration and papillary tumors may result; such lesions are usually easily recognized. The more one studies lichen planus the more one is struck by the multiplicity and variety of aspects that this disease may present. The clinical variations may range from hypertrophic to atrophic; the color, from red to white. Almost the entire body surface may be involved or only an isolated spot. Subjective symptoms may be severe, while in other cases extensive eruption may be present without even the slightest itching. In spite of all the clinical variations, there is a fairly constant microscopic picture.Lichen planus erythematosus is a rare and unusual form of lichen planus. The rarity of the congenital occurrence of pemphigus simplex or impetigo neonatorum seems to me to be reason enough to report and describe two cases that have come to my attention.There is little in the literature on nonsyphilitic pemphigus. It is true that there have been a number of reports, but many of these do not stand critical analysis and appear to be examples of other conditions, such as toxic bullous eruption following the ingestion of drugs by the mother, epidermolysis bullosa hereditaria and congenital defects of the skin or traumatic denudations. In 1941, we1 reported observations based on a study of 18 cases of balanitis xerotica obliterans, a disease of the male genitals to which attention was first called by Stühmer2 in 1928. Balanitis xerotica obliterans may be briefly defined as a chronic, progressive, atrophic, sclerosing process of the glans and prepuce leading to urethral stenosis. Operations on the genitals, such as circumcision and dorsal incision, are not etiologic factors in our opinion. In contradistinction to Stühmer's2 belief that the disease occurs only in young adults, we feel that it may occur at any age.The onset of balanitis xerotica obliterans is as a rule insidious, although it may appear as an acute disease with pain, itching, burning, prickling and profuse purulent secretion.
