Résumé Le carcinome neuroendocrine de la prostate est une entité rare posant des problèmes aussi bien diagnostiques que thérapeutiques. On distingue essentiellement quatre formes histologiques (les adénocarcinomes à différenciation neuroendocrine, les tumeurs carcinoïdes, les carcinomes à petites cellules, les carcinomes à grandes cellules), pouvant être pures ou mixtes, associées à un adénocarcinome prostatique. Il n’existe pas de consensus sur la prise en charge ni sur le pronostic de ces divers sous-types tumoraux. Nous avons effectué une revue de la littérature afin de déterminer les implications thérapeutiques potentielles. Introduction As part of a phase 2 randomized clinical trial that compares Internal Target Volume (ITV) and Mid-Position (Mid-P) conformational planning strategies in locally advanced non-small cell lung cancer treatment, we investigated the impact of the type of CT image, i.e. Maximum Intensity Projection (MIP) image and Mid-P image on the inter-observer delineation variability. Methods We considered 12 patients among the 40 patients included in the clinical trial. For each patient, dataset for the delineation consisted in MIP image and Mid-P image, computed from four-dimensional CT scan (Big Bore, Philips). Primary tumor (IGTVT for MIP image and GTVT for Mid-P image) and pathological lymph nodes for 8 of the 12 patients (IGTVN for MIP image and GTVN for Mid-P image) were delineated on both images by an expert in lung radiation oncology and 2 or 3 radiation oncologists. The expert contours were considered as a reference and were compared to the others using paired Dice coefficient to estimate the overlap, Bidirectional Local Distance (BLD) to evaluate a Hausdorff distance-based between contours and the reference and Root-Mean-Square Deviation (RMSD) to measure volume variations with respect to the reference. Results The Mid-P delineated volumes were smaller than the MIP ones in average: 2.96 ± 2.75 cc and 16.7 ± 20.7 cc for nodes and primary tumors respectively. For the whole cohort, the DICE means and standard deviations were 0.685 ± 0.102, 0.706 ± 0.124, 0.777 ± 0.0926 and 0.784 ± 0.0756 for the GTVN, IGTVN, GTVT and IGTVT respectively. The absolute means BLD between the expert contours and the others were, in the same order, 2.08 ± 0.819 mm, 2.22 ± 1.36 mm, 2.60 ± 1.34 mm and 2.92 ± 1.35 mm. The RMSD were 3.05 ± 1.92 cc, 4.15 ± 4.66 cc, 13.4 ± 13.1 cc and 17.9 ± 18.3 cc respectively. Conclusions For one patient, we excluded an observer because of a non-complete contour. Regarding GTVN and IGTVN, we observed differences in terms of node selections and also delineation interpretations (nodes vs. lymph node areas). To ensure robust data, we only kept nodes selected by the expert. The presence of atelectasis and fibrosis led to large variations of primary tumor delineation (GTVT and IGTVT). The volumes and its variations decreased between MIP and Mid-P contours. Dice coefficients were lower with MIP compared to Mid-P. However, this metric is volume-dependent and tends to be lower for smaller volume. It may not be adapted here. The BLD was not statistically different. Finally, the volume variations were reduced but our results depended on delineation difficulties and did not demonstrate the statistical superiority of the Mid-P strategy in terms of inter-observer delineation variability. Objectif de l’étude Évaluer l’incidence à long terme de la dysfonction érectile après curiethérapie de prostate, chez des hommes sans dysfonction érectile avant le traitement. Matériel et méthode Entre 2006 et 2012, les données de 97 patients, avec une version de l’I nternational Index of Erectile Function à 5 items (IIEF5) initial de ≥ 22, pris en charge par curiethérapie par iode 125 pour un cancer de la prostate, ont été recueillies prospectivement. La fonction érectile a été évaluée avec le questionnaire IIEF5 à 6 semaines, puis chaque année pendant le suivi. L’absence de dysfonction érectile a été définie par un IIEF5 de ≥ 22/25 et une dysfonction érectile légère par un score entre 17 et 21. Résultats Le suivi médian était de 44 mois. Sur l’ensemble des patients, les scores IIEF5 moyens étaient de 20,9 (6 semaines), 20,6 (1 an) ; 19,9 (2 ans), 19,4 (3 ans) et 17,6/25 (4 ans). Le taux de conservation de la fonction érectile (dysfonction érectile absente ou légère) était de 80 % à 6 semaines et 1 an, 72 % à 2 et 3 ans, et 69 % à 4 ans. Chez les patients de ≤ 65 ans, ils étaient de 79 % (6 semaines), 83 % (1 an), 93 % (2 ans), 75 % (3 ans) et 79 % (4 ans). Pour les patients de plus de 65 ans, ils étaient de 87 % (6 semaines), 77 % (1 an), 69 % (2 ans), 69 % (3 ans) et 62 % (4 ans). Conclusion Avec 4 ans de suivi après la curiethérapie, les patients âgés au plus de 65 ans avaient un taux de conservation de la fonction érectile proche de 80 %. Pour les patients âgés au-delà de 65 ans, ce taux diminuait autour de 60 %. Cette différence entre les deux groupes d’âge n’est probablement pas liée à une plus grande toxicité de la curiethérapie après 65 ans mais à l’évolution naturelle de la fonction érectile de l’homme avec l’âge. Objectif de l’étude Après prostatectomie radicale, la dose standard d’une radiothérapie de rattrapage est de 66 Gy. En cas de récidive macroscopique, cette dose est probablement insuffisante. Une escalade de dose focalisée sur les récidives visibles en IRM a été évaluée en termes de tolérance et efficacité. Matériel et méthode Entre 2011 et 2015, 14 patients atteints d’une récidive (avec une concentration de PSA de plus de 0,2 ng/mL) localement visible sur une IRM et confirmée par biopsies, ont reçu une dose de 60 Gy dans la loge prostatique avec un boost de 12 Gy dans la récidive. Trois grains d’or étaient implantés au contact du nodule. Une technique de RCMI était utilisée avec un repositionnement quotidien de basse énergie (kV/kV) sur os jusqu’à 60 Gy puis sur grains d’or pour la focalisation. Le PSA et la toxicité ont été évalués tous les 6 mois pendant 3 ans. Résultats Le suivi moyen était de 26,2 mois (18–36). La concentration moyenne de PSA au moment de la rechute biochimique était de 0,84 ng/mL (0,3–1,65). Elle a chuté respectivement à 0,18 ng/mL et 0,06 ng/mL à 6 et 12 mois de suivi. Les taux de contrôle local et biochimique étaient respectivement de 100 % et 85,7 % à la date de l’analyse. Deux patients (14,3 %) étaient en situation d’échec biochimique à 12 et 24 mois après la d’une radiothérapie de rattrapage (une récidive métastatique osseuse et une récidive ganglionnaire). Un patient a souffert d’une toxicité de grade 2 urinaire pendant le premier temps d’irradiation, qui est passée en grade 1 lors de la focalisation de dose sur le nodule. Aucune toxicité aiguë ni tardive de grade ≥ 2 n’a été observée par la suite. Conclusion Une escalade de dose sur le nodule macroscopique visible sur l’l’IRM après prostatectomie radicale est réalisable en utilisant une RCMI guidée par l’image. Résumé Objectifs de l’étude L’esthésioneuroblastome ou neuroblastome olfactif représente une entité rare. Nous rapportons dans cet article notre expérience et une synthèse des principales données de la littérature. Patients et méthodes Nous avons analysé rétrospectivement les données cliniques et thérapeutiques des patients pris en charge au centre hospitalier Lyon Sud pour des esthésioneuroblastomes olfactifs histologiquement confirmés. Résultats Les dossiers de dix patients pris en charge entre 1993 et 2015 ont été analysés. Le stade initial, selon la classification de Kadish, était C dans 90 % des cas. Le suivi médian était de 136 mois. La probabilité de survie globale à 10 ans était de 90 %. Celle de survie sans progression était de 70 % et 50 % respectivement à 5 et 10 ans. Neuf patients (90 %) ont bénéficié d’une résection chirurgicale de première intention. Sept des neuf patients opérés (77 %) ont reçu une radiothérapie adjuvante conformationnelle tridimensionnelle, un une radiothérapie avec modulation d’intensité statique, un une arcthérapie volumétrique modulée ( n = 1). La dose moyenne délivrée au site tumoral était de 61 Gy. Aucun patient n’a reçu d’irradiation ganglionnaire prophylactique. Deux patients ont bénéficié d’une chimiothérapie concomitante. Cinq tumeurs (50 %) ont récidivé, une localement, deux dans les ganglions et deux dans le cerveau. Conclusion Ces données semblent comparables à celles de la littérature. En l’absence d’étude prospective, et devant le faible nombre de cas rapportés dans la littérature, il est important de poursuivre l’étude de ces tumeurs et de publier l’expérience de chaque centre afin d’améliorer leur prise en charge. Purpose Olfactory neuroblastoma or esthesioneuroblastoma is a rare entity among head and neck neoplasms. In this paper, we report the experience of our institution and compare it with a comprehensive review of the literature. Patients and methods We retrospectively analysed clinical and treatment data of patients referred to the Lyon Sud University Hospital (France) for histologically proven olfactive esthesioneuroblastoma. Results Ten patients treated between 1993 and 2015 have been analysed. Disease stage at diagnosis, according to the Kadish staging system, was C in 90% of cases. Median follow-up was 136 months. Ten-year overall survival was 90%. Five- and ten-year progression-free survival were 70% and 50%. Nine patients (90%) underwent surgical resection first. Seven of the nine patients who underwent resection (77%) received adjuvant three-dimensional (3D)-conformal radiotherapy ( n = 7), intensity-modulated radiotherapy ( n = 1), or volumetric arctherapy ( n = 1). The mean dose to the tumour volume was 61 Gy. None of the patients received elective nodal irradiation. Two patients received concurrent chemotherapy. Five patients (50%) presented with disease recurrence, which was local ( n = 1), nodal ( n = 2) and cerebral ( n = 2). Conclusion Our results are consistent with the literature. Because of the lack of prospective study and the low number of cases in the literature, each institution's experience is of the utmost important to improve standardised management of these tumours. Purpose: To determine whether SBRT of spinal metastasis using a dedicated treatment planning system (TPS) and delivered with a gantry-based LINAC could provide plans of similar quality to the Cyberknife technology. Additional comparison was also done with other commercial TPS used for volumetric modulated arc therapy (VMAT) planning., Materials and Methods: Thirty Spine SBRT patients, previously treated in our institution with CyberKnife (Accuray, Sunnyvale) using Multiplan TPS, were replanned in VMAT with an dedicated TPS (Elements Spine SRS, Brainlab, Munich) and our clinical TPS (Monaco, Elekta LTD, Stockholm), using exactly the same arc geometry. The comparison was done by assessing differences in dose delivered to PTV, CTV and spinal cord, calculating modulation complexity scores (MCS) and performing quality control (QA) of the plans., Results: Regardless of the vertebra level, in general, no statistical difference was found in PTV coverage between all TPS. Conversely, PTV and CTV D 50% were found significantly higher for the dedicated TPS compared to others. In addition, the dedicated TPS also resulted in better gradient index (GI) than clinical VMAT TPS, whatever the vertebral level, and better GI than Cyberknife TPS for the thoracic level only. The D 2% to the spinal cord was generally significantly lower with the dedicated TPS compared with others. No significant difference was found in the MCS between both VMAT TPS. Introduction: Indications of adjuvant radiotherapy (RT) for high-risk cutaneous squamous cell carcinoma (cSCC) are not clearly defined. We aimed to identify factors predicting relapse in cSCC patients treated with surgery or RT alone and to assess in which clinical setting adjuvant RT was beneficial in term of progression free survival (PFS)., Methods: This retrospective analysis included patients with resectable primary cSCC treated with surgery and/or RT in curative intent, managed at Centre Léon Bérard (Lyon, France) from April 2010 to September 2020., Results: A total of 303 patients with 529 cSCC were included. 31 (5.9%) cSCC were treated with surgery and adjuvant RT. With a median follow-up of 54 (0.2-126) months, 103 (19.5%) cSCC relapsed. In multivariate analysis, the highest predictive factor of relapse in cSCC was the number of risk factors (HR = 15.110 [95% CI: 3.91-58.40] for ≥3 risk factors p < 0.001), followed by poor differentiation (HR = 4.930 [95% CI: 2.47-9.86], p < 0.001) and perineural invasion (HR = 2.442 [95% CI: 1.11-5.38], p = 0.027). For cSCC with ≥3 risk factors, PFS was significantly higher in cSCC treated with surgery and adjuvant RT compared to those treated with surgery or RT alone (the 36-month PFS was 74% [95% CI: 43-90%] and 31% [95% CI: 10-54%] respectively, p = 0.008)., Conclusion: An increased number of risk factors was identified as being the highest predictive factor of relapse in cSCC.
