Background Axillary vein (AV) puncture, an emerging route for cardiac implantable electronic devices (CIED), can be performed under ultrasounds (US) or fluoroscopic (Fluo) guidance. Objective To compare US to fluroroscopy-guided AV puncture in a multicenter randomized controlled trial. Methods Consecutive patients admitted for CIED (first implant or upgrade, including resynchronization therapy) intervention were prospectively randomized between US or Fluo guidance for AV puncture in the three participating centers. Access performances, radiation exposure and complications were compared in both groups. Results 102 patients were included (n = 51 patients per group). The two groups had similar characteristics concerning age (79.4 ± 10 years), and LVEF (47 ± 17%). 92 leads were implanted in both groups. AV catheterization was successful in 50/51 (98 %) in the US group versus 49/51 in the Fluo group (96 %, p = 0.56). AV access time and procedure time were not different between the two groups, respectively (156 ± 274 in the US group versus 137 ± 151 sec, p = 0.66; 54 ± 24 versus 61 ± 26 min; p = 0.13). Total fluoroscopy time (FT) and dose-area product were respectively lower in the US group, but without reaching significance: 197 ± 231 versus 247 ± 293 sec, p = 0.32; 0.39 ± 0.95 versus 0.75 ± 1.58 mGy.m², p = 0.14. FT for AV access was significantly higher in the Fluo group (51 ± 55 sec versus 0, p < 0.0001). There were two complications in each group during the 9 ± 6 months follow-up. Conclusion Our study demonstrates that both US and fluoroscopy-guided AV catheterization for CIED are highly effective and safe techniques. Despite similar AV access time, compared to fluo guidance, US reduce time radiation exposure by 20 %.

4. Incidence and clinical characteristics of patients with torsades de pointes complicating acquired atrioventricular block

Background Female gender, degree of QT prolongation and genetic susceptibility are known risk factors for developing torsade de pointe (TdP) during high-degree atrioventricular block (HD-AVB). Objective To analyze the incidence and clinical characteristics of patients presenting with TdP and AVB (TdP [+]) in comparison with non-TdP patients with AVB (TdP [-]). Methods All the ECG from patients prospectively admitted for advanced AVB (2 to 1 and complete) at our university hospital were analyzed. Automated corrected QT (QTc), and manual measurements of QT and JT intervals, and Tpeak-Tend were performed at the time of most severe bradycardia. Results From Sept 2020 to Nov 2021, 100 patients were admitted for HD-AVB. Among them, 17 patients with TdP were identified (8 men; 81 ± 10 years). No differences could be identified concerning automated QTc, manual QTc (Bazett correction), baseline QRS width and mean left ventricular ejection fraction between the two groups. Potassium serum level on admission and mean number of QT-prolongating drugs per patient were not significantly different between the two groups, respectively: 4.34 ± 0.5 in TdP [+] versus 4.52 ± 0.6 mmol/L (p = 0.33); and 0.6 ± 0.7 in group A versus 0.5 ± 0.7 (p = 0.15). In contrast, manual QTcFR (Fridericia correction), corrected JT, Tpeak-to-end and Tpe/QT ratio were significantly increased in the TdP [+] group: respectively 486 ± 70 versus 456 ± 53 ms (p = 0.04); 433 ± 98 versus 381 ± 80 (p = 0.02); 153 ± 57 versus 110 ± 40 (p < 0.001); 0.27 ± 0.08 versus 0.22 ± 0.06 (p < 0.001). Conclusion The incidence of TdP complicating acquired AVB was 17 % in a medium-volume center. Longer QTc (Fridericia), corrected JT and Tpeak-to-end were electrocardiographic predictors of TdP, while no clinical characteristics may predict the occurrence of this complication.

Background The fundoscopic examination of hypertensive patients, which is an established hypertension-related target organ damage (TOD), tends to be underutilized in clinical practice. We sought to investigate the relationship between retinal alterations and left atrium (LA) volumes by means of real-time, three-dimensional echocardiography (RT3DE), which is the most pivotal predictor of diastolic dysfunction. Methods Our population consisted of 88 consecutive essential hypertensive patients (age 59.2 ± 1.2 years, 53 females) without overt cardiovascular disease. All subjects underwent a fundoscopy examination and were distributed into five groups according to the Keith-Wagener-Barker (KWB) classification. Comprehensive transthoracic echocardiographic and RT3DE measurements were performed to assess LA volumes and phasic functions. Results The four groups (KWB grades 0-4: including 26, 20, 26, and 16 patients, respectively) did not differ with regards to age, gender, or metabolic profile. There were no significant differences between groups with regards to parameters reflecting left ventricle (LV) systolic function and diastolic dysfunctions in conventional echocardiography, except isovolemic relaxation time (IVRT) and deceleration time (DT). Nevertheless, patients in the higher KWB category had higher values of LA volumes (LA maximal volume, LA minimal volume, preatrial contraction volume, LA total stroke volume, LA active stroke volume, p< 0.001) regarding RT3DE (table 1). Introduction L’hypertension pulmonaire associee a la neurofibromatose de type 1 (HTP-NF1) est une complication rare mais severe de la NF1. Les donnees disponibles dans la litterature sont limitees a des cas isoles ou de petites series ( Methodes Nous rapportons les caracteristiques cliniques, fonctionnelles, radiologiques et hemodynamiques ainsi que l’evolution de patients souffrant d’HTP-NF1, issues du registre francais de l’HTP. Une analyse systematique des TDM thoraciques (n = 36) et des prelevements histologiques disponibles (3 poumons explantes) a ete realisee. Les resultats sont exprimes en mediane (min–max). Resultats Nous avons identifie 41 cas d’HTP-NF1, caracterises par une predominance feminine avec un ratio femme/homme de 3,6 et un âge au diagnostic d’HTP de 64 ans [min–max 35–82]. Au diagnostic, 90 % des patients etaient en classe fonctionnelle NYHA III ou IV. La distance parcourue lors du test de marche de 6 minutes etait de 225 m [50–460]. Alors que les EFR montraient des volumes pulmonaires normaux ou legerement diminues, il existait une DLCO effondree a 30 % [18–62] et une hypoxemie severe (PaO2 en air ambiant 56 [38–79] mmHg). L’hemodynamique retrouvait une HTP severe : pression arterielle pulmonaire moyenne 43[29–71] mmHg, index cardiaque 2,3[1,2–3,7] L/min/m2 et resistances vasculaires pulmonaires (RVP) 10,5 [4,5–20,1] UW. L’analyse des TDM thoraciques montrait des kystes pulmonaires (81 %), des opacites en verre depoli (75 %), de l’emphyseme (53 %) et des reticulations (44 %). Des traitements specifiques de l’HTP ont ete inities chez 35 patients avec une amelioration significative fonctionnelle et hemodynamique a la premiere evaluation. Lors du suivi, 4 patients ont ete transplantes et la survie sans transplantation a 1, 3 et 5 ans etait de 88 %, 56 % et 41 %. L’etude histolologique a montre des lesions de pneumopathie interstitielle desquamative et de pneumopathie interstitielle non specifique associees a un important remodelage vasculaire chez les 3 patients. Conclusion La NF1 peut se compliquer d’HTP severe le plus souvent disproportionnee a l’atteinte parenchymateuse. Le pronostic de l’HTP-NF1 est severe malgre l’initiation de traitement specifique de l’HTP. La transplantation pulmonaire represente une option therapeutique pour les patients eligibles.

Purpose: Pulmonary hypertension (PH) is a rare and severe disease for which right ventricular (RV) remodelling and function are associated with survival. The unique anatomy and structure of the RV limit 2D analysis, and its regional function has not been studied yet. We show the contribution of regional 3D deformation analysis for the study of RV remodelling in patients with PH, as compared to healthy controls. Methods: 3D RV echocardiographic sequences were prospectively obtained from 32 patients with PH (57±21y, 15 male) and 10 healthy controls (38±13y, 6 male). Myocardial tracking was performed by semi-automatic Tomtec 4D RV-Function 2.0 software. Volumes and EF were directly obtained from the software. Output RV meshes were post-processed to extract local deformation (area strain) and radial/circumferential/longitudinal displacement. Data were temporally realigned to allow inter-subject pattern comparison. Spatial correspondences were obtained from the software, and allowed statistical computation in 8 standard zones of the RV. Results: 19 patients (59%) were NYHA class III-IV at baseline. Over a mean FU of 49±41days, 5 patients died from cardiopulmonary causes. The highest contraction was at the RV lateral and inferior levels. Regional motion and deformation and global volume patterns for patients was significantly affected at the RV anterior, lateral, inferior, and trabecular and inlet septum. The first 4 segments were significantly associated with NYHA class and survival. Global area strain >-15% was a significant predictor of death (HR 411e12 [140 – 1.2e27]). Conclusion: Our study confirms that the segments into which RV contraction predominates are also the determinants of symptoms and survival in PH patients. This is of high relevance to establish the prognosis of such patients.

Cardiopulmonary exercise testing (CPX) has become the cornerstone of risk stratification for heart failure patients. Peak oxygen consumption (VO2) was the first CPX variable to demonstrate prognostic value and is still the most frequently analyzed variable in clinical practice. More recently, several investigations have shown that ventilatory efficiency, typically expressed as the minute ventilation/carbon dioxide production (VE/VCO2) slope, is a strong prognostic marker in patient with HF. The majority of studies report the VE/VCO2 slope to be prognostically superior to peak VO2 which underscore the clinical importance of assessing ventilatory efficiency in HF patients. Other expressions of ventilatory inefficiency like exercise oscillatory breathing (EOB), oxygen uptake efficiency slope (OUES), end-tidal carbon dioxide pressure (PET CO2) at rest, and haemodynamic responses such as heart rate recovery (HRR) are strong predictors of outcomes in patients with heart failure (HF). So there is a need for simplified approaches that integrate the additive prognostic information from cardiopulmonary exercise testing.

The complications of definitive cardiac stimulation must not be forgotten or sub estimate. The aim of our Registry is to compare the complications of the implantation of a pacemaker in the national and international literature. The assessment of our professional practices has been achieved. We suggest improved procedures. The late complications are not exactly known.

Objectifs Definir le retour veineux pulmonaire anormal (RVPA), en decrire les differentes formes anatomiques et connaitre leurs correlations clinico-radiologiques. Savoir identifier et faire une description precise d’un RVPA sur un angio-scanner. Sujets Illustration du cas d’un nourrisson presentant une hypoxie severe d’etiologie rare : un RVPA total bloque de forme supra-cardiaque droite. Analyse retrospective d’angio-scanners et revue de la litterature illustrant les principales formes de RVPA et leurs presentations clinico-radiologiques. Resultats Le RVPA est une cardiopathie congenitale rare : connexion anormale du systeme veineux pulmonaire dans la circulation systemique. Les tableaux cliniques sont varies : des formes asymptomatiques de certains RVPA partiels a la decompensation hypoxique brutale d’un RVPA total bloque necessitant une chirurgie en urgence. L’angio-scanner est une technique d’imagerie fiable qui confirme le diagnostic de RVPA, parfois suspecte en echographie, en fait une description anatomique rigoureuse, parfois essentielle en preoperatoire (trajet, position du canal collecteur, stenose, compression). Conclusion L’angio-scanner permet un bilan exhaustif des RVPA. Dans les formes totales bloquees, il permet de guider le geste chirurgical a realiser en urgence.

Chez le nourrisson la ligature du canal arteriel (CA) par thoracotomie est regulierement associee a un drainage thoracique post-operatoire, source d’inconfort post-operatoire. But Evaluer les consequences d’une absence de drainage post-operatoire. Materiel et Methode Etude retrospective monocentrique. 24 sujets inclus sur 3 ans (2007-2009) Indication operatoire : clinique (detresse respiratoire et/ou circulatoire, intolerance alimentaire), paraclinique (echographie cardiaque) et echec du traitement medical (ibuprofene 2 cures). Patients Terme : 27 SA (24-41 SA); poids de naissance : 1055 gr (600 – 1 800 gr); âge moyen a l’intervention : 16 j (5-37 j). Intervention meme operateur, thoracotomie trans-pleurale (4e ou 5e EIC), le CA etant occlus par 1 ou 2 clips (/taille et accessibilite). Analgesie post-operatoire systematique pendant 72 h (nalbuphine / paracetamol) puis adaptee aux besoins. Resultats aucun pneumothorax ni epanchement pleural significatif postoperatoire. Aucun geste de ponction ou drainage pleural post-operatoire necessaire. Conclusion L’abstention du drainage thoracique en cas de ligature de CA par thoracotomie est envisageable compte tenu de l’absence de complications pleurales de la thoracotomie et du benefice escompte sur l’antalgie post-operatoire.
