Pulmonary arterial hypertension (PAH) is a form of precapillary pulmonary hypertension caused by a complex process of endothelial dysfunction and vascular remodeling. If left untreated, this progressive disease presents with symptoms of incapacitating fatigue causing marked loss of quality of life, eventually culminating in right ventricular failure and death. Patient management is complex and based on accurate diagnosis, risk stratification, and treatment initiation, with close monitoring of response and disease progression. Understanding the underlying pathophysiology has enabled the development of multiple drugs directed at different targets in the pathological chain. Vasodilator therapy has been the mainstay approach for the last few years, significantly improving quality of life, functional status, and survival. Recent advances in therapies targeting dysfunctional pathways beyond endothelial dysfunction may address the fundamental processes underlying the disease, raising the prospect of increasingly effective options for this high-risk group of patients with a historically poor prognosis. Resumo: A hipertensão arterial pulmonar (HAP) é um tipo de hipertensão pulmonar pré-capilar causada por um processo complexo de disfunção endotelial e remodelação vascular desfavorável. A história natural desta doença progressiva inclui sintomatologia de insuficiência cardíaca e perda de qualidade de vida, culminando em falência ventricular direita e morte. A gestão dos doentes é complexa, baseando-se num diagnóstico preciso, estratificação de risco e início de terapêutica adequada, sendo essencial a monitoração apertada da resposta e progressão da doença.A compreensão da fisiopatologia subjacente permitiu o desenvolvimento de múltiplos fármacos dirigidos a diferentes alvos na cadeia patológica. A terapêutica vasodilatadora destaca-se como a abordagem de base nas últimas décadas, tendo permitido melhoria significativa da qualidade de vida, estado funcional e sobrevida dos doentes. Os avanços terapêuticos recentes, com o advento da utilização de fármacos dirigido aos mecanismos subjacentes à doença, trazem a promessa de opções terapêuticas cada vez mais eficazes para este grupo de doentes de alto risco, com um prognóstico historicamente reservado.

La valorización y utilización de la biomasa lignocelulósica proveniente del procesamiento de alimentos para la obtención de productos con valor añadido es crucial para mejorar la sostenibilidad y reducir costos de gestión de residuos, permitiendo transformar los desechos agroindustriales en recursos valiosos, contribuyendo a la economía circular. Este estudio se enfocó en la caracterización composicional y estructural de once tipos de biomasa lignocelulósica (BLC) con el fin de evaluar su potencial en la producción de nanopolisacáridos y polihidroxialcanoatos. Se analizaron parámetros como humedad, ceniza, proteínas, extractivos, carbohidratos estructurales y lignina en cáscaras de maracuyá, brácteas de alcachofa, cáscaras de espárrago, limón, naranja, semilla de palta, papa, yuca, bagazo de caña de azúcar, cáscara de arroz y paja de arroz. Los resultados mostraron que las cáscaras de frutas y otros residuos presentaron un alto contenido de extractivos (28,05%), mientras que el contenido de lignina y carbohidratos estructurales varió entre 69,66% y 30,53% y 22,2% y 8,84%, respectivamente. Además de la caracterización, se exploró el potencial de esta BLC para la producción de biopolímeros, destacando su relevancia en diversas industrias como la alimentaria y la ingeniería de materiales. En este sentido, estos hallazgos subrayan la importancia de utilizar recursos naturales locales de forma sostenible, abriendo nuevas oportunidades para desarrollar aplicaciones innovadoras como emulsiones pickering, envases biodegradables, aerogeles, hidrogeles y alimentos funcionales. Estas aplicaciones representan áreas prometedoras para futuras investigaciones y desarrollo tecnológico.

Atherosclerotic cardiovascular disease (ASCVD) remains the major cause of premature death and disability; effective cardiovascular (CV) risk prevention is fundamental. The World Heart Federation (WHF) Cholesterol Roadmap provides a framework for national policy development and aims to achieve ASCVD prevention.At the invitation of the WHF, a group of experts from the Portuguese Society of Cardiology (SPC), addressed the cholesterol burden at nationally and discussed possible strategies to include in a Portuguese cholesterol roadmap. The literature review showed that the cholesterol burden in Portugal is high and especially uncontrolled in those with the highest CV risk. An infographic scorecard was built to include in the WHF collection, for a clear idea about CV risk and cholesterol burden in Portugal, which would also be useful for health policy advocacy.The expert discussion and preventive strategies proposal followed the five pillars of the WHF document: awareness improvement; population-based approaches for CV risk and cholesterol; risk assessment/population screening; system-level approaches; surveillance of cholesterol and ASCVD outcomes. These strategies were debated by all the expert participants, with the goal of creating a national cholesterol roadmap to be used for advocacy and as a guide for CV prevention.Several key recommendations were outlined: include all stakeholders in a multidisciplinary national program; create a structured activities plan to increase awareness in the population; improve the quality of continuous CV health education; increase the interaction between different health professionals and non-health professionals; increment the referral of patients to cardiac rehabilitation; screen cholesterol levels in the general population, especially high-risk groups; promote patient self-care, engage with patients’ associations; use specific social networks to spread information widely; create a national database of cholesterol levels with systematic registry of CV events; redefine strategies based on the evaluation of results; create and involve more patients’ associations – invert the pyramid order.In conclusion, ASCVD and the cholesterol burden remain a strong global issue in Portugal, requiring the involvement of multiple stakeholders in prevention. The Portuguese cholesterol roadmap can provide some solutions to help urgently mitigate the problem. Population-based approaches to improve awareness and CV risk assessment and surveillance of cholesterol and ASCVD outcomes are key factors in this change. A call to action is clearly needed to fight hypercholesterolemia and ASCVD burden. Resumo: A doença aterosclerótica cardiovascular (DACV) mantém-se a maior causa de morte prematura e de incapacidade, sendo uma prevenção eficaz de risco cardiovascular (CV) fundamental. O WHF Cholesterol Roadmap fornece enquadramento para o desenvolvimento de políticas nacionais, tendo como objetivo a prevenção de DACV. A convite da WHF, um grupo de peritos da Sociedade Portuguesa de Cardiologia (SPC) avaliou a carga do colesterol a nível nacional e discutiu possíveis estratégias para um Roadmap português.A revisão da literatura mostrou uma carga elevada de colesterol em Portugal, sobretudo em pessoas de alto risco CV. Uma infografia, Scorecard, foi construída pela WHF, para mostrar claramente o risco CV e o peso do colesterol em Portugal, sendo útil para advocacia em políticas de saúde.A discussão de peritos e proposta das estratégias preventivas seguiram os cinco pilares do documento da WHF: Melhoria da consciencialização; Abordagens do risco CV e do colesterol nas populações; Avaliação de risco/screening das populações; abordagens a nível do sistema; Seguimento do colesterol e complicações da DACV.Várias recomendações chave foram definidas: incluir todos os stakeholders em programa multidisciplinar nacional; criar plano de atividades estruturadas para aumentar a consciencialização da população; aumentar qualidade da educação contínua em saúde CV; aumentar interação entre profissionais de saúde e outros profissionais; aumentar referenciação de doentes para reabilitação cardiovascular; avaliar níveis de colesterol na população geral, especialmente em alto risco; promover autocuidado dos doentes, envolvendo associações de doentes; usar redes sociais específicas para disseminar informação; criar base nacional de níveis de colesterol com registo sistemático de eventos CV; redefinir estratégias baseadas na avaliação de resultados; criar e envolver mais doentes em associações – inverter a ordem da pirâmide!Em conclusão, a DACV e o peso de colesterol permanecem uma questão global forte em Portugal, necessitando o envolvimento de múltiplos stakeholders na prevenção. O Roadmap de colesterol português pode fornecer algumas soluções para urgentemente mitigar o problema. Abordagens a nível das populações para melhorar a consciencialização e avaliar o risco CV, seguimento do colesterol e das complicações de DACV são fatores chave nesta mudança. Uma call to action é claramente necessária para combater a carga da hipercolesterolemia e DACV.

[Purpose] This paper seeks to develop, from the current doctrine and regulations, some guidelines for the debate that the Law will have to give shortly, about a new type of personality and its attributes those humanoid robots that are capable of questioning their initial programming and take actions on your own. [Methodology] It is a work developed through a legal-hermeneutical and documentary analysis methodology. The authors used legal doctrine as sources, especially books and book chapters, research results, as well as articles in scientific journals evaluated by peers and indexed in legal databases. To develop the objective of the paper, the authors outlined several stages, first, the search and recovery of information, second a critical analysis, and finally the interpretation of all of the above. [Findings] The authors present their results, on the one hand, with an analysis of the personality of humanoid robots, and on the other, with some reflections on their attributes. The paper presents a critical study on the legal situation of the regulation of the autonomy of the will; which must be understood as the materialized faculty of self-determination that these robots could have in making their own decisions and concerning the rights, obligations, and responsibilities that the Law decides to recognize. All of the above, from artificial intelligence systems that are not subordinated to their initial programming, or the immediate and permanent control of a natural person.

In this article, we present several apparent paradoxes of special relativity and their respective solutions. These paradoxes have appeared since the advent of relativity in 1905, and in fact they are never paradoxes. From a didactic point of view, paradoxes are an excellent learning tool. They lead the student to confront, and abandon, several central concepts of Galilean theory, such as simultaneity and rigidity of extended bodies. In particular, we review a new and simple solution to the twin paradox, recently found by one of the present authors [1]. La velocidad de crecimiento en altura de Swietenia macrophylla destaca en términos de resistencia al ataque del barrenador Hypsipyla grandella y la selección de familias de rápido crecimiento puede viabilizar las plantaciones comerciales. Los caracteres altura, diámetro, número de hojas y forma del tronco de árboles de 4 familias de S. macrophylla, de 47,5 meses de edad, plantados en Ucayali, Perú, se utilizaron para investigar la posibilidad de seleccionar genotipos de rápido crecimiento. La heredabilidad promedio (h2m) y la precisión selectiva (Acprog) entre familias fueron moderadas para la altura total (Ht: h2m: 0,407; Acprog: 0,638) y la altura comercial (Hc: h2m: 0.472; Acprog: 0.687), además de que Hc correlacionó positivamente con la susceptibilidad a el ataque del taladro. La forma del fuste mostró diferencias importantes entre las familias y alta h2m (0,984) y Acprog (0,921). Las correlaciones genéticas (rg) y fenotípicas (rf) entre rasgos mostraron que una selección de individuos con mayores alturas generalmente resultará en un mayor crecimiento en diámetro, número de hojas y mejor forma del tallo. El estudio destaca la importancia de seleccionar familias con rápido crecimiento en altura y tallos rectos para posibilitar, mejorar la calidad y aumentar la productividad de las plantaciones comerciales con la especie.

RESUMO As reflexões de Marx sobre o dinheiro e seu papel nas economias capitalistas estão espalhadas por suas obras. Em particular, constituem a maior parte do 3º volume do Capital e dos Grundrisse. Nessas duas obras, Marx apresenta os fundamentos de uma teoria do dinheiro muito próxima das visões de Keynes. No artigo, essa semelhança é mostrada através da sistematização da teoria monetária de Marx em linhas (pós)keynesianas. A demanda por dinheiro mostra-se enraizada não apenas no motivo da transação, mas também nas propriedades de liquidez do dinheiro. Essas propriedades tornam-se importantes nas economias capitalistas caracterizadas pela incerteza do mercado. Tanto Marx quanto Keynes usam o conceito de dinheiro para quebrar a lei de Say. Uma diferença essencial entre os dois autores é mostrada em relação à função de medida de valor do dinheiro e são desenvolvidas maneiras de reconciliá-los nessa questão.

La Región de las Américas ha experimentado históricamente desigualdades sociales enraizadas en el colonialismo, las cuales se reflejan y reproducen en el ámbito de la salud. La incursión de la pandemia de COVID-19 afectó a toda la Región, pero golpeó con mayor fuerza a los grupos socialmente más desaventajados, y agravó las inequidades en salud. Bajo la premisa que las pandemias no son fenómenos socialmente neutrales, en este informe especial se analizan los impactos desiguales de la pandemia desde distintas perspectivas –histórica, epidemiológica, política, social, económica, ambiental y poblacional. Se ofrecen aquí reflexiones críticas sobre las implicaciones negativas de las desigualdades para el bienestar, no solo de las poblaciones más afectadas, sino de la sociedad en su conjunto. Se concluye con recomendaciones estratégicas para progresar hacia la equidad en salud en el escenario pospandémico. Se destaca la importancia de avanzar en la madurez de los sistemas de información para el monitoreo de la equidad en salud, la resiliencia de los sistemas de salud, y la implementación de políticas y prácticas explícitas dirigidas a eliminar las inequidades en salud. Se espera que todo lo anterior allane el camino hacia la prosperidad y el desarrollo sostenible en la Región.

15. Improving spatial accessibility to health care services in Cali, Colombia: stakeholder assessment of an innovative platform

Objectives. To (i) ascertain stakeholders’ perceptions of the contextual factors and resources necessary to successfully implement the AMORE platform, a tool that provides accessibility assessments for health care services, considering factors such travel time and traffic conditions, and (ii) identify potential barriers to and facilitators for enhancing spatial accessibility to health care services within the Colombian urban context. Methods. In this qualitative study, semi-structured interviews were conducted with a purposive sample of seven key stakeholders. The sample was drawn from individuals involved in development of policies in Colombia, service providers, and users, among others, who had expertise in the field. Interviews were conducted until saturation was reached. Results. The participants had positive views on the appearance of the AMORE platform, highlighting its user-friendly visualization. Suggestions were made about the variables used in the dashboard, the implementation of the platform, potential usage areas, and barriers and facilitators to implementation and use. Barriers included economic, political, and personnel challenges, while facilitators included creating a minimum viable product at a low cost and building interinstitutional and international cooperation. Conclusions. Innovations such as the AMORE platform have the potential to support decision-making processes across various sectors, including public policies and internal processes within private organizations, academia, and the community. However, implementing such a tool has financial, contextual and environmental challenges. The study identified key factors that were considered prerequisites for successfully implementing the AMORE platform in Colombian cities.

Introdução: A referenciação hospitalar, cumprindo critérios bem estabelecidos e seguindo parâmetros de qualidade, permite uma melhor gestão de recursos humanos e económicos. O nosso objetivo foi caraterizar a referenciação à consulta hospitalar em idade pediátrica de uma Unidade de Saúde Familiar pertencente ao Agrupamento de Centros de Saúde do Grande Porto III, durante o ano de 2019. Material e métodos: Estudo retrospetivo e descritivo que incluiu indivíduos em idade pediátrica, que frequentaram as consultas de Saúde Infantil e Juvenil desta Unidade de Saúde Familiar, durante o ano de 2019 e que foram referenciados a consulta hospitalar via AlertP1®. Resultados: A taxa de referenciação global foi de 11,1%, 40,3% no grupo etário com idade superior a nove anos. As consultas hospitalares mais solicitadas foram as das áreas cirúrgicas/médico--cirúrgicas. 88,9% foram motivadas por patologias do foro somático (vs 11,1% do foro do desenvolvimento). Verificou-se uma associação estatisticamente significativa entre entidade de quem partiu a iniciativa para a necessidade de referenciação e motivo da referenciação, sendo as patologias do desenvolvimento/comportamento maioritariamente sinalizadas pela Escola/Infantário. Quanto à qualidade das referenciações, a maioria foi classificada como “Aceitável”, com uma correlação positiva entre anos de prática clínica e má qualidade das referenciações. Verificamos que mais de 50% das consultas não respeitaram os 120 dias admitidos para uma referenciação de prioridade normal fixados pela portaria que regula a “Consulta a Tempo e Horas”. Conclusão: A nossa taxa de referenciação foi superior à observada por outros autores, com 5,2% dos pedidos a serem recusados.

Lograr transformaciones cualitativas en el proceso de enseñanza - aprendizaje como consecuencia de un amplio y generalizado empleo de las TIC ha sido una de las premisas en la actual transformación curricular llevada a cabo en la Educación Superior, estos cambios han de expresarse fundamentalmente en la renovación de concepciones y prácticas pedagógicas que impliquen reformular el papel del profesor y desarrollar modelos de enseñanza - aprendizaje distintos a los tradicionales. Es precisamente, en este entorno educativo, donde aparecen los conceptos de las TAC y las TEP con el objetivo de reorientar el empleo de las TIC hacia un desarrollo formativo y colaborativo, tanto para el estudiante como para el profesor. El presente trabajo tiene como objetivo proponer la aplicación educativa sobre amplificadores operaciones (OpEdu) en su versión 0.5 como forma organizativa de integración de las TIC – TAC – TEP en el proceso de enseñanza – aprendizaje de la carrera de ingeniería eléctrica de la Universidad Tecnológica de la Habana José Antonio Echeverría, a partir de las actuales trasformaciones curriculares. La aplicación está basada en el software libre Scilab y permite el análisis, la experimentación, demostración y diseño de las funciones de los amplificadores operacionales. Dentro de los resultados obtenidos se encuentran el desarrollo del diseño de siete módulos, además permite que los estudiantes sean capaces de realizar de forma independiente sus propios experimentos y diseños en un ambiente seguro. Con el desarrollo de la aplicación se facilita un mayor acceso entre los estudiantes y especialistas a fines.

Objective. To compare the adequacy, agreement, and acceptability of Papanicolaou testing (cytology) for cervical cancer screening using self-collected samples compared to physician-collected samples in Grenada in the Caribbean. Furthermore, the study identifies the human papillomavirus (HPV) genotypes present among asymptomatic women testing positive for HPV, the etiologic cause of cervical cancer. Methods. Participants were divided into two groups and two cervical samples were collected from the women in each group: a self-collected sample and a physician-collected sample. Cervical specimens were tested for cytology and HPV. HPV genotyping was performed on positive specimens. Results. Self-collected samples were adequate and in agreement with physician-collected samples, showing no difference between the two sampling methods. Oncogenic high-risk HPV genotypes were identified in cervical samples which were positive for atypical squamous cells and low-grade squamous intraepithelial lesions. The high-risk HPV genotypes found, notably HPV 45 and 53, differed from those most commonly reported. Although the commonly reported high-risk genotypes HPV 16 and 18 were found, so were 31, 33, 35, 52, 66, 68, and 82. Conclusions. Using self-collection facilitated the discovery of unexpected HPV genotypes among asymptomatic women in Grenada. These findings add new information to the literature regarding cervical cancer and neoplasia screening and HPV genotypes in the Caribbean. This genotype information may impact surveillance of women with low-grade lesions, HPV vaccine selection, and possibly further vaccine research. This research focuses on analyzing the effect of high employment temporality on technical efficiency and productivity. Previous literature points out a set of benefits and drawbacks related to fixed-term contracts. To ascertain the influence of temporality on efficiency, the critical case of the hotel sector in the Canary Islands is studied where the temporary employment rate (40%). So this paper explores the effect of temporary employment on technical efficiency in the hotel sector of the Canary Islands (a Spanish region). The magnitude of this case study can be clearly checked, given that the Canary Islands is the most popular region in the European Union visited by residents. A production stochastic frontier is estimated by distinguishing between two types of employment: permanent and temporary. Results suggest that high levels of temporality tend to reduce technical efficiency and productivity. A high conversion rate from temporary to permanent jobs seems to increase efficiency. The contribution of this paper is threefold: (i) filling the gap in the literature on the influence of temporary employment on technical efficiency, evaluating its effect for a case study where job temporality is dramatically high, (ii) partial factor productivities for permanent and fixed-term workers are estimated, and (iii) exploring the influence of less studied variables like the level of conversion from fixed-term to permanent employment on efficiency. The findings can be relevant for human resources management for firms where temporary rates are high. Human resource management should consider the type of labor contracts used, as well as the perspectives of conversion into permanent ones for their temporary workers, given their potential influence in technical efficiency, labour productivity, and firm performance.

A prevenção da obesidade infantil é uma questão crítica para a saúde pública, pois a grande maioria do excesso de peso é adquirido ainda em idade pré-escolar. De acordo com a Organização Mundial de Saúde, em 2016, 41 milhões de crianças apresentavam excesso de peso ou obesidade com menos de 5 anos. A obesidade é difícil de combater, persistindo na idade adulta, e aumenta o risco de doenças crónicas. Embora os comportamentos de estilo de vida modificáveis, como alimentação saudável e atividade física, sejam determinantes no sucesso do combate à obesidade, outros fatores da sociedade atual decorrentes do avanço tecnológico têm um peso marcado nesta doença. Para prevenir a obesidade na primeira infância, é importante entender os fatores de risco e as suas interações. As intervenções multidisciplinares demonstram ser eficazes, quando há disponibilidade e motivação, principalmente do ambiente familiar e escolar, onde as crianças estão inseridas. No entanto, desenvolver e avaliar intervenções complexas para a prevenção da obesidade é um desafio para todos os envolvidos, profissionais de saúde, crianças, famílias e outros cuidadores.Esta revisão tem como objetivo discutir as linhas mais recentes para a prevenção e abordagem da obesidade infantil, analisando as evidências das intervenções e discutindo as barreiras da sua aplicação.

The flora and fauna of Panama is species-rich due to its location and topography, and the resulting diversity of microclimates and habitats. The last two summaries (benchmarks) of information about the caddisfly fauna (Insecta, Trichoptera) Panama were published in1992 and 2015. From 1861 to 2015, researchers from outside of Panama recorded 13 families, 45 genera, and 257 species of caddisflies. From 2015 through 2023, a series of publications by the Aquatic Invertebrate Research Group at the Universidad Autónoma de Chiriquí in David, Panamá have recorded an additional 2 families,11 genera, and 278 species. Thus, a total of 535 species of caddisflies, now recorded from the Republic of Panama, are distributed among 15 families and 56 genera. Panama’s Trichoptera fauna shows greatest affinity to other Central American countries, and to Costa Rica in particular. Indeed, 289 Costa Rican species are shared with Panama, including 91 former Costa Rican endemics now known from both countries. The Hydroptilidae is the most species rich family in Panama. Although the number of new species and new country records of macro-Trichoptera has diminished somewhat over this third benchmark period, no similar fall-off has yet been detected in the micro-Trichoptera. The caddisfly fauna of Panama is now better known, but much more work remains to define the fauna and, just as importantly, to map the surficial and altitudinal distribution of each genus and species.

This work presents an optimal control problem for an optional public goods game when a fractional punishment is used to improve cooperation. The objective function is mainly determined by the error trajectory, as well as the effort of the controller over a prespecified period. The results show that having chosen the parameters of the objective function appropriately, several optimal solutions can be obtained. Still, most importantly, given the desired kind of solution trajectory and the cost of investment in time, money, and capable personnel, the optimal control provides a more affordable solution (in terms of control effort) than the fractional punishment itself.

This study investigated the preparation of a thermoplastic starch-based (ATP) biocomposite reinforced with eucalyptus fibers (FE), focusing on how fiber size affects the properties. The natural fibers were chemically processed using alkaline and bleaching treatments of eucalyptus bark. Subsequently, they were characterized by infrared spectroscopy (FTIR) and X-ray diffraction (XRD) to evaluate alterations in fiber composition and crystalline structure. Samples of biocomposites were prepared with a fixed fiber content (20 wt.%) and variations in particle size (dp): ATP/FE600 (600 μm ≤ dp < 1180 μm), ATP/FE250 (250 μm ≤ dp < 600 μm), and ATP/FE75 (75 μm ≤ dp < 250 μm). The samples were characterized by optical and scanning electron microscopy, mechanical tests, moisture absorption, and swelling analysis. Results indicated significant effects of fiber size on the biocomposite properties. Notably, the larger fibers present in ATP/FE600 exhibited superior tensile strength and elastic modulus, while this sample demonstrated reduced moisture absorption and swelling tendencies. Microscopic images revealed enhanced matrix-fiber adhesion in ATP/FE600, highlighting the importance of proper fiber size. The optimization of fiber size emerges as a relevant strategy for improving the mechanical attributes and mitigating moisture/water absorption tendencies in starch-based thermoplastic biocomposites, contributing to sustainable and high-performance materials.

Introdução: O uso de medicamentos intravenosos aumenta o risco potencial de infecção pelo rompimento de defesa da pele integra, abrangendo desde quadro inflamatório não infeccioso até quadros de septicemia, uma vez que constitui porta aberta entre o meio externo e o meio intravascular. O termo terapia sequencial pode ser definido como programa de conversão precoce do medicamento intravenoso por sua apresentação oral quando o mesmo apresentar alta biodisponibilidade. O ciprofloxacino é um dos medicamentos que possui apresentação intravenosa e oral e quando comparados, apresentam biodisponibilidades equivalentes, o que garantiria a substituição das apresentações de maneira segura e eficaz, levando em consideração particularidades que inviabilizem essa conversão. O objetivo do trabalho foi analisar a possibilidade de conversão de uso do medicamento ciprofloxacino da sua forma intravenosa para a oral. Material e Método: Foi realizada uma análise transversal retrospectiva e descritiva das prescrições de pacientes em uso de ciprofloxacino intravenoso, no mês de dezembro de 2016, em um hospital público da Bahia. A realização dessa análise ocorreu em duas etapas, a primeira com levantamento das prescrições dos pacientes internados, que fizeram uso de ciprofloxacino intravenoso no período de 1 a 31 de dezembro de 2016 e a segunda através da avaliação do quadro clínico desses pacientes, via prontuário eletrônico, levando-se em consideração infecção a ser tratada e condições específicas que inviabilizassem a conversão IV-VO. Resultados: Durante a análise foram encontrados 20 pacientes em uso de ciprofloxacino IV. Após avaliação criteriosa de cada paciente, foi verificado que 95% (19) desses teriam a possibilidade de utilizar ciprofloxacino oral sem interferência na eficácia do tratamento, o que acarretaria em menos riscos de infecção e reações adversas locais. Discussão: Verificou-se que a possibilidade de conversão do ciprofloxacino IV-VO acarretaria numa economia de 97% (R$ 2.458,06) do custo de ciprofloxacino para a instituição, no período estudado, quando comparada terapia venosa versus oral. Introdução: Os antimicrobianos (ATB) são utilizados para combater uma infecção estabelecida, com o intuito de eliminar o patógeno ou evitar o seu crescimento. A resistência aos ATB é um problema de saúde pública em todo o mundo e o custo de uma falha terapêutica por uma bactéria resistente tem se tornado cada dia maior. O objetivo deste trabalho foiidentificar o perfil de utilização de ATB em uma UTI de um hospital público da Bahia. MATERIAL E MÉTODO: Foi realizado um estudo transversal, retrospectivo e descritivo, no qual as informações foram coletadas das prescrições e evoluções médicas registradas em prontuário eletrônico, considerando o período de outubro a dezembro de 2016. RESULTADOS/DISCUSSÃO: Foram analisadas 45 prescrições eletrônicas, das quais 54% de pacientes do sexo feminino e 45% do sexo masculino. Referente aos diagnósticos analisados, 40% dos pacientes utilizaram antibiótico para sepse, 17% para ITR, ITU, Endocardite, infecção de corrente sanguínea ou osteomelite e 37% para outras infecções. Destes, 69% dos casos tiveram programação de antibiótico para 14 dias, 29% para 10 dias e os outros 2% distribuídos entre 7 dias, 4 semanas e outros. Do grupo de antibióticos utilizados no tratamento das infecções, a piperacilina + tazobactam foi a droga mais utilizada (54%), seguida da vancomicina (20%) e teicoplanina (17%). Das prescrições, 82% apresentaram solicitação de culturas. Destes, 76% das culturas foram realizadas previamente à Introdução: do antibiótico, 5% solicitaram após inicio do antibiótico e 19% não solicitaram. Das culturas solicitadas, 54% positivaram para algum patógeno, sendo que 32% dos pacientes apresentaram Staphyloccocussp., 22% apresentaram Klebisiella (ESBL) e/ou Escherichia coli, 10% Enteroccocus e 36%, outros. Quanto à sensibilidade, 50% das bactérias foram sensíveis ao antibiótico escolhido. No entanto, 60% dos esquemas foram alterados após resultado das culturas, destes 40% foram para inclusão de novos fármacos e 60% para troca de classe. Das trocas realizadas, 55% foram por falha terapêutica e 25% para ampliação de espectro, os outros 20% não houve registro do motivo da alteração. Conclusão: Este estudo apresentou grande quantidade de prescrições de antimicrobianos sem solicitação de culturas e alteração de esquema terapêutica sem justificativa prévia, o que evidencia a necessidade da existência de protocolos para uso de ATB e evidencia a importância farmacêutico, garantindo o uso racional de medicamentos.

A instituição de profilaxia para TEV na presença de situações de risco conhecidas constitui-se em medida de custo efetivo e é amplamente recomendada por comitês normativos internacionais, tendo como base grandes ensaios clínicos, duplo-cegos e controlados por placebo. Este trabalho teve como objetivo analisar o manejo farmacotarapêutico para profilaxia do tromboembolismo venoso, nos pacientes clínicos de um hospital público referência em Cardiologia, em Salvador. Material e Método: Foram avaliados 94 prontuários de pacientes admitidos no período de 1 – 15/06/2017 com intuito de avaliar a quimioprofilaxia para TEV. Como método de avaliação do risco, empregou-se o escore de Pádua e foram incluídos no estudo apenas pacientes adultos, admitidos para tratamento clínico. Resultados: Dos 94 pacientes avaliados, 16 foram excluídos por apresentarem contraindicação para utilização de profilaxia. Sendo assim, dos 78 pacientes elegíveis, 54% eram do sexo masculino e 46% do sexo feminino, com prevalência de pacientes idosos. A maioria dos pacientes estava internada em enfermaria, tendo como diagnóstico principal Infarto Agudo do Miocárdio. Red harvester ants, Pogonomyrmex barbatus (Smith, 1858) (Hymenoptera: Formicidae), common in the Lower Rio Grande Valley of Texas, are known to gather seeds from areas around their nests and store the seeds inside their nests for later consumption. As these ants often nest in and near agricultural fields, some of these seeds may be from crops and may also be infected with fungal plant pathogens. These pathogens can degrade seed coats and may cause the seeds to rot within the ant nests, decreasing storage time and potentially spreading the pathogen to other stored seeds. We studied how head mold, a common sorghum disease, changed ant preferences for sorghum seeds. Using seed depots, we evaluated foraging preferences for sorghum seeds with and without head mold and then monitored how many seeds of each type were collected by the colonies after 1, 2, 4, and 24 hours. We found that red harvester ants did not have any significant preference for infected or uninfected seeds, taking both equally over time. Given this non-preference, ants were assumed to be storing infected seeds next to uninfected seeds within their colonies. However, the risk that stored pathogen-infected seeds poses as a source of future seed infection to seeds within the nest and plants in the surrounding field needs to be further examined.

Background: Vascular access is a rapidly evolving field in modern healthcare that presents challenges across diverse clinical settings. Recent data indicate shortcomings in peripheral vascular access care for patients and nurses in Portugal. Objective: Given the ongoing challenges faced by Portuguese nurses in providing quality care to patients requiring peripheral intravenous access, we aim to identify and discuss potential ways forward in this field Main topics under analysis: The advancement of the field in Portugal involves the development of a clinical standard of care for peripheral intravenous catheterization in pediatric and adult patients, as well as the formal recognition of vascular access as an enhanced nursing competency. Additionally, a long-term focus on institutional initiatives to establish permanent Vascular Access Specialist Teams in Portuguese primary and tertiary care settings is recommended. Conclusion: The proposed pathways are consistent with the current evidence-based, patient-centered nursing care paradigm, as well as with international calls to action on fostering efficient, sustainable, and safe patient care.

The extraction and processing of ornamental rocks generate a very significant amount of waste, representing an environmental and public health problem. The residue is a fine-grained compound by rock powder and inputs used in the processing process have a potential economic value. The present study was carried out to evaluate the efficiency of the waste generated in the discoloration of an aqueous solution containing methylene blue dye, such as alternative for treating textile effluents. The samples were characterized by X-ray diffraction, inductively coupled plasma optical emission spectroscopy, scanning electron microscopy, transmission electron microscopy and energy dispersive X-ray spectroscopy. The catalytic properties of the samples were evaluated in the Fenton and photo Fenton reactions at pH 3 and natural pH. The best decolorization efficiency (≈ 95%) was achieved using the GRM catalyst at pH 3, after 5 min of photo Fenton reaction. Importantly, owing to its excellent structural and chemical stability, the samples showed a decolorization efficiency above 95% after 5 consecutive cycles of 30 min of reaction. This innovative strategy not only offers a new prospect for the rational disposal of abrasive mud waste, but also contributes to minimizing environmental impacts, promoting sustainable development in the stone industry.

We investigate the problem of determining time dependent parameters for discrete-time epidemiological compartmental models such as the Susceptible-Infected-Recovered (SIR). We show how to determine parameters based on minimal error type iterative schemes. Such methods involve the computation of the adjoint of the derivative operator of a nonlinear function. This is a nontrivial task that we accomplish by carefully crafting auxiliary problems. To show the efficiency of the method, we consider examples involving real COVID-19 data.

ABSTRACT Objective: Tofacitinib, an oral Janus kinase inhibitor, has been tested against a placebo in 289 patients with COVID-19 pneumonia. We analyzed the data from the tofacitinib- and placebo-treated patient cohorts to evaluate the laboratory profiles between baseline and day 7. Methods: We performed post hoc analyses on the following laboratory tests over time during the first 7 days after randomization: hemoglobin, leukocytes, neutrophils, lymphocytes, platelets, alanine aminotransferase, and aspartate aminotransferase. Results: Through the first 7 days after randomization, the levels of hemoglobin, white blood cells, neutrophils, and platelet counts were not significantly different between patients treated with tofacitinib or a placebo (all p>0.05). ABSTRACT: Analyzing the causes of death in dogs is fundamental for controlling and preventing diseases, thereby promoting greater longevity and quality of life. However, the current literature lacks standardization and uniformity in the diagnostic description, forms, and instruments for collecting data and maintaining dog obituary records. Using data from the Data Platform of the Animal Mortality Information System (DATASIMA), which uses the Declaration of Animal Death and the International Classification of Diseases (ICD-10), the classification of the causes of death of dogs attended by veterinary doctors was conducted at the “Universidade Federal da Paraíba”, from January to December 2022. The causes of death were stratified, promoting their geo-referencing and the characterization of the epidemiological profile of the dogs. A total of 264 cadavers were analyzed, and the most prevalent groups of diseases causing death were pathophysiological disorders (37.87%, n=100), infectious and parasitic diseases (28.4%, n=75), neoplasms (21.59%, n=57), and external causes (12.12%, n=32). The causes of death were categorized according to sex and age range, showing a higher prevalence of pathophysiological disorders and infectious diseases in adults, neoplasms in elderly dogs, and infections and parasitic diseases and pathophysiological disorders in offspring. The findings of this study demonstrate the feasibility of using DATASIMA for the mapping and standardization of data on the causes of animal death, allowing for strategies for intervention in public health, the prevention of zoonoses, the reduction of garbage codes, better and more efficient communication between veterinary doctors, and the possibility of using ICD-10 to classify the causes of animal death.

En 2023 se conmemora el décimo aniversario de la presentación del concepto de "Comunidad de destino compartido para la humanidad" por el presidente Xi Jinping, que busca promover la solidaridad, la equidad y la justicia en un mundo donde prevalezca la armonía en la diversidad. Esta iniciativa surge como respuesta a la necesidad histórica global de superar las desigualdades y exclusiones impuestas por el colonialismo y el subdesarrollo. Enfatiza además la necesidad de rechazar la imposición de un modelo único a nivel global, al tiempo que se defiende el derecho al desarrollo sostenible e inclusivo. China se posiciona como líder en la promoción de esta visión a través de acciones concretas como iniciativas globales, cooperación Sur-Sur y participación en organismos internacionales. Sin embargo, enfrenta resistencia por un ejemplo de cooperación en la construcción de esta comunidad de destino compartido. Igualmente, la declaración final de la Cumbre de jefes de Estado y de Gobierno del G-77 y China, que tuvo lugar en 2023 en La Habana, Cuba, reafirma la necesidad de una cooperación global para alcanzar un desarrollo científico y tecnológico equitativo.

Abstract Objective: Peer violence is a serious type of school violence that is associated with emotional and behavioral problems. The objective of this study was to analyze violence between peers and its associations with students’ social skills. Methods: We used a cross-sectional survey nested within a cluster-randomized controlled trial (REBEC/Brazil, U1111-1228-2342) to evaluate peer violence among elementary school students and its association with prosocial behaviors and mental problems. Teachers answered an adapted version of the Revised Olweus Bully/Victim Questionnaire (OBVQ) and the Brazilian adaptation of the Teacher Observation of Classroom Adaptation-Checklist (TOCA-C) scale for each student. Children completed a sociodemographic questionnaire. The participants were 1,152 5-to-14-year-old children from Brazilian public schools, 79.70% of whom reported being involved in violent situations. Results: Children who had both committed and suffered violence were less likely to exhibit prosocial behaviors. Children who committed and suffered violence and those who only committed violence were more likely to experience concentration problems and exhibit disruptive behaviors. Conclusion: This study suggests that peer violence is associated with fewer prosocial behaviors and more behavioral problems. Thus, more specialized mental health care is required for children involved in peer violence, in addition to implementation and maintenance of programs to prevent and reduce violence and develop prosocial behaviors in schools.

Se caracterizó la diversidad específica del ecosistema frágil «Lomas de Mangomarca», ubicada en el distrito de San Juan de Lurigancho (Lima, Perú), a través de la identificación de las especies de líquenes, briófitos, plantas vasculares, reptiles, aves y mamíferos. Se incidió en el estado de conservación de las especies que habitan este ecosistema, conforme a la normatividad nacional de especies amenazadas. Se evaluaron las amenazas al ecosistema de lomas, mediante la observación directa en campo y la percepción de los pobladores por medio de encuestas. Se registraron 10 especies de líquenes, 4 de briófitos y 36 plantas vasculares, las cuales fueron complementadas con datos de estudios anteriores, dando un total de 67 especies, 28 aves, 3 reptiles y 1 mamífero, de las cuales 18 son especies endémicas de Perú. El estado de conservación de las especies a nivel global presenta 40 especies en riesgo de extinción incluidas en la Lista Roja global de la IUCN y 8 especies amenazadas a nivel nacional. El principal impulsor directo de amenaza al ecosistema de las Lomas de Mangomarca es la pérdida de hábitat causada por la expansión del casco urbano, y el impulsor indirecto, la débil gobernanza en las lomas. La conservación en este ecosistema es vital para evitar la extinción a nivel local de especies claves que habitan estos ecosistemas frágiles, debido a que el 35% de las especies registradas se encuentran en riesgo de extinción a nivel global y hay una reducción de la cobertura vegetal del 49% al 2020 con referencia al año 2000.

44. First genotyping confirmation of Pichia kudriavzevii in subclinically mastitic cows in Iraq

Fungal pathogens exist obviously in environment; therefore, animals may act as a source of infection to human. Pichia kudriavzevii is ubiquitous fungus of lastly great attention due to its potential use in biotechnology and processing of food, and controversial safety. This study aims to determining the prevalence rate of subclinical mastitis in lactating cows, and demonstration the presence of Pichia kudriavzevii in milk of positively mastitic cows using the molecular phylogeny. Totally, 400 adult lactating cows were subjected for collection an approximately 50 ml of fresh milk that tested initially with the California Mastitis Test (CMT); and then, positive samples have tested molecularly using conventional polymerase chain reaction (PCR). Some molecularly positive samples were analyzed phylogenetically for confirming of local isolates in the National Centre for Biotechnology Information (NCBI). Overall, 54.25% cases were positively reacted by CMT. According to score of positivity, 83.87%, 11.98% and 4.15% were showed weak, distinct, and strong positive infections, respectively. Targeting the ITS region, 28.11% of samples were reacted positively to P. kudriavzevii at 278 bp. Phylogenetic analysis of eight local P. kudriavzevii isolates showed the nucleotide alignment similarity and substitutions. Phylogenetic tree analysis revealed that the local P. kudriavzevii isolates were showed a genetic identity to the NCI-BLAST P. kudriavzevii Mexico isolates (KY646192.1) at total genetic changes ranged 0.0035-0.005%. In conclusion, this represents first molecular phylogenic study in Iraq implicates the presence of P. kudriavzevii in subclinical mastitic cows. Nationwide surveys are useful in monitoring udder health, studying the impact of structural changes, and estimating the factor(s) contribute in incidence of disease and the role of different fungi in it.

The expansion of great-scale photovoltaic (PV) power plants indicates the need for an accurate lifetime assessment of inverters to maintain energy supply availability. In this context, the study contributes in two ways. First, we use machine-learning (ML) models for junction temperature prediction. Second, we perform reliability assessments using a 10-year mission profile in three Brazilian cities. The thermal loadings are obtained through a look-up table approach. Although the ML models exhibit different performances in regression, other factors must be considered, such as easy-to-apply, interpretability, and generalization capability. The reliability assessment is typically based on an annual mission profile, assuming damage repeats until failure. However, only the historical series can confirm whether this choice was acceptable, pessimistic, or optimistic. For instance, in Campos do Jordão-SP, if the chosen mission profile is 2014, the expected failure of 10\% of inverter samples occurs three years earlier than suggested by the historical series. Regardless of the methodology used to estimate thermal loading or accumulated damage, the mission profile significantly influences photovoltaic inverter reliability, indicating that if more data is available, the chosen mission profile should align with the historical series.

The world is currently witnessing a rapid transformation in the production and utilization of electrical energy. The traditional centralized generation model for electric power is swiftly evolving into a more decentralized system, known as Distributed Generation (DG), which incorporates renewable energy sources situated closer to end-users. This shift towards DG has paved the way for the emergence of grid-forming converters, which play a pivotal role in enhancing voltage and frequency stability within microgrids (MGs) and isolated applications. This study focuses on assessing the performance of the Zero Harmonic Distortion (ZHD) in both stand-alone and parallel operation modes. The distinctive feature of this converter lies in its inherent ability to generate a sinusoidal voltage source without the need for capacitive filtering components, which can adversely affect cost, efficiency, and size while potentially contributing to resonance problems. This is achieved through a judicious combination of harmonic cancellation within a three-winding transformer and the utilization of Selective Harmonic Elimination Pulse Width Modulation (SHE PWM) dismissing a closed-loop control structure. Simulation and hardware-in-the-loop results presented in this work demonstrate the satisfactory performance of the ZHD grid-forming converter.

Neste artigo, o controle preditivo do tipo FCS-MPC (Finite-Control-Set Model Predictive Control) é utilizado para controlar a inserção de correntes harmônicas da rede elétrica por meio de um filtro ativo de potência em derivação (FAPD), sendo o propósito atenuar a distorção harmônica total destas correntes e melhorar o fator de potência da rede elétrica. Este tipo de controle preditivo pode ter seu desempenho melhorado a partir de alterações de sua estrutura interna, como o tipo de discretização utilizado para a descrição do preditor. Sendo assim, o objetivo deste trabalho é de realizar uma análise comparativa de desempenho entre diferentes tipos de discretização do preditor. Para tanto serão apresentadas quatro abordagens diferentes, a saber, Euler Regressivo, Trapezoidal, Forma Centrada e Passo Duplo Progressivo (PDP). Os modelos de interação entre o FAPD e a rede elétrica, a construção do algoritmo do controle FCS-MPC e as discretizações do preditor são apresentados e discutidos. Os resultados de simulação são extraídos a partir de uma plataforma do sistema completo (rede + FAPD + controle) operando de forma on-line no Simulink/Matlab. Esta proposta é validada com o uso de um simulador de tempo real OPAL-RT OP5700 disponível no laboratório.

49. Simulações interativas do PhET nas práticas de ensino da física: uma meta-análise

Ao combinar os resultados de várias pesquisas independentes, um estudo de meta-análise sintetiza uma visão geral do conhecimento empírico produzido, outrora limitado a estudos individuais. Tal estudo foi realizado para o caso específico da produção científica relativa ao uso das Simulações Interativas do PhET no ensino da Física, tendo em vista a magnitude do projeto PhET. O presente trabalho consiste em uma revisão sistemática de 1589 publicações indexadas nas bases ERIC, Scopus, IOP Science e WOS, das quais foram selecionados 22 artigos para realização de um estudo de meta-análise. As questões de investigação as quais se responde são: (1) Qual o impacto das simulações PhET no aprendizado da Física? (2) Qual a eficácia dessas simulações quando comparadas a métodos convencionais de aprendizagem, sejam eles teóricos ou experimentais? As análises levaram em consideração a eficácia das simulações PhET no aprendizado da Física, por meio da coleta de dados referentes ao tamanho das amostras, média e desvio padrão. A eficácia das simulações PhET no aprendizado da Física foi devidamente comprovada, revelando superioridade, ou no mínimo equivalência, quando comparadas ao uso de laboratórios convencionais e métodos tradicionais de ensino.

ABSTRACT The call for expanding educational opportunities for all (including the most vulnerable groups) has been both promising and disruptive, particularly as inclusive education policies spread across the globe. Notably, inclusive education embodies a transformative promise for contexts where inequality is ubiquitous. However, different perspectives on the categorization of vulnerable groups are emerging around the globe, which influences the way Inclusive Education is designed and implemented. Thus, in this paper, we reflect on the differences among countries in their approach to implementing Inclusive Education, we discuss the hierarchies of difference that are created, and we set an initial discussion to foster the analysis of categories of difference and their implications for Inclusive Education in future research.
