Although cryptorchidism is probably the most frequent genital anomaly encountered in boys, this pathology remains largely unexplained and its long-term prognosis is still uncertain. Despite the relative easiness of this diagnosis, attention should be paid to some clinical traps, which can mislead the sexual identity. Indeed, beside the isolated common cryptorchidism, many troubles of the testicular migration can be associated with various malformative syndromes and can be named as “cryptorchidism symptoms”. Most of these syndromes are obvious, but some can be suspected only by the way of an abnormal descent of testis. Cryptorchidism, which is systematically screened out during any pediatric consultation, should play the role of clinical marker for various endocrine or genetic disorders. Among them, the intersex states should be underlined to avoid the harmful effects of misdiagnosis. We dealt with this subject through a clinical and practical approach. [Copyright &y& Elsevier]

Objective. – Outpatient surgery in pediatrics is attractive for several reasons. Although 6–8 months has been generally acknowledged as the lower age limit, no minimal age has been clearly established. This study evaluated the outcomes and limits of outpatient surgery in infants from 4 months to 1 year of age.Material and methods. – Between 1996 and 2003, 935 patients were admitted to our hospital for outpatient surgery; of these, 152 were between 4 months and 1 year (mean: 6 months). The indications were essentially surgery to the external genitalia and surgery to remove hernias. The following were exclusion criteria: age below 4 months, ASA scores equal to or above 3, and premature birth with either respiratory distress syndrome or a near-miss of sudden death.Results. – More than 98% of the infants met the criteria for outpatient surgery. Cancellations (1.97 vs. 0.51%, P > 0.05), conversions to classic hospitalization (0.65 vs. 0.37%, P > 0.05), postoperative complications (0.65 vs. 0.25%, P > 0.05), rehospitalization (0.65 vs.0.25%, P > 0.05) and relapse of the initial pathology (0.65 vs. 0.63%, P > 0.05) were not more frequent in these infants than in infants over 1 year.Conclusion. – Pediatric outpatient surgery can be proposed from the age of 4 months without increasing the postoperative risk, either anesthetic or surgical, included for selected former premature infants. Ultrasonography has radically changed the approach of these rare malformations, still diagnosed with difficulties in spite of modern iconography. Bladder anomalies may easily be discovered when associated with otherwise obvious malformations of genitalia. The rest must be suspected for non-specific signs as micturition troubles, multiple urinary tract infections, chronic renal failure. The abnormal bladder is the only common feature of a polymorphic spectrum of diseases including complete or partial duplication or septum, congenital diverticula, vesicovaginal fistula, agenesis and urachal anomalies. Treatment must be accorded to each case and it needs perfect knowledge of open and endoscopic bladder surgery. [Copyright &y& Elsevier]

Objectif: Des mutations de CXorf6, gène impliqué dans la synthèse de la testostérone, ont récemment été décrites chez les enfants 46 XY porteurs teurs d’anomalie du développement sexuel (46XY DSD). Des anomalies de CXorf6 n’ont pas été rapportées à ce jour chez des enfants porteurs d’hypospadias isolés sans ambiguïté vraie. L’objectif de cette étude est d’évaluer le rôle de CXorf6 chez des patients hypospades. Matériel et méthode: 41 patients porteurs d’hypospadias isolés de sévérité variable, et 30 contrôles ont été inclus dans cette étude. L’AND a été extrait à partir de peau de prépuce en excès collectée au cours de la correction chirurgicale de la malfromation. Un séquençage direct de CXorf6 a été réalisé. La structure tridimensionnelle des protéines a été prédite par les serveurs NNPredict et PhyrEngine (UC San Francisco et Imperial College of London). Résultat: Quatre mutations (9,7 %) ont été identifiées. [-] Une mutation missense (V432A) et 2 délétions (del324G, à l’origine d’un codon stop L112X et donc d’une protéine tronquée) ont été retrouvées chez des patients avec des hypospades proximaux et pénoscrotaux. Il est à noter que ces mutations surviennent toutes dans le même exon (exon 3 qui code pour la majeure partie de la protéine) et qu’elles sont associés aux phénotypes les plus sévères. [-] Une amplification du domaine polyglutamine par augmentation du nombre de triplés CAG (un type de mutation généralement impliqué dans les maladies enzymatiques) a été également identifiée chez un patient porteur d’un hypospadias antérieur. La prédiction de la structure tertiaire a montré une altération de la forme de la protéine, et un encombrement stérique entre les domaines. Ces résultats renforcent la présomption d’altération de la fonction de la protéine. [-] Enfin, nous rapportons une incidence élevée de polymorphismes (12,2 %) chez les patients hypospades (haplotype N589S/P286S dans 4 cas). Conclusion: Les mutations de CXorf6 sont présentes non seulement chez les patients 46XY, DSD, mais également dans une proportion significative de patients porteurs d’hypospadias isolés. Les mutations portant sur l’exon principal sont associées aux formes les plus sévères d’hypospades. Introduction: ATF3, gène œstrogéno-dépendant exprimé durant le développement génital, est considéré comme un gène candidat de l’hypospadias du fait de son expression anormalement élevée dans le prepuce d’hypospades, et du fait de son implication dans l’arrêt du cycle cellulaire qui pourrait interférer avec la croissance urétrale. Le but de cette étude est de préciser l’implication d’ATF3 dans l’hypospadias en réalisant son séquençage chez des patients hypospades et en étudiant son expression dans le tubercule génital de fœtus humains. Matériel et méthode: Le séquençage des exons codants et des sites d’épissage d’ATF3 a été réalisé chez 41 garçons porteurs d’hypospadias de sévérité variable et chez 30 contrôles. L’expression d’ATF3 dans le tubercule génital de fœtus humains avec et sans hypospadias a été étudiée par immunohistochimie. Résultats: [-] La mutation missense (Leu23Met) a été identifiée chez un garcon porteur d’un hypospadias antérieur. Cette acide aminé, habituellement hautement conservé à travers les espèces, pourrait avoir un rôle fonctionnel significatif. [-] Trois mutations (C53070T, C53632A, Ins53943A) ont été identifiées dans ou à proximité de l’exon 6. Cette région importante comprend une zone d’épissage alternatif à l’origine de la fomre ATF3Δ zip. Cette forme alternative est impliquée dans la régulation de la fonction d’ATF3. Ces mutations pourraient donc être altérer sa formation et ainsi la régulation de l’action d’ATF3. Les phénotypes associés à ces mutations étaient des hypospadias périnéal, pénoscrotal et antérieur. Aucune mutation n’a été identifiée chez les contrôles. [-] ATF3 n’est pas exprimé dans l’urètre du fœtus normal. Il est par contre exprimé de façon aberrante dans la plaque urétrale chez le fœtus hypospade, en particulier au niveau de l’orifice urétral ectopique. Conclusion: Nous rapportons les premières mutations du gène ATF3 chez des patients porteurs d’hypospadias. Ces résultats, associés à une expression aberrante d’ATF3 dans l’urètre de fœtus hypospade, confirment le rôle de ce gène dans la survenue de cette malformation. [Copyright &y& Elsevier]

Objectif: Des études rétrospectives récentes suggèrent que le pronostic fonctionnel urinairedes malformations du sinus-urogénitalest incertain, voire peu satisfaisant. Diverses approches chirurgicales ont été proposées mais les résultats à long terme des techniques les plus récentes restent à évaluer. Dans l’attente de ces résultats, la connaissance précise de la neuroanatomie du pelvis fœtal pourrait permettre l’optimisation des corrections chirurgicales de ces malformations. Méthode: Des coupes sériées ont été réalisées à partir de pelvis de 10fœtus humains féminins. Des coupes sélectionnées ont été coloré par Masson trichrome et par immunohistochimie avec un marqueur neuronal (anticorps anti S100). A partir de photos numériques de chaque coupe, une reconstruction tridimensionnelle assistée par ordinateur a été réalisée pour obtenir une image animée des organes pelviens et de leur relation avec les structures nerveuses. Résultat: Trois structures nerveuses ont été identifiées : [1-] Un réseau dense périviscéral constituant un feuillet autour des organs pelviens. La majorité de ces nerfs est située sur la face externe des viscères avec une fraction limitée dans les septum recto-vaginal et urétro-vaginal. Cette énervation s’étend en haut, de la face antérieure de l’urètre, vers le bas en arrière du rectum. [2-] Une quantité importante de nerfs enveloppe les faces antérieures et postérieures de l’urètre, notamment dans sa portion supérieure supra-pubienne. [3-] Une structure plus large et plus distante, prenant naissance au niveau du plexus sacré, descend en bas et en avant vers le rectum, croisant ainsi le feuillet nerveux périviscéral. Conclusion: Ces résultats anatomiques sont en faveur, au cours de la correction des malformations des sinus urogénitaux, d’une dissection minimale des faces latérales du vagin d’une part, et de l’urètre proximal au-dessus du ligament urétropubien d’autre part. La mobilisation partielle du sinus urogénital semble ainsi être la seule technique actuelle à respecter l’ensemble des structures nerveuses décrites. [Copyright &y& Elsevier]

Objectif: L’objectif de cette étude était d’évaluer l’alimentation entérale précoce chez les nouveaux-nés atteints de laparoschisis. Méthodes: Deux groupes de nouveaux nés ont été comparés; le premier groupe de référence (16 patients nés entre 2000 et 2004) : la nutrition entérale était débutée après résolution complète de l’occlusion fonctionnelle. Le second groupe (20 nourrissons nés entre 2004 et 2008) a été évalué de manière prospective avec nutrition entérale autour du 5e jour après fermeture de la paroi par allaitement au sein. Résultats: Le temps d’initialisation de l’alimentation entérale a été réduit pour le second groupe (5 jours contre 14 jours pour le groupe de référence). Les durées moyennes d’alimentation entérale totale ont été significativement modifiées (27 jours contre 55 jours). L’incidence des maladies nosocomiales a été diminuée (15 % contre 38 %) ainsi que la durée d’hospitalisation des patients (38 jours contre 62 jours). Conclusion: La nutrition entérale précoce systématique permet de diminuer de manière significative la durée de nutrition parentérale et donc de diminuer les infections sur cathéter central, une des premières causes de morbidité du laparoschisis. Objectif: Nous rapportons ici le cas d’une adolescente de 12 ans, chez laquelle a été diagnostiqué un ovaire surnuméraire. Cas clinique: Une adolescente agée de 12 ans prépubère, sans antécédents particuliers, bénéficie d’une échographie abdomino-pelvienne dans le cadre d’un bilan d’anorexie mentale. Celle-ci retrouve une masse mobile de 37 mm de diamètre, échogène, inter utéro-ovarienne droite, vascularisée avec des petites formations kystiques de quelques mm. Les marqueurs tumoraux sont négatifs. L’IRM pelvien ne confirme le caractère mobile de cette masse dont les caractéristiques tissulaires évoquent un ovaire surnuméraire. Il est réalisé une éxérèse de cette masse sous coelioscopie. L’examen anatomo-pathologique confirme qu’il s’agit d’un ovaire surnuméraire nécrosé. Discussion: L’ovaire surnuméraire est une anomalie gynécologique rare (27 cas recensés dans la littérature dont 3 avant l’âge de 18 ans). Il se définit comme une structure avec du tissu ovarien typique sans connection avec l’ovaire normal. Il peut être retrouvé dans le mésentère, l’épiploon, la cavité pelvienne ou en rétropéritonéal. Le plus souvent, il est asymptomatique et de découverte fortuite (lors d’une laparotomie,d’une lararoscopie ou d’une autopsie). Il peut également se compliquer (kyste, torsion, endométriose ou tumeur). Conclusion: L’originalité de cette observation est double. D’une part, le diagnostic d’ovaire surnuméraire a été fait chez une adolescente prépubère. D’autre part, ce diagnostic a été fait en pré-opératoire sur des données d’imagerie. L’objectif était de créer et de diffuser, à la demande des professionnels de la périnatalité (sages femmes, gynécologues échographistes, obstétriciens, pédiatres, autres) des fiches d’information pratiques sur les principales malformations chirurgicales et leur prise en charge néonatale. Les fiches ont été réalisées dans le cadre du Réseau Périnatal Régional Naître et Grandir en Languedoc Roussillon. Après appel à participation des chirurgiens infantiles plusieurs réunions ont été réalisées pour définir les thèmes d’information et le contenu. Douze fiches d’information ont été établies pour servir de référentiel. Elles portent sur les malformations chirurgicales les plus fréquentes ou celles qui posent le plus de problème. Pour chaque fiche le même plan a été utilisé pour faciliter la lecture. Si nécessaire l’iconographie échographique prénatale et l’aspect clinique postnatal a été inséré. Après validation les fiches seront accessibles de façon sécurisée sur la plate-forme internet du réseau. Les fiches d’information permettront aux professionnels de la périnatalité d’avoir une conduite à tenir pour mieux informer les familles et orienter la prise en charge du nouveau-né. Cette démarche a été réalisée dans le cadre d’un réseau régional de soins et son utilité devra être évaluée. [ABSTRACT FROM AUTHOR]

But: faisabilité de la voie d’abord péri-ombilicale en fonction des pathologies et de l’âge de l’enfant Matériel et Méthode: étude rétrospective de 31 cas de voie d’abord périombilicale utilisé pendant 8 ans (2001-2008). Inclusion : enfants de 1 jour à 18 ans opérés pour des pathologies congénitales ou acquises abdominales (à l’exclusion des pathologies pyloriques ou ombilicales). Résultats: Cette voie d’abord a pu être utilisée chez les 31 patients pour 9 pathologies différentes du tube digestif (7 duplications, 5 Meckel compliqués, 8 atrésies intestinales, 2 lymphangiomes kystiques, 1 iléus méconial, 4 jéjunostomies, 2 corps étrangers, 1 invagination, 1 polype jéjunal). Dans 21 cas (67 %), elle a été combinée à une cœlioscopie première autorisant une exploration de la lésion et dans certains cas facilitant le traitement par abord périombilical. Nous avons eu 3 complications (9,6%) : un hématome de la paroi du grêle, un hématome sous cutané, une occlusion sur plicature de l’anastomose. Conclusion: La voie d’abord périombilicale est de réalisation et d’apprentissage plus aisé que la cœlioscopie avec un bénéfice cosmétique similaire et une faible morbidité. De plus son association possible avec la coelioscopie première permet d’en étendre ses indications. [ABSTRACT FROM AUTHOR]

But: le traitement non opératoire des traumatismes hépatiques fermés est un gold standard chez l’enfant hémodynamiquement stable, mais il n’est pas dénué de complications. Le but de cette étude était d’évaluer l’incidence, les moyens diagnostiques, le traitement et le devenir de ces complications. Méthodes: Étude rétrospective incluant les patients de moins de 16 ans admis au CHU de Montpellier pour un traumatisme fermé du foie sur une période de 10 ans. Résultats: 51 enfants ont bénéficié d’une approche non chirurgicale, avec un taux de succès de 94,1 % (48/51). Trois patients (5,9 %) ont présenté un total de 13 complications directement liées à leurs hépatiques. Les complications hépatiques étaient biliaires, hémorragiques, thrombotiques et un syndrome de compartiment. Ces complications ont été prises en charge par un traitement mini invasif dans 61,5 % des cas (drains percutanés, embolisation) avec succès; Les autres complications ont eu un traitement chirurgical. Tous les enfants ont évolué de manière favorable avec un recul moyen de 7 mois (2 – 54 mois). Conclusion: Les complications du traitement conservateur sont rares mais variées. Beaucoup d’entre elles peuvent être traitées par des techniques mini invasives avec des résultats très satisfaisants. [ABSTRACT FROM AUTHOR]

Les tumeurs de la granulosa juvéniles (TGJ) se développent à partir du stroma gonadique et des cordons sexuels. Elles représentent 5 % des tumeurs ovariennes pédiatriques. L’objectif était de dégager les éléments cliniques et paracliniques importants à prendre en compte pour améliorer leur prise en charge. Sujets et Méthodes: Etude rétrospective monocentrique portant sur 12 patientes atteintes de TGJ, ayant un âge moyen au diagnostic de 6 ans [0-14 ans]. La période d’étude s’est étalée de 1994 à 2007 (recul moyen : 6 ans [3 mois–13 ans]). Les données anamnestiques, cliniques, biologiques, radiologiques, anatomopathologiques, moléculaires, thérapeutiques et l’évolution ont été recueillies. Résultats: Deux modes de présentation ont été mis en évidence, correspondant à deux tranches d’âge. Les signes endocriniens (hyperœstrogénie et hyper-androgénie) étaient au premier plan chez les patientes pré-pubères (n = 9) alors que le diagnostic a été posé devant une masse abdominale chez les patientes pubères (n = 3). Des signes de virilisation ont été trouvés dans 9 cas/12. L’inhibine B était un bon marqueur tumoral (élevé dans 6 cas/6). Le traitement a comporté une ovariectomie complète (11 cas), une tumorectomie (1 cas), une chimiothérapie (2 cas). Les TGJ avaient une double composante cellulaire (cellules de la granulosa et thécale), et une composante kystique (10 cas/12). L’analyse immuno-histochimique conforte le diagnostic. Une corrélation entre les signes endocriniens, les sécrétions et la taille tumorales ainsi que la présence d’une ascite a pu être décrite (dans la population pré-pubère). Il n’a pas été trouvé de lien entre la symptomatologie, le délai de prise en charge et le stade FIGO des tumeurs. Dans 2 cas/10, une mutation somatique R201C du gène codant pour la protéine Gαs était présente dans les cellules tumorales. Elle était associée à des critères macroscopiques (FIGO Ic) ou histologiques de gravité. Conclusions: Le diagnostic de TGJ doit être évoqué devant une puberté précoce et/ou une masse ovarienne avec élévation des taux sériques d’inhibine B. Grace à un diagnostic précoce et à un traitement chirurgical adapté complété par une chimiothérapie dans les formes agressives, le pronostic est excellent mais nécessite une surveillance prolongée. Endocrine disruptor chemicals (EDCs) are ubiquitous contaminants in the environment, wildlife, and humans. During the last 20 years, several epidemiological, clinical and experimental studies have demonstrated the role of EDCs on the reduction of male and female fertility. The concept of foetal origins of adult disease is particularly topical in the field of reproduction. Moreover, exposure to EDCs during pregnancy has been shown to influence epigenetic programming of endocrine signalling and other important physiological pathways, and provided the basis for multi- and transgenerational transmission of adult diseases. However, the large panel of EDCs simultaneously present in the air, sol and water makes the quantification of human exposition still a challenge. Gas chromatography coupled with mass spectrometry, the measurement of total plasmatic hormonal bioactivity on stably transfected cell lines as well as the EDC analysis in hair samples are useful methods of evaluation. More recently, microRNAs analysis offers a new perspective in the comprehension of the mechanisms behind the modulation of cellular response to foetal or post-natal exposure to EDCs. Sex-cord stromal tumors (SCT) are rare pediatric tumors accounting for less than 5% of gonadal tumors in children and adolescents. They differ from those diagnosed in adults by their presentation, histology, evolution and treatment modalities. Testicular SCT occur mostly in infants less than 6 months. Testicular swelling is often the only symptom, but signs of hormonal secretion with gynecomastia may be present. Juvenile granulosa SCT is the main histologic subtype. Sertoli SCTs are much less frequent while Leydig tumors occurred in older children and adolescents. Prognosis is excellent after inguinal orchiectomy. Testis sparing surgery could be performed but indications and modalities have to be strongly defined. Ovarian SCT are diagnosed in older children and adolescents and present with abdominal symptoms and/or signs of hormonal secretion: estrogenic manifestations (isosexual pseudoprecocity, menometrorrhagia) or virilization (hirsutism, amenorrhea). Main histologic subtype is juvenile granulosa (rarely Sertoli-Leydig). If oophorectomy (or salpingo-oophorectomy) may be curative for localized disease, adjuvant cisplatin-containing chemotherapy is mandatory in case of tumor rupture or peritoneal dissemination to prevent recurrences. Pheochromocytomas and paragangliomas (PHEO/PGL) are neuroendocrine tumors that arise from sympathetic and parasympathetic paraganglia. Although well described in the adult population, diagnosis and treatment of these exceptionally rare neoplasms remains poorly characterized in children. This article reviews recent advances in clinical presentation, genetics, biochemistry, imaging and treatment of children with benign or malignant PHEO/PGL. Compared to adults, pediatric PHEO/PGL are more frequently familial, bilateral, multifocal and malignant. Approximately 50% of pediatric PHEO/PGL are associated with a mutation of one of the 12 known susceptibility genes. Von Hippel-Lindau disease, type 1 neurofibromatosis, type 2 multiple endocrine neoplasia and familial PGL syndrome are hereditary tumor syndromes associated with an increased risk of developing such diseases. Clinical presentation includes symptoms related to catecholamine hypersecretion and/or tumor mass effect. Plasma and/or urine metanephrine dosages are recommended as first-line diagnostic biochemical tests. Magnetic resonance imaging is useful as initial radiological approach. Most pediatric PHEO/PGLs are benign. Surgical resection, with appropriate perioperative management of catecholamine-related symptoms, remains the treatment of choice. In case of metastatic disease, surgical removal of metastases when possible and I-131-MIBG radiotherapy provide limited results whereas chemotherapy is reserved for more advanced stages.

Ovarian tumors in childhood are rare, often organic with 10% of malignant cases. Functional pathology dominates in adolescence and its management is the same as the adult. The clinical symptoms of PBOT (presumed benign ovarian tumor) are non-specific. The main clinical signs are acute pain, associated with peritoneal irritation syndrome, which can suggest an ovarian torsion, a mass or the development of secondary sexual characters. Hyperestrogenemia suggests a McCune-Albright syndrome or a granulosa tumor. Hyperandrogenism evokes a malignant tumor. Pelvic ultrasound is the main examination. Pure liquid cysts are benign but could be organic if persisting beyond 6 months. MRI and tumor markers are needed for heterogeneous cyst diagnosis. The protected extraction of a cyst is recommended during the laparoscopic cystectomy. If case of doubt of malignancy, laparoscopy allows the peritoneal cavity exploration. In case of torsion, ovarian untwisting must be performed. After untwisting, the ovary must be preserved because macroscopic aspect is not predictive of the ovarian function recovery. Aims: To analyze the results of treatment of major renal injuries according imaging data in order to determine their function after follow-up., Patients and Methods: This is a retrospective study of 22 cases of fracture of the kidney (grade V) in two pediatric surgical services that were reviewed over a period of 16 years. After initial conservative treatment in 19 patients (86.5%), a scan and/or Uro-MRI were realized in all patients in monitoring evolving. Three children with vascular injury were treated by interventional radiology. The morphology and functional evolution of the injured kidney were determined., Results: A DMSA scan investigation was performed in 21 patients (95.5%) associated with Uro-MRI in two cases; one patient was only explored with Uro-MRI. A complete restitution of the renal parenchyma was confirmed in 10 children (45.5%), we noted an atrophy of the upper pole in 30%, a lower pole atrophy in 4.5%, two complete renal atrophy in 9%. An urinoma was present in six patients (27%) that required drainage in five cases and declined during the surveillance in one case. Normal function of the injured kidney was noticed in half of grade V (11 of 22 patients) with a mean follow-up of 19 months. None of our patients did present hypertension., Conclusion: Non-operative conservative treatment in severe renal trauma was efficient, morphological and functional sequelae were present in 50% on scintigraphy and/or Uro-MRI., (Copyright © 2011. Less than 1% of cancer occurs in children. With the progress made by national and international cooperative groups 75% of them are actually cured. However some entities have an incidence so weak that we can't actually establish standardized therapeutics guidelines. To improve our knowledge on these rare tumours a national organisation become necessary as well as an international collaboration. A French rare tumour committee was created within the French Society for Children Cancer (SFCE). Others European countries have such organisation. The objectives of these tasks groups are to enhance our knowledge of the real incidence of these rare tumours, their evolution, and to propose therapeutic recommendations for each of them. This article focuses on the specific French organization for rare tumours treatment. It also describes the draft for the creation of a new data base for prospective registry of clinical, therapeutics and follow up data. To provide a better understanding of these pathologies, the "Bulletin du Cancer's" editorial board decided to regularly publish an update on a rare paediatric tumour in a specific section.

Ovarian sex cord-stromal tumors are rare tumors that originate from the nongerminal cells of ovary. Two decades ago, the identification of juvenile granulosa-cell tumors (GCT), as a specific entity inside this group, allowed a better treatment of these tumors in children. However, little data have been reported on the natural course of the disease and reliable prognostic factors have not been yet defined. We here review the clinical and genetics aspects of granulosa tumors, based on a series of 40 children. This national collaborative study involved the French Society of Children Cancer and eight clinical departments of pediatric endocrinology. We found that early diagnosis of a tumor, revealed by clinical signs of hyperoestrogeny, is an important prognostic factor. The pathophysiology of these tumors is still debatable and several cellular- and molecular-abnormal signals could be implicated in their development. The role of growth factors and oncogenes through the signaling pathway of MAP kinase is still discussed. According to our data, FSH signaling-transduction pathway, such as a constitutionally activated Galphas, could also be implicated in the induction of granulosa cell proliferation and seems to modulate the invasiveness of the tumor. Last, we have described a low-expression pattern or an extinction of an ovarian-determination gene, FOXL2, which is related to a worse prognosis of this tumor.

Objective: Congenital giant hydronephrosis due to ureteropelvic junction obstruction is exceptional and treatment often requires nephrectomy of a poorly functioning kidney. However, a more conservative approach by nephroplication is also possible. The objective of this study was to evaluate the long-term functional outcome of these kidneys treated by nephroplication during the neonatal period., Material and Methods: From 1996 to 1998, 5 neonates with congenital giant hydronephrosis were managed in our department (3 antenatal diagnoses). The mean anteroposterior pelvic dilatation was 145 mm (range: 110-180 mm). All infants were treated by pyeloplasty with nephroplication, regardless of the preoperative ultrasound and scintigraphic appearance of the renal parenchyma., Results: The mean follow-up was 9 years (range: 8-10 years). A marked initial reduction of global kidney volume was constantly observed (more than 50% of the initial volume). The mean relative renal function was 32.4% (range: 10-42%) without any residual obstruction. Renal growth was satisfactory in 4 cases (40 mm at 1 year, 72 mm at 6 years) with an atrophic kidney in 1 case. No child developed hypertension or elevated serum creatinine., Conclusion: Congenital giant hydronephrosis does not systematically justify primary nephrectomy even in the presence of a healthy contralateral kidney. Despite the pejorative initial ultrasound and scintigraphic appearance, which is always difficult to interpret, early nephroplication improves drainage of pyelocaliceal cavities, decreases the residual dilatation and allows significant nephron sparing in most of these children.

The paediatric endocrinologist is frequently asked whether pubertal development in a girl is normal, early or too early (precocious). This review will cover all clinical expression of premature development of puberty: central precocious puberty (neurogenic, secondary, and idiopathic) where treatment with GnRHa is considered, early puberty, partial puberty or pubertal variants and peripheral or pseudo precocious puberty related to an antonomous hypersecretion of estrogens by the ovaries. A special attention should be paid also to the role of environmental disruptors in the development of peripheral precocious puberty. GnRHa treatment should be considered only when evidence of central activation of the gonadotropic axis is proved by the LHRH-test.

Ultrasonography has radically changed the approach of these rare malformations, still diagnosed with difficulties in spite of modern iconography. Bladder anomalies may easily be discovered when associated with otherwise obvious malformations of genitalia. The rest must be suspected for non-specific signs as micturition troubles, multiple urinary tract infections, chronic renal failure. The abnormal bladder is the only common feature of a polymorphic spectrum of diseases including complete or partial duplication or septum, congenital diverticula, vesicovaginal fistula, agenesis and urachal anomalies. Treatment must be accorded to each case and it needs perfect knowledge of open and endoscopic bladder surgery.
