Introduction Pregnancies in women with lupus nephritis are at high-risk of complications, while scarcity of scientific knowledge on prognostic factors impedes a fair medical counseling. We aimed to identify determinants associated with maternal and fetal complications. Materials We retrospectively reviewed medical charts of pregnancies that lasted more than 22 weeks in 66 patients with pre-existing lupus nephritis between 2004 and 2013 in France. Univariate and multivariate analyses were conducted to identify determinants for maternal complications, lupus renal flare and fetal prematurity or death. Results Eighty-four pregnancies were identified. A maternal complication occurred in 31 pregnancies (36.9%): mostly preeclampsia (17 pregnancies, 20.2%) and renal flares (12 pregnancies, 14.3%). Overall fetal survival was 94.0% (79/84). Maternal pregnancy complications were independently associated with prepregnancy body mass index >25 kg/m2 (OR 3.81, 95% CI 1.03–14.09) and immunological activity (positive anti-dsDNA antibodies or Farr assay lupus) (OR 4.95, 95% CI 1.33–18.43). Renal lupus flares were independently associated with maternal age (OR 1.50, 95% CI 1.12–2.01) and prepregnancy immunological activity (OR 15.99, 95% CI 1.57–162.68) while a remission time >12 months had a protective effect (OR 0.17, 95% CI 0.04–0.68). Three parameters were associated with a higher risk of fetal prematurity or death: a prepregnancy body mass index >25 kg/m2 (HR 3.58, 95% CI 1.45–8.83), hypertension (HR 8.97, 95% CI 3.32–24.25), and immunological activity (HR 3.34, 95% CI 1.30–8.63). Conclusion Maternal age, prepregnancy hypertension, body mass index >25 kg/m2 and lupus immunological activity may be considered as the main determinants for fetal and maternal complications. Objective To study the influence of Maghrebian ethnicity on lupus nephritis. Methods We retrospectively reviewed the files of a cohort of 194 patients with proliferative lupus nephritis followed in seven lupus centres belonging to three groups: Europeans living in Belgium/France (E; n = 111); Maghrebians living in Europe, in casu Belgium/France (ME; n = 43); and Maghrebians living in Morocco (MM; n = 40). Baseline presentation was compared between these three groups but complete long-term outcome data were available only for E and ME patients. Results At presentation, the clinical and pathological characteristics of lupus nephritis did not differ between E, ME and MM patients. Renal relapses were more common in ME patients (54%) than in E patients (29%) (P < 0.01). Time to renal flare and to end-stage renal disease was shorter in ME patients compared to E patients (P < 0.0001 and P < 0.05, respectively). While proteinuria measured at month 12 accurately predicted a serum creatinine value of less than 1 mg/dl at 7 years in E patients, this was not the case in the ME group, in whom serum creatinine at month 12 performed better. Conclusion Despite a similar disease profile at onset, the prognosis of lupus nephritis is more severe in Maghrebians living in Europe compared to native Europeans, with a higher relapse rate. Introduction To provide information about the needs of patients with systemic lupus erythematosus (SLE) using Carenity, the first European online platform for patients with chronic diseases. Methods At one year after its creation, all posts from the Carenity SLE community were collected and analysed. A focused cross-sectional online survey was performed. Results The SLE community included 521 people (93% females; mean age: 39.8 years). Among a total of 6702 posts, 2232 were classified according to disease-related topics. The 10 most common topics were ‘lupus and …’ either ‘treatment’, ‘fatigue’, ‘entourage’, ‘sun exposure’, ‘diagnosis’, ‘autoimmune diseases’, ‘pregnancy’, ‘contraception’, ‘symptoms’ or ‘sexuality’. 112 SLE patients participated in the online survey. At the time of diagnosis, only 17 (15%) patients had heard of SLE and 84 (75%) expressed a need for more information on outcomes (27%), treatments (27%), daily life (14%), patients' associations (11%), symptoms (8%), the disease (8%) and psychosocial aspects (7%). When treatment was initiated, 48 patients (43%) would have liked more information about side effects (46%), long-term effects (21%), treatment duration/cessation (12.5%) and type (10%) and mechanism of action (8%) of treatments. All participants except one had used the internet to find information about SLE. Sources of information included healthcare providers (51%/61%/67%), journals/magazines (7%/12%/6%), lupus Websites (51%/77%/40%), web forums/blogs (34%/53%/19%), patients' associations (11%/23%/9%) accessed at ‘just before diagnosis’, ‘just after diagnosis’ and ‘before treatment initiation’. Objectives: The objective of this article is to describe the outcome (mortality, kidney transplantation) of patients with systemic lupus erythematosus (SLE) on chronic dialysis. Methods: The overall and cardiovascular (CV) mortality and access to kidney transplantation were studied in all SLE patients incident on chronic dialysis in France between 2002 and 2012 (REIN registry). They were compared to age- and sex-matched patients with diabetic nephropathy and with autosomal dominant polycystic kidney disease (PKD) on chronic dialysis. Results: A total of 368 SLE patients were included in the national REIN registry between 2002 and 2012. Cumulative incidence of death was 16.9% at five years, with no difference between haemodialysis and peritoneal dialysis. Independent risk factors of death were age, past history of cardiovascular disease (CVD) and chronic respiratory insufficiency. At five years, CV and all-cause mortality in SLE patients were lower than in matched diabetic patients, but three-fold higher than in matched PKD patients. Access to the kidney transplant waiting list and to kidney transplantation was higher in SLE patients than in matched diabetic patients, but lower than in matched PKD patients. Conclusions: SLE patients on chronic dialysis are a population at high risk of death influenced by CV burden and chronic respiratory failure, but not by the method of dialysis. Patients with chronic kidney disease (CKD) have a much higher risk of cardiovascular diseases than the general population. Endothelial dysfunction, which participates in accelerated atherosclerosis, is a hallmark of CKD. Patients with CKD display impaired endothelium-dependent vasodilatation, elevated soluble biomarkers of endothelial dysfunction, and increased oxidative stress. They also present an imbalance between circulating endothelial populations reflecting endothelial injury (endothelial microparticles and circulating endothelial cells) and repair (endothelial progenitor cells). Endothelial damage induced by a uremic environment suggests an involvement of uremia-specific factors. Several uremic toxins, mostly protein-bound, have been shown to have specific endothelial toxicity: ADMA, homocysteine, AGEs, and more recently, p-cresyl sulfate and indoxyl sulfate. These toxins, all poorly removed by hemodialysis therapies, share mechanisms of endothelial toxicity: they promote pro-oxidant and pro-inflammatory response and inhibit endothelial repair. This article (i) reviews the evidence for endothelial dysfunction in CKD, (ii) specifies the involvement of protein-bound uremic toxins in this dysfunction, and (iii) discusses therapeutic strategies for lowering uremic toxin concentrations or for countering the effects of uremic toxins on the endothelium. L'atteinte rénale est fréquente au cours des AAV et peut conduire à une IRCT. Le maintien des immunosuppresseurs en IRCT pour prévenir les rechutes extrarénales est débattu. Comparer, chez les patients AAV incidents en dialyse chronique en France, le risque de rechute de la maladie, de complications infectieuses et cardiovasculaires avant et après mise en dialyse, et déterminer la proportion de patients inactifs au cours du temps. Étude rétrospective incluant les patients AAV du registre REIN incidents en dialyse en 2008–2012 et appariés aux données du SNDS (système national des données de santé). Diagnostic d'AAV : codage SNDS. Rechute sévère (exclusion de la 1re poussée d'AAV), complication cardiovasculaire ou infectieuse : nécessitant une hospitalisation (SNDS). Rechute non sévère : majoration des corticoïdes/immunosuppresseurs (SNDS) sans hospitalisation. Patient inactif : pas de chimiothérapie depuis ≥ 6 mois, pas de traitement immunosuppresseur, corticothérapie ≤ 7,5 mg/jour ; pas de poussée dans les 6 mois suivants. Inclusion de 229 patients (66,4 ans, 88 F/141 H, 142 GPA/87 PAM), AAV diagnostiquée depuis 1,7 an à la mise en dialyse. Suivis en dialyse 3,2 ans : 29 % transplantés, 50 % décédés (32 % de cause infectieuse, 22 % de cause cardiovasculaire et 5 % d'une poussée d'AAV). Rechutes totales : 0,57/patients/années avant vs 0,07 après dialyse (p < 0,05) ; hospitalisation pour rechute : 12,9 jours/patients/années avant vs 0,96 après dialyse (p < 0,001). Complications cardiovasculaires : 0,13/patients/années avant vs 0,30 après (p < 0,05) ; 2,10 vs 2,91 jours d'hospitalisation (p < 0,001). Infections : 0,16/patients/années avant vs 0,29 après (NS) ; 1,55 vs 3,22 jours d'hospitalisation (p < 0,001). Évolution de la proportion de patients inactifs : analyse en cours. Les patients porteurs d'une maladie rénale chronique (MRC) présentent une altération des fonctions psycho-cognitives parallèlement à la dégradation de la fonction rénale. Dans deux modèles de MRC chez le rat, nous avons mis en évidence une altération des performances cognitives et une augmentation de la perméabilité de la barrière hématoencéphalique (BHE). L'altération des performances cognitives était corrélée à la perméabilité de la BHE, et aux taux d'indoxyl sulfate (IS) circulant. L'objectif principal de ce projet est d'évaluer la perméabilité de la BHE par scintigraphie cérébrale au 99mTc-DTPA chez les patients ayant une MRC au stade terminal (MRCT), et de la comparer à des sujets sains contrôles appariés en âge, sexe et niveau d'étude. Seront inclus des patients majeurs, hémodialysés chroniques, et des contrôles sains appariés en âge, sexe et niveau d'étude à un patient. Les évaluations seront réalisées en ambulatoire sur 3 jours : – caractéristiques démographiques et cliniques ; – réalisation de quatre tests psycho-cognitifs (MoCA, Échelle de Beck, test des portes et test de Stroop) et un auto-questionnaire de plaintes cognitives (Échelle de McNair) ; – évaluation de la perméabilité de la BHE : réalisation de la SPECT-CT cérébrale après injection de DTPA avec quantification de la radioactivité cérébrale du DTPA en aveugle du groupe (patients ou contrôles) et des autres paramètres recueillis, par le même médecin nucléaire ; – dosages sanguins : biochimie, dosage des toxines urémiques dont l'IS, activation d'AhR par le sérum, comptage de la radioactivité circulante. Notre hypothèse est que les patients atteints d'une MRC présenteraient une perméabilité accrue de la BHE par rapport à des contrôles sains appariés, et que cette perméabilité serait corrélée aux taux d'IS circulants (Fig. 1). Les patients en hémodialyse chronique (HDC) ne peuvent rester confinés (séances de dialyse), et sont à risque de formes graves de COVID-19 (comorbidités, immunodépression). Leurs soignants sont également exposés. Objectif principal : prévalence à M0 de la séroconversion SARS-CoV-2 dans une cohorte de patients HDC. Objectifs secondaires : (1) prévalence à M0 de la séroconversion chez les soignants en dialyse ; (2) proportion de COVID-19 asymptomatiques, et association avec les caractéristiques cliniques et comorbidités ; (3) prévalence de la séroconversion à M0 chez les participants ayant présenté une COVID-19 documentée ; (4) cinétique de la séroconversion et évolution de l'épidémie (2e test à M3) ; (5) valeur prédictive d'une séroconversion à M0 sur le risque de COVID-19 symptomatique à M6 ; (7) performances diagnostiques intrinsèques du test rapide (vs gold standard sérologique). Étude de cohorte multicentrique (NCT04420338), 4 centres de dialyse d'Aix-Marseille, tri à l'arrivée à chaque séance (signes cliniques de COVID-19) depuis début mars 2020 (arrivée de l'épidémie dans la région). Critères d'inclusion : âge ≥ 18 ans, patient HDC ou soignant de dialyse. Critères de non-inclusion : personne protégée. Déroulement : inclusion synchronisée des participants en juin 2020. Recueil des données cliniques et tests sérologiques rapides Biosynex à M0 et M3 (patients et soignants). Sérum prélevé à M0 et M3 (patients uniquement) pour la sérologie gold standard ultérieure (ELISA et séro-neutralisation). Suivi jusqu'à M6 (survenue d'une COVID-19 ?). Nombre de participants : 800 (561 patients, 239 soignants). Prévalence de la séroconversion à M0 chez les patients HDC et leurs soignants, prévalence des formes asymptomatiques, et association avec les caractéristiques cliniques et comorbidités. Peu de données sont disponibles sur le rapport bénéfice/risque de la biopsie rénale chez les sujets très âgés. Étude rétrospective monocentrique évaluant les indications de biopsie, le résultat et l'impact thérapeutique, et les complications de la biopsie rénale chez les patients de plus de 85 ans. Identification des patients ≥ 85 ans biopsiés, à partir de la base de données du laboratoire d'anatomie pathologique du CHU de Marseille (patients de Marseille, Aix-en-Provence, Avignon, Toulon, Martigues, Hyères). Recueil des données cliniques et biologiques dans les dossiers médicaux avec l'aide du néphrologue référent. Cent quatre patients biopsiés entre janvier 2010 et décembre 2018 (2 % des biopsies réalisées sur la même période) : âge médian 87 ans, IMC médian 25 kg/m2, 67 % HTA, 21 % diabète, 63 % comorbidités cardiovasculaires. Créatininémie médiane 262 umol/L (20 patients nécessitant la dialyse [19 %]). Biopsie pour : IRA chez 66 % des patients, IRA et syndrome néphrotique chez 22 %, syndrome néphrotique chez 7 %, protéinurie non néphrotique chez 5 %. Complications : hématome 8 %, 2 patients transfusés (1,9 %), dont un choc hémorragique avec embolisation (0,8 %). Diagnostics les plus fréquents : néphropathie hypertensive (28 %), néphrite interstitielle aiguë immunoallergique (16 %), glomérulonéphrite extra-capillaire (11 %), atteinte rénale d'une hémopathie (10 %), nécrose tubulaire aiguë (9 %), autres glomérulopathies non diabétiques (19 %). Cinquante-sept pour cent des biopsies ont montré une atteinte rénale accessible à un traitement, et 49,5 % des patients ont effectivement reçu un traitement spécifique (corticoïdes chez 42 %, cyclophosphamide chez 7 %, rituximab chez 7 %). Analyses de survie : résultats attendus pour la SFNDT. La biopsie rénale peut révéler une néphropathie accessible à un traitement spécifique chez les sujets très âgés. Introduction Le syndrome des antiphospholipides (SAPL) peut s’associer à plusieurs types de lésions rénales. L’atteinte microvasculaire est responsable de la néphropathie du SAPL (NSAPL). Le pronostic de la NSAPL, quand elle n’est pas associée à une glomérulonéphrite lupique, n’est pas connu. Notre objectif était de décrire le pronostic rénal, vasculaire et global des patients avec une NSAPL isolée. Patients/matériels et méthodes Étude rétrospective multicentrique de patients avec lésions histologiques de NSAPL sans autre néphropathie, associées à la présence d’anticorps antiphospholipides. Les patients ont été identifiés après appel à observations. Les survies sans insuffisance rénale chronique terminale (IRCT), sans thrombose et décès ont été étudiées ainsi que les paramètres associés à une variation du débit de filtration glomérulaire estimé selon MDRD (DFGe). Résultats Vingt-cinq patients ont été inclus. Au diagnostic de NSAPL, on note : 18 (72 %) femmes, âge médian : 38,8 (24,6–68,7) ans ; 17 (68 %) présentaient un SAPL selon les critères de classification de 2006 (12 thrombotiques, 2 obstétricaux et 3 thrombotiques et obstétricaux), 9 (36 %) un lupus ; 22 (88 %) patients sont porteurs d’un anticoagulant circulant, 14 (56 %) sont triple positifs ; pression artérielle : 132 (110–214)/79,5 (60–124) mmHg, DFGe médian : 47,8 mL/min/1,73 m2 (5,8–103), protéinurie médiane : 0,95 g/24 h (0,14–5) ; au plan histologique : microangiopathie thombotique glomérulaire et vasculaire chez 13 (52 %) et 9 (36 %) patients respectivement, hyperplasie intimale fibreuse chez 17 (68 %) et atrophie corticale focale chez 3 (12 %) patients. La fibrose interstitielle était modérée à sévère (≥ 25 %) chez 11 (44 %) patients. Après un suivi médian de 4,4 ans (0,4–38,3), 3 patients présentaient une IRCT. La survie rénale à 5 ans était de 82,3 % (IC95 % : 65,5 %–100 %). Deux autres patients présentaient un déclin de plus 30 % du DFGe. La fibrose interstitielle (≥ 25 %) était le seul facteur associé à la variation annuelle du DFGe (p = 0,06). La survie sans thrombose était de 96 % à 5 ans (IC95 % : 88,7 %–100 %), 74 % à 10 ans (IC95 % : 51 %–100 %). Une patiente était décédée au cours du suivi. La survie globale était de 100 % à 5 ans et de 90 % à 10 ans (IC95 % : 73 %–100 %). Conclusion Le pronostic rénal de la NSAPL isolée est sévère dans cette population jeune, près de 20 % des patients évoluant vers l’IRCT à 5 ans. L’importance des lésions de fibrose font penser que la biopsie rénale est tardive. Un diagnostic précoce pourrait améliorer le pronostic et permettre d’évaluer les traitements antiprolifératifs. Introduction Des études anciennes ont suggéré une association entre l’infection à plasmodium et la survenue de glomérulopathies décrites essentiellement sous la forme de GNMP. L’objectif de cette étude est de décrire le spectre des atteintes glomérulaires pouvant survenir au décours d’une crise de paludisme. Patients/matériels et méthodes L’ensemble des CHU de néphrologie ont été contactés afin de colliger rétrospectivement les observations des patients ayant présenté un syndrome glomérulaire au décours d’un accès palustre documenté par une PBR. Résultats Quinze patients (7 hommes et 8 femmes, âge moyen 47 ans) ont été identifiés. Dans 87 % des cas l’espèce identifiée était le Plasmodium falciparum (parasitémie moyenne à 5 %). Au cours de la crise de paludisme, tous les patients présentaient une insuffisance rénale qui était considérée comme aiguë dans 12 cas (7 ont eu recours à l’hémodialyse). Au moment de la PBR (délai moyen de 26 jours après la crise de paludisme), 13 patients avaient un syndrome néphrotique (SN) avec une protéinurie moyenne de 900 mg/mmol et une albuminémie moyenne à 20,4 g/L. L’étude anatomopathologique rénale a mis en évidence chez 6 patients une néphropathie liée au VIH, une forme collapsante d’HSF sans infection par le VIH dans 6 cas, des lésions d’HSF dans 2 cas et un SNLGM chez le dernier patient. Les patients VIH positifs avaient tous sauf un une charge virale élevée (valeur moyenne de 560 000 copies/mL). Tous les patients testés (2 HIVAN et 4 HSF collapsante) présentaient des variants du gène APOL1 (4 homozygotes G1/G1 et 2 hétérozygotes composites G1/G2). Sept patients ont eu un traitement spécifique par corticoïdes (6 HSF collapsantes 2 HSF et 1 HIVAN) permettant une rémission partielle du SN dans 3 cas. Au terme du suivi moyen de 21 mois, 5 patients présentaient une IR terminale. Chez les 10 patients sans traitement de suppléance le DFG moyen était de 48 mL/min/1,73 m2. Conclusion Après une crise de paludisme, les atteintes glomérulaires les plus fréquemment observées sont la révélation d’une néphropathie liée au VIH ou une HSF collapsante. La présence dans les 6 cas testés de variant pathogène du gène APOL1 nous laisse penser que sur un terrain génétiquement prédisposé, une infection par le paludisme peut provoquer des lésions podocytaires et/ou des cellules épithéliales pariétales. Une étude de la localisation rénale de la protéine APOL1 chez ces patients est en cours ainsi que la recherche du parasite dans le rein de ces patients. [ABSTRACT FROM AUTHOR]

Introduction La leucémie myélomonocytaire chronique (LMMC) est une maladie rare caractérisée par une monocytose persistante associée à des phénomènes myéloprolifératifs et myélodysplasiques, qui atteint principalement les personnes âgées et présente un pronostic sombre. Comme dans la plupart des pathologies hématologiques, celle-ci peut se compliquer d’atteintes rénales de plusieurs types. Si l’infiltration parenchymateuse et le syndrome de lyse sont bien connus, il existe une atteinte tubulaire secondaire à l’accumulation de lysozyme, rare et probablement sous diagnostiquée. Observation Un patient âgé de 82 ans, présentant pour principal antécédent une hypertension artérielle ainsi qu’un myélome multiple stade 1 est adressé en néphrologie devant une insuffisance rénale d’allure chronique dont le bilan reviendra négatif. Le suivi sera marqué par la découverte d’une monocytose avec un diagnostic confirmé de LMMC-1 de bas grade avec surveillance simple. Devant l’apparition simultanée d’une protéinurie sans chaîne légère ni albumine et la progression de l’insuffisance rénale, le patient sera biopsié, posant le diagnostic de néphropathie au lysozyme. Cette atteinte rénale signe la progression de la maladie et la nécessité de mise en place d’un traitement sans en avoir les critères hématologiques. Discussion La néphropathie au lysozyme est une pathologie peu décrite, marquée par une accumulation de lysozyme cytoplasmique au niveau des cellules tubulaires. Le lysozyme, protéine contenue particulièrement dans les granulocytes, est normalement réabsorbé au niveau tubulaire puis dégradé par les phagolysosomes. Lorsque son taux s’élève, les capacités d’exocytose peuvent être dépassées et aboutir à une accumulation, responsable alors d’un tableau de tubulopathie. Le taux de lysozyme sanguin aurait une corrélation positive avec l’atteinte rénale. Marquant par ailleurs une progression de la maladie hématologique, cette pathologie semble de mauvais pronostic. Conclusion La néphropathie au lysozyme est une pathologie rénale peu décrite car probablement insuffisamment recherchée. La LMMC touche des patients âgés et fragiles, comme notre patient, présentant de multiples causes possibles d’insuffisance rénale chronique, faisant ainsi souvent discuter la réalisation d’une biopsie rénale à risque. Le dosage du lysozyme sanguin pourrait être un marqueur intéressant en faveur du diagnostic de néphropathie au lysozyme et de progression de la maladie. [ABSTRACT FROM AUTHOR]

Introduction La maladie des anticorps anti-membrane basale glomérulaire (anti-MBG) ou maladie de Goodpasture est très rare (incidence de 1/1 million). Les patients présentent une insuffisance rénale rapidement progressive, nécessitant souvent la dialyse d’emblée. Le devenir des patients qui vont évoluer vers l’IRCT malgré le traitement immunosuppresseur, en termes de survie et d’accès à la greffe, est mal connu. Cette étude repose sur les données du registre REIN. Patients/Matériels et méthodes Tous les patients inscrits dans le registre REIN entre 2002 et décembre 2015 ont été inclus dans cette étude rétrospective. Les données renseignées à l’initiation du traitement de suppléance, la survie et l’accès à la transplantation rénale ont été analysés. Les patients anti-MBG ont été comparés à 2 populations contrôles : les patients porteurs d’une vascularite à ANCA (AAV), et un groupe de patients mis en dialyse sur la même période pour d’autres néphropathies sélectionnées au hasard. Observation/Résultats Deux-cent cinquante-sept patients anti-MBG ont débuté la dialyse durant cette période en France : 123 femmes et 134 hommes. L’âge médian était de 61 ans, avec 2 pics (20–44 ans et > 65 ans). La dialyse a été débutée en urgence chez 184 (71 %) patients, et 95 % ont débuté en hémodialyse. Les patients anti-MBG étaient plus jeunes et avaient moins de comorbidités que les 716 patients AAV ayant débuté la dialyse durant la même période. Chez les patients anti-MBG, 62 patients sont décédés : 11 (17 %) de cause cardiovasculaire et 8 (13 %) de cause infectieuse. Leur survie à 1, 5 et 10 ans était de 90 %, 73 % et 66 %. Le seul facteur de risque de mortalité dans cette population était l’âge de plus de 75 ans. Comparés aux patients AAV et aux 1000 patients sélectionnés sur la cohorte, la maladie à anti-MBG était un facteur protecteur de survie en analyse multivariée. L’accès à la transplantation était meilleur chez les patients anti-MBG que chez les patients AAV : 52 % des patients anti-MBG ont été inscrits sur liste et 40 % ont bénéficié d’une transplantation rénale. Discussion/Conclusion Nous montrons que les patients porteurs d’une maladie à anti-MBG ont un pronostic favorable en dialyse chronique en France, avec un accès correct à la transplantation rénale, comme cela a été rapporté dans le registre ANZDATA. La survie en dialyse des patients anti-MBG est en outre meilleure que celle des patients AAV et des autres patients dialysés, probablement du fait d’une moindre fréquence des comorbidités. Introduction L’estimation de la volémie chez le patient dialysé chronique constitue un point clé de sa prise en charge. À l’heure actuelle, il n’existe pas de gold standard réalisable en routine quotidienne malgré les outils de plus en plus nombreux comme la bioimpédancemétrie et l’échographie cardiaque trans-thoracique (ETT) et pulmonaire. Nous avons réalisé une étude prospective chez des patients hémodialysés chroniques dont le but est d’évaluer les performances diagnostiques de l’échographie pulmonaire et de l’examen clinique pour l’évaluation de la surcharge volémique en utilisant comme gold standard l’ETT. Patients et méthodes Trente et un patients hémodialysés chroniques depuis plus de 3 mois en centre lourd ont été inclus. Les patients présentant une fibrose pulmonaire n’ont pas été inclus. L’évaluation volémique a été réalisée au début de la deuxième séance de dialyse hebdomadaire par : un score clinique (Critères majeurs : dyspnée NYHA > III, orthopnée ; Critères mineurs : turgescence jugulaire (TJ), reflux hépato-jugulaire (RHJ), crépitants à l’auscultation, œdèmes périphériques et HTA pré-dialyse) ; un score en ETT (diamètre de la veine cave inférieure, rapport E/E’ et les pressions artérielles pulmonaires systoliques) ; et l’Echo-Comet score en échographie pulmonaire (nombre de lignes B antérieures et latérales sur 28 régions thoraciques). Résultats Cinq patients présentaient une surcharge à l’ETT. En comparaison à l’ETT, les performances diagnostiques du score de surcharge clinique présentent une sensibilité (Se) de 100 %, une spécificité (Sp) de 81 %, une valeur prédictive positive (VPP) de 50 % et une valeur prédictive négative (VPN) : 100 %. Seulement l’orthopnée (p = 0,008), la TJ (p = 0,005) et le RHJ (p = 0,008) étaient significativement associés au diagnostic de surcharge à l’ETT. Les performances diagnostiques de l’Echo-Comet score par échographie pulmonaire présentent une Se de 80 %, une Sp de 59 %, une VPP 26 % et un VPN 94 %. 10 patients (32,3 %) présentaient une augmentation de l’eau extra-pulmonaire pulmonaire sans signe de surcharge à l’ETT et à l’examen clinique. Conclusion Il s’agit de la première étude qui définit un score clinique pour évaluer la surcharge hydrosodée chez les patients hémodialysés chroniques. Ce score clinique présente des performances diagnostiques satisfaisantes en comparaison à l’ETT et serait facilement utilisable en routine clinique quotidienne pour ajuster le poids sec. L’évaluation de la surcharge par l’Echo-Comet score en échographie pulmonaire paraît quant à elle mal corrélée à la surcharge évaluée par ETT. En revanche, la présence d’eau extrapulmonaire non détectée par l’examen clinique et l’ETT reste un paramètre à investiguer d’un point de vue diagnostique et pronostique chez les patients hémodialysés. Introduction Le diagnostic et la prise en charge du syndrome néphrotique primitif (SNI ; LGM/HSF) de l’enfant et de l’adulte jeune sont bien codifiés. L’application du même protocole thérapeutique chez le patient âgé pourrait s’accompagner d’une moins bonne réponse au traitement et d’un surcroît de complications métaboliques, cardiovasculaires et rénales. Patients/Matériels et méthodes Étude rétrospective multicentrique (11 centres) ayant inclus des patients présentant un SNI à début tardif (≥ 60 ans). Observation/Résultats Parmi les 124 patients inclus (61 % d’homme ; âge médian 69 ans [64 ; 72]), 87 présentaient un aspect histologique de LGM (70 %) et 37 un aspect d’HSF (30 %). Le DFG estimé au diagnostic était de 50 mL/min/1,73 m2. Il coexistait une hématurie microscopique chez 88 patients (46 %) et une HTA chez 55 patients (73 %). Le traitement de 1re ligne était : une corticothérapie orale aux doses usuelles (n = 110 ; 88 %), associée à du MMF ou des anticalcineurines chez 7 et 2 patients, respectivement ; une anticalcineurine (n = 3 ; 2,5 %) ou du rituximab en monothérapie (n = 1, 0,8 %) ; des immunoadsorptions (n = 1, 0,8 %). Une réponse a été observée chez 99 patients (81 % ; complète [RC] n = 79 [65 %], partielle n = 20 [16 %]). Le délai médian de réponse était de 60 jours [30–120]. Parmi les 99 répondeurs, 47 (47 %) ont présenté une rechute du syndrome glomérulaire après un délai médian de 11 mois [6–24], majoritairement sous la forme d’un syndrome néphrotique (60 %) (nombre moyen de rechute : 1,07 par patient). Un traitement d’épargne cortisonique a été proposé dès la 1re rechute chez 23 patients (48,9 %). Au cours du suivi (médiane 30 mois [10,6–59,6]), 25 patients ont bénéficié d’un traitement par rituximab dont 16 ont été mis en RC. Les principales complications étaient des infections nécessitant une hospitalisation (22 %), l’apparition ou l’aggravation d’un diabète (26 %) et l’apparition d’un cancer (14 % ; délai médian 32 mois). A la fin du suivi, 66,4 % des patients étaient vivants et en RC (DFGe médian 62,5 μmol/L/1,73 m2 [40–79]), six patients (4,8 %) étaient en dialyse chronique, et huit patients étaient décédés (7,3 %). Discussion Comparativement aux formes pédiatriques, l’obtention d’une RC est moins fréquente chez les patients débutant un SNI après 60 ans. Les complications infectieuses et cardiovasculaires sont fréquentes, et la mortalité non négligeable, suggérant qu’une épargne cortisonique pourrait permettre d’améliorer le pronostic de ces patients. Conclusion La place du rituximab (seul ou en association à une corticothérapie brève) à visée d’épargne cortisonique doit être particulièrement précisée dans cette population fragile. L'allopurinol, largement utilisé dans le traitement de l'hyperuricémie et de la goutte, est pourvoyeur de réactions cutanées sévères pouvant aller jusqu'au syndrome de Stevens-Johnson et la nécrolyse épidermique toxique, ainsi que de réactions systémiques telles que le DRESS (Drug Reaction with Eosinophilia and Systemic Symptoms). L'allèle HLA-B*5801 est connu comme facteur de risque de manifestations cutanées sévères d'hypersensibilité à l'allopurinol, essentiellement dans les populations asiatiques. Nous rapportons l'observation d'un homme, d'origine chinoise, âgé de 47 ans, sans antécédent, porteur de l'allèle HLA-B*5801, qui a développé une vascularite rénale nécrosante d'hypersensibilité à l'allopurinol isolée, sans manifestation cutanée. La recherche de cet allèle doit être proposée avant la prescription d'allopurinol chez les patients originaires de certaines régions d'Asie, et l'imputabilité de l'allopurinol doit être évoquée en cas de vascularite nécrosante rénale, même sans atteinte cutanée associée. We report the observation of a 47-year-old Chinese patient, with no previous medical history, carrying the HLA-B*5801 allele, who developed an isolated allopurinol hypersensitivity necrotizing renal vasculitis without cutaneous manifestations.. With “checkpoint inhibitors” targeting PD1/PD-1-ligands or CTLA-4/CD28 pathways, immunotherapy has profoundly modified therapeutic strategies in oncology. First approved in refractory metastatic neoplasms (melanoma and lung adenocarcinoma), it is now being tested broadly in other cancers and/or as adjuvant treatment. For a significant proportion of patients, immunotherapy is responsible for “immunological” events, identified as Immune-Related Adverse Events (irAEs). Owing to the increasing number of prescriptions, identification and management of specific immunological side effects is crucial and requires close collaboration between oncologists and internists and/or other organ specialists. Within irAEs, we propose to individualize the induced autoimmunity by the term “Opportunistic Autoimmunity Secondary to Cancer Immunotherapy” (OASI). The aims of this article are (1) to present the different available checkpoint inhibitors and the OASIs reported with these treatments and (2) to propose practical recommendations for diagnosis, pre-therapeutic assessment and management of OASIs. Cyst infection is a common complication of autosomal dominant polycystic kidney disease (ADPKD). Diagnosis is challenging with standard imaging techniques. We aimed to evaluate the diagnostic performance of [18F]fluorodeoxyglucose positron emission tomography–computed tomography (18-FDG PET–CT) for the diagnosis of cyst infections among ADPKD patients, in comparison with computed tomography (CT) and magnetic resonance imaging (MRI). All APKD patients who underwent 18-FDG PET–CT for suspected cyst infection between 2006 and 2013 in a French teaching hospital were included. Diagnosis of cyst infection was retained a posteriori on an index of clinical suspicion. 18-FDG PET–CT findings were was considered to be positive in cases of cyst wall hypermetabolism. CT or MRI findings were were considered to be positive in cases of cyst wall thickening (and enhancement if contrast medium was injected) and infiltration of the adjacent fat. A control group of ADPKD patients with 18-FDG PET–CT performed for other reasons was included. Thirty-two 18-FDG PET–CT scans were performed in 24 ADPKD patients with suspected cyst infection. A diagnosis of cyst infection was retained in 18 of 32 cases: 14 with positive 18-FDG PET–CT findings, and four false negatives. There were no false positives and no hypermetabolism of cyst walls in nine ADPKD control patients. 18-FDG PET–CT had a sensitivity of 77%, a specificity of 100%, and a negative predictive value of 77%. 18-FDG PET–CT allowed a differential diagnosis in three patients. In contrast, CT had a sensitivity of 7% and a negative predictive value of 35% (p <0.001 vs. 18-FDG PET–CT). Only eight MRI scans were performed. The diagnostic performance of 18-FDG PET–CT is superior to that of CT in cyst infections, for comparable radiation doses and with no injection of nephrotoxic contrast medium, in ADPKD patients. Il reste controversé de classifier les vascularites associées aux ANCA selon la spécificité des ANCA ou selon le diagnostic phénotypique et de tenir compte de la spécificité des ANCA pour adapter le traitement. Une large cohorte française de patients atteints de granulomatose avec polyangéite (GPA, Wegener) a été analysée pour évaluer si l'absence d'ANCA ou la présence d'ANCA dirigés contre la myéloperoxydase (MPO) permettait de définir des GPA différentes de celles avec ANCA dirigés contre la protéinase 3 (PR3). Les données des patients du registre du Groupe français d'étude des vascularites (GFEV), avec un nouveau diagnostic de GPA selon les critères de l'ACR et/ou de la Conférence de Consensus révisée de Chapel Hill (CHCC), ont été analysées selon les résultats de l'immuno-essai de leur ANCA. Les caractéristiques démographiques, les manifestations de GPA au diagnostic et les données de suivi à long terme ont été extraites de la base de données. Les patients sans ANCA ou avec ANCA anti-MPO ont été comparés aux patients avec ANCA anti-PR3. Parmi les 727 patients avec GPA, 62 (8,5 %) avec un résultat négatif des ANCA et 119 (16,4 %) avec des ANCA anti-MPO ont été comparés à 546 (75,1 %) avec ANCA anti-PR3. Leur suivi médian global était de 3,5 ans. Les patients sans ANCA étaient caractérisés par une vascularite plus limitée que ceux avec ANCA anti-PR3 ou anti-MPO, avec davantage de forme limitée de GPA (17,7 % vs 5,7 % ou 6,7 % ; p = 0,001 et p = 0,04), moins souvent d'atteinte rénale (35,5 % vs 57,5 % ou 65,5 % ; p = 0,002 et p < 0,001) ou de neuropathie périphérique (8,1 % vs 20,7 % ou 21,0 % ; p = 0,03 et p = 0,04) et des indices d'activité BVAS plus faibles au diagnostic (13,3 vs 17,8 ou 18,2 ; p < 0,001 et p = 0,001) ; par contre, leurs taux de survie sans rechute (p = 0,14 et p = 0,93) et de survie globale (p = 0,55 et p = 0,16) étaient comparables. Les patients avec ANCA anti-MPO par rapport à ceux avec ANCA anti-PR3 étaient plus souvent de sexe féminin (52,9 % vs 41,8 %, p = 0,03), plus âgés (59,8 vs 51,6 ans ; p < 0,001), avec moins souvent d'arthralgies (34,5 % vs 56,4 % ; p < 0,001), de purpura (8,4 % vs 17,9 % ; p = 0,02) ou d'atteinte oculaire (18,5 % vs 28,2 % ; p = 0,04) ; le taux de survie sans rechute des patients avec ANCA anti-MPO n'était pas supérieur (p = 0,12) mais leur taux de survie globale était significativement inférieur (p = 0,001). Par comparaison aux patients sans ANCA et à ceux avec ANCA anti-MPO, les patients avec ANCA anti-PR3 avaient un moindre taux de survie sans rechute (p = 0,04) mais un meilleur taux de survie globale (p < 0,01). À partir d'une des plus larges cohortes rapportées à ce jour de patients avec GPA sans ANCA et avec ANCA anti-MPO, ceux sans ANCA avaient plus volontiers une vascularite limitée mais des taux de survie comparables. Les patients avec ANCA anti-PR3 avaient une probabilité de rechute supérieure mais un taux de survie plus élevé. Même si la CHCC considère le diagnostic phénotypique de GPA comme étant l'élément majeur, ces données renforcent l'idée que la spécificité des ANCA doit être ajoutée au phénotype et prise en compte dans la discussion et l'analyse des stratégies thérapeutiques. Les vascularites associées aux ANCA (VAA) sont une cause fréquente de glomérulonéphrite rapidement progressive (GNRP). Au décours d'une poussée, le pronostic est dominé par le risque d'évolution vers une insuffisance rénale terminale (IRT) ou le décès. L'étude prospective multicentrique PEXIVAS, incluant des patients en poussée sévère de VAA avec hémorragie alvéolaire et/ou insuffisance rénale (avec un DFGe < 50 mL/min/1,73 m2), a montré que les échanges plasmatiques (EP) ne réduisaient pas la survenue d'IRT et/ou de décès. Cette étude n'a cependant pas évalué l'impact pronostique de la biopsie rénale sur le devenir de la fonction rénale. Notre objectif était d'évaluer si les lésions histologiques rénales pouvaient prédire le devenir de la fonction rénale chez les patients avec VAA traités par EP. Nous avons inclus dans cette étude rétrospective multicentrique des patients avec granulomatose avec polyangéite (GPA), polyangéite microscopique (PAM) ou vascularite limitée au rein, traités par EP pour une GNRP et ayant eu une biopsie rénale initiale. Deux classifications histologiques ont été utilisées : celle de Berden distinguant 4 classes (focale, à croissants, mixte, scléreuse), et celle de Brix distinguant 3 groupes à risque (faible, intermédiaire, élevé) et utilisant le pourcentage de glomérules normaux, le pourcentage de fibrose interstitielle et atrophie tubulaire et le DFGe initial. Le critère de jugement principal était l'indépendance de la dialyse et la survie à 12 mois. Les critères secondaires étaient un DFGe > 30 mL/min/1,73 m2 à 12 mois et un gain de DFGe > 15 mL/min/1,73 m2 à 12 mois par rapport au début de la poussée. Nous avons inclus 163 patients : 99 (60 %) hommes, 92 (56 %) avec ANCA-MPO, 65 (40 %) avec ANCA-PR3 ; créatininémie initiale 555 ± 274 μmol/L, 62 (38 %) avec hémorragie alvéolaire ; 139 (85 %) ont reçu du cyclophosphamide et 30 (18 %) du rituximab ; 74 (45 %) ont nécessité initialement le recours à l'épuration extra-rénale. Le nombre moyen d'EP était de 7,0 ± 2,3. Les classes de Berden étaient focales dans 19 %, à croissants dans 39 %, mixtes dans 20 % et scléreuses dans 21 %. Selon la classification de Brix, 11 % des patients avait un risque bas, 48 % un risque intermédiaire et 41 % un risque élevé. Les classifications de Berden et de Brix étaient toutes les deux fortement associées à la survenue du critère de jugement principal : indépendance de la dialyse et survie à 12 mois (respectivement c224,9, p < 0,0001, et c211,7, p = 0,003). De plus, la créatininémie initiale n'était pas différente dans les deux groupes (526 ± 252 μmol/L dans le groupe avec bon pronostic vs 597 ± 300 μmol/L dans le groupe avec mauvais pronostic, p = 0,11). Concernant les critères secondaires : un DFGe > 30 mL/min/1,73 m2 à 12 mois était associé à la classification de Berden (c214,0, p = 0,003) et de Brix (c217,3, p = 0,0002) ; et un gain de DFGe > 15 mL/min/1,73 m2 était également associé avec la classification de Berden (c218,1, p = 0,0004) et de Brix (c217,7, p = 0,0001). Dans cette cohorte de patients traitées par EP pour poussée rénale de VAA, nos résultats suggèrent que les résultats de la biopsie rénale initiale sont fortement associés au devenir de la fonction rénale à 12 mois. La biopsie rénale doit être réalisée chez les patients avec poussée rénale de VAA pour aider les cliniciens à identifier les patients qui pourraient bénéficier le plus des EP. [ABSTRACT FROM AUTHOR]

Le rituximab a démontré sa supériorité sur l'azathioprine dans le traitement de maintenance des vascularites associées aux ANCA (VAA) [1]. L'essai MAINRITSAN2 [2] a été conçu pour comparer un schéma de perfusion de rituximab « à la demande » au schéma de 5 perfusions de rituximab à dates fixes. Dans le bras « à la demande », les patients ont reçu une perfusion de 500 mg de rituximab lors de la randomisation, avec réinjection uniquement après la réapparition des lymphocytes CD19 ou des ANCA, ou une augmentation marquée du titre des ANCA, et ce tous les 3 mois jusqu'au 18e mois. Dans le bras « systématique », les patients ont reçu 500 mg de rituximab lors de la randomisation, puis à j14 et aux mois 6, 12 et 18. Les taux de rechute ne différaient pas significativement entre les 2 groupes à 28 mois. L'objectif de cette étude post-hoc était d'évaluer l'effet de l'abandon de la perfusion de 500 mg de rituximab à j14 sur les taux de rechutes précoces. Une analyse post-hoc des données de l'essai MAINRITSAN 2 a été réalisée. Les critères d'évaluation principaux étaient le taux de survie sans rechute aux mois 3, 6, 9 et 12 dans chaque bras de l'essai MAINRITSAN2. Les rechutes ont été définies comme une réapparition ou une aggravation des symptômes du VAA, c'est-à-dire un BVAS > 0. Nous avons effectué des analyses exploratoires en sous-groupes en fonction du traitement d'induction (cyclophosphamide ou rituximab) et du type de maladie (première poussée de VAA ou rechute). L'évolution des lymphocytes CD19 et des ANCA ont été analysées. Parmi les 161 patients inclus dans l'étude, le nombre de patients vivants sans rechute était 80/81 (99 %) à 3 mois, 78/81 (96 %) à 6 mois, 76/81 (94 %) à 9 mois et 76/81 (94 %) à 12 mois pour les patients recevant des perfusions de rituximab à j0 et j14 et 78/80 (98 %) à 3 mois, 75/80 (95 %) à 6 mois, 73/80 (91 %) à 9 mois, 72/80 (90 %) à 12 mois pour les patients recevant une seule perfusion à j0. Il n'y avait pas de différence entre les groupes. Que les patients aient reçu du cyclophosphamide ou du rituximab comme traitement d'induction ou le statut rechuteur ou non des patients n'ont pas modifié ces résultats. La déplétion lymphocytaire B était similaire dans les 2 bras. La suppression de la dose de 500 mg au jour 14 du traitement d'entretien de rémission n'a pas modifié le taux de survie sans récidive précoce, ce qui peut suggérer que cette perfusion puisse être supprimée. Because Systemic Lupus Erythematosus (SLE) is a rare disease, and due to the significant prognostic impact of early management, a diagnosis confirmed by a physician with experience in SLE is recommended, for example from an expert center. Once the diagnosis is confirmed, existing manifestations should be identified in particular, renal involvement by an assessment of proteinuria, disease activity and severity should be determined, potential complications anticipated, associated diseases searched for, and the patient's socioprofessional and family context noted. Therapeutic management of SLE includes patient education on recognizing symptoms, understanding disease progression as well as when they should seek medical advice. Patients are informed about routine checkups, treatment side effects, and the need for regular vaccinations, especially if they are receiving immunosuppressive treatment. They are also advised on lifestyle factors such as the risks of smoking, sun exposure, and dietary adjustments, especially when they are receiving corticosteroids. The importance of contraception, particularly when teratogenic medications are being used, and regular cancer screening are emphasized. Support networks can help relieve a patient's isolation. The first-line medical treatment of SLE is hydroxychloroquine (HCQ), possibly combined with an immunosuppressant and/or low-dose corticosteroid therapy. The treatment of flares depends on their severity, and typically involves HCQ and NSAIDs, but may be escalated to corticosteroid therapy with immunosuppressants or biologic therapies in moderate to severe cases. Because there is no curative treatment, the goals of therapy are patient comfort, preventing progression and flares, and preserving overall long-term health and fertility. The frequency of follow-up visits depends on disease severity and any new symptoms. Regular specialized assessments are necessary, especially when treatment changes, but a frequency of every 3 to 6 months is recommended during periods of remission and monthly during active or severe disease, especially in children. Objectives: Discontinuation or continuation of maintenance immunosuppressive therapy (MIST) after a severe lupus nephritis (LN) requires measuring the risk of relapse but reliable clinical and biological markers are lacking. The WIN-IgE study assesses the value of serum anti-dsDNA IgE autoantibodies as a biomarker for the prediction of relapse in severe LN., Methods: WIN-IgE is an ancillary study of the WIN-Lupus study (NCT01284725), a prospective controlled clinical trial which evaluated the discontinuation of MIST after 2-3 years in class III or IV±V LN with active lesions. WIN-IgE included all patients with available serum collected at randomisation for continuation or discontinuation of MIST. In these sera, anti-dsDNA antibodies, IgE and IgG, were quantified by ELISA and compared between patients who experienced LN relapse and those who did not during the 24 months of follow-up., Results: 52 patients were included, 25 in the MIST continuation group and 27 in the MIST discontinuation group, 12 experienced a biopsy-proven relapse of LN. Initial anti-dsDNA IgE antibodies levels were higher in patients with subsequent LN relapse. Anti-dsDNA IgG was not associated with relapse. Survival without LN relapse was lower in patients with anti-dsDNA IgE levels above vs below a threshold of 1.9 arbitrary units (p=0.019), particularly in the subgroup of patients randomised to discontinue MIST (p=0.002). Introduction: Apheresis allows the fast removal of autoantibodies in anti-glomerular basement membrane (anti-GBM) disease, and in severe antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis. The CINEVAS study tested whether immunoadsorption (IA) allowed a faster removal of ANCA and/or anti-GBM antibodies than plasma exchanges (PEx)., Methods: CINEVAS was a prospective multicenter study comparing IA to PEx in consecutive patients with ANCA and/or anti-GBM vasculitides. The primary objective was the reduction rate in autoantibody titers between the beginning of the first and the end of the seventh apheresis session. Secondary objectives were number of sessions needed to obtain desired reduction rates; reduction rates of total Ig levels; tolerance of sessions; and patients' outcome., Results: The results of 38 patients (16 treated with IA and 22 with PEx), and 43 autoantibodies, were analyzed. There was no difference in the reduction rates in autoantibody titers between IA and PEx over 7 sessions (respectively 98% vs. 96%, P  = 0.39). The numbers of sessions needed to obtain undetectable autoantibodies, or 50%, 75%, or 90% reductions, did not differ between techniques. Greater reduction rates of autoantibodies were observed when plasma was separated by filtration compared to centrifugation, with IA and PEx. IA allowed a greater reduction in total IgG levels, and better preservation of total IgA and IgM levels than PEx. PEx sessions required higher volumes of plasma, IA sessions higher volumes of citrate; IA sessions were longer., Conclusions: IA and PEx were comparable in ANCA or anti-GBM removal kinetics, despite a faster reduction in total IgG with IA., (© 2024 International Society of Nephrology. Introduction: The objective of this study was to describe kidney involvement in patients with myelodysplastic syndromes (MDS), their treatments, and outcomes., Methods: We conducted a multicenter retrospective study in seven centers, identifying MDS patients with acute kidney injury (AKI), chronic kidney disease (CKD), and urine abnormalities., Results: Fifteen patients developed a kidney disease 3 months after MDS diagnosis. Median urine protein-to-creatinine ratio was 1.9 g/g, and median serum creatinine was 3.2 mg/dL. Ten patients had AKI at presentation, and 12 had extra-renal symptoms. The renal diagnoses included anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV), ANCA negative vasculitis, C3 glomerulonephritis, immune complex-mediated glomerulonephritis, polyarteritis nodosa, and IgA vasculitis. All patients but one received a specific treatment for the MDS-associated kidney injury. The effect of MDS treatment on kidney injury could be assessed in six patients treated with azacitidine, and renal function evolution was heterogenous. After a median follow-up of 14 months, four patients had CKD stage 3, five had CKD stage 4, and three had end stage kidney disease. On the other hand, three evolved to an acute myeloid leukemia and three died. Compared to 84 MDS controls, patients who had kidney involvement were younger, had a higher number of dysplasia lineages, and were more eligible to receive hypomethylating agents, but no survival difference was seen between the two groups. In the field of complex autoimmune diseases such as systemic lupus erythematosus (SLE), systems immunology approaches have proven invaluable in translational research settings. Large-scale datasets of transcriptome profiling have been collected and made available to the research community in public repositories, but remain poorly accessible and usable by mainstream researchers. Enabling tools and technologies facilitating investigators' interaction with large-scale datasets such as user-friendly web applications could promote data reuse and foster knowledge discovery. Microarray blood transcriptomic data from the LUPUCE cohort (publicly available on Gene Expression Omnibus, GSE49454), which comprised 157 samples from 62 adult SLE patients, were analyzed with the third-generation (BloodGen3) module repertoire framework, which comprises modules and module aggregates. These well-characterized samples corresponded to different levels of disease activity, different types of flares (including biopsy-proven lupus nephritis), different auto-antibody profiles and different levels of interferon signatures. A web application was deployed to present the aggregate-level, module-level and gene-level analysis results from LUPUCE dataset. Users can explore the similarities and heterogeneity of SLE samples, navigate through different levels of analysis, test hypotheses and generate custom fingerprint grids and heatmaps, which may be used in reports or manuscripts. Introduction: During COVID-19, renal impairment is associated with poor prognosis in intensive care unit (ICU). We aimed to assess the existence and incidence of early renal dysfunction and its prognostic value in patients with COVID-19-related acute respiratory distress syndrome (ARDS)., Methods: In this prospective multicenter study, patients aged over 18 years with invasive mechanical ventilation (MV) for ARDS were enrolled in 3 ICUs. Precise evaluation of renal dysfunction markers, including urinary protein electrophoresis (UPE) and quantification, was performed within 24 hours after MV onset., Results: From March 2020 to December 2021, 135 patients were enrolled as follows: 100 with COVID-19 ARDS and 35 with non-COVID-19 ARDS. UPE found more tubular dysfunction in patients with COVID-19 (68% vs. 21.4%, P  < 0.0001) and more normal profiles in patients without COVID-19 (65.0% vs. 11.2%, P  = 0.0003). Patients with COVID-19 significantly displayed early urinary leakage of tubular proteins such as beta-2-microglobulin (ß2m) and free light chains, tended to display acute kidney injury (AKI) more frequently (51.0% vs. 34.3%, P  = 0.088), had longer MV (20 vs. 9 days, P  < 0.0001) and longer ICU stay (26 vs. 15 days, P  < 0.0001). In COVID-19 ARDS, leakage of free lambda light chain was associated with the onset of Kidney Disease: Improving Global Outcomes (KDIGO) ≥2 AKI (odds ratio [OR]: 1.014, 95% confidence interval [CI] 1.003-1.025, P  = 0.011)., Conclusion: Patients with COVID-19-related ARDS display a proximal tubular dysfunction before the onset of AKI, which predicts AKI. Proximal tubular damage seems an important mechanism of COVID-19-induced nephropathy. Objectives: Lupus activity has long been considered to decline after initiation of maintenance dialysis (MD). This assumption is based on limited historical data. We aimed to describe the natural history of lupus in patients undergoing MD., Methods: We assembled a national retrospective cohort of lupus patients who started dialysis between 2008 and 2011, included in the REIN registry with a 5-year follow-up. We analysed healthcare consumption from the National Health Data System. We evaluated the proportion of patients 'off-treatment' (i.e. receiving 0-5 mg/d of corticosteroids, without any immunosuppressive therapy) after the start of MD. We describe the cumulative incidences of non-severe and severe lupus flares, cardiovascular events, severe infections, kidney transplantation and survival., Results: We included 137 patients (121 females and 16 males), with a median age of 42 years. The proportion of patients 'off-treatment' at dialysis initiation was 67.7% (95% CI: 61.8, 73.8%), and increased to 76.0% (95% CI: 73.3, 78.8) at 1 year and 83.4% (95% CI: 81.0, 85.9%) at 3 years, with a lower proportion in younger patients. Lupus flares mainly occurred in the first year after MD initiation, and at 12 months 51.6% of patients had presented a non-severe lupus flare and 11.6% a severe lupus flare. In addition, 42.2% (95% CI: 32.9, 50.3%) and 23.7% (95% CI: 16.0, 30.7%) of patients at 12 months had been hospitalized for cardiovascular events or infections, respectively., Conclusion: The proportion of lupus patients off-treatment increases after MD initiation, but non-severe and severe lupus flares continue to occur, mainly during the first year. Kidney Biopsy (KB) is a crucial diagnostic tool in the field of renal diseases and is routinely performed in nephrology departments. A previous survey conducted by the Société Francophone de Néphrologie Dialyse Transplantation (SFNDT) revealed significant disparities in clinical practices, sometimes conflicting with the existing literature and recently published recommendations. In response, the SFNDT wished to promote the development of best practice guidelines, under the auspices of the French National Authority for Health (HAS), to establish a standardized framework for performing kidney biopsies in France.

Objective: To compare the long-term efficacy and safety of azathioprine (AZA), 18-month fixed-schedule rituximab (RTX), 18-month tailored RTX and 36-month RTX in preventing relapses in patients with antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis who achieved a complete remission after induction therapy. Patients treated with 36-month RTX received either a fixed or a tailored regimen for the first 18 months and a fixed regimen for the last 18 months (36-month fixed/fixed RTX and 36-month tailored/fixed RTX, respectively)., Methods: The Maintenance of Remission using Rituximab in Systemic ANCA-associated Vasculitis (MAINRITSAN) trials sequentially compared: 18-month fixed-schedule RTX versus AZA (MAINRITSAN); 18-month fixed-schedule RTX versus 18-month tailored-RTX (MAINRITSAN2); and extended therapy to 36 months with four additional RTX infusions after MAINRITSAN2 versus placebo (MAINRITSAN3). Patients were then followed prospectively through month 84 and their data were pooled to analyse relapses and adverse events. The primary endpoint was relapse-free survival at month 84., Results: 277 patients were enrolled and divided in 5 groups: AZA (n=58), 18-month fixed-schedule RTX (n=97), 18-month tailored-RTX (n=40), 36-month tailored/fixed RTX (n=42), 36-month fixed/fixed RTX (n=41). After adjustment for prognostic factors, 18-month fixed-schedule RTX was superior to AZA in preventing major relapses at month 84 (HR 0.38, 95% CI 0.20 to 0.71). The 18-month tailored-RTX regimen was associated with an increased risk of major relapse compared with fixed-schedule regimen (HR 2.92, 95% CI 1.43 to 5.96). The risk of major relapse was similar between 36-month fixed/fixed and 18-month fixed-RTX (HR 0.69, 95% CI 0.38 to 1.25)., Conclusions: According to these results, it appears that the 84-month remission rate is higher with an 18-month fixed RTX regimen compared with AZA and 18-month tailored RTX. Also, extending RTX to 36 months does not appear to reduce the long-term relapse rate compared with the 18-month fixed RTX regimen. Objectives: Adult IgA vasculitis (IgAV) is more common in males, but the potential impact of gender remains unclear. We aimed to describe the impact of gender on presentation and outcome in adult IgAV., Methods: We retrospectively analysed data from a multicentre retrospective cohort of 260 patients (IGAVAS). Comparisons were made according to gender status., Results: Data from 259 patients (95 females and 164 males) were analysed. Compared with females, baseline presentation in males was similar for cutaneous involvement (100% vs 100%, p= 1.0), joint involvement (60% vs 63%, p= 0.7), gastrointestinal involvement (57% vs 45%, p= 0.093) and glomerulonephritis (73% vs 64%, p= 0.16). Glomerulonephritis was more severe at baseline in males than in females, with a lower median estimated glomerular filtration rate (eGFR) (90 [IQR 59-105] vs 97 ml/min/1.73m2 [76-116], p= 0.015) and increased median proteinuria (0.84 vs 0.58 g/day, p= 0.01). There were no differences in histological findings in patients who had a kidney biopsy. Methylprednisolone was more frequently used in males (40% vs22%, p= 0.015), as were immunosuppressants, especially cyclophosphamide 24% vs 6%, p= 0.0025) and azathioprine (10% vs 2%, p= 0.038). Analysis of treatment response showed that males had more frequent refractory disease (30% vs 13%, p= 0.004). Long-term outcomes (mortality and progression to chronic kidney failure) did not differ., Conclusion: Kidney involvement in IgAV appears to more severe in males, which is supported by more intensive treatment contrasting with a lower response rate. Introduction: Rituximab is a first-line treatment for membranous nephropathy. Nephrotic syndrome limits rituximab exposure due to urinary drug loss. Rituximab underdosing (serum level <2 μg/ml at month-3) is a risk factor for treatment failure. We developed a machine learning algorithm to predict the risk of underdosing based on patients' characteristics at rituximab infusion. We investigated the relationship between the predicted risk of underdosing and the cumulative dose of rituximab required to achieve remission., Methods: Rituximab concentrations were measured at month-3 in 92 sera from adult patients with primary membranous nephropathy, split into a training (75%) and a testing set (25%). A forward-backward machine-learning procedure determined the best combination of variables to predict rituximab underdosing in the training data set, which was tested in the test set. The performances were evaluated for accuracy, sensitivity, and specificity in 10-fold cross-validation training and test sets., Results: The best variables combination to predict rituximab underdosing included age, gender, body surface area (BSA), anti-phospholipase A2 receptor type 1 (anti-PLA2R1) antibody titer on day-0, serum albumin on day-0 and day-15, and serum creatinine on day-0 and day-15. The accuracy, sensitivity, and specificity were respectively 79.4%, 78.7%, and 81.0% (training data set), and 79.2%, 84.6% and 72.7% (testing data set). In both sets, the algorithm performed significantly better than chance ( P  < 0.05). Background: C3 glomerulopathy and idiopathic immunoglobulin-mediated membranoproliferative GN (Ig-MPGN) are rare complement-mediated kidney diseases. Inherited forms of C3 glomerulopathy/Ig-MPGN are rarely described., Methods: Three hundred ninety-eight patients with C3 glomerulopathy ( n =296) or Ig-MPGN ( n =102) from a national registry were screened for three complement genes: factor H ( CFH ), factor I ( CFI ), and C3 . Patients with rare variant (minor allele frequency <0.1%) were included. Epidemiologic, clinical, and immunologic data at diagnosis and kidney outcomes of patients were retrospectively collected., Results: Fifty-three different rare variants, including 30 (57%), 13 (24%), and ten (19%) in CFH , CFI , and C3 variants, were identified in 66/398 (17%) patients. Thirty-eight (72%) variants were classified as pathogenic, including 20/30 (66%) and 11/13 (84%) variants in CFH and CFI , respectively, impairing synthesis of factor H or factor I regulators. Fifteen of 53 (27%) variants were of unknown significance. At diagnosis, 69% of patients were adult (median age of 31 years). With the exception of biologic stigma of thrombotic microangiopathy, which was more frequent in patients with CFI variants (5/14 [36%] versus 1/37 [3%] and 0% in the CFH group and C3 group, respectively, P < 0.001), the clinical and histologic features were similar among the three variants groups. The kidney outcome was poor regardless of the age at onset and treatment received. Sixty-five percent (43/66) of patients with rare variant reach kidney failure after a median delay of 41 (19-104) months, compared with 28% (55/195) after a median delay of 34 (12-143) months in the nonvariant group. Among 36 patients who received a kidney transplant, 2-year recurrence was frequent, occurring in 39% (12/31), without difference between variant groups, and led to graft failure in three cases., Conclusions: In our cohort, 17% of C3 glomerulopathy/Ig-MPGN cases were associated with rare variants in the CFH , CFI , or C3 genes. In most cases, a quantitative deficiency in factor H or factor I was identified.
