Objective To investigate the value of magnetic resonance image-diffusion weighted imaging (MRI-DMI) combined with attenuation imaging (ATI) in the diagnosis of focal nodular lesions < 2 cm in the background of fatty liver. Methods The clinical data of 68 patients with fatty liver complicated with focal nodular lesions < 2 cm were retrospectively analyzed from June 2022 to June 2023 in Huaihe Hospital of Henan University. All patients underwent MRI, pathology and ATI examination. According to the degree of fatty liver disease, 68 patients were divided into severe group (n = 24), moderate group (n = 27) and mild group (n = 17). The apparent diffusion coefficient (ADC) and acoustic attenuation coefficient (AC) values of patients with different focal lesions were compared with clinical data, and the ADC values and AC values of patients with different fatty liver lesions were compared with clinical data, and the correlation was analyzed by Pearson. To analyze the value of MRIDWI combined with AC value in the diagnosis of malignant fatty liver disease with focal nodular disease < 2 cm. Results Pathological diagnosis showed that 41 of the 68 patients were benign and 27 were malignant. There were no significant differences in low density lipoprotein cholesterol (LDL-C), age, triglyceride (TG), sex, total cholesterol (TC) and high density lipoprotein cholesterol (HDL-C) between benign nodule group and malignant nodule group (P > 0.05). There was no difference in TG, age, HDL-C, sex, TC and LDL-C among mild, moderate and severe groups (P > 0.05). The ADC value of malignant nodule group was lower than benign nodule group under different b values (P < 0.05), and the AC value of malignant nodule group was higher than that of benign nodule group (P < 0.05). ADC value under different b values in the severe group was lower than mild and moderate groups (P < 0.05), ADC value under different b values in the moderate group was lower than mild group (P < 0.05), AC value in the severe group was higher than mild and moderate groups (P < 0.05), and AC value in the moderate group was higher than mild group (P < 0.05). ADC values at different b values were negatively correlated with the degree of fatty liver disease (P < 0.05), but not with TG, HDL-C, TC and LDL-C (P > 0.05). AC was positively correlated with the degree of fatty liver disease (P < 0.05), negatively correlated with HDL-C (P < 0.05), but not with TG, TC and LDL-C (P > 0.05). The area under the curve (AUC) values of MRI-DWI, AC and their combination in the diagnosis of fatty liver complicated with focal nodule disease < 2 cm into malignancy were 0.775, 0.773 and 0.909, respectively (P < 0.05), and the AUC value of the combination was higher (P < 0.05). Conclusion MRI-DWI and ATI have important value in the differential diagnosis of benign and malignant lesions of focal nodular lesions < 2 cm in the background of fatty liver, and the combination of MRI-DWI and ATI has higher diagnostic value. In June 2024, the European Association for the Study of the Liver, the European Association for the Study of Diabetes, and the European Association for the Study of Obesity jointly released the latest edition of clinical practice guidelines on the management of metabolic dysfunction-associated steatotic liver disease(MASLD), which comprehensively elaborates on the definition, prevalence rate, natural disease history, screening, diagnosis, and treatment of MASLD and proposes 33 statements and 72 recommendations. Objective: To investigate the relationship between triglyceride glucose index (TyG), platelet-to-lymphocyte ratio(PLR) and liver fibrosis in patients with metabolic associated fatty liver disease (MAFLD) complicated with chronic hepatitis B (CHB), in order to provide help for clinical diagnosis and treatment. Methods: 100 patients with MAFLD combined with CHB were selected from The Second Hospital of Lanzhou University from January 2021 to August 2023. The patients were divided into the excluded liver fibrosis group (n=44), the uncertain liver fibrosis group(n=38) and the liver fibrosis group(n=18) according to the liver fibrosis 4 factor index(FIB-4). Baseline data and laboratory test indicators were compared among the three groups. Spearman was used to analyze the correlation between FIB-4 index and clinical indicators. Multivariate logistic regression was used to analyze the influencing factors of liver fibrosis. Receiver operating characteristic curve(ROC) was used to analyze the efficacy of TyG and PLR in evaluating liver fibrosis. Results: The patients in the liver fibrosis group were the oldest, the levels of ALT, AST, GGT, DBIL and TyG were the highest, and the levels of diastolic blood pressure,lymphocyte count, PLT, LDL-C, UA, PLR, TC/HDL-C and LDL/HDL-C were the lowest. FIB-4 index was positively correlated with age, DBIL, TG/HDL-C and TyG(all P<0.01), and negatively correlated with ALB, TC/HDL-C, LDL/HDL-C and PLR(all P<0.01). Age, DBIL, TyG and LDL / HDL-C were independent risk factors for liver fibrosis in MAFLD patients with CHB(all P<0.05). ROC curve analysis showed that the area under the curve(AUC) of TyG in predicting liver fibrosis was 0.674, the sensitivity was 88.9 %, and the specificity was 56.1 %. The AUC of PLR in predicting liver fibrosis was 0.718, the sensitivity was 77.8 %, and the specificity was 65.9 %. Conclusion: TyG and PLR are related to the severity of liver fibrosis in MAFLD patients with CHB. Both of them have high predictive value for the occurrence of liver fibrosis in MAFLD patients with CHB, and can be used to identify the risk of liver fibrosis in MAFLD patients with CHB early. Objective To investigate the effect of ginkgolide B on hepatic ischemia-reperfusion injury in mice with fatty liver disease and its mechanism. Methods A total of 25 male C57BL/6J mice were fed with 60% high-fat diet for 16 weeks, and then they were divided into sham-operation group (laparotomy and suture), model group (occlusion of blood flow in the middle and left lobes of the liver for 1 h, followed by reperfusion for 6 h), ginkgolide B group (intraperitoneal injection of 25 mg/kg ginkgolide B twice, followed by the treatment in the model group), GW9662 group (intraperitoneal injection of 1 mg/kg GW9662 twice, followed by the treatment in the model group), and GW9662+ginkgolide B group (intraperitoneal injection of 25 mg/kg ginkgolide B+1 mg/kg GW9662 twice, followed by the treatment in the model group). At 6 h after administration and surgery, the serum levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured for each group; Western blotting was used to measure the protein expression levels of Bax, Bcl-2, and PPARγ in liver tissue; oil red O staining was used to measure the degree of fatty changes in the liver, and HE staining was used to measure the degree of necrosis in liver tissue. Results Oil red O staining showed that the degree of fatty changes in the liver had reached the level of fatty liver disease in each group. Objective To explore the effect of moderate fatty liver on cardiac function in orthotopic autologous liver transplantation model rats. Methods Sixty SPF-grade healthy male SD rats were randomly divided into three groups (20 rats in each group). Group A was fed ordinary diet for 6 weeks and underwent laparotomy followed by separation of the blood vessels and ligaments around the liver and abdominal closure; group B was fed ordinary diet for 6 weeks and underwent orthotopic autologous liver transplantation; group C was fed high-fat diet for 6 weeks and underwent orthotopic autologous liver transplantation. The body mass, liver wet mass, liver index, and serum levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) after 6 weeks of feeding, as well as intraoperative blood pressure, pulse rate, respiratory rate, proportion of arrhythmia occurring in the new liver stage, and serum levels of lactate dehydrogenase (LDH), α-hydroxybutyrate dehydrogenase (α-HBDH), creatine kinase (CK), creatine kinase isoenzyme (CK-MB), and high-sensitivity cardiac troponin (hs-cTn) at 0, 5, 10, and 60 min in the new liver stage were compared. Results The body mass, liver wet mass, liver index, and serum AST and ALT levels, as well as intraoperative systolic blood pressure, diastolic blood pressure, pulse rate, respiratory rate, and proportion of arrhythmia occurring in the new liver stage in group C were significantly higher than those in groups A and B (F=36.40-4 060.54,χ2 =10.91,P0.05). Objective To investigate the population distribution of non-alcoholic fatty liver disease before and after renaming and the association between the types of metabolic risk factors (MRF) for metabolic dysfunction-associated steatotic liver disease (MASLD) and advanced liver fibrosis. Methods This study was conducted among 515 patients who were admitted to The Affiliated Hospital of Xuzhou Medical University and Wuxi Fifth People’s Hospital from January 2019 to January 2022 and had hepatocyte steatosis ≥5% by liver biopsy. Among these patients, 2 patients did not meet the diagnostic criteria for nonalcoholic fatty liver disease (NAFLD) and metabolic associated fatty liver disease (MAFLD), respectively, and were classified as steatotic liver disease (SLD) with other specific causes, and the other 513 patients were divided into MASLD group with 275 patients, comorbid group with 216 patients (MASLD comorbid with other liver diseases), and cryptogenic SLD group with 22 patients. The above groups were compared in terms of clinical features, laboratory markers, and advanced liver fibrosis. The MASLD patients with different types of MRF were compared in terms of clinical features, laboratory markers, and advanced liver fibrosis, and the risk factors for advanced liver fibrosis in patients with MASLD were analyzed. The Kruskal-Wallis H test was used for comparison of continuous data with skewed distribution between multiple groups and further comparison between two groups； the chi-square test was used for comparison of categorical data between multiple groups, and Bonferroni correction was used for further comparison between two groups. The logistic regression analysis was used to identify the risk factors for liver fibrosis. Results Among the 515 patients with SLD, 297 patients (57.7%) met the diagnostic criteria for NAFLD, among whom 22 were classified as cryptogenic SLD and 275 met the diagnostic criteria for MASLD, and 467 (90.7%) were diagnosed with MAFLD. There were significant differences between the three groups in sex, body mass index (BMI), gamma-glutamyl transpeptidase, triglyceride, cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, fasting plasma glucose, NAFLD fibrosis score (NFS), fibrosis-4 (FIB-4), and F3-4 (all P<0.05). Compared with the MASLD group and the cryptogenic SLD group, the comorbid group had the highest proportion of patients with advanced liver fibrosis (P<0.001) . With the increase in the type of MRF, the patients tended to have an older age, a significantly higher proportion of female patients, a higher possibility of hypertension and diabetes, and higher levels of metabolic parameters including BMI, blood lipids, and blood glucose (all P<0.05) . With the increase in the types of MRF in MASLD patients, they tended to have significantly higher noninvasive fibrosis scores (NFS and FIB-4) and a significantly higher proportion of patients with advanced liver fibrosis (P<0.05). The multivariate logistic regression analysis showed that age ≥50 years (odds ratio [OR] =2.622, 95% confidence interval [CI] : 1.091—6.300, P=0.031) and the increase in the type of MRF (OR=1.876, 95%CI: 1.194—2.947, P=0.006) were independent risk factors for MASLD with severe liver fibrosis. Conclusion The new definition of MASLD is based on the positive identification of MRF, and the reclassified population of MASLD is smaller than that of MAFLD, with little difference from that of NAFLD. objective: To investigate the alleviating mechanism of camel milk on non-alcoholic fatty liver disease (NAFLD) in mice. Methods: Thirty C57BL/6J mice were randomly assigned into three groups: negative control group (NC group), model group (Mod group) and camel milk group (CaM group). The negative control group was fed control diets, the model and camel milk groups were fed high-fat diets, and camel milk was given daily by gavage to the camel milk group mice. The experiment lasted for 12 weeks, and the NAFLD model was established. The alleviation mechanism of NAFLD in mice by camel milk was investigated by analyzing biochemical indices, liver pathological structure and liver lipidome. Results: With the intervention of camel milk, the body weight, blood glucose and liver index of NAFLD mice were significantly reduced to 31.45 g, 8.85 mmol/mL and 3.98%, respectively (L<0.05); the levels of total triglyceride (TG), total cholesterol (TC) and low density lipoprotein (LDL-c) in serum were significantly reduced to 0.657 mmol/mL, 4.689 mmol/mL, and 9.923 mol/mL, respectively (L<0.05); the levels of TG and TC in liver were dramatically reduced to 0.034 mmol/mL and 0.009 mmol/mL, respectively (L<0.05); liver tissue structure was intact and clear, and the lipid accumulation was reduced in the liver. Lipidomic analysis showed that the relative levels of choline (CEs), diglycerides(DGs), unsaturated triglycerides (TGs), phosphatidylcholine (PC) and lysophosphatidylcholine (LPCs) were increased in the liver of mice in the CaM group; lipid metabolites were enriched in sphingolipid metabolism, glyc-erol ester metabolism and glycerophospholipid metabolism. Conclusion: Camel milk alleviates NAFLD by modulating sph-ingolipid metabolism, triglyceride metabolism and glycerophospholipid metabolism in mice with NAFLD by altering liver lipid profiles. Objective To investigate the value of magnetic resonance imaging-proton density fat fraction (MRI-PDFF) in evaluating hepatic steatosis in patients with chronic hepatitis B virus (HBV) infection. Methods The patients, aged >16 years, who visited the outpatient service or were hospitalized in Beijing Tsinghua Changgung Hospital from January 2018 to December 2022 and were diagnosed with chronic HBV infection were enrolled, and all patients underwent MRI examination of the liver in our hospital. The patients were divided into groups based on the presence or absence of liver cirrhosis, and the consistency in PDFF between different hepatic segments was compared between groups. The Kappa consistency test and intraclass correlation coefficient (ICC) were used for consistency analysis. Results A total of 76 patients treated with nucleoside analogues were enrolled, among whom 23（30.26%) had liver cirrhosis. For all patients, the simple arithmetic average of PDFF fluctuated between 1.49% and 30.93%. According to MRI-PDFF ≥5% as the diagnostic criterion for fatty liver disease, there were 29 patients (38.16%) with fatty liver disease among all patients. For all 76 patients, the simple arithmetic average of PDFF was lower than the weighted average of PDFF for the whole liver, and there was no significant difference between the simple arithmetic average of PDFF, the weighted average of PDFF, and the PDFF values of the left and right lobes of the liver (F=0.39, P=0.76). The consistency test showed that the PDFF values of each hepatic segment and the left and right lobes of the liver had strong consistency with the weighted average and simple arithmetic average of PDFF, with an ICC of >0.75, but the consistency between the PDFF value of the right lobe and the weighted average of PDFF was higher than that between the PDFF value of the left lobe and the weighted average of PDFF. In the consistency test of differentiating fatty liver disease in patients with liver cirrhosis, there was poor consistency between the PDFF value of segment Ⅶ and the weighted average of PDFF (Kappa=0.39), with moderate consistency for the left lobe and the Ⅰ, Ⅱ, Ⅲ, Ⅴ, Ⅵ, and Ⅷ segments. For the patients with liver cirrhosis, the lowest consistency was observed between the PDFF value of Ⅶ segment and the weighted average of PDFF for the whole liver, and the highest consistency was observed between the PDFF value of Ⅵ segment and the weighted average of PDFF for the whole liver. For the patients without liver cirrhosis, the lowest consistency was observed between the PDFF value of Ⅱ segment and the weighted average of PDFF for the whole liver, and the highest consistency was observed between the PDFF value of Ⅴ segment and the weighted average of PDFF for the whole liver. Conclusion MRI-PDFF is more comprehensive in evaluating hepatic steatosis in patients with chronic HBV infection, and for the patients with liver cirrhosis, there is poor consistency between the PDFF value of each segment and the weighted average of PDFF. Objective To understand the relationship between different lifestyles and the formation of fatty liver, and to provide a scientific basis for the prevention of fatty liver. Methods A total of 15 858 physical examination subjects who had no history of fatty liver in the hospital in 2018 were selected as the study objects. A retrospective cohort study was adopted, questionnaire survey and physical examination data were extracted, and Chi-square and logistics regression analysis were used to statistically analyze the influence of different lifestyles on the formation of fatty liver after two years of follow-up. Results The cumulative incidence of fatty liver was 8. 1%, the incidence of male was higher than that of female, the difference was statistically significant(P <0.05), and the incidence of fatty liver increased with the increase of age. The results of univariate analysis showed that men, frequent consumption of high fat, infrequent consumption of milk and soy products, excessive intake of sugar-sweetened beverages, short or long sleep time, smoking and drinking were prone to fatty liver disease(P <0.05). logistics regression analysis shows that high-fat diet and excessive intake of sugary drinks were risk factors for fatty liver(P <0.05), and exercise was a protective factor for fatty liver(P <0.05). Conclusion Keeping a light diet, reducing the intake of sugary drinks and strengthening exercise in daily life can reduce the occurrence of fatty liver. [ABSTRACT FROM AUTHOR]

Metabolism-associated fatty liver disease (MAFLD) is the hepatic manifestation of metabolic syndrome, with a global incidence up to 25% in adults, and has replaced viral hepatitis as the first chronic liver disease in China. Adenosine monophosphate-activated protein kinase (AMPK) is an important key enzyme in the regulation of glycolipid metabolism in vivo. The activation of AMPK is thought to have the ability to improve glucose and lipid metabolism disorder, alleviate oxidative stress, possess anti-inflammatory, regulate autophagy affects, and protect the mitochondria to play a therapeutic role in the treatment of MAFLD. As an important treasure trove of medicinal resources in traditional Chinese medicine, natural products of Li medicine have been widely found to activate AMPK and exhibit excellent effects in improving MAFLD. Therefore, this review is planned to summarize the association mechanisms between MAFLD improvement and AMPK activation of Li medicine and its natural products, to provide theoretical references for the exploration and development of Li medicine and its natural products. The purpose of the study was to investigate the effect of Bacteroides on liver lipid metabolism in laying hens with fatty liver hemorrhage syndrome (FLHS) induced by high-energy and low-protein diet. Eighty 210-day-old Dawu Jinfeng laying hens weighing (1.5±0.2) kg were randomly divided into four groups with five replicates in each group and four hens in each replicate. The control group (Group C) was fed with basal diet, the model group (Group M) was fed with high energy and low protein (HELP) diet, the low dose Bacteroides group (Group BL) was fed with HELP diet + 1 x 109 CFU/mL Bacteroides, and the high dose Bacteroides group (Group BH) was fed with HELP diet + 1 x 1011 CFU/mL Bacteroides. The trial period was 70 days. The results showed that compared with Group C, the liver of Group M was greasy, brittle and fragile, with a large number of fat vacuoles and red-stained fat droplets, the liver index and abdominal fat rate increased significantly (P<0.05), and the serum TG, TC, LDL-C concentrations and AST, ALT activities increased significantly (P<0.05), while HDL-C concentration decreased significantly (P<0.05). Compared with Group M, Group BL and Group BH showed significant improvement in liver fat vacuoles and red stained lipid droplets in laying hens, the liver index significantly decreased (P<0.05), the levels of serum TG, TC, LDL-C, as well as the activities of AST and ALT, were significantly reduced (P<0.05), while the levels of HDL-C were significantly increased (P<0.05). Compared with Group BH, serum TG, TC concentrations and AST activity in BL group decreased significantly (P<0.05), while HDL-C concentration increased significantly (P<0.05). Study shows that Bacteroides can improve lipid metabolism disorder and liver function damage caused by FLHS. To observe the improvement effect of the intragastric administration of curcumin on intestinal mucosal barrier injury rats with fatty liver, and to explore its mechanism of action. Methods Non-alcoholic fatty liver disease rat models were established by high-fat diet, and then were randomly divided into the high fat group, low-dose curcumin group, and high-dose curcumin group, with 10 mice in each group. Anther 10 normal rats were used as the control group. Rats in low-dose curcumin group were given curcumin 200 mg/ (kg · d) by gavage, while rats in the high-dose group were given curcumin 400 mg/ (kg · d) by gavage. Rats in the control and model groups were given sodium carboxymethyl cellulose by gavage, and rats in each group were sacrificed after 8 weeks of gavage. We took the liver and small intestine tissues from rats of each group, and observed pathological changes under light microscopy by HE staining. We took the portal vein blood from rats and measured LPS, diamine oxidase (DAO）, ALT and AST. The small intestine tissues were taken to measure the content of malondialdehyde (MDA) and the activity of superoxide dismutase (SOD) . Western blotting was used to detect nuclear factor E2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) proteins in the small intestine tissues. Results Hepatocyte architecture was normal in the control group. Lipid droplets of varying sizes were seen in hepatocytes of the model group. Hepatocellular steatosis was attenuated in the low-dose group and high-dose group. The villi of rats in the control group showed no congestion and edema, while the villi of rats in the model group showed structural damage. The villi of rats in the low-dose and high-dose curcumin groups showed loss and reduction. Compared with the control group, the blood levels of DAO, LPS, AST, and ALT in the model group, low-dose curcumin group, and high-dose curcumin group increased (all P<0. 05) . However, those in the low-dose and high-dose curcumin groups were lower than those of the model group (all P<0. 05）, with a dose-dependent manner (all P<0. 05) . Compared with the control group, MDA content of intestine in the model group, low-dose curcumin group, and high-dose curcumin group increased (P<0. 05）, but that in the low-dose and high-dose curcumin groups was lower than that of the model group, with a dose-dependent manner (P<0. 05) . Compared with the control group, SOD activity of intestine in the model group, low-dose curcumin group, and high-dose curcumin group decreased (P<0. 05）, but that in low-dose curcumin group and high-dose curcumin group was higher than model group, with a dose-dependent manner (P<0. 05) . Compared with the control group, the SOD activity of small intestine tissues in the model group, low-dose curcumin group, and highdose curcumin group decreased (all P<0. 05）, but that in the low-dose curcumin group and high-dose curcumin group was higher than that in the model group (all P<0. 05）, with a dose-dependent manner (all P<0. 05) . Compared with the control group, the relative expression levels of Nrf2 and HO-1 proteins in the small intestine tissues of the model group, lowdose curcumin group, and high-dose curcumin group increased (all P<0. 05）, and those were higher in the low-dose curcumin group and high-dose curcumin group than in the model group (all P<0. 05）, and there was in a dose-dependent manner (all P<0. 05) . BACKGROUND: High-methionine diet can cause liver injury in Cbs+/- mice, and hyperhomocystinemia is related to the occurrence and progression of various liver-related diseases, such as hepatic steatosis, autoimmune hepatitis, and alcoholic fatty liver disease. MicroRNAs (miRNAs) are involved in various cellular processes including cell survival, differentiation and autophagy, which are of great significance. OBJECTIVE: To investigate the critical role of miR-144-3p on Cbs+/- mouse hepatocyte autophagy induced by high methionine die. METHODS: (1) Ten male cystathione-β-synthase normal (Cbs+/+) mice and another 10 male mice with single gene knockout (Cbs+/-) of similar body mass, 4 weeks of age, were fed a high-methionine diet and executed after 12 weeks to take liver tissue. (2) Human hepatocytes (HL-7702) were cultured in vitro and divided into control [0 μmol/L homocysteine (Hcy)], Hcy (100 μmol/L Hcy), mimic-NC (transfected with mimic-NC), mimic-NC + Hcy (mimic-NC transfecton+100 μmol/L Hcy), miR-144-3p mimic (transfected with miR-144-3p mimic), and miR-144-3p mimic + Hcy (miR-144-3p mimic transfection+100 μ mol/L Hcy), inhibitor-NC (transfected with inhibitor-NC), inhibitor-NC + Hcy (inhibitor-NC transfection + 100 μmol/L Hcy), miR-144-3p inhibitor (transfected with miR-144-3p inhibitor), and miR-144-3p inhibitor + Hcy (miR-144-3p inhibitor transfection + 100 μmol/L Hcy). Quantitative real-time PCR was used to detect the expression of miR144-3p in liver tissue and hepatocytes. After transfection of miR-144-3p mimic or inhibitor, quantitative real-time PCR and western blot were used to detect the transfection efficiency of miR-144-3p and its effect on the expression of autophagy-related proteins LC3B and p62. The levels of alanine transferase and aspartate aminotransferase in hepatocyte supernatants were determined by enzyme linked immunosorbent assay. The correlation between the expression of miR-144-3 in hepatocyte and the levels of alanine transferase and aspartate aminotransferase in hepatocyte supernatants were analyzed by Pearson correlation analysis. RESULTS AND CONCLUSION: Compared with the Cbs+/+ group and control group, the expression of miR-144-3p in the liver tissue of the Cbs+/- group and in hepatocytes of the Hcy group was decreased (P < 0.01). The expression of LC3B-II/I was decreased in hepatocyte after transfection of miR-144-3p mimic, while the protein expression of p62 was increased (P < 0.01). The opposite results were obtained after transfection of miR-144-3p inhibitor (P < 0.01). Compared with the mimic-NC group, the levels of alanine transferase and aspartate aminotransferase were decreased in the miR-144-3p mimic group (P < 0.01), while the opposite results were obtained in the inhibitor-NC group (P < 0.01). The expression of miR-144-3p in hepatocytes was negatively correlated with the levels of alanine transferase (P < 0.01, r=-0.887 6) and aspartate aminotransferase (P < 0.01, r=-0.829 9) in the supernatant of hepatocytes. Hepatic steatosis can be observed in chronic liver diseases of different etiologies. The main predisposing factors for hepatic steatosis include chronic viral hepatitis, cholestatic liver disease, alcoholic liver disease, and nonalcoholic fatty liver disease. Simple fatty liver disease is the initial manifestation of hepatic steatosis, followed by steatohepatitis, liver fibrosis, liver cirrhosis, and even hepatocellular carcinoma. With the development of medical imaging technology, magnetic resonance imagingproton density fat fraction (MRI-PDFF) has been widely used in the diagnosis of fatty liver disease (FLD) in clinical practice. This study aimed to investigate the intricate mechanisms underlying the modulatory effects of Anhua dark tea on autophagy to ameliorate steatosis induced by a high-fat and high-sucrose diet (HFHS) in mice with non-alcoholic fatty liver disease (NAFLD). Male C57BL/6J mice were divided into different groups, including a normal group, a model group, a Western medicine group (10 mg·kg-1), and various doses of dark tea groups (0.75, 1.5, 3.0 g·kg-1). The therapeutic regimen was administered concurrently with the modeling process for a duration of 10 weeks using the HFHS-induced NAFLD model. At the end of the experiment, liver indices, blood lipids, liver function, liver pathology indicators, autophagy markers, and expression levels of key genes in the autophagy-related signaling pathway were assessed. Comparative analyses with the normal group revealed significant increases in liver index and levels of serum cholesterol (CHO), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), aspartate aminotransferase (AST), and alanine aminotransferase (ALT), as well as a substantial reduction in high-density lipoprotein cholesterol (HDL-C) levels in the model group. The liver of the mice exhibits signs of steatosis, characterized by an abundance of lipid droplets of different sizes. Protein expression analysis reveals a marked decrease in the levels of microtubule-associated protein light-chain-3B (LC3B), Bcl-2-interacting coiled-coil protein 1 (Beclin1), and phosphorylated adenosine monophosphate-activated protein kinase/adenosine monophosphate-activated protein kinase (p-AMPK/AMPK). Conversely, there was a significant increase in the levels of sequestosome-1 (p62) and phosphorylated mammalian target of rapamycin/mammalian target of rapamycin (p-mTOR/mTOR). Compared to the model group, gavage with dark tea decreased the liver index, serum levels of CHO, TG, LDL-C, AST, ALT, p62, and p-mTOR/mTOR in NAFLD mice, and increased serum HDL-C, along with LC3B, Beclin1, and p-AMPK/AMPK protein levels. The improvements were confirmed by tissue staining results and observations using transmission electron microscopy. In summary, our findings suggest that dark tea, by activating the AMPK/mTOR signaling pathway, may regulate autophagy, thereby alleviating hepatic steatosis and improving non-alcoholic fatty liver disease (NAFLD). Objective: To clarify the role of the liver/bone/kidney alkaline phosphatase gene (ALPL) in high-fat diet-induced hepatic fat deposition. Methods: A fatty liver model was induced by administration of high-fat diet in wild-type (WT) and ALPL knockout (ALPL+/-) mice for 8 weeks. Intrahepatic fat deposition and serum glucose, triglyceride and cholesterol levels in serum were measured. RT-PCR, Western blotting and immunofluorescence staining were used to detect the expression of genes related to fatty acid synthesis and transport in liver. Results: No significant change in liver of fat deposition was observed in the ALPL+/- group compared with the WT group on normal diet, while serum glucose and cholesterol levels were increased; under high-fat conditions, intrahepatic fat deposition in the ALPL+/- mice was significantly increased and accompanied by an increase in serum triglyceride. RT-PCR and Western blotting results showed that the expression of the key fatty acid synthesis genes ACC1, ACC2 and PPAR-γ, and the fatty acid synthesis gene LPL in the liver of ALPL+/- mice were significantly increased under high-fat induction. In addition, immunofluorescence staining showed that PPAR-γ-positive hepatocytes were also significantly increased in the liver of ALPL+/- mice under high-fat induction. Conclusion: ALPL knockdown promotes the synthesis and transport of intrahepatic fatty acid and accelerates the fat deposition in the liver of mice induced by high fat. Objective To evaluate the association between abnormal renal function indicators (SUA and eGFR) and non 一 alcoholic fatty liver disease (NAFLD). Methods Based on 2017 - 2021 Beijing Health Management Cohort, the cross 一 lagged panel model was used to investigate the causal temporal relationship between abnormal SUA and glomerular filtration rate and the onset of NAFLD. Results (1) Increased SUA and NAFLD: ① Path coefficients from baseline SUA to follow 一 up HSI were statistically significant in both the general population (p = 0. 018, 95% CI: 0. 003 一 0. 032) and BMI 24 kg/m² = 0. 051, 95% CI: 0. 032 - 0. 070), but not the other way around.② Path coefficients from baseline SUA to follow - up HSI (p = 0. 048, 95% CI: 0. 028 - 0. 068) and baseline HSI to foUow - up SUA (= 0. 023, 95% CI: 0. 005 - 0. 041) were statistically significant in BMI < 24 kg/m. Objective To investigate the application value of liver/spleen CT value （CTL/S）， controlled attenuation parameter （CAP）， and magnetic resonance imaging-proton density fat fraction （MRI-PDFF） in chronic hepatitis B （CHB） patients with hepatic steatosis. Methods A retrospective analysis was performed for the clinical data of 213 CHB patients who underwent liver CT， CAP， and MRI-PDFF examinations in Affiliated Hospital of Yan’an University from October 2018 to December 2022. According to MRI-PDFF， the 213 patients were divided into CHB group with 111 patients （MRI-PDFF<5%） and CHB+hepatic steatosis group with 102 patients （MRI-PDFF≥5%）， among whom there were 69 patients with mild hepatic steatosis and 33 patients with moderate to severe hepatic steatosis. The independent-samples t test was used for comparison of normally distributed continuous data between groups， and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups. The Bland-Altman plot was used to evaluate the consistency in MRI-PDFF measurement between two physicians. The Spearman’s correlation coefficient was used to analyze the correlation between CTL/S and MRI-PDFF and between CAP and MRI-PDFF. The receiver operating characteristic （ROC） curve was plotted and the area under the ROC curve （AUC） was calculated to investigate the value of CTL/S and CAP in the diagnosis of different degrees of hepatic steatosis， and the DeLong test was used to compare the AUCs of the two radiological examinations. Results MRI-PDFF had relatively high repeatability and stability in CHB patients. There is a significant negative correlation between CTL/S and MRI-PDFF （r=-0.800， P<0.001） and a significant positive correlation between CAP and MRI-PDFF （r=0.692， P<0.001）. Both CTL/S and CAP had a relatively high accuracy in the diagnosis of hepatic steatosis in CHB patients， with an AUC of 0.951 and 0.902， respectively， and CTL/S had a better accuracy than CAP （P<0.05）. In the diagnosis of mild and moderate-to-severe hepatic steatosis， CTL/S had an AUC of 0.921 and 0.895， respectively， and CAP had an AUC of 0.859 and 0.825， respectively， suggesting that CTL/S had a slightly higher diagnostic efficiency than CAP. Conclusion MRI-PDFF has high repeatability and stability in CHB patients， and CTL/S and CAP have a high diagnostic value for different degrees of hepatic steatosis in CHB patients. [Objectives]This study aimed to investigate the effect of dietary bile acids(BA)supplementation on alleviating fatty liver syndrome(FLS)induced by corticosterone(CORT)in broilers, and the mechanism involving in this process. [Methods]One hundred and twenty 1-day-old AA male broilers were randomly divided into two groups and fed a basal diet or a BA containing diet respectively. BA were periodically added to the diet using the dose-escalation strategy including early, middle and late stages. At 28 days of age, twenty broilers were randomly selected from the basal diet group and divided into control group(CON)and corticosterone treatment group(CORT). Ten broilers fed BA were randomly picked up and set as corticosterone plus bile acids group(CORT+BA). The broilers were slaughtered at 40 days of age. The weight of organs and tissues was recorded. The serum and liver tissues were sampled for biochemical analysis and hepatic genes or proteins expression analysis. [Results]BA treatment significantly alleviated the deposition of abdominal fat and the decrease of leg muscle weight induced by CORT injection, and also increased leg muscle percentage(P<0.05). The results of biochemical analysis showed that CORT treatment significantly decreased serum high-density lipoprotein(HDLC)concentration, but increased serum non-esterified fatty acid(NEFA)level, while BA significantly increased the level of serum HDLC(P<0.05). The level of hepatic triglyceride(TG)was significantly increased by CORT(P<0.01)and the lipid droplets in liver were significantly enriched and vacuolated. However, these changes were markedly alleviated by BA treatment(P<0.05). Compared to the CON group, CORT significantly up-regulated the mRNA expression of stearoyl-CoA desaturase 1(scd1)and acetyl CoA carboxylase(acc)genes, and also significantly up-regulated the protein expression of ACC and fatty acid synthase(FASN)but down-regulated peroxisome proliferator activated receptor alpha(PPARα)protein expression in the liver(P<0.05). BA supplementation ameliorated the disorders of hepatic lipid metabolism caused by CORT and increased the mRNA expression of carnitine palmitoyl-transferase 1A(cpt1a)and decreased protein expression of fatty acid translocase(CD36)(P<0.05). [Conclusions]Dietary BA supplementation can significantly alleviate hepatic TG deposition induced by CORT through decreasing hepatic TG synthesis, promoting fatty acid decomposition, as well as inhibiting hepatic fatty acids uptake in broilers. Objective To investigate the risk factors for severe hyperlactacidemia after liver transplantation, and to provide a basis for adopting effective preventative strategies in clinical practice. Methods A retrospective analysis was performed for the clinical data of 120 patients who underwent liver transplantation in our hospital from January to December, 2021, and according to the value of arterial lactic acid (LAC) after surgery, they were divided into severe hyperlactacidemia group and non-severe hyperlactacidemia group. The trend line chart was plotted to analyze the association between blood biochemical parameters and arterial LAC level in the perioperative period; the chi-square test, the t-test, the non-parametric test, and the multivariate logistic regression model analysis were used to investigate the effect of the above indicators on severe hyperlactacidemia after liver transplantation. Results The trend line chart showed that in all patients, the changing trend of serum alanine aminotransferase, serum aspartate aminotransferase, serum albumin, plasma international normalized ratio, and prothrombin time was consistent with that of arterial LAC level from day 3 before surgery to day 3 after surgery. Objective To explore the inhibitory effect of Jianpi Huatan Formula on "inflammation-cancer transformation" in a mouse model of non-alcoholic fatty liver disease based on the lipoprotein lipase (LPL)/carnitine palmitoyltransferase 1 (CPT1)/fatty acid binding protein 4 (FABP4) pathway. Methods According to the random table method; 30 C57BL/6 mice were randomly divided into the control group 'model group' Jianpi Huatan Formula low-dose group (19.2 g/kg) ; Jianpi Huatan Formula high-dose group (38.4 g/kg) ; and ezetimibe group (10. 0 mg/kg) ; six mice in each group. Except for the control group; mice in the other groups were injected intraperitoneally with diethylnitrosamine (25 mg/kg). After 4 weeks ; except for the control group ; mice in the other groups were fed with a high-fat and high-cholesterol diet for 26 weeks to establish the non-alcoholic steatohepatitis-related hepatocellular carcinoma (NASH-HCC) model. The treatment was administered once daily for 26 weeks. The number of liver tumors and the maximum tumor surface area were compared among the groups. HE staining and oil red O staining were used to observe liver inflammation' steatosis and ballooning. The content of tumor necrosis factor-a (TNF-α) in liver was determined by ELISA. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL), and high-density lipoprotein (HDL) levels were measured by a biochemical analyzer. The mRNA levels of tumor-related markers such as Birc5, Ly6d, CD44, and Mki67 in liver were determined by real-time PCR. The protein expression levels of LPL, CPT1, and FABP4 in the liver of each group of mice were measured by Western blotting. Results Compared with the control group' mice in the model group showed tumors in the liver, elevated mRNA levels of Ly6d, Birc5, CD44, and Mki67, obvious liver damage' severe steatosis and ballooning' large amounts of lipid droplet accumulation in the liver, significantly elevated serum ALT, serum AST, and hepatic TNF-α levels elevated levels of serum TC, TG, LDL, and HDL, and elevated protein expression levels of LPL, CPT1B, and FABP4 in the liver (P<0. 05). Compared with the model group' the mRNA expression of Birc5 was reduced in the Jianpi Huatan Formula low-dose group and the ezetimibe group (P<0. 05) ; liver injury was alleviated to varying degrees in the Jianpi Huatan Formula low-and high-dose groups and the ezetimibe group' as evidenced by a decrease in steatosis and ballooning, a decrease in lipid droplet accumulation, and a decrease in the hepatic inflammatory factor TNF-α (P < 0.05). Compared with the model group ' serum TC, TG, LDL, and HDL levels were decreased in the Jianpi Huatan Formula low-dose group and the ezetimibe group (P < 0.05) ; and compared with the Jianpi Huatan Formula low-dose group, serum TC, LDL, and HDL levels were significantly decreased in the ezetimibe group (P<0.05). Compared with the model group' the protein expression levels of CPT1B and FABP4 were decreased in the Jianpi Huatan Formula low-dose group' and the protein expression levels of LPL and FABP4 were decreased in the Jianpi Huatan Formula high-dose group (P < 0.05). Conclusion Jianpi Huatan Formula may exert anti-NASH-HCC effects by regulating the LPL/CPT1/FABP4 lipid metabolism signaling pathway, promoting the breakdown of TC and TG, improving liver tissue steatosis and its inflammatory damage in model mice, and further inhibiting tumor development. Sestrin 3 （Sesn3） is a highly conserved stress-inducible protein that is stimulated during various stress conditions, including DNA damage, oxidative stress, endoplasmic reticulum stress and inflammation, to protect organisms against different types of stimuli. Sesn3 is regulated by forkhead box O transcription factors, which activates both AMP-activated protein kinase and mammalian target of rapamycin complex 2（ mTORC2） and suppresses mTORC1. Moreover, Sesn3 can directly or indirectly scavenge reactive oxygen species to alleviate cellular oxidative damage. Sesn3 also plays an important role in the development of multiple diseases by participating in biological processes such as antioxidation, anti-aging, nutrient sensing, and maintenance of metabolic homeostasis. This study reviews the role of Sesn3 in cardiovascular and fatty liver diseases, diabetes mellitus, malignancies and epilepsy to explore novel concepts and potential therapeutic targets for addressing related diseases. Objective: To explore the role of OSBPL3 in mediating metabolism-related fatty liver disease and its possible mechanism. Study on the role and mechanism of OSBPL3 in metabolism related fatty liver disease. Hepatic specific silencing OSBPL3 mouse model and empty vector control mouse model were established. They were fed normal diet and high fat diet for 12 weeks respectively. They were divided into normal control group, OSBPL3 silencing group, obese control group and obese OSBPL3 silencing group. To observe the general situation of mice, the lipid synthesis genes and lipid decomposition gene mRNA were detected by Real time PCR, and the key proteins of Akt/mTOR pathway were detected. Human HepG2 cell lines were treated with OA at different concentrations, and the staining changes of O were observed after treatment with different oleic acid concentrations. OSBPL3 expression levels were detected by resting western blot assay. There were no significant differences in all indexes between normal control group and OSBPL3 silencing group (P>0.05). Compared with the control group, the body mass, visceral fat and visceral fat index of the obese control group and the obese OSBPL3 silent group were higher (P<0.05). Compared with the obese control group, the body mass, visceral fat and visceral fat index in the obese OSBPL3 silencing group were lower (P<0.05). Compared with the control group, the TC, TG, LDL-C and HDL-C of the obese control group and the obese OSBPL3 silent group were higher (P<0.05). Compared with the obese control group, the TC, TG, LDL-C and HDL-C in the obese OSBPL3 silenced group were lower (P<0.05). There were no significant differences in the expression levels of SREBP-1C, FAS and PPARα compared with normal control group and OSBPL3 silencing group (P>0.05). Compared with the control group, the levels of SREBP-1C, FAS and PPARα were higher in the obese control group and the obese OSBPL3 silenced group (P<0.05). Compared with the obese control group, the expression levels of SREBP-1C and FAS in the obese OSBPL3 silenced group were lower, and the expression levels of PPARα were higher (P<0.05). Compared with the control group, the expression levels of Akt and mTOR phosphorylation in the obese control group were higher (P<0.05). Compared with the obese control group, the expression levels of Akt and mTOR phosphorylation in the obese OSBPL3 silencing group were lower (P<0.05). Compared with 0 μmol/L oleic acid, oil red O staining was gradually deepened with the increase of OA concentration. Compared with 0 μmol/L oleic acid and 0 μmol/L oleic acid, oil red O staining was gradually deepened with the increase of OA concentration. Compared with 0 μmol/L oleic acid, OSBPL3 mRNA levels of 50 μmol/L, 100 μmol/L and 400 μmol/L oleic acid were increased successively, with statistical difference (P<0.05). The expression level of OSBPL3 increased with the increase of lipid accumulation. ObjectiveTo investigate the prevalence of overweight， obesity and fatty liver disease （FLD） in adult residents in a suburban area of Shanghai， and to explore the relationship between these conditions and eating out of home （EOH）.MethodsAdult residents from Zhongshan Street， Songjiang， Shanghai， were invited to report their frequency of EOH and undergo a physical examination using B-ultrasonography from May to September 2017. Binary logistic regression analyses were conducted to examine the relationship between EOH and conditions of overweight， obesity， and FLD.ResultsAmong the final sample size of 6 608 participants， the prevalence rates of overweight， obesity， and FLD were 41.65%， 14.71% and 38.29%， respectively. After adjusting for potential confounders， logistic analyses revealed that participants with an EOH frequency > once/week had a 1.44-fold higher prevalence of obesity （P=0.006）， a 1.35-fold higher prevalence of overweight/obesity （P=0.005）， and a 1.36-fold higher prevalence of FLD （P=0.008）.ConclusionThe prevalence of overweight， obesity， and FLD is relatively high in Songjiang， Shanghai. EOH may be a risk factor for these diseases， and residents are recommended to reduce the frequency of EOH.

Background Both fatty liver disease and thyroid nodules are common clinical chronic diseases that are closely related to metabolism, but there is little research on whether there is a certain correlation between them. Objective To explore the overall and disease severity-specific correlations between metabolic-associated fatty liver disease (MAFLD) and thyroid nodules. Methods This study selected 666 MAFLD patients〔including 316 mild cases (group A) , and 350 moderate to severe cases (group B) 〕, and 330 health examinees (group C) from Shengjing Hospital of China Medical University from December 2020 to December 2021. All the subjects underwent general anthropometric measurement, blood tests and ultrasound examination of the liver and thyroid. Multivariate Logistic regression was used to explore the influencing factors of thyroid nodules in MAFLD. 为了探究复羽叶栾树种仁油活性成分可能作用于脂肪肝治疗的药效物质基础、潜在靶标及作用机制，基于网络药理学手段和分子对接，获得复羽叶栾树种仁油中6种核心有效成分花生一烯酸、棕榈酸、亚油酸、油酸、芥酸、花生酸，筛选出治疗脂肪肝相关靶点69个，预测可能作用于ALB、IGF1、PPARα、PPARγ、SRC 5个核心靶点蛋白，通过活性成分与靶点相互作用发挥治疗脂肪肝的作用。复羽叶栾树种仁油治疗脂肪肝具有多成分、多靶点、多通路的作用特点。 In order to investigate the possible pharmacological basis, potential targets and mechanism of action of the active ingredients of Koelreuteria bipinnata seed oil in the treatment of fatty liver, based on network pharmacology and molecular docking, six core active ingredients in Koelreuteria bipinnata seed oil eicosenoic acid, palmitic acid, linoleic acid, oleic acid, erucic acid and arachidic acid were obtained, and 69 targets related to the treatment of fatty liver were screened, predicting the possible action on five core target proteins ALB, IGF1, PPARα, PPARγ and SRC, and the therapeutic effect of fatty liver was exerted through the interaction between active ingredients and targets. Koelreuteria bipinnata seed oil has multi-component, multi-target and multi-pathway action characteristics for the treatment of fatty liver.

Background Metabolic-associated fatty liver disease (MAFLD) is a group of liver diseases related to metabolic stresses. Studies have shown that serum potassium is closely related to MAFLD. However, their relationship in gout patients remains unclear. Objective To investigate the relationship between serum potassium and MAFLD in gout patients, to provide a theoretical basis for the management of serum potassium in gout patients. Methods A total of 347 primary gout patients were recruited from Chu Hsien-I Memorial Hospital, Tianjin Medical University from April 2014 to March 2017. They were divided into simple gout group (n=96) , mild fatty liver group (n=136) and moderate-to-severe fatty liver group (n=115) according to the severity of fatty liver to compare baseline data, serum biochemical indices, overweight/obesity and insulin resistance indices. Spearman rank correlation was used to study the relationship between serum potassium level and the severity of fatty liver. Multivariate Logistic regression analysis was used to identify influencing factors of MAFLD in gout. Results Two hundred and fifty-one (72.3%) of the 347 gout patients had MAFLD. Objective: To design an auxiliary diagnostic system for liver ultrasound images using artificial intelligence (AI) methods, to assist doctors in standardizing and efficiently diagnosing large samples of liver ultrasound images, and to achieve accurate diagnosis of non-alcoholic fatty liver disease (NAFLD) based on liver ultrasound images. Methods: By developing three modules: recognition and classification of liver ultrasound images, grading analysis of fatty liver, and quantitative analysis of liver fat content, a set of ultrasound image AI assisted diagnosis system for NAFLD is established. The system can automatically distinguish the types of ultrasound images with different sampling fields input into the system, perform digital analysis of liver ultrasound images, and provide whether the ultrasound image to be tested presents fatty liver and the percentage of liver fat content. Results: The ultrasound image recognition and classification module in this study could distinguish two types of ultrasound images with high throughput, namely hepatic/renal ratio images and hepatic attenuation rate images, with a classification accuracy of 100%. The accuracy of the fatty liver grading analysis module in the test set data reached 84%, demonstrating the ability to assist doctors in diagnosis. The accuracy of the liver fat content quantitative analysis module developed based on the artificial liver fat content quantitative method reached 67.74% . Conclusion: This study has developed an AI diagnostic system based on liver ultrasound imaging, which can assist doctors in quickly, simply, and non-invasive screening of potential patients with fatty liver. Background The prevalence of metabolic-associated fatty liver disease (MAFLD) has increased rapidly. And there is no conclusion on body fat composition, characteristics of metabolic indicators, and their predictive values for MAFLD in non-obese populations. Objective To identify the risk factors for MAFLD by comparing body fat composition and key metabolic indicators (blood lipids, blood sugar, uric acid) between obese and non-obese MAFLD patients, and to assess their associations with MAFLD as well as their predictive values for MAFLD in non-obese patients. Methods Physical examinees with and without liver B-ultrasound-detected fatty liver were recruited from Health Management Center, Nanjing Drum Tower Hospital from January 2018 to January 2019 after excluding those with non-MAFLD, and divided into obese group (including 129 cases with MAFLD, and 129 without fatty liver) and non-obese group (including 129 without fatty liver cases, and 129 with MAFLD) by BMI. The body fat composition and metabolic indices in non-obese MAFLD cases were compared with those of the other three subgroups. The correlation of each index with MAFLD in non-obese cases was analyzed. The independent risk factors of MAFLD in non-obese cases were identified by using Logistic regression. The predictive value of each index for MAFLD in non-obese was assessed using the receiver operating characteristic (ROC) curve. Objective To investigate the dynamic change of fatty liver（FL) in patients with fatty liver-related acute pancreatitis（FLAP) and related influencing factors. Methods A total of 136 FLAP patients who were admitted to The First Affiliated Hospital of Baotou Medical College, Inner Mongolia University of Science and Technology, from December 2017 to December 2021 were enrolled as subjects. With the first CT examination after admission as the starting point and the last CT examination before discharge as the ending point, the subjects were divided into FL aggravation group（FLAG group), FL mild mitigation group（FLMMG group), and FL significant mitigation group（FLSMG group) according to the degree of change in FL. General information and clinical data were compared between groups. General information included age, sex, length of hospital stay, etiology of acute pancreatitis（AP), and changes in body weight and temperature, and clinical data included treatment modality and the changes in laboratory markers and AP severity. The chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Mann-Whitney U test with Bonferroni correction was used for further comparison between two groups. Dynamic changes of data were expressed as the difference from the starting point to the ending point, and a covariance analysis was used for comparison of data with dynamic changes. The Spearman correlation analysis was used to investigate the correlation of indices with significant changes with the degree of change in FL. Results Among the 136 FLAP patients, 61 achieved mild mitigation of FL, 59 achieved significant mitigation of FL, and 16 experienced aggravation of FL at the ending point of the study. There were significant differences between the three groups in the length of hospital stay（χ²=16.215, P＜0.001) and the change in body weight（F=3.908, P＜0.05), and the FLSMG group had a greater reduction in body weight and a longer length of hospital stay. There were also significant differences between the three groups in the number of fasting days（χ²=11.020, P=0.004) and the degree of changes in C-reactive protein（CRP)（F=8.589, P＜0.001), white blood cell count（WBC)（F=5.448, P=0.005), and CT severity index（CTSI)（F=7.544, P=0.001), and the FLSMG group had greater reductions in CRP, WBC, and CTSI and a longer duration of fasting. Length of hospital stay, number of fasting days, and changes in CRP and CTSI were significantly correlated with the change in FL（r=0.352, 0.372,-0.365, and-0.350, all P＜0.001). Nonalcoholic fatty liver disease, also known as metabolic associated fatty liver disease, is the most common chronic liver disease in the world. Studies have found that nonalcoholic fatty liver disease is associated with an increased risk of cardiovascular disease, and nonalcoholic fatty liver disease itself may be an independent risk factor for cardiovascular disease. In view of the close association between nonalcoholic fatty liver disease and cardiovascular disease, this article reviews the pathophysiological mechanisms linking nonalcoholic fatty liver disease and cardiovascular disease, and provides ideas for the diagnosis and treatment of cardiovascular disease in patients with nonalcoholic fatty liver disease. Objective: To evaluate the efficacy of Gexia Zhuyu Decoction in the treatment of nonalcoholic fatty liver disease (NAFLD). Methods: Four Chinese databases (CNKI, CMB, Wanfang, VIP) and four English databases (PubMed, Embase, Cochrane Library, MEDLINE) were searched to screen randomized controlled trials (RCTs) of Gexia Zhuyu Decoction in the treatment of NAFLD from inception to March 15, 2023. Clinical registration tests and grey literatures were also searched. RCTs screening and relevant data exreacting were based on preset criteria. RevMan5.4.1 software was used for risk of bias assessment and meta analysis of the included studies. Results: Eleven RCTs were included, with 963 patients with NAFLD, including 499 in the treatment group and 494 in the control group. Meta-analysis results showed that Gexia Zhuyu Decoction alone or combined with other medications for the treatment of NFALD was superior to that in the control group in terms of total effective rate [RR=1.23, 95%CI (1.16, 1.31), Z=6.58 (P<0.05)]. Meanwhile, it improve liver function index significantly: ALT[subgroup① WMD=-14.93, 95%CI (-18.66, -11.20), Z=7.84 (P<0.01); subgroup② WMD=-3.97, 95%CI (-8.01, 0.07), AST [subgroup① WMD=-13.25, 95%CI (-18.10, -8.41), Z=5.36 (P<0.05); subgroup② WMD=-2.70, 95%CI (-6.94, 1.54), Z=1.25 (P>0.05)]; GGT[WMD=-9.30, 95%CI (-15.78, -2.81), Z=2.81 (P<0.05)]; Significantly patients with therapy obviously decreased hepatic fibrosis index: IV-C[SMD=-1.15, 95%CI (-1.48, -0.81), Z=6.77 (P<0.05)], LN [SMD= -1.49, 95%CI (-2.23, -0.75), Z=3.97 (P<0.05)]; effectively reduced the level of LDL-C[WMD=-0.30, 95%CI (-0.54, -0.06), Z=2.48 (P<0.05)] and improved the rate of ultrasound renormalization [RR=1.94, 95%CI (1.45, 2.60), Z=4.48 (P<0.05)]. However, existing data cannot prove that the effect of reducing TC, TG and BMI in the treatment group is better than that in the control group: TC[WMD=-0.88, 95%CI (-1.88, 0.11), Z=1.74 (P>0.05)], TG[WMD= -0.50, 95%CI (-1.05, 0.06), Z=1.76 (P>0.05)], BMI[WMD=-1.61, 95%CI (-4.77, 1.54), Z=1.00 (P>0.05)]. Descriptive analysis results showed that the levels of LDL, HA and PC III were significantly reduced in the treatment group, and the differences were statistically significant (P<0.05). In addition, no adverse drug reactions related to Gexia Zhuyu Decoction were reported. Conclusion: Gexia Zhuyu Decoction can significantly improve the liver function, liver fibrosis, and hepatic steatosis in NAFLD patients, thereby improving the clinical efficacy. By analyzing the physiological characteristics of “ liver-blood-heart-vessel” and “ liver heart harmony” in traditional Chinese medicine, it can be seen that “disease involving liver and heart” is more common in clinic, which is the core pathogenesis basis for the clinical prevention and treatment of metabolic-associated fatty liver disease (MAFLD) and atherosclerosis (AS). “ Stagnation of liver and heart, obstruction of heart vessel”, “ deficiency of liver and heart, intermingled phlegm and blood stasis”, and “ disharmony of liver and heart, toxin damaging vessels and collaterals” are the core pathogenesis evolution rules of MAFLD combined with AS, reflecting the transformation process of “ stagnation → deficiency → toxin ”. Combined with the pathological characteristics of cholesterol metabolism disorder and mitochondrial dysfunction and the causal relationship of their cross transformation, the microscopic syndrome differentiation value of the “disease involving liver and heart” in the prevention and treatment of MAFLD combined with AS is observed. The stage-tailored treatment strategies are adopted, such as smoothing liver and regulating qi, regulating heart harmonizing vessel; nourishing liver and protecting heart, resolving phlegm and removing blood stasis; protecting liver and nourishing heart, removing toxicity and dredging collaterals, in order to provide a useful reference for clinical differentiation and treatment of MAFLD combined with AS. Objective: To analyze the independent risk factors for the occurrence of moderate-to-severe metabolic-associated fatty liver disease (MAFLD), to construct a prediction model for moderate-to-severe MAFLD, and to verify the validity of the model. Methods: In the first part, 278 medical examiners who were diagnosed with MAFLD in Medical Examination Center at the Second Affiliated Hospital of Hainan University from January to May 2022 were taken as the study subjects (training set), and they were divided into mild MAFLD group (200) and moderate-severe MAFLD group (78) based on ultrasound results. Demographic data and laboratory indexes were collected, and risk factors were screened by univariate and multifactor analysis. In the second part, a dichotomous logistic regression equation was used to construct a prediction model for moderate-to-severe MAFLD, and the model was visualized in a line graph. In the third part, the MAFLD population (200 people in the external validation set) from our physical examination center from November to December 2022 was collected as the moderate-to-severe MAFLD prediction model, and the risk factors in both groups were compared. The receiver operating characteristic (ROC) curves, calibration curves, and clinical applicability of the model were plotted to represent model discrimination for internal and external validation. Results: The risk factors of moderate-to-severe MAFLD were fasting glucose (FPG), blood uric acid (UA), triglycerides (TG), triglyceride glucose index (TyG), total cholesterol (CHOL), and high-density lipoprotein (HDL-C) . UA [OR=1.021, 95% CI (1.015, 1.027), P<0.001] and FPG [OR=1.575, 95% CI (1.158, 2.143), P=0.004] were independent risk factors for people with moderate to severe MAFLD. The visualized line graph model showed that UA was the factor contributing more to the risk of moderate to severe MAFLD in this model. The ROC curves showed AUC values of 0.870 1, 0.868 6 and 0.799 1 for the training set, internal validation set and external validation set, respectively. The curves almost coincided with the reference line after calibration of the model calibration degree with P>0.05 in Hosmer- Lemeshow test. The decision curve analysis (DCA) plotted by the clinical applicability of the model was higher than the two extreme curves, predicting that patients with moderate to severe MAFLD would benefit from the prediction model. Objective: To investigate the effect of Growth and Differentiation Factor 15 ( GDF15) on alcoholic fatty liver (AFL). Methods: A model of mouse with alcoholic fatty liver disease was constructed with liquor, and then the control group and the model group were injected with AAV8-GDF15 molecules overexpressing the liver in the tail vein, and the mice were divided into four groups-normal + tail vein injection control AAV8-NC group (Con+AAV8-NC), model (alcohol) + tail vein injection AAV8 control virus group (AFL+AAV8-NC), normal + tail vein injection overexpressed AAV8-Gdf15 group (Con+ AAV8-Gdf15), alcohol + tail vein injection overexpressed AAV8-Gdf15 group (AFL+AAV8-Gdf15). By measuring body weight, fasting blood glucose, Intraperitoneal glucose tolerance test (IPGTT), insulin release test (ITT), and using test kits for their serum lipids, liver fats, alanine transaminase (ALT),Determination of aspartate transaminase (AST); liver biopsy sections were detected by hematoxylin-eosinstaining (HE) staining to detect structural abnormalities; The effect of over expression of GDF15 on alcoholic fatty liver was observed via q-PCR detecting of lipid metabolism-related molecular RNA levels, etc. Results: Compared with the Con+AAV8-NC group, weight loss was observed in group AFL+AAV8-NC. The weight loss in the AFL+AAV8-Gdf15 group was reduced compared to AFL+AAV8-NC after over expression of GDF15. Compared with the Con+AAV8-NC group, the fasting blood glucose in the AFL+AAV8-NC group increased, and the sugar tolerance and insulin resistance decreased. After overexpression of GDF15, the AFL+AAV8-Gdf15 group decreased significantly in fasting blood glucose and increased glucose tolerance and insulin resistance compared with AFL+AAV8-NC. The AFL+AAV8-NC group was significantly elevated compared with the Con+AAV8-NC group, and the lipids of the AFL+AAV8-Gdf15 group decreased significantly compared with those in the AFL+AAV8-NC group after overexpression of GDF15. Compared with the Con+AAV8-NC group, the liver weight of the AFL+AAV8-NC group increased, the degree of liver function damage was more severe, and the fat content of the liver increased. After the expression of GDF15, the liver weight, degree of damage and fat content of the liver were significantly reduced in group AFL+AV8-Gdf15 compared with AFL+AAV8-NC group. Conclusion: Alcoholic fatty liver disease increases the expression of GDF15, and increased expression of GDF15 will improve the damage and metabolic abnormalities of alcoholic fatty liver. Objective To explore the status of cardiopulmonary fitness in type 2 diabetes mellitus (T2DM) patients with fatty liver disease and its influencing factors. Methods One hundred and fourteen T2DM patients with fatty liver disease were selected and underwent cardiopulmonary exercise test (CPET). They were divided into the lower group and the higher group according to the median of VO2 peak. The difference of general data and clinical data were compared between the 2 groups. Multivariate Logistic regression analysis was used to analyze the influencing factors of cardiopulmonary fitness in T2DM patients with fatty liver disease. Results The median of VO2 peak was 20.95 (18.38, 23.72) mL·kg-1·min-1 in all 114 patients, accounting for 69% (62%, 81%) of the expected value. Compared with the higher group, patients in the lower group were older, the proportion of men was lower, the height was lower, the waist circumference was higher, the waist hip ratio was higher, the grip strength/weight was lower, the proportion of patients with smoking history was higher, the sedentary time was longer and the average daily activity was lower (P＜0.05). Alanine aminotransferase (ALT), gamma glutamyl transferase (GGT) and triacylglycerol (TG) were increased in the lower group (P＜0.05). Multivariate Logistic regression analysis showed that gender, grip/weight, smoking history and average daily activity were the independent influencing factors of VO2 peak in T2DM patients with fatty liver disease. Conclusion VO2 peak of T2DM patients with fatty liver disease is significantly reduced. Being female and having smoking increase the risk of reduced VO2 peak.
