A 65-year-old woman with a history of bipolar disorder experienced lack of efficacy during treatment for catatonia with amantadine, levodopa/carbidopa, lorazepam, and memantine. Despite receiving multiple treatments, including electroconvulsive therapy, her catatonia remained refractory. Elevated levels of anti-D1 receptor antibodies and anti-tubulin antibodies were found, and she was subsequently treated with immune-globulin and plasmapheresis, which led to improvement.
