Background: Growing evidence suggests the type of periodontal treatment could differentially influence the reduction of key cardiovascular risk mediators in periodontitis patients. This randomized, controlled clinical trial compared the impact of minimally invasive non‐surgical therapy (MINST) with quadrant‐wise subgingival instrumentation (Q‐SI) on C‐reactive protein (CRP) together with lipoprotein‐associated phospholipase A2 (Lp‐PLA2) levels, and clinical periodontal outcomes in patients with periodontitis. Moreover, it was evaluated if baseline CRP levels impacted the efficacy of non‐surgical periodontal therapy protocols. Methods: Forty‐two periodontitis patients were enrolled and randomly treated by means of MINST (n = 21) or Q‐SI (n = 21). The outcomes assessed were serum CRP and Lp‐PLA2, and periodontal parameters (probing depth [PD], clinical attachment level [CAL], full‐mouth bleeding score [FMBS]), at baseline and at follow‐ups at 1, 3, and 6 months and at 1 year after treatment. Results: At 1 year, MINST significantly reduced, among others, mean PD (p = 0.007), mean CAL (p = 0.007), the number of pockets >4 mm (p = 0.011) and ≥6 mm (p = 0.005), and FMBS (p = 0.048) compared to Q‐SI. Generalized multivariate analysis evidenced that high baseline CRP (p = 0.039) and FMBS (p = 0.046) levels, together with MINST treatment (p = 0.007) were significant predictors of PD reduction at 1‐year follow‐up. Moreover, the Jonckheere–Terpstra test showed that patients with high baseline CRP levels gained more benefits from MINST treatment at 1‐year follow‐up than they did from Q‐SI. Conclusion: Patients receiving MINST showed a greater reduction in CRP levels than patients with Q‐SI after 1 year of follow‐up. Moreover, patients with high baseline levels of CRP and Lp‐PLA2 gained more benefits from the MINST approach at 1‐year follow‐up. [ABSTRACT FROM AUTHOR]

Periprosthetic joint infection (PJI) is a devastating complication after total hip and total knee arthroplasty (THA/TKA). While some guidelines no longer recommend routine use of prophylactic antibiotics for dental procedures, many surgeons continue to prescribe antibiotics for their THA/TKA patients. In a setting of increasing antibiotic resistance, it is important to reduce unnecessary antibiotic use. This study aims to evaluate antibiotics prior to dental procedures and the association between dental procedures and PJI. We conducted a retrospective cohort study of patients who underwent THA/TKA between January 1, 2019 and December 31, 2020. The primary outcome was late-presenting PJI, occurring > 90 days after surgery. Patients were designated in the antibiotic group (2,000 mg of amoxicillin) or non-antibiotic group based on their surgeon's prophylaxis protocol. Dental-associated PJIs were considered if the patient had evidence of poor dentition or a recent dental procedure prior to the onset of PJI symptoms. There were 2,871 (26.4%) patients in the no antibiotics group and 8,023 (73.6%) patients in the antibiotics group. We found 27 (0.3%) late-presenting PJIs and 4 dental-associated PJIs. In the univariate and multivariable analyses, body mass index ≥-30 and revision surgery were the only variables that increased the odds of late-presenting PJI. All 4 dental-associated PJIs occurred in patients prescribed antibiotics. We found a low rate of late-presenting PJI. Routine antibiotics prior to dental procedures were not shown to affect the risk of late-presenting PJI. Dentists are an overlooked group of prescribers but are responsible for up to 10% of antibiotic prescriptions written in the United States annually, most of which are for prophylaxis. Dental prophylaxis in orthopedic patients has been an area of confusion, with discordance among societal guidelines. In 2020, an antimicrobial stewardship (AS) group spearheaded a collaborative effort among dental, orthopedic surgery, and infectious diseases specialties to develop a protocol for the use of dental antibiotic prophylaxis for patients with total joint replacements (TJRs) based on the most current American Dental Association-American Academy of Orthopaedic Surgeons statement. This study aimed to assess antibiotic prescribing for dental prophylaxis before and after an AS intervention. This study is an interventional, pre-post review of dental encounters with patients with a history of TJR before (October 2019 to July 2021) and after (August 2021 to April 2023) the intervention. Charts were reviewed to determine the frequency and appropriateness of dental procedure prophylactic antibiotics. A total of 1587 encounters from October 2019 to April 2023 were identified. Notably, 179 encounters in the pre- and 183 encounters in the postgroup were analyzed, 31 encounters (17%) in the pregroup versus 33 (18%) in the postgroup had a prophylactic antibiotic associated with it (P > 0.05), 26 of 31 antibiotics (84%) in the pregroup were prescribed for prosthetic joint infection prophylaxis compared with 18 of 33 (55%) in the postgroup (P > 0.05), and 151 of 179 encounters (84.3%) were guideline adherent in the preintervention group compared with 160 of 183 encounters (87.4%) in the postintervention group (P > 0.05). This study observed a shift in the distribution of antibiotic indications and a decreased rate of inappropriate prophylactic prescriptions in dental patients with a history of TJR after an AS-led, collaborative intervention. Although the overall rate of prophylactic antibiotic prescribing remained the same and the changes did not reach statistical significance, lessons learned through this process can help inform future interventions in our institution and for others. To evaluate the effectiveness of photobiomodulation therapy (PBMT) in reducing gingivitis following professional dental prophylaxis in dogs in a prospective, randomly controlled, double-blind study. The study included 47 dogs (male n = 23, female n = 24), with a mean age of 6.5 ± 2.6 years and a bodyweight of 28.9 ± 5.3 kg. Breeds included: Belgian Malinois Shepherd Dogs (n = 23), German Shepherd Dogs (n = 14), Labrador Retriever (n = 4), and Dutch Shepherd Dogs (n = 3). The left side of the mouth was treated by performing dental prophylaxis (control group). The right side received dental prophylaxis and a single session of PBMT using a Class IV therapeutic laser (treatment group). Each side was graded individually for periodontal disease (PD), gingivitis, and calculus. Follow-up observations were conducted on days 1, 3, 8, and 15 post-treatment. Results were compared using a Mann-Whitney Test. Multiple regression was run to predict PD, gingivitis, and calculus scores using age, sex, and breed. A p <.05 was considered significant. The PBMT group (PG) showed significantly lower gingivitis scores from day 1 to 15 post-treatment. PD had an increasing prevalence with age. Age and breed contributed to the prediction of PD, gingivitis, and calculus grading. This study showed that PBMT significantly reduced the grade of gingivitis following dental prophylaxis, and may be a useful adjuvant procedure following dental prophylaxis. [ABSTRACT FROM AUTHOR]

Preserving the outcome of orthodontic treatment is both important and challenging. However, there is insufficient evidence regarding the best way to ensure long-term treatment outcome. The aim of this study was to evaluate a pre-fabricated chain retainer (PFCR) in terms of: ability to maintain satisfactory treatment outcomes; periodontal and dental health; complications; and patient satisfaction. Overall, 130 patients who had completed orthodontic treatment with a fixed appliance during the period 2016–2019 (follow-up time range, 24–55 months) at a specialist orthodontic clinic in Varberg, Sweden and who had a PFCR in the lower jaw were invited to take part in the study. Little's irregularity index (LII) was recorded on dental casts. Caries, gingivitis, calculus, probing pocket depth, and gingival retractions were registered during clinical examinations. Patient satisfaction and retainer complications were evaluated using a questionnaire. In total, 76 patients (58.5%) agreed to participate. All patients, except for one, had their retainer still in place, and the complication rate was 40%. The LII scores were in the range of 0–4 mm (mean, 1.42 mm). At the retainer site, 82% had calculus, 74% had gingivitis, 1% had pocket depth >4 mm, 10.5% had gingival retractions >2 mm, and 0% had caries. All the patients expressed satisfaction with their retainer. PFCRs exhibit characteristics similar to those of traditional bonded retainers in terms of complications, stability, side-effects and patient experience. Purpose: To evaluate the clinical performance of two proposed onlay designs. Methods: Molars with occlusal and/or mesial/distal defects after root canal treatment were classified by design into three groups. Onlays without shoulders were the control group (Group C, n = 50). The designed onlays were Group O (n = 50) and the designed mesio-occlusal/disto-occlusal onlays were Group MO/DO (n = 80). All onlays had an occlusal thickness of approximately 1.5-2.0 mm, and the designed onlays had a shoulder depth and width of approximately 1 mm. In Groups C and O, the box-shaped retention was 1.5 mm in depth. The proximal box was connected with a dovetail retention in Group MO/DO. Patients were examined every 6 months and followed for 36 months. Restorations were evaluated by using the modified United States Public Health Service Criteria. Statistical analysis was performed by using Kaplan-Meier analysis, the chi-square test, and Fisher’s exact test. Results: No tooth fracture, debonding, secondary caries, or gingivitis was observed in any group. Groups O and MO/DO had satisfactory survival and success rates, and there was no significant difference in performance characteristics among the three groups (P > 0.05). The aim of this in vitro study was to compare the efficacy and durability of bleaching with diode lasers over a 9-month period. This study evaluated 27 extracted teeth categorized into 3 bleaching groups (n = 9): 940-nm diode laser, 980-nm diode laser, and 40% hydrogen peroxide whitening gel (control). After the baseline color of the teeth was measured using a spectrophotometer, the teeth underwent dental prophylaxis and their color was measured again. Bleaching treatment was then performed. The color of the teeth was measured Immediately after bleaching and 1 day (24 hours), 1 week, 1 month, 3 months, 6 months, and 9 months after bleaching. The Commission Internationale de l'Eclairage (CIE) L'a'b* color change (ΔE*) at each timepoint was calculated and compared using repeated-measures analysis of variance. All bleaching techniques effectively whitened the teeth to a clinically perceptible level (ΔE* > 3.3), and there were no statistically significant differences among the groups (P > 0.05). The mean ΔE* values of the laser groups were not significantly different from the mean value of the control group during the 9-month evaluation period (P > 0.05). However, statistically significant differences among the groups were noted when color measurements at the following timepoints were compared: immediately after bleaching vs 1 day; 1 day vs 1 week; and 3 months vs 6 months (P < 0.05). Use of a 940-nm diode laser, 980-nm diode laser, or conventional hydrogen peroxide gel provided equally effective bleaching. Transpositions are rare anomalies where there is an exchange of positions between two adjacent teeth. Treatment, ideally involves alignment of teeth, consideration for aesthetic, social and functional purposes. This article/case report describes transposition in two sisters of age 11 and 14 year and discusses the treatment possibilities for aesthetic and functional needs. The treatment considerations encompass comprehensive caries management: restorative and endodontic treatments including a gutta percha obturation of a pulpally involved primary molar for delaying the resorption, preventive care and regular follow-ups until definitive treatment. Periodontal disease has been identified in horses and donkeys through the millennia at a prevalence of up to 75% and is a leading cause of tooth loss in horses. We hypothesize that there is a correlation between tooth elongations and periodontal disease. Dental records of all client owned horses examined by a single AVDC/equine board certified practitioner in 2016-2018 were analyzed. Inclusion criteria consisted of the examination finding of tooth elongation of one of the following teeth pairs 106/107, 206/207, 310/311, or 410-411. The included cases were then examined for the finding of periodontal disease in Stages 1-4 (PD 1-4). Bayesian correlation analysis revealed significant positive correlations of elongations and periodontal disease existed between PD1 and proximal tooth pairs 106-107 and 206-207, PD2 and PD4 with all four tooth pairs, and PD3 with 106-107. Clinically, early diagnosis and treatment of newly formed elongations, or prevention of their formation altogether through diligent dental prophylaxis could prove key in preservation of teeth through prevention of the associated development of periodontal disease in the horse. Background: The parenting style is shifting universally. Children lack the skills needed to deal with life stressful situations like those present in dental setting. Previous studies showed a correlation between parenting style and the child's dental behaviour. No studies have been performed in the Kingdom of Saudi Arabia (KSA) to relate parenting style to child's dental behaviour. The purpose of this study is to correlate parenting styles with the dental behaviour of their children, in Riyadh, KSA. Methodology: Two hundred and eighty-two healthy preschool children aged 3–6 years were recruited in their first dental visit, to a postgraduate Paediatric Dentistry Clinics of the Dental University Hospital at KSU, Riyadh, KSA. The Parenting Styles and Dimensions Questionnaire was completed by the primary caregiver/parents. The behaviour of the children was evaluated after dental prophylaxis and fluoride application in the dental chair using Frankl behavior rating scale. Family and child demographic data were evaluated through a questionnaire. Results: Two hundred and eighty-two healthy Saudi children participated in the study with their parents. Two parenting styles were identified among Saudi parents including authoritative (94%, n = 265) and permissive parenting styles (6%, n = 17). Most of the children behaved positively (n = 205, 72.7%). Significant correlations were detected between parenting style and parent's dental anxiety (P value 0.02) but not significantly correlated to a child's dental behaviuor. Conclusion: In the KSU sample studied, two parenting styles were identified among Saudi parents, authoritative and permissive parenting styles. Parenting style was correlated to parental dental anxiety and had an influence on a child's dental behaviour. Aim: To evaluate the heart rate (HR) and behaviour of children and teenagers with Cerebral Palsy (CP) when having a dental appointment. Methods: A comparative cross-sectional study was carried out with 60 participants with CP, between 2 and 14 years old (study group—SG), and 60 normotypical individuals (CG). The sample was paired according to age, gender and socioeconomic status. Behaviour was evaluated during dental prophylaxis using the Frankl Scale, and HR was measured at five moments: before the appointment, when sitting in the dental chair, during the clinical examination, during prophylaxis and immediately after prophylaxis. Mann–Whitney, Friedman and Chi-square tests were applied at a significance level of 5%. Results: SG presented significantly higher HR (p < 0.001) and more participants with uncooperative behaviour (p < 0.001) than CG in all observational periods. Furthermore, SG participants with uncooperative behaviour presented higher HR values than those in SG who were cooperative in all observational periods (p ≤ 0.002). Conclusions: Individuals with CP have a higher HR before and during the clinical session, and are frequently more uncooperative with the procedure than normotypical individuals. Statement of the Problem: Modeling is one of the non-pharmacological approaches to manage anxiety behavior and encourage children’s cooperation in dentistry. This method is based on social learning theory in which the children learn the skills of overcoming anxiety and adapting their skills and behaviors during dental treatment. Purpose:The aim of this study was to evaluate the effect of an animated-movie modeling approach, named Jilo, on cooperation and anxiety of children in comparison with the conventional Tell-Show-Do (TSD) method in a dental setting. Materials and Method:In this randomized controlled trial, 50 healthy children (aged 4-6) with no history of dental treatment were recruited and randomly allocated to experimental (n= 24) and control (n= 24) groups. During the first visit, the experimental group watched the Jilo animated movie. After a 30-minute break, they received prophylaxis followed by fluoride therapy. The control group received the conventional TSD technique and then underwent prophylaxis and fluoride therapy. One week later, a dental restoration, which required mandibular nerve block anesthesia, was performed for both groups. The cooperation and anxiety levels of patients were evaluated during two visits using Venham Clinical Cooperation Scale (VCCS) and Venham Clinical Anxiety Scale (VCAS). Results:The mean VCAS in the experimental group was significantly lower compared to the control group during the first and second visits (p = 0.008 and p = 0.044, respectively). The mean VCCS was also significantly lower in the experimental group during the first (p = 0.015) and second visits (p = 0.019) compared to the control group. Background: Hereditary angioedema (HAE) is a rare disease that affects 1 in 60,000; however, despite being extremely rare, the severity of the disease can cause significant limitations to quality of life. In addition, attacks can be fatal and require urgent care. Methods: We searched PubMed and Google for Hereditary Angioedema and prophylaxis, short term prophylaxis, surgery, medical procedures, dental work, triggers. Results: The main triggers are estrogens, Angiotensin Converting Enzyme Inhibitors (ACI) inhibitors, trauma, dental work, stress, surgery, manipulation of the upper airway, and medical procedures. Prophylaxis is often used long term to prevent attacks; before known triggers, prophylaxis is referred to as short-term prophylaxis (STP). When to initiate STP, what to use, and what dose to use have not been adequately researched, but there is consensus that, whenever the upper airway is manipulated, STP is essential. In addition, consensus has been reached that an IV C1 inhibitor is the preferred STP agent, and it is my opinion that dosing at 20 units/kg allows dosing for all ages and also allows average-size adults to receive >1000 units because failures at 1000 units have been documented in the literature. Conclusions: This article focused on triggers and preprocedural STP and not on pre-event STP, which is often used before important life events; however, medications and dosing are the same for pre-event prophylaxis. Noonan syndrome (NS) is a relatively common congenital multiple-anomaly syndrome, resembling Turner syndrome, but without chromosomal anomaly. Besides the unusual facies, the maxillofacial and dental features of patients with NS are not well-summarized in the literature. The aim of this study was to describe these features and propose specific treatment guidelines for practitioners involved in oral and maxillofacial care. A retrospective multicentric study was conducted of 14 patients who were referred for NS screening. In total, 10 patients were found to carry a mutation involved in NS or NS-related disorders. Fifty percent of the mutations affected PTPN11. All patients presented with the typical extraoral features, such as macrocephaly, hypertelorism, ptosis, triangular face shape and ear dystrophy. Intraoral manifestations, including malocclusion (maxillary transversal deficiency, crossbite, anterior open-bite and class II malocclusion), dental anomalies (delayed eruption, agenesis and dystrophy, odontoma) and radiologic jaw lesions were identified in five out of 10 patients. These findings were searched in a review of the literature to obtain a comprehensive description of oral and maxillofacial features in patients with NS. The proposed treatment guidelines emphasize frequent coagulation anomalies that need to be considered prior to surgery. Early dental assessment and yearly follow-up with oral prophylaxis are recommended. Orthodontics and orthognathic surgery are also of primary importance in the management of NS patients. Plasma cell gingivitis (PCG) is characterised by a heavy plasma cell infiltration into the gingival connective tissue. It has been associated with the use of flavouring agents like cinnamon and mint in candy, chewing gum and herbal toothpastes. PCG secondary to khat (Catha edulis) chewing has recently been recognised. Khat is a psycho-stimulating herb that is cultivated and used in East Africa and the Arabian Peninsula. It produces a stimulating and euphoric effect on its users. Plasma cell gingivitis in a khat user is presented. A 40-year-old male presented with a generalised diffuse erythema and painful gingival enlargement. He also reported bleeding upon eating and brushing. Generalised periodontal pocketing and moderate to severe attachment loss were detected on probing. Radiographs revealed generalised alveolar bone loss. A gingival biopsy revealed a dense infiltrate of plasma cells in the connective tissue. A histopathological diagnosis of plasma cell gingivitis was made. It was revealed that the patient was a Yemeni immigrant and a habitual khat chewer for the last 10 years. A marked improvement in gingival health was noted a few weeks following cessation of the khat chewing habit. Gingival erythema and swelling further diminished following initiation of oral prophylaxis. A diagnosis of plasma cell gingivitis associated with khat use was made. A worldwide population of immigrants habituated to khat chewing could potentially increase the incidence of plasma cell gingivitis. Bacteremia is a common occurrence in patients with periodontal diseases, even simple gingivitis. Tooth brushing, especially improper and/or overzealous tooth brushing, routinely occasions a significantly perceptible elevation in the quantity of oral-origin microorganisms in the circulating blood. Initial therapy, whether a routine prophylaxis in a gingivitis case, or scaling and root planing in a periodontitis patient, always leads to bacteremia, often massive; hitherto, most such bacteremias have been assumed to be both transient and inconsequential for the patient's systemic health. This assumption is no longer tenable in the light of new clinical and scientific knowledge. The purposes of the present article are: 1) to critically review the contemporary use of antimicrobial agents in dental and dental hygiene practice in the USA and internationally; 2) to revisit and review the peculiarities that characterize inflammatory gingival/periodontal disorders which may demand more aggressive use of antimicrobial agents now and in the future; and 3) to conceptualize the emerging treatment concepts that are targeted towards treatment of gingival/periodontal diseases as acutely contagious clinical entities. Aim: For orthodontic patients, optimal oral prophylaxis is essential. In this article, a classification system for toothbrushing habits, according to three criteria that can indicate the direction of guidance for primary school children, is discussed. Methods: Toothbrushing habits of children are evaluated according to three criteria (frequency of toothbrushing, attitude toward voluntary toothbrushing, and thoroughness of toothbrushing). Computer-generated classification graphs, performance tables, and component tables were obtained based on the responses to 26 questions. Results: The children can be classified into different types, according to the three criteria, on the graphs. Performance tables indicate the items that require guidance, while component tables indicate the priority of the items that require guidance. Purpose: The evaluation of the efficacy of diode laser treatment in cases of periimplantitis was the goal of this clinical research as illustrated by a case report. Materials and Methods: In addition to an oral hygiene session and one week of local prophylaxis with 0.12% chlorhexidine, each of 21 patients (bearing 23 implants) received initial treatment with ultrasonic debridement and diode laser (wavelength 810 nm) decontamination (1 to 1.5 W for 20 s, continuous wave mode) of the implant surfaces and periimplantits defects. After one week, each patient underwent flap surgery with a second, deeper ultrasound and laser decontamination, and guided bone regeneration (GBR) with purified and deproteinized bovine bone and a collagen membrane. A case report is given of one 73-year-old male patient. Results: This case, like the other 20, presented resolution of the periimplant mucositis and good improvement of the PAL index, also after 12 months. Conclusions: Laser therapy was found to efficiently reduce the bacterial contamination and inflammation. This bactericidal effect is fundamental to obtaining a good prognosis for GBR therapy. Further clinical and animal research is necessary to better evaluate all the effects of the diode laser on the bone and the soft tissue around implants and its postulated capacity to promote the new bone formation. To evaluate the effect of two different prophylaxis protocols on the friction force in sliding mechanics during in vivo leveling and alignment. The sample comprised 48 hemi-arches divided into three groups according to the prophylactic protocol adopted. Group 1 consisted of patients undergoing prophylaxis with sodium bicarbonate, group 2 consisted of patients submitted to prophylaxis with glycine, and group 3 consisted of patients without prophylaxis, as a control. All patients received hygiene instructions and, with the exception of group 3, prophylaxis was performed monthly. After 10 months, the brackets were removed from the oral cavity and submitted to friction force tests and qualitative analysis by scanning electron microscopy. Analysis of variance followed by Tukey tests was performed for intergroup comparison regarding the friction force. The experimental groups presented significantly smaller friction forces than the group without prophylaxis. Accordingly, qualitative analysis showed greater debris accumulation in the group without the prophylactic procedures. Prophylactic blasting with sodium bicarbonate or glycine can significantly prevent an increase of the friction force during sliding mechanics. [ABSTRACT FROM AUTHOR]

The purpose of this study was to develop effective dental hygiene chews for cats based on the anatomical features of the dentition and patterns of chewing motion. All cats were volunteered for dental prophylaxis followed by dental impressions using yellow stone and alginate under general anesthesia. Twenty parameters related to dentition were defined in order to compare dental impressions using digital caliper. For the chewing motion study, patterns of chewing motion were identified based on recordings made with a digital camera. Ten cats (4 domestic shorthairs, 2 Russian blues, 1 American shorthair, 1 Persian, 1 Turkish Angora, and 1 Devon Rex) were recruited for the study. The parameters related to teeth and oral size were similar among the studied cats. Chewing motion can be described as more of a guillotine-like motion rather than a crushing motion, with cats chewing 3-7 times before swallowing. The chewing pattern of cats involves shearing for a short period of time followed by immediate swallowing. Therefore, the overall size of the dental hygiene chew could be determined based on the measurement of the oral size for inducing chewing. The surface details of the dental hygiene chew could be designed for prevention and removal of dental calculus and plaque in cats considering the anatomical teeth parameters. To assess plaque and gingivitis reduction in orthodontic patients after 4 weeks' use of an oscillating-rotating power brush, irrigator, and mouthrinse. This was a randomized, examiner-blind, clinical trial comparing plaque and gingivitis outcomes for an experimental power brush/irrigator/mouthrinse oral hygiene routine vs a dental prophylaxis followed by regular manual brushing (positive control). Fifty-one participants with fixed orthodontic appliances in the upper and lower jaw and a minimum of 15 gingival bleeding sites were randomly assigned to experimental or positive control treatment. Both groups were instructed to use their products at least twice daily. At baseline, week 1, and week 4, plaque was evaluated using digital plaque imaging analysis and a conventional subjective index. Gingival inflammation and bleeding were also measured. Analysis of covariance was used to compare groups. Fifty-one participants (mean age = 13.9 years) were randomized; 50 (25 per group) completed the study. At baseline, group means were not statistically different (P >.1) for gingival inflammation or bleeding. At week 4, the experimental and control groups had a 10.0% to 32.7% and 5.9% to 6.7% reduction vs baseline, respectively, in plaque (across both methods); 12.6% and 8.3% reduction, respectively, in gingival inflammation; and 50.6% and 37.8% reduction, respectively, in bleeding. At week 4, group differences favoring the experimental group were statistically significant (P <.05) for gingival inflammation, gingival bleeding, and plaque (by conventional and digital imaging indexes). Use of a power brush/irrigator/mouthrinse resulted in statistically significantly greater plaque and gingivitis reductions than prophylaxis followed by manual brushing in patients with fixed appliances over 4 weeks. A recurrent unilateral headache which occurs episodically. Males are affected predominantly. Initially occurring for a few weeks followed by a remission. The condition often becomes chronic, occurring daily for months or years. Associated phenomena such as epiphora, Homer's syndrome, and unilateral facial swelling indicate disturbances of the autonomic nervous system. Attacks are associated with intracranial dilatation and possibly vasoconstriction in the carotid siphon. The diagnosis is essentially clinical. Treatment can be prophylactic with lithium, methysergide, or calcium antagonists. Symptomatic relief may be achieved with oxygen inhalation or fast-acting ergot preparations. Migraine occurs in several different forms. The headache may be accompanied by minimal neurologic symptoms (common, migraine) or with conspicuous neurologic complaints (classical migraine) and may occur with inconspicuous headache (migraine equivalents). Vasodilatation in the distribution of the external carotid artery must be responsible for the pain. Primary neuronal disturbances may result in secondary changes in the cranial circulation. Purpose: To monitor extrinsic stain formation over 24 weeks with twice-daily use of an anhydrous 0.454% SnF2 dentifrice containing 5% sodium tripolyphosphate (STP) ('Test'), compared to a standard fluoride dentifrice (0.76% fluoride as sodium monofluorophosphate [SMFP]) ('Comparator') following a professional prophylaxis. Methods: Primary efficacy variables were between-treatment differences in extrinsic dental stain of anterior teeth after 4, 8, 12, and 24 weeks' use using mean area and intensity scores of the MacPherson modification of the Lobene stain index (MLSI), and to monitor oral tolerability. Results: Overall, 214 subjects were randomized to treatment. The Test dentifrice group demonstrated statistically significantly less stain compared with the Comparator at each timepoint for all outcome variables (MLSI - Area x Intensity, Area only, and Intensity only) with the exception of MLSI (Area x Intensity) at Week 12. Products were generally well tolerated with 26 treatment-related adverse events (TRAEs) reported (10 with Test, 16 with Comparator). Five subjects withdrew from the 24-week study due to TRAEs, three in the Test group, two in the Comparator group. Statistically significantly less anterior tooth staining was observed with up to 24 weeks twice-daily brushing with a 0.454% SnF2/5% STP anhydrous dentifrice compared to a marketed fluoride dentifrice with 0.76% SMFP. Aim: To evaluate glycosylated hemoglobin (HbA1c) before and after non-surgical periodontal treatment in patients with type 2 diabetes mellitus (DM-2). Methods: Twenty subjects diagnosed with periodontitis and DM-2 were treated using an NSPT protocol. Periodontal examination and blood measurements were performed at baseline and after three months. Patients with DM-2 treated for at least a year, with at least 10 teeth and with probing depths between 4-6 mm in more than three regions were included. The variables evaluated were HbA1c in blood and periodontal measures (probing depths, insertion level, gingival bleeding on probing, dental plaque, calculus, inflammation, clinical attachment and mobility). All patients were informed of the conditions of the therapy used. Scaling and root planning (SRP) of the full mouth was performed using an ultrasonic scaler and hand instrument under local anesthesia, supragingival prophylaxis and oral hygiene instruction. Also, 0.12% chlorhexidine digluconate was formulated twice a day for two weeks. Statistical analyses were performed using StataIC 14. The values are shown as the mean, median and standard deviation (SD) or interquartile rank (IR), and p<0.05 was considered significant. Results: Twenty subjects completed the three-month follow-up and were included in the analysis. Three months after the non-surgical periodontal treatment, most periodontal parameters had a meaningful reduction (p< 0.05) and a large effect size >0.8. Clinical attachment level showed no improvement. The HbA1c values were not significantly decreased (p=0.94). Conclusions: Although non-surgical periodontal therapy eliminates local inflammation, it is insufficient to significantly reduce HbA1c levels in a short time period. [ABSTRACT FROM AUTHOR]

This study assessed the blood pressure (BP) of normotensive subjects and subjects with pharmacologically controlled hypertension after sodium bicarbonate jet prophylaxis. Forty subjects were divided into 2 groups: a normotensive control group (n = 20) and a hypertensive group (n = 20). Blood pressure measurements were conducted at 4 timepoints: prior to the dental prophylaxis (TO), immediately after treatment (Ti), 15 minutes after treatment (T15), and 30 minutes after treatment (T30). The systolic BP (SBP) values for both groups were significantly increased at Ti (P < 0.05) and returned to their initial state at T15. Both groups also showed a significant increase in diastolic BP (DBP) values at Ti (P < 0.05); however, the basal conditions in hypertensive subjects were not restored until T30, whereas the values for normotensive subjects were restored at T15. The results indicated that systemic BP changed significantly after sodium bicarbonate jet prophylaxis in both study groups; while initial SBP values were restored by 15 minutes in both groups, the return to initial DBP values took longer in the hypertensive group than in the normotensive group. Periodontal disease is extremely common in companion animal practice. Patients presenting for a routine oral examination and prophylaxis may be found to have extensive periodontal disease and attachment loss. Vertical bone loss is a known sequela to periodontal disease and commonly involves the distal root of the mandibular first molar. This case report outlines two dogs presenting for oral examination and prophylaxis with general anesthesia. Both patients did not have any clinical symptoms of periodontal disease other than halitosis. Both patients were diagnosed with three-walled vertical bone loss defects of one or both mandibular first molars utilizing dental radiography as well as periodontal probing, measuring, and direct visual inspection. These defects were consistent with periodontal disease index stage 4 (>50% attachment loss). The lesions were treated with appropriate root planing and debridement. Bone augmentation products readily available and marketed for veterinary use were then utilized to fill the defects to promote both the re-establishment of normal alveolar bone height and periodontal ligament reattachment to the treated surface. Follow-up assessment and owner dedication is critical to treatment outcome. Background: This study was aimed at assessing the level of knowledge, attitude and practice of dental surgeons on human immunodeficiency virus (HIV) and hepatitis B virus (HBV) post-exposure prophylaxis (PEP). Methods: A descriptive, cross-sectional study was conducted among all the consenting dental surgeons at the University of Benin Teaching Hospital, Benin City, Nigeria, between May and July, 2015. The survey was carried out using a self-administered, anonymous, twenty-five item, and structured questionnaire. Results: A total of 40 (74.1%) males and 14 (25.9%) females responded in the study. The majority, 37 (68.5%), of the dental surgeons had inadequate knowledge about PEP. Majority of the dental surgeons, 44 (81.5%), had a good attitude towards PEP for HIV/HBV. From the study participants, 52 (96.3%) agreed on the importance of PEP for HIV/HBV in the work place. A high proportion of the respondents, 47 (87.0%), had been vaccinated against HBV. Among all of the respondents, 25 (46.3%) were exposed to HIV/HBV risky conditions. Sixteen of the 25 exposed dental surgeons, 16/25 (64.0%), did not take PEP. Among the respondents who did not take PEP, 15 of those 16 (93.6%) stated that their reason for not taking PEP was due to fear of stigma and discrimination. Conclusion: This study shows that knowledge and practice of dental surgeons towards PEP for HIV/HBV is inadequate. This study aimed at comparing the antiplaque and antigingivitis potential of a mouth rinse containing Essential Oil, zinc and fluoride with a CPC-containing mouth rinse over a short term period. This study is a double blind randomized cross over clinical trial with a 3 days run in phase (wash out period). A non-brushing 3 days intervention was conducted with 18 females with mean age of 21 years old and a single examiner throughout the study. Prophylaxis was done after each intervention. Four mouth rinse were tested on 4 groups; 1: negative control, 2: mouthwash with alcohol, 3: mouthwash with CPC 0.1%, and 4: mouthwash with CPC 0.07%. Oral hygiene scores which consist of Plaque Index (PI), Gingival Index (GI), and bacterial load measurement using CariScreen were taken and analyzed. The result shows that the most effective to the less effective mouth rinse in inhibiting bacterial built up (antiplaque) are mouthwash 2, 3, 4 and 1 respectively. Chediak-Higashi syndrome (CHS) is an autosomal recessive disorder characterized by leukocytes with giant secretory granules and a myriad of clinical features. However, it is unknown whether oral lesions are part of the syndrome or are refractory to systemic treatment. Herein, we integrated the available data published in the literature on the oral manifestations of individuals with CHS. Searches on PubMed, Web of Science, Embase, Scopus, and LILACS were conducted to identify studies published up to March/2022. The Joanna Briggs Institute tool was used for the critical appraisal of studies. Fourteen articles (21 cases) were detected. The mean age of individuals was 15.9±8.8 years. There was a slight predominance of males (52.4%). The major manifestation was periodontal disease (81%), although ulceration of the oral mucosa (14.3%), gingival/labial abscess (4.8%), and periodontal abscess (4.8%) were also reported. Oral rehabilitation including dental implants (9.5%) was performed after tooth losses due to the poor prognosis of periodontal therapy. CHS is usually diagnosed in an early stage due to its systemic manifestations such as classic oculocutaneous albinism, recurrent infections, and a propensity for bleeding. Background: With declining general health, the maintenance of oral health becomes increasingly difficult. Unfortunately, health care resources are limited. An assessment tool for detecting a patient's need for oral health care assistance would promote its adequate distribution. The aim of this study is to evaluate if the Shulman clock-drawing test score is a suitable assessment tool for determining the need for oral hygiene assistance.Methods: Data collected from recent dental checkups of 126 inhabitants of five Austrian residential care facilities were analyzed. The Shulman clock-drawing test score was used as a measure for cognitive abilities and related to tooth and denture hygiene indices, which were assessed using the approximal space plaque index (API) and the denture plaque index (DPI).Results: Shulman score values were distributed almost evenly in the study population. The mean API was 83.6% (SD 20.2%) for the maxilla and 94.9% (SD 15.6%) for the mandible. The mean DPI was 43.9% (SD 31.4%). Cognitive impairment was not statistically related to the dental or denture hygiene index.Conclusions: A high Shulman clock-drawing test score indicates the need for oral hygiene assistance, but it is not suitable as a single tool to assess this need. Patients with other disabilities might equally qualify for assistance. Dentures, however, can be cleaned quickly and efficiently independently of patient compliance; therefore, cleaning the patients' dentures should be part of nursing personnel's daily care routine for every patient. Background/purpose: No data has been available on prophylaxis for stress ulcer development during the process of weaning patients off mechanical ventilators. We conducted a randomized study to evaluate the efficacy of stress ulcer prophylaxis with lansoprazole OD in patients being weaned from mechanical ventilators.Methods: A total of 120 patients were randomly allocated into two groups using blocked randomization, with 60 patients in each group. Group A was the treatment group, receiving lansoprazole OD 30 mg from a nasogastric tube for 14 days, while Group B, the control group, received no proton pump inhibitors or other medications for treating peptic ulcers. The primary end point of our study was apparent upper gastrointestinal bleeding within 2 weeks of enrollment.Results: Apparent upper gastrointestinal bleeding occurred in zero patients and five patients in Groups A and B, respectively (Group A: 0% vs. Group B: 8.3%, p = 0.057). There was no significant difference between the two groups in ventilator-associated pneumonia (Group A: 6.7% vs. Group B: 10.0%, p = 0.509) and 30-day survival rates (Group A: 96.7% vs. Group B: 100%, p = 0.496).Conclusion: Stress ulcer prophylaxis with lansoprazole in patients being weaned from mechanical ventilators led to a lower but not statistically significant incidence of apparent upper gastrointestinal bleeding. There was no significant increase of incidence of ventilator-associated pneumonia in the prophylaxis group. Objectives A systematic review of the literature was undertaken to assess the evidence to support a specific time interval between periodontal maintenance (PM) visits. Methods Relevant articles were identified through searches in MEDLINE, EMBASE and PubMed using specific search terms, until April, 2014, resulting in 1095 abstracts and/or titles with possible relevance. Critical Appraisal Skills Programme (CASP) guidelines were used to evaluate the strength of studies and synthesize findings. If mean recall interval was not reported for study groups, authors were contacted to attempt to retrieve this information. Results Eight cohort studies met the inclusion criteria. No randomized control trials were found. All included studies assessed the effect of PM recall intervals in terms of compliance with a recommended regimen (3–6 months) as a primary outcome. Shorter PM intervals (3–6 months) favored more teeth retention but also statistically insignificant differences between RC and IC/EC, or converse findings are also found. In the 2 studies reporting mean recall interval in groups, significant tooth loss differences were noted as the interval neared the 12 month limit. Conclusions Evidence for a specific recall interval (e.g. every 3 months) for all patients following periodontal therapy is weak. Further studies, such as RCTs or large electronic database evaluations would be appropriate. The merits of risk-based recommendations over fixed recall interval regimens should be explored. We investigated the effect of antibiotic prophylaxis on postoperative inflammatory complications after operations for impacted mandibular third molars in Chinese patients. A total of 207 patients had their bilateral third molars removed in a split-mouth, double-blind, self-controlled, clinical trial in two visits. For one side amoxicillin (or clindamycin) was given (antibiotic group) from one hour before operation until 3 days postoperatively. For the other side a placebo was given (placebo group) at the same time. The outcome, including alveolar osteitis, surgical wound infection, prebuccal infection, and infection of the anterior isthmus of fauces, was assessed 2 and 10 days postoperatively. A total of 192 patients completed the study, and there was no difference between the groups in the incidence of inflammatory complications. In the treatment group, there were 4 cases of alveolar osteitis (2%), 2 infections of the wound (1%), and 14 other reactions (gastrointestinal ( n = 4), bleeding ( n = 2), ulcer ( n = 2), and fever ( n = 6)). In the placebo group, there were 6 cases of alveolar osteitis (3%), 2 wound infections (1%), and 22 other reactions (bleeding ( n = 6), ulcer ( n = 2) and fever ( n = 14)). There was no significant difference in the extraction time and postoperative reactions, except the pain score on day 10 ( p = 0.005). Prophylactic amoxicillin (or clindamycin) is not effective for the prevention or reduction of postoperative inflammatory complications after the removal of impacted mandibular third molars in Chinese patients.
