2. Second generation multiple channeling using platelet-rich plasma enhances cartilage repair through recruitment of endogenous MSCs in bone marrow

- Abstract
In the treatment of cartilage defects, a key factor is the adequate and specific recruitment of endogenous stem cells to the site of injury. However, the limited quantity and capability of endogenous bone marrow stem cells (BM MSCs) often result in the formation of fibrocartilage when using bone marrow stimulation (BMS) procedures. We engineered second-generation platelet-rich plasma (2G PRP) with defibrinogenating and antifibrinolytic agents for injection into the condyle of the right femur, followed by multiple channeling (MCh) 5 days later. This approach aims to enhance repair by promoting the local proliferation and migration of BM MSCs to the full-thickness knee cartilage defect (ftKD). In our in vitro study, 2G PRP increased the number of endogenous BM MSCs and their ability to migrate toward an IL-1β-induced inflammatory condition. This significance was further confirmed by in vivo proliferation results after injection of 2G PRP into the condyle of rats. Fifty-four healthy male Sprague-Dawley rats were divided into 3 groups (ftKD, MCh, 2G MCh) for 3 time points (2 weeks, 4 weeks, 8 weeks). The 2G MCh (2G PRP injection + MCh) groups significantly improved cartilage formation at 4 and 8 weeks compared to the ftKD and MCh groups. The 2G MCh initiated cartilage repair earlier than MCh and significantly enhanced up to 8 weeks. This study demonstrated that 2G PRP increased the number of BM MSCs through the enhancement of proliferation and recruitment into the injured site, thereby improving articular cartilage repair.Graphical Abstract

4. Pulmonary Artery Measurements as Postnatal Prognostic Tool in Right Congenital Diaphragmatic Hernia.

- Abstract
Right-sided congenital diaphragmatic hernia (RCDH) is a rare and often fatal congenital anomaly, primarily attributed to lung hypoplasia, which is associated with small branch pulmonary artery (PA). This study investigated whether postnatal PA measurements obtained through echocardiography are associated with mortality or the extracorporeal membrane oxygenation (ECMO) requirement in neonates with RCDH. A retrospective study was conducted on neonates with RCDH born between 2008 and 2022. Echocardiography was performed on the day of birth. The diameter of the main PA (MPA) was measured at the maximal dimension, and the diameters of the left PA (LPA) and right PA (RPA) were measured at the bifurcation. The primary outcome was mortality or ECMO requirement. Parameters, including the LPA:MPA ratio, RPA:MPA ratio, Nakata index, McGoon ratio, and ejection fraction (EF), were analyzed and compared with the observed-to-expected lung-to-head ratio (o/e LHR), initial blood gas, and defect size as predictive values. Among 39 neonates with RCDH, 25 (64.1 %) survived without ECMO. The non-survivor or ECMO group exhibited lower o/e LHR, reduced EF, smaller LPA and RPA diameters, and larger MPA diameter than survivors. Lower LPA:MPA ratio, Nakata index, McGoon ratio, and higher initial PaCO 2 were associated with adverse outcomes. Notably, the LPA:MPA ratio showed the highest predictive capability (area under the curve, 0.983; p < 0.001). The LPA:MPA ratio is a promising postnatal predictor of mortality or ECMO requirement in neonates with RCDH. Additionally, Nakata index, McGoon ratio, and initial PaCO 2 are significantly correlated with outcomes. This is a level III. Prognostic study. • What is currently known about this topic? 5. Application of the National Early Warning Score in patients with suspected COVID‐19 in Korea

Coronavirus disease 2019 (COVID‐19) infection has a wide range of severity, ranging from asymptomatic infection to severe respiratory distress and multiple organ dysfunction. To optimize the utilization of limited resources, a system is needed to rapidly classify the patients requiring monitoring and urgent intervention. The purpose of this study was to determine whether the National Early Warning Score (NEWS) at admission is a useful prediction tool for in‐hospital mortality in patients who visited the emergency department (ED) and required isolation during the COVID‐19 pandemic. This study was conducted in adults aged 19 years or older who visited the ED and were preemptively isolated due to fever or respiratory symptoms from January 2021 to December 2021. The NEWS was calculated at the time of ED admission. The primary outcome was all‐cause in‐hospital mortality. Of the 6304 patients included in the study, 5759 survived and 545 died in the hospital. The NEWS was higher in non‐survivors (6.1 ± 4.2) than in survivors (2.8 ± 2.5). When the NEWS was examined as a continuous variable, the adjusted odds ratio for in‐hospital mortality was 1.176 (95% CI, 1.12–1.23). The area under the curve (AUC) of NEWS for predicting in‐hospital mortality was 0.756 (95% CI, 0.734–0.778). The NEWS at ED admission was associated with in‐hospital mortality in preemptively isolated patients during the COVID‐19 pandemic. The use of NEWS in patients with suspected COVID‐19 infection would help predict the severity and prognosis of patients.

6. Self-Management Using eHealth Technologies for Liver Transplant Recipients: Scoping Review.

Background: Liver transplantation has become increasingly common as a last-resort treatment for end-stage liver diseases and liver cancer, with continually improving success rates and long-term survival rates. Nevertheless, liver transplant recipients face lifelong challenges in self-management, including immunosuppressant therapy, lifestyle adjustments, and navigating complex health care systems. eHealth technologies hold the potential to aid and optimize self-management outcomes, but their adoption has been slow in this population due to the complexity of post–liver transplant management. Objective: This study aims to examine the use of eHealth technologies in supporting self-management for liver transplant recipients and identify their benefits and challenges to suggest areas for further research. Methods: Following the Arksey and O'Malley methodology for scoping reviews, we conducted a systematic search of 5 electronic databases: PubMed, CINAHL, Embase, PsycINFO, and Web of Science. We included studies that (1) examined or implemented eHealth-based self-management, (2) included liver transplant recipients aged ≥18 years, and (3) were published in a peer-reviewed journal. We excluded studies that (1) were case reports, conference abstracts, editorials, or letters; (2) did not focus on the posttransplantation phase; (3) did not focus on self-management; and (4) did not incorporate the concept of eHealth or used technology solely for data collection. The quality of the selected eHealth interventions was evaluated using (1) the Template for Intervention Description and Replication guidelines and checklist and (2) the 5 core self-management skills identified by Lorig and Holman. Results: Of 1461 articles, 15 (1.03%) studies were included in the final analysis. Our findings indicate that eHealth-based self-management strategies for adult liver transplant recipients primarily address lifestyle management, medication adherence, and remote monitoring, highlighting a notable gap in alcohol relapse interventions. The studies used diverse technologies, including mobile apps, videoconferencing, and telehealth platforms, but showed limited integration of decision-making or resource use skills essential for comprehensive self-management. The reviewed studies highlighted the potential of eHealth in enhancing individualized health care, but only a few included collaborative features such as 2-way communication or tailored goal setting. While adherence and feasibility were generally high in many interventions, their effectiveness varied due to diverse methodologies and outcome measures. Conclusions: This scoping review maps the current literature on eHealth-based self-management support for liver transplant recipients, assessing its potential and challenges. Future studies should focus on developing predictive models and personalized eHealth interventions rooted in patient-generated data, incorporating digital human-to-human interactions to effectively address the complex needs of liver transplant recipients. This review emphasizes the need for future eHealth self-management research to address the digital divide, especially with the aging liver transplant recipient population, and ensure more inclusive studies across diverse ethnicities and regions. 7. REXO5 promotes genomic integrity through regulating R-loop using its exonuclease activity

- Abstract
Chronic myeloid leukemia (CML), caused by BCR::ABL1 fusion gene, is known to regulate disease progression by altering the expression of genes. However, the molecular mechanisms underlying these changes are largely unknown. In this study, we identified RNA Exonuclease 5 (REXO5/LOC81691) as a novel gene with elevated mRNA expression levels in chronic myeloid leukemia (CML) patients. Additionally, using the REXO5knockout (KO) K562 cell lines, we revealed a novel role for REXO5 in the DNA damage response (DDR). Compared to wild-type (WT) cells, REXO5KO cells showed an accumulation of R-loops and increased DNA damage. We demonstrated that REXO5 translocates to sites of DNA damage through its RNA recognition motif (RRM) and selectively binds to R loops. Interestingly, we identified that REXO5 regulates R-loop levels by degrading mRNA within R-loop using its exonuclease domain. REXO5KO showed ATR-CHK1 activation. Collectively, we demonstrated that REXO5 plays a key role in the physiological control of R-loops using its exonuclease domain. These findings may provide novel insights into how REXO5expression changes contribute to CML pathogenesis.

8. Novel Architecture of Energy Management Systems Based on Deep Reinforcement Learning in Microgrid

In microgrids, energy management systems (EMS) have been considered essential systems to optimize energy scheduling, control and operation for reliable power systems. Conventional EMS researches have been predominantly performed by employing demand-side management and demand response (DR). Nonetheless, multi-action control in EMS is confronted with operational challenges in terms of the profitability and stability. In this paper, energy information systems (EIS), energy storage systems (ESS), energy trading risk management systems (ETRMS), and automatic DR (ADR) are integrated to efficiently manage the profitability and stability of the whole EMS by optimal energy scheduling. The proposed microgrid EMS architecture is optimized by using proximal policy optimization (PPO) algorithm, which has been known to have good performance in terms of learning stability and complexity. A novel performance metric, represented as a burden of load and generation (BoLG), is proposed to evaluate the energy management performance. The BoLG is incorporated into the reward settings for optimizing the management of multi-action controls such as load shifting, energy charging-discharging, and transactions. From the simulation results, it is confirmed that the proposed architecture can improve energy management performance with the proper trade-off between stability and profitability, compared to dynamic programming (DP)-based and double deep Q-network (DDQN)-based operation.

Particulate matter (PM) exposure during pregnancy increases the risk of developing asthma in children. However, the placental mechanisms have yet to be elucidated. This study aims to evaluate the mechanisms associated with PM exposure during pregnancy and asthma susceptibility via placental epigenetic dysregulation. We analyzed data from two independent Korean birth cohorts (COCOA, 684 children; PSKC, 818 children). Physician-diagnosed current asthma and bronchial hyperresponsiveness (BHR) via methacholine challenge tests were evaluated at age seven. We estimated PM exposure with a diameter <10 μm (PM 10) during pregnancy using land-use regression models. We performed genome-wide methylation profiling in the placenta of 40 samples in the COCOA study and analyzed the gene expression levels. High PM 10 exposure during pregnancy increased the risk of developing current asthma and BHR in the COCOA study (aOR 2.36, 95% CI 1.06 – 5.22; aOR 2.14, 95% CI 1.40 – 3.27, respectively) and current asthma in the PSKC (aOR 2.62, 95% CI 1.35 – 5.09). The genes involved in neuronal differentiation and proliferation and Notch signaling pathways were significantly hypermethylated in children with high PM 10 -exposed asthma. The methylation and expression levels of eight genes (PAX6, REST , OLIG2, GLI1, ZBTB7A, NOTCH4, NOTCH1, and NOTCH3) in these pathways correlated with clinical parameters. This may effectively predict PM-related asthma through a prediction model using degrees of gene-based or CpG-based methylation (AUC = 0.96 and 0.93, respectively). 11. Digital Health Interventions for Adult Patients With Cancer Evaluated in Randomized Controlled Trials: Scoping Review.

Background: Digital care has become an essential component of health care. Interventions for patients with cancer need to be effective and safe, and digital health interventions must adhere to the same requirements. Objective: The purpose of this study was to identify currently available digital health interventions developed and evaluated in randomized controlled trials (RCTs) targeting adult patients with cancer. Methods: A scoping review using the JBI methodology was conducted. The participants were adult patients with cancer, and the concept was digital health interventions. The context was open, and sources were limited to RCT effectiveness studies. The PubMed, CINAHL, Embase, Cochrane Library, Research Information Sharing Service, and KoreaMed databases were searched. Data were extracted and analyzed to achieve summarized results about the participants, types, functions, and outcomes of digital health interventions. Results: A total of 231 studies were reviewed. Digital health interventions were used mostly at home (187/231, 81%), and the web-based intervention was the most frequently used intervention modality (116/231, 50.2%). Interventions consisting of multiple functional components were most frequently identified (69/231, 29.9%), followed by those with the self-manage function (67/231, 29%). Web-based interventions targeting symptoms with the self-manage and multiple functions and web-based interventions to treat cognitive function and fear of cancer recurrence consistently achieved positive outcomes. More studies supported the positive effects of web-based interventions to inform decision-making and knowledge. The effectiveness of digital health interventions targeting anxiety, depression, distress, fatigue, health-related quality of life or quality of life, pain, physical activity, and sleep was subject to their type and function. A relatively small number of digital health interventions specifically targeted older adults (6/231, 2.6%) or patients with advanced or metastatic cancer (22/231, 9.5%). Conclusions: This scoping review summarized digital health interventions developed and evaluated in RCTs involving adult patients with cancer. Systematic reviews of the identified digital interventions are strongly recommended to integrate digital health interventions into clinical practice. 14. Nonadherence to health promotion depending on chronic obstructive pulmonary disease severity.

- Abstract
• The current national diagnostic rate and awareness for early diagnosis and treatment for COPD is still low. • COPD patients with more stress and comorbidities are more likely to show nonadherence to health behaviors. • Nonadherence to health behaviors is higher in moderate and severe COPD groups than in the mild COPD group, so it is necessary to tailor healthcare steps to the severity of the disease. Nonadherence to recommended health behaviors reportedly correlates with the prevalence and health outcomes of COPD, but the factors contributing to multiple health promotions are not well understood. To examine the factors associated with nonadherence to health behaviors, specifically focusing on COPD severity. We conducted a secondary data analysis with data from the 2016–2019 Korea National Health and Nutrition Examination Survey. The analyzed data were from 1,627 individuals aged over 40 years who had a FEV1/FVC ratio < 70% measured via spirometer. In this study, nonadherence to health behaviors is the sum of seven health-promoting behaviors in terms of smoking cessation, alcohol abstinence, regular exercise, weight control, recommended sleep time, influenza vaccination, and medical check-up. COPD patients' nonadherence to health behaviors was significantly associated with comorbidity (p =0.017), stress (p =0.002), and COPD severity (moderate, severe; p =0.010, p =0.039, respectively). In addition, the moderate and severe COPD groups showed a higher nonadherence rate than the mild COPD group. Specifically, nonadherence to weight control was the most significantly different behavior according to COPD severity levels, based on the GOLD classification of airflow limitation. Our study findings support that those with higher levels of stress, comorbidities, and severe disease conditions are less likely to adhere to health behaviors. Thus, healthcare providers and policymakers should provide tailored approaches according to COPD severity to educate and support appropriate health behaviors for individuals with COPD. 15. Characteristics of Convectively Induced Turbulence in East Asia Using Geostationary Korea Multi‐Purpose Satellite‐2A (GK‐2A) and In Situ Aircraft Data

Deep convection and its vicinity are important areas of turbulence encountered in cruising aircraft, called convectively induced turbulence (CIT). In this study, deep convection area (DCA) in East Asia is identified using the infrared channel of Geostationary Korea Multi‐Purpose Satellite‐2A (GK‐2A) over 49 months (from August 2019 to August 2023), and CIT is determined by using in situ aircraft data adjacent to the DCA. Based on the satellite‐based CIT algorithm, the results indicate that 13.85%, 15.65%, and 18.23% of the total turbulence cases are classified as CIT over the Korean Peninsula, East Asia, and the full‐disk area, respectively, which are consistent with those in the previous studies. In East Asia, the CIT ratio is the highest in summer primarily due to the East Asian summer monsoon, and surprisingly, it is the second highest in winter. In winter, DCA occurs along the storm tracks accompanied by Aleutian Low, which provides favorable conditions for flights to encounter CIT in the North Pacific Ocean. The CIT intensity decreases rapidly as the horizontal distance between the DCA boundary and CIT location increases. However, at a distance exceeding 40 km from the DCA boundary, the intensity of turbulence gradually increases, which is most significant in winter. This trend is more likely related to the broader areas of DCA in the winter season than those in summer mainly due to the stronger upper‐level jet and its interaction with a storm system. Eventually, this algorithm can provide better information of avoidance guidance for CIT over the ocean. Deep convective clouds and surrounding areas are vulnerable for encountering severe turbulence events by cruising aircraft, which often results in serious injuries and fatalities on in‐flight crews and passengers. This study uses geostationary satellite data and in situ aircraft data to identify possible areas of convectively induced turbulence (CIT) in East Asia. From the satellite‐based CIT detection algorithm, it is found that CIT events are most frequent in summer due to the East Asian summer monsoon. Surprisingly, the second peak is in winter, which is mainly driven by frequent activities of mid‐latitude cyclones in the North Pacific Ocean that affects the dominant flight routes between East Asia and North America. Eventually, the CIT detection technique will be very useful for providing better information of turbulence avoidance guidance especially over the ocean. 16. Quantitative Two-Stage Classification of Gas Mixtures Using 2-D TMDC and PGM Chalcogenides

Accurate and quantitative classification of gas mixtures is an important issue in various fields, including the healthcare and food industries. However, traditional classification approaches, such as gas chromatography, mass spectroscopy, and chemical analysis, not only require specialized skills but are also time-consuming, inaccurate, and expensive. For these reasons, we used a chemiresistive sensor based on 2-D transition metal dichalcogenides (2-D TMDCs) and platinum group material-based chalcogenides, which have high responsivity, selectivity, and stability toward target gases. Raman spectroscopy, scanning electron microscopy, and X-ray photoelectron spectroscopy were used to characterize the WS2 and RuS2 sensing channels. Moreover, the gas-sensing properties toward NO2, NH3, and their mixtures (1:1 and 2:1) were analyzed, and the classification of these gases was carried out via our proposed two-stage classification model consisting of dimensionality reduction and classification processes. The proposed model achieved more than 90% accuracy in all cases when classifying single gases and their mixtures, which could be industrially applicable in the future.

- Abstract
Emotional states influence bodily physiology, as exemplified in the top-down process by which anxiety causes faster beating of the heart1–3. However, whether an increased heart rate might itself induce anxiety or fear responses is unclear3–8. Physiological theories of emotion, proposed over a century ago, have considered that in general, there could be an important and even dominant flow of information from the body to the brain9. Here, to formally test this idea, we developed a noninvasive optogenetic pacemaker for precise, cell-type-specific control of cardiac rhythms of up to 900 beats per minute in freely moving mice, enabled by a wearable micro-LED harness and the systemic viral delivery of a potent pump-like channelrhodopsin. We found that optically evoked tachycardia potently enhanced anxiety-like behaviour, but crucially only in risky contexts, indicating that both central (brain) and peripheral (body) processes may be involved in the development of emotional states. To identify potential mechanisms, we used whole-brain activity screening and electrophysiology to find brain regions that were activated by imposed cardiac rhythms. We identified the posterior insular cortex as a potential mediator of bottom-up cardiac interoceptive processing, and found that optogenetic inhibition of this brain region attenuated the anxiety-like behaviour that was induced by optical cardiac pacing. Together, these findings reveal that cells of both the body and the brain must be considered together to understand the origins of emotional or affective states. More broadly, our results define a generalizable approach for noninvasive, temporally precise functional investigations of joint organism-wide interactions among targeted cells during behaviour.

Peripheral nerve injury causes severe loss of motor and sensory functions, consequently increasing morbidity in affected patients. An autogenous nerve graft is considered the current gold standard for reconstructing nerve defects and recovering lost neurological functions; however, there are certain limitations to this method, such as limited donor nerve supply. With advances in regenerative medicine, recent research has focused on the fabrication of tissue-engineered nerve grafts as promising alternatives to the autogenous nerve grafts. In this study, we designed a nerve guidance conduit using an electrospun poly(lactide- co -ε-caprolactone) (PLCL) membrane with a visible light-crosslinked gelatin hydrogel. The PLCL nanoporous membrane with permeability served as a flexible and non-collapsible epineurium for the nerve conduit; the inner-aligned gelatin hydrogel paths were fabricated via 3D printing and a photocrosslinking system. The resultant gelatin hydrogel with microgrooved surface pattern was established as a conducting guidance path for the effective regeneration of axons and served as a reservoir that can incorporate and release bioactive molecules. From in vivo performance tests using a rat sciatic nerve defect model, our PLCL/gelatin conduit demonstrated successful axonal regeneration, remyelination capacities and facilitated functional recovery. Hence, the PLCL/gelatin conduit developed in this study is a promising substitute for autogenous nerve grafts. Nerve guidance conduits (NGCs) are developed as promising recovery techniques for bridging peripheral nerve defects. However, there are still technological limitations including differences in the structures and components between natural peripheral nerve and NGCs. In this study, we designed a NGC composed of an electrospun poly(lactide- co -ε-caprolactone) (PLCL) membrane and 3D printed inner gelatin hydrogel to serve as a flexible and non-collapsible epineurium and a conducting guidance path, respectively, to mimic the fascicular structure of the peripheral nerve. In particular , in vitro cell tests clearly showed that gelatin hydrogel could guide the cells and function as a reservoir that incorporate and release nerve growth factor. 23. The needs for digital health and eHeath literacy of cancer patients, caregivers, and healthcare providers: A multicenter, descriptive correlational study.

Digital health is an indispensable tool, but its use depends on the eHealth literacy (eHL) of end-users. This study aimed to understand the need for digital health and eHL among cancer patients, caregivers, and healthcare providers and to identify differences in digital health needs related to the eHL of cancer patients. A multicenter, descriptive correlational study was conducted and included a total of 209 patients, 150 caregivers and 150 healthcare providers. Digital health needs were identified, and eHL was measured using the Korean version of the eHealth Literacy Scale. Differences in digital health needs in relation to the eHL of patients were analyzed. The most necessary digital health functions among cancer patients and caregivers were 'information and education on symptom management after cancer treatment' and 'education on coping methods for each type of cancer' (87.1–94.0%). Healthcare providers reported the need for a digital health function for 'medication information' and assisting in 'medical appointments' (96.7–98.0%). The preferred types of digital health were telemonitoring, mobile services, and telemedicine by telephone (81.3–90.5%). The mean eHL score of the cancer patients was 28.84 ± 6.75. Differences existed in the need for digital health functions and preferences for digital health types between cancer patients with high and low eHL. Cancer patients and caregivers expressed strong needs for digital health that provide information and education about symptom management and coping with cancer. Digital health interventions for cancer care need to be developed to reflect the identified needs and preferences and eHL of end-users. • The most needed digital health function for patients, caregivers, and healthcare providers was 'information and education.' • Telemonitoring, mobile services, and telemedicine by telephone, were the preferred types of digital healthcare across all groups. • There were differences in the types and functions of digital health preferred depending on the level of eHealth literacy. 25. Impact of buffer concentration on the thermal stability of immunoglobulin G

- Abstract
Purpose: This study determined the effect of buffer concentration on the unfolding and aggregation propensity of immunoglobulin G (IgG) in acidic buffers and evaluated the viscosity of highly concentrated IgG solutions. Methods: Nano differential scanning fluorimetry (nanoDSF) was used to determine the thermal stability of IgG as a function of pH and buffer concentration. Protein–protein interaction (PPI) properties were measured using dynamic light scattering (DLS). A stability study was performed at 40 °C for 8 weeks, and the monomers and multimers of IgG were determined using size-exclusion chromatography. The viscosities of the IgG formulations were measured using m-VROC viscometer. Results: In sodium acetate buffer at pH 4.6, the midpoint temperature of thermal unfolding (T
m ) and the onset temperature of protein aggregation (Tagg ) determined using nanoDSF tended to increase as the buffer concentration decreased from 50 to 10 mM. The PPI study using DLS suggested that the intermolecular interactions became repulsive as the buffer concentration decreased. In a stability study with IgG samples formulated in sodium acetate or sodium citrate buffers, the best stability was observed in 10 mM sodium acetate buffer. At IgG concentrations above 194 mg/mL in 10 mM sodium acetate buffer, the viscosity was 9.6 ± 0.1 cP. Conclusion: The aggregation propensity of IgG in sodium acetate buffer depended on the buffer concentration, attributed to the charge shielding effect by binding the buffer anions to the positively charged region of IgG. This study showed that IgG could be concentrated to 200 mg/mL with a viscosity of approximately 10 cP or less in 10 mM sodium acetate buffer.- Published

This paper describes the synthesis of eight novel zirconium and hafnium complexes containing N-alkoxy carboxamidate-type ligands, as potential precursors for metal oxides and atomic layer deposition (ALD) for HfO2. A series of ligands, viz., N-ethoxy-2,2-dimethylpropanamide (edpaH), N-ethoxy-2-methylpropanamide (empaH), and N-methoxy-2,2-dimethylpropanamide (mdpaH), were used to afford complexes Zr(edpa)4(1), Hf(edpa)4(2), Zr(empa)4(3), Hf(empa)4(4), Zr(mdpa)4(5), Hf(mdpa)4(6), ZrCp(edpa)3(7), and HfCp(edpa)3(8). Thermogravimetric analysis curves assessed for the evaporation characteristics of complexes 1–8revealed single-step weight losses with low residues, except for the mdpa-containing complexes. Single-crystal X-ray diffraction studies of 1, 2, 5, and 6revealed that all the complexes have monomeric molecular structures, with the central metal ion surrounded by eight oxygen atoms from the four bidentate alkoxyalkoxide ligands. Among the complexes prepared, 8exhibited a low melting point (64 °C), good volatility (1 Torr at 112 °C), high thermal stability, and excellent endurance over 6 weeks at 120 °C. Therefore, an ALD process for the growth of HfO2was developed using HfCp(edpa)3(8) as a novel precursor. Furthermore, the HfO2film exhibited a low capacitance equivalent oxide thickness of ∼1.5 nm, with Jgas low as ∼3 × 10–4A/cm2at Vg−1 V in a metal–insulator–semiconductor capacitor (Au/HfO2/p-Si).

27. The association between L1 skeletal muscle index derived from routine CT and in-hospital mortality in CAP patients in the ED.

Introduction: Low muscle mass is associated with an increased mortality risk due to medical comorbidities such as chronic obstructive pulmonary disease, cardiovascular disease, and cerebrovascular disease. Computed tomography (CT) has been identified as the gold standard for measuring body composition. We evaluated the relationship between the L1 SMI measured from CT and in-hospital mortality in patients with community-acquired pneumonia (CAP).Methods: From January 2015 to June 2015, 311 patients who were diagnosed with CAP and underwent CT in the ED were retrospectively analyzed. Multivariate binary logistic regression analysis was used to assess independent predictors of in-hospital mortality. All variables with a significance level < 0.1 by univariate analysis were included in a multivariate logistic regression model. The primary outcome was all-cause in-hospital mortality.Results: Among the 311 patients, 33 (10.6%) died. We divided the patients into two groups based on the optimal sex-specific cut-off value of the L1 SMI (45 cm2/m2 in males and 40 cm2/m2 in females). A low L1 SMI was present in 90 (28.9%) of the 311 patients. In multivariate analysis, low L1 SMI, diabetes mellitus, albumin and APACHE II score were significantly associated with in-hospital mortality (aOR 3.39, 3.73, 0.09 and 1.10, respectively).Conclusion: SMI assessment at L1 is achievable in patients with CAP receiving routine chest CT, and the L1 SMI is associated with high in-hospital mortality, more hospitalizations and ventilator application in patients with CAP in the ED. This could help establish an early strategy for critical care of patients with L1 SMI obtained by chest CT for diagnosis in CAP patients in the ED. [ABSTRACT FROM AUTHOR]- Published

28. Neutrophil to lymphocyte ratio is associated with in-hospital mortality in older adults admitted to the emergency department.

Background: The objective of this study was to test the hypothesis that an elevated neutrophil to lymphocyte ratio (NLR) at admission is associated with and increased risk of mortality in older patients admitted to the emergency department (ED).Methods: We performed a retrospective analysis of patients admitted to the ED between November 2016 and February 2017. We included patients who were older than 65 years who visited the ED with any medical problem. We excluded patients with hematologic malignancy. Baseline NLR values were measured at the time of admission to the ED. The primary outcome was all-cause in-hospital mortality. A multivariate logistic analysis was performed.Results: A total of 2777 patients were included in this study. The median age was 75 years (IQR 70-81), and 1359 (48.9%) patients were male. The in-hospital mortality rate was 5.0% (140 patients). The NLR value was higher in nonsurvivors (median, 8.08, IQR 4.29-15.25) than in survivors (median, 3.69, IQR 2.1-6.92, P < 0.001). In the multivariate logistic regression analysis, the NLR was associated with all cause in-hospital mortality after adjusting for confounding factors (OR = 1.03, 95% CI = 1.014-1.046).Conclusions: These results show that the NLR at admission is associated with in-hospital mortality among patients older than 65 years without hematologic malignancy. 29. Risk prediction models for the development of oral-mucosal pressure injuries in intubated patients in intensive care units: A prospective observational study.

- Abstract
Oral-mucosal pressure injury (PI) is the most commonly encountered medical device-related PIs. This study was performed to identify risk factors and construct a risk prediction model for oral-mucosal PI development in intubated patients in the intensive care unit. The study design was prospective, observational with medical record review. The inclusion criteria stipulated that 1) participants should be > 18 years of age, 2) there should be ETT use with holding methods including adhesive tape, gauze tying, and commercial devices. Data of 194 patient-days were analysed. The identification and validation of risk model development was performed using SPSS and the SciKit learn platform. The risk prediction logistic models were composed of three factors (bite-block/airway, commercial ETT holder, and corticosteroid use) for lower oral-mucosal PI development and four factors (commercial ETT holder, vasopressor use, haematocrit, and serum albumin level) for upper oral-mucosal PI development among 10 significant input variables. The sensitivity and specificity for lower oral-mucosal PI development were 85.2% and 76.0%, respectively, and those for upper oral-mucosal PI development were 60.0% and 89.1%, respectively. Based on the results of the machine learning, the upper oral-mucosal PI development model had an accuracy of 79%, F1 score of 88%, precision of 86%, and recall of 91%. The development of lower oral-mucosal PIs is affected by immobility-related factors and corticosteroid use, and that of upper oral-mucosal PIs by undernutrition-related factors and ETT holder use. The high sensitivities of the two logit models comprise important minimum data for positively predicting oral-mucosal PIs. • For lower oral-mucosal PI development, bite-block/airway and commercial ETT holder use, and steroid use were risk factors. • For upper oral-mucosal PI development, commercial ETT holder use and undernutrition-related factors were risk factors. • The high sensitivities of the two logit models comprise important minimum data for positively predicting oral-mucosal PIs. - Abstract
The atomic layer deposition (ALD) of Ru using a metal–organic precursor, tricarbonyl(trimethylenemethane)ruthenium [Ru(TMM)(CO)3] and O2as a reactant is reported. The high vapor pressure, thermal stability, and relatively small ligands of the precursor facilitate efficient ALD. Typical self-limiting growth and an ALD temperature window of 220–260 °C are observed along with significantly high growth per cycle (GPC) (∼1.7 Å) and short incubation cycles (∼6) at 220 °C. Density functional theory calculations indicate that the high growth rate and self-limiting behavior can be attributed to the characteristics of the trimethylenemethane ligand. The as-grown polycrystalline films (average grain size ∼20 nm and negligible impurities) were evident from plan-view transmission electron microscopy. The variation in film resistivity with increasing film thickness and deposition temperature was investigated with and without annealing. Films deposited at 260 °C show low resistivity (∼12.9 μΩ cm), which further decreases (∼9.8 μΩ cm) postannealing at 500 °C. A thin Ru film is successfully deposited with 100% step-coverage on a dual-trench structure having an aspect ratio of ∼6.3 (minimum width: ∼15 nm). The interfacial adhesion energy measured using the four-point bending test exceeds 7 J m–2, regardless of the dielectric material and annealing treatment. The Ru precursor permits enhanced nucleation and GPC at relatively low deposition temperatures to construct high-quality Ru films with significantly low resistivity using simple, plasma-free techniques, and is suitable for the fabrication of emerging Ru films to replace Cu-based interconnects.

We proposed the concept of atomic layer modulation (ALM) based on precursor chemical reactivities and steric hindrance effects to fabricate multicomponent nanofilms. Because ALM employs consecutive precursor exposures followed by exposure to a counter reactant, the composition of ALM films is determined by the molecular size and chemical reactivities of the precursors. For the demonstration, dicarbonyl-bis(5-methyl-2,4-hexanediketonato)Ru(II) (Carish) and trimethylaluminum (TMA) were used as Ru and Al precursors, respectively, and H2O was used as the counter reactant. Prior to the experiments, the chemical reactivity and sterically hindered physisorption of the Ru and Al precursors were theoretically calculated using density functional theory (DFT) and Monte Carlo (MC) simulations, respectively. The transmission electron microscopy (TEM) and X-ray photoelectron spectroscopy (XPS) results were highly consistent with the theoretical results, and the growth characteristics were well explained by the MC- and DFT-based reaction models. We believe that ALM could be extended to other material systems, thereby providing a different method of fabricating multicomponent nanofilms for various applications including semiconductors and nanodevices.

Polyamines are critical elements in mammals, but it remains unknown whether adenosyl methionine decarboxylase (AMD1), a rate-limiting enzyme in polyamine synthesis, is required for myeloid leukemia. Here, we found that leukemic stem cells (LSCs) were highly differentiated, and leukemia progression was severely impaired in the absence of AMD1 in vivo. AMD1was highly upregulated as chronic myeloid leukemia (CML) progressed from the chronic phase to the blast crisis phase, and was associated with the poor prognosis of CML patients. In addition, the pharmacological inhibition of AMD1 by AO476 treatment resulted in a robust reduction of the progression of leukemic cells both in vitro and in vivo. Mechanistically, AMD1 depletion induced loss of mitochondrial membrane potential and accumulation of reactive oxygen species (ROS), resulting in the differentiation of LSCs via oxidative stress and aberrant activation of unfolded protein response (UPR) pathway, which was partially rescued by the addition of polyamine. These results indicate that AMD1 is an essential element in the progression of myeloid leukemia and could be an attractive target for the treatment of the disease.

Although graphene has considerable potential as a next-generation transparent conducting electrode (TCE) material owing to its excellent optical transparency and flexibility, its electrical properties require further improvement for industrial application. This study reports a pathway of doping graphene by selective atomic layer deposition (ALD) of metals to elevate the electrical conductivity of graphene. Introduction of a novel Pt precursor [dimethyl(N,N-dimethyl-3-butene-1-amine-N)platinum(II); C8H19NPt; DDAP] facilitates a low-temperature (165 °C) process. The sheet resistance (Rs) of graphene is reduced significantly from 471 to 86.8 Ω sq–1after 200 cycles of Pt ALD, while the optical transmittance at 550 nm (T) is maintained above 90% up to 200 cycles due to the selective growth of Pt on the defects of graphene. Furthermore, comprehensive analysis, including metal (Ru, Pt, and Ni) ALD on graphene, metal (Ru, Pt, Ni, Au, and Co) evaporation on graphene, and change in the ALD chemicals, demonstrates that ALD allows efficient graphene doping and the oxygen affinity of the metal is one of the key properties for efficient graphene doping. Finally, Pt ALD is applied to a multilayer graphene to further reduce Rsdown to 75.8 Ω sq–1yet to be highly transparent (T:87.3%) after 200 cycles. In summary, the selective ALD of metals opens a way of improving the electrical properties of graphene to a level required for the industrial TCE application and has the potential to promote development of other types of functional metal–graphene composites.

AbstractWe investigated the antifungal susceptibilities and the cyp51mutant strains among Aspergillus fumigatusclinical isolates obtained from 10 university hospitals in Korea. Of the 84 isolates examined, two itraconazole-resistant isolates were found with no amino acid substitution in the cyp51A/cyp51Bgenes. However, 19 (23.2%) azole-susceptible isolates harbored amino acid substitutions:Nine isolates harbored one to five mutations in cyp51Awith high polymorphism, and 11 isolates exhibited the same Q42L mutation in cyp51B. Overall, a low azole resistance rate and high frequency of cyp51A/cyp51Bamino acid substitutions were observed in the azole-susceptible A. fumigatusisolates in Korea.

Spinal cord injury (SCI) is a devastating lesion lacking effective treatment options currently available in clinics. The inflammatory process exacerbates the extent of the lesion through a secondary injury mechanism, where proinflammatory classically activated macrophages (M1) are prevalent at the lesion site. However, the polarized alternatively activated anti-inflammatory macrophages (M2) are known to play an important role in wound healing and regeneration following SCI. Herein, we introduce porcine brain decellularized extracellular matrix (dECM) to modulate the macrophages in the injured spinal cord. The hydrogels with collagen and dECM at various dECM concentrations (1, 5, and 8 mg/ml) were used to cultivate primary macrophages and neurons. The dECM hydrogels were shown to promote the polarization of macrophages toward M2 phase and the neurite outgrowth of cortical and hippocampal neurons. When the dECM hydrogels were applied to rat SCI models, the proportion of M1 and M2 macrophages in the injured spinal cord was substantially altered. When received dECM concetration of 5 mg/ml, the expression of molecules associated with M2 (CD206, arginase1, and IL-10) was significantly increased. Consistently, the population of total macrophages and cavity area were substantially reduced in the dECM-treated groups. As a result, the locomotor functions of injured spinal cord, as assessed by BBB and ladder scoring, were significantly improved. Collectively, the porcine brain dECM with optimal concentration promotes functional recovery in SCI models through the activation of M2 macrophages, suggesting the promising use of the engineered hydrogels in the treatment of acute SCI. Spinal cord injury (SCI) is a devastating lesion, lacking effective treatment options currently available in clinics. Here we delineated that the treatment of injured spinal cord with porcine brain decellularized matrix-based hydrogels for the first time, and could modulate the macrophage polarization and the ultimate functional recovery. When appropriate formulations were applied to a contused spinal cord model in rats, the decellularized matrix hydrogels shifted the macrophages to polarize to pro-regenerative M2 phenotype, decreased the size of lesion cavity, and finally promoted the locomotor functions until 8 weeks following the injury. We consider this work can significantly augment the matrix(biomaterial)-based therapeutic options, as an alternative to drug or cell-free approaches, for the treatment of acute injury of spinal cord. 43. Relationship Between Isokinetic Muscle Strength and Functional Tests in Chronic Ankle Instability.

Isokinetic muscle strength measurements and functional tests are usually performed to evaluate ankle condition in chronic ankle instability (CAI), yet there is no clear demonstration of the relationship between isokinetic muscle strength and functional tests. The objective of this study was to evaluate the relationship between isokinetic muscle strength and functional tests in CAI. Between April 2014 and August 2016, 103 patients with unilateral CAI were studied. Single-leg balance, single-heel raise, and single-leg squat tests were performed for static balancing assessment. Single-leg hop, double-leg jump, and sidestep tests were performed for dynamic balancing assessment. The isokinetic muscle strength of both ankles was measured using a dynamometer. The involved ankle showed lower muscle strength in inversion than the uninvolved ankle, while eversion, dorsiflexion, and plantarflexion muscle strength had no significant differences between ankles. There were significant correlations between the isokinetic muscle strength of inversion and the single-leg balance test, single-heel raise test, and sidestep test (Pearson's r ; 0.246, 0.514, and 0.229 at 30°/second; 0.288, 0.473, and 0.239 at 180°/second, respectively). The single leg balance, single heel raise, and sidestep tests are useful to assess not only ankle functional performance but also isokinetic muscle strength. Among these tests, the single heel raise test was the most reliable test to reflect muscle strength deficiency in CAI. [ABSTRACT FROM AUTHOR]

The present study was conducted to determine whether black garlic (BG) which is aged by pectinase treatment with high hydrostatic pressure processing has the protective effects in experimental model of acute reflux esophagitis (RE). BG was orally administered at a dose of 100 mg/kg body weight 2 h prior to induction of RE, and the effects were compared with those of raw garlic (RG). Esophageal pathological changes were analyzed using gross and histological examinations. The anti-oxidant and inflammatory protein levels were determined using western blotting. Moreover, RE-induced histological change was markedly suppressed by BG rather than RG. The reduced catalase was increased significantly by BG supplementation; however, superoxide dismutase levels showed a tendency to increase in the esophagus. Overall, the ameliorative effects of BG were superior to those when RG. These data reveal that that BG treatment ameliorates the development of esophagitis via regulation of NF-κB mediated inflammation. [ABSTRACT FROM AUTHOR]

50. Factors associated with absent microhematuria in symptomatic urinary stone patients.

Introduction: The aim of this study was to identify factors associated with absent hematuria in patients with symptomatic urinary stones.Methods: This retrospective study analyzed the clinical and imaging findings of emergency department patients who underwent computed tomography (CT) for suspected ureteral colic over the past 2years. All patients also underwent a microscopic urinalysis, and the presence of 4 or more red blood cells/high-power field was defined as microhematuria.Results: A total of 798 patients were included in this study. Of these patients, 750 (94.0%) presented with hematuria, while 48 (6.0%) urine samples did not have evidence of hematuria. The group with an absence of hematuria was more likely to have a lower stone location (located in an area from the distal ureter to the bladder) and perinephric stranding on CT than the hematuria group (75.0% vs. 54.3%, p=0.005; 47.9% vs. 30.5%, p=0.012, respectively). The degree of hematuria at each stone location was significantly different (p=0.001). In multivariate analysis, perinephric stranding (odds ratios (OR) 1.87 [95% confidence interval (CI) 1.01-3.46], p=0.047), a lower stone location (OR 2.72 [95% CI 1.37-5.36], p=0.004), and elevated serum blood urea nitrogen (BUN) levels (OR 1.06 [95% CI 1.01-1.12], p=0.026) were associated with absent hematuria.Conclusions: In this large cohort of patients with renal colic, 6% had no microhematuria. Although some CT findings and elevated BUN were independently associated with hematuria absence, there was no difference in the demographics, time of presentation and degree and location of pain between the groups.
