Integrated behavioral health (IBH) in primary care is growing as an approach to decrease barriers and provide earlier intervention for youth experiencing behavioral and mental health symptoms. However, increasing access to mental health providers alone without ensuring evidence-based treatment (EBT) is provided will not sufficiently address the current mental health crisis. To spread to scale, IBH needs to be implemented and tested in community-based practices with integrity to EBT. This paper provides a model for implementing cognitive behavioral therapy (CBT) in community pediatric primary care via master's prepared therapists through an academic-community partnership. This paper describes the hiring practices, training in CBT, ongoing supervision and consultation, and use of data to inform the evolution of the model. Using a retrospective review of the rate of emergency department (ED) admissions per 1,000 patients with a diagnosis of depression, we compared practices enrolled in our academic-community partnership IBH model to practices who implemented their own IBH and practices with no IBH. Across a four-year period, we found that practices enrolled in the academic-community partnership IBH model had a lower number of patients per 1,000 patients with a diagnosis of depression admitted to the ED with behavioral health concerns compared to practices that did not incorporate an IBH therapist. There was no difference in ED admission rates per 1,000 between practices that hired their own IBH and those without IBH. Analysis of costs of the program against generated revenue from billed services demonstrated that the academic-community partnership model shows promise of sustainability. [ABSTRACT FROM AUTHOR]

Migrants who relocate to the United States from the Middle East are more likely to face a host of structural and individual barriers that can significantly contribute to their mental health issues and affect their psychological well-being. Addressing mental health problems and incentivizing help-seeking behavior is important among women who are more likely to face daily stressors in childcare, household responsibilities, and marital relationships. In this paper, we discuss factors that impact help-seeking for mental health problems among Middle Eastern migrant women including English language proficiency, structural challenges, and cultural factors such as shame and stigma. We argue that when considering its potential effectiveness, targeting mental health literacy may serve as the best direction for future research and social work intervention in order to enhance help-seeking behavior among this population. Recommendations for social workers include using community-based partnerships to provide educational resources regarding mental health services through healthcare centers, social service agencies, and local Muslim and Arab organizations. Primary care health checks, screenings and GP consultations are often the gateway for people with intellectual disabilities to access their physical and mental healthcare. For a population who experience greater levels of health difficulties alongside significant health inequality, improving care quality and access is of major importance. This meta-ethnographic, qualitative review aims to explore people with intellectual disabilities experiences of health checks, screenings and GP visits, while assessing the quality of the current literature and synthesising findings to consider clinical and research recommendations based on third order constructs. A systematic search identified 20 studies that met inclusion criteria. Quality assessment of each paper was conducted. Meta-ethnography methods were used to analyse and synthesis findings. One overarching concept was identified: Include Me, along with seven core concepts; Empowerment and Disempowerment, Communication and Interpersonal Factors, Access and Adaptations and Biased Narratives and Shifting Perspectives. • Conversion practices cause significant harms to mental health and wellbeing. • Health practitioners typically have gaps in knowledge about these practices. • Practitioners need to be prepared to recognise, support and affirm survivors. • There is an urgent need for the health sector to be engaged with these issues. Conversion practices include a range of efforts that attempt to change or suppress LGBTQA+ individuals' sexual or gender identity. Formal versions of these practices are occurring less frequently in Western settings, yet informal versions and the ideology underpinning them continue to cause psychological and spiritual harm to people who are subjected to them. As evidence for the harmful nature of conversion practices increases, and some governments and professional bodies are responding with measures that restrict their use, there is a growing need for the mental health sector to be engaged with these issues so that practitioners are appropriately prepared to recognize and support survivors in ways that are effective and affirming of sexual and gender diversity. In this paper, we review the state of the evidence concerning associated harms and their lack of efficacy in changing sexuality or gender identity, and highlight the changing nature of research in this space to focus on the negative impacts of conversion practices on survivors. We then discuss the evidence around mental health practitioners' knowledge and support capacity for conversion practices survivors. • Transgender and gender diverse (TGD) people experience health disparities. • CBT interventions must be tailored to TGD people to address these disparities. • Case vignettes are used to demonstrate CBT principles tailored to TGD clients. Mental and physical health disparities for transgender and gender diverse (TGD) communities have been well-documented. While advancements have been made in the development of guidelines when providing mental health care to TGD clients, gaps remain, particularly related to concrete applications of cognitive behavior therapy (CBT) approaches to address the unique mental health needs of TGD people. Such gaps leave many mental health professionals inadequately prepared to assess and treat clinical distress in TGD people, which in turn maintains health disparities. Utilizing case vignettes reflecting diverse TGD identities, this paper discusses minority stress and intersectional stigma frameworks and demonstrates their integration with CBT principles in the delivery of culturally tailored assessment, case conceptualization, and treatment of TGD clients. In this paper I explore Clifford Geertz’s distinction between experience-near and experience-distant language. In the process, I draw from mad studies and mental health service user epistemology, both written and generated through my work. I also draw on the work of the historian of emotion Tiffany Watt Smith. Three specific practices in relation to language use are proposed: how we can invigorate the agency or meaning-making in language use; how we can use language to assist people to become familiar with, not alienated from, their experience; and how we might name and question the values or obligations that get smuggled in with emotion talk. Purpose: In this paper we analyzed the complex issue of mental illness in older persons living in rural areas using the Strength and Vulnerability Integration (SAVI) model as a conceptual framework to bring balance to negative accounts of older persons' emotional experiences. Method: A narrative review was conducted to examine the mental health issues of older persons living in rural areas. Three databases were searched for data pertaining to rural mental health and the SAVI model. Theoretical and empirical articles that analyzed the strengths and vulnerabilities in relation to mental illness in older persons living in rural areas were included and analyzed. Additionally, policy and position papers were used to interrogate this issue. Findings: Analysis revealed three themes: individual vulnerabilities, system vulnerabilities and strengths. Rural individuals' struggles with chronic stress, a loss of social belonging, and neurological dysregulation across their lifespans were discussed in how they developed strengths in ageing and overcame vulnerabilities. Barriers to accessing mental health services, caregiving respite care, and health promotional services in rural areas included system vulnerabilities that exacerbated the rates of mental illness and poor health outcomes in older persons. Strengths included the rural social connection and community engagement that fostered a sense of community. Conclusions: Research and practice recommendations situated within the SAVI model include the importance of acknowledging individual differences viewing the strengths of ageing, cultural perceptions of time and leveraging community-based strengths to overcome vulnerabilities of ageing in rural areas. These changes will facilitate nurses and other health care providers assess, respond to, and prevent mental illness and poor health outcomes in the diverse ageing population living in rural areas. This review addresses the impact of the COVID-19 pandemic on workers’ mental health, in particular, teleworking conditions. Our analysis is based on documents from several international organizations and specialized scientific publications. The retrieved information reveals that the pandemic has had a significant effect on the mental health of frontline workers and employees who had to migrate to a virtual environment without prior warning. However, a positive aspect is reflected in the development of several remote mental health care devices implemented to protect vulnerable population, frontline workers, and the general population. • PTSD and alcohol use are highly comorbid, yet integrated treatment is underutilized. • Offering treatment in primary care may increase use of integrated interventions. • Case illustrations of a brief integrated intervention in primary care are presented. • Both cases presented reduced their PTSD symptoms and alcohol use. • Clinicians provide strategies to help facilitate use of integrated interventions. Symptoms of posttraumatic stress disorder (PTSD) and hazardous alcohol use are highly comorbid. Research on integrated interventions to address PTSD symptoms and hazardous alcohol use concurrently has demonstrated efficacy, yet integrated treatments are underutilized. Both patient (e.g., stigma, scheduling/logistics) and clinician (e.g., concern about symptom exacerbation and/or treatment dropout) barriers may impede utilization of integrated interventions among those with comorbid PTSD symptoms and hazardous alcohol use. Primary care behavioral health models (PCBH), in which embedded behavioral health providers deliver treatment to individuals with mild or moderate behavioral health symptoms in primary care, may help address treatment barriers by offering accessible behavioral health interventions in a destigmatizing setting. This paper presents two case examples from a randomized controlled trial testing the efficacy of an integrated intervention for PTSD symptoms and hazardous alcohol use developed for and delivered in primary care. Outcome data and session-by-session content for two participants are included, along with discussion of barriers encountered during the course of treatment. With the continuous development of the times, internet technology has gradually matured, and internet medicine has gradually entered the field of mental health. The mental health service platform can help people identify their psychological problems and provide treatment, but there are still shortcomings in current mental health service platforms, so new technologies need to be added for improvement. This article focuses on the research and application of cloud computing in mental health service platforms, aiming to improve the quality of mental health service platforms through cloud computing. Through experiments, this paper tests that after using cloud computing technology, the accuracy of the mental health service platform for users' psychological test is 78% at the highest and 70% at the lowest, while the accuracy of the mental health service platform for users' psychological test before using cloud computing is 60% at the highest and 50% at the lowest. • Digital Mental Health Services (DMHS) can greatly increase access to care. • Training is needed so providers can effectively integrate DMHS into their treatments. • More research on DMHS is needed, along with regulatory standards. • DMHS may help reduce health disparities but questions remain about cultural tailoring. The papers in this special series make a compelling case for the value of digital mental health services (DMHS; including technology-based interventions, assessments, and prevention programs) to help address some of the currently unmet needs in mental health care. At the same time, the papers highlight the work that needs to be accomplished for DMHS to fulfill their promise. We review the papers' contributions in terms of (a) the imperative to increase access to evidence-informed, high-quality care, especially for underserved populations, both in the United States and globally; (b) ways to use DMHS to improve the ways that clinical care is provided to make treatment provision more effective and efficient; and (c) the current state of the research on DMHS for emotional disorders. We then consider lessons learned and recommendations to move the field forward, such as increasing (and making transparent) the research base on DMHS, adopting regulatory standards for DMHS, attending carefully to training issues for DMHS and best practices for dissemination and implementation, designing specifically for digital platforms, and being intentional about efforts to reduce disparities regarding who benefits from DMHS. Smartphones are becoming increasingly ubiquitous. The app market is growing in all the fields, including health and medical consultancy. Although mental health care sector has been slow to adopt technology, the availability and use of health care apps has exponentially grown in recent years. The use of mobile apps to treat mental illness is widespread and growing. The main motive of this paper is to present a survey on the existing chatbot applications as well as study the different techniques applied to understand the behavioral change and mood swings of individuals and detect whether or not they have any kind of mental illness. Based on the in-depth survey conducted across 8 different chatbot applications with regards to the features, accessibility, availability and technical specifications, the research shows that around 70% users were not satisfied with the services provided. Using multiple categorization models, this paper also seeks to improve the accuracy of predictions (such as Logistic regression, Random Forest classifier, decision tree classifier, Stacking, KNN etc.). This research study is aimed toassist the generality, nature and reasons of mental illness at the primary levels of the conditions and the identification of these disorders. Depression is a mental disease affecting 5% of the population, and its prevalence is increasing. Depression is characterized by feelings of worthlessness, hopelessness, disinterest in enjoyable activities, and sadness, which can result in suicidal thoughts. Traditional approaches to recognizing depression have relied on manually crafted techniques to extract facial expressions, which have their limitations. To address these limitations, this paper proposes using convolutional neural networks (CNNs) as a practical approach for depression recognition. The proposed model in this study involves an eight-step process that includes input data, preprocessing, rescaling, model training, multi-classified results, selecting emotions based on accuracy, retraining the model, and finally, multi-classified results to determine the percentage of depression with greater accuracy. The paper concerns the community-based model of psychiatric care implemented within the pilot programme in Mental Health Centres (CZP) during the SARS-CoV-2 pandemic. The aim of the study is to characterise the changes in the operation of Mental Health Centres (CZPs) in Poland during the COVID-19 pandemic. The change in the concept of psychiatric care and the shift towards a community-based model within CZPs meant, among other things, that providers did not have to present and implement strictly defined schedules, which made it possible to respond to the current needs of patients and made it possible to adapt the conditions and ways of providing psychiatric care in Poland during the pandemic. Mental health issues among American youth continue to increase. Technology integration and acceptance represent modern opportunities to provide more access to licensed professionals. School counselors are mental health experts in schools uniquely positioned to provide social and emotional support to students. However, not all students have access to a school counselor or another mental health professional. In 2018, Utah developed the SafeUT app to increase services provided by school counselors and licensed clinicians to students in the state. This free mobile app provides confidential, real-time crisis intervention through an anonymous live chat and confidential tip program. Although the number of tips continue to increase each year, little research explores the design principles influencing the user's experience of anonymous live counseling applications. This research in progress paper uses data mining techniques on app reviews from the SafeUT mobile application to explore the common themes relating to design principles. Our results may inform future research and improve the design of similar applications to increase students' access to counselors and the much-needed mental health services these professionals can provide. • Transdiagnostic treatments may optimize mental health services at this time. • The Unified Protocol (UP) is a promising transdiagnostic treatment for this context. • Adaptations of the UP for telehealth delivery are discussed. • Considerations specific to COVID-19 are reviewed. The COVID-19 pandemic has necessitated an abrupt transition to remote delivery of psychology services at a time when patients and practicing clinicians are experiencing an increase in life stressors (e.g., job loss, social isolation, need to adapt to telehealth practice), which can exacerbate mental health concerns and contribute to clinician burnout. Because the COVID-19 pandemic is affecting diverse individuals in myriad ways, these circumstances can elicit a wide range of emotions and emotional responses. Thus, treatment during this time must be able to address heterogeneous presenting problems while placing minimal burden on clinicians who are adjusting to continuously changing circumstances. Transdiagnostic, emotion-focused, cognitive behavioral treatments (CBT), such as the Unified Protocol for Transdiagnostic Treatment of Emotional Disorders (UP), may be particularly well suited to address the challenges faced by practicing psychologists, and their patients, in the current COVID-19 pandemic. This paper discusses the applicability and adaptability of transdiagnostic treatments to telehealth, focusing primarily on the UP in the context of the COVID-19 pandemic. Further, while many CBT skills (e.g., mindfulness) can be easily translated to tele-delivery, other skills, such as exposure, can be more difficult to implement remotely, especially in the midst of a pandemic. Mental health issues are all too common consequences of conflict and atrocity crimes, often causing upwards of one-quarter of the postconflict, post-atrocity population to suffer from physical and mental sequelae that linger long after weapons have been silenced. After more than six years of ongoing conflict, Yemen's already weak health care system is on the brink of collapse, and population resilience has been severely stressed by indiscriminate attacks, airstrikes, torture, food insecurity, unemployment, cholera, and now the COVID-19 pandemic. This paper examines Yemen's responsibilities regarding the right to mental health and details the few actions the government has taken to date toward fulfilling this right. It also presents the current status of mental health care in Yemen, discussing some of the barriers to accessing the available care, as well as alternative models of mental health support being used by the population. In light of the pandemic presently facing the world, the paper also discusses COVID-19's impact on Yemen, detailing its further degrading effects on the country's health care system and people's mental health. • Clients act as entrepreneurs in mental health services under austerity. • Budget cuts incentivise creative fundraising activities. • Access to services becomes conditional on these entrepreneurial practices. • This is a new form of conditionality that is flexible and self-imposed. • Austerity is eroding the distinction between client and staff labour. This paper investigates work undertaken by clients of voluntary sector mental health services in the context of austerity. While mental health geographers have often interrogated 'work cures', little attention has been paid to client work within traditional care-oriented services. Drawing on interviews and fieldwork in a mental health day centre threatened with closure, I demonstrate that client fundraising can become a crucial source of income when marketised services face budget cuts. Clients are incentivised to act as entrepreneurs, developing creative strategies to secure funding. I conceptualise this as a novel form of conditionality, as access to services comes to depend on entrepreneurial activities. Existing theories offer a narrow conception of conditionality, as behavioural standards imposed through rules. Entrepreneurship appears more flexible, autonomous and voluntary than such rule-based conditionalities. Yet in practice clients face the same choice: work or lose access to welfare provision. I argue that entrepreneurship transforms the institutional geographies of services, directing them towards economic productivity. This can undermine longstanding practices of care, leading some clients to withdraw from services. By focussing on client responses to austerity, this paper makes a crucial contribution to geographical research into voluntary sector precarity, which has tended to focus on staff. More broadly, it advances understandings of the institutional geographies which embed the politics and economics of austerity within everyday experiences of care provision. [ABSTRACT FROM AUTHOR]

Hence, diversity considerations in the diagnosis, assessment, and treatment of psychological disorders remain vital and imperative in clinical practice. There is also substantial work indicating that cultural values likely play an essential role in shaping how individuals conceptualize and experience mental or emotional health. Therefore, the present study was developed for community-engaged clinicians and researchers to outline the process by which to create a culturally responsive, evidence-based treatment framework for community mental health interventions in close partnership with communities of color. This study was conceptualized and designed collaboratively with community leaders across five different communities of color: African American/Black, Latino/a, Native American, Pacific Islander, and African immigrant communities in a major U.S. city. This paper describes the mixed methods for such a community-engaged collaboration, highlighting critical elements for ongoing culturally engaged clinical work/research, and providing recommendations for culturally adapted interventions for mental health based on specific process observations made in the current example. BACKGROUND Socioeconomically disadvantaged groups disproportionately reported experiencing adverse circumstances resulting from the COVID-19 pandemic's socioeconomic impacts. Overarching factors associated with differentiated risks in the United States include race and ethnicity. OBJECTIVE We aim to examine: (1) the differentiated risk of experiencing adverse circumstances by race and ethnicity in the United States and (2) the trend in adverse outcomes and racial/ethnic differences in the past two years. METHODS The study utilized 49 data cycles from the Household Pulse Survey from April 2020 to September 2022. The outcomes are adverse experiences, including loss of employment income, food scarcity, housing insecurity, and unmet needs for mental health services. The racial and ethnic groups are non-Hispanic White, non-Hispanic Black, non-Hispanic Asian, non-Hispanic other minorities, and Hispanic. We compared weighted percentages of the total population and racial and ethnic groups reporting having experienced adverse circumstances during every data collection period. RESULTS We found that except for non-Hispanic Asians, racial and ethnic minorities were more likely to report loss of employment income, food scarcity, housing insecurity, and unmet needs for mental health services. Prevalence estimates by race/ethnicity for each cycle illustrated the persistent racial/ethnic disparities from April 2020 to the present. CONCLUSIONS The adverse socioeconomic impacts of the COVID-19 pandemic tended to be disproportionately higher for most racial and ethnic minorities compared to non-Hispanic Whites, and this trend continues. CONTRIBUTION This paper analyzes real time population survey data to demonstrate the extent of unequal and adverse socioeconomic impacts of the COVID-19 pandemic on minority groups and highlights the persistence of these trends in adverse socioeconomic outcomes. Much of the focus on human rights and mental health in low- and middle-income countries (LMICs) has been on protection from coercion and abuse and on expanding access to mental health services, rather than on promoting and protecting social and economic rights. Despite the importance of work for mental health, there has been very limited consideration of the relationship between work and mental health in LMICs. This paper draws on ethnographic and participatory research in urban and rural sites in Ghana to illustrate the meanings and value of work, as well as experiences of support, exclusion, and discrimination, among people with lived experience of mental illness in Ghana. The paper outlines the policy context of mental health and human rights in Ghana and evaluates the challenges of implementing mental health, disability, and labor legislation to protect the rights of persons with mental illness-- particularly the poorest and most vulnerable--in both formal and informal employment. • Telehealth has been rapidly adopted to provide continuity of delivery of mental health services in light of COVID-19. • The remote implementation of intensive treatments like dialectical behavioral therapy (DBT) has been vastly understudied. • The current paper describes potential obstacles, proposes solutions, and discusses advantages of transitioning from traditional in-person to a virtual format for full model DBT. • We outline multiple levels of relevance: (1) considerations about the virtual format, (2) considerations for the virtual delivery of DBT, (3) person-level considerations. • The commentary uses examples from our DBT team's rapid transition to telehealth in response to COVID-19-related shut-downs. Telehealth has been rapidly adopted to provide continuity of delivery of mental health services in light of COVID-19. The aim of the current commentary is to describe potential obstacles, propose solutions, and discuss advantages of transitioning from traditional in-person to a virtual delivery format for full model DBT. We speak to these points at multiple levels of relevance to clinical scientists and practitioners: (1) considerations about the virtual delivery format, (2) considerations for the virtual delivery of DBT specifically, and (3) person-level considerations (e.g., client, clinician). We illustrate the commentary using examples of obstacles encountered and solutions identified during our DBT team's rapid transition to telehealth in response to COVID-19-related shut-downs and discuss considerations for the extension of telehealth delivery of DBT going forward. We are hopeful that the rapid transition to telehealth delivery of mental health services in response to COVID-19 serves as a call to action for clinicians to adapt and leverage technology to deliver DBT on a broader scale to improve patient health outcomes. Phelan-McDermid syndrome (PHMDS)/22q13.3 deletion syndrome is a rare genetic disorder associated with autism spectrum disorder (ASD), intellectual disability (ID), and bipolar disorder. While numerous cases have been reported describing successful pharmacological treatment of bipolar disorder in PHMDS, there is currently little guidance available on how to organize and execute such treatment. The aim of the current case study was to explore how pharmacological treatment of bipolar disorder in PHMDS may be organized and evaluated in an outpatient setting. Through a complex process of try and fail, including systematic evaluation of any change to the intervention and never implementing more than one change at the time, the patient gradually improved, regaining his communicative and adaptive skills. Four years passed from referral to this result was achieved. Organizing assessment and treatment as a collaborative effort involving specialized mental health professionals, professional caregivers and the patient's family proved feasible. Many of the challenges present in assessment of psychiatric disorder in individuals with ASD and ID are likely to be present also in evaluation of treatment effects, particularly in disorders where symptoms occur in phases. Recognizing the central role of mental health in global health and affirming the complexity of the universalization of care policy, this paper discusses aspects of global mental health in Brazil and Italy - two countries that are a potential reference for exchange between the global North and South. Using ethnographic and action research methodologies, we conducted a study of a virtual community of practice composed of Brazilians and Italians interested in community mental health care. The results are presented in scenes that provide insights for the international debate in three categories: the doctor-centered approach to care; the institutionalization of care and medicalization of suffering; and the contribution of community practices and non-specialized local knowledge. The locally situated scenes cast light on globally shared critical knots, elucidating a plural set of relationships that run through work processes and mental health care. The sharing of knowledge and experiences highlight what should be universalized: opportunities for horizontal exchange, rather than the production of national identities that radiate universalizing practices and policies. This paper is a collaboration between David Newman, an experienced narrative therapy practitioner and teacher, and Philippa Byers, a narrative therapy student with an academic background in philosophy. The paper charts ideas developed during Philippa's student placement with David, as they discussed narrative practice, other mental health practices and philosophy. The paper draws on philosophy of language and the philosophy of Paul Ricoeur, applying this to Michael White's injunction to look (and listen) for the experience-near in the words and phrases that are offered to narrative therapists. It offers philosophical reflections on an ethical principle of narrative practice which Philippa and David call a first person principle. The first person principle is elaborated in a discussion of David's narrative practice with young people. This offers philosophical and practical insights to some of the issues and questions that may arise for narrative therapists who, like David, practice within mainstream services, encountering 'neuro' and other professionalised discourses and accompanying expectations. • School professionals need support to address the mental health needs of youth during the COVID-19 pandemic. • This paper describes the development of a skills-based group intervention named Coping with COVID-19 (CC-19). • CC-19 seems acceptable by school professionals and has reached hundreds of students. School-aged youth have been significantly impacted by the COVID-19 pandemic. The effects of the pandemic will likely have long-standing effects on the well-being of youth, and access to mental health care is even more critical during this time. For the past 5 years, TRAILS (Transforming Research into Action to Improve the Lives of Students) has been working throughout the state to increase utilization of evidence-based mental health practices among K-12 school mental health professionals (SMHPs). By leveraging SMHPs who are widely accessible to students, TRAILS seeks to improve youth access to effective mental health care and reduce current mental health inequities. In March 2020, TRAILS responded to the COVID-19 pandemic by developing a group manual designed to be delivered virtually by SMHPs to help students develop effective coping skills to mitigate the impact of COVID-19. TRAILS focuses on promoting use of CBT and mindfulness, as these skills are ideally suited for school-based delivery, and thus the new manual, Coping with COVID-19 (CC-19), was grounded in these modalities. This article will describe the design, development, and deployment of the CC-19 program to address the mental health needs of students in the context of the pandemic.
