We studied whether in patients with COPD the use of metformin for diabetes treatment was linked to a pattern of lung function decline consistent with the hypothesis of anti-aging effects of metformin. Patients of GOLD grades 1–4 of the COSYCONET cohort with follow-up data of up to 4.5 y were included. The annual decline in lung function (FEV1, FVC) and CO diffusing capacity (KCO, TLCO) in %predicted at baseline was evaluated for associations with age, sex, BMI, pack-years, smoking status, baseline lung function, exacerbation risk, respiratory symptoms, cardiac disease, as well as metformin-containing therapy compared to patients without diabetes and metformin. Among 2741 patients, 1541 (mean age 64.4 y, 601 female) fulfilled the inclusion criteria. In the group with metformin treatment vs. non-diabetes the mean annual decline in KCO and TLCO was significantly lower (0.2 vs 2.3, 0.8 vs. 2.8%predicted, respectively; p 1 and FVC. These results were confirmed using multiple regression and propensity score analyses. Our findings demonstrate an association between the annual decline of lung diffusing capacity and the intake of metformin in patients with COPD consistent with the hypothesis of anti-aging effects of metformin as reflected in a surrogate marker of emphysema.

Alterations of cognitive functions have been described in COPD. Our study aimed to disentangle the relationship between the degree of cognitive function and COPD characteristics including quality of life (QoL).Data from 1969 COPD patients of the COSYCONET cohort (GOLD grades 1–4; 1216 male/ 753 female; mean (SD) age 64.9 ± 8.4 years) were analysed using regression and path analysis. The DemTect screening tool was used to measure cognitive function, and the St. George‘s respiratory questionnaire (SGRQ) to assess disease-specific QoL.DemTect scores were =60 years of age. For statistical reasons, we used the average of both algorithms independent of age in all subsequent analyses. The DemTect scores were associated with oxygen content, 6-min-walking distance (6-MWD), C-reactive protein (CRP), modified Medical Research Council dyspnoea scale (mMRC) and the SGRQ impact score. Conversely, the SGRQ impact score was independently associated with 6-MWD, FVC, mMRC and DemTect. These results were combined into a path analysis model to account for direct and indirect effects. The DemTect score had a small, but independent impact on QoL, irrespective of the inclusion of COPD-specific influencing factors or a diagnosis of cognitive impairment.We conclude that in patients with stable COPD lower oxygen content of blood as a measure of peripheral oxygen supply, lower exercise capacity in terms of 6-MWD, and higher CRP levels were associated with reduced cognitive capacity. Furthermore, a reduction in cognitive capacity was associated with reduced disease-specific quality of life. As a potential clinical implication of this work, we suggest to screen especially patients with low oxygen content and low 6-MWD for cognitive impairment.

We studied whether in patients with stable COPD blood gases (BG), especially oxygenated hemoglobin (OxyHem) as a novel biomarker confer information on disease burden and prognosis and how this adds to the information provided by the comorbidity pattern and systemic inflammation. Data from 2137 patients (GOLD grades 1–4) of the baseline dataset of the COSYCONET COPD cohort were used. The associations with dyspnea, exacerbation history, BODE-Index (cut-off ≤2) and all-cause mortality over 3 years of follow-up were determined by logistic and Cox regression analyses, with sex, age, BMI and pack years as covariates. Predictive values were evaluated by ROC curves. Capillary blood gases included SaO2, PaO2, PaCO2, pH, BE and the concentration of OxyHem [haemoglobin (Hb) x fractional SaO2, g/dL] as a simple-to-measure correlate of oxygen content. Inflammatory markers were WBC, CRP, IL-6 and -8, TNF-alpha and fibrinogen, and comorbidities comprised a broad panel including cardiac and metabolic disorders. Among BG, OxyHem was associated with dyspnoea, exacerbation history, BODE-Index and mortality. Among inflammatory markers and comorbidities, only WBC and heart failure were consistently related to all outcomes. ROC analyses indicated that OxyHem provided information of a magnitude comparable to that of WBC, with optimal cut-off values of 12.5 g/dL and 8000/µL, respectively. Regarding mortality, OxyHem also carried independent, additional information, showing a hazard ratio of 2.77 (95% CI: 1.85–4.15, p 8000/µL was 2.33 (95% CI: 1.60–3.39, p 2. It thus appears well suited for clinical use with minimal equipment, especially for GPs.

Bronchiectasis (BE) might be frequently present in COPD but masked by COPD symptoms. We studied the relationship of clinical signs of bronchiectasis to the presence and extent of its radiological signs in patients of different COPD severity. Visit 4 data (GOLD grades 1-4) of the COSYCONET cohort was used. Chest CT scans were evaluated for bronchiectasis in 6 lobes using a 3-point scale (0: absence, 1: ≤50%, 2: >50% BE-involvement for each lobe). 1176 patients were included (61%male, age 67.3y), among them 38 (3.2%) with reported physicians' diagnosis of bronchiectasis and 76 (6.5%) with alpha1-antitrypsin deficiency (AA1D). CT scans were obtained in 429 patients. Within this group, any signs of bronchiectasis were found in 46.6% of patients, whereby ≤50% BE occurred in 18.6% in ≤2 lobes, in 10.0% in 3-4 lobes, in 15.9% in 5-6 lobes; >50% bronchiectasis in at least 1 lobe was observed in 2.1%. Scores ≥4 correlated with an elevated ratio FRC/RV. The clinical diagnosis of bronchiectasis correlated with phlegm and cough and with radiological scores of at least 3, optimally ≥5. In COPD patients, clinical diagnosis and radiological signs of BE showed only weak correlations. Correlations became significant with increasing BE-severity implying radiological alterations in several lobes. This indicates the importance of reporting both presence and extent of bronchiectasis on CT. Further research is warranted to refine the criteria for CT scoring of bronchiectasis and to determine the relevance of radiologically but not clinically detectible bronchiectasis and their possible implications for therapy in COPD patients.

The COPD Assessment Test (CAT) is in widespread use for the evaluation of patients with chronic obstructive pulmonary disease (COPD). We assessed whether the CAT items carry additional information beyond the sum score regarding COPD characteristics including emphysema. Patients of GOLD grades 1 to 4 from the COPD cohort COSYCONET (German COPD and Systemic Consequences - Comorbidities Network) with complete CAT data were included (n = 2270), of whom 493 had chest CT evaluated for the presence of emphysema. Comorbidities and lung function were assessed following standardised procedures. Cross-sectional data analysis was based on multiple regression analysis of the single CAT items against a panel of comorbidities, lung function, or CT characteristics (qualitative score, 15th percentile of mean lung density), with age, BMI and gender as covariates. This was supported by exploratory factor analysis. Regarding the relationship to comorbidities and emphysema, there were marked differences between CAT items, especially items 1 and 2 versus 3 to 8. This grouping was basically confirmed by factor analysis. Items 4 and 5, and to a lower degree 1, 2 and 6, appeared to be informative regarding the presence of emphysema, whereas the total score was not or less informative. Regarding comorbidities, similar findings as for the total CAT score were obtained for the modified Medical Research Council scale (mMRC) which was also informative regarding emphysema. Our findings suggest that the usefulness of the CAT can be increased if evaluated on the basis of single items which may be indicating the presence of comorbidities and emphysema.

Background: An association between idiopathic pulmonary fibrosis (IPF) and advancing age is suspected since IPF occurs primarily in patients over 60 years of age. Though, little is known about the disease in the elderly. The aim of this study was to characterize elderly IPF patients using data from the longitudinal, German-wide INSIGHTS-IPF registry. Methods: Patients were grouped into elderly (â¥75 years) and nonelderly IPF (75 years old. Elderly IPF patients compared to younger patients had a higher number of comorbidities (3.6 ± 2.5 vs. 2.8 ± 2.3; p < 0.001). The mean ± SD EQ-5D score (0.64 ± 0.21 vs. 0.69 ± 0.21; p = 0.005), and the overall WHO-5 score (13.1 ± 5.9 vs. 14.3 ± 6.0; p = 0.015) were significantly lower while the UCSD-SOBQ (52.6 ± 31.2 vs. 45.5 ± 31.2; p = 0.030) was significantly higher in elderly patients, indicating a more impaired HRQoL and more breathlessness. At baseline, 55.4% of elderly and 56.8% of nonelderly patients with IPF were treated with antifibrotic therapy (p = 0.687). For FVC decline after initiation of antifibrotic therapy, there was neither a significant difference between age groups at the different time points over 24 months (beta: 0.41; 95%-CI: −0.98 to 1.81; p = 0.563) nor over the whole course of time (beta: −0.05; 95%-CI: −0.20 to 0.09; p = 0.478). All-cause mortality was higher in elderly patients (49.1 vs. 37.9%; HR 1.65; 95%-CI 1.36-2.00; p < 0.001). Antifibrotic therapy was associated with improved survival in IPF patients, independent from age (75 years: beta 0.71; 95%-CI: 0.51-0.98; p = 0.043). Conclusion: In real life, a significant proportion of IPF patients are >75 years old, characterized by higher number of comorbidities and global reduced HRQoL. However, the effect of an antifibrotic therapy was similar between age groups and associated with a survival benefit emphasizing the importance for an early antifibrotic therapy in IPF, independent from age.

We conducted a study in which COPD patients with stable, chronic hypercapnia were treated with NIV and nasal high-flow (NHF) to compare effectiveness.Methods: In a multi-centered, randomized, controlled, cross-over design, patients received 6 weeks of NHF ventilation followed by 6 weeks of NIV ventilation or vice-versa (TIBICO) between 2011 and 2016. COPD patients with stable daytime hypercapnia (pCO2≥50 mmHg) were recruited from 13 German centers. Rationale Alterations of acid-base metabolism are an important outcome predictor in acute exacerbations of COPD, whereas sufficient metabolic compensation and adequate renal function are associated with decreased mortality. In stable COPD there is, however, only limited information on the combined role of acid-base balance, blood gases, renal and respiratory function on exacerbation risk grading. Methods We used baseline data of the COPD cohort COSYCONET, applying linear and logistic regression analyses, the results of which were implemented into a comprehensive structural equation model. As most informative parameters it comprised the estimated glomerular filtration rate (eGFR), lung function defined via forced expiratory volume in 1 s (FEV1), intrathoracic gas volume (ITGV) and (diffusing capacity for carbon monoxide (DLCO), moreover arterial oxygen content (CaO2), partial pressure of oxygen (PaCO2), base exess (BE) and exacerbation risk according to GOLD criteria. All measures were adjusted for age, gender, body-mass index, the current smoking status and pack years. Results 1506 patients with stable COPD (GOLD grade 1–4; mean age 64.5 ± 8.1 y; mean FEV1 54 ± 18 %predicted, mean eGFR 82.3 ± 16.9 mL/min/1.73 m2) were included. BE was linked to eGFR, lung function and PaCO2 and played a role as indirect predictor of exacerbation risk via these measures; moreover, eGFR was directly linked to exacerbation risk. These associations remained significant after taking into account medication (diuretics, oral and inhaled corticosteroids), whereby corticosteroids had effects on exacerbation risk and lung function, diuretics on eGFR, BE and lung function. Conclusion Even in stable COPD acid-base metabolism plays a key integrative role in COPD risk assessment despite rather small deviations from normality. It partially mediates the effects of impairments in kidney function, which are also directly linked to exacerbation risk.

Background. The aim of this study was to analyze daytime sleepiness in a sample of patients diagnosed with sarcoidosis. Methods. A sample of 1197 German sarcoidosis patients was examined with the Epworth Sleepiness Scale (ESS), the Fatigue Assessment Scale, the Hospital Anxiety and Depression Scale, the Pittsburgh Sleep Quality Index, and the Short-Form Health Survey (SF-8). The patients’ ESS mean scores were compared with those obtained from a large general population sample. Results. Exactly 50% of the patients reached the criterion (ESS > 10) for excessive daytime sleepiness, compared with only 22.1% in the general population. The effect size for the mean score difference between both samples was d=0.62. The number of affected organs and the number of concomitant diseases proved to be significant independent predictors of daytime sleepiness. Sleepiness was associated with fatigue (r=0.45), anxiety (r=0.23), depression (r=0.28), sleep problems (r=0.23), and detriments in physical (r=−0.29) and mental (r=−0.28) quality of life. Conclusions. The issue of excessive daytime sleepiness should be considered in the management of sarcoidosis.

Background Nasal Highflow (NHF) delivers a humidified and heated airflow via nasal prongs. Current data provide evidence for efficacy of NHF in patients with hypoxemic respiratory failure. Preliminary data suggest that NHF may decrease hypercapnia in hypercapnic respiratory failure. The aim of this study was to evaluate the mechanism of NHF mediated PCO2 reduction in patients with chronic obstructive pulmonary disease (COPD). Methods In 36 hypercapnic COPD patients (PCO2 > 45 mmHg), hypercapnia was evaluated by capillary gas sampling 1 h after NHF breathing under four conditions A to D with different flow rates and different degrees of leakage (A = 20 L/min, low leakage, two prongs, both inside; B = 40 L/min, low leakage, two prongs, both inside; C = 40 L/min, high leakage, two prongs, one outside and open; D = 40 L/min, high leakage, two prongs, one outside and closed). Under identical conditions, mean airway pressure was measured in the hypopharynx of 10 COPD patients. Results Hypercapnia significantly decreased in all patients. In patients with capillary PCO2 > 55 mmHg (n = 26), PCO2 additionally decreased significantly by increased leakage and/or flow rate in comparison to lower leakage/ flow rate conditions (A = 94.2 ± 8.2%; B = 93.5 ± 4.4%; C = 90.5 ± 7.2%; D = 86.8 ± 3.8%). Nasal highflow (NHF) provides a warmed and humidified air stream up to 60 L/min. Recent data demonstrated a positive effect in patients with acute hypoxemic respiratory failure, especially when caused by pneumonia. Preliminary data show a decrease in hypercapnia in patients with COPD. Therefore, NHF should be evaluated as a new ventilatory support device. This study was conducted to assess the impact of different flow rates on ventilatory parameters in patients with COPD.This interventional clinical study was performed with patients suffering from severe COPD. The aim was to characterize flow-dependent changes in mean airway pressure, breathing volumes, breathing frequency, and decrease in partial pressure of CO2 (pCO2). Mean airway pressure was measured in the nasopharyngeal space (19 patients). To evaluate breathing volumes, we used a polysomnographic device (18 patients). All patients received 20 L/min, 30 L/min, 40 L/min, and 50 L/min and - to illustrate the effects - nasal continuous positive airway pressure and nasal bilevel positive airway pressure. Capillary blood gas analyses were performed in 54 patients with hypercapnic COPD before and two hours after the use of NHF. We compared the extent of decrease in pCO2 when using 20 L/min and 30 L/min. Additionally, comfort and dyspnea during the use of NHF were surveyed.NHF resulted in a minor flow dependent increase in mean airway pressure. Tidal volume increased, and breathing rate decreased. The calculated minute volume decreased under NHF breathing. In spite of this fact, hypercapnia decreased with increasing flow (20 L/min vs 30 L/min). Additionally, an improvement in dyspnea was observed. The rapid shallow breathing index shows a decrease when using NHF.NHF leads to a flow-dependent reduction in pCO2. This is most likely achieved by a washout of the respiratory tract and a functional reduction in dead space. In summary, NHF enhances effectiveness of breathing in patients with COPD, reduces pCO2, the work of breathing, and rapid shallow breathing index as an indicator of respiratory work load.

Background: In times of limited organs for transplantation, anti-HBc positive organs can be accepted for lung transplantation to increase the number of donors. Transplant recipients should be vaccinated against hepatitis B to prevent HBV infection. However, response after HBV vaccination has only been poorly evaluated in patients with end-stage pulmonary disease. Material/Methods: Anti-HBs titers of 40 anti-HBc negative patients with end-stage pulmonary disease evaluated for lung transplantation were analyzed with the Architect® system (Abbott, Germany). Responders, partial responders, or non-responders after HBV vaccination were defined by anti-HBs titers >100 IU/L, 10–100 IU/L, and 10 IU/L. Thus, anti-HBs titers should be regularly monitored. Nonresponders should be considered for booster vaccinations, alternative vaccination schedules, or prophylactic treatment with a nucleos(t)ide analogue in case of transplantation of an anti-HBc–positive organ.

Background Guidelines on the diagnosis and management of idiopathic pulmonary fibrosis (IPF), a rare manifestation of chronic progressive fibrosing interstitial pneumonia, have been updated by ATS/ERS/JRS/ALAT in 2011. In Europe, data are limited on the characteristics and management of such patients. Methods/design Investigating significant health trends (INSIGHTS)-IPF is a prospective observational longitudinal registry designed to describe the characteristics and management of newly diagnosed (incident) and prevalent patients with IPF on the long term. The registry uses a non-probability sampling approach to collect data on characteristics, therapeutic interventions, health-related quality of life and health economic parameters. At least 500 patients in ambulatory care will be included consecutively in about 30 centres. The study has been initiated in November 2012, and currently (December 2013) follows 344 patients. ClinTrials.gov identifier is NCT01695408. Discussion INSIGHTS-IPF documents one of the largest IPF cohorts in Europe. The registry is expected to provide much-needed data on the characteristics and management situation of patients with IPF in Germany. It will allow comparisons with other countries. Gap analyses based on current guidelines for management of these patients will be possible.

We describe a patient who underwent thoracic radiation therapy for biopsy-proven pulmonary spindle cell sarcoma in the left lower lobe, 15 months after birth. At the age of 37 she developed shoulder pain, fatigue, and progressive exertion dyspnoea. Chest X-ray revealed a pulmonary mass in the left lower lobe due to a cytology-proven malignant tumour.The patient underwent left pneumonectomy. Histology revealed a myosarcoma of the lung, similar to the previous sarcoma. Furthermore, the patient was diagnosed to have Turner syndrome mosaic and chromosomal analysis revealed a translocation t(1;13) in 3/50 metaphases. However a germline mutation of the p53 tumour suppressor gene was excluded. After 2 years of follow-up the patient is stable and there are no signs of recurrence of the tumour.We conclude a re-occurrence of this very rare malignant disorder of the lung after a 36-year interval in a patient with Turner syndrome mosaic. Following initial curative radiation therapy, with a remission over 36 years, lung resection was now successfully performed.

Die Lymphangioleiomyomatose (LAM) ist eine sehr seltene, ausschließlich Frauen betreffende zystische Lungenerkrankung. Vor einigen Jahren wurde das VEGF-D als diagnostischer Marker etabliert, jedoch ist noch immer unklar, welche Rückschlüsse sich aus der Höhe des VEGF-D auf die Erkrankungen ziehen lassen. Zielstellung dieser Arbeit war es daher, mögliche Confounder herauszufinden, therapiebedingte Veränderungen von VEGF-D und der LAM zu erfassen und daraus eine Aussage zur Relevanz des Parameters als Diagnose-, Verlaufs- und Prognosemarker abzuleiten. Hierfür wurden insgesamt 123 Frauen zwischen 18 und 85 Jahren vom Uniklinikum Leipzig, der Uniklinik Heidelberg sowie aus der LIFE-Studie der Universität Leipzig rekrutiert, darunter 46 LAM-Patientinnen sowie 40 anderweitig lungenkranke und 37 lungengesunde Probandinnen. Bei ihnen allen wurde (zum Teil mehrmals) der VEGF D-Spiegel im Blutserum bestimmt sowie anhand der Krankenakte und eines entworfenen Fragebogens die Anamnese erhoben. Schwerpunkt der Datenerhebung stellten neben der VEGF-D-Messung die allgemeinen deskriptiven Angaben, Informationen zur LAM-Erkrankung, östrogenbetonte Situationen, Nikotinanamnese und Lungenfunktions- und Blutgasparameter im Längs- und Querschnitt dar. Insgesamt bestätigte sich das VEGF-D als relevanter labordiagnostischer Marker für die LAM und zeigte sich bei den Patientinnen signifikant erhöht gegenüber der anderweitig lungenkranken und lungengesunden Kontrollgruppe. Die Abhängigkeit des VEGF-D vom CT-Stadium sowie von der Beeinträchtigung der Diffusionsfähigkeit der Lunge wiesen darauf hin, dass seine Höhe zudem ein Marker für die Schwere der Erkrankungsausprägung ist. Darüber hinaus zeigte sich bei erhöhtem Ausgangs-VEGF D-Spiegel eine schlechtere Entwicklung der DLCO/VA, wodurch er möglicherweise auch prognostische Aussagen erlaubt. Als Confounder des VEGF-D-Spiegels wurde die Nikotinanamnese festgestellt, die so noch keine Erwähnung in der Literatur fand. Ein anamnestischer Nikotinkonsum erhöhte den VEGF-D-Spiegel signifikant (0,820 vs. 1,920 ng/ml, p=.047), darüber hinaus zeigte sich zudem eine Abhängigkeit von der Dauer des Nikotinkonsums. Auch das Vorliegen lymphatischer Ergüsse in der Anamnese ging mit einem erhöhten VEGF D einher. Für östrogenabhängige Situationen wie bspw. eine Schwangerschaft, die zum Teil eine klinische Verschlechterung der Erkrankung bewirken, konnte kein Einfluss auf den VEGF-D-Spiegel nachgewiesen werden. Auch typische Erscheinungen im Rahmen einer LAM wie Pneumothoraces oder das Vorliegen von Angiomyolipomen stellten keine Confounder des VEGF-D dar. Der CO2-Partialdruck, der in bisherigen Studien kaum untersucht worden war, wies von den untersuchten Lungenparametern die stärkste Korrelation zum VEGF-D-Spiegel auf. Zudem zeigte er signifikante Unterschiede je nach CT-Stadium der LAM. Die Patientinnen scheinen in einem bislang noch nicht bekannten Maß mit zunehmender Schwere der Erkrankung eine Hypokapnie und damit Hyperventilation aufzuweisen, die sich jedoch im Verlauf unter Sirolimustherapie signifikant bessert (Differenz des PaCO2: -3,64 ±2,66 mmHg ohne Therapie vs. +2,48 ±3,07 mmHg unter Therapie, p

Beatmungsbedingte Lungenschäden in der Therapie des akuten Atemnotsyndroms (ARDS) sind aufgrund der inhomogenen Vorschädigung der Lunge praktisch unvermeidbar. Die unphysiologische mechanische Belastung der Lunge führt über Volutrauma, Atelektotrauma und Biotrauma nicht selten zur Exazerbation des Syndroms und trägt zur hohen Mortalität des ARDS bei. Pharmakologische Interventionsmöglichkeiten sind Gegenstand der aktuellen Forschung. Diesbezüglich vielversprechend ist die zentrale Komponente des Kinin-Kallikrein-Systems, namentlich Bradykinin, das über seinen B2-Rezeptor anti-apoptotische Signalwege aktivieren kann und somit zellprotektive Wirkung besitzt. In der vorliegenden Arbeit wurde untersucht, in welcher Weise zyklische mechanische Dehnung die Konzentration einzelner Komponenten des Kinin-Kallikrein-Systems in isolierten alveolären Epithelzellen (Typ II) der Ratte beeinflusst. Dafür wurden die alveolären Typ-II-Zellen auf speziellen BioFlex®-Membranen kultiviert und für 24 Stunden zyklisch mit hoher Dehnungsamplitude gedehnt. Anschließend wurden mit etablierten Analysemethoden in Zellüberständen bzw. Zelllysaten die Konzentrationen von Kininogen 1, Bradykinin und vom B2-Rezeptor gemessen, sowie die Aktivität des Enzyms Kallikrein und des Bradykinin-Abbaus bestimmt - jeweils im Vergleich mit Überständen bzw. Lysaten ungedehnter AT-II-Zellkulturen. Es zeigte sich dehnungs-bedingt eine Zunahme der Bradykinin-Produktion durch Kininogen und Kallikrein und eine stark gesteigerte Bradykinin-abbauende Aktivität, sodass sich der Bradykininspiegel insgesamt verringerte. Die Konzentration des B2-Rezeptors blieb unverändert. Detailliertes Wissen über den Einfluss zyklischer mechanischer Dehnung auf die Einzelkomponenten des Kinin-Kallikrein-Systems ist eine Grundvoraussetzung, um die zellprotektive Wirkung von Bradykinin im Sinne einer pharmakologischen Interventionsmöglichkeit bei ARDS nutzbar machen zu können.:Inhaltsverzeichnis………………………………………………………………………3 1. Adipositas gehört zu den häufigsten Stoffwechselkrankheiten. Zur Basistherapie der Adipositas gehören eine gesunde Ernährungsweise und die Erhöhung der körperlichen Aktivität mit dem Ziel der Gewichtsreduktion. Körperliches Training führt neben der Verbesserung der Leistungsfähigkeit zu einer Reihe metabolischer Veränderungen, wie zur Reduktion der Fettmasse, zu Verbesserungen von chronischer Hyperglykämie, des Lipidstoffwechsels und des atherogenen, pro-inflammatorischen Adipokin-Serumprofils. Bone morphogenetic proteins (BMPs) werden auch im Fettgewebe produziert und spielen eine wichtige Rolle in der Adipogenese und Transdifferenzierung von Adipozyten. Vor kurzem wurde ein Zusammenhang zwischen der BMP-7 Serumkonzentration und Adipositas postuliert. Allerdings ist bisher nicht bekannt, ob eine Gewichtsreduktion mit Veränderungen der BMP-7 Serumkonzentration assoziiert ist. Ziel dieser Arbeit war es deshalb zu untersuchen, ob die Serumkonzentration von BMP-7 im Zusammenhang mit Körpergewicht, Fettverteilungsmuster, Parametern des Glukosestoffwechsels und der Insulinsensitivität bei Patienten mit Adipositas und Typ 2 Diabetes (n=213) stehen. Im Rahmen von zwei Interventionsstudien mit dem Ziel der Gewichtsreduktion wurde der Einfluss einer hypokalorischen Ernährungsweise mit zusätzlichem körperlichen Training über 6 Monate (n=19) sowie 6 Monaten nach einer bariatrischen Operation (n=14) auf die BMP-7 Serumkonzentration untersucht. Zusätzlich wurde die BMP-7-mRNA Expression im humanen omentalen und subkutanen Fettgewebe von 233 Patienten im Rahmen einer unabhängigen Querschnittstudie charakterisiert. Die BMP-7 Serumkonzentration ist unabhängig von Alter und Geschlecht, korreliert signifikant mit den nüchtern Insulin, C-Peptid und Glukose Serumkonzentrationen, mit der SFRP5-Konzentration bei Probanden ohne Typ2 Diabetes mellitus, signifikant negativ mit den Serumkonzentrationen von Gesamteiweiß, Chemerin, freien Fettsäuren (bei Männern), und DLK-1 (bei Frauen). Bei Patienten mit Typ 2 Diabetes zeigte sich eine negative Korrelation zwischen der BMP-7-Serumkonzentration und ALAT. Die BMP-7 mRNA Expression ist im viszeralen Fettgewebe signifikant höher als im subkutanen Fettgewebe. Sowohl eine moderate Gewichtsreduktion von 8,8±6,5kg [2,0-26,6kg] durch kalorienreduzierte Ernährung als auch ein Gewichtsverlust von 45,3±7,4kg 6 Monate nach bariatrischer Chirurgie führten nicht zu einer signifikanten Veränderung der zirkulierenden BMP-7 Spiegel. Zusammengefasst zeigten die Querschnitts- und Interventionsstudien, dass erhöhte BMP-7- Serumkonzentrationen mit Hyperinsulinämie assoziiert sind, aber durch Gewichtsreduktion und Erhöhung der körperlichen Aktivität nicht signifikant beeinflusst werden können. Die erhöhte BMP-7-Expression im viszeralen Fettgewebe könnte eine intrinsische Besonderheit dieses Fettdepots sein und zur viszeralen Fehlverteilung des Fettgewebes bei Adipositas beitragen.

Bibliographische Zusammenfassung 82 Seiten gesamt, 54 Seiten Textteil, 108 Literaturangaben, 20 Abbildungen, 5 Tabellen, 4 Anlagen Referat: Ziel der vorliegenden Arbeit war die Betrachtung der Krankenhausmortalität und deren Risikofaktoren sowie des 3-Jahres-Überlebens bei invasiv beatmeten Patienten der internistischen Intensivstation (ITS). Es wurden 250 erwachsene Patienten des Universitätsklinikums Leipzig mit Aufnahme im Jahr 2007 in diese Studie eingeschlossen. Erfasst wurde eine große Anzahl an Patientendaten: Erkrankungen, der Aufnahmegrund, die Aufenthaltsdauer, Informationen zur Intubation/Tracheotomie und zur Beatmung, das hämodynamische Monitoring, erweiterte invasive Maßnahmen, der APACHE-Score sowie Informationen zum Tod (APACHE= Acute Physiology and Chronic Health Evaluation). Eine Krankenhausmortalität von 49,6 % konnte ermittelt werden - eine relativ hohe Mortalität verglichen mit anderen Studien. Das Vorliegen eines Tumorleidens, einer Niereninsuffizienz oder einer Reanimation im Verlauf wurden als Risikofaktoren, im Krankenhaus zu versterben, erkannt. Die unterschiedliche Mortalität thematisch ähnlicher Studien könnten u.a. durch die unterschiedlichen Ein- und Ausschlusskriterien erklärt werden. In der Regel ist die Mortalität auf internistischen höher als auf chirurgischen ITS. Die Notwendigkeit zur invasiven Beatmung weist auf einen kritischen Gesundheitszustand hin, der häufiger zum Tode führt. Eine Aussage über das genaue Mortalitätsrisiko jedes einzelnen Patienten ist nicht möglich. Grenzentscheidungen zwischen Leben und Tod müssen im Hinblick auf die Gesamtsituation getroffen werden. Der Gesundheitszustand sowie der Patientenwille sollten bei der Entscheidung eine ausschlaggebende Rolle spielen. Die ermittelten Risikofaktoren können während dieses Prozesses als Hilfsmittel bei der Entscheidungsfindung herangezogen werden.

Obwohl die künstliche bzw. mechanische Beatmung bei der Therapie von ALI / ARDS eine wichtige und bedeutende Rolle spielt, kann sie selbst eine akute Lungen-schädigung auslösen oder bestehende pulmonale Beeinträchtigungen verstärken. Zentraler Schädigungsmechanismus ist die alveoläre Überdehnung durch hohe Ti-dalvolumina. Selbst bei der Anwendung kleiner, protektiver Tidalvolumina in Lungen mit einem nur geringen Anteil belüfteter Alveolen kann es in diesen zu alveolärer Überdehnung kommen. Diese Überdehnung führt einerseits zu mechanisch induzier-te Apoptose sowie Nekrose und andererseits zu einer mechanisch induzierten Ver-änderung der Mediatorenfreisetzung hin zu einem pro-inflammatorischen Muster. Da der Transkriptionsfaktor NF-κB zahlreiche Mediatoren aktiviert bzw. von ihnen beeinf-lusst werden kann, nimmt er in diesem Geschehen eine ganz besondere Schlüssel-position ein. In der vorliegenden Arbeit wird der Hypothese nachgegangen, ob die NF-κB-Aktivierung bei der mechanischen Dehnung und dem daraus resultierenden inflam-matorischen Verhalten von pulmonalen Strukturzellen verändert wird und in wie weit ein Zusammenhang zwischen Dehnung, Zellschädigung und NF-κB besteht. Dafür wurden sowohl frisch isolierte alveoläre Ratten-Typ-II Zellen, Zellen der hu-man-alveolaren Epithelzelllinie A549 sowie Lungen- Fibroblasten der Zell-Linie Wi 38 untersucht. Alle drei Zellarten wurden auf einem speziellen elastischen Silikonboden von 6er-Well-Platten inkubiert, wo sie mit Hilfe des Flexercell-Stretch-Gerätes (FX 3000) als Zellmonolayer equibiaxial für 24 Stunden gedehnt wurden. Auch die zeitliche Abhängigkeit der NF-κB-Expression von der mechanischen Deh-nung wurde untersucht. Dabei konnte festgestellt werden, dass ein Zusammenhang zwischen NF-κB-Aktivierung, Zellschädigung und mechanischer Dehnung existiert. Wobei bei unter-schiedlichen Zellarten auch variierende Ergebnisse beobachtet werden konnten. Im Zusammenhang mit anderen aus unserer Forschungsgruppe und in der Literatur stammenden Erkenntnissen konnte so eine Verknüpfung zwischen NF-κB-Aktivierung, Zytokinfreisetzung und inflammatorischer pulmonaler Reaktion nachge-wiesen werden.
