The operating room is a specific workplace that requires operating room nurses to possess key competencies in the areas of perioperative healthcare, ethics, legislation, communication, and organizational skills, with continuous professional development through education and research. This knowledge will be acquired through basic and specialized training, which will continue to be integrated into the process in the field of perioperative care in a targeted manner. The need for immediate and effective action by nurses in the operating room in unpredictable situations requires them to respond appropriately, which requires knowledge and practice only possible through structured formal training. These trainings have clear learning outcomes and competencies for knowledge assessment procedures. The current challenge is poorly designed processes for integrating inexperienced staff into perioperative care without explicit requirements to complete perioperative specialist training., Operacijska je dvorana specifično radilište koje od operacijske medicinske sestre / medicinskog tehničara zahtijeva ključne kompetencije iz područja perioperativne zdravstvene njege, etike, zakonodavstva te komunikacijskih i organizacijskih vještina uz kontinuirani profesionalni razvoj koji se stječe obrazovanjem i istraživanjem. Medicinske sestre / medicinski tehničari svoje kompetencije trebaju stjecati temeljnim i specijalističkim obrazovanjem na koje se nastavlja proces dobro osmišljene integracije u područje perioperacijske skrbi. Potreba za hitnim i učinkovitim djelovanjem u nepredvidivim situacijama imperativ je za medicinske sestre / tehničare u operacijskim dvoranama koji moraju reagirati na pravi način, što podrazumijeva znanje i praksu koji su jedino mogući uz strukturiranu formalnu izobrazbu jasno definiranim ishodima učenja, postupcima evaluacije znanja i izlaznim kompetencijama. Trenutni je izazov nedovoljno dobro osmišljen proces integracije novozaposlenih u područje perioperacijske skrbi bez jasnih zahtjeva u kontekstu završetka specijalističke izobrazbe.

Objective: to identify the state-of-the-art developed about surgical Nursing care in the operating room. Method: an integrative review with search and selection of primary studies conducted in April and May 2022 in four major data sources in the health field, namely: LILACS, SciELO, BDENF and PubMed, from 2018 to 2022. The sample consisted of 13 studies, 8 selected from a universe of 293 articles and 5 obtained through the manual search. Results: three categories were generated: Associating surgical care with care, people and material resources management for patient safety; Surgical care and ethical sensitivity in the search for patients' well-being; and Operating room nurses' manifestation and omission experiences and surgical care. Conclusions: surgical Nursing care in the operating room is varied and is manifested through meeting the administrative demands, ethical sensitivity, body temperature maintenance, preservation of individuality and meeting all the needs in the intraoperative period when patients are more vulnerable, including being duly informed as a way to provide safe surgical care. On the other hand, nurses acknowledge the need to change their world view and direct surgical care towards a more human, social and cultural perspective. RESUMO Objetivo: identificar o estado da arte que foi desenvolvido sobre os cuidados cirúrgicos do enfermeiro da sala de cirurgia. Método: revisão integrativa com pesquisa e seleção de estudos primários realizada entre abril e maio de 2022 em quatro fontes de dados destacadas no campo da saúde: LILACS, SciELO, BDENF e PubMed de 2018 a 2022. A amostra consistiu-se de 13 estudos, 8 selecionados de um total de 293 artigos e 5 pesquisados manualmente. Resultados: foram geradas três categorias: associação do cuidado cirúrgico com a gestão dos cuidados, pessoas e recursos materiais para a segurança do paciente; cuidado cirúrgico e sensibilidade ética na busca do bem-estar do paciente e as experiências de manifestação e omissão do enfermeiro da sala de cirurgia e o cuidado cirúrgico. Conclusões: o cuidado cirúrgico na sala de cirurgia é variado e se manifesta através da atenção às exigências administrativas, sensibilidade ética, manutenção da temperatura corporal, preservação da individualidade e atenção a todas as necessidades no ambiente intra-operatório onde o paciente é mais vulnerável, incluindo o dever de estar informado como um meio de proporcionar um cuidado cirúrgico seguro. Por outro lado, reconhece a necessidade de mudar sua cosmovisão e direcionar os cuidados cirúrgicos para uma perspectiva mais humana, social e cultural. RESUMEN Objetivo: identificar el estado de arte que se ha desarrollado sobre el cuidado quirúrgico de la enfermera de sala de operaciones. Método: revisión integrativa con indagación y selección de estudios primarios efectuada en abril y mayo de 2022 en cuatro fuentes de datos destacadas en el campo de la salud: Lilacs, SciELO, BDENF y PubMed de 2018 a 2022. La muestra se conformó por 13 estudios, 8 seleccionados de un universo de 293 artículos y 5 de búsqueda manual. Resultados: se generaron tres categorías: asociando cuidado quirúrgico con gestión de cuidado, de personas y de recursos materiales para la seguridad del paciente; el cuidado quirúrgico y la sensibilidad ética en la búsqueda de bienestar para el paciente y las experiencias de manifestación y omisión del enfermero de quirófano y el cuidado quirúrgico. Conclusiones: el cuidado quirúrgico en la enfermera de sala de operaciones es variado y se manifiesta a través de la atención a las demandas administrativas, la sensibilidad ética, el mantenimiento de la temperatura corporal, conservación de la individualidad y atención a todas las necesidades en el intraoperatorio donde el paciente es más vulnerable, incluyendo el estar informada como una forma de brindar un cuidado quirúrgico seguro. Por otro lado, reconoce la necesidad de cambiar su visión de mundo y dirigir el cuidado quirúrgico hacia una mirada más humana, social y cultural.

Objective: To implement, on health management software, electronic records of the perioperative nursing process and the stages of transoperative and immediate postoperative nursing diagnoses, based on the NANDA International taxonomy. Method: Experience report conducted from the completion of the Plan-Do-Study-Act cycle, which allows improvement planning with a clearer purpose, directing each stage. This study was carried out in a hospital complex in southern Brazil, using the software Tasy/Philips Healthcare. Results: For the inclusion of nursing diagnoses, three cycles were completed, predictions of expected results were established, and tasks were assigned, defining “who, what, when, and where”. The structured model covered seven possibilities of aspects, 92 symptoms and signs to be evaluated, and 15 nursing diagnoses to be used in the transoperative and immediate postoperative periods. Conclusion: The study allowed implementing electronic records of the perioperative nursing process on health management software, including transoperative and immediate postoperative nursing diagnoses, as well as nursing care. RESUMEN Objetivo: Implementar, en un software de manejo de la salud, registros electrónicos del proceso de enfermería perioperatorio y la etapa de diagnósticos de enfermería transoperatorios y postoperatorios inmediatos, con base en la taxonomía NANDA internacional. Método: Informe de experiencia realizado a partir de la consecución del ciclo Plan-Do-Study-Act), que permite planificar la mejora con un propósito más claro, dirigiendo cada etapa. Este estudio fue realizado en un complejo hospitalario en el sur de Brasil, utilizando el Software Tasy/Philips Healthcare. Resultados: Para la inclusión de diagnósticos de enfermería, se concluíran tres ciclos, se estableceran predicciones de los resultados esperados, y se asignaron tareas, definiendo “quién, qué, cuándo y dónde”. El modelo estructurado contempló siete posibles aspectos, 92 signos y síntomas para ser evaluados y 15 diagnósticos de enfermería para ser utilizados en el transperatorio y postoperatorio inmediato. Conclusión: El estudio permitió implementar, en un software de manejo de la salud, registros electrónicos del proceso de enfermería perioperatorio, que comprende diagnósticos de enfermería transoperatoria y postoperatoria inmediata, además de los cuidados de enfermería. Método: Relato de experiência conduzido a partir da realização do ciclo PDSA (Plan-Do-Study-Act), o qual permite o planejamento de melhoria com um propósito mais claro, direcionando cada etapa. Este estudo foi realizado em um complexo hospitalar da região sul do Brasil, utilizando o software Tasy/Philips Healthcare. Resultados: Para a inclusão dos diagnósticos de enfermagem, rodaram-se três ciclos, estabeleceram-se previsões de resultados esperados, e as tarefas foram atribuídas, definindo “quem, o quê, quando e onde”. O modelo estruturado contemplou sete possibilidades de aspectos, 92 sinais e sintomas a serem avaliados e 15 diagnósticos de enfermagem para serem utilizados no transoperatório e pós-operatório imediato. Conclusão: O estudo possibilitou implementar, em um software de gestão em saúde, os registros eletrônicos do processo de enfermagem perioperatório, compreendendo diagnósticos de enfermagem do transoperatório e pós-operatório imediato, além de cuidados de enfermagem.

AMAÇ: Ameliyathaneler, sağlık bakım alanları içerisinde stresin üst düzeyde yaşandığı ve hızlı karar vermenin gerekli olduğu alanlar olması nedeniyle hata yapma riskinin yüksek olduğu çalışma ortamlarıdır. Ameliyathane hemşirelerinin profesyonel bir şekilde rol ve sorumluluklarını yerine getirebilmesi alana özgü yeterli bilgi ve beceriye sahip olmasına bağlıdır. Ameliyathane hemşirelerinin intraoperatif dönem hasta bakımına hakim olmalarının, hasta güvenliğine yönelik düşme, basınç yarası, yanık, yabancı cisim unutulması, cerrahi alan enfeksiyonu gibi riskleri azaltacağı ve ameliyat kalitesini artıracağı düşünülmektedir. Bu amaçlarla çalışmamızda Afyonkarahisar ilindeki ameliyathane hemşirelerinin intraoperatif dönem hasta bakımı bilgi düzeylerinin belirlenmesi amaçlanmıştır.GEREÇ VE YÖNTEM: Tanımlayıcı türde olan bu çalışma Ağustos - Eylül 2014 tarihleri arasında, Afyonkarahisar ili devlet ve özel hastanelerinde çalışan bilgilendirilmiş sözlü onamı alınan 60 ameliyathane hemşiresine uygulanmıştır. Çalışma verileri, iki bölümden oluşan anket formu ile toplanmıştır. Birinci bölümde katılımcıların sosyodemografik özelliklerini içeren 6 soru bulunmaktadır. İkinci bölümde ise araştırmacılar tarafından ilgili literatür doğrultusunda ve uzman görüşü alınarak hazırlanan ameliyathane hemşirelerinin intraoperatif hasta bakımına ilişkin bilgi düzeylerinin belirlenmesine yönelik 3’lü likert tipte, 36 madde bulunmaktadır. Verilerin değerlendirilmesinde ve analizinde SPSS for Windows 18.00 programından yararlanılmıştır. Araştırmadaki sürekli değişkenlere ait tanımlayıcı istatistikler için ortalama, standart sapma, median, minimum, maksimum değerleri, kategorik değişkenlere ait tanımlayıcı istatistikler frekans ve yüzde ile hesaplanmıştır.BULGULAR: Çalışmaya katılan hemşirelerin yaş ortalamasının 32,03±7,32 olduğu (min:19 max:50), %86,7’nin kadın, %45,0’ inin lisans mezunu, %73,3’ ünün hemşirelik mezunu olduğu, %61,7’ sinin 6 yıldan daha uzun süredir meslekte ve %53,3 ’ünün 6 yıldan daha uzun süredir ameliyathane de çalıştığı saptanmıştır. Çalışanların intraoperatif dönem hasta bakım bilgi puanı ortalaması 26,13±5,42 olarak bulunmuştur.SONUÇ: Bu çalışmanın sonucunda Afyonkarahisar ili ameliyathanelerinde çalışan hemşirelerin intraoperatif dönem hasta bakımı bilgi düzeylerinin yeterli seviyede olduğu ancak düzeltilmesi ve geliştirilmesi gereken uygulamalar olduğu belirlenmiştir., OBJECTIVE: Operating rooms are work environments where the risk of making mistakes is high in health care areas, as they are areas where stress is experienced at a high level and quick decision-making is required. The ability of operating room nurses to fulfill their roles and responsibilities professionally depends on having sufficient knowledge and skills specific to the field. It is thought the fact that operating room nurses have a grasp of intraoperative period patient care will reduce the risks such as falling, pressure wound, burns, foreign body forgetting, surgical site infection, and increase the quality of the surgery. For these purposes, we aimed to determine the intraoperative patient care knowledge levels of the operating room nurses in the city of Afyonkarahisar.MATERIAL AND METHODS: This descriptive study was carried out with 60 operating room nurses working in the public and private hospitals of the city of Afyonkarahisar , after their informed verbal consent was obtained, between August and September 2014. The data were collected through a survey form consisting of two parts. In the first part, there are 6 questions about the socio-demographic characteristics of the participants. In the second part, a 3-point Likert type questionnaire consisting of 36 items designed by the researchers after the review of relevant literature and taking expert opinion was used to determine the knowledge levels of operating room nurses regarding intraoperative patient care. The SPSS for Windows 18.00 program was used in the evaluation and analysis of the data. Descriptive statistics of continuous variables were shown with mean, standard deviation, median, minimum and maximum values, while descriptive statistics of categorical variables were shown with frequency and percentage.RESULTS: Of the nurses participating in the study, the mean age was 32.03 ± 7.32 (min: 19 max: 50), 86.7% were female, 45.0% had an undergraduate degree, and 73.3% were nursing graduates. It was determined that 61.7% of them had been in the profession for more than 6 years while 53.3% working in the operating room for more than 6 years. The intraoperative period patient care knowledge mean score of the nurses was found to be 26.13 ± 5.42.CONCLUSIONS: As a result of this study, nurses working in the operating rooms of the city of Afyonkarahisar were determined to have a sufficient level of intraoperative patient care knowledge, however, there have been practices identified that need to be corrected and improved.

The nursing work in the operating room has the characteristics of long time, strong technicality, and heavy work, which have an important influence on the quality of the operation. Operating room nursing recommendations based on data mining technology can solve a series of practical problems in clinical nursing and nursing management. This paper selects the clustering algorithm in commonly used data mining technology as the research object and actually analyzes the impact of this algorithm in operating room nursing recommendations. At this stage, there is little research on data mining technology in the field of nursing in China. This paper aims to provide new ideas for the field of nursing research by exploring the actual application in the field of nursing.

Background Operating room (OR) nurses play an important role in preventing the pressure injuries (PIs) that may develop during the perioperative process. This study was conducted to determine OR nurses’ level of knowledge about PIs, and how they manage them. Methods This is a cross-sectional, quantitative and descriptive study. The sample of the study consisted of 234 OR nurses working in eight different public hospitals in Ankara. The questionnaire applied in this study was prepared in accordance with the guidelines. This questionnaire consisted of three sections: demographic profiling, common preventive practices, and the knowledge of OR nurses about intraoperative PI prevention. Findings 66.7% of the participants had received education about PIs during their basic nursing training, and 41.5% had received education after graduation. 97.4% of OR nurses did not follow international guidelines about PIs. The mean total score of the OR nurses for the questions about PIs was 52.0 ± 13.7 out of a possible score of 100. The lowest mean score was obtained for the topic of ‘staging pressure injuries', and the highest score was obtained from ‘interventions to prevent pressure injuries'. In addition, 81.5% of the OR nurses stated that they were not given information about patients with a high PI risk by clinical nurses. 97.9% of the OR nurses did not use a scale to assess intraoperative PI risks. Fewer than half of the nurses said that they assessed the risk of PIs during surgery. 90.8% the nurses did not record risk assessment and interventions to prevent PIs. Conclusion There is a need to incorporate basic operating room PI (ORPI) training into both the basic nursing and in-service training to improve the knowledge of OR nurses about how to prevent and manage ORPIs. In addition, measures to assess PI risk and prevent ORPI should be included in institutional policies and procedures.

Objetivo:evaluar la efectividad de la oxigenación suplementaria con FiO2elevada en comparación con la FiO2 convencional para prevenir la infección del sitio quirúrgico.Método:revisión sistemática de eficacia con metaanálisis en cinco bases de datos y portales internacionales. La investigación se guio por la pregunta: ¿Qué tan eficaz es la oxigenación suplementaria con FiO2alta (más del 80%) en comparación con la FiO2 convencional (del 30 al 35%) para prevenir la infección del sitio quirúrgico en adultos?Resultados:se incluyeron quince ensayos clínicos aleatorizados. Aunque todos los subgrupos mostraron un efecto general a favor de la intervención, en las cirugías colorrectales esa relación tenía significancia estadística (I2=10%;X²=4,42; p=0,352).Conclusión:una fracción inspirada de oxígeno superior al 80% durante el perioperatorio en cirugías colorrectales ha demostrado ser eficaz en la prevención de la infección del sitio quirúrgico, reduciendo su incidencia hasta en un 27% (p=0,006). Se sugiere realizar más estudios en grupos de pacientes sometidos a cirugías en otras especialidades, como cardiaca y vascular. Registro PROSPERO: 178453. Objective:to assess the effectiveness of supplemental oxygenation with high FiO2when compared to conventional FiO2 in the prevention of surgical site infection.Method:an effectiveness systematic review with meta-analysis conducted in five international databases and portals. The research was guided by the following question: Which is the effectiveness of supplemental oxygenation with high FiO2(greater than 80%) when compared to conventional FiO2 (from 30% to 35%) in the prevention of surgical site infections in adults? Results:fifteen randomized clinical trials were included. Although all the subgroups presented a general effect in favor of the intervention, colorectal surgeries had this relationship evidenced with statistical significance (I2=10%;X2=4.42; p=0.352). Conclusion:inspired oxygen fractions greater than 80% during the perioperative period in colorectal surgeries have proved to be effective to prevent surgical site infections, reducing their incidence by up to 27% (p=0.006). The present study was conducted at the Hospital Unit I of the National Câncer Institute of Brazil to investigate the expectations of câncer patients with respect to the preoperative orientation. A quantitative and qualitative approach was used, providing us the means to improve our knowledge about the impact of the surgery in the life of câncer patient. Among the main implications of the results for nursing care, there are the search for truth and the need of higher integration between the patient and the surgical team professionals. According to the patients, the preoperative nursing care should be emphasized, as a better knowledge about the surgery and the human and physical environment of the surgical center may be offered during the realization of the preoperative nursing visit. O presente estudo foi realizado na Unidade Hospitalar I do Instituto Nacional de Câncer do Brasil, com o objetivo de analisar quais as expectativas dos pacientes com câncer em relação às orientações pré-operatórias. Para compreender melhor o objeto de estudo, foi utilizada uma abordagem quanti-qualitativa, que proporcionou meios de aumentar nosso conhecimento sobre o fenômeno da cirurgia na vida do paciente com câncer. Entre as principais implicações para a assistência de enfermagem destaca-se: a busca da verdade e a necessidade de maior integração do paciente com a equipe cirúrgica. Segundo os pacientes entrevistados, a assistência de enfermagem no período pré-operatório merece maior destaque, já que um maior conhecimento sobre a cirurgia e o ambiente físico e humano do centro cirúrgico, pode ser oferecido durante a realização da visita pré-operatória realizada pelo enfermeiro.

Objective Operating room nurses, as essential members of health care teams, often face ethical challenges in the operating room. By using the ethical experiences of operating room nurses, a better understanding of ethics in the operating room can be achieved, which can lead to better nursing decisions in the face of these challenges. Therefore, this study was conducted to investigate operating room nurses’ lived experiences of ethical codes. Methods A hermeneutic phenomenological study was performed in Hamadan (Iran) from February 2019 to November 2020. Ten operating room nurses were selected as participants by purposive sampling. Data were collected through in-depth and semi-structured interviews. Data analysis was performed based on Van Manen methodology. Results Data analysis revealed three main themes and 11 sub-themes representing the operating room nurses experience of the ethical code. The main themes were; adherence to professional commitments, preserving patient dignity, and respect to colleagues. Conclusion The results underlined ethics and ethical values in the operating room. Due to the intense interactions between operating room nurses with the patient and surgical team, commitment to ethics by nurses can lead to improving quality of care and interactions among members of the surgical team. It is suggested that using these codes as a guideline and a framework could be developed to improve the ethical and professional performance of operating room nurses.

This scoping review aims to map the activities/roles performed by nurses in the Surgical Center.

En 2009, la OMS desarrollo la Lista de Verificación Quirúrgica (LVQ) como una herramienta de seguridad quirúrgica. Actualmente, se encuentran resultados controvertidos en la efectividad de la LVQ. Se desarrolla una evaluación realista a través del método mixto desarrollando: una scoping review, un análisis cualitativo para conocer el contexto, un cuestionario para conocer el mecanismo y un análisis de indicadores de cirugía segura. Se observó una mejora en las percepciones de seguridad y cultura se seguridad del paciente y las estrategias de implantación. En servicios como el de cirugía general un aumento del grado de cumplimentación 0,19%-79,90% provoca un descenso de las reintervenciones del 0,62-0,55% y, además, un descenso de la mortalidad posquirúrgica generalizada del 0,31%. Este estudio pone de manifiesto la necesidad de establecer planes de implantación efectivos en contextos que hayan sido previamente estudiados de forma mixta, para garantizar el éxito de la seguridad del paciente. The surgical checklist (SSC) was born in 2009 in order to avoid preventable adverse events, reduce mortality, morbidity, however, its results are controversial. The objective of this thesis was to realistically evaluate the current situation of SSC in a tertiary hospital through its professionals. A realistic evaluation was carried out combining different mixed methodological designs. The qualitative and quantitative results allow a deepening in the knowledge of the context, while the professionals perceive an improvement in the patient safety culture, increased the use of the SSC and, in turn, improvements in postoperative mortality were observed with a reduction from 1.5% to 1.19% and in the ratio of surgical reinterventions from 0.62% to 0.55%. This study is an excerpt from a study on the structuring of the preoperative nursing visit process for adult patients who will undergo cardiac surgery. Its objective is: to apply the process mapping tool in the restructuring of the instrument of the preoperative nursing visit. Methodology: Action research with application of the process mapping tool. Carried out in 2 stages: In the first, the analysis of data from the instrument of preoperative adult visits was carried out and had the year 2018 as a cut-off (n=257). In the next step, the assistant nurses from the surgical center participated in a meeting where they suggested improvements to the process and the instrument through the brainstorming technique. Results: The analysis of the preoperative visits of the adult patient showed a scenario of variability in this process and the meeting with the nurses culminated in the reformulation of the instrument with the participation of the team. Conclusion: The scientific intervention in the reformulation of the instrument aligned with the context of global patient safety recommendations in the preoperative period and with the participation of the subjects who perform the task was ratified. Este estudio es un extracto de un estudio sobre la estructuración del proceso de visita de enfermería preoperatoria para pacientes adultos que serán sometidos a cirugía cardíaca. Tiene como objetivo: aplicar la herramienta de mapeo de procesos en la reestructuración del instrumento de la visita preoperatoria de enfermería. Metodología: Investigación acción con aplicación de la herramienta de mapeo de procesos. Realizada en 2 etapas: En la primera se realizó el análisis de datos del instrumento de visitas preoperatorias del adulto y tuvo como corte el año 2018 (n=257). En el siguiente paso, los auxiliares de enfermería del centro quirúrgico participaron de una reunión donde sugirieron mejoras al proceso y al instrumento a través de la técnica de lluvia de ideas. Resultados: El análisis de las visitas preoperatorias del paciente adulto mostró un escenario de variabilidad en este proceso y la reunión con los enfermeros culminó con la reformulación del instrumento con la participación del equipo. Conclusión: Se ratificó la intervención científica en la reformulación del instrumento alineado al contexto de las recomendaciones globales de seguridad del paciente en el preoperatorio y con la participación de los sujetos que realizan la tarea. Este estudo é um recorte de um estudo da estruturação do processo de visita pré-operatória de enfermagem ao paciente adulto que será submetido a cirurgia cardíaca. Tem como objetivo: aplicar a ferramenta de mapeamento de processo na reestruturação do instrumento da visita pré-operatório de enfermagem. Metodologia: Pesquisa-ação com aplicação da ferramenta de mapeamento de processo. Realizado em 2 etapas: Na primeira foi realizada a análise dos dados do instrumento de visita pré-operatória de adulto realizadas e teve como recorte o ano de 2018 (n=257). Na etapa seguinte, os enfermeiros assistenciais do centro cirúrgico participaram de uma reunião onde sugeriram as melhorias do processo e do instrumento através da técnica de brainstorming. Resultados: A análise das visitas pré-operatórias do paciente adulto, demostrou um cenário de variabilidade neste processo e a reunião com os enfermeiros culminou na reformulação do instrumento com a participação da equipe. Conclusão: Ratificada a intervenção cientifica na reformulação do instrumento alinhado contexto de recomendações globais de segurança do paciente no pré-operatório e com a participação dos sujeitos que realizam a tarefa.

Background The ethical climate is an essential component of organizational climate or culture. The ethical climate is necessary for clinical practice in operating rooms. Objective This study aimed to compare the ethical climate from the perspective of operating room nursing students and operating room staff of hospitals affiliated with Mashhad University of Medical Sciences, Iran. Method This analytical study was performed on 95 operating room nursing students and 169 operating room staff of hospitals in Mashhad, Iran. The students were selected through the census method and the staff were selected by using stratified random sampling. The data were collected via Olson’s Hospital Ethical Climate Survey and analyzed by using BMI SPSS version 21. Results The total mean scores of the ethical climate were 3.44 ± 0.45 and 3.32 ± 0.48 from students and staff’s perspectives, and the independent t-test showed no significant difference between them ( p 0.05). Furthermore, the results of the study indicated a significant difference in communication with physicians’ score between operating room staff (2.82 ± 0.49) and students’ views (3.25 ± 0.6) ( p Conclusion Although it is difficult to create a right ethical climate in operating rooms, but hospital managers should implement practical programs to improve the ethical climate of operating rooms and inter-professional teamwork. Besides, deans of faculties can provide ethical-based education for students, who can effectively create an ethical climate.

Purpose The purpose of this study was to examine the experiences of nursing students concerning operating room (OR) practice. Design A qualitative design was used in this study. Methods Data were collected from second-year nursing students using the semistructured interview form including open-ended questions about their OR experiences. Perspectives of phenomenological approach including existentials of lived body, lived relations, lived space, and lived time were used in data analysis. Findings The study data regarding experiences of nursing students concerning OR practice were organized under three themes, namely, “OR environment,” “emotions,” and “career plans after graduation.” Nine subthemes emerged: educational experience, teamwork, and communication, excitement, enjoyment, anxiety, fear, different emotions, working as a surgical nurse, and working in departments other than OR. Conclusions In clinical environments, student nurses should be supported by staff and instructors to facilitate learning and create meaningful learning experiences.

Objective. To explore the effects of seamless operating room nursing combined with multistyle health education on the psychological state, rehabilitation quality, and nursing satisfaction in patients with internal fixation of femoral fracture. Methods. Eighty patients who received internal fixation of femoral fracture in our hospital (November 2020–November 2021) were chosen as the research objects, and their clinical data were retrospectively analyzed. They were divided into experimental group and control group according to the sequence of hospital admission. In perioperative period, the control group received routine nursing measures and the experimental group received seamless operating room nursing combined with multistyle health education. The patients’ psychological state, rehabilitation quality, and nursing satisfaction after intervention were compared between the two groups. Results. Introduction Counting of accountable items used during surgery, frequently called 'the count', is a fundamental practice to ensure that items such as surgical instruments, sponges and sharps are not forgotten within patients. Although inadvertently leaving behind a sponge or instrument at the end of an operation is a rare event, it is an error that may have serious implications. Objectives The aim of this evidence implementation project was to contribute to promoting evidence-based practice in surgical counts in open abdominal and pelvic surgeries and thereby improving the outcomes of the surgical patients at a surgical centre of a university hospital. Methods The current evidence implementation project used the JBI Practical Application of Clinical Evidence System and Getting Research into Practice (GRiP) audit and feedback tool. The JBI Practical Application of Clinical Evidence System and GRiP framework for promoting evidence-based healthcare involves three phases of activity: first, establishing a project team and undertaking a baseline audit based on evidence-informed criteria; second, reflecting on the results of the baseline audit and designing and implementing strategies to address non-compliance found in the baseline audit informed by the JBI GRiP framework; third, conducting a follow-up audit to assess the outcomes of the interventions implemented to improve practice, and identify future practice issues to be addressed in subsequent audits. These three phases were performed over a period of 6 months, from August 2017 to March 2018. Results The baseline audit revealed deficits between old and best practice in all criteria. Barriers for implementation of a protocol for surgical counts were identified, and strategies were implemented. The postimplementation (follow-up) audit showed improvement in compliance with best practice in six of the audit criteria selected except two, a multidisciplinary team approach to decrease retained surgical items, and limited staff hand-offs during surgical procedures to ensure the same team is present in all counting episodes. Conclusion The main achievements of the study included substantial increases in compliance with best practice. However, registration and report and commitment of all members of surgical team to apply the entire protocol, avoiding to skip any step, persist as challenges.

Yapılan çalışmada, hemşire adaylarının yaşlı bakım hemşireliği ve diğer hemşirelik alanlarına yönelik tutumları araştırılmıştır. Araştırmanın amacına uygun olarak, ilgili literatür taranarak hemşire adaylarının ilgili alanlara yönelik tutumlarını olumlu ya da olumsuz etkileyeceği düşünülen bir takım iş değerleri (kriterler) belirlenmiş ve bu kriterler analize uygun hiyerarşik yapıya dönüştürülmüştür. Araştırmanın evrenini İstanbul’daki kamu üniversitelerinde öğrenim gören hemşirelik öğrencileri, örneklemini ise bir kamu üniversitesinin hemşirelik bölümünde öğrenim gören üçüncü ve dördüncü sınıf hemşirelik öğrencileri oluşturmuştur. Çalışmanın başında, hemşirelik fakültesi öğrencilerinin özellikle yaşlı bakım hemşireliğine yönelik tutumlarının olumsuz olduğu hipotezi ile yola çıkılmıştır. Analiz sonucunda, kariyer tercihi olarak yaşlı bakım hemşireliğinin dezavantajlı olduğu iş değerleri önem derecesine göre sıralanmıştır. Anketler matematiksel bir metot olan Analitik Hiyerarşi Prosesi (AHP) ile değerlendirilmiştir. Çalışma kapsamında AHP yöntemi kullanılarak hemşire adaylarının yaşlı bakım hemşireliği kariyeri ile ilgili kalitatif yargıları kantitatif şekle dönüştürülmüştür. Araştırma, hemşirelik öğrencileri tarafından en fazla Ameliyathane Hemşireliği alanının tercih edildiğini ve Yaşlı Bakım Hemşireliği alanının orta sıralarda yer aldığını ortaya koymuştur. Yaşlı Bakım Hemşireliği tercih eden ve tercih etmeyen hemşirelik öğrencilerinin çalışma değerleri arasında önemli bir farklılık olmadığı bulunmuştur. Son olarak, hemşirelik öğrencileri için çalıştığı kişilerle iyi ilişkiler kurmak ve iş garantisine sahip olmak en önemli çalışma değerleri olarak belirlenmiştir., In the study, the attitudes of the candidate nurses towards elderly care nursing and other nursing fields were investigated. In accordance with the purpose of the research, the relevant literature was scanned and a number of job values (criteria), which are thought to affect positively or negatively the attitudes of the nurse candidates towards the relevant fields, were determined and these criteria were transformed into a hierarchical structure suitable for the analysis. The universe of the research consists of nursing students studying at public universities in Istanbul, and the sample consists of nursing students from third and fourth classes studying at the nursing department of a public university. At the beginning of the study, it was set out with the hypothesis that nursing faculty students’ attitudes towards elderly care nursing were negative. As a result of the analysis, the job values that the elderly care nursing is disadvantageous as a career choice are listed according to their importance level. The surveys were evaluated with a mathematical method Analytical Hierarchy Process (AHP). In terms of the study, the qualitative judgments of the nursing candidates regarding the elderly care nursing career were transformed into quantitative form by using AHP method. The research has revealed that the nursing students mostly preferred the Operating Room Nursing field and the Elderly Care Nursing field is in the middle ranks. It was found that there is no significant difference between the values of nursing students who prefer Elderly Care Nursing and those who do not prefer it. Finally, establishing good relationships with the people with whom they work and having job guarantee are determined as the most important work values for nursing students.

Ethical values and respect for human dignity make operating room nurses responsible for nursing decisions that are clinically and technically sound and morally appropriate.AimTo learn what ethical issues operating room nurses perceive as important regarding non-technical skills.Research designQualitative individual in-depth interviews were conducted. The interviews were analysed using Braun and Clarke’s six phases for thematic analysis.Participants and research contextEleven experienced perioperative/operating room nurses working in an operating unit at a Norwegian university hospital.Ethical considerationsApproval was given by The Norwegian Social Science Data Service in care of the hospital’s Data Protection Officer.FindingsThree main themes were found: respect and care for the patient, making the patient feel safe, and respect within the perioperative team. These features or themes, which incorporate collaboration and communication, are closely connected to patient safety.DiscussionDefending the patient’s dignity is part of caring for and respecting the patient. The manner in which the operating room team collaborates is important for the patient to feel safe and secure. Poor teamwork may have dire consequences. Reciprocal respect within the team includes respect for each other’s tasks and responsibilities and to talk to one another in a friendly manner.ConclusionBeing respectful and contributing to a caring atmosphere are central ethical skills in the operating room. To patients, harmonious teamwork translates into a feeling of safety and being cared for. The nurses see respect and patient safety, and respect and reciprocal politeness among the members of the perioperative team as central ethical non-technical skills. Lack of respect influences the team negatively and is detrimental for patient safety. Good communication is an important safety measure during surgery and creates a feeling of good ‘flow’ within the operating room team.

Background: The aim of this study was to investigate the aseptic technique used in the preparation of skin by operating room nurses.Materials and Methods: The data of this descriptive study was collected from the internet between March-June 2019 with the data collection form created by the researchers. The sample of the study consisted of 154 operating room nurses who voluntarily agreed to participate in the study and able to use internet. Mean, standard deviation, number and percentage were used for data analysis.Results: In this study, only 30.5% (n:47) of the nurses stated that patients were routinely showered/bathed before surgery in the institution they work. Of the nurses, 63.6% (n:98) reported that hairs at the operation site were routinely removed and 60% (n:111) reported that hair removal was performed with a clipper. 69.8% (n:125) of the nurses stated that povidone iodine was used in skin antisepsis. 69.5% (n:107) of the nurses stated that they placed compresses on both sides of the patient to prevent accumulation of the antiseptic solution used in skin antisepsis.Conclusions: In conclusion, it was determined in this study that skin antisepsis was performed with appropriate antiseptic solution in a sufficient area, in a sufficient time and with appropriate technique. However, it is recommended to update the knowledge of the operating room nurses to prevent surgical site infections due to the low rate of routine application of the bath / shower, making the routine mostly regardless of whether hair removal is required or not, and using the razor blade.Key words, Amaç: Bu çalışmanın amacı, ameliyathane hemşirelerinin deri hazırlığında kullandıkları aseptik teknik uygulamalarının incelenmesidir.Materyal ve Metod: Tanımlayıcı tipte olan bu çalışmanın verileri Mart-Haziran 2019 tarihleri arasında araştırmacılar tarafından oluşturulan veri toplama formu ile internet üzerinden toplanmıştır. Çalışmanın örneklemini, çalışmaya katılmayı gönüllü olarak kabul eden, internet kullanabilen 154 ameliyathane hemşiresi oluşturmuştur. Verilerin analizinde ortalama, standart sapma, sayı ve yüzde kullanılmıştır.Bulgular: Bu çalışmada hemşirelerin sadece %30,5 (n:47)’i çalıştıkları kurumda hastalara ameliyat öncesi rutin olarak duş/banyo yaptırıldığını belirtmiştir. Hemşirelerin %63,6 (n:98)’sı ameliyat bölgesindeki tüylerin rutin olarak temizlendiğini ve %60 (n:111)’ı tüy temizliğinin elektrikli tıraş makinesi ile yapıldığını belirtmişlerdir. Hemşirelerin %69,8 (n:125)’i deri antisepsisinde povidon iyodin kullanıldığını ifade etmişlerdir. Hemşirelerin %69,5 (n:107)’i deri antisepsisinde kullanılan antiseptik solüsyonun hastanın altında birikmesini önlemek için hastanın her iki tarafına kompres yerleştirdiklerini belirtmişlerdir.Sonuç: Sonuç olarak, bu çalışmada deri antisepsisinin uygun antiseptik solüsyon ile yeterli alanda, yeterli sürede ve uygun teknik ile yapıldığı belirlenmiştir. Ancak banyo/duşun rutin uygulanma oranının düşük olması, tüy temizliğinin gerekli olup olmadığı düşünülmeksizin çoğunlukla rutin yapılması ve tıraş bıçağının kullanılması nedeniyle ameliyathane hemşirelerinin cerrahi alan enfeksiyonlarını önlemeye yönelik bilgilerinin güncellenmesi önerilmektedir.

Background: A culture of patient safety is one of the cornerstones of good-quality healthcare, and its provision is one of the significant challenges in healthcare environments. Aim: The purpose of this study was to evaluate the effect of a surgical safety educational programme on the attitudes of nurses to patient safety in operating rooms (OR). Design: An interventional one-group pre-/post-test design, which sought to measure changes in OR nurses' attitudes toward patient safety culture. Methods: A simple random sampling technique was used to recruit 66 OR nurses working at six Royal Medical Service hospitals in Amman, Jordan. All participants took part in a 4-hour educational workshop. Pre-tests and post-tests were done. Results: The results of this study showed that OR nurses' attitudes towards a culture of patient safety was originally negative; significant improvement after attending the programme was found (3.3 ± 0.20 versus 3.8 ± 0.30). There was a negative correlation between years of experience and nurses' attitudes towards patient safety. Conclusions: Incorporating courses about safety culture into continuing education programmes may improve nurses' attitudes towards patient safety. Nurses should be qualified to play an important role in creating a culture of patient safety.

Objectives: To assess the actions performed by the operating room nurse during anesthesia and their behavior for patient safety regarding the reporting on adverse events, and to analyze their knowledge about anesthetic practices. Method: This is a cross-sectional study carried out using an electronic questionnaire consisting of socio-demographic, professional practice, knowledge in anesthesia, patient safety, and professional practice questions, conducted from January to March 2019 with operating room nurses. Results: One hundred nurses participated, 89 (89%) being women, with a mean age of 41.09 years (SD = 9.36), time of undergraduate completion of 14.33 years (SD = 8.34). The average attendance was 4.69 operating rooms (SD = 2.07) per nurse, with an emphasis on action before induction (49; 49%). Professionals reported performance of simultaneous activities (72; 72%) and insufficient number of employees (57; 57%) as difficulties of their daily practice. Among the participants, 77 (77%) correctly cited the periods of general anesthesia and 80.4% always reported the occurrence of an adverse event. Conclusion: Nurses identified their role in anesthesia, with limitations for assistance from multiple activities and lack of professionals. RESUMEN Objetivos: Evaluar las acciones realizadas por los enfermeros en el quirófano durante la anestesia y sus conductas para la seguridad del paciente en la notificación de eventos adversos, analizando sus conocimientos sobre prácticas anestésicas. Método: Estudio transversal realizado con un cuestionario electrónico compuesto por preguntas sociodemográficas, práctica profesional, conocimientos en anestesia, seguridad del paciente y cuestiones de ejercicio profesional, realizado de enero a marzo de 2019 con enfermeros de quirófano. Resultados: Participaron 100 enfermeros, 89 (89%) mujeres, con una edad media de 41,09 años (DE = 9,36), tiempo de formación de 14,33 años (DE = 8,34). La asistencia media fue de 4.69 quirófanos (DE = 2.07) por enfermero, con énfasis en la actuación antes de la inducción (49; 49%). Los profesionales informaron como limitación de su práctica diaria la ejecución de actividades simultáneas (72; 72%) y la insuficiencia de empleados (57; 57%). Método: Estudo transversal realizado com questionário eletrônico composto por perguntas sócio-demográficas, prática profissional, conhecimento em anestesia, segurança do paciente e questões de exercício profissional, conduzido de janeiro a março de 2019 com enfermeiros de centro cirúrgico. Resultados: Participaram 100 enfermeiros, sendo 89 (89%) do sexo feminino, com média de idade de 41,09 anos (DP = 9,36), tempo de formação de 14,33 anos (DP = 8,34). A média de atendimento foi de 4,69 salas operatórias (DP = 2,07) por enfermeiro, com destaque para a atuação antes da indução (49; 49%). Os profissionais relataram como limitação de sua prática diária a execução de atividades simultâneas (72; 72%) e funcionários insuficientes (57; 57%). ObjectiveTo observe the influence of nursing intervention in operation rooms on incision infection of patients undergoing gastrointestinal surgery and the improvement of gastrointestinal function.MethodsA total of 340 patients who underwent gastrointestinal surgery in our hospital from June 2020 to August 2021 were included. According to the random number table, they were divided into the conventional nursing group (n = 170) and the operating room nursing group (n = 170). The conventional nursing group was treated with routine nursing intervention, while the operating room nursing group was treated with operating room nursing intervention. The incision infection, healing, gastrointestinal function recovery, and complications in the two groups were compared, and the patient care satisfaction was recorded.ResultsThe incidence of incision swelling, pain, and incision secretion in the operating room nursing group was significantly lower than that in the conventional nursing group (p < 0.05). The patients in the operating room nursing group had higher grade A healing than in the conventional nursing group, and lower grade B and grade C healing than in the conventional nursing group (p < 0.05). The time of anal exhaust, first defecation, and the time of gastric tube removal in the operating room nursing group were lower than those in the conventional nursing group (p < 0.05). The incidence of postoperative complications, such as incision infection, incision dehiscence, early inflammatory bowel adhesion, and abdominal abscess, in the operating room nursing group was lower than that in the conventional nursing group (p < 0.05). The total satisfaction degree in the operating room nursing group was significantly higher than that in the conventional nursing group (p < 0.05).ConclusionNursing intervention in operation room can reduce complications and improve gastrointestinal function when applied to patients undergoing gastrointestinal surgery due to incision infection.

Cross-sectional study in wich were studied the perioperative nursing care practies spectific aplly to patients with cleft lips and palate, realized on the reference hospital to diagnosis and treatment of this grievance of health on Federal District – Brazil. It was collected data referring to patients that were submitted to surgical procedures to correct this clefts on the period of june to octuber of 2017 and it was realized an artificial pairing to compair with patients operatted on the same period of the year before. The data were divided on the three phases that constitue the perioperative to identify the ideals strategies to realize the care systematization of this patients group. Therefore, it’s possibe to note that if apply the care based on the cientific evidences it was obtained as result a lower rate of complications, an optimization of the treatment and recovery of the patients and the improvement of the utilization from the material and human resources. Estudo analítico observacional transversal, no qual foram estudadas as práticas de assistência de enfermagem perioperatória especificamente aplicadas aos pacientes com fissuras labiopalatinas, realizadas no hospital de referência para diagnóstico e tratamento deste agravo de saúde do Distrito Federal – Brasil. Foram coletados dados referentes a pacientes que se submeteram a procedimentos cirúrgicos para a correção destas fissuras no período de junho a outubro de 2017 e realizado um emparelhamento artificial para comparação com pacientes operados no mesmo período do ano do ano anterior. Os dados foram divididos nas fases que constituem o perioperatório para que fossem identificadas as estratégias ideais para realizar a sistematização do cuidado desse grupo de pacientes. Assim, pode-se perceber que ao aplicar o cuidado baseado nas evidências científicas obteve-se como resultado uma taxa de complicações menor, otimização do tratamento e recuperação dos pacientes e a melhoria da utilização de recursos materiais e humanos.

Objective: To know the conceptions regarding patient safety during their surgical positioning from the perspective of nurses in a Surgical Center. Method: Qualitative descriptive exploratory study, with seven nurses from private hospital in southern Brazil in August 2018. Data generation using the Focus Group technique, with content analysis by Bardin. Results: From the data analysis, six categories emerged: “patient safety”, “surgical patient risks”, “nurses in the operating room”, “training”, “involvement of the interprofessional” team and application of the risk assessment scale. The development of injuries resulting from positioning was identified as the greatest risk for the surgical patient, highlighting the presence of the nurse in the operating room as essential for the prevention of this risk, through patient assessment and staff training. Final considerations: Nurses identified risk to patient safety in relation to surgical positioning and considered they presence in the operating room to be indispensable. RESUMEN Objetivo: Conocer los conceptos sobre seguridad en el posicionamiento del paciente quirúrgico desde la perspectiva del enfermero en un Centro Quirúrgico. Método: Estudio exploratorio descriptivo cualitativo, con siete enfermeras de un hospital privado del sur de Brasil, em agosto de 2018. Generación de datos mediante la técnica de Focus Group, con análisis de contenido de Bardin. Resultados: Del análisis de los datos surgieron seis categorías: “seguridad del paciente”, “riesgos del paciente quirúrgico”, “enfermeras en quirófano”, “formación”, “implicación del equipo interprofesional” e “implementación de la escala de valoración de riesgos”. El desarrollo de las lesiones derivadas del posicionamiento fue identificado como el mayor riesgo para los pacientes quirúrgicos, destacándose la presencia de enfermeras en el quirófano como fundamental para la prevención de este riesgo, a través de la valoración del paciente y la formación del equipo. Consideraciones finales: Los enfermeros identificaron riesgos para la seguridade del paciente en relación con el posicionamiento quirúrgico, considerando indispensable su presencia en quirófano. RESUMO Objetivo: Conhecer as concepções em relação à segurança do paciente durante seu posicionamento cirúrgico sob a ótica das enfermeiras de um Centro Cirúrgico. Método: Estudo exploratório descritivo qualitativo, com sete enfermeiras de hospital privado da região sul do Brasil, no mês de agosto de 2018. Geração de dados pela técnica de Grupo Focal, com análise de conteúdo de Bardin. Resultados: Da análise dos dados emergiram seis categorias: “segurança do paciente”, “riscos do paciente cirúrgico”, “enfermeiro em sala cirúrgica”, “capacitação”, “envolvimento da equipe interprofissional” e “implementação da escala de avaliação de risco”. Identificou-se como maior risco o desenvolvimento de lesões decorrentes do posicionamento, sendo a presença do enfermeiro em sala cirúrgica destacada como fundamental para prevenção deste risco, através da avaliação do paciente e capacitação da equipe. Considerações finais: As enfermeiras identificaram riscos à segurança do paciente em relação ao posicionamento cirúrgico, considerando indispensável a presença dos mesmos em sala cirúrgica.

The purpose of this study is to apply refined nursing in the operation room of thoracoscopic lung cancer radical surgery in southwestern China ** hospital through refined nursing. Patients in the control group were given routine nursing care, and patients in the experimental group were given refined nursing care. The intraoperative blood loss, extubation time, length of stay, and total nursing satisfaction rate were compared between the two groups of patients The intraoperative blood loss of the patients in the experimental group was 192.94 ± 3.63 mL, and the hospital stay was 10.08 ± 1.64 days, which were lower than those in the control group (221.46 ± 4.31 mL and 16.62 ± 1.83 days, respectively), and the difference was statistically significant (P < 0.05). There was no significant difference in extubation time between the experimental group and the control group (P > 0.05). The complications in the experimental group were lower than those in the control group, and the overall nursing satisfaction rate was higher in the experimental group than in the control group. The differences between the two groups were statistically significant (P < 0.05). Refined nursing applied to the operation room of thoracoscopic lung cancer radical surgery surgery can reduce the intraoperative bleeding volume and complications of patients, shorten the length of hospitalization of patients, improve the patient's nursing satisfaction, and is worthy of clinical application.

Spanç ve gaz kompresler gibi yumuşak ürünler, iğneler ve kesiciler en çok unutulan cisimler olup, abdomen/pelvis, vajina ve eklemler yabancı cisimlerin en çok unutulduğu anatomik bölgelerdir. Forgetting a foreign body in the surgical field ranks first among the medical errors that increase patient safety risk. The issue, which has been up to date for many years, is still being addressed by many communities and strategy studies are carried out to prevent it. Soft products such as sponges and gas compress, needles, and cutters are the most forgotten items, and the abdomen/pelvis, vagina, and joints are the anatomical regions where foreign bodies are mostly forgotten. Forgetting a foreign body, which is among the preventable medical errors, threatens patient safety due to human, environmental and surgical factors; adversely affects the patient, healthcare profession, and institution. Factors such as the use of an evidence-based communication model among the surgical team, consistent counting methods, record keeping, and technology can prevent counting errors. Forgetting a foreign body is the responsibility of the entire surgical team. However, as team members who manage the operating room and perform the count, operating room nurses play a crucial role in preventing foreign body forgetting. Objective. To explore the effect of combining operating room nursing based on clinical quantitative assessment with WeChat health education on postoperative complications and quality of life (QOL) of femoral fracture patients undergoing internal fixation. Methods. Ninety femoral fracture patients treated in our hospital (July 2018 to July 2021) were chosen as the research objects and split into the control group (routine intervention) and the study group (combination of operating room nursing based on clinical quantitative assessment and WeChat health education) according to the nursing intervention modes, with 45 cases each. After nursing, the postoperative complications and QOL of patients were compared between the two groups. Results. Objective. This study aimed to provide scientific management methods to prevent nosocomial infection based on the systematical evaluation of the effect of operating room nursing management on nosocomial infection in orthopedic surgery. Methods. PubMed, Web of Science, Embase, China National Knowledge Internet, and Wanfang Databases were systematically searched for relevant studies published from 2013 to 2020. In this meta-analysis, comprehensive estimates of effect size estimates and 95% confidence intervals (CIs) for nursing satisfaction and incidence of infection were obtained. Results. Twenty studies with 2962 orthopedic patients were included in the meta-analysis. The experimental group received operating room nursing management while the routine nursing management was given for the control group. Meta-results showed that, in comparison with the control group, the nursing satisfaction in the experimental group was increased (OR = 6.22, 95% CI: 4.63–8.35, P < 0.001 ), while the incidence of infection was reduced (OR = 0.20, 95% CI: 0.15–0.28, P < 0.001 ), and the differences had statistical significance. Conclusions. Operating room nursing management could reduce the incidence of infection while prevent nosocomial infection in orthopedic surgery, which could be utilized to guide the hospital management.

Healthcare systems internationally are working under increasing demand to use finite resources with greater efficiency. The drive for efficiency utilises process improvement methodologies such as Lean Six Sigma. This study outlines a pilot Lean Six Sigma intervention designed to release nursing time to care within a peri-operative environment, this was achieved by collaborating with stakeholders to redesign the process for laparoscopic hernia surgical case preparation (set up) material. Across 128 laparoscopic hernia surgical cases, the pilot resulted in a 55% decrease in overall nursing time spent in gathering and preparing materials for laparoscopic hernia surgical cases, with a corresponding reduction in packaging waste. The major impact of releasing nursing time to care within busy Operating Room environments enabled nurses to focus on continuing to deliver high-quality care to their patients and reduce pressure expressed by the Operating Room nurses. The results have led to an ongoing review of other surgical procedures preparation to further release nursing time and will be of interest to perioperative teams internationally.

Background During the recovery period of general anesthesia, patients are disturbed by residual anesthetic drugs, manifesting as clinical signs of confusion, drowsiness and disorientation, and even abnormal psychology and limb agitation at varying degrees in severe cases. These stress reactions are detrimental to the postoperative recovery, which can be life-threatening. Operating room nursing intervention (ORNI) is a novel nursing model that prevents stress reactions during the recovery period of general anesthesia. However, whether ORNI can improve the psychological condition and reduce the incidence of emergence agitation in the recovery period of general anesthesia remains controversial. Therefore, this study aims to evaluate the effect of ORNI on the psychological status and incidence of emergence agitation in the recovery period of general anesthesia through a systematic review and meta-analysis, thus providing clinical evidence to support it. Methods Randomized controlled trials reporting the effect of ORNI on the recovery period of general anesthesia published before October 2021 will be searched in the Chinese Scientific Journal Database, China National Knowledge Infrastructure Database, Wanfang, China Biomedical Literature Database, PubMed, Embase, the Cochrane Library, and Web of Science. Eligible literatures will be screened out according to inclusion and exclusion criteria, and their quality will be assessed using the Cochrane Risk of Bias Assessment Tool. Meta-analysis will be performed using Revman 5.4 software. Results This study will evaluate the effect of the ORNI on the recovery period of general anesthesia by calculating the incidence of emergence agitation, and grading the self-rating anxiety scale and self-rating depression scale scores. Objectives: to report the construction of a process model to support the decision making of operating room nurses to control the risk for perioperative positioning injury. Methods: experience report on a process model that helps nurses with decision making regarding clients at risk for perioperative positioning injury. By following the steps, it was possible to identify intrinsic and extrinsic variables of the literature and of the workflows of teams involved in the positioning of the client for surgery. The Business Process Model and Notation, the Bizagi Modeler software and terms from the International Classification for Nursing Practice were used in the model. Results: the experience allowed the observation of the knowledge integration between different areas, which enabled the process modeling and its validation. Conclusions: process modeling is an innovative option for the development of support systems for clinical nursing decisions.

Background: The advantages of evidence-based nursing (EBN) intervention in health care settings have been widely disseminated to nurses throughout the world. More researches are reporting the effectiveness of EBN intervention in operating room nursing. However, the results are inconsistent. This study focuses on conducting a meta-analysis and systematic evaluation aimed at determining the usefulness of EBN intervention in operating room nursing. Methods: The Preferred Reporting Items for Systematic Reviews and Meta-analysis for Protocols criteria were used to write this paper. We will look for relevant studies from 2 Chinese databases (China National Knowledge Infrastructure and Wanfang database and also from 3 English databases such as Web of Science, Cochrane Library, PubMed, and EMBASE), to locate all relevant randomized controlled trials and observational studies assessing the application value of EBN intervention in operating room nursing from their commencement to June 2021. Separately, 2 authors will choose the studies, do the data extract and conduct the assessment probing into the likelihood of bias. If there is a disagreement, it will be resolved by the third author. RevMan 5.3 software and Stata 15.0 software will be used to conduct the meta-analysis. Results: The usefulness of EBN intervention in operating room nursing will be assessed in this study. Conclusion: The purpose of this research is to conclude the value of EBN intervention in operating room nursing and the quality of current data. Introdução: O interesse em alargar o conhecimento sobre os incidentes adversos que ocorrem no Bloco Operatório constitui-se o foco do presente Trabalho de Projeto, tendo por finalidade contribuir para o aumento da segurança do doente, constituindo-se igualmente como uma oportunidade para disponibilizar ao Serviço essa informação. Objetivo: Verificar quais os incidentes adversos mais prevalecentes no Bloco Operatório. Métodos: Estudo quantitativo correlacional-descritivo e transversal. A amostra será composta por todos os enfermeiros que exercem num Bloco Operatório Central da região centro do país. Trata-se de uma amostra do tipo não probabilístico por conveniência. Como instrumento de recolha de dados, aplicar-se-á um protocolo constituído por um questionário sociodemográfico, de caracterização profissional e pelo questionário “Perceção dos Enfermeiros sobre Notificação de Incidentes e Segurança dos doentes” (Antunes, 2015), que contém três escalas adaptadas. Resultados/conclusão: Espera-se que os resultados apurados sirvam de ponto de partida para minimizar os eventos adversos e incentivar à sua notificação, sem medo de punição por parte dos enfermeiros, mas como um meio de melhorar a segurança do doente e garantir a qualidade dos cuidados prestados. Abstract Introduction: The interest in broadening the knowledge about adverse incidents that occur in the surgical unit is the focus of this Project Work, aiming to contribute to the increase of patient safety, also constituting an opportunity to make available to the Service that information. Objective: Verify which adverse incidents are most prevalent in the surgical unit. Methods: Quantitative correlational-descriptive and cross-sectional study. The sample will consist of all nurses who work in a Central Surgical Unit in the central region of the country. This is a non-probabilistic convenience sample. As a data collection instrument, a protocol will be applied consisting of a sociodemographic, professional characterization questionnaire and the questionnaire “Nurses' Perceptions on Incident Notification and Patient Safety” (Antunes, 2015), which contains three adapted scales. Results/conclusion: The results obtained are expected to serve as a starting point to minimize adverse events and encourage their notification, without fear of punishment by nurses, but as a means of improving patient safety and ensuring quality of the care provided. Background: Expanded applications and increasing volumes of complex endovascular procedures have increased the risk of unintended intraoperative fragmentation and retention of catheters and sheaths. We describe a series of retained or fragmented endovascular devices, a quality improvement program to address this unmet need for improved detection of catheter fragmentation, and the results of this program. Methods: Cases utilizing endovascular devices that resulted in a retained catheter fragment were identified and analyzed during divisional quality improvement review. One consistent area of concern was operating room (OR) staff unfamiliarity with verifying the integrity of an endovascular device. In response, a slide-based training protocol of focused, endovascular nursing education was implemented. Following perceived lack of improvement after this approach, we developed a handheld visual reference display of the tips of common catheters and sheaths. Staff was surveyed before and after intervention to assess the educational value of the display and the impact on staff device familiarity. Results: All 4 described cases resulted in an unplanned return to the OR for retrieval of the fragmented catheter or sheath. No thromboembolic complications were observed, although associated intra-arterial thrombus was noted in 2 cases. Thirty-four OR nurses were polled to trial the visual reference display initiative, with 70% of those reporting primary surgical assignments outside of cardiovascular ORs. Introduction of the new visual reference display improved staff confidence in their ability to identify a broken device (2.4-3.7, P < .001). This improvement was most significant in OR staff with primary assignments in noncardiovascular services. Conclusion: Current OR standard operating procedures fail to address the potential for unintentionally retained catheters and wires during endovascular procedures. Our novel visual reference display of common endovascular items rather than conventional in-service training improved the ability of staff to identify device fragmentation at the time of the index procedure.

The purpose of this article is to describe interprofessional relations in order to better understand their impact on nurse retention, while considering the operating room culture and its specific context. A focused ethnography was performed between September and October 2017 at a university hospital in an urban center in the province of Quebec, Canada. This was a secondary analysis of 11 nurses' semistructured one-on-one interviews. Additional data were collected through 6 days of observations, informal conversations, field notes, and a journal. A thematic analysis followed. Interprofessional relations and the need for recognition are important for nurse retention. In addition, a nurse's personality appears to be an important aspect in the complex and specific context of the operating room. Nurse retention in the operating room is multifactoral, and like the need for recognition, interprofessional relations are important issues. Interventions to improve working relationships, recognition of nurses, and consideration of a nurse's personality during hiring appear to be promising avenues for improving retention in the operating room.

Objective: This study sought to develop a method that supports a more evidence-based approach to evaluating multiple design options in virtual reality (VR), combining subjective insights gathered using traditional approaches and objective feedback gathered using the VR platform. Additionally, this study sought to understand how objective data garnered from the VR platform could be used to compliment traditional evaluation strategies. Background: VR can be a viable research platform for supporting evidence-based design practices. Prior studies have predominately utilized experiential user feedback. While able to provide valuable subjective insights, these approaches are less effective in making objective comparisons between multiple designs alternatives. Method: A repeated measures study was conducted with nursing faculty. User feedback was captured through surveys, interviews, and the VR platform. Results: The survey, interview, and the objective VR data converged in terms of identifying the highest performing design option. Survey data showed that Room 2 performed best in terms of perceived physical access to supplies, unobstructed movement, and availability of space to accommodate additional equipment. VR data showed that participants in Room 2 had significantly higher visibility to both patient and care partners throughout their simulated interaction. Conclusion: Simulation-based evaluations in VR that use a combination of users’ subjective insights and objective data obtained from VR can be an effective tool for helping designers evaluate multiple design options. The use of scenario-based simulations provided a structured and clinically relevant approach to comparing three preoperative rooms, supporting a more robust assessment of users’ physical response to a simulated healthcare environment.

Background It is known that job satisfaction has an important impact on efficacy or burnout syndrome of medical personnel. Many studies have concerned job satisfaction among critical care nurses. Not as many have focused on anesthesia nurses working in operating theaters. In Poland, anesthesia and intensive care is a combined specialty for nurses. However, nurses work in an intensive care unit (ICU) or in an operating room (OR), and very rarely in both settings. We would like to compare satisfaction between ICU and OR nurses. Methods It was a multicenter cross-sectional study. 406 nurses from thirteen hospitals participated in this study. All respondents filled in the questionnaire that contained fifteen Likert-like questions reflecting different aspects of job satisfaction. Demographic data were also collected. Results We did not find a significant difference between ICU and OR nurses in the overall job satisfaction. Furthermore, the type of hospital did not significantly influence satisfaction of our study participants. The most important factor which differentiated the level of satisfaction among nurses was the region of Poland in which they worked. Interestingly, nurses who worked in ICUs were significantly younger in comparison to their colleagues from ORs. Conclusion The results of our study suggest that the region of the country in which nurses work might play a very important role in their satisfaction.

Due to their wide applications, concern exists regarding possible genotoxic effects of inhalational anesthetics (IAs) among operating room personnel. This study was undertaken to examine genotoxic properties of co-exposure to nitrous oxide, sevoflurane, and isoflurane on induction of micronucleus (MN) and chromosomal aberrations (CAs) and to determine whether any associations exist between polymorphisms of GST genes and the level of genomic damage measured by MN and CAs assays. Sixty operating room personnel and 60 unexposed referent nurses were studied. The workers' exposure to the IAs was determined. DNA damage was evaluated by MN and CAs assays. Additionally, the GSTM1, GSTT1, and GSTP1 polymorphisms were detected. The mean concentrations of nitrous oxide, isoflurane, and sevoflurane were found to be 850.92 ± 919.78, 2.40 ± 0.86, and 0.18 ± 0.14 ppm, respectively. The frequency of MN and CAs in the exposed group was significantly higher than that of the non-exposed group. The frequency of MN was significantly higher in referent nurses with null GSTT1, compared to referent nurses with positive GSTT1. The frequency of MN was significantly higher in exposed individuals carrying the combined genotype of GSTT1 (-), GSTM1 (-), and GSTP1 AG as compared with subjects carrying a combination of GSTT1 (+), GSTM1 (+), and GSTP1 AA. Statistically significant associations were noted between exposure to the IAs, gender, and the combination of the three GSTs genotypes with MN frequency. These findings indicate that inhalation exposure to IAs induces genotoxic response and the polymorphisms of GSTs genes might modulate the effect of exposure to IAs on MN.

Objective: To analyze the indicators of surgical patients after the implementation of an Internal Bed Regulation Committee in a university hospital. Method: Longitudinal, quantitative, and retrospective study. The data collection was conducted in the Hospital Management Information institutional system, from which the information of patients submitted to surgical procedures from January 2015 to June 2018 were obtained. To verify the data trends, a simple linear regression model was used. Results: The predominance of patients aged 20 to 39 and hospitalized on an emergency basis was observed. An ascending trend for structure indicators was verified regarding the number of surgical procedures and patients per surgical room. The process indicators were stagnant. An ascending trend was presented by the result indicators related to the number of surgical patients, hospitalized surgical patients, surgical procedures, and patients with Hospitalization Authorization. Conclusion: A change in the mean values of the process indicators was observed, showing the performance of this service. Organizational changes were also observed regarding the establishment of norms, processes, and flows. RESUMEN Objetivo: Analizar los indicadores de los pacientes quirúrgicos tras la implantación del Comité Interno de Regulación de Camas en un hospital universitario. Método: Estudio longitudinal, cuantitativo y retrospectivo. La recolección de datos se realizó en el Sistema de Información de la Gestión Hospitalaria institucional, del cual se obtuvo la información de los pacientes sometidos a procedimientos quirúrgicos entre enero de 2015 y junio de 2018. Para verificar la tendencia de los datos, se utilizó un modelo de regresión lineal simple. Resultados: Se observó un predominio de pacientes de entre 20 y 39 años y la hospitalización de urgencia. Hubo una tendencia al alza en los indicadores de estructura relativos al número de procedimientos quirúrgicos y de pacientes por sala quirúrgica. Los indicadores de proceso fueron estables. Los indicadores de resultados relacionados con el número de pacientes quirúrgicos, los pacientes quirúrgicos hospitalizados, los procedimientos quirúrgicos y los pacientes con Autorización de Ingreso Hospitalario mostraron una tendencia al alza. Conclusión: Se observaron cambios en los valores medios de los indicadores de proceso, lo que demuestra la actuación de este servicio. También se verificaron cambios organizativos en cuanto al establecimiento de normas, procesos y flujos. RESUMO Objetivo: Analisar os indicadores de paciente cirúrgico após a implantação de Núcleo Interno de Regulação de Leitos em hospital universitário. Método: Estudo longitudinal, quantitativo e retrospectivo. A coleta de dados foi realizada no sistema institucional Informação de Gestão Hospitalar, do qual foram obtidas as informações dos pacientes submetidos a procedimentos cirúrgicos entre janeiro de 2015 e junho de 2018. Para a verificação de tendência dos dados, foi utilizado modelo de regressão linear simples. Resultados: Observou-se predominância de pacientes com idade entre 20 e 39 anos e internação em caráter de urgência. Verificou-se a tendência crescente dos indicadores de estrutura quanto ao número de procedimentos cirúrgicos e de pacientes por sala operatória. Os indicadores de processo apresentaram-se estagnados. Apresentaram tendência crescente os indicadores de resultado relacionados aos números de pacientes cirúrgicos, pacientes cirúrgicos internados, procedimentos cirúrgicos e pacientes com Autorização de Internação Hospitalar. Conclusão: Observou-se alteração nos números médios dos indicadores de processo, demonstrando a atuação deste serviço. Também foram verificadas mudanças organizacionais quanto ao estabelecimento de normas, processos e fluxos.
