43 p. ill., Pour obtenir l’autorisation de mise sur le marché européen, tout nouveau médicament doit faire la preuve de sa qualité, de sa sécurité et de son efficacité. Pour les dispositifs médicaux à haut risque (comme p. ex. les implants), les exigences sont moindres. Mais aux yeux de l’assurance maladie, des médecins et des patients, ce qui compte surtout, c’est la valeur du nouveau produit par comparaison aux traitements déjà existants. Le problème est que les études comparatives – et qui mesurent des paramètres significatifs pour les patients – ne sont pas obligatoires pour obtenir une autorisation de mise sur le marché. La principale recommandation du Centre fédéral d’Expertise des Soins de santé (KCE) est que les firmes pharmaceutiques ou de dispositifs médicaux ne conçoivent pas seulement leurs études cliniques pour rencontrer les exigences actuelles de l’accès au marché européen, mais qu’elles collectent aussi, dès le départ, des données comparatives leur permettant de répondre aux demandes des organismes d’assurance maladie nationaux, des médecins et des patients. 43 p. ill., Om een Europese marktvergunning te krijgen, moet voor elk nieuw geneesmiddel de kwaliteit, veiligheid en werkzaamheid worden aangetoond. Voor “hoog-risico” medische hulpmiddelen (bijvoorbeeld implantaten) zijn de eisen minder streng. Voor de ziekteverzekering, de artsen en de patiënten telt vooral de waarde van het nieuwe product in vergelijking met de bestaande behandeling. Dergelijke vergelijkende studies met patiëntrelevante eindpunten zijn echter niet noodzakelijk voor een markttoelating. De belangrijkste aanbeveling van het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) is dat bedrijven die geneesmiddelen of medische hulpmiddelen op de markt brengen, hun klinische studies niet alleen afstemmen op de huidige vereisten voor Europese markttoegang, maar meteen ook vergelijkende gegevens verzamelen zodat ook aan de informatienoden van de nationale zorgbetalers, clinici en patiënten wordt voldaan. 28 p. ill, Alle zwangere vrouwen in ons land die dit wensen kunnen vandaag het risico op het syndroom van Down (trisomie 21 - T21) laten vaststellen door een biochemische bloedtest en een echografie. Bij een verhoogd risico wordt vervolgens de diagnose bevestigd of weerlegd door een invasieve test (vlokkentest of vruchtwaterpunctie). Bij deze aanpak worden echter nog heel wat foetussen met T21 gemist, en bij de invasieve testen is er ook een risico van 1% op het onvrijwillig afbreken van de zwangerschap. De niet-invasieve prenatale test (NIPT) is een relatief nieuwe genetische bloedtest, die gevoeliger en nauwkeuriger is dan de klassieke testen, maar die nog erg duur is.De minister vroeg een evaluatie van de NIPT aan de Hoge Gezondheidsraad (HGR) en het Federaal Kenniscentrum voor de Gezondheidszorg (KCE). De evaluatie door het KCE ging vooral over de gezondheidseconomische aspecten. Twee mogelijke manieren om NIPT in te voeren werden onderzocht: (1) NIPT in de eerste lijn ter vervanging van de huidige screeningsprocedure, of (2) NIPT in de tweede lijn enkel nadat de huidige screeningtesten met bloed en echo een verhoogd risico aangeven. Beide opties geven een veel betere voorspelling dan de huidige screening, doen het aantal invasieve testen dalen en zijn een verbetering ten opzichte van de huidige screening. Optie (1), waarbij NIPT wordt aangeboden als screeningstest is puur technisch te verkiezen omdat niet alleen het aantal invasieve testen daalt maar ook minder foetussen die drager zijn van T21 gemist worden. Maaronder meer omdat de kost van de NIPT test vandaag relatief hoog is (460€) wordt op dit ogenblik echter aanbevolen NIPT te vergoeden in optie (2). Mits een substantiële daling van de prijs van de NIPT-test en voldoende kwaliteitsgaranties wordt de terugbetaling van NIPT in optie (1) aanbevolen. 28 p. ill, En Belgique, chaque femme enceinte peut, si elle le souhaite, obtenir une estimation du risque que le fœtus soit porteur du syndrome de Down (trisomie 21 – T21). Actuellement, cette estimation est basée sur une biochimie sanguine et une échographie. Lorsque ce risque s'avère élevé, un test invasif (une biopsie de villosités choriales ou une amniocentèse) est alors pratiqué pour confirmer ou infirmer ce diagnostic. Toutefois, le nombre de fœtus porteurs d’une T21 non décelés par cette approche demeure important. A cela s’ajoute le fait que ces tests invasifs sont associés à un risque de fausse-couche de l’ordre de 1 %. Le test prénatal non invasif (NIPT) est un test génétique relativement récent effectué sur le sang de la mère, plus sensible et plus précis que le dépistage classique. Toutefois, il est toujours relativement onéreux. La ministre a demandé au Conseil Supérieur de la Santé (CSS) et au Centre Fédéral d'Expertise des Soins de Santé (KCE) d'effectuer une évaluation du NIPT. L'évaluation du KCE concernait surtout les aspects relatifs à l'économie de la santé. L'examen a porté sur deux façons dont le NIPT pourrait être introduit dans le cadre du dépistage prénatal du syndrome de Down : (1) Soit le NIPT est effectué en première ligne au lieu de la procédure de dépistage actuelle ; (2) Soit le NIPT est pratiqué en seconde ligne et uniquement lorsque les tests de dépistage actuels sur la base d'une analyse sanguine et d'une échographie indiquent un risque accru. Chacune de ces deux options permet d'aboutir à une estimation du risque nettement plus fiable que celle obtenue dans le cadre du dépistage actuel. Elles permettent en outre de réduire le nombre de tests invasifs pratiqués. Dès lors, elles représentent une amélioration par rapport au dépistage actuel. Au niveau purement technique, la préférence doit être accordée à l'option (1), dans laquelle le NIPT est proposé comme test de dépistage en première instance. En effet, grâce à celle-ci, non seulement le nombre de tests invasifs pratiqués sera moindre, mais le nombre de fœtus porteurs d’une T21 non décelés sera, lui aussi, réduit. Néanmoins, le coût du NIPT est, à l’heure actuelle, relativement élevé (460 €). C’est notamment pour cette raison que, dans un premier temps, son remboursement dans le cadre de l'option (2) est préconisé. Moyennant une baisse significative du coût du NIPT et des garanties suffisantes quant à la qualité, son remboursement se fera alors de préférence dans le cadre de l'option (1). 35-41 Objectives: We studied the cost-effectiveness of tenofovir and entecavir in e antigen positive (CHBe+) and negative (CHBe-) chronic hepatitis B. Methods: Using a multicenter survey including 544 patients we measured patient quality of life and attributable costs by clinical disease stage. Natural disease progression was studied in 278 patients in a single center. A Markov model was constructed to follow hypothetical cohorts of treated and untreated 40-year-old CHBe+ and CHBe- patients and 50-year-old patients with compensated cirrhosis. Results: We did not find an improvement in quality of life when viral load was reduced under treatment. Transition rates to liver cirrhosis were found to be age-dependent. Assuming equal effectiveness, tenofovir dominates the entecavir strategy because of its lower price in Belgium. The incremental cost-effectiveness ratio (ICER) of tenofovir after 20 years is more favorable for treating Caucasian cirrhotic patients (mean ICER €29,000/quality-adjusted life-year [QALY]) compared with treating non-cirrhotic patients (mean ICER €110,000 and 131,000/QALY for CHB e+ and e-, respectively). Within the non-cirrhotic patients the ICER decreases with increasing cohort starting age from 30 to 50 years. Conclusions: Results of long-term models for tenofovir or entecavir treatment of CHB need to be interpreted with caution as long-term trials with hard end points are lacking. Especially the effect on HCC remains highly uncertain. Based on cost-effectiveness considerations such antiviral treatment should be targeted at patients with cirrhosis or at risk of rapid progression to this disease stage.

278-284 Objectives: High-quality clinical evidence is most often lacking when novel high-risk devices enter the European market. At the same time, a randomized controlled trial (RCT) is often initiated as a requirement for obtaining market access in the US. Should coverage in Europe be postponed until RCT data are available? We studied the premarket clinical evaluation of innovative high-risk medical devices in Europe compared with the US, and with medicines, where appropriate. Methods: The literature and regulatory documents were checked. Representatives from industry, Competent Authorities, Notified Bodies, Ethics Committees, and HTA agencies were consulted. We also discuss patient safety and the transparency of information. Results: In contrast to the US, there is no requirement in Europe to demonstrate the clinical efficacy of high-risk devices in the premarket phase. Patients in Europe can thus have earlier access to a potentially lifesaving device, but at the risk of insufficiently documented efficacy and safety. Variations in the stringency of clinical reviews, both at the level of Notified Bodies and Competent Authorities, do not guarantee patient safety. We tried to document the design of premarket trials in Europe and number of patients exposed, but failed as this information is not made public. Furthermore, the Helsinki Declaration is not followed with respect to the registration and publication of premarket trials. Conclusions: For innovative high-risk devices, new EU legislation should require the premarket demonstration of clinical efficacy and safety, using an RCT if possible, and a transparent clinical review, preferably centralized.

Reden is dat de fabrikant in Europa geen bewijs moet leveren dat de patiënt even goed of beter geholpen is met het nieuwe hulpmiddel, dan met de standaard aanpak van het probleem. Les nouveaux dispositifs médicaux à haut risque pour le patient, tel qu’un nouveau type de valve cardiaque ou de prothèse de hanche, arrivent souvent plus rapidement sur le marché européen que sur le marché américain. La raison en est que le fabricant ne doit, en Europe, fournir aucune preuve que ce nouveau dispositif médical aidera le patient, aussi bien ou mieux que le traitement standard. Ainsi, le patient peut avoir accès plus rapidement aux nouvelles technologies, mais est aussi confronté à des risques encore inconnus. Le KCE s’est penché, en collaboration avec d’autres agences européennes similaires, sur le type de résultats de recherche que les fabricants devraient présenter avant d’introduire sur le marché des implants et d’autres nouvelles technologies médicales. Background Infections with SARS-CoV-2 continue to cause significant morbidity and mortality. Interleukin (IL)-1 and IL-6 blockade have been proposed as therapeutic strategies in COVID-19, but study outcomes have been conflicting. We sought to study whether blockade of the IL-6 or IL-1 pathway shortened the time to clinical improvement in patients with COVID-19, hypoxic respiratory failure, and signs of systemic cytokine release syndrome. Methods We did a prospective, multicentre, open-label, randomised, controlled trial, in hospitalised patients with COVID-19, hypoxia, and signs of a cytokine release syndrome across 16 hospitals in Belgium. Eligible patients had a proven diagnosis of COVID-19 with symptoms between 6 and 16 days, a ratio of the partial pressure of oxygen to the fraction of inspired oxygen (PaO2:FiO(2)) of less than 350 mm Hg on room air or less than 280 mm Hg on supplemental oxygen, and signs of a cytokine release syndrome in their serum (either a single ferritin measurement of more than 2000 mu g/L and immediately requiring high flow oxygen or mechanical ventilation, or a ferritin concentration of more than 1000 mu g/L, which had been increasing over the previous 24 h, or lyrnphopenia below 800/mL with two of the following criteria: an increasing ferritin concentration of more than 700 mu g/L, an increasing lactate dehydrogenase concentration of more than 300 international units per L, an increasing C-reactive protein concentration of more than 70 mg/L, or an increasing D-dimers concentration of more than 1000 ng/mL). The COV-AID trial has a 2 x 2 factorial design to evaluate IL-1 blockade versus no IL-1 blockade and IL-6 blockade versus no IL-6 blockade. Patients were randomly assigned by means of permuted block randomisation with varying block size and stratification by centre. In a first randomisation, patients were assigned to receive subcutaneous anakinra once daily (100 mg) for 28 days or until discharge, or to receive no IL-1 blockade (1:2). In a second randomisation step, patients were allocated to receive a single dose of siltuximab (11 mg/kg) intravenously, or a single dose of tocilizumab (8 mg/kg) intravenously, or to receive no IL-6 blockade (1:1:1). The primary outcome was the time to clinical improvement, defined as time from randomisation to an increase of at least two points on a 6-category ordinal scale or to discharge from hospital alive. The primary and supportive efficacy endpoints were assessed in the intention-to-treat population. Safety was assessed in the safety population. This study is registered online with ClinicalTrials.gov (NCT04330638) and EudraCT (2020-001500-41) and is complete. Findings Between April 4, and Dec 6,2020,342 patients were randomly assigned to IL-1 blockade n=112) or no IL-1 blockade (n=230) and simultaneously randomly assigned to IL-6 blockade (n=227; 114 for tocilizumab and 113 for siltuximab) or no IL-6 blockade (n=115). Most patients were male (265 [77%] of 342), median age was 65 years (IQR 54-73), and median Systematic Organ Failure Assessment (SOFA) score at randomisation was 3 (2-4). All 342 patients were included in the primary intention-to-treat analysis. The estimated median time to clinical improvement was 12 days (95% CI 10-16) in the IL-1 blockade group versus 12 days (10-15) in the no IL-1 blockade group (hazard ratio [HR] 0.94 [95% CI 0.73-1.21]). For the IL-6 blockade group, the estimated median time to clinical improvement was 11 days (95% CI 10-16) versus 12 days (11-16) in the no IL-6 blockade group (HR 1.00[0-78-1-29]). 55 patients died during the study, but no evidence for differences in mortality between treatment groups was found. The incidence of serious adverse events and serious infections was similar across study groups. Interpretation Drugs targeting IL-1 or IL-6 did not shorten the time to clinical improvement in this sample of patients with COVID-19, hypoxic respiratory failure, low SOFA score, and low baseline mortality risk. 4-5 Background Silicone adhesive multilayer foam dressings are used as adjuvant therapy to prevent hospital-acquired pressure ulcers (PUs). Objectives To determine whether silicone foam dressings in addition to standard prevention reduce the incidence of PUs of category 2 or worse compared with standard prevention alone. Methods This was a multicentre, randomized controlled medical device trial conducted in eight Belgian hospitals. At-risk adult patients were centrally randomized (n = 1633) to study groups based on a 1 : 1 : 1 allocation: experimental groups 1 (n = 542) and 2 (n = 545) – pooled as the treatment group – and the control group (n = 546). The experimental groups received PU prevention according to hospital protocol, and a silicone foam dressing on the relevant body sites. The control group received standard of care. The primary endpoint was the incidence of a new PU of category 2 or worse at the studied body sites. Results In the intention-to-treat population (n = 1605), PUs of category 2 or worse occurred in 4·0% of patients in the treatment group and 6·3% in the control group [relative risk (RR) 0·64, 95% confidence interval (CI) 0·41–0·99, P = 0·04]. Sacral PUs were observed in 2·8% and 4·8% of the patients in the treatment group and the control group, respectively (RR 0·59, 95% CI 0·35–0·98, P = 0·04). Heel PUs occurred in 1·4% and 1·9% of patients in the treatment and control groups, respectively (RR 0·76, 95% CI 0·34–1·68, P = 0·49). Conclusions Silicone foam dressings reduce the incidence of PUs of category 2 or worse in hospitalized at-risk patients when used in addition to standard of care. The results show a decrease for the sacrum, but no statistical difference for the heel and trochanter areas.

Hélas une grande partie de ces médicaments sont remboursés par l’assurance maladie belge sans preuves suffisantes d’un réel bénéfice pour les patients. Le KCE plaide pour un système plus transparent qui mette davantage l’accent sur la véritable valeur ajoutée pour le patient de chaque nouveau médicament. 58 p. ill.c Au moment où nous écrivons cet avis, la Belgique est bien avancée dans sa campagne de vaccination, avec un taux d’adhésion élevé par rapport à d'autres pays. Sur le plan technologique, les vaccins que nous avons pu utiliser sont hautement efficaces et sûrs ; ils sont proposés à toute personne âgée de 12 ans et plus, essentiellement via les centres de vaccination. Les personnes complètement vaccinées sont largement protégées contre les infections graves et les décès dus au SARS-CoV-2, y compris ceux dus au variant delta actuellement dominant. La plupart des personnes non vaccinées sont pour leur part toujours exposées au risque d'hospitalisation et de formes graves de la maladie. Il est donc primordial de poursuivre les efforts pour convaincre les personnes qui refusent le vaccin, afin de les protéger, elles et leurs proches. Ce rapport offre une vision d'ensemble des données probantes actuellement disponibles. p. 181-188 Background: High-risk medical devices may not always provide a therapeutic added value to patients. In Europe, no proof of efficacy is required to receive a CE label, making it difficult for policymakers to decide on reimbursement of (often expensive) high-risk medical devices. We explore, within the framework of the European legislation, the possibilities at a national level for a guided introduction of such devices. Research design and Methods: HTA and legal experts worked in close collaboration with medical specialists and government representatives making a legal analysis of what is possible under the (revised) European and national legislation. Results: At national level, measures for a better evidence-based introduction can be taken that are not in contradiction with the European regulation. From a legal point of view, all restrictive measures must be justified, necessary and proportional. Several measures are possible, a.o.making use of reference centres, applying the IDEAL framework or the 6-step plan set up by the Dutch Order of Medical Specialists. Conclusions: Within the framework of the (revised) European legislation, measures at national level can be taken to temporarily restrict and follow up the use of high-risk medical devices with a greater focus on the therapeutic added value for the patients. p. 551-560 AIM: Many questions of relevance to patients/society are not answered by industry-sponsored clinical trials. We consider whether there are benefits to governments in funding practice-oriented clinical trials. METHODOLOGY: A literature search including publications on institutions' websites was performed and supplemented with information gathered from (inter)national stakeholders. RESULTS: Areas were identified where public funding of clinical trials is of importance for society, such as head-to-head comparisons or medical areas where companies have no motivation to invest. The available literature suggests publicly funded research programs could provide a positive return on investment. The main hurdles (e.g., sufficient funding and absence of equipoise) and success factors (e.g., selection of research questions and research infrastructure) for the successful conduct of publicly funded trials were identified. CONCLUSION: Governments should see public funding of pragmatic practice-oriented clinical trials as a good opportunity to improve the selection and quality of treatments and stimulate efficient use of limited resources.

p. 267-278 Because it spares many normal tissues and reduces the integral dose, proton therapy (PT) is the preferred tumor irradiation technique for treating childhood cancer. However, to the best of our knowledge, no systematic review of the clinical effectiveness of PT in children has been reported in the scientific literature. A systematic search for clinical outcome studies on PT published between 2007 and 2015 was performed in Medline (through OVID), EMBASE, and the Cochrane Library. Twenty-three primary studies were identified, including approximately 650 patients overall. The median/mean follow-up times were limited (range, 19-91 months). None of the studies were randomized, 2 were comparative, and 20 were retrospective. Most suffered from serious methodologic limitations, yielding a very low level of clinical evidence for the outcomes in all indications. For example, for retinoblastoma, very low-level evidence was found that PT might decrease the incidence of second malignancies. For chondrosarcoma, chordoma, craniopharyngioma, ependymoma, esthesioneuroblastoma, Ewing sarcoma, central nervous system germinoma, glioma, medulloblastoma, osteosarcoma, and rhabdomyosarcoma, there was insufficient evidence to either support or refute PT in children. For pelvic sarcoma (ie, nonrhabdomyosarcoma and non-Ewing sarcoma), pineal parenchymal tumor, primitive neuroectodermal tumor, and "adult-type" soft tissue sarcoma, no studies were identified that fulfilled the inclusion criteria. Although there is no doubt that PT reduces the radiation dose to normal tissues and organs, to date the critical clinical data on the long-term effectiveness and harm associated with the use of PT in the 15 pediatric cancers under investigation are lacking. High-quality clinical research in this area is needed.

La Ministre de la Santé a déjà annoncé qu’elle souhaitait élargir leur remboursement, à partir de 2017, aux patients à un stade moins avancé. Le Centre fédéral d’Expertise des soins de santé (KCE) suggère de continuer sur cette voie et d’envisager de traiter toutes les personnes porteuses du virus, étant donné que plus on traite tôt, plus les bénéfices pour la santé sont importants. Un tel élargissement du remboursement devrait toutefois se faire par paliers de manière à garder le budget de l’assurance maladie sous contrôle. Momenteel wordt deze behandeling enkel terugbetaald voor patiënten met gevorderde leverschade en bij levertransplantatie. De minister van Volksgezondheid heeft al aangekondigd dat ze de terugbetaling vanaf 2017 zal uitbreiden naar patiënten met minder gevorderde schade. Het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) pleit ervoor om op de ingeslagen weg verder te gaan, met het oog op het uiteindelijk behandelen van alle besmette patiënten, want hoe eerder men behandelt, hoe groter de gezondheidswinst.
