Nonvolatile memory using intersubband transitions and quantum-well electron accumulation in GaN/AlN resonant tunneling diodes (RTDs) is a promising candidate for high-speed nonvolatile memory operating on a picosecond timescale. This memory has been fabricated on sapphire(0001) substrates to date because of the high affinity between the nitride materials and the substrate. However, the fabrication of this memory on Si(111) substrates is attractive to realize hybrid integration with Si devices and nonvolatile memory and three-dimensional integration such as chip-on-wafer and wafer-on-wafer. In this study, GaN/AlN RTDs are fabricated on a Si(111) substrate using metal-organic vapor phase epitaxy. The large strain caused by the differences in the thermal expansion coefficients and lattice constants between the Si(111) substrate and nitride materials are suppressed by a growth technique based on the insertion of low-temperature-grown AlGaN and thin AlN layers. The GaN/AlN RTDs fabricated on Si(111) substrates show clear GaN/AlN heterointerfaces and a high ON/OFF ratio of >220, which are equivalent to those for devices fabricated on sapphire(0001) substrates. However, the nonvolatile memory characteristics fluctuate by repeated write/erase memory operations. Evaluation of the ON/OFF switching time and endurance characteristics indicates that the instability of the nonvolatile memory characteristics is caused by electron leakage through deep levels in the quantum-well structure. Objective: Immunoglobulin A (IgA) is suggested to have pathogenic effects in respiratory inflammatory diseases, including asthma. We aimed to analyze the relationship between serum IgA, and clinical indicators and biomarkers of asthma.Methods: This study was a post hoc analysis of the NHOM Asthma Study. In this study, serum IgA was measured using serum samples stored. We determined an association between the serum IgA level and clinical variables and biomarkers using multivariate linear regression and analyzed the differences in clinical indices between IgA high- and IgA low-asthma.Results: In this study, 572 patients with asthma were included in the final analysis. Lower percentage forced expiratory volume in the first second (%FEV1), higher serum eotaxin levels, lower serum ST2 levels, and higher serum MIP-1β levels, were independently and significantly associated with higher serum IgA levels among asthma patients by multivariate linear regression analysis (%FEV1, 95% confidence interval [CI], −8.18– −0.613, p < 0.05; eotaxin, 95% CI, 8.95–46.69, p < 0.001; ST2, 95% CI, −73.71– −7.37, p < 0.05; and MIP-1β, 95% CI, 1.47–18.71, p < 0.05). Furthermore, IgA high-asthma (serum IgA ≥ 238 mg/dL, n = 270) and IgA low-asthma (serum IgA < 238 mg/dL, n = 302) were compared separately. %FEV1 was significantly lower, the percentage of atopy was higher, and serum MIP-1β level was higher in IgA high-asthma.Conclusions: This study suggests that serum IgA may be involved in the worsening of asthma outcomes, as assessed by %FEV1 and enhanced inflammation via elevated serum MIP-1β. In this study, the influence of carrier doping on the thermo- and galvano-magnetic effects of Bi–Sb alloys was investigated. Seven types of Bi88Sb12 alloys, namely, the undoped and 0.01, 0.02, and 0.04 at. % Te- and Sn-doped alloys, were fabricated via the spark plasma sintering method and solid phase reaction. The magnetic field dependencies of the diagonal resistivity, Hall resistivity, thermal conductivity, Seebeck thermopower, and Nernst thermopower were measured in the range of −5–5 T at 300 K, and the dimensionless figures of merit for the Seebeck and Nernst effects, i.e., the zST and zNT, respectively, were determined. As a result, both the zT values in the magnetic field were improved by the addition of a trace amount of Te. To better understand this behavior, a computational model that qualitatively describes the Seebeck, Nernst, magnetoresistance, and Hall effects, as well as the carrier thermal conductivity of Bi in the magnetic field, was developed based on the Boltzmann equation by considering the relaxation time approximation. The influence of carrier doping on the thermo-magnetic effect was considered by shifting the chemical potential with the rigid-band model in the established computational model. The computational results demonstrated that zST and zNT in the magnetic field were improved by shifting the chemical potential. These findings revealed that carrier doping could be a promising approach to improve the zT, namely, the zST for the Seebeck effect and zNT for the Nernst effect in Bi–Sb alloys in a magnetic field. Gold (or electrum) in hydrothermal fluid precipitates directly from gold sulfide complex and/or partly via suspended nanoparticles. The hydrothermal fluid contains "invisible gold" that is atomically dispersed in sulfide minerals or as nanoparticles with a size of less than 10 nm. However, the contribution of these gold nanoparticles to the formation of native gold and its alloy with silver (electrum) remains unclear. The Higashi–Aogashima Knoll Caldera hydrothermal field, south of Tokyo, Japan, is an area of significant seafloor hydrothermal activity that is known for high-grade gold-containing minerals in sulfide-rich rocks. In this study, dry-polished thin sections were created to minimize sample damage and scanning and transmission electron microscopy were used to investigated the cross-sectional and three-dimensional morphologies of native gold grains in a sulfide-rich mound rock from the Central Cone site of the caldera. The surfaces of the gold grains comprised nanoparticles with sizes of 5–50 nm that were also attached to their periphery, which suggests that gold nanoparticles in deep-sea hydrothermal fluid were involved in the mineralization of the gold. In addition, the distribution of silver was uneven within the gold grains, which suggests that the gold precipitation comprised multiple stages at different temperatures that resulted in the post-deposition or secondary remobilization of silver. Sotorasib is a novel KRASG12C inhibitor that has shown robust efficacy, safety, and tolerability in patients with KRASG12C mutation. The objectives of the population pharmacokinetic (PK) analysis were to characterize sotorasib population PK in healthy subjects and patients with advanced solid tumors with KRASG12C mutation from 6 clinical studies, evaluate the effects of intrinsic and extrinsic factors on PK parameters, and perform simulations to further assess the impact of identified covariates on sotorasib exposures. A two-compartment disposition model with three transit compartments for absorption and time-dependent clearance and bioavailability well described sotorasib PK. Sotorasib exposure increased in a less-than-dose proportional manner across the evaluated 180 mg to 960 mg dose range. Disease burden, measured by Eastern Cooperative Oncology Group (ECOG) score and baseline tumor size, were identified as significant covariates on clearance. Lower disease burden was associated with higher clearance (and thus lower sotorasib exposure). Low albumin was found to be associated with lower clearance of sotorasib. Use of PPIs was associated with reduced sotorasib exposures. Although sex, race, and high fat meal were significantly associated with PK parameters, their impact on exposures were not clinically relevant. Other evaluated covariates, including body weight, age, renal impairment (evaluated by chronic kidney disease stage score) and hepatic impairment (evaluated by NCI criteria) were not statistically significant. Greater baseline disease burden and low albumin were associated with lower sotorasib clearance; based on the established efficacy and safety profiles from clinical trials, the estimated magnitude of these effects does not warrant a dose adjustment. Purpose: The exact incidence of and risk factors for interstitial lung disease (ILD), a serious side effect of abemaciclib, remain unknown in real-world settings. We examined the incidence of and risk factors for abemaciclib-induced ILD in patients with advanced breast cancer (ABC) in Japan. Patients and Methods: Retrospective clinical information was collected from charts of patients with ABC who had started abemaciclib treatment at 77 participating institutions between November 30, 2018 and December 31, 2019. The clinical information of patients who developed ILD (including suspected cases) were reviewed by an independent committee of extramural experts to adjudicate abemaciclib-induced ILD. We performed a nested case–control study for efficient identification of ILD risk factors and conducted multivariate Cox regression analysis to identify independent predictors of ILD. Results: Among patients taking abemaciclib, the incidence of abemaciclib-induced ILD was 5.0% (n = 59/1189), and the mortality rate was 0.7% (n = 8). The timing of ILD onset varied but occurred most frequently within 180 days of beginning abemaciclib treatment (64.4%). The incidence of abemaciclib-induced ILD was significantly associated with Eastern Cooperative Oncology Group performance status (ECOG PS) ≥ 2 [hazard ratio (HR) 5.03; 95% confidence interval (CI) 2.26–11.11] or a past medical history of interstitial pneumonia (IP) (HR 6.49; 95% CI 3.09–13.70). Conclusions: In this study, we have for the first time determined the real-world incidence of and risk factors for abemaciclib-induced ILD in Japan. Although abemaciclib-induced ILD is serious in real-world settings, careful patient selection and close monitoring of those with poor ECOG PS and/or a history of IP may minimize ILD risk. This study aimed to investigate the prevalence of daytime sleepiness (DS) and its impact on quality of life (QOL) in outpatients with schizophrenia in the maintenance phase, as well as to identify the factors associated with DS. A total of 191 outpatients with schizophrenia completed a self-administered questionnaire including questions on lifestyle, sleep habits, DS, QOL, and sleep disorders. Insomnia, DS, and QOL were evaluated by the Athens Insomnia Scale (AIS), the Epworth Sleepiness Scale (ESS), and the MOS 8-Item Short-Form Health Survey (SF-8), respectively. The prevalence of DS was assessed with two cut-off points, ESS ≥ 11 (ESS11-DS) and ESS ≥ 8 (ESS8-DS). Psychiatric symptoms were assessed with the Positive and Negative Syndrome Scale (PANSS). Logistic regression analyses were used to identify factors associated with DS. The prevalence of ESS11-DS and ESS8-DS was 7.3% and 21.5%, respectively. Seven of eight QOL domains were reduced in the ESS11-DS group, and four of eight QOL domains were reduced in the ESS8-DS group. In both groups, the Mental Component Summary Score of the SF-8 was decreased. On logistic regression analyses, severity of insomnia was associated with both ESS11-DS and ESS8-DS. Moreover, negative symptoms were associated with ESS11-DS. Psychotropic medications were not associated with either ESS11-DS or ESS8-DS. The present findings suggest that focusing on improving insomnia, rather than reducing medication dosage, may be more important in ameliorating DS and, consequently, QOL in patients with schizophrenia in the maintenance phase. Purpose: Inferior alveolar nerve (IAN) and lingual nerve (LN) blocks are commonly performed using the intraoral landmark techniques. However, these methods have a risk of unanticipated nerve and arterial injury or a higher failure rate. We developed a novel extraoral approach for the IAN and LN blocks, the "inferior alveolar nerve block mandibular angle approach (IANB-MA)," using ultrasound guidance. The mechanism of action of this nerve block was examined anatomically, and its clinical feasibility was reported. Methods: We performed the IANB-MA on four cadavers using different dye volumes (2, 4, 6 and 8 mL). The ultrasound probe was placed on the lower edge of the mandibula of each cadaver, and the needle was advanced to the mandibular inner surface. Blue acrylic paint solution was injected, and its spread was evaluated by dissection. Results: Our study showed that the medial pterygoid muscle fascia was stained in all cadavers. The dye reached the LN consistently, and the IAN was stained with higher volumes (6 mL and 8 mL). The pterygomandibular space was filled with 6 mL and 8 mL of the dye. The IANB-MA successfully reduced pain in three patients with trigeminal neuralgia, tongue or jaw pain. Conclusions: The IANB-MA is a novel ultrasound-guided approach to the IAN and the LN. The clinical feasibility and effectiveness of this technique were confirmed in our patients. It may be a good alternative analgesic approach to other conventional approaches. [ABSTRACT FROM AUTHOR]

Postoperative complications of pulmonary resection in patients with pulmonary aspergilloma are common. Preoperative antifungal therapy is a perioperative management strategy to reduce postoperative complications; however, the benefit of the treatment remains controversial. A retrospective cohort study was conducted using data from the Diagnosis Procedure Combination database (July 2010–March 2021), a national inpatient database in Japan. Patients with pulmonary aspergilloma with or without chronic necrotizing pulmonary aspergillosis who underwent pulmonary resection were identified and divided into two groups based on whether they received preoperative antifungal therapy. Propensity-score overlap weighting was performed to compare the occurrence of composite postoperative complications and postoperative length of hospital stay between the two groups. We identified 98 patients with pulmonary aspergilloma who received preoperative antifungal therapy and 399 patients who did not. After propensity-score overlap weighting, the preoperative antifungal therapy group had a significantly lower proportion of composite postoperative complications than the non-preoperative antifungal therapy group (6.8% vs. 14.2%, P  = .038). There was no significant difference in postoperative length of stay between the groups with and without preoperative antifungal therapy (median 14.6 vs. 15.8 days, P  = .41). Preoperative antifungal therapy was associated with a reduction in postoperative complications in patients with pulmonary aspergilloma. Preoperative treatment with antifungals may be beneficial in reducing the risk of postoperative complications in patients with pulmonary aspergilloma undergoing pulmonary resection. Simple Summary: We evaluated the efficacy of bevacizumab (BEV) in first-line chemotherapy for clear cell carcinoma (CCC). We categorized patients with advanced clear cell carcinoma (CCC) who received platinum-based first-line chemotherapy into a non-BEV group and a BEV group, and compared the progression-free survival (PFS), overall survival (OS), response rates, adverse events, and platinum agent-resistant recurrence rates. The PFS was improved and response rates were higher in the BEV group. One patient in the BEV group had grade 4 gastrointestinal perforation, although no treatment-related death was reported. First-line chemotherapy with BEV in advanced CCC showed the potential for high response rates and an improved PFS. Objective: The usefulness of bevacizumab (BEV) as first-line chemotherapy for advanced ovarian clear cell carcinoma (CCC) was retrospectively evaluated at Tohoku Gynecologic Cancer Unit institutions. Patients and Methods: A total of 81 patients (52 patients without BEV and 29 with BEV) with advanced ovarian CCC who received initial platinum-based chemotherapy were enrolled. We selected 26 patients each without and with BEV according to propensity score matching methods, and compared the platinum-resistant recurrence rate, response rate, progression-free survival (PFS), overall survival (OS), and adverse events between the two groups. Results: The objective response rates for the patients with measurable disease in the non-BEV and BEV groups were 60.0% and 95.5%, respectively (p = 0.007). The recurrence of platinum-resistant patients was observed in 50.0% of the non-BEV group and 23.1% of the BEV group (p = 0.117). The median PFS was 12 and 22 months, respectively (p = 0.034), and the median OS was 32 and 47 months, respectively (p = 0.223). In the BEV group, grade 3 hypertension and proteinuria occurred in two patients, and thrombosis and carboplatin hypersensitivity occurred in one patient. Grade 4 gastrointestinal perforation occurred in one patient. Conclusions: First-line chemotherapy with BEV in advanced CCC showed the potential for high response rates and improved PFS. Background: This study aimed to investigate the relationship between low-dose prednisolone (PSL) and the incidence of hypokalaemia at abiraterone acetate (abiraterone) plus PSL combination therapy targeting Japanese patients with metastatic castration-resistant prostate cancer (mCRPC). Methods: This retrospective observational study included 153 Japanese patients treated with abiraterone and PSL for mCRPC at Kariya Toyota General Hospital and Gifu General Medical Center between September 2014 and October 2022. The incidence of grade ≥ 2 hypokalaemia as well as serum potassium level variations and the continuous combination therapy duration were compared between the low-dose (5 mg/day of PSL) and the standard-dose (10 mg/day of PSL) groups. Results: This study included 153 patient of which 95 were matched to establish the analysis population. The low-dose and the standard-dose groups consisted of 13 and 82 patients, respectively. No significant difference in the incidence of grade ≥ 2 hypokalaemia was observed between the two groups [15.4% (2/13 patients) in the low-dose group and 12.2% (10/82 patients) in the standard-dose group, P = 0.667]. The low-dose group exhibited a decrease in serum potassium levels from 4.63 on day − 7 − 0 to 4.16 mmol/L on day 84 ± 10 (n = 7, P = 0.066), and serum potassium levels from day − 7 − 0 to 84 ± 10 in the low-dose group appeared to be great in the standard-dose group (n = 37, P = 0.475). The Kaplan–Meier curves for continuity of abiraterone and PSL therapy were not significantly different between the low-dose group (n = 13) and standard-dose group (n = 82, P = 0.427). Conclusion: Combination therapy with abiraterone and 5 mg/day of PSL in Japanese patients with mCRPC did not change the incidence of grade ≥ 2 hypokalaemia. However, although not significant, 5 mg/day of PSL demonstrated a decreasing trend in serum potassium levels with a larger degree of change than that of 10 mg/day of PSL. Therefore, the combination of abiraterone and 5 mg/day PSL can be administered to Japanese patients with mCRPC. The patients must be monitored for hypokalaemia through measurement of serum potassium levels and observation of subjective symptoms such as muscle weakness, convulsion etc. In addition, the doctor or the pharmacist must explain these symptoms to the patient and instruct them to consult their medical staff immediately in the event of development of such symptoms. This article explores the multifaceted potential of pullulan‐based films across food‐packaging, pharmaceutical, biomedical, and cosmetic applications. In food‐packaging, pullulan films serve as transparent, flexible, and high‐oxygen barrier materials, effectively preserving the freshness and quality of a wide range of fruits and vegetables. Edible pullulan films extend the shelf life and enhance food safety, while active pullulan films inhibit microbial growth and oxidation, thus supports food preservation. In the pharmaceutical industry, pullulan‐based films offer promising solutions for oral drug delivery, providing biodegradable and rapid disintegration for enhanced solubility and bioavailability of drugs. Additionally, due to their mechanical strength, biocompatibility, and antimicrobial properties, pullulan films demonstrate potential in wound dressings and tissue engineering applications. Furthermore, pullulan's utility extends to the cosmetic industry, where it is used widely in various ingredients in skincare products, cosmetics, and personal care items. Its moisturizing, stabilizing, and film‐forming properties make pullulan an attractive component in the industry. Future research directions should focus on cost‐effective production methods and expanding industrial applications to further enhance their effectiveness and versatility. Topics include evaluating SVT mechanisms through VOP responses; distinguishing between atypical atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT); and challenges posed by inadvertent capture of the Purkinje conduction system during pacing.

Ectoparasites cause serious problems during the aquaculture production of food fishes. In this study, we set out to develop and test protocols for maintenance and sampling European catfish (Silurus glanis L., 1758) stocks infected with a gill monogenean, Thaparocleidus vistulensis (Siwak 1932) Lim 1996. When we compared the feasibility of two cohabitation-based parasite culture systems (i.e., static vs. flow-through), we found that the life cycle of T. vistulensis was completed in both habitats. In our experience, static tank systems with regular water exchange allowed better daily quality control of the parasite culture than continuous flow-through systems. We investigated the microhabitat preference of T. vistulensis on the gills of infected European catfish. A balanced distribution on the two lateral gill sets and a decreasing trend in parasite numbers from anterior gill holobranches towards the posterior ones was observed. Using these results, we developed a minimally invasive sampling protocol to estimate the parasite load of individuals. The biopsy aimed at four sectors (#6, #7, #10, and #11) situated within the distal and middle zones of the first holobranch on the left side, encompassing both rows of filaments. Biopsy-based estimates of parasite loads were validated by comparing them to full parasite counts of the same individuals and showed statistically significant correlations. Our biopsy-based method is designed to identify experimental animals with similar parasite loads and create groups of hosts with comparable burdens. This setup is expected to generate reduced between-group differences for expensive experiments (e.g., high throughput transcriptomic or epigenetic studies). We propose that the biopsy-based pre-sorting procedure should be considered in similar experiments with other cultured fish species and their gill monogeneans following a thorough fine-tuning of the experimental conditions. Effects of the ultrathin 1 nm thick AlGaN regrown layer before gate insulator deposition on the performance of ZrO2/AlGaN/GaN metal‐insulator‐semiconductor (MIS) structures are investigated. In comparison with the reference ZrO2/AlGaN/GaN MIS high‐electron‐mobility transistors (MIS‐HEMTs), MIS‐HEMTs with AlGaN regrown layer exhibit increased maximum drain current (>1050 mA mm−1) and broader full‐width at half maximum (FWHM) of transconductance profile (11 V). Moreover, the gate leakage current in the forward direction is reduced by about four orders of magnitude while the hysteresis of the transfer curves is cut‐down to more than 40%. Compared with the control MIS‐capacitors (MIS‐caps), MIS‐caps with regrowth show highly evident sudden increase in capacitance known as "spill‐over" in the capacitance–voltage (C–V) profiles, suggesting improved ZrO2/AlGaN interfaces. Extracted electron states from the C–V profiles, reveal 6 times to 14 times reduction of interface states density within 0.2 to 0.8 eV from the AlGaN conduction band edge. Using photo‐assisted C–V measurements, the near mid‐gap electron states density of MIS‐caps is extracted with regrowth to be at least three times less than those of reference MIS‐caps. Findings in this work are very similar to those recently reported Al2O3/regrown‐AlGaN interfaces, suggesting that the regrowth technique could be universal in reducing electron states along insulator/AlGaN interfaces. Background: Edoxaban, the only factor Xa inhibitor with active metabolites, is metabolized by carboxylesterase-1 to its main active metabolite, M-4, which is a substrate of organic anion transporting polypeptide 1B1 (OATP1B1) and is excreted in bile and urine. Because the area under the plasma concentration–time curve ratio of M-4 to parent compound is typically less than 10% in healthy subjects, M-4 is generally considered to exhibit negligible antithrombotic activity in patients treated with edoxaban. However, we identified a case in which drug interactions and kidney impairment led to a substantive increase in plasma M-4 concentrations. Case presentation: This case report involved a 68-year-old man with pancreatic cancer who was orally administered edoxaban tablets for prevention of thrombus formation in non-valvular atrial fibrillation, in addition to rifampin and clarithromycin (CAM) for treatment of mycobacterium avium complex lung disease. These medications were temporarily discontinued for a pancreaticoduodenectomy but were resumed 8 days post-surgery (POD8). On POD9, the patient developed acute kidney injury, and the trough concentrations of edoxaban and M-4 were 131.1 ng/mL and 115.8 ng/mL, respectively (M-4 ratio: 88.3%). On POD11, the M-4 trough concentration and M-4 ratio increased to 216.2 ng/mL and 186.2%, respectively. The plasma concentration of coproporphyrin-I, an endogenous biomarker of OATP1B1 activity, increased during this period. Conclusions: This case suggests that in patients with impaired renal function taking edoxaban, co-administration of carboxylesterase-1 inducers such as rifampin and/or OATP1B1 inhibitors such as rifampin or clarithromycin may increase plasma concentrations of M-4 to clinically non-negligible levels. This study aimed to investigate the effect of diagnosis and treatment of chronic endometritis (CE) on the outcome of assisted reproductive technology (ART) with or without repeated implantation failure (RIF). This retrospective analysis included patients who underwent pathological examination for diagnosis of CE at Yamagata University Hospital. The examination was performed for all patients planned for ART with or without RIF. Patients who were examined within 6 months of the first oocyte retrieval or embryo transfer were included. We counted the number of CD138-positive cells within the endometrial stroma in patients' specimens and analyzed the patients' clinical information. Clinical rates of pregnancy and implantation were determined. A total of 80 women met the inclusion criteria: 13 CE-negative patients (17.3%) and 67 CE-positive patients (83.7%). A significant decrease was noted in the CD138-positive cell count between the first biopsy and second biopsy after CE treatment (p < 0.001). In addition, no significant differences were noted in ongoing pregnancy rates between the CE-negative patients and those who underwent CE treatment. The CD138-positive cell counts at first biopsy tended to be lower in each pregnancy group than in the non-pregnancy group. For patients planned to undergo ART, examination for diagnosis of CE with or without RIF could be considered. Pathological CD138-positive cell counts were considered useful for CE diagnosis and treatment decision-making. This study investigated the applicability of fluorescent chromophores for exciton‐coupled circular dichroism (ECCD) exploiting fluorescence‐detected circular dichroism (FDCD). FDCD had been previously reported useful in allowing the sensitive detection of ECCD in favorable conditions. However, fluorescence detection may prevent applications of the combined method especially when solutions are polarized in emission. Even without polarization of emission, FDCD deviates from circular dichroism (CD) in some cases when the fluorophore of interest interacts with nonfluorescent chromophore. Herein, it was confirmed that employing 6‐methoxy‐2‐naphthoate always yielded interpretable exciton‐coupled FDCD spectra even when coupling with nonfluorescent p‐substituted benzoates. The 6‐methoxy‐2‐naphthoate chromophore (6‐MN) is prescribed in special cases when only a small amount of sample is available for determining the absolute stereochemistry by the CD exciton chirality method observed by FDCD. Persistent infection with human papillomavirus (HPV) is a key driver in the development of cervical cancer (CC). We aimed to elucidate the relationship between preoperative high-risk HPV status and prognosis of cervical intraepithelial neoplasia (CIN) in patients undergoing cervical conization. We retrospectively analyzed data from 2546 patients with CIN who underwent HPV deoxyribonucleic acid (DNA) testing and cervical conization in two individual years, i.e., 2009 and 2013, at 205 Japanese institutions. Patients were categorized into five groups based on their high-risk HPV status: highrisk HPV negative (Group 1); HPV 16/18 positive (Group 2); positive for HPV types 31, 33, 35, 45, 52 or 58 (Group 3); other high-risk HPV positive (Group 4); and unconfirmed high-risk HPV status (Group 5). Logistic and Cox regression analyses were conducted for statistical assessment. The distribution of participants across Groups 1 to 5 was 8.1%, 26.3%, 20.1%, 3.0% and 42.5%, respectively. Cervical conization identified CC in 3.9% (99 patients) of the cohort. Multivariate analysis revealed that diagnostic conization, preoperative diagnosis of CIN grade 3 and HPV 16/18 positivity were significant risk factors for post-conization CC. Notably, no correlation was found between preoperative HPV status and post-conization recurrence in patients without CC. HPV types 16 and 18 emerged as significant independent risk factors for CC development following conization. The study findings underscore the need for vigilant management of this patient group. Teratoma is a germ cell tumor composed of 2 or 3 germ cell layers, and it can occur in various parts of the human body. However, teratomas of the renal hilum are particularly rare, and those complicated by carcinoids are even more uncommon. Herein, we report the example of an asymptomatic 49-year-old woman in whom a tumor in the right renal hilum was unexpectedly discovered on imaging. Histological examination revealed a carcinoid tumor arising from a simple cyst composed of teratomatous tissue. Although the tumor was located in the renal hilum and touched the renal parenchyma, it appeared independent of the kidney and urinary tract. The electrochemical corrosion potential (ECP) data of nickel-base alloy 82 weld metal (Alloy 82) in both O2 and H2O2 environments were measured. Although Alloy 82 is used at various welds in boiling water reactors (BWRs), the ECP data of Alloy 82 under BWR conditions are limited. The ECP data measured in this study revealed that Alloy 82 had higher ECPs than 304 and 316 L stainless steels and nickel-base alloy 182 weld metal (Alloy182) at low oxidant concentrations in both O2 and H2O2 environments. The ECPs of Alloy 82 calculated with an analytical ECP model that was developed in this study agreed with measured values in the cases that it was acceptable for the model to have an error range of ± 0.1 V in the tested range of O2 and H2O2 concentrations. The possible reason for the ECPs of Alloy 82 to be higher than those other materials at low oxidant concentrations is given as follows: the anodic polarization curve of Alloy 82 exhibits a lower current density at low potentials because of its higher Ni and Cr contents. Background: An adaptive cardiac resynchronization therapy (aCRT) algorithm allows continuous adjustments of pacing timings of atrioventricular delays by periodic automatic evaluation of electrical conduction. This applies to patients with an atrioventricular block and is effective in cardiac resynchronization therapy (CRT) devices; however, whether this algorithm benefits patients with pacemaker dependency is uncertain. Methods: This study examined the clinical impact of an aCRT algorithm in patients diagnosed with heart failure with reduced ejection fraction and pacemaker dependency. A total of 359 patients underwent CRT between January 2016 and December 2022. Patients undergoing pacemaker-dependent CRT with the aCRT algorithm function were selected. Sixty-four patients with pacemaker dependency (31 with aCRT algorithm and 33 without) were included. Pacemaker dependency was defined as the absence of spontaneous ventricular activity during the sensing test at VVI 30 bpm or prolonged atrioventricular delay (> 300 ms). The primary endpoint was the composite clinical outcome of all-cause death or hospitalization for heart failure. Results: No significant differences were observed in baseline characteristics between groups. During a median follow-up of 1,067 days (interquartile range 553–1,776 days), aCRT reduced the risk of composite clinical outcomes in patients with pacemaker dependency (log-rank P = 0.028). In addition, using the aCRT algorithm was an independent predictor of the composite clinical outcomes in the multivariate analysis (hazard ratio 0.34, 95% confidence interval: 0.12–0.94, P = 0.038). Conclusion: The aCRT algorithm significantly reduced the risk of adverse clinical outcomes in patients with pacemaker dependency. This algorithm may be an important tool for managing such patients. Ovarian cancer (OC) is the most lethal gynecologic cancer, mainly due to late diagnosis with widespread peritoneal spread at first presentation. We performed metabolomic analyses of ovarian and paired control tissues using capillary electrophoresis-mass spectrometry and liquid chromatography-mass spectrometry to understand its metabolomic dysregulation. Of the 130 quantified metabolites, 96 metabolites of glycometabolism, including glycolysis, tricarboxylic acid cycles, urea cycles, and one-carbon metabolites, showed significant differences between the samples. To evaluate the local and systemic metabolomic differences in OC, we also analyzed low or non-invasively available biofluids, including plasma, urine, and saliva collected from patients with OC and benign gynecological diseases. All biofluids and tissue samples showed consistently elevated concentrations of N1,N12-diacetylspermine compared to controls. Four metabolites, polyamines, and betaine, were significantly and consistently elevated in both plasma and tissue samples. These data indicate that plasma metabolic dysregulation, which the most reflected by those of OC tissues. Purpose: Bladder dysfunction associated with type 2 diabetes mellitus (T2DM) includes urine storage and voiding disorders. We examined pathological conditions of the bladder wall in a rat T2DM model and evaluated the effects of the phosphodiesterase-5 (PDE-5) inhibitor tadalafil. Materials and methods: Male Otsuka Long-Evans Tokushima Fatty (OLETF) rats and Long-Evans Tokushima Otsuka (LETO) rats were used as the T2DM and control groups, respectively. Tadalafil was orally administered for 12 weeks. Micturition behavior was monitored using metabolic cages, and bladder function was evaluated by cystometry. Bladder blood flow was evaluated by laser speckle imaging, and an organ bath bladder distention test was used to measure adenosine triphosphate (ATP) release from the bladder urothelium. The expression levels of vesicular nucleotide transporter (VNUT), hypoxia markers, pro-inflammatory cytokines and growth factors in the bladder wall were measured using real-time polymerase chain reaction and enzyme-linked immunosorbent assay. Bladder wall contractions in response to KCl and carbachol were monitored using bladder-strip tests. Results: With aging, OLETF rats had higher micturition frequency and greater urine volume than LETO rats. Although bladder capacity was not significantly different, non-voiding bladder contraction occurred more frequently in OLETF rats than in LETO rats. Bladder blood flow was decreased and ATP release was increased with higher VNUT expression in OLETF rats than in LETO rats. These effects were suppressed by tadalafil administration, with accompanying decreased HIF-1α, 8-OHdG, IL-6, TNF-α, IGF-1, and bFGF expression. The impaired contractile responses of bladder strips to KCl and carbachol in OLETF rats with aging were restored by tadalafil administration. Conclusions: The T2DM rats had polyuria, increased ATP release induced by decreased bladder blood flow and impaired contractile function. The phosphorylation state of 20 types of intracellular proteins in the presence of the protein phosphatase 1 (PP1)- and PP2A-specific Ser/Thr phosphatase inhibitor calyculin A or the Tyr phosphatase inhibitor pervanadate was visualized by Phos-tag SDS-PAGE followed by immunoblotting. All blots showed a Phos-tag pattern indicating increased phosphorylation in the presence of one or both phosphatase inhibitors. The increase in phosphorylation stoichiometry per protein tends to be greater for Ser/Thr phosphatase inhibition than for Tyr phosphatase inhibition. This is consistent with the fact that the number of Ser/Thr kinase genes in the human genome is greater than that of Tyr kinases and with the fact that the phospho-Ser/phospho-Thr ratio in the actual human phosphoproteome is far greater than that of phospho-Tyr ratio. This suggests that cellular proteins are routinely and randomly phosphorylated by different kinases with no biological significance, simply depending on the frequency of substrate encounters. Phosphatase is responsible for routinely removing these unwanted phosphate groups systematically and maintaining the dynamic equilibrium of physiological protein phosphorylation. The patient was diagnosed with perimembranous ventricular septal defect (VSD). She underwent VSD closure and muscle bundle resection across right ventricular outflow tract at the age of 3 months. Since then, she had suffered from severe heart failure and complete heart block. Permanent pacemaker generator was implanted in the left hypochondrium. She was depended on continuous catecholamine administration, so transferred to our hospital for further management. On arrival, her body weight was 5686 g (− 2.7 SD). She underwent Excor pediatric left ventricular assist device implantation at the age of 9 months. Because the position of the left ventricular assist device cannula interfered with the pacemaker, herein, the pacemaker pocket was newly created in the left thoracic cavity. An 1 mm in thickness of expanded polytetrafluoroethylene sheet was trimmed and sutured under the anterolateral wall of left thoracic cavity as a pacemaker pocket. Bipolar ventricular lead was sutured on left ventricular apex and basal wall to face each other, mimicking cardiac regeneration therapy. Even though she unfortunately required right diaphragmatic plication for iatrogenic phrenic nerve palsy, her respiratory function was well maintained; therefore, secondary right heart failure was not observed. TiZrHfAgV0.2 (Ti23.8Zr23.8Hf23.8Ag23.8V4.8, at%) high entropy alloys (HEAs) with a dual bcc structure were developed. Fine lamella structure was observed in the arc-melted ingots and melt-spun ribbons. The TiZrHfAgV0.2 HEAs with a dual bcc phase structure were designed by exploiting the concept of immiscibility of the constituent elements. The influence of sample geometry on various measured physical properties (including the magneto-Seebeck effect, Nernst effect, magnetoresistance effect, Hall effect, and thermal conductivity) in the presence of a magnetic field was examined using a polycrystalline Bi-Sb sample. The sample, consisting of polycrystalline Bi88Sb12, was prepared through spark plasma sintering and subsequent annealing. Measurements of the physical properties were conducted under a magnetic field of 5 T, and the obtained values were compared with simulated results derived using the finite element method for different sample geometries. Distinct shapes were found to be necessary for accurate measurements, with each physical property requiring a specific aspect ratio of sample length (l) to width (w). These ratios were determined to be l/w > 0.57, 2.9, 4.2, 1.2, and 3.1 for the magnetoresistance, Hall, two-wire magneto-Seebeck, four-wire magneto-Seebeck, and Nernst effects, respectively. Additionally, to achieve a minimal error of less than 2% in thermal conductivity measurement, a thermal conductance ratio of Ks/Kw > 27 was necessary, where Ks and Kw denote the thermal conductance of the sample and lead wire, respectively. Mesonephric-like adenocarcinoma is a rare neoplasm of the uterine corpus and ovary. Unlike prototypical mesonephric adenocarcinoma of the uterine cervix, which is considered of Wolffian origin, recent evidence suggests that mesonephric-like adenocarcinoma is a Mullerian tumor associated with endometriosis. We report here on a 48-year-old woman with a mixed carcinoma of the ovary that consisted of mesonephric-like adenocarcinoma, clear cell carcinoma, and endometrioid carcinoma, arising from an endometriotic cyst. The mesonephric-like adenocarcinoma consisted of cuboidal cells with vesicular nuclei presenting with a tubular, ductal, papillary, and solid architecture forming nodules. Each component showed distinct immunophenotypes that were consistent with their morphology. The mesonephric-like adenocarcinoma showed diffuse positive staining for paired box 8 and GATA binding protein 3, and negative staining for estrogen and progesterone receptors. A p53 stain exhibited wild-type immunoreactivity. A complete loss of AT-rich interactive domain-containing protein 1A (ARID1A) expression was suggestive of an ARID1A mutation. Manual macrodissection and Sanger sequencing revealed identical KRAS and PIK3CA mutations in all three components. To the best of our knowledge, this is the first report of mesonephric-like adenocarcinoma combined with a clear cell carcinoma and endometrioid carcinoma, which supports the hypothesis that mesonephric-like adenocarcinoma is an endometriosis-associated neoplasm. Objective: Hospital treatment volume affects survival in patients with endometrial cancer; notably, initial treatment at high-volume centers improves survival outcomes. Our study assessed the effect of hospital treatment volume on cost-effectiveness and survival outcomes in patients with endometrial cancer in Japan. Methods: A decision-analytic model was evaluated using the following variables and their impact on cost-effectiveness: 1) hospital treatment volume (low-, intermediate-, and high-volume centers) and 2) postoperative recurrent risk factors based on pathological findings (high- and intermediate-risk or low-risk). Data were obtained from the Japan Society of Obstetrics and Gynecology database, systematic literature searches, and the Japanese Diagnosis Procedure Combination database. Quality-adjusted life years (QALY) was used as a measure of effectiveness. The model was built from a public healthcare perspective and the impact of uncertainty was assessed using sensitivity analyses. Results: A base-case analysis showed that the incremental cost-effectiveness ratio at high-volume centers was below a willingness-to-pay (WTP) threshold of ¥5,000,000 with a maximum of ¥3,777,830/4.28 QALY for the high- and intermediate-risk group, and ¥2,316,695/4.57 QALY for the low-risk group. Treatment at the high-volume centers showed better efficiency and cost-effectiveness in both strategies compared to intermediate- or low-volume centers. Sensitivity analyses showed that the model outcome was robust to changes in input values. With the WTP threshold, treatment at high-volume centers remained cost-effective in at least 73.6% and 78.2% of iterations for high- and intermediate-risk, and low-risk groups, respectively. Objectives: Non-viral mediated plasmid DNA transfection by electroporation (EP) is an established method for gene transfection. In this study, the usefulness of direct EP at an intradermal (i.d.) site (DEP) with implanted electrodes to achieve a high protein expression level was investigated. In addition, DEP application with various intervals with a low application voltage was also evaluated to confirm its effect on protein expression. Methods: Green fluorescent protein (GFP)- and luciferase-encoding DNA were administrated, and GFP and luciferase were evaluated. Results: A higher protein expression level was observed after green fluorescent protein (GFP)- and luciferase-encoding DNA were delivered by i.d. injection followed by DEP application. When luciferase expression was observed with an in vivo imaging system, continuous expression was confirmed over 21 days after i.d. injection followed by DEP at 100 V. This approach provided increased gene expression levels compared with conventional EP methods via the stratum corneum layer. In addition, the effect of application voltage on luciferase expression was investigated; two-time applications (repeated DEP) at 20 V with 5 min intervals showed similar luciferase expression level to single DEP application with 100 V. Histological observations showed the skin became thicker after a single DEP at 100 V, whereas no apparent thickness changes were confirmed after repeated DEP at 20 V with 5 min intervals. Conclusions: This study revealed that direct i.d. voltage application achieved high protein expression levels even at low voltages. Background: Techniques for diagnosing intestinal parasites need technological advancements in the preanalytical (collection/processing) and analytical (detection) stages. The dissolved air flotation (DAF) technique effectively recovers parasites from processed feces for routine diagnosis. Artificial intelligence (AI) is a practical and affordable alternative to modernize the analysis stage of microscopy images and generates high efficiency in the parasitological examination of feces. Methods: The objective of this study was to standardize a laboratory protocol for stool processing using the DAF technique in conjunction with an automated diagnosis of intestinal parasites (DAPI) system. A total of 400 samples were obtained to perform the tests with the use of DAF to verify the recovery of the parasites as a function of the chemical reagent (polymer and surfactant), the volume of the flotation tube, and standardization of smear assembly on a microscopy slide, with automated analysis by DAPI. The DAF protocol that obtained the most satisfactory results in terms of parasite recovery (P < 0.05) and slide positivity was compared with the Three Fecal Test (TF-Test) protocol with manual (microscopists) and automated (DAPI) evaluation. We compared the sensitivity with the modified TF-Test technical protocol and the diagnostic agreement with the gold standard (Kappa) result. Results: There was no significant difference in the parasite recovery between the 10 ml and 50 ml tubes (P > 0.05). The surfactants showed a range of parasite recoveries between 41.9% and 91.2% in the float supernatant. We obtained a maximum positivity of 73% of the assembled slides when we applied DAF processing with 7% CTAB surfactant and 57% positivity with the modified TF-Test technique. Regarding diagnostic performance, the TF-Test-modified and DAF techniques used in fecal processing for subsequent computerized analysis by AI presented sensitivities of 86% and 94%, with kappa agreements of 0.62 and 0.80 (substantial), respectively. Conclusions: The DAF protocol defined in this study and the DAPI system are innovative processes for parasite recovery and fecal debris elimination that are favorable for effectively detecting pathogenic structures in laboratory diagnosis. Conductive polyoxovanadate inorganic‐organic hybrid crystals comprising alkaline earth metal (divalent) cations were successfully obtained with the use of an ionic‐liquid cation having imidazolium and methacryloyl moieties (denoted as MAImC1). Two types of crystals containing decavanadate ([V10O28]6−, V10) anion were obtained as [MAImC1]2Ca2[V10O28] ⋅ 16H2O ⋅ 2 C2H5OH (MAImC1−Ca−V10) and [MAImC1]2Mg2[V10O28] ⋅ 18H2O ⋅ 2 C2H5OH (MAImC1−Mg−V10). Connection modes of the divalent cations were different: Ca2+ cations were connected to V10 to form a {[Ca(H2O)5]2V10O28]}2− anion in MAImC1−Ca−V10, while a discrete [V10O28]6− and hydrated [Mg(H2O)6]2+ were present in MAImC1−Mg−V10. Conductivities under a fully humidified condition at 353 K (80 °C) were high values of 3.0×10−4 S cm−1 for MAImC1−Ca−V10 and 3.3×10−3 S cm−1 for MAImC1−Mg−V10, respectively. The higher conductivity under hydrated conditions suggests proton as a conductive carrier. The better conductivity of MAImC1−Mg−V10 is plausibly derived from the more effective hydrogen‐bonded network in the crystal lattice. [ABSTRACT FROM AUTHOR]

To date, there have been no reports of recording epicardial electrograms at the onset of spontaneous ventricular fibrillation (VF) in patients with Brugada syndrome (BrS). In the case of BrS, unipolar and bipolar electrogram recording on the right ventricular epicardium revealed that dispersion of repolarization with delayed potential was associated with spontaneous occurrence of VF. Phase 2 reentry associated with shortening and dispersion of action potential could have been recorded for the first time in BrS. Modern society's keen regard for aesthetics made hair products an integral part of a multi‐billion‐dollar cosmetic industry. Hair care products (e.g., shampoos and conditioners) and chemical treatments (e.g., bleaching and permanent waving) result in various effects on the morphological attributes of hair. Generally, water adsorbed on the hair surface is known to significantly dictate the hair's mechanical characteristics (smoothness and friction), and hair's macroscopic wettability has been commonly used to indicate its surface properties. However, an approach to selectively characterize the hydration water in the hair surface is required to accurately understand the intermolecular events between the hair and its vicinal water. In this paper, we successfully obtained the infrared (IR) absorption spectra of the hydration water of human hair. We employed the multivariate curve resolution‐alternating least square (MCR‐ALS) method to separate the hydration and bulk water spectra from the whole spectra. Comparing the IR spectra of the hydration water of chemically untreated and bleached hair samples, we conclude that water molecules form strong hydrogen bonds with the bleached hair surface due to the destruction of the hair's hydrophobic outer layer and the consequent formation of hydrophilic residues. Background: Understanding the factors related to patient-reported outcomes (PROs) after anterior cruciate ligament reconstruction (ACLR) can lead to more effective interventions. Purpose/Hypothesis: The purpose of this study was to identify factors associated with PRO scores after ACLR. It was hypothesized that concomitant meniscal treatment and postoperative range of motion (ROM) would be associated with early postoperative PRO scores and that postoperative physical findings would be associated with 2-year postoperative PRO scores. Study Design: Cohort study; Level of evidence, 2. Methods: We examined the data from the Tokyo Medical and Dental University Multicenter Arthroscopic Knee Surgery (TMDU MAKS) Study for patients who underwent primary ACLR with autologous hamstring tendon grafts; 1252 patients in the TMDU MAKS Study were eligible for inclusion. The International Knee Documentation Committee (IKDC) subjective score, Knee injury and Osteoarthritis Outcome Score (KOOS), and Lysholm score of the patients at 3 months (n = 675), 1 year (n = 660), and 2 years (n = 375) postoperatively were assessed using multiple regression to evaluate the strength of the relationship between PRO scores and the following predictor variables: patient-specific factors, treatment-specific factors, and physical findings. Results: Improvement in all PROs significantly exceeded the minimal important change at 1 and 2 years postoperatively. Older age and female sex were predictive of lower PRO scores up to 2 years postoperatively. Concurrent meniscal treatment and limited range of knee motion were predictive of lower PRO scores at 3 months and 1 year postoperatively. A tighter knee (ie, less anterior translation) on the injured side compared with the contralateral knee was predictive of lower KOOS–Quality of Life and IKDC scores at 2 years postoperatively. At all 3 postoperative time points, greater side-to-side difference in knee ROM was predictive of lower PRO scores. Conclusion: Inferior PRO scores were associated with concomitant meniscal treatment and limited postoperative ROM until 1 year postoperatively and with older age and female sex up to 2 years postoperatively. A 72-year-old man was referred to our hospital for the examination of a pancreatic head mass. Abdominal computed tomography revealed a contrasted 8-cm-diameter tumor extending from the dorsal pancreatic head to the porta hepatis. The preoperative diagnosis was challenging due to the absence of specific imaging findings and the inability to perform a biopsy. Positron emission tomography/computed tomography and diffusion-weighted imaging suggested a malignant tumor originating from the organs surrounding the pancreatic head. Subtotal stomach-preserving pancreaticoduodenectomy with regional lymph node dissection was performed, as dissection from the pancreatic head proved unfeasible. Pathological examination identified the tumor as an enlarged lymph node consisting of pleomorphic large cells forming clusters, positive for follicular dendritic cell markers cluster of differentiation (CD) 21 and CD23. No evidence of tumor capsule infiltration, other organ infiltration, or metastasis to other lymph nodes was observed. The final diagnosis was nodal follicular dendritic cell sarcoma (FDCS) originating from the pancreatic head lymph nodes. No recurrence occurred at 3 years postoperatively with no postoperative treatment. Intraperitoneal nodal FDCS is extremely rare, and occasionally, it can lead to postoperative recurrence and progression. Intense infrared (IR) rays can heat matters and evaporate waters thermally. One of the possible applications will be hair dryer, although the irradiation effects of IR rays on the hair have not been fully explored. In this study, we first examined the interaction of IR rays at various wavelengths from 3.0 µm (near IR) to 90 µm (far IR) with the surface structure of human hair by using IR free electron lasers (FELs). IR-FEL is an accelerator-based pico-second pulse laser, and the feature is the wavelength-tunability with the high-photon density. When one thread of hair was irradiated by the FEL of 6–7 mJ energy at 60 µm, the cleavage occurred, and the morphological destruction was observed on scanning-electron microscopy images after the irradiations at 70 µm and 6.1 µm (amide I). Synchrotron-radiation infrared microspectroscopy showed that those FEL irradiations decreased a shoulder band at 1710 cm−1 that corresponds to carboxyl group in melanin or fatty acids and increased absorption intensity at 500–600 cm−1. On the contrary, the FEL at 90 µm little changed either the surface morphology or the infrared absorption spectra. Interestingly, near-IR FELs at 3.0–3.5 µm induced bending of a hair, and 2D mapping of protein secondary conformations revealed that β-sheet was more increased than the other conformations on the surface of the bending area even at low pulse energy (1–3 mJ). As a result, the structural damage of the hair was least at 90 µm, which implies an ideal wavelength for drying hair mildly.Article highlights: A unique assessment of hair structure is demonstrated by using accelerator-based infrared laser system. A new far-infrared wavelength for drying hair softly is discovered by this research. Applicability of accelerator-based infrared laser for activities of human life is first shown by this research.
