1. Improving the quality of inpatient discharge summaries.

Abstract

Background: Discharge summaries (DCS) are vital in facilitating handover to community colleagues. Unfortunately, at Whittington Health, General Practitioners (GPs) found it difficult to identify relevant information in DCS, and use of medical jargon meant patients did not understand details of their admission. With this quality improvement project, the team aimed to improve DCS to enhance patient-centered care., Objective: The aim of this quality improvement project (QIP) was to improve the quality of DCS by critiquing the ones produced within our trust and implementing various interventions., Methods: Multiple Plan-Do-Study-Act (PDSA) cycles were completed. A multi-disciplinary meeting was conducted to identify the needs of each party in a DCS. A new template was subsequently launched. Teaching was conducted and educational leaflets were disseminated hospital-wide. Quality of written communication was audited quarterly, and evaluated against quality indicators. Problems with DCS were identified via GP and patient feedback, and these became the focus of subsequent PDSA cycles., Results: From March 2019 to February 2020, all the audited categories improved, with an overall improvement from 67% to 92%. We also received positive feedback from GPs., Conclusions: Quality of DCS can be improved with appropriate interventions, leading to improved patient care. A similar PDSA cycle could be utilized elsewhere to achieve similar results.

OBJECTIVE: Evidence on how best to intervene to improve paediatric acute care and therefore reduce unplanned hospital admissions is weak. We describe service evaluation work at one hospital to assess interventions at critical clinical and service decision points. DESIGN: We conducted an observational study using routine daily-collected data (April 2009-December 2015) from a medium-sized district general hospital in south-west UK, using before-and-after comparisons of admissions-related data to evaluate two interventions implemented in April and November 2014, respectively: (1) an advice and guidance (A&G) phone line, where a senior paediatrician is available for general practitioners (GPs) and emergency department (ED) and (2) a Short Stay Paediatric Assessment Unit (SSPAU). We analysed data on all admitted children (<18 years) in the catchment area (population estimate 27 740 in 2015). Outcomes were GP-referred attendances, ward admissions, less than 1 day admissions and length of stay. RESULTS: A&G phone line was associated with a reduction in the mean number of less than 1 day admissions per month (difference in means before and after intervention -16.6 (95% CI -0.2 to -32.9)) and an increase in overall monthly bed-days (difference 72.5 (95% CI 21.0 to 124.0)), but there was little evidence of a change in GP-referred attendances or ward admissions. SSPAU was associated with a reduction in the mean number of monthly ward admissions (difference -34.6 (95% CI -21.3 to -48.0)) and less than 1 day admissions (difference in means -21.7 (95% CI -8.4 to -35.1)) and a reduction in the mean number of overall bed-days per month (difference -50.2 (95% CI -12.1 to -88.3)). CONCLUSIONS: Interventions for reducing time taken to senior clinician review may be effective in better managing paediatric acute care. Further work should explore results by age, condition and injury/illness status.

3. Stratification of Patients With Sjögren's Syndrome and Patients With Systemic Lupus Erythematosus According to Two Shared Immune Cell Signatures, With Potential Therapeutic Implications.

Abstract

Objective: Similarities in the clinical and laboratory features of primary Sjögren's syndrome (SS) and systemic lupus erythematosus (SLE) have led to attempts to treat patients with primary SS or SLE with similar biologic therapeutics. However, the results of many clinical trials are disappointing, and no biologic treatments are licensed for use in primary SS, while only a few biologic agents are available to treat SLE patients whose disease has remained refractory to other treatments. With the aim of improving treatment selections, this study was undertaken to identify distinct immunologic signatures in patients with primary SS and patients with SLE, using a stratification approach based on immune cell endotypes., Methods: Immunophentyping of 29 immune cell subsets was performed using flow cytometry in peripheral blood from patients with primary SS (n = 45), patients with SLE (n = 29), and patients with secondary SS associated with SLE (SLE/SS) (n = 14), all of whom were considered to have low disease activity or be in clinical remission, and sex-matched healthy controls (n = 31). Data were analyzed using supervised machine learning (balanced random forest, sparse partial least squares discriminant analysis), logistic regression, and multiple t-tests. Patients were stratified by K-means clustering and clinical trajectory analysis., Results: Patients with primary SS and patients with SLE had a similar immunologic architecture despite having different clinical presentations and prognoses. Stratification of the combined primary SS, SLE, and SLE/SS patient cohorts by K-means cluster analysis revealed 2 endotypes, characterized by distinct immune cell profiles spanning the diagnoses. A signature of 8 T cell subsets that distinctly differentiated the 2 endotypes with high accuracy (area under the curve 0.9979) was identified in logistic regression and machine learning models. In clinical trajectory analyses, the change in damage scores and disease activity levels from baseline to 5 years differed between the 2 endotypes., Conclusion: These findings identify an immune cell toolkit that may be useful for differentiating, with high accuracy, the immunologic profiles of patients with primary SS and patients with SLE as a way to achieve targeted therapeutic approaches., (© 2021 The Authors. Abstract

Precision Medicine is an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle. Autoimmune diseases are those in which the body's natural defense system loses discriminating power between its own cells and foreign cells, causing the body to mistakenly attack healthy tissues. These conditions are very heterogeneous in their presentation and therefore difficult to diagnose and treat. Achieving precision medicine in autoimmune diseases has been challenging due to the complex etiologies of these conditions, involving an interplay between genetic, epigenetic, and environmental factors. However, recent technological and computational advances in molecular profiling have helped identify patient subtypes and molecular pathways which can be used to improve diagnostics and therapeutics. This review discusses the current understanding of the disease mechanisms, heterogeneity, and pathogenic autoantigens in autoimmune diseases gained from genomic and transcriptomic studies and highlights how these findings can be applied to better understand disease heterogeneity in the context of disease diagnostics and therapeutics. Abstract

Social prescribing (SP) is increasing in popularity in the UK and can enable healthcare providers to respond more effectively to a range of non‐clinical needs. With the NHS commitment to establish an SP link worker in all GP practices, there is a rapid increase in the number of SP schemes across the country. There is currently insufficient evidence concerning the implementation and acceptability of SP schemes. In this paper, we report our analysis of the descriptions of the experiences of SP link workers, regarding the early implementation of SP link workers in two SP programmes in the South West. Data were gathered using the 'Researcher in Residence' (RiR) model, where the researcher was immersed in the environments in which the SP was managed and delivered. The RiR undertook conversations with 11 SP link workers, 2 SP link worker managers and 1 SP counsellor over six months. The RiR visited seven link workers at their GP practices (service 1) and four at their head office (service 2). The RiR met with the link worker managers at their offices, and the RiR spoke with the SP counsellor on the telephone. Data from these conversations were analysed using Thematic Analysis and six codes were constructed to advance our understanding of the components of early implementation of the SP programmes. Training (particularly around mental health), workforce support, location and SP champions within GP practices were found to be key strategies of SP implementation, link worker involvement acting as a conduit for the impacts of these strategies. This paper suggests that the implementation of SP programmes can be improved by addressing each of these areas, alongside allowing link workers the flexibility and authority to respond to challenges as they emerge. Abstract

We present experimental evidence on favouritism practices. Children compete in teams in a tournament. After the first round of a real effort task, children indicate which group member they would prefer to do the task in the second round, for the benefit of the team. Friends are much more likely to be chosen than others after controlling for performance. We also find that children who are favoured by their friend subsequently increase performance. Consequently, favouritism does not hurt efficiency. These results show the importance of observing performance ex post in order to properly evaluate the efficiency implications of favouritism. Abstract

This issue begins with an article about the history of Red Fife Wheat from its origins in Galicia to its importation to Peterborough. Agricultural articles pertain to the dietary benefits of human consumption of rye and timothy grasses, and continued research in soil nutrient depletion. Other articles include the presentation of A. T. Christie's Victorian medals to the Ontario Agricultural College. An alumnus continues his 1931 story as a stowaway. The English column addresses the use of proper English and the origin of the Little Theatre Movement. While the literary column provides book reviews, an account of the Canadian Author Lecture by H. Napier Moore, and the composition of epitaphs. The athletics column reports on the results of the competitions in basketball, swimming, hockey, boxing, wrestling, and fencing. In the Macdonald Institute column a student describes the O. A. C. campus to foreign students, there are also accounts of the "Feminine Section of O. A. Abstract

The Editorial comments on the College's successful school year in championships, College Royal, and the theatre production of H. M. S. Pinafore. Articles in this issue include the anticipation of Lord Tweedsmuir speaking at the Canadian Authors' evening, a student's travel diary of bicycling through Europe, and the annual Ontario Ice Cream Manufacturers conference. An article is devoted to the history of the O. A. C. Co-operative Store initiated by the College students in 1912. For the 15th year, the annual College Royal was an outstanding success. The agricultural article pertains to chemistry and farming. The English column summarizes the proper use of language, and includes a letter from an alumnus living in Sudbury. Campus news reviews several campus personalities, the Year 1940 Dance, candid photographs from across campus, student humour, and escapades of preparing for College Royal. Brothers Chippy and Chap Chapman produce the Sports column and provide the team results for basketball, wrestling, boxing, fencing, swimming, and hockey. Abstract

The highlight of this issue is the Macdonald Institute graduation ceremonies and examination results, including photographs. Research articles include the bacteriology of fat globules, milk testing of dairy cattle, the deterioration of stored butter, and minerals in the human body. This issue also includes profiles of notable visitors to the Ontario Agricultural College, and a continuation of the travel diary of a professor's journey through the West Indies. A few students write of their experiences as representatives of the College, as they travel to conferences and annual meetings, and of the pasture composition in New Zealand. Of interest is the new animal nutrition department and a history of the research conducted on campus since 1905. An overview of the work of the agricultural extension department explains the wealth of information provided to farmers. Reprinted in this issue is the poem, The First Sheaf, along with some farm humour. An article addresses the necessity of plant conservation with the establishment of arboretums. The Macdonald Institute column provides a listing of all of the graduates, a summary of the graduation ceremony, and a photograph of the Daisy Chain. The Macdonald Alumnae column includes a review of the new food laboratory at the Institute, reports on the Homecoming Weekend and the Alumnae Tea, and the Institute's contribution to the Red Cross Nutrition Classes. Abstract

This issue begins with an editorial on compulsory attendance at lectures and the debate it has created amongst students. A student writes of his work with Western Canadian farmers affected by drought, soil reclamation, and the Prairie Farm Rehabilitation Act. Student contributed articles report on student involvement with the Danforth Foundation Fellowship, a tobacco farmer's perspective on tobacco farming, and grain harvesting with a motorized combine. Abstract

This volume includes eight chapters that detail advances in health and disease. Chapter one highlights recent advances in minimally invasive VLNT with a particular focus on VOLT. Chapter two gives a comprehensive review of structure and function of perforin, its association with diverse human diseases including SLE-MAS, and associated CD8+ T and NK cell dysfunction in such conditions. The authors of chapter three use the method of gravitational mass spectroscopy (GMS) to study the state of the genomes of the Influenza A and HSV-1 viruses in the organisms of two probants of the same family. Chapter four provides a comprehensive overview of the recent developments in RSM, emphasizing its utility in drug formulation, design, and optimization. The authors of chapter five explore the detrimental effects of depleted uranium (DU) on the environment, as well as the many radiological and biological implications. Chapter six investigates the mechanism of the second stage of treatment of prostate hyperplasia using non-invasive biophysics and biochemistry (destruction of adenoma). Chapter seven describes a new approach for processing surgically retrieved specimens using differential centrifugation. Chapter eight presents gravitational mass spectroscopy (GMS) data on the molecular structure in the local function of the right hemisphere of the brain (LFP-1) responsible for controlling the prostate.

Abstract

This book aims to provide physicians and scientists with the basics of Artificial Intelligence (AI) with a special focus on medical imaging. The contents of the book provide an introduction to the main topics of artificial intelligence currently applied on medical image analysis. The book starts with a chapter explaining the basic terms used in artificial intelligence for novice readers and embarks on a series of chapters each one of which provides the basics on one AI-related topic. The second chapter presents the programming languages and available automated tools that enable the development of AI applications for medical imaging. The third chapter endeavours to analyse the main traditional machine learning techniques, explaining algorithms such as random forests, support vector machines as well as basic neural networks. The applications of those machines on the analysis of radiomics data is expanded in the fourth chapter to allow the understanding of algorithms used to build classifiers for the diagnosis of disease processes with the use of radiomics. Chapter five provides the basics of natural language processing which has revolutionized the analysis of complex radiological reports and chapter six affords a succinct introduction to convolutional neural networks which have revolutionized medical image analysis enabling automated image-based diagnosis, image enhancement (e.g. denoising), protocolling etc. The penultimate chapter provides an introduction to data preprocessing for use in the aforementioned artificial intelligence applications. The book concludes with a chapter demonstrating AI-based tools already in radiological practice while providing an insight about the foreseeable future. It will be a valuable resource for radiologists, computer scientists and postgraduate students working on medical image analysis.

In Animals as Legal Beings, Maneesha Deckha critically examines how Canadian law and, by extension, other legal orders around the world, participate in the social construction of the human-animal divide and the abject rendering of animals as property. Through a rigorous but cogent analysis, Deckha calls for replacing the exploitative property classification for animals with a new transformative legal status or subjectivity called'beingness.'In developing a new legal subjectivity for animals, one oriented toward respecting animals for who they are rather than their proximity to idealized versions of humanness, Animals as Legal Beings seeks to bring critical animal theorizations and animal law closer together. Throughout, Deckha draws upon the feminist animal care tradition, as well as feminist theories of embodiment and relationality, postcolonial theory, and critical animal studies. Her argument is critical of the liberal legal view of animals and directed at a legal subjectivity for animals attentive to their embodied vulnerability, and desirous of an animal-friendly cultural shift in the core foundations of anthropocentric legal systems. 'Ich habe vor kurzem auf die bedauerliche Thatsache hingewiesen, daß comparativ-pathologisch-anatomische Kenntnisse fast vollständig mangeln, und zwar zum größten Theile deshalb, weil viele Erkrankungen des Nervensystems, denen wir beim Menschen so häufig begegnen, die Thiere vollständig verschonen. Sicher sind solche Erkrankungen auf dem Gebiet des Nervensystems bei Thieren selten, aber vielleicht weniger selten, als wir geneigt sind anzunehmen; es ist eben in dieser Hinsicht zu wenig Fleiß aufgewendet worden. In der letzten Zeit macht sich ein sehr erfreulicher wissenschaftlicher Auf­ schwung in den Veterinärinstituten bemerkbar, und unsere Kenntnisse von den Nervenkrankheiten der Thiere und deren anatomischem Substrat haben dadurch angefangen - aber eben nur angefangen - sich in erwünschter Weise zu er­ weitern. Wenn wir aber auch die neuesten Lehrbücher über die Pathologie der Hausthiere durchmustern, so ist die Ausbeute an pathologisch-anatomischem :Material bezüglich der Nervenkrankheiten immer noch eine sehr unbefriedigende, namentlich dann, wenn wir etwa gar eine feinere mikroskopülChe Untersuchung erwarten.'Diese Zeilen, die OBERSTEINER in einer Übersichtsarbeit zur Methodologie in der neurologischen Forschung im 2. Heft der'Arbeiten aus dem Neurologischen Institut Wien'schrieb, haben leider auch heute noch Geltung. Es brauchte mehr als 50 Jahre, bis diese Lücke ausgefüllt wurde. Sie wurde es nun durch die prächtige Monographie, welche uns heute die Professoren FRAUCHIGER und FANKHAusER vorlegen. Für diesen Rückstand im Ausbau unserer pathologisch-anatomischen und klinisehen Kenntnisse der tierischen Nervenkrankheiten gibt es übrigens zahlreiche Gründe und Entschuldigungen.
