Background: Several clinical and individual factors may play a role in the survival rate of dental restorations, such as characteristics related to the child's age and oral hygiene, and factors associated with the tooth, such as the type of material and number of surfaces to be restored., Aim: To analyse the survival rate of adhesive restorations on primary teeth and factors associated with restoration survival., Design: The study included dental records of children aged 3-12 years having received adhesive restorations on primary teeth at a Brazilian dental school between 2009 and 2019. A Kaplan-Meier survival curve was used to plot survival rates using the log-rank test. A multivariate Cox regression model was run to identify individual and dental factors associated with restoration failure., Results: The sample comprised 269 restored teeth in 111 children. Survival curves were similar for all materials (p = .20) and types of isolation (p = .05). The annual failure rate was 3.60% for glass ionomer cement, 1.23% for resin-modified glass ionomer cement and 0.40% for composite resin. The following variables were associated with more failures: Class II restoration compared with Class I (HR = 1.96; 95%CI: 1.28-2.99, p < .001), proportion of decayed teeth (HR = 11.89; 95%CI: 2.80-50.57, p < .001) and child's age (HR = 1.17; 95%CI: 1.06-1.29, p < .001)., Conclusion: The different materials and types of isolation had similar survival rates. Objective: To analyze the prevalence of hypomineralized second primary molar (HSPM) and its association with socioeconomic characteristics and dental caries in a Brazilian population of preschoolers., Material and Methods: 603 preschoolers, enrolled in public preschools in Itajaí (state of Santa Catarina, Brazil), took part in the study. To assess the participants' socio-economic characteristics, an original questionnaire was formulated and sent to the children's parents. The clinical evaluation was carried out by a calibrated examiner using the deft/DMFT index for dental caries and Ghanim et al. (Ghanim et al., Eur Arch Paediatr Dent, 2015) criteria for HSPM. The data were analyzed through Poisson regression, using STATA statistical software, and the association analyses were presented by prevalence ratios (PR)., Results: The prevalence of at least one HSPM-affected second molar was 24.5%. The prevalence of HSPM was associated to the city's geographical regions of the Educational hubs (p < 0.001). A significant association was found between dental caries and HSPM (p = 0.003; PR: 1.31; 95% CI 1.09-1.56). Children with HSPM were 31% more likely to experience dental caries than children without HSPM. Geographical regions of educational hubs were also significantly associated with HSPM (p < 0.001). None of the socioeconomic characteristics was associated with HSPM (p > 0.05)., Conclusion: HSPM is a common developmental defect of enamel in children in our study. The HSPM distribution was associated with the city's geographic regions. Children with HSPM are more likely to experience dental caries. Antimicrobial photodynamic therapy (a-PDT) is a modality that aims to induce microorganisms through visible light, a photosensitizer, and molecular oxygen. This therapy has shown promising results in controlling cariogenic biofilm in vitro and in vivo counterparts. This study investigated bacterial viability and morphological characterization of Streptococcus mutans mature biofilms after combination of erythrosine and a high potency dental curing light. Biofilms were formed on saliva-coated hydroxyapatite disks in batch culture. The samples were performed in triplicates. Fresh medium was replaced daily for five days and treated using 40 μM of E activated by HL 288 J/cm 2 and total dose of 226 J at 1200 mW/cm 2 . Phosphate buffer saline and 0.12% of chlorhexidine were used as negative and positive control, respectively. After treatment, biofilms were assessed for microbial viability and morphological characterization by means of bio-volume and thickness. COMSTAT software was used for image analysis. Data were analyzed using two-way ANOVA followed by Tukey test with significance level 5%. The application of a-PDT and CHX treatments decreased S. mutans bacterial viability. The image analysis showed more red cells on biofilms when compared to other groups, demonstrating photobacterial killing. Erythrosine irradiated with a high potency curing light can potentially act as an antimicrobial tool in the treatment of cariogenic biofilms. The morphology and viability of microorganisms were impacted after treatment. Treatment with photodynamic therapy may be able to reduce the bio-volume and viability of bacteria present in biofilms. CLINICAL RELEVANCE AND RESEARCH HIGHLIGHTS: The use of the a-PDT technique has been applied in dentistry with satisfactory results. Some applications of this technique are in stomatology and endodontics. Materials and methods: The sample consisted of 48 metallic orthodontic brackets divided randomly into two groups: CL (n=24) and SL brackets (n=24). S. mutans biofilms were grown over the bracket surface (96 h) and treated with CHX (positive control) or 0.9% phosphate-buffered saline (PBS) (negative control) for 1 min each. Quantitative analysis was assessed by colony-forming units, and fluorescence microscopy was performed aiming to illustrate the outcomes. The tests were done in triplicate at three different times (n=9). Data were analyzed using ANOVA and Tukey test (P0.05). In conclusion, a lower colonization was achieved in SL brackets and S. mutans biofilms were susceptible to CHX treatment to both studied brackets. Dental caries is a multifactorial, non-communicable disease. Effective treatment options for minimally invasive removal of carious tissue include Papacarie Duo® gel and antimicrobial photodynamic therapy (aPDT). aPDT involves a combination of a light source and photosensitizer. Given that Papacarie Duo® contains a percentage of blue dye, this study aims to explore the antimicrobial potential of Papacarie Duo® when associated with a light source against Streptococcus mutans strains. The chosen light source was a low-power diode laser (λ = 660 nm, E = 3 J, P = 100 mW, t = 30 s). To assess antimicrobial capacity, planktonic suspensions of Streptococcus mutans were plated on Brain Heart Infusion Agar (BHI) to observe the formation of inhibition halos. The studied groups included methylene blue (0.005%), Papacarie Duo®, distilled water (negative control), 2% chlorhexidine (positive control), Papacarie Duo® + laser, and methylene blue (0.005%) + laser. Following distribution onto plates, each group was incubated at 37 °C for 48 h under microaerophilic conditions. Inhibition halos were subsequently measured using a digital caliper. The results showed that chlorhexidine had the greatest antimicrobial effect followed by the group of irradiated methylene blue and irradiated Papacarie Duo®. All experimental groups demonstrated antimicrobial potential, excluding the negative control group. Tooth hypersensitivity is a common symptom in molar incisor hypomineralisation (MIH) patients and can affect children's quality of life. During daily routine, children with MIH often report sensitivity to various thermal and mechanical stimuli, and difficulty in achieving effective analgesia is a common issue becoming a challenge for dentists. Research has focused on the possible pathophysiological mechanisms behind this phenomenon, which, in turn, have not been determined. Even with the apparently intact enamel of MIH-teeth, the porosity of the hypomineralised enamel acts as an open door for the invasion of oral microorganisms, which reach the dentinal tubules, and stimulate subclinical inflammatory reactions in the pulp. Tissue inflammation may, in turn, lead to morphological and cytochemical changes within sensory neurons, resulting in sensitization of these nerve fibers. This phenomenon is complex, and the treatment modalities focus on inflammation management followed by tubule obliteration by using different materials and technologies. In conclusion, this chapter reviews the concept and etiology of hypersensitivity in teeth with MIH and summarizes the clinical management according to the best evidence available., (© 2024 S. Oral health-related quality of life (OHRQoL) is a multifaceted concept that surpasses an exclusively clinical perception and includes functional, social, emotional, and environmental issues. The measure of OHRQoL represents a holistic approach for research and clinical practice. Negative impacts of oral conditions on OHRQoL in childhood can reflect on health development, especially in a life stage marked by social and cognitive maturation. Therefore, such problems can impact negatively on the daily lives of the individuals and their families. Individuals with molar incisor hypomineralisation (MIH) experience more frequent posteruptive breakdown, an elevated risk of tooth decay, filling failures, the need for recurrent dental treatment, and a higher prevalence of dental hypersensitivity. Children with severe MIH may struggle with everyday activities, such as brushing their teeth, speaking, smiling, chewing, and consuming hot or cold foods. MIH-affected incisors may exhibit opacities that can impact the aesthetics of their smiles. This condition may discourage children from smiling and can indirectly affect their parents as well. The management modalities are focused on solving functional, aesthetic, and hypersensitivity problems and to evaluate OHRQoL values before and after therapies. Background: Developmental defects of enamel (DDE) are a result of disturbances during formation and maturation of the enamel. Evaluating the most-cited DDE papers can provide important tools that point to the gaps and strengths of this important topic in dentistry., Summary: This bibliometric study analyzed the 100 most-cited papers on DDE. Using a combined keyword search strategy, the 100 most-cited papers were selected in the Web of Science Core Collection. Papers that addressed any type of DDE were included. The extracted data were title, number of citations, study theme, authorship, journal, type of DDE, type of dentition (primary or permanent), type of diagnosis, study design, year, and country of publication. The bibliometric networks were generated through VOSviewer software. The 100 papers had a range from 78 to 459 citations. The main themes of studies were etiopathogenesis (53%), prevalence and incidence (22%), and diagnosis (8%). The authors with the highest number of citations were Goodman AH and Rose JC (459 citations). Most articles were published in dental journals (47%). The most studied types of DDE were fluorosis and amelogenesis imperfecta in the permanent dentition (47%). Observational (24%) and non-systematic reviews (24%) were the most common study designs and ranged from 1977 to 2019. The country with the highest number of publications was the USA (41%)., Key Messages: Most of the top 100 DDE papers were about fluorosis and amelogenesis imperfecta, with top papers from three continents with English as the native language. The aim of the present study was to investigate the frequency of missing data on routine dental care appointments and restorative procedures from the clinical records of children treated at a pediatric dental clinic. A descriptive retrospective study was conducted involving the clinical records of children three to 12 years of age treated only with restorations. The inclusion criteria were clinical records from the past 10 years of children with at least one restored tooth. Data collection was performed by a trained examiner who extracted information from the clinical records on appointments for routine dental care and restorative procedures. The frequency of missing data on clinical records was submitted to descriptive analysis. Among the 249 clinical records analyzed, boys accounted for little more than half (54.2%) and mean patient age was 6.9 ± 1.8 years. Ninety-four of the 249 clinical records were of appointments for routine dental care. Missing data were found for the gingival bleeding index (18.1%), visible plaque index (22.3%) and dietary logs (74.5%). Forty-seven children were submitted to a total of 618 restorative procedures. Information was missing on the type of restorative material (5%), brand of the material used (65.2%), the type of isolation (50.8%) and whether pulp capping was performed (75.9%). This bibliometric study analyzed the 100 most-cited papers about the use of lasers and their modalities in dentistry. A search strategy was created using specific keywords related to the topic. A comprehensive search was then conducted in the Web of Science Core Collection (WoS-CC) database up to July 2021. Papers that addressed the application of any type of laser and its modalities in dentistry were included. Each paper was cross-matched with the number of citations on Scopus and Google Scholar. The following data were extracted from papers: title, number of citations, authorship, country, year of publication, journal, study design, subject, laser type, and oral health outcomes. The VOSviewer software was used to generate bibliometric networks. The total number of citations ranged from 120 to 4,124 and 23 papers received more than 200 citations. Papers were published from 1964 to 2015. Most papers were from Europe (42%) and Anglo-Saxon America (27%). The USA was the country with more top 100 papers (25%). Papers were published mainly in Lasers in Surgery and Medicine (15%) and Lasers in Medical Science (7%). VOSviewer maps demonstrated the existence of national and international research collaborations among institutions and authors. Most studies had a laboratory design (57%) and were about restorative dentistry (32%) and periodontics (21%). This bibliometric study of the top 100 most-cited papers on lasers in dentistry allowed a quantitative and qualitative analysis of this very promising research field, revealing a net of collaboration and the importance of this topic in dentistry.

Molar-incisor hypomineralization (MIH) is a qualitative defect of the dental enamel on permanent first molars and may be associated with permanent incisors. There is currently no defined therapeutic protocol for managing the hypersensitivity associated with this condition. The present case report describes the treatment of a patient with MIH and the impact on the patient's quality of life as assessed using the Child Perceptions Questionnaire for children aged 11 to 14 years (CPQ 11-14 ). The maxillary central incisors, maxillary and mandibular first molars, and mandibular right canine of an 11-year-old girl were compromised by MIH. The long version of the CPQ 11-14 was self-administered before treatment. Initially, the patient reported hypersensitivity of her maxillary right central incisor, maxillary and mandibular left first molars, and mandibular right canine when consuming cold food and beverages. This was confirmed by applying a jet of air to the teeth and asking the patient to report the level of sensitivity using the Wong-Baker FACES Pain Rating Scale. The patient reported that hypersensitivity was present every day or almost every day. Desensitizing treatment included 4 applications of photobiomodulation therapy (infrared diode laser) and 4 applications of fluoride varnish. After desensitization, the maxillary central incisors received direct composite resin restorations, while the maxillary left first molar, which had an amalgam restoration and recurrent caries, received a resin-modified glass ionomer cement restoration. At the conclusion of the treatment, the patient completed the CPQ 11-14 again and reported less difficulty with consuming hot and cold foods and beverages, indicating that treatment had reduced hypersensitivity and its negative impact on her quality of life., Competing Interests: The authors report no conflict of interest pertaining to any of the products or companies discussed in this article.

Introduction: Due to the anatomical constraints of the mandible, mandibular dental arch usually serves as a guideline to determine the required changes in the maxillary transverse dimension. The Schwarz appliance and the Lip Bumper are the traditional orthodontic appliances for mandibular arch expansion in patients with borderline amounts of crowding, and/or transverse discrepancy. However, they often require patient cooperation, which may be a concern for orthodontists in daily practice., Objectives: This article illustrates a simple fixed orthodontic device as an alternative to achieve mandibular arch expansion in patients with moderate tooth-size/arch-length discrepancy. The four reported cases refer to 8 to 10-year-old patients in the mixed dentition, with an Angle Class I or Class II malocclusion, transverse deficiency in both arches, moderate crowding and/or posterior crossbite, combined with compromised smile aesthetics. The patients were treated with rapid maxillary expansion (RME) using Hass expander appliance and the modified Arnold expander (MAE)., Conclusion: This low-cost compliance-free orthodontic appliance provided dentoalveolar decompensation by means of uprighting the posterior teeth, with minimal or no adjustments during treatment. The final results were achieved in only three to four months, and fulfilled all treatment objectives, such as an increase in the arch perimeter and width, and a better teeth alignment.

Aim: To test a chemical-mechanical method and antimicrobial photodynamic therapy (a-PDT) in combination to manage a deep dental carious lesion in a permanent molar pediatric patient., Background: Due to specific characteristics of early-erupted teeth, as first permanent lower molars, techniques that provide less invasive tissue removal aiming keep healthy tooth parts, such as pulp tissue, are desired. Furthermore, especially to children, provide comfort, less noise and vibration are important issues that should be considered on the management of deep caries., Case Description: The following case report presents an association of Papacarie Duo® (PD) and a-PDT used in a 9-year-old patient with a deep dental caries in the mandibular right first molar. After PD application, the carious tissue was carefully removed with the cavity stained for 1 min with rose bengal solution, irradiated with a high intensity LED constituting the a-PDT technique, and definitively restored with composite resin. At 6 months of follow-up, no signs of caries were verified, showing success to applied techniques. A microbiological evaluation attested the antimicrobial effects of the alternative procedures., Conclusion: Although it reached optimal clinical and bacterial reduction outcomes, the approach demonstrated an excessive length of time taken for the management of carious lesion., Clinical Significance: The use of new technologies should be encouraged, especially when treating deep caries aiming for a less invasive approach., (Copyright © 2020 Elsevier B.V. Antimicrobial photodynamic therapy (aPDT) has emerged as a promising antimicrobial treatment to control microorganisms including those involved in oral diseases, especially dental caries. Hence, the aim of this study was to evaluate the influence of aPDT - pre-irradiation time (PIT), at different periods, on antimicrobial rate of Streptococcus mutans (S. mutans). A standard suspension of S. mutans UA159 was prepared and submitted at sensitization of 0.005 % methylene blue (MB) for 0, 1, 3 and 5 min (G1 - G4 groups, respectively) and irradiated with a red laser (660 nm; 321 J/cm 2 ; 9 J; 90 s) afterward. A control group using PBS instead of MB was performed as well (G5). The number of colony-forming units (CFU)/mL was recorded, transformed into log 10 and analyzed by ANOVA and Tukey's test at a cutoff value at 0.05. Overall, the aPDT groups tested achieved a bacterial reduction > 1-log 10 when compared to G5 (p < 0.05) with no statistical difference among the different PIT tested. The need of PIT before aPDT application deserves attention, since its time reduction implies on shorter clinical approaches without compromising the photodynamic antibacterial efficacy in the in vitro parameters employed., (Copyright © 2020 Elsevier B.V. Objective: The present study investigated the erosive potential of children's mouthrinses on glass ionomer cement (GIC) samples after simulated toothbrushing., Study Design: Forty round-shaped samples of GIC were divided into 3 groups: G1- cetylpyridinium chloride, G2- xylitol and triclosan and G3-Malva sylvestris and xylitol and G4-distilled water as a control group. Prior to the main tests, the samples were submitted to the surface roughness measurement (Ra) and weight analysis (W). Afterward, they were brushed twice day (2× / day) for 15 days and immersed in mouthrinses after the last daily brushing. The final surface roughness (R2) and weight (W2) were determined after completing the tooth brushing-mouth rinsing cycles and the real increase in roughness (ΔRa) and real weight loss (ΔW) were calculated. In addition, stereoscopic images taken at 30× magnification. The data was analyzed by one-way ANOVA and Tukey-test post hoc tests for intergroup comparison and the T-test for dependent samples (α = 0.05)., Results: Only group G2 showed increased in roughness ΔRa (1.53 ± 0.94) whereas ΔW values were not significant. However, evident cracks and voids were verified for all tested children's rinses., Conclusion: Thus, children's mouthrinse containing xylitol / triclosan increased the GIC roughness, especially when associated with brushing.

Background: Molar incisor hypomineralization (MIH) is a change in the formation of dental enamel of systemic origin that affects at least one of the first 4 permanent molars and usually affects incisors. During the eruption, the affected surfaces tend to fracture, exposing the dentin, which causes excessive sensitivity in addition to making the region very susceptible to the appearance of carious lesions. The objective of this research will be to evaluate the clinical effect of antimicrobial photodynamic therapy (aPDT) in permanent teeth with severe and sensitive MIH., Methods: The methodology will be based on the selection of patients from 6 to 12 years of age with permanent molar teeth, randomly divided in 2 groups. The selected teeth should have MIH on the occlusal surface, indicated for clinical restorative treatment. In Group 1, aPDT will be applied for the treatment of infected dentin. Afterward, the teeth will be restored with high viscosity glass ionomer cement. In Group 2, the removal of the softened dentin around the side walls of the cavity with sharp dentine curettes and posterior restoration with high viscosity glass ionomer cement will be performed. All patients will have clinical and radiographic follow-up with a time interval of 6 and 12 months. The data obtained will be submitted to descriptive statistical analysis to evaluate the association of categorical variables. Chi-square test and Fisher exact test will be applied, to analyze the correlation between the continuous variables, Pearson correlation test will be applied. For the analysis of dentin density in the scanned radiographic images and the microbiological results for colony-forming units, ANOVA and Kruskal-Wallis will be applied., Discussion: Often in the presence of severe MIH, the presence of dentin sensitivity is also associated with caries lesion, making it even more necessary to respect the principles of minimal intervention., Trial Registration: NCT03904641.

Considering the challenge to control Candida -associated denture stomatitis, the search for antifungal substances derived from natural sources has become a trend in the literature. In this study the following effects of Chenopodium ambrosioides extract (CAE) were investigated: action against biofilms of Candida albicans , its cytotoxic potential, and changes caused in acrylic resin. The CAE was characterized by High Performance Liquid Chromatography (HPLC). The susceptibility of C. albicans to CAE was investigated by Minimum Inhibitory Concentration and Minimum Fungicidal Concentration (MIC and MFC) tests. Acrylic resin disks were fabricated, and C. albicans biofilms were developed on these for 48 h. Afterward the disks were immersed for 10 min in: PBS (Negative Control); 1% Sodium Hypochlorite (1% SH, Positive Control) or CAE at MIC or 5xMIC. The biofilms were investigated relative to counts and metabolic activity. The cytotoxic potential in keratinocytes and fibroblasts was verified by MTT test. Change in color and roughness of the acrylic resin was analyzed after 28 days of immersion in CAE. The data were analyzed by the ANOVA considering a 5% level of significance. The main compounds detected by HPLC were kaempferol and quercetin. Both MIC and MFC obtained the value of 0.25 mg/mL. The MIC was sufficient to significantly reduce the counts and activity of the biofilm cells ( p < 0.0001), while 5xMIC resulted in almost complete eradication, similar to 1% SH. Keratinocytes and fibroblasts exposed to the MIC and 5xMIC presented cell viability similar to that of the Control Group ( p > 0.05). No important changes in acrylic resin color and roughness were detected, even after 28 days. It could be concluded that the immersion of acrylic resin in C. ambrosioides extract in its minimum inhibitory concentration was effective for the reduction of C. albicans biofilms without any evidence of cytotoxic effects or changes in roughness and color of this substrate.

Objective: The aim of this in vitro study was to evaluate the effectiveness of antimicrobial photodynamic therapy (aPDT) composed of the association of the photosensitizer (PS) erythrosine irradiated by a high-intensity dental light source against a culture of Streptococcus mutans , comparing this effect with that of a 0.12% chlorhexidine solution. Materials and methods: For this purpose, planktonic suspensions of S. mutans were subjected to experimental conditions in which three different concentrations of erythrosine (E) (2, 4, and 8 μM) associated with three different doses emitted by the light source (L) (48, 96, and 144 J/cm 2 ) were crossed, corresponding to the exposure times of 40, 80, and 120 sec, respectively, delivered in pulsed mode. The following experimental conditions were evaluated: G1-treatment with dye and light source (E+L+); G2-treatment with the dye only (E+L-); G3-treatment with the light source only (E-L+); G4-absence of dye and light (negative control); and G5-0.12% chlorhexidine (positive control). After treatment, aliquots of each group were plated on blood agar, then the colony forming units per milliliter (CFU/mL) later counted. The results were subjected to ANOVA and Tukey tests, considering the level of significance of 5%. Results: Group aPDT showed complete eradication of microorganisms as from the concentration of 4 μM irradiated for 40 sec, demonstrating statistically significant difference in comparison with the negative control group ( p  ≤ 0.05) and efficacy similar to that of the 0.12% chlorhexidine group ( p  ≥ 0.05). Conclusions: The authors concluded that the light-polymerizing appliance used in pulsed mode, associated with the PS erythrosine, was efficient for the control of S. mutans in a planktonic suspension in a short period of irradiation time.

Background: The assessment of oral health should consider clinical and biopsychosocial aspects., Aim: To evaluate the impact of untreated dental caries on the quality of life of children., Design: A cross-sectional study was conducted with 647 Brazilian children. The Child Perceptions Questionnaire (CPQ 8-10 ) and the 'oral health' global indicator were administered to the children and used as the outcomes. Parents answered a general questionnaire, and oral clinical examinations were performed. Data analysis involved descriptive statistics, chi-squared test, Mann-Whitney test, and Poisson's regression analysis (α = 5%). Despite their demonstrated biocompatibility and osteogenic properties, oyster shells have been reported as a potential alternative to other commonly used materials for bone substitution. This study evaluated whether an experimental bone substitute (EBS) made from a typical oyster shell of Northeastern Brazil (Crassostrea rhizophora) has effects on bone development using an animal model. Oysters were collected from a biologically assisted vivarium, and their inner layer was used for preparing an EBS. Chemical and surface characterization of EBS was performed using Individually Coupled Plasma Optical Emission Spectrometry (ICP-OES) and Scanning Electron Microscope (SEM), respectively. Seventy-two rats were randomly assigned to groups according to the treatment of bone defects created in the submandibular area: Negative Control (-C), Positive Control (+C; Bio-Oss®) and EBS. Euthanasia occurred at 7, 21, 42 and 56 days postoperatively. The bone pieces were stained with hematoxylin and eosin (H&E). The formation of bone tissue was evaluated histologically and histomorphometrically. Data were analyzed through the Kruskal-Wallis test and ANOVA considering a significant level of 5%. The main element found in EBS was calcium (71.68%), and it presented heterogeneity in the particle size and a porosity aspect at SEM analysis. Histological results revealed the absence of inflammatory cells in all groups, being that EBS presented the most accelerated process of bone formation with a statistically significant difference between this group and the +C and -C groups in the 21-day time-point (p < 0.05). After 21 days, the bone formation process was similar between all groups (p > 0.05), showing an immature lamellar bone pattern after 56 days of experimentation (p > 0.05). Background: The failure of endodontic treatment is linked to the presence of microorganisms, particularly Enterococcus faecalis, in the root canals. This study evaluated the effectiveness of photodynamic therapy (PDT) using erythrosine irradiated by a high-power curing light on a planktonic suspension culture of E. faecalis., Methods: Bacterial suspensions of E. faecalis were adjusted and then mixed in a 1:1 proportion, in triplicate, in treatment groups by varying the length of irradiation time (120 and 240s) and the molarity of the erythrosine (5 and 10μM). In order to verify the post-treatment bactericidal effect, a count of the viable bacteria was performed (CFUmL -1 ) and transformed into Log10 CFU. The one-way ANOVA with Tukey post-hoc test was applied to check for differences between the groups., Results: The bacteria were completely eradicated in the groups that used PDT with 5μM 240s, 10μM 120s and 10μM 240s (p≪0.001). The effect of the PDT 5μM 120s group was significant (p≪0.05) in comparison with the groups using only light or only erythrosine. Positive control (exposure to 2.5% NaClO for 120 and 240s) completely eradicated E. faecalis. The negative control (PBS) did not alter the quantities of E. faecalis CFU with 9.605 Log10 CFU at 120s and 9.621 Log10 CFU at 240s., Conclusion: PDT with erythrosine in a concentration of 10μM and high-power LED is capable of totally eliminating E. faecalis in planktonic suspension., (Copyright © 2017 Elsevier B.V. Introduction: The restorative management of molars with molar incisor hypomineralization (MIH) represents a challenge in the clinical practice with high failure rate., Objective: The aim of this study is to evaluate the clinical survival of direct composite resin restorations in first permanent molars (FPMs) that are affected by MIH, comparing two adhesive systems., Material and Methods: We selected 41 FPMs with MIH from children aged 6-8 years. FPM fully erupted and with restorative treatment needed were the inclusion criteria. We excluded FPMs with destroyed crowns. The FPMs were randomly assigned to two groups: self-etching adhesive (SEA) and total-etch adhesive (TEA). Clinical evaluation was performed by a blinded examiner during 18 months according to the modified US Public Health Service (USPHS) criteria. The actuarial method was used to evaluate survival of the restorations, and Chi-square and Fisher's exact tests were used to compare differences between the groups (α = 5 %)., Results: The cumulative survival rates were 100 % at 1 month, 89 % at 6 months, 73 % at 12 months, and 68 % at 18 months in SEA, and 95 % at 1 month, 72 % at 6 months, 59 % at 12 months, and 54 % at 18 months in TEA; there was no significant difference between groups., Conclusions: There was no difference in clinical survival of restorations in FPMs affected by MIH using TEA or SEA adhesives in the end of 18 months., Clinical Relevance: It was suggested that SEAs as well as TEAs can be applied to restore molars affected by MIH, when it is performed a conservative cavity preparation. Once, cavosurface margins (cavity design) in hypomineralized enamel have less bonding capability.

Background: Restoring first permanent molars affected with molar incisor hypomineralization (MIH) is challenging. Focusing on improving the quality of life for children affected by MIH, at least until the complete eruption of first permanent molars to receive full coverage, to decrease the hypersensitivity and to be able to perform proper oral hygiene measures, the purpose of this study was to compare silver modified atraumatic restorative technique (SMART) versus the conventional restoration and fluoride varnish application on moderate to severe hypomineralized molars. The comparison considered the restoration survival, hypersensitivity, and digital surface area changes after one year follow up. Methods: Twenty-eight children were selected (20 girls and 8 boys) with at least 2 MIH molars with the same defect severity. The study comprised 2 groups; MOD group (moderate hypomineralized molar severity) and SEV group (Severe hypomineralized molar severity) (n = 28 tooth). Each group was further subdivided into 2 subgroups according to the technique of restoration: SMART subgroup and CONV subgroup (high viscosity glass ionomer restoration and fluoride varnish application) (each = 14 tooth). Evaluation was done in terms of the restoration survival (6 months and 12 months), hypersensitivity at 1 weak, 6 months and 12 months and occlusal surface area changes at 12 months). Professional Fluoride varnish application and home prophylaxis using MI paste were the protocol for each child patient. Results: There was no significant difference between the 4 subgroups, regarding tooth restoration integrity at 6-months vs. 12-months. However, a statistically significant difference in tooth restoration integrity between the 4 subgroups at 12-months (P =.049). Also, the hypersensitivity score, there was a statistically significant difference between the 4-time intervals (P <.001) and a statistically significant difference in surface area changes between the 4 subgroups. Conclusions: Selective removal of carious tissue and SMART restoration, combined with dental home and professional preventive measures every 3 months maintained the integrity of restorations in severely and moderately affected permanent molars up to 1 year. Background: Early childhood caries (ECC), also known as dental caries, poses a significant challenge to the oral health and overall well-being of young children. This study aims to investigate the potential relationship between the mode of delivery and the odds of ECC. Methods: We conducted a meticulous systematic review and meta-analysis, gathering studies from reputable databases, including Web of Science, Scopus, PubMed, Cochrane, Google Scholar, and Embase. The selected studies underwent rigorous evaluation for heterogeneity, utilizing statistical tests such as the Chi-square test, I2, meta-regression analysis, sensitivity analysis, and subgroup analysis. We assessed the presence of publication bias using Begg's and Egger's tests, while the quality of the articles was evaluated using the Newcastle-Ottawa Scale checklist. Results: Our analysis included a robust dataset of 24 studies conducted between 1997 and 2024, involving a total of 71,732 participants. The meta-analysis revealed an odds ratio of 1.05 (95% CI: 0.86–1.30; P-value = 0.610) for dental caries in children born via cesarean section. Importantly, we found no evidence of publication bias, as indicated by non-significant results from Begg's test (P-value = 0.568) and Egger's test (P-value = 0.807). Meta-regression analysis did not identify any significant associations between the study variables and the observed heterogeneity (P > 0.10). Sensitivity analysis further confirmed the stability of the study results, with no significant changes detected. Conclusion: In conclusion, our comprehensive meta-analysis provides compelling evidence that there is no significant association between the mode of delivery and the odds of ECC. Background: Treatment of Molar-incisor hypomineralisation (MIH) poses significant challenges for pediatric dentists due to its varied clinical manifestations and treatment needs. Understanding and evaluating different treatment options can improve patient outcomes. This study aimed to analyze available evidence on treatment options for restoring MIH-affected young permanent teeth. Methods: This scoping review followed the Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. A systematic literature search was conducted using Scopus, PubMed, and Science Direct databases, covering publications from 2014 to 2024. The search focused on therapies for young permanent teeth with MIH in children, employing predefined keywords and the Population, Concept, and Context framework. Results: A total of 20 studies were included from Turkiye, Brazil, Syria, Germany, Egypt, and India. Thirteen articles examined first molars, five focused on incisors, and two covered both. The population studied ranged from 6 to 18 years old, involving up to 281 children and between 30 to 326 teeth. The study provides insights into various management and treatment approaches for MIH-affected teeth, along with the effectiveness and long-term stability of different methods and materials. Conclusion: Materials such as resin infiltration, SDF, HVGI, full metal crowns, SSC, lithium disilicate, zirconia crowns, and CAD/CAM ceramic restorations offer greater longevity and require less retreatment in managing MIH-affected teeth. The rise of phytotherapy has enabled the utilization of various plant species for medicinal purposes, such as Cymbopogon citratus (C. citratus), providing solutions for oral pathologies, such as oral candidiasis. The PubMed, Web of Science, Scopus, and SciELO databases were searched. In vivo and in vitro studies on the action of C. citratus against oral candidiasis were included, and ROBINS-I was used to determine study quality and risk of bias. The search yielded 1922 articles, of which 10 met the inclusion criteria. Limited scientific evidence exists regarding the use of C. citratus for oral candidiasis. However, studies have indicated its potent antifungal effects. Further studies, preferably clinical trials, are necessary to confirm this effect and to enable its clinical use as a therapeutic option. [ABSTRACT FROM AUTHOR]

Purpose: This review aimed to highlight the aetiology and risk factors of food impaction along with the treatment in each case. Materials and Methods: A search was conducted in PubMed from 1947 to March 28, 2023. The search terms utilised included (food impaction) OR (interdental impaction). No filter was applied. Articles related to the classification, aetiology, treatment, and associated factors of food impaction in dentistry and published in English or with an abstract in English were selected. Results: A total of 72 articles were included in the review, which revealed the variety and complexity of aetiological factors and treatment of food impaction in dentistry, as well as the heterogeneity of previous studies. Based on the aetiology, different treatment plans and management should be considered. Conclusion: This review indicated the need to identify the pathology of food impaction before treatment. Considering the causal factors of food impaction -- including proximal contact loss, occlusal disharmony, morphological deformity, positional abnormality, and interdental papillae loss -- different management approaches such as restoration, occlusal adjustment, orthodontic, nonsurgical or surgical treatment could be applied. Previous findings indicated that the laser photobiomodulation is more effective than the control or placebo in preserving the alveolar socket. This study aimed to compare two different lasers regarding their effectiveness in aiding alveolar socket preservation. Twenty extraction sockets were selected then divided into two equal groups. Group A was exposed to 650 nm Diode laser, and Group B to 810 nm Diode laser following the same protocol and parameters after a standard alveolar socket preservation procedure with collagen plug. Radiographic analysis with cone beam computed tomography was done to compare the alveolar bone surface area immediately after extraction and three months post-operatively, while bone samples collected before implant drilling were histologically examined for newly formed bone evaluation and histomorphometric analysis in terms of percentage of new bone surface area, percentage of unmineralized bone and finally, immunohistochemical analysis of Osteocalcin reaction surface area as well as optical density. Radiographically, infrared (810 nm) Diode effect on alveolar bone surface area has significantly exceeded the red laser, while histologically, red (650 nm) Diode has demonstrated statistical significance regarding all parameters; newly formed bone surface area percentage, unmineralized bone area percentage and finally Osteocalcin bone marker reaction surface area percentage and optical density. Under the specified conditions and laser parameters, photobiomodulation using the 810 nm Diode got the upper hand radiographically, yet histologically, the red 650 nm Diode managed to dominate all histological parameters when both employed as an adjunct to alveolar socket preservation procedures. The purpose of the meta‐analysis was to evaluate and compare the photodynamic therapy's effectiveness in treating infected skin wounds. The results of this meta‐analysis were analysed, and the odds ratio (OR) and mean difference (MD) with 95% confidence intervals (CIs) were calculated using dichotomous or contentious random‐ or fixed‐effect models. For the current meta‐analysis, 6 examinations spanning from 2013 to 2021 were included, encompassing 154 patients with infected skin wounds were the used studies' starting point. Photodynamic therapy had a significantly lower wound ulcer size (MD, −4.42; 95% CI, −7.56–−1.28, p = 0.006), better tissue repair (MD, −8.62; 95% CI, −16.76–−0.48, p = 0.04) and lower microbial cell viability (OR, 0.13; 95% CI, 0.04–0.42, p < 0.001) compared with red light exposure in subjects with infected skin wounds. The examined data revealed that photodynamic therapy had a significantly lower wound ulcer size, better tissue repair and lower microbial cell viability compared with red light exposure in subjects with infected skin wounds. Purpose: To develop and assess lemongrass-incorporated tissue conditioners (LG-TCs) with a potent and long-lasting inhibitory effect against Candida albicans cultures to control the accumulation of fungi.Materials and Methods: LG essential oil with concentrations of 7.17%, 3.56%, 1.77%, and 0.89% (w/w) or nystatin were mixed with the liquid part of the TC before being added to the powder part to form 486 TC samples of 6-mm diameter x 2-mm thickness (n = 81 samples for each group of LG-/nystatin-incorporated or unmodified TCs). After being immersed in 37°C water for 1, 2, 3, 5, 7, 9, 12, or 14 days, these TC samples were removed, blotted with sterile filter paper, and then exposed to fungal suspension (1 × 105 CFU/mL). The TC samples were evaluated for their capacity to inhibit fungal growth by 99.9%.Results: The anti-Candida effect of the unmodified TCs was reduced significantly after the samples were immersed in water. Interestingly, a long-lasting anti-Candida effect was observed in the TCs incorporated with LG essential oil. After being immersed in water for at least 14 days, the TCs with 1.77% LG oil were still able to inhibit fungal growth substantially. In contrast, a shorter-lasting (5 days) anti-Candida effect was found in the TCs with 0.89% (w/w) LG oil. Additionally, the TCs incorporated with LG oil at concentrations of 3.56% (w/w) or more inhibited the growth of the fungus by 99.99%, and its anti-Candida effect lasted for 14 days.Conclusion: LG-TCs showed an impressive and long-lasting inhibitory effect against C. albicans. Teeth affected by molar incisor hypomineralization (MIH) present micromorphologic changes and hypersensitivity, which increase the risk of developing caries lesions and affect bonding procedures. Considering that practitioners still misdiagnose teeth affected by MIH, there is an urge for more knowledge about this topic in order to propose a more adequate and conservative treatment. The purpose of this study was to report the clinical challenges regarding the restorative management of a patient with MIH. A 13-year-old girl sought dental treatment, complaining about hypersensitivity in posterior teeth. Under clinical exams, notable enamel alterations mainly affecting posterior teeth (including molars) presented particular characteristics frequently attributed to MIH, and visible stains on maxillary central incisors were present. Because the mineral and organic content of MIH-affected enamel differ from sound enamel, it may imply special care for bonding of a restorative material. Thus, in order to promote a minimally invasive approach, selective removal of carious tissue and restoration with microhybrid composite resin was conducted after application of a universal bonding system used as self-etching strategy. After a 2-year followup, the restorations presented good clinical performance and the patient had limited hypersensitivity discomfort, suggesting a promising performance. [ABSTRACT FROM AUTHOR]

Objective: To analyze the effects of the time span between archwire changes on the amount of transverse dental arch expansion. The design was a prospective, controlled clinical trial. Randomization was performed by computer-generated allocation tables., Methods: In all, 35 patients were evaluated in three groups treated with fixed appliances and the same four sequential leveling archwires, however, with different replacement periods: 28 days (4-week [4W] group), 56 days (8-week [8W] group), and 84 days (12-week [12W] group). Digital models were measured before the treatment (T0) and at the end of the evaluation period (T1) by a blinded operator with OrthoAnalyzer® software (3-Shape, Copenhagen, Denmark) and compared using MANOVA. A multiple linear regression was also used to evaluate the influences of age, initial dental crowding, incisor inclination, and facial pattern on the amount of expansion., Results: Mean expansion was 2.77 ± 0.84 mm (upper arch) and 3.12 ± 0.88 mm (lower arch) in the 12W group; 1.96 ± 0.82 mm and 2.27 ± 0.96 mm in the 8W group, and 1.11 ± 0.93 mm and 1.32 ± 0.91 mm in the 4W group, respectively for the upper and lower arches. The amount of expansion varied significantly between the groups in the area of the canines, first premolars, and second premolars for both arches. Expansion was less in older patients. Initial dental crowding, incisor inclination and facial pattern had no influence on dental expansion., Conclusion: Longer time spans between archwire changes provided greater amounts of expansion, while age is inversely related, with younger patients expressing greater amounts of expansion. The aim of this study was to assess the impact of molar-incisor hypomineralisation (MIH) on oral health-related quality of life (OHRQoL) in children and adolescents, including information on restorative care, tooth sensitivity, as well as sociodemographic factors. Thirty-five patients aged between 7 and 17 years underwent a comprehensive oral examination. Severity of MIH was graded using the MIH Treatment Need Index (MIH-TNI), OHRQoL using the Child Oral Health Impact Profile (COHIP-19). Clinical quality of restorations was assessed according to modified FDI-criteria, tooth sensitivity using the Schiff Cold Air Sensitivity Scale (SCASS). The mean age was 11.3 ± 3.0 years, 34% were female. On average, 6.9 ± 2.8 teeth were affected, 62,9% had hypersensitive teeth (SCASS ≥ 1). Eighty-nine percent of patients had received restorative care, with a mean of 3.3 ± 2.1 teeth restored, most often with composite, followed by fissure sealing. Nine percent of restorations failed by the FDI-criteria. Mean estimated survival times for success were 4.9 years (95% CI 3.5; 6.2) and 5.6 years (95% CI 5.0; 6.3) for fissure sealants and composite restorations, respectively. The mean COHIP-19 score was 64.3 ± 8.2 (max. possible score = 76). A higher severity of MIH-TNI correlated significantly with impaired OHRQoL (rs = − 0.38, p = 0.013). However, this was not mirrored in multiple regression analysis. Despite the high rate of restorative treatment with an acceptable failure rate, OHRQoL is reduced in children with MIH. Background: Sickle cell anemia (SCA) is the most common hemoglobinopathy in Brazil and worldwide and is part of a group of chronic genetic diseases resulting from abnormalities in the structure of hemoglobin., Aim: To evaluate the impact of oral health conditions on the quality of life (QoL) of children and adolescents with SCA., Design: This is a cross-sectional study with a sample of 76 children and adolescents aged 8-14 years. For inclusion, they were required to have a diagnosis of HbSS SCA in their medical records, without a pain crisis or any dental emergency in the last three months. The children and adolescents with SCA were from Hematology and Hemotherapy Center of Maranhão. Demographic characteristics, socioeconomic status, oral hygiene, caries, malocclusion, and oral health-related quality of life (OHRQoL) were assessed. OHRQoL was assessed using the Child Perceptions Questionnaire. Descriptive statistics, Student's t and Mann-Whitney tests were performed (α = 5%)., Results: Brown race was the most prevalent for both age groups (8-10 years-63.2% and 11-14 years-57.9%). Predominant monthly family income for both age groups was below $106. Visible plaque and gingival bleeding were higher in children aged 8-10 years. Dental caries significantly impacted the QoL of adolescents through the domain "oral symptom" (p = .031). Objectives: The current research on single-nucleotide polymorphism (SNP) mutation sites at different positions of the FAM83H gene and their phenotypic changes leading to amelogenesis imperfecta (AI) is inconsistent. We identified a previously reported heterozygous nonsense mutation c.1192C>T (p.Q398*) in the FAM83H gene and conducted a comprehensive analysis of the dental ultrastructure and chemical composition changes induced by this mutation. Additionally, we predicted the protein feature affected by this mutation site. The aim was to further deepen our understanding of the diversity of AI caused by different mutation sites in the FAM83H gene. Methods: Whole-exome sequencing (WES) and Sanger sequencing were used to confirm the mutation sites. Physical features of the patient's teeth were investigated using various methods including cone beam computer tomography (CBCT), scanning electron microscopy (SEM), contact profilometry (roughness measurement), and a nanomechanical tester (nanoindentation measurement). The protein features of wild-type and mutant FAM83H were predicted using bioinformatics methods. Results: One previously discovered FAM83H heterozygous nonsense mutation c.1192C>T (p.Q398*) was detected in the patient. SEM revealed inconsistent dentinal tubules, and EDS showed that calcium and phosphorus were lower in the patient's dentin but higher in the enamel compared to the control tooth. Roughness measurements showed that AI patients' teeth had rougher occlusal surfaces than those of the control tooth. Nanoindentation measurements showed that the enamel and dentin hardness values of the AI patients' teeth were both significantly reduced compared to those of the control tooth. Compared to the wild-type FAM83H protein, the mutant FAM83H protein shows alterations in stability, hydrophobicity, secondary structure, and tertiary structure. These changes could underlie functional differences and AI phenotype variations caused by this mutation site. Conclusions: This study expands the understanding of the effects of FAM83H mutations on tooth structure. Clinical relevance: Our study enhances our understanding of the genetic basis of AI and may contribute to improved diagnostics and personalized treatment strategies for patients with FAM83H-related AI. Objective: This systematic review aimed to answer whether, in (P) individuals undergoing orthognathic surgery, (I) low-level laser therapy (LLLT) during or shortly after the surgical procedure compared to (C) pharmacologic therapy alone or no therapy for improvement of post-intervention complications relieves (O) postoperative inflammatory events in (S) randomized clinical trials. Material and methods: A preliminary electronic search was conducted for references with language following a Latin (Roman) alphabet in seven databases index, and gray literature without restriction on language or publication period. Risk of bias was performed by RoB 2.0 tool, and meta-analysis used mean differences (MD) for edema and mouth opening and standardized mean differences (SMD) for pain scores (p < 0.05, Revman®). Results: A total of 91 control patients and 114 LLLT patients were included. The wavelengths ranged from 660 to 940 nm, and the applied energy density was between 5 and 100 J/cm2 at mostly extraoral distributed points. LLLT significantly reduced edema (MD = − 4.27, CI95% − 5.13 to − 3.41 mm) in the period from 5 days to 2 weeks postoperatively (p < 0.001) and showed strongly reduced pain scores (SMD = − 1.37, I95% = − 1.99 to − 0.75) between 1 day and 5 weeks (p < 0.001), without significantly interfering with mouth opening (p = 0.110). Despite the low risk of bias, high heterogeneity among studies (I2 > 70%) and small sample sizes made the certainty of GRADE evidence low. Conclusion: This SR demonstrated that LLLT effectively controls pain and edema after orthognathic surgery.
