Kang Shuai Lao Pian (KSLP) is a traditional Chinese medicine (TCM) preparation used to delay aging. However, due to the lack of research on the chemical composition and pharmacokinetic behavior of KSLP, its material basis and in vivo components with high exposure remain ambiguous. The UPLC/Q-Orbitrap-MS/MS was performed to identify the preliminary chemical profile of KSLP. A total of 138 compounds, including ginsenosides, phenylethanol glycosides, iridoids, alkaloids, ionones and others, were identified in accordance with their retention times, accurate masses and characteristic MS/MS fragment patterns. Moreover, considering the active components and characteristic components of KSLP, the extraction process of KSLP was optimized, and the quantitative analysis by UPLC/QQQ-MS/MS of 13 compounds in KSLP was established. The method was stable and sensitive, and could be used for the quality control of KSLP. Then, the pharmacokinetic study was carried out by further refining the components of KSLP. Besides, quantitative method for 6 compounds in rat plasma was validated and developed by UPLC/QQQ-MS/MS. The established approach was successfully applied to characterize the pharmacokinetic features of components in KSLP and it was found that the absorption and elimination of ginsenosides in KSLP was slow. Altogether, this study laid a solid foundation and provided theoretical guidance for further clarification of bioactive components of KSLP.

Objective: The perspective of real-world study is especially relevant to newborns, enabling dosage regimen optimization and regulatory approval of medications for use in newborns. The aim of the present study was to conduct a pharmacokinetic analysis of cefotaxime and evaluate the dosage used in newborns with early-onset sepsis (EOS) using real-world data in order to support the rational use in the clinical practice. Methods: This prospective, open-label study was performed in newborns with EOS. A developmental pharmacokinetic-pharmacodynamic model of cefotaxime in EOS patients was established based on an opportunistic sampling method. Then, clinical evaluation of cefotaxime was conducted in newborns with EOS using real-world data. Results: A one-compartment model with first-order elimination was developed, using 101 cefotaxime concentrations derived from 51 neonates (30.1-41.3°C weeks postmenstrual age), combining current weight and postnatal age. The pharmacokinetic-pharmacodynamic target was defined as the free cefotaxime concentration above MIC during 70% of the dosing interval (70% fT > MIC), and 100% of neonates receiving the dose of 50 mg/kg, BID attained the target evaluated using the model. Additionally, only two newborns had adverse reactions possibly related to cefotaxime treatment, including diarrhea and feeding intolerance. Conclusion: This prospective real-world study demonstrated that cefotaxime (50 mg/kg, BID) had a favorable efficacy and an accepted safety profile for neonates with EOS. [ABSTRACT FROM AUTHOR]

Fluorescein isothiocyanate (FITC) was used for the fluorescent labeling of sweet corn cob polysaccharide (SCP), followed by identification using fluorescence spectroscopy, UV–Visible spectroscopy, infrared spectroscopy, and NMR analysis. The results confirmed the successful attachment of FITC to SCP with a substitution degree of 1.28%. This established fluorescent sweet corn cob polysaccharide (FSCP) was then subjected to an in vivo quantitative analysis, in which mice were administered with FSCP, fluorescent sweet corn cob polysaccharide nanoemulsion (FSCP-NE), and fluorescent sweet corn cob polysaccharide nano emulsion microcapsule (FSCP-NE-ME) via gavage followed by investigating the in vivo plasma pharmacokinetics and tissue distribution. The results revealed that FSCP-NE could improve the utilization efficiency of SCP, and the FSCP-NE microcapsules exhibited good controlled-release performance, which significantly improved the utilization efficiency of SCP. The absorption and metabolism of SCP in vivo were explored next, which provided the reference for future research and development on SCP for application in the fields of nutraceuticals and pharmaceuticals. Background: Meropenem is being investigated for repurposing as an anti-tuberculosis drug. This study aimed to develop a meropenem population pharmacokinetics model in patients with pulmonary tuberculosis and identify covariates explaining inter-individual variability.Methods: Patients were randomized to one of four treatment groups: meropenem 2 g three times daily plus oral rifampicin 20 mg/kg once daily, meropenem 2 g three times daily, meropenem 1 g three times daily, and meropenem 3 g once daily. Meropenem was administered by intravenous infusion over 0.5–1 h. All patients also received oral amoxicillin/clavulanate together with each meropenem dose, and treatments continued daily for 14 days. Intensive plasma pharmacokinetics sampling over 8 h was conducted on the 14th day of the study. Nonlinear mixed-effects modeling was used for data analysis. The best model was chosen based on likelihood metrics, goodness-of-fit plots, and parsimony. Covariates were tested stepwise.Results: A total of 404 concentration measurements from 49 patients were included in the analysis. A two-compartment model parameterized with clearance (CL), inter-compartmental clearance (Q), and central (V1) and peripheral (V2) volumes of distribution fitted the data well. Typical values of CL, Q, V1, and V2 were 11.8 L/h, 3.26 L/h, 14.2 L, and 3.12 L, respectively. The relative standard errors of the parameter estimates ranged from 3.8 to 35.4%. The covariate relations included in the final model were creatinine clearance on CL and allometric scaling with body weight on all disposition parameters. An effect of age on CL as previously reported could not be identified.Conclusion: A two-compartment model described meropenem population pharmacokinetics in patients with pulmonary tuberculosis well. Covariates found to improve model fit were creatinine clearance and body weight but not rifampicin treatment. The final model will be used for an integrated pharmacokinetics/pharmacodynamics analysis linking meropenem exposure to early bactericidal activity.

[Display omitted] A magnetic enteric-coated tablet containing diclofenac sodium was produced, and its performance under physiological and disturbed gastrointestinal motility was assessed through pharmacomagnetography analysis. In vitro studies were performed using conventional methods and in vivo studies were conducted on healthy volunteers before (control) and after domperidone administration. The magnetic tablet's gastrointestinal (GI) transit and disintegration process were monitored using the Alternating Current Biosusceptometry sensors combined with drug plasmatic concentration. The Gastric Residence Time, Colon Arrival Time, Small Bowel Transit Time, Disintegration Time and the pharmacokinetics parameters were calculated. The pH-dependent polymers used to coat the magnetic tablets were able to avoid the premature drug release on gastric or small intestine simulated medium. Gastric Residence Time was accelerated compared with the control group (p < 0.01). No significant differences were found regarding small bowel transit, colon arrival, disintegration process, or pharmacokinetics parameters. A strong correlation between magnetic monitoring and pharmacokinetics parameters analysis was determinant to evaluate the efficiency in the drug delivery at a specific site in the human gastrointestinal tract. In addition, a tablet with a damaged coating was used as a proof of concept to show the suitability of our methodology to evaluate the tablet. The research was aimed at exploring the biological activities of novel series of β-lactam derivatives against MCF-7 breast cancer cell lines via computer modeling such as quantitative structure-activity relationship (QSAR), designing new compounds and analyzing the drug likeliness of designed compounds. The QSAR model was highly robust as it also conforms to the least minimum requirement for QSAR model from the statistical assessments with a correlation coefficient squared (R2) of 0.8706, correlation coefficient adjusted squared (R2adj) of 0.8411, and cross-validation coefficient (Q2) of 0.7844. The external validation of R2pred was calculated as 0.6083 for model 4. The model parameters (MATS5i and MATS1s) were used in designing new derivative compounds with higher potency against estrogen-positive breast cancer. The pharmacokinetics test on the restructured compounds revealed that all the compounds passed the drug likeness test and they could further proceed to clinical trials. Remdesivir is an antiviral drug that results in clinical improvement after five days of treatment and accelerates recovery by 31%. No studies have discussed the pharmacokinetic analysis of remdesivir in patients with severe COVID-19 requiring extracorporeal membrane oxygenation (ECMO). A 63-year-old American man who underwent mechanical ventilation and ECMO for severe COVID-19 was administered remdesivir for ten days. The loading dosage was 200 mg at 7 PM on day 12 and 100 mg daily at 0:00 PM from day 13–21, administered within 1 h. The pharmacokinetic analysis was performed. The serum creatinine concentration was within the normal range of 0.5–0.7 mg/dL during treatment. According to the pharmacokinetic analysis, the plasma concentrations of remdesivir and GS-441524 4 h after administration (C4) were 662 ng/mL and 58 ng/mL, respectively, and the concentrations 18 h after administration (C18) were 32 ng/mL and 44 ng/mL, respectively. Therefore, the half-life of remdesivir and GS-441524 was 3.2 and 35.1 h, respectively. Monitoring the plasma concentrations of remdesivir and GS-441524 in patients undergoing ECMO may be necessary.

Echinococcosis is a serious helminthic zoonosis with a great impact on human health and livestock husbandry. However, the clinically used drugs (benzimidazoles) have a low cure rate, so alternative drugs are urgently needed. Currently, drug screenings for echinococcosis are mainly phenotype-based, and the efficiency of identifying active compounds is very low. With a pharmacophore model generated from the structures of active amino alcohols, we performed a virtual screening to discover novel compounds with anti-echinococcal activity. Sixty-two compounds from the virtual screening were tested on Echinococcus multilocularis protoscoleces, and 10 of these compounds were found to be active. After further evaluation of their cytotoxicity, S6 was selected along with two active amino alcohols for in vivo pharmacodynamic and pharmacokinetic studies. At the two tested doses (50 and 25 mg/kg), S6 inhibited the growth of E. multilocularis in mice (14.43 and 9.53%), but no significant difference between the treatment groups and control group was observed. Treatment with BTB4 and HT3 was shown to be ineffective. During the 28 days of treatment, the death of mice in the mebendazole, HT3, and BTB4 groups indicated their toxicity. The plasma concentration of S6 administered by both methods was very low, with the Cmax being only 1 ng/ml after oral administration and below the detection limit after intramuscular administration. In addition, the plasma concentrations of BTB4 and HT3 in vitro did not reach high enough levels to kill the parasites. The toxicities of these two amino alcohols indicated that they are not suitable for further development as anti-echinococcal drugs. However, further attempts should be made to increase the bioavailability of S6 and modify its structure. In this study, we demonstrate that pharmacophore-based virtual screenings with high drug identification efficiency could be used to find novel drugs for treating echinococcosis. Background: Carbamazepine (Cbz) is the first-line drug for epileptic seizures but exhibits fluctuation at the plasma level and side effects after oral administration.To overcome these problems, Cbz should be targeted directly into the brain. Therefore, the current experimental design was aimed to formulate and optimize the Cbz containing solid lipid nanoparticles (SLNs) for brain delivery via intranasal administration to get rid of oral complications associated with Cbz., Methods: A full factorial design was performed to evaluate the effect of variables (X1 lipid concentration, X2 surfactant concentration, and X3 sonication time) on the response variables (size of nanoparticles, entrapment efficiency, and drug release). A two-level, three-factor design was employed herewith, and eight formulations were developed. Further, the formation of Cbz containing SLNs was characterized by compatibility, particle size, entrapment efficiency, and drug release with the support of Fourier Transform Infra-Red (FTIR), Zeta sizer, Transmission Electron Microscopy (TEM), Ultra-violet (U.V.), and High-Performance Liquid Chromatography (HPLC)., Results: All eight formulations were characterized through particle size, entrapment efficiency, and invitro drug release performance. Out of eight characterized formulations, SN1 showed the most promising results, including particle size of 210 ± 2.14 nm, entrapment efficiency of 42.1 ± 1.09%, and drug release of 61.3 ± 2.02% and considered an optimized batch. Additionally, the optimized batch SN1was further evaluated for an in-vivo study on male Wistar Rats., Conclusion: The study revealed that a high amount of drug was reached into the brain through intranasal administration compared to the intravenous route. Therefore, it can minimize the unwanted side effects of the Cbz associated with oral administration. The formulation SN1 possesses an excellent drug targeting efficiency of 3.014. Finally, the current experimental work concluded that there is a direct pathway from the intranasal route to the brain. The study aimed to compare the anti-inflammatory and anti-nociceptive activities of Curcuma wenyujin Y.H. Chen et C. Ling (Curcuma wenyujin) and Scutellaria baicalensis Georgi (Scutellaria baicalensis). This study used three parts to compare the two herbs. Firstly, animals were randomly divided into a Scutellaria baicalensis group, a Curcuma wenyujin group, an indomethacin group, and a model-control group to perform an ear edema test, a carrageenin-induced paw edema test, a cotton pellet-induced granuloma formation test, and an acetic acid-induced writhing test. Secondly, model rats with pelvic inflammation were established, and the serum levels of TNF-α and IL-6 in each group was detected with the Enzyme-Linked Immunosorbent Assay (ELISA). Thirdly, pharmacokinetics analysis of Scutellaria baicalensis and Curcuma wenyujin was conducted on the model rats. The ear edema test, carrageenin-induced paw edema test, cotton pellet-induced granuloma formation test, and acetic acid-induced writhing test all showed that Curcuma wenyujin had stronger anti-inflammatory and anti-nociceptive effects than Scutellaria baicalensis. There is significant difference between the effects of Curcuma wenyujin and Scutellaria baicalensis on the levels of TNF-α and IL-6 for the model rats. Curcuma wenyujin decreased the levels of TNF-α and IL-6 more than Scutellaria baicalensis. The pharmacokinetics analysis showed that curcumol's Tmax, Cmax, and the area under the curve (AUC) were all higher than baicalin's. This study indicated that for pelvic inflammation, Curcuma wenyujin had better anti-inflammatory and anti-nociceptive effects than Scutellaria baicalensis.

Assessment of mitochondrial oxidative metabolism has wide-ranging importance, from pharmacokinetic analysis to studies in cell viability and apoptosis. Here we present the Perfusion File Analyzer (PFA) application for the real-time analysis of spectral data to measure cytochrome c reduction, cytochrome a3 reduction, and other parameters important to cellular metabolism, which are collected during tissue perfusion experiments. Our current efforts are focused on quantitating changes in mitochondrial function by normalizing baseline drift of spectral data while addressing two major challenges: (1) a lack of real-time feedback from the system when aiming is compromised, and (2) an inability to adjust calculated data in the event of spectral shift. PFA has been developed to address these issues, and is currently used for quality assessment of human islets prior to clinical transplantation.
