Purpose: Valid and reliable scales which have been developed for assessing individuals' attitudes toward oral health and are based on multiple theoretical views are limited in number in the literature. Hence, there is a need for more scale development studies for further evaluation of the psychometric properties of the oral and dental health attitudes of students. The aim of this study was to develop a scale for measuring college undergraduates' attitudes toward oral and dental health. Materials and Methods: A sample of 770 college undergraduates (241 male, 529 female) enrolled in various academic programs of three universities in Turkey participated in this study. We collected data from two separate samples. The data obtained from sample 1 (n = 470) were used for Exploratory Factor Analysis (EFA) and the data from sample 2 (n = 300) were utilized for undertaking Confirmatory Factor Analysis (CFA). To test the construct validity, EFA, CFA, convergent validity, and measurement invariance were used, respectively. Results: In the first stage, EFA was conducted on a 48-item scale. EFA results showed that the final version of the Oral Health Attitude Scale (OHA-S) had a six-factor structure: sensitivity, importance, avoidance of harmful elements, tendency towards products and activities, awareness, and social impact. To confirm this structure, CFA was used. CFA results showed good model fit indexes. The final version of the scale consisted of 41 items with six factors. Moreover, Cronbach's alpha and Spearman-Brown Split-Half coefficients showed a good level of reliability. Moreover, t-scores were statistically significant for 27% of the lower and upper groups. Conclusion: The developed scale was found to be a potential tool for measuring and evaluating university students' attitudes toward oral and dental health. [ABSTRACT FROM AUTHOR]

Abstract Background Oral health is essential for overall well-being and can significantly improve quality of life. However, people with special health care needs (SHCN) often face challenges in accessing dental services. This study aimed to systematically review all available evidence on the oral and dental service utilization determinants among these individuals. Based on the findings, we also explore strategies to increase their access to dental care. Method This study is a systematic review of reviews based on the PRISMA 2020. Six databases were systematically searched including PubMed, Web of Science, Scopus, Embase, ProQuest, and Cochrane Library. Related keywords were applied up to 30 October 2023. This study includes all systematic, scoping, and rapid reviews written in English that examine the factors affecting dental service use among SHCNs. Microsoft Power BI was used for descriptive quantitative analysis, and MAXQDA version 10 was applied for qualitative thematic analysis. Results The number of 2238 articles were retrieved based on the search strategy. After excluding duplications and appraising the eligibility, 7 articles were included. An examination of these 7 articles shows that they were all carried out from 2016 to 2022. Of these, 42% were systematic reviews, 42% used a scoping method, and one study (14%) was a rapid review. According to the thematic analysis, there were five main themes concerning determinants of oral and dental utilization of SHCN: “Financial considerations,” “Patient-Provider Relationship,” “Accessibility and Availability of Services,” “Patient Factors,” and “Quality of Care.” Additionally, regarding strategies for improving utilization, three main themes emerged: “Education and Training,” “Service Improvement,” and “Policy Solutions. Conclusion This study delves into the intricate challenges SHCNs face in accessing dental services, highlighting the imperative for comprehensive interventions addressing supply and demand. Supply-oriented measures encompass dentist education, implementing financial policies for affordable services, and integrating dental care into primary healthcare systems. On the demand side, strategies revolve around empowering patients and caregivers and enhancing cultural inclusivity. Despite sustained efforts, current utilization rates fall short of optimal levels. Thus, effective strategic planning by policymakers and healthcare leaders is paramount to bolster dental service utilization among SHCNs, thereby enhancing their overall well-being.

Background: Equity in health refers to ensuring that individuals have timely and sufficient access to healthcare services when needed. Human resources are a vital component of healthcare organizations, and they play a crucial role in improving people's access to services. Dentists, in particular, are essential healthcare professionals as oral and dental health significantly impacts overall societal health. This study aims to investigate the key factors contributing to the inequality in the distribution of dentists. Methods: A scoping review was conducted, systematically searching three databases—Scopus, PubMed, and WOS—using relevant keywords until January, 2023. A total of 565 preliminary results were screened, and 447 titles and abstracts were reviewed. Ultimately, 23 full-text articles were included in the analysis. Qualitative content analysis and thematic network were used to synthesize the extracted data. Results: Thematic analysis of the evidence yielded four main themes that contribute to inequality in the distribution of dentists: the development of the inequality situation, methods for analyzing inequality, causes of inequality, and potential solutions to address inequality. Conclusion: Policymakers should gain an understanding of the determinants that contribute to inequality and how these factors manifest in their respective countries. Policymakers can work towards achieving a more equitable distribution of dentists by directing resources towards policies and measures that improve access to oral and dental services. This will ultimately enhance overall societal access to oral healthcare services.

IntroductionCerebrovascular stroke, a leading cause of global morbidity and mortality, is influenced by several modifiable risk factors such as hypertension, diabetes, and smoking. Emerging evidence highlights the significant role of inflammation in stroke pathogenesis, with conditions like periodontitis potentially exacerbating this risk. The aim of this systematic review was to identify and quantify the real impact of periodontal disease in individuals with cerebral stroke through the analysis of inflammatory periodontal indicesMethodsFollowing PRISMA guidelines, we conducted a systematic review and meta-analysis of case-control studies assessing periodontal indices in stroke patients. Data sources included PubMed, Scopus, and Cochrane Library, with searches extended to grey literature. The review protocol was registered on PROSPERO (CRD42024529767). Studies were evaluated using the Newcastle-Ottawa Scale to assess risk of bias, and meta-analyses were conducted using Review Manager 5.4 and TSA software.ResultsThe review included seven case-control studies, comprising 723 stroke patients and 787 controls. Meta-analyses revealed significant differences between stroke and control groups in clinical attachment loss [MD 1.04 mm, 95% CI (0.54, 1.54)], probing pocket depth [MD 0.68 mm, 95% CI (0.31, 10.6)], and radiological bone loss (MD 2.15 mm, 95% CI [−1.58, 5.89]. These findings indicate that stroke patients exhibit worse periodontal health compared to controls, supporting a potential link between periodontal inflammation and stroke.ConclusionThis study confirms the significant impact of periodontal disease on stroke patients and highlights the importance of oral health in preventing adverse cerebrovascular events.Systematic Review RegistrationPROSPERO, identifier (CRD42024529767).

Abstract This short communication paper aimed to compile the main determinants of inequality in dental services by distinguishing between access, utilisation, and provision of dental services. Recent findings integrated, and a dedicated conceptual framework entitled “Triangle of inequality in dental services” has been suggested. These can contribute a rich knowledge in this area and open a new window for policymakers and researchers to seek applied interventions to decrease inequality and improve access and utilisation in communities. This paper aims to synthesise the available evidence and add value to the scope. It highlights a dedicated concept for inequality in dental services beyond other areas of public health.

Objective: Diabetes is closely related to oral and dental health. Several oral diseases and disorders are associated with diabetes. The present study was conducted to compare the effectiveness of face-to-face and non-face-to-face training of oral and dental health behaviors based on the Health Belief Model (HBM) in patients with type 2 diabetes. Materials and methods: This study was conducted among patients with type 2 diabetes referring to comprehensive health centers in Andimeshk in 2021. One hundred twenty patients with type 2 diabetes were randomly divided into two groups: a face-to- -face (60 patients) training group and a non-face-to-face (60 patients) training group. For the face-to-face training group, training programs were conducted through lectures with questions and answers, group discussions, and videos in three 60-minute sessions; and for the other group through the presentation of patient education pamphlets. The results were collected in two stages before and two months after the intervention using a questionnaire. Then the data were analyzed using a chi-squared test, t-test, and paired t-tests in SPSS Statistics 22.0. Results: A total of 120 patients participated in this study. The patients' mean age was 51.5 ± 8.57 and 51.9 ± 10.1 years in the control and the intervention group, respectively. In both groups, 60% of the participants were female and 95% were married. The duration of diabetes in 57.5% of them was 5-10 years. After the implementation of the intervention, a significant increase was observed in the mean scores of all HBM constructs in the face-to-face training group (p < 0.001). However, there was no significant change in the mean scores of the HBM constructs in the non-face-to-face training group after the intervention (p > 0.05). Conclusions: This study showed that the design and implementation of an in-person training program had a positive effect on improving oral and dental health behaviors in patients with diabetes based on the HBM. One of the primary causes of disability and mortality in the adult population worldwide is stroke. A person's general health is significantly impacted by their oral and dental health. People who have poor oral health are more susceptible to conditions such as stroke. Stroke risk has long been linked to oral and dental conditions. The risk of stroke and its cost impact on the healthcare systems appear to be significantly reduced as a result of the decline in the incidence and prevalence of oral and dental illnesses. Hypothetically, better management of oral hygiene and dental health lead to reduced stroke risk. To the authors' best knowledge, for the first time, the potential link between dental health and stroke were cross-examined. The most typical stroke symptoms, oral and dental illnesses linked to stroke, and the role of oral healthcare professionals in stroke prevention are revealed. The potential mediating processes and subsequent long-term cognitive and functional neurological outcomes are based on the available literature. It must be noted that periodontal diseases and tooth loss are two common oral health measures. Lack of knowledge on the effects of poor oral health on systemic health together with limited access to primary medical or dental care are considered to be partially responsible for the elevated risk of stroke. Concrete evidence confirming the associations between oral inflammatory conditions and stroke in large cohort prospective studies, stratifying association between oral disease severity and stroke risk and disease effects on stroke survival will be desirable. In terms of clinical pathology, a predictive model of stroke as a function of oral health status, and biomarkers of systemic inflammation could be useful for both cardiologists and dentists.

Objective: Oral and dental diseases are among the most common non-communicable diseases in the world and are one of the major global public health problems. The aim of this research is to determine the changes in productivity levels of the Oral and Dental Health Centers (ODHC) of the Ministry of Health in Turkey in the 5-year period between 2014-2018. Material-Method: The universe of the research consists of ODHC in Turkey, which continued its activities continuously between 2014 and 2018 and reached the capacity of 50 units and more in 2017. In the study, the entire universe that had not been sampled was reached. The Malmquist Total Factor Productivity Index (MI) was used to detect changes in productivity levels in the study. Results: ME analysis found that ODHC's total factor productivity (TFP) increased by 3.8% in the period 2014-2018. Productivity increases in technological change observed in all periods except the 2014-2015 period also led to a positive change in TFP. The increase in technological change indicates that the input-output compositions in the production processes of the ODHCs give positive signals. Furthermore, in other periods other than 2014-2015, there has been a negative change in technical efficiency. Conclusions: Each 1-degree increase in efficiency scores will contribute significantly to the performance of ODHCs. Therefore, in order to increase the efficiency of ODHC in Turkey; manpower, capital, medical technology etc. health policies should be developed to increase the output levels that are insufficient by ensuring optimum benefits from resources, reducing the differences between regions to a minimum, withdrawing resources from areas where they are used inefficiently to areas where they are used efficiently, preventing waste and making the necessary plan. - Abstract
Aim: Due to the common nature of oral and dental problems, awareness needs to be improved, expanded, and maintained. The aim of this study was to determine the oral and dental health knowledge levels of students, many of who would be teachers of the future, in this health field, and to help them work toward the elimination of the identified deficiencies. Materials and Methods: This cross-sectional descriptive study involved 475 students from the Erzurum Ataturk University dentistry, medicine, pharmaceutical sciences, nursing, and education faculties. Data were collected through a questionnaire by random stratified technique, investigating participants' demographic characteristics and oral health knowledge levels. Results: We observed that 77.3% of students correctly answered 15 or more out of 31 questions addressing levels of knowledge of oral and dental health. This figure was 100% for students in the dentistry faculty and 56.3% for those in the education faculty. The difference between faculties in terms of students correctly answering 15 or more questions was statistically significant (P <.001). A statistically significant difference was also determined between the faculties in terms of students' sense of competence in oral and dental subjects (P <.001). Conclusion: In the light of the data obtained, since prospective health care professionals and teachers both play an important role in raising public awareness, we recommend that a basic oral health course be introduced into the curricula of the relevant faculties. - Abstract
Chewing, transfer of nutrients and swallowing are continuous events. Chewing function covers the coordinated movement of teeth, chewing muscles, temporomandibular joints, tongue, cheeks and lips. Swallowing is a dynamic and complex movement with a widespread sensory motor neural network, requiring coordinated work of lips, tongue, palate, pharynx, larynx and esophageal muscles. In case of dysphagia, defined as deterioration at any stage of the swallowing sequence, the safety, efficiency or adequacy of food intake may be impaired. Patients with dysphagia are known to be at greater risk of developing aspiration pneumonia Diagnosis and treatment of these patients often requires a multidisciplinary approach. It is critical for an impaired oral and dental health condition and oral infections, especially for aspiration and related conditions. In order to prevent such infections, patients with swallowing dysfunction should care to oral and dental health, and they should be trained to provide oral hygiene. In these patients, it will be possible to contribute to the chewing function and indirectly to swallow problems by completing the oral and dental treatments and providing a good occlusion. Objective: The present research aimed to investigate the effect of an educational intervention based on the health belief model in pregnant women visiting the healthcare centers in the west of Tehran in 2016. Materials and Methods: The present experimental research was conducted on 135 pregnant women visiting the healthcare centers in the west of Tehran. The subjects were selected through a randomized multistratified sampling and were divided into an experimental and a control group. The collected data were then statistically analyzed in SPSS ver.16. To do so, descriptive statistics and inferential statistics were used including the independent‐samples t‐test, repeated‐measures one‐way ANOVA and paired‐sample t‐test. Results: The present findings revealed a statistically significant difference between the experimental and control groups in terms of immediate post‐test results including perceived sensitivity, severity, benefits, barriers, and self‐efficacy along with awareness and performance constructs (p ˂ 0.001). Moreover, two months after the intervention, except for the perceived sensitivity construct (p = 0.088), the two groups diverged significantly in terms of the other constructs (p ˂ 0.001). Conclusions: The educational intervention driven by the health belief theory showed to be effective on pregnant women and can help to promote preventive behaviors of tooth decay.
