Background: This study examines the phenomenon of arrangements limiting terms of office for directors of nursing (DONs) as a policy change in the Israeli public healthcare system since nursing is a major component of the healthcare system's personnel, and DONs have a crucial influence on achieving the organization's goals in a changing and developing environment with limited resources. Aim of Article: To understand how senior role-holders in the Israeli public healthcare system view arrangements limiting terms of office for directors of nursing as a policy change in the Israeli nursing system. Methods: Qualitative design comprising face-to-face interviews with sixteen purposefully sampled senior managers at three medical centers in central Israel. Data were analyzed thematically. Findings: Five themes emerged: ambivalence concerning the need for arrangements limiting terms of office, arrangements limiting terms of office as an organizational change, arrangements limiting terms of office as a method of career management, arrangements limiting terms of office – where they are headed, and length of the term of office. Conclusions: This study shows that the answer to the question whether to perpetuate the past or shape the future is not unequivocal; it is evident from ambivalent attitudes towards the option of instituting arrangements limiting terms of office.

PDF, 要 旨 千葉県近代的医療の起源となった共立病院は、その後公立千葉病院、県立千葉病院へと改組・改称している。その3病院の諸規則をもとに千葉県の看護の黎明について、看病から看護への移行の時期及び看護の進展経過を明らかにすることを目的とし、さらに同時期の全国及び千葉県内の病院に関する文献から考察を加えた。研究方法は、共立病院~県立千葉病院の諸規定史料を対象に、明治7年~明治 20 年代の期間の歴史調査研究である。結果、病院創設時以降の病院規則には「看病」の語句しかみられなかったが、明治 15 年の病院改組時の規則に「看護及び看護婦」の語句があり、看病から看護への移行期に入ったことが窺えた。この当時の看病・看護は医師主導で、佐倉順天堂及び東京大学医学部(前身の時代も含む)卒の医師が3病院の運営に携わり、千葉県の医療・医学を率いていた。共立病院~県立千葉病院への発展に伴って看護も徐々に形成されたが、3病院の看病人・看護人は医師の管理の下に置かれていた。しかし、与薬、病状観察、室内環境調整、病床整備、診察時の介助等、諸規定からみた職務内容は、現行法規上の『療養上の世話と診療の補助』の原点となり得るものだった。

7. Impact of an Electronic Medical Record-Connected Questionnaire on Efficient Nursing Documentation: Usability and Efficacy Study.

- Abstract
Background: Documentation tasks comprise a large percentage of nurses' workloads. Nursing records were partially based on a report from the patient. However, it is not a verbatim transcription of the patient's complaints but a type of medical record. Therefore, to reduce the time spent on nursing documentation, it is necessary to assist in the appropriate conversion or citation of patient reports to professional records. However, few studies have been conducted on systems for capturing patient reports in electronic medical records. In addition, there have been no reports on whether such a system reduces the time spent on nursing documentation., Objective: This study aims to develop a patient self-reporting system that appropriately converts data to nursing records and evaluate its effect on reducing the documenting burden for nurses., Methods: An electronic medical record-connected questionnaire and a preadmission nursing questionnaire were administered. The questionnaire responses entered by the patients were quoted in the patient profile for inpatient assessment in the nursing system. To clarify its efficacy, this study examined whether the use of the electronic questionnaire system saved the nurses' time entering the patient profile admitted between August and December 2022. It also surveyed the usability of the electronic questionnaire between April and December 2022., Results: A total of 3111 (78%) patients reported that they answered the electronic medical questionnaire by themselves. Of them, 2715 (88%) felt it was easy to use and 2604 (85%) were willing to use it again. The electronic questionnaire was used in 1326 of 2425 admission cases (use group). The input time for the patient profile was significantly shorter in the use group than in the no-use group (P<.001). Stratified analyses showed that in the internal medicine wards and in patients with dependent activities of daily living, nurses took 13%-18% (1.3 to 2 minutes) less time to enter patient profiles within the use group (both P<.001), even though there was no difference in the amount of information. By contrast, in the surgical wards and in the patients with independent activities of daily living, there was no difference in the time to entry (P=.50 and P=.20, respectively), but there was a greater amount of information in the use group., Conclusions: The study developed and implemented a system in which self-reported patient data were captured in the hospital information network and quoted in the nursing system. In Bangladesh, shrimp post-larvae (PL) are mainly produced in hatcheries. From 2000s shrimp PL are reared in nurseries containing small earthen ponds by relatively poor people for a few days before stocking into the shrimp farms. This study assesses the physical and chemical parameters of five such nurseries at Satkhira district, Bangladesh using scientific instruments and semi-structured questionnaire. The nursing process involved stocking of PL (4151.8 ± 1581.35 PL/m2) in small earthen ponds (<18m2 area/pond) and raring for up to 10 days with feeding. Most of the physico-chemical parameters were within the suitable range recommended for shrimp culture. The survival rate of PL ranged from 80% to 90%. The nursing system was profitable as the average Benefit-Cost Ratio (BCR) was 1.19. The establishment and operation of these nurseries do not require any expensive equipment or complex technology and can be run by less educated and poor people. However, constraints like lack of credit and continuous water supply are hindering the scaling up of these nurseries in size and number. By providing means to overcome the constraints these nurseries can be an excellent income generating source for the poor entrepreneurs as well as contribute in increasing shrimp production in Bangladesh. 9. Responding to COVID-19: Establishing a nursing system that is appropriate for the new post-epidemic era.

Hospitals that admit patients with COVID-19 face the challenge of not only dealing with these patients but also balancing normal medical care and hospital management to cope with the challenges posed by the new post-epidemic era. Over the past two years of responding to COVID-19 as a front-line clinical nurse, my colleagues and I have fully appreciated the need to establish a nursing system that is appropriate for the new post-epidemic era. Organization, work and work reactions: a study of the relationship between organizational aspects of nursing and nurses' work characteristics and work reactions This article describes a quantitative, correlational study of the relationship between organizational aspects (such as the predictability of the care), work characteristics (autonomy and workload) and work reactions (work satisfaction and health complaints) in nursing work. The variables have been measured by questionnaires. Subjects were 155 nurses from nine units in two general hospitals in the Netherlands. Several organizational aspects seem to be correlated with work characteristics and work reactions. Organizational aspects particularly influence work characteristics and play, therefore, a mediating role in the relationship between organizational aspects and work reactions. Job satisfaction is not only correlated with the autonomy, but also with workload. Health complaints appeared to be mainly correlated with the workload. It is concluded that for an improvement of job satisfaction and reduction of health complaints, it is not only necessary to improve autonomy and reduce workload, but also to pay attention to organizational aspects because these may influence the work characteristics. 11. Evaluation study of the nurse staffing situation in the general ward of an acute hospital after introduction of the nursing system of one nurse for every seven inpatients

It has been5years since an introduction of the nursing system of one nurse for every seven inpatients. The aim of this study was to determine whether this nursing system leads to meet the demands of the inpatients and to clarify improvement factors in adequate personnel distribution. Participants were196head nurses of28acute hospitals in the Chugoku and Shikoku regions in Japan. As a result of analysis, the numbers of nurses have been increased by the introduction of this system. However, the employing of a large number of new nurses with almost no experience, it caused shortage of personnel, and reduction in nursing service quality. Also, as inclined allocation of personnel was not conducted according to the degree of nursing care need, imbalanced personnel allocation was observed in survey data. Furthermore, there was no significant correlation between bed occupancy rate and the number of nurses ; therefore it was thought that there is not staff placement depending on busyness of nurse. For the adequate personnel allocation and careful nursing system, it was thought that calculation of the number of nurse placement based on appropriate measurement of nursing care need and evaluation of nurses’ competence was necessary.

Bakgrund: Sjuksköterskan har ansvar att tillämpa delaktighet i omvårdnad, både utifrån lagar men även utifrån ett etiskt perspektiv. Sjuksköterskans arbete på somatisk vårdavdelning påverkas av faktorer såsom hög arbetsbelastning och att bli avbruten. Detta påverkar sjuksköterskans tillämpning av delaktighet. Kunskap och förståelse om delaktighet kan underlätta främjandet av delaktighet i sjuksköterskans arbete. Orems omvårdnadsteori, som examensarbetet tagit förankring i, bygger på att sjuksköterskan ska stärka patientens förmåga till egenvård varvid delaktighet är en förutsättning. Syfte: Syftet är att belysa begreppet delaktighet för patienter på somatisk vårdavdelning i relation till sjuksköterskan. Metod: En begreppsanalytisk modell av Walker och Avant. Resultat: Delaktighet utgörs av orden del och akt och har ursprung i latinska pars. Delaktighet finns i olika former och är en aktiv handling där patientens inre resurser beaktas. Delning av information, en etablerad relation och tillit är viktigt vid ansvarsfördelning. Delaktighet som begrepp består av de sex attributen integration, informationsutbyte, partnerskap, interaktion, kapacitet och ansvar. Alla attributen är beroende av varandra för att fånga in begreppet, vilket demonstreras i olika fallbeskrivningar. Slutsats: De sex definierande attributen har tydliggjort hur delaktighet kan användas i praktiken. Med konkretisering av delaktighet finns ökad förståelse för hur sjuksköterskan kan främja patientens förmåga till egenvård. Background: A nurse has a responsibility to apply participation in nursing, both from laws but also from an ethical point of view. A nurse’s work in a somatic ward is affected by factors as high workload and being interrupted, which affects participation. Knowledge and understanding about participation may facilitate participation in the nurse’s work. Orem’s theory of nursing, on which this bachelor thesis has taken abutment in, is based on that the nurse should consolidate the patient’s ability to self-care wherein participation is a condition. Aim: To highlight the concept of participation of patients in a somatic ward in relation to the nurse. Method: A concept analysis model by Walker and Avant. Results: Participation constitute from the words part and act and has its origin in Latin pars. Participation comes in different forms and is an active action where the patient’s inner resources are taken in regard. Sharing of information, an established relationship and trust is important for division of responsibility. Participation as a concept consists of the six attributes integration, information exchange, partnership, interaction, capacity and responsibility. All these attributes depend on each other to capture the concept, which has been demonstrated in different case studies. Conclusion: The six defining attributes have clarified how participation can be used in practice. With the concretization of participation there is greater understanding of how the nurse can promote the patient's ability to self-care.

The paper evaluate and describes methods and techniques needed for implementation of SIP protocol in nursing environments. Since this is quite new and unexplored area, some proposals in the paper can serve as groundwork for next generations of nursing telemedicine systems as well as nursing units based on SIP protocol. That could enable era of unified messaging in nursing informatics and telemedicine.

- Abstract
本研究は、諸外国の看護システムについて調査し、わが国の現状との比較分析を行うことにより、看護の質の向上のための看護職員のあり方について検討することを目的に行った。特に諸外国の看護システムおよび准看護婦(士)に類似する看護職の実状を明らかにすることに焦点を当てて研究を進めた。 調査対象は、わが国の今後に示唆を得られると思われる国として、アメリカ、カナダ、オーストラリア、フランス、スウェーデン、イギリスの6ヵ国を選んだ。看護推進における先駆的な活動および今後の方向性を得るため、WHOコラボレーションセンターおよび看護専門職能団体を中心に、各機関の長、29名に質問紙調査を依頼した。質問紙は19通回収され、回収率は65.5%、有効回答は15通であった。さらに、医療現場における実態を詳細に把握する目的で、アメリカの看護管理および教育の専門家3名に、面接調査を実施した。その結果、以下の知見を得た。 1.准看護婦を有していた国は、7か国中6か国(フランス以外)であり、准看護婦の養成を停止していたのは1か国(イギリス)であった。 2.人口1, 000人当りの看護婦数は、7か国中、わが国が最低であった。わが国の准看護婦と看護婦とを合わせても、イギリスの看護婦数には届かなかった。准看護婦と看護婦の比率が近似していたのは、日本であり、他の国においては、准看護婦数が看護婦数の1/3~1/4であった。 3.准看護婦のいる5か国においては、准看護婦と看護婦の能力規定あるいは業務範囲が明確に認められたが、わが国においては不明瞭であった。 4.准看護婦の養成に必要な教育期間は11~14年であり、日本は最短の11年であった。入学資格が学校教育9年であったのは、日本のみであり、他は高等学校卒業レベルであった。 5.准看護婦から看護婦への移行プログラムは、4か国で、大学相当レベルの高等教育に位置づけられていた。 6.准看護婦の養成を停止したイギリスでは、看護婦を高いレベルに統一することができたが、同時に看護補助者養成の必要性が明らかになった。 現代のように、医療が高度化・専門家し、地域におけるケアニーズが急増している現状では、それに対応した看護を行える人材の養成として、わが国の現行の准看護婦養成では不十分である。今後、准看護婦の養成を停止し、看護教育の一元化に向けて早急な検討が必要である。, The present study was designed to investigate nursing systems in other countries in comparison with the current system in Japan, and thereby assess how nursing personnel should be in order to improve the quality of nursing.The study focused on the actual situation of the nursing system and licensed practical nurses(LPNs)or second-level nurses similar to LPNs in other countries. The survey was conducted in regard to six countries considered to be advanced in the development of nursing activities and likely to provide usueful suggestions for the future direction of nursing in Japan:Australia, Canada, France, Sweden, the United Kingdom and the United States.Questionnaires were sent to the heads or staff of a total of 29 organizations, including WHO Collaborating Centers and professional organizations for nursing, in these countries.Nineteen organizations responded(response rate:65.5%), and an effective response with completed questionnaire forms was obtained from 15 organizations.In addition, an interview survey was conducted on three nursing administration and education experts in the United States in order to acquire clear understanding of the actal state of nursing in health care practice.The results were as follows:1.Six countries(Japan included)excluding France had LPNs.Educational preparation for LPNs has been discontinued in the United Kingdom.2.The number of nurses per 1, 000population was lowest in Japan:the total number of registered nurses(RNs)and LPNs in Japan was less than the number of RNs in the United Kingdom.Only in Japan the proportions of LPNs and RNs were similar, while in other countries of number of LPNs was one-third to one-fourth of the number of RNs.3.In the five other countries having LPNs, the nurse's competency or scope of practice was clearly defined for both LPNs and RNs.In contrast, no clear line was drawn betweenthe two in Japan.4.The length of education required for LPNs ranged from 11 to 14 years(including the period of compulsory educ
