新型コロナウイルス感染拡大下において、母性看護学実習が余儀なく縮小されたことで、看護実践 能力の低下が指摘されている。そのため、演習科目では、補完的に実践的で質の高い新たな教育手法 が課題となっている。本研究の目的は、臨地場面を再現した高機能周産期全身シミュレーター(high- performance perinatal whole-body simulator: HPWS) を用いて、学修目標の達成度とシミュレーション教育の成果を評価し課題を明らかにすることである。 対象者は看護大学 3 年次 85 名、実習前の演習科目で、従来の演習を基に HPWS を活用する学生 と活用しない学生の 2 群に分別した。学生は、妊産婦と褥婦の同2事例で演習を行った。演習後に学生 は ICT を活用した学務システム “MORIPA” でアンケート調査を実施した。調査項目は学生の「学修目 標達成度」「母性看護技術」「演習の興味・関心」「授業への満足度」を群別で比較し今後の課題を明ら かにした。 妊産婦の演習では、HPWS を活用した学生群がすべての項目で、活用していない学生群よりも高 得点で、胎児の健康状態の評価と演習への充実で、両群に有意差を認めた (p, There has been a decline in students’ nursing practice ability. This decline has been attributed to the gradual decrease in clinical maternal nursing practice due to COVID 19 and its variants. Therefore, a new educational methodology that is complementary, practical, and high- quality is necessary to train nurses students. This study aimed to evaluate the achievement level of learning objectives and the results of educational simulation using a "high-performance perinatal whole-body simulator (HPWS)" that reproduces clinical situations and clarifies related issues. The participants were 85 students in their third year of a university nursing program. The students were divided into two groups: students who used the simulator (the Intervention Group: IG) and conventional training (the Control Group: CG). The students performed nursing diagnosis, practice, and evaluation of two identical pregnancy and puerperium cases. After the training, they answered a questionnaire using the academic affairs system "MORIPA" which uses information and communication technology. A questionnaire survey was administered to the participants, with the following survey items: “Goal Achievement Level (GAL),” “Maternity Nursing Skills (MNS),” “Interest in Training (IT),” and “Student Satisfaction Level (SSL).” Higher scores indicated higher academic achievement. 本報告はCOVID-19 感染拡大により学内実習に変更した基礎看護実習１の教育実践を振り返り，基礎看護学における看護実践能力の育成につなげるための学内実習の有効であった点と課題を明確にすることを目的とした．学内実習では看護の役割や連携に関する講義，病院と病棟ならびに看護活動に関する動画の視聴，看護師へのインタビューを実施した．学習内容の順序性を考慮し，関連付けて学習できるようにした．学生は臨床の場での看護活動や対象者との関わりを知り，対象者やチーム医療に必要な対人関係能力，倫理観について学ぶことができたと考える．学内実習により対象者との援助関係の構築や学生指導看護師と関わりチームの一員として役割を果たすことは実際の体験を通して学ぶことはできず，この点は２年次以降の学習で補う必要がある．基礎看護学において看護実践能力の基盤を形成し，看護に対する洞察を深めていけるよう教育活動を行う必要がある．

本報告は新型コロナウイルス感染症の拡大前後の実践過程を通して，臨地実習を学内実習へ変更したことにより，看護実践能力を育成する上で基礎看護学領域が担う課題を明らかにすることを目的とした．学内実習は，感染対策を講じた上で対面での模擬実践と自宅からの遠隔での学内実習（模擬患者の看護過程）を組み合わせた．このような教育方法によって看護過程の思考を学ぶことが可能だと考えた．一方，直接的な患者とのコミュニケーションや援助関係の構築，学生指導看護師への報告や相談といった対人的な経験および看護過程の実施と評価の学習は限界があった．患者に看護を提供するには，健康状態の変化に対応した思考力や判断力，対人関係能力といった看護実践能力を高めることが重要である．今後，基礎看護学領域が担うべき課題は，学生が知識や技術および態度を主体的にその後の講義や臨地実習の学習と統合させ，看護実践能力の基盤を育成することである．

目的：看護職者の多重課題遂行における疲労感や達成感・充実感の有無と看護実践能力，自己効力感，認知的熟慮性の違いを明らかにする．方法：看護職者277名を分析対象とし，看護実践能力を看護実践の卓越性自己評価尺度（NESCP）の総得点および7つの下位尺度，自己効力感を一般性セルフ・エフィカシー尺度（GSES），認知的熟慮性を認知的熟慮性ー衝動性尺度を用いて評価し，多重課題遂行における疲労感や達成感・充実感の有無による，各尺度の得点の差をMann-Whitney U検定を用いて検討した．結果：多重課題に疲労感を感じる看護職者は，NESCPの総得点（p=0.015）やNESCPの下位尺度の情報を収集し活用する能力（p=0.001），GSES（p=0.003）の得点が低く，達成感・充.039）の得点が低かった．認知的熟慮性については有意差がみられなかった．結論：多重課題遂行において，疲労感を感じる看護職者および達成感・充実感を感じない看護職者の看護実践能力や自己効力感が低いことが示された．看護実践能力については，疲労感と達成感・充実感で異なる能力が影響することが示唆され，看護職者の個別性に合わせた支援が必要であると推察された．

A大学の4年生対象の看護技術演習に，看護実践能力の向上を目指して，シミュレーション学習と結びつけたOSCEを導入した．本稿の目的は，本演習科目について，科目の実践内容と評価を記述することである．本演習科目の学習目的は，看護職者として必要な知識・技術等の能力が身についているか客観的に評価するとともに，自らの課題解決に向けて取り組むことであった．シミュレーション学習を実施した結果，90%以上の学生が「十分できる」と回答した項目は19項目中16項目であった．また，評価表の自由記載欄の記述内容より，学生は自分の実力を見直し，卒業までに克服すべき課題を明確にしていた．OSCEを実施した結果，60点満点で平均45.4点（75.7%）であった．本演習科目は，学生の看護実践能力の向上を促進できたと考える．一方で，シミュレーション学習とOSCEを結びつけることによる学習効果を，より高められるような工夫が必要であることが示唆された．, 実践報告

本研究ではリスク感性が高い看護師の看護実践能力や臨床での経験からの学びについての関連性を明らかにすることを目的とした平成22年l月～2月に病棟看護師422名（看護師長以上を除く）を対象に真下（2009）が開発した「看護実践能力尺度」と属性（年齢・看護師経験年数・教育背景・現在の職場経験年数・部署での役割）と看護師としての知識・技術・考え方を身につける上で印象に残る職務上の経験とそれからの学びについて調査した回収数349名（回収率82.7%）.有効回答率242名（69.3%）. 年齢31.7歳（SD±9.l),看護師経験年数9.8年（SD±8.8），職場経験3.5歳（SD主3.2），男性13名（5.4%），女性229名（94.6%）だ、った項目別に「リスク感性」と比較すると「基本的看護ケアJ(r = 0.79）「個別性に合わせた看護過程の展開J(r = 0.74)はきわめて強い正の相関を示したが，「役割遂行J(r = 0.61）「医療依存度が高い看護ケアJ(r = 0.58）とはやや強い正の相関を示した年齢・経験年数・役割経験による比較では差は無かった職務上での経験内容は［自己学習］［他者（医療者）からの学び｝［患者ケア｝｛失敗経験］に分類された「リスク感性Jの点数が平均（3.2）より高い看護師96人中．職務上での経験内容を記載した人50名の中で「経験からの内省化と教訓を概念化した事例jを記載した「状況の中で直感的に問題を見つけて対応した事例」や「経験後．行動変容を起こした内容］を記載した人は7 名（1~%）で．平均より低い点数で記載していた人は77 名中6 名（7.8%）だった．「リスク感性」が高い看護師と看護実践能力との関連性はありJ特に「基本的看護ケア」と「看護過程の展開」がきわめて高い．「リスク感性」は看護師の年齢や経験年数を重ねるだけでは高まらない．日頃の看護ケアから「経験からの内省化と教訓の概念化Jすることが「リスク感性Jを高める為には重要であることが示唆された．, The purpose of this study was to identify relationships between the risk sensitivity of nurses and nursing practice ability/learning from clinical experience. Between January and February 2010, the study investigated basic attributes (age, years of nursing experience, educational background, years of nursing experience at the present hospital, types of nursing job), vocational experience that left the biggest impression in acquiring knowledge, skills, and values, and its learning involving 422 ward nurses (excluding nurses holding any positions higher than manager) using a nursing practice ability scale. Of the 349 responses collected (response rate: 82.7%), 242 responses (13 male (5.4%) and 229 female (94.6%)) were considered valid (69.3%). Their basic attributes were as follows: mean age: 31.7 years old (SD±9.l), mean years of nursing experience: 9.8 years (SD±8.8), and mean years of nursing experience at the present hospital: 3.5 years (SD士3.2). On comparing the data and risk sensitivity, a very strong positive correlation was observed in "basic nursing care" (r=0.79) and "expansion of nursing practice based on patient's individuality" (r=0.74); whereas, a reasonably strong positive correlation was observed in "role performance" (r=0.61) and "nursing care for patients requiring intensive medical treatment" (r=0.58). No significant difference was observed in age, nursing experience, and years of nursing experience at the present hospital. The subjects vocational experience was categorized into the following: self-study" "learning from others (medical treatment person)", "patient caring", and "experience of failure". Of the 96 subjects who scored above average (3.2) regarding "risk sensitivity", 50 subjects stated their vocational experience. Of the 50 subjects, 7 (14%) described "introspection from experience and lesson are made concept"; whereas, only 6 (7.8%) of the 77 who scored below average stated their vocational experience. "Risk sensitivity" has the relevance of a high nurse and nursing practice capability, and "a fundamental nursing care" and "deployment of a nursing process" are very high especially. "Risk sensitivity" does not increase only by piling up a nurse's age and years of experience. The important thing was suggested in order for introspection-izing from experience and conceptualizing of teachings from a daily nursing care to improve risk sensitivity.

本研究の目的は，「経管栄養法」において，実践的な看護技術習得を目指すための教材，オリジナル人形を製作・使用し，その有効性と経管栄養法における看護技術教育方法について検討することである．本講義・演習を受講し，研究同意の得られた学生のアンケート結果を分析し，検討した．その結果，オリジナル人形を製作・使用したことにより，経管栄養法で一番重要であるカテーテルが胃内に挿入できているかどうかの確認方法や，実際に栄養剤を注入する練習ができており，より実践に近い演習を行うことができた． しかし，オリジナル人形と成人モデル人形の2つのモデル人形を併用したことによる経管栄養法の手技の流れは，十分に理解できておらず，看護技術習得には不十分であった． 経管栄養法におけるオリジナル人形は有効であったと考えるが，今後は，経管栄養法の手技の流れを理解しつつ其々ポイントとなる手技を確実に実施できるように，講義演習の工夫が必要である．, A purpose of this research is I produce the teaching materials to aim at the practical nursing technology acquisition, an original model doll, and to use it in tubal feeding. And it is to examine the effectiveness and a nursing technical education method in the tubal feeding. I attended this lecture / practice and analyzed the questionnaire result of a provided student of the study agreement and examined. As a result, there are a confirmation method whether or not I can insert the most important catheter in a stomach by tubal feeding by I produce an original doll, and having used it and an exercise to really inject a nutritional supplement. In the as possible clinical similar, near form was able to practice it technically. The student was able to perform the technical practice that was as possible almost practice. However, I cannot understand the flow of the manual skill of the tubal feeding by having used two model doll of the adult type doll for medical practice together with an original doll enough. The students were inadequate for the nursing technology acquisition. I think that the original doll in the tubal feeding was effective. A device of the lecture practice will be necessary in future to be able to carry out a manual skill to become each point while understanding the flow of the manual skill of the tubal feeding surely.

application/pdf, 本研究では,「『静脈血採血』技術の修得を促す教育方法」における課題「様々な技術修得の過程に反映できるかの検証」を受け,「基礎看護技術論II」『環境を整える・ベッドメーキング』の技術演習において,看護の初学者である学生に『静脈血採血』の技術演習に準じた教授方略を用い検証を行った.『静脈血採血』に準じた習熟度別の教授方略は,習熟度による各コースの技術修得に有意な差がみられ,学年の終盤に修得する『静脈血採血』とは異なる結果となったが,技術試験では表現されない学生の思考の過程は,教授一学習方法を検討する際の要素となった.「ベッドメーキング」の技術修得の過程は,演習前のイメージ作りから立体像を描く過程の教授方略,演習での習熟度別の教授方略,課外練習の活用方法,および技術試験の評価を一連とし,その教授-学習方法の構造化を行った.看護の初学者を教授する基礎看護学領域において,新カリキュラムに盛り込まれている「様々な環境の変化の中で常に社会から必要とされる看護師」であるために養うべき学生の資質,そこにかかわる教員の資質を看護実践能力を主眼として論考した.学生は,技術を,模倣ではなく自分のものとし,巧妙化するために,"知る段階""身につける段階"を実体験から学んでいる.主体性に欠ける学習姿勢を指摘される近年の学生は,この実体験が,基礎看護学領域においては要であり,看護の初学者である学生の純粋な思いや疑問など,学生の感じたままに正面から向き合い,その感じたままの感情を大切に育むことが,教授一学習方法の構造化に必要不可欠であることを述べる.

大学教育における卒前教育のプログラム開発への基礎資料を得るために、新卒看護師の11ヶ月時点を対象に大室らの「大卒看護師の看護実践能力の到達度評価100項目」を用いて看護実践能力評価を実施した。その結果、看護実践を行う卒業生とその職場の上司から看護実践能力評価の差異が確認された。新卒が有意に高く評価していた内容は、領域では、患者の自己決定を尊重することができる等3項目、領域では、罨法など心身に必要な安楽促進ケア、口腔ケアができる等17項目であった。領域では、新卒と上司の間に明らかな差異は認めなかった。また看護実践能力の大室らと本学新卒の調査結果を比較すると30%の差がみられた項目は、「個別性の高い看護計画を立案できる」、「医師や他の医療者に、自分の考えを表現できる」の2項目であった。, To get a basic document for developement of the program for the front education before graduation in the university, we used "100 items of the evaluation of achievement in the nursing practice ability of the university graduate nurse" by Omuro, and carried out the evaluation of the nursing practice ability for an object of the new college graduate nurse in the 11 monts' carrier.
