2. The prehistoric settlement landscapes of Mayo : the evidence and impact of development-led archaeology

This thesis provides an extensive examination of the evidence and impact of three decades of development-led archaeological work in county Mayo on the western Irish Atlantic seaboard. The research makes a significant and original contribution to knowledge, creating a new baseline dataset for prehistoric enquiry in Mayo. The work involved rigorous, systematic research, sourcing, evaluating and contextualising a vast body of development-led data, distributed within 'grey literature', unpublished excavation, monitoring and testing reports, archives, radiocarbon certificates, specialist reports and information held privately by archaeologists. The research examined how development-led archaeology came to dominate archaeological practice in Mayo, Ireland and globally. Taking a historiographical approach, the path of scholarly enquiry in Mayo was charted through an evaluation of myth histories, pseudohistorical narratives, scholarly antiquarianism and early scientific archaeological investigations. Understanding the evolution of development-led archaeology necessitated a global perspective, demonstrating how when faced with threats to the finite archaeological resource archaeologists adapted, evolving from concepts of 'rescue' and 'salvage', to structured, mandated archaeological mitigation. This research is the first stand-alone prehistoric examination of Mayo as a region. The results show that development-led archaeological activity in Mayo has been transformative, providing a critical mass of well-dated settlement evidence, impacting every period of Mayo's prehistory. It provides the first indications of Early Mesolithic activity in Mayo, refining Neolithic periodisation and expanding domestic settlement evidence. It reveals empirical Chalcolithic data, a vast and varied range of Bronze Age settlement indicators and the first block of datable Iron Age settlement evidence from Mayo. The information in this PhD thesis is a research contribution of broad interest especially to those working on the prehistory of Mayo and the west of Ireland. The data contained in this volume can be used and built upon into the future, feeding into wider prehistoric research in Ireland and beyond.

The careers of the Mayo brothers, Harvey Cushing, and Sir William Osler greatly shaped medical and surgical practice in the late 19th century and early 20th century and created a legacy to influence decades of physicians to follow. Additionally, these individuals were instrumental in the founding of neurosurgery as a distinct surgical specialty. Alongside these great men, Dr. Albert L. Rhoton Jr., revolutionized neurosurgical practice through his study of neuroanatomy and development of microsurgical technique in the second half of the 20th century. This review of the interactions and relationships between the Mayo brothers, Cushing, and Osler and their influences on Rhoton highlights the 100-year-long interconnectedness shared between these giants in the history of neurosurgery. 4. Nibbling MAYO: Optimized Implementations for AVX2 and Cortex-M4

MAYO is a popular high-calorie condiment as well as an auspicious candidate in the ongoing NIST competition for additional post-quantum signature schemes achieving competitive signature and public key sizes. In this work, we present high-speed implementations of MAYO using the AVX2 and Armv7E-M instruction sets targeting recent x86 platforms and the Arm Cortex-M4. Moreover, the main contribution of our work is showing that MAYO can be even faster when switching from a bitsliced representation of keys to a nibble-sliced representation. While the bitsliced representation was primarily motivated by faster arithmetic on microcontrollers, we show that it is not necessary for achieving high performance on Cortex-M4. On Cortex-M4, we instead propose to implement the large matrix multiplications of MAYO using the Method of the Four Russians (M4R), which allows us to achieve better performance than when using the bitsliced approach. This results in up to 21% faster signing. For AVX2, the change in representation allows us to implement the arithmetic much faster using shuffle instructions. Signing takes up to 3.2x fewer cycles and key generation and verification enjoy similar speedups. This shows that MAYO is competitive with lattice-based signature schemes on x86 CPUs, and a factor of 2-6 slower than lattice-based signature schemes on Cortex-M4 (which can still be considered competitive).

6. Retrospective comparison of conservative treatment and surgery for widely displaced olecranon fractures in low-demanding geriatric patients.

Introduction: The aim of this study was to evaluate the difference of the clinical outcome of elderly patients who were treated surgically or conservatively for a displaced olecranon fracture (Mayo type IIA or IIB). Patients and methods: Patients above the age of 70 years who were treated surgically (n = 11) for a displaced Mayo type IIA and IIB olecranon fracture between July 2015 and February 2019 were retrospectively compared with patients who were treated conservatively (n = 6). The range of motion, elbow strength, grip strength, VAS, DASH, OES, MEPI and Broberg and Morrey scores were evaluated. Results: The conservative group showed a non-union with a persistent fracture gap of 17 mm (SD 12 mm) at the articular rim and 31 mm (15 mm) at the dorsal rim while there was no case of non-union in the surgical group. The arch of motion was 120° in the conservative group and 136° in the surgical group. There was no obvious difference in elbow extension strength in comparison to the healthy contralateral side (p = 0.20; 88% group I/87% group II). There was no difference in the OES (p = 0.30; 42 (SD 7) vs. 45 (SD 5)) and MEPI score (p = 0.46; (SD 8) vs. 96 (SD 19)). The conservative group presented a slightly worse DASH [p = 0.10; 26 (SD 25) vs 7 (SD 14)] and a significantly worse Broberg and Morrey score (p = 0.02; 84(SD 9) vs. 95 (SD 7)). The conservative group presented one complication (ulnar nerve palsy), while the surgical group presented two cases (prolonged lymphedema; blocked forearm rotation due to screw length with consecutive revision surgery). Conclusion: Widely displaced olecranon fractures can successfully be treated conservatively in low-demanding geriatric patients with a satisfactory outcome. Patient selection is essential as patients that are more active might benefit from surgical treatment. Summary: The presence of personally identifiable information (PII) in natural language portions of electronic health records (EHRs) constrains their broad reuse. Despite continuous improvements in automated detection of PII, residual identifiers require manual validation and correction. Here, we describe an automated de-identification system that employs an ensemble architecture, incorporating attention-based deep-learning models and rule-based methods, supported by heuristics for detecting PII in EHR data. Detected identifiers are then transformed into plausible, though fictional, surrogates to further obfuscate any leaked identifier. Our approach outperforms existing tools, with a recall of 0.992 and precision of 0.979 on the i2b2 2014 dataset and a recall of 0.994 and precision of 0.967 on a dataset of 10,000 notes from the Mayo Clinic. The de-identification system presented here enables the generation of de-identified patient data at the scale required for modern machine-learning applications to help accelerate medical discoveries. The bigger picture: Clinical notes in electronic health records convey rich historical information regarding disease and treatment progression. However, this unstructured text often contains personally identifiable information such as names, phone numbers, or residential addresses of patients, thereby limiting its dissemination for research purposes. The removal of patient identifiers, through the process of de-identification, enables sharing of clinical data while preserving patient privacy. Here, we present a best-in-class approach to de-identification, which automatically detects identifiers and substitutes them with fabricated ones. Our approach enables de-identification of patient data at the scale required to harness the unstructured, context-rich information in electronic health records to aid in medical research and advancement.

Aim: The aim of this research was for knowing and testing: 1) weight reduction after done the diet for 13 days; 2) the effect of mayo and keto diet towards consumer’s physical activity; and 3) interaction between diet and physical activities. Methods: The reseach method that used is experimental with the use of 2x3 factorial design. Sample for this research was whole population which is 30 people of dandelion catering consumer, starting from junior high school students to house wives. The instrument used for measuring abdominal circumference was meter, for measuring weight was scale, and for measuring the fat thickness used skinfold. Analysis of this research was using ANOVA two paths with the score of significant level α= 0,05. Results: The result of this research is: 1) The effect of keto and mayo diet towards weight, waist circumference, and fat thickness reduction, in significant level of < α 0,05; 2) diet effect (mayo and keto) and physical activity toward weight, waist circumference, and fat thickness reduction, in sigficant score for weight 0.706 > α 0,05, for waist circumference 0.631 > α 0,05, and for fat thickness . 710 > α 0,05. Conclusion: The conclusion towards this reserach was: 1) there was a significant different effect between keto and mayo diet towards weight, waist circumference, and fat thickness reduction; 2) there was no different significant effect between diet (mayo and keto) and physical activity towards weight, waist circumference, and fat thickness reduction; and 3) there was no interaction between diet (keto and mayo) and activitiy (light, medium, high) towards weight, waist circumference, and fat thickness reduction.

Introduction: A fracture classification system should provide a reliable and reproducible means of communication between different parties. It should be logical and understandable, with few categories to memorize. The aim of this study was to determine the intra- and interobserver reliability of the Schatzker and Mayo classification systems for the assessment of proximal ulna fractures., Materials and Methods: Intra- and interobserver reliability studies were conducted on 39 X-rays of injured elbows drawn randomly from 74 cases previously used in a series on predictors of ulnohumeral osteoarthritis in proximal ulna fractures. Ten observers independently reviewed these X-rays on 2 separate occasions 3 months apart. The fracture type was assessed according to the Schatzker and Mayo classification systems during each reading session. Cohen's and Fleiss' kappa were used to measure the intra- and interobserver reliability., Results: The Schatzker classification had a fair interobserver reliability for the first (Schatzker R
1 , Fleiss' κ: 0.394) and second (Schatzker R2 , Fleiss' κ: 0.351) readings. The mean intraobserver reliability value between the 10 reviewers for the Schatzker classification was rated as substantial (0.61). The Mayo classification had a fair interobserver reliability for the first (Mayo R1 , Fleiss' κ: 0.278) and second (Mayo R2 , Fleiss' κ: 0.292) readings. The mean intraobserver reliability value between the 10 reviewers for the Mayo classification was rated as fair (0.52)., Discussion: The classification systems for proximal ulna fractures showed poor reproducibility between the different observers since they had low interobserver agreement values. In several issues of the London Medical Gazette during June–July of 1837 there was an interchange of letters between Robert Graves, Regius Professor of the Institutes of Medicine at Trinity College, Dublin, and the London physician and experimental physiologist Marshall Hall, often considered the discoverer of the phenomenon of reflex activity. Graves asserted that he, rather than Hall, was the originator of the idea of reflex action as a disease mechanism. Hall rejected that assertion and, after exchange of some verbal "pleasantries," began a tirade about a somewhat different, although not unrelated issue into which the journal editor interjected some not exactly dispassionate comments. Graves soon let the matter of priority lapse, and Hall continued his war with the Council of the Royal Society, but examination of the contemporary and earlier literature suggests that Graves probably was correct, by a narrow time margin, in relation of his claim for priority in using the concept of reflex action in explaining neurological disease mechanisms (not a claim for discovering reflex action), that Hall had used the phrase "reflex action" earlier than Graves, and that others before Hall had gone a long way in studying reflex mechanisms, although Hall's writings had familiarized the medical profession with the concept. In this article we analyze the interaction between the Grandmothers of the Plaza de Mayo and the transnational advocacy networks for human rights during the years of the last military dictatorship in Argentina. In the first place, we analyze the main characteristics and actors of these networks and reconstruct the process and the modalities of insertion of the Grandmothers of the Plaza de Mayo in them. Secondly, we inquire into the repertoire of action and the different forms of collaboration of international human rights organizations with the Argentinian group. Finally, we discuss the medium and long-term effects that these relationships had for the Grandmothers of the Plaza de Mayo, both for the dissemination of its cause and for its consolidation as an independent organization. The landscape for patenting products and processes tied to the natural world has changed dramatically in recent times as a result of a series of decisions of the US Supreme Court, particularly Mayo Collaborative Services v Prometheus Laboratories 566 U.S. 66 (2012) and Association for Molecular Pathology v Myriad Genetics, Inc. 569 U.S. 576 (2013) (Myriad). This article critically analyses these decisions and the multitude of lower court decisions that have followed them. This analysis provides support for the growing concern in the United States that it will be increasingly difficult to use the patent system to encourage the development of therapies and research intermediates useful in developing new therapeutic interventions. One option being posited in the industry to deal with this problem is to lobby Congress to reform the threshold patent eligibility standard in US patent law. It is argued in this paper that a more nuanced approach is preferable. Using the experience in Australia as a case study, this paper argues that such an approach is feasible. Australia has been chosen for analysis because the threshold patent eligibility standard is similar in both countries, much more so that with the European Union, and because the highest court in Australia has ruled on essentially the same patent as in Myriad, in D'Arcy v Myriad Genetics, Inc [2015] HCA 35. In addition to the nuanced approach to eligibility currently exercised by the Australian courts and patent office, Australia also has a number of post-grant options for addressing the dynamics of patent monopolies. These include experimental use, compulsory licensing, and government use. It is concluded that, while it would be impractical to attempt to replicate the Australian environment in the United States, there is no reason why some lessons can't be learned from the Australian experience with patenting nature. 23. Ethnobotany of the Yoreme People and Environs

The Yoreme people of northwestern Mexico have a vibrant body of traditional knowledge and practices (TKP) related to natural landscapes amidst drastic technological changes in the region. This apparent contrast presents an exemplar model system to conduct ethnobotanical studies on traditional knowledge and practice dynamics. Using a range of methods from in person surveys, vegetation surveys, geographic data analysis, and biochemical assays, I sought to gain insight as to how TKP change and adapt. This research lead to four main findings within the concept that I call traditional knowledge and practice remodeling. (1) Traditional herbalists adapt to online shopping trends and are moving their storefronts online. (2) Local pitaya-product producers create value-added fruit products to stay relevant in an increasingly competitive market. (3) Ancestral use and phylogenetic studies can lead to hypotheses and assays on the mode and mechanism of action for local medicinal plants such as Ibervillea sonorae. Mechanistic studies show how I. sonorae inhibits α-glucosidase and stimulates insulin secretion in vitro. Mechanistic studies can help strengthen cultural moral, help physicians plan treatment programs for patients who consume herbal medicine, and provide literature support for cultural initiatives and funding such as medicinal plant gardens. (4) While ethnobotanists often study TKP dynamics and even hold community events to strengthen TKP, little follow-up data is collected. Museum evaluation techniques should be implemented to strengthen the data pool of TKP conservation to refine the current and general ideas that time on the land with elders is the best way to transmit TKP. In all, my research has shown that TKP remodeling is driven by the needs and opportunities of community members. Given the large pool of talent in the Yoreme villages I visited and the innovative ideas of herbalists and artisan product producers, I propose that networking tools from social entrepreneurship incubators would help bolster new ideas and move them from an idea of a few individuals into a broad scale reality. This may also give more voice to those who want to preserve land and keep traditional practices active and vibrant.

Aim: To evaluate various clinical, endoscopic and histological parameters in assessing disease activity and to find out various risk factors involved in the exacerbation of ulcerative colitis especially the role of Cytomegalo Virus (CMV) infection. Materials and Methods: It was a prospective study of patients diagnosed as ulcerative colitis presenting with acute exacerbation of symptoms (cases) and those who were in remission (controls). A detailed evaluation of the disease history including personal history, treatment compliance and clinical disease severity were noted. Investigations including blood routine, endoscopic examination with biopsy, histopathological examination and immunohistochemistry for CMV were done on the biopsy sample. Results: A total of 58 patients with ulcerative colitis were studied which included 37 cases and 21 controls. Out of the various clinical and demographic parameters, Good treatment compliance (p =0.0003) and Perceived Stress Scale (PSS) score (p=0.0001) showed significant difference between cases and controls. Basic laboratory parameters {Haemoglobin level, Total Leucocyte Count (TLC) and Erythrocyte Sedimentation Rate (ESR)}, clinical disease severity predictors (Truelove and Witt’s criteria, Mayo score and endoscopic disease severity grade) and Geboes histological scoring showed significant difference between cases and controls. The prevalence of CMV colitis in our study was only 5.4% (two cases). Conclusion: Clinical and endoscopic disease severity indicators can be used as predictors of histological activity in ulcerative colitis. Poor treatment compliance and stress are important risk factors for acute exacerbation of ulcerative colitis. Clinicians should be aware of the possibility of concurrent CMV infection while treating patients with acute exacerbation of ulcerative colitis not responding to the conventional management. Reduced prevalence of CMV colitis in cases of acute exacerbation of ulcerative colitis in our study may be due to the small sample size, reduced number of steroid dependent cases or reduced severity of our cases.

¿Cómo sería la vida de los sonorenses sin el mezcal? La interrogante guía el propósito general del trabajo, orientado a identificar la importancia que tradicionalmente ha revestido el Agave angustifolia en la vida cotidiana de los sonorenses. En su mayor parte, los estudios existentes en Sonora en torno a esta especie se enfocan, básicamente, en su uso para la elaboración industrial del licor, popularmente conocido como bacanora. La investigación pretende contribuir, en alguna medida, al mayor conocimiento del valor biológico y cultural de la planta. Con ese objetivo se recurre a la etnobotánica para obtener una mirada más completa de las formas en las que los hombres y mujeres se han relacionado con el agave. La región de estudio comprende dos zonas mezcaleras: la sierra orien-te, donde la mayor parte de la población es mestiza, y la zona sur, donde los grupos indígenas Mayo y Guarijío cohabitan con mestizos. Se realizaron entrevistas a profundidad a diversos pobladores, así como a investigadores y otros estudiosos de la planta. Se consultaron las fuentes históricas disponibles. Los resultados muestran la existencia de diferencias regionales en los usos del agave y técnicas de preparación, determinadas por un número de factores estrechamente vinculados con el grupo étnico de los pobladores y su historia.

La indagación sobre el nacionalismo en el posperonismo en Mendoza nos ha permitido recuperar una serie de itinerarios intelectuales que dan cuenta de la complejidad de un período, en el que la circulación y tránsito de ideas evidencian no sólo fuertes procesos de mutación de identidades, sino también la cristalización de posiciones ciertamente anacrónicas. En este trabajo pretendemos relevar a propósito de la celebración del sesquicentenario de Mayo la producción historiográfica mendocina que circuló en espacios de divulgación, constituyéndose en una fuerte tradición que perduró en los espacios académicos locales. Más allá de explicar la incidencia que tuvieron en la refracción de otras corrientes de pensamiento, las tesis interpretativas que ven a Mayo como un momento de reafirmación de la hispanidad y que niegan su sentido emancipatorio, así como los recorridos biográficos de sus autores, revelan redes de sociabilidad intelectual que bajo la nominación de nacionalismo encubrían no pocas especificidades, al mismo tiempo que revelaban las vinculaciones existentes con la escuela historiográfica sevillana, por entonces usina intelectual del franquismo y nutriente del pensamiento nacionalista conservador y autoritario de los años sesenta.Research on the nationalism present in post- Peronism in Mendoza has allowed us to retrieve a number of intellectual itineraries that exemplify the complexity of a period in which the circulation and traffi c of ideas show not only strong identity mutation processes, but also the crystallization of undoubtedly anachronistic positions. In this article we aim to survey -in the context of the sesquicentennial of the May Revolution celebrations-, the important historical production that circulated in zones of wide diffusion, and which contributed to implant a powerful tradition that lasted for a considerable time in the local academic scene. Rather than explaining the impact that they had on the refraction of other schools of thought, interpretative theories that see May as a reaffi rmation of Spanishness and a negation of its emancipatory sense -as well as their authors' biographical trajectories- they reveal an intellectual sociability that under the rubric of nationalism, veiled many internal differences, just as they at the same time revealed strong connections with the Sevillian school of historiography, then an intellectual engine of Francoism and a buttress of the conservative, authoritarian Nationalist thought of the Nineteensixties.

Los chiles son especies vegetales que tienen gran trascendencia en la cultura e identidad del pueblo de México. El chiltepín, especie silvestre de la sierra de Sonora, ocupa un lugar significativo en la cultura de los sonorenses. Sin embargo, los estudios sobre esta especie son escasos y unidimensionales. El presente trabajo tiene como principal objetivo destacar la importancia del chiltepín en la vida de los sonorenses. Con base en la distribución geográfica del chiltepín, se seleccionaron dos poblaciones: una ubicada en el río de Sonora y otra en la sierra de Álamos, áreas donde crece y se recolecta. El estudio de campo en estas dos áreas sirvió de base para el mejor conocimiento de la relevancia de este chile entre los serranos y dos grupos indígenas: los mayo y los guarijío. Entre las herramientas metodológicas utilizadas se incluyen la observación directa y participante, entrevistas personales a profundidad con hombres y mujeres de mayor edad, recolectores de chiltepín, curanderos y amas de casa. Algunos resultados demuestran que el chiltepín trasciende con facilidad distintos espacios de la vida cotidiana: alimenta, cura, protege, invade la cocina, el trabajo, las fiestas y las actividades recreativas.

33. Involvement of the lesser sigmoid notch in elbow fracture dislocations.

Background This study addressed the primary null hypothesis that there is no difference in the articular surface area of the lesser sigmoid notch involved among Mayo classes. Secondarily, we analyzed the fracture line location and the pattern of lesser sigmoid notch articular surface involvement among Mayo classes. Methods Using quantitative 3-dimensional computed tomography, we reconstructed and analyzed fractures involving the lesser sigmoid notch articular surface in 52 patients. Further, we assessed the surface area involved in the fracture, the number of fracture fragments, and the location and direction of the fracture lines. Coronoid fractures were classified according to Mayo types. Results There was no significant difference between Mayo types 1 and 2 in any characteristic of the involvement of the lesser sigmoid notch articular surface, whereas Mayo type 3 was significantly different from both Mayo types 1 and 2 in the area involved in the fracture (42% in Mayo type 3 vs. 9% in Mayo types 1 and 2), the number of articular fragments (>3 fragments in type 3 vs. 2 fragments in types 1 and 2), and the direction of fracture line (both horizontal and vertical lines in type 3 vs. only horizontal line in types 1 and 2). Conclusion Mayo type III results in a more complex fracture, which might need to be addressed directly or indirectly during open reduction with internal fixation of olecranon fracture dislocations because changes in the geometry of lesser sigmoid notch may affect the radioulnar joint if it remains incongruent. Ethnopharmacological relevance Tuberculosis (TB) is an infectious disease mainly caused by Mycobacterium tuberculosis ( Mtb ), which generates 9 million new cases worldwide each year. The Mayo ethnicity of southern Sonora, Mexico is more than 2000 years old, and the Mayos possess extensive knowledge of traditional medicine. Aims of the study To evaluate the antimycobacterial activity levels of extracts of medicinal plants used by the Mayos against Mtb and Mycobacterium smegmatis ( Msm ) in the treatment of TB, respiratory diseases and related symptoms. Materials and methods A total of 34 plant species were collected, and 191 extracts were created with n -hexane, dichloromethane, ethyl acetate (EtOAc), methanol and water. Their minimum inhibitory concentrations (MICs) and minimum bactericidal concentrations (MBCs) were determined against Mtb H37Rv using the microplate alamar blue assay (MABA) and against Msm using the resazurin microplate assay (REMA) at 6 and 2 days of exposure, respectively, and at concentrations of 250–1.9 µg/mL ( n- hexane extracts) and 1000–7.81 µg/mL (extracts obtained with dichloromethane, EtOAc, methanol and water). Results Rhynchosia precatoria (Willd.) DC. ( n -hexane root extract), Euphorbia albomarginata Torr. and A. Gray. (EtOAc shoot extract) and Helianthus annuus L. ( n- hexane stem extract) were the most active plants against Mtb H37Rv, with MICs of 15.6, 250, 250 µg/mL and MBCs of 31.25, 250, 250 µg/mL, respectively. R. precatoria (root) was the only active plant against Msm , with MIC and MBC values of ≥250 µg/mL. None of the aqueous extracts were active. Conclusions This study validates the medicinal use of certain plants used by the Mayo people in the treatment of TB and related symptoms. R. precatoria , E. albomarginata and H. annuus are promising plant sources of active compounds that act against Mtb H37Rv. To our knowledge, this is the first time that their antimycobacterial activity has been reported. Crude extracts obtained with n- hexane, EtOAc and dichloromethane were the most active against Mtb H37Rv. [ABSTRACT FROM AUTHOR]

Sample for this research was whole population which is 30 people of dandelion catering consumer, starting from junior high school students to house wives. The instrument used for measuring abdominal circumference was meter, for measuring weight was scale, and for measuring the fat thickness used skinfold. Analysis of this research was using ANOVA two paths with the score of significant level α= 0,05. Results: The result of this research is: 1) The effect of keto and mayo diet towards weight, waist circumference, and fat thickness reduction, in significant level of < α 0,05; 2) diet effect (mayo and keto) and physical activity toward weight, waist circumference, and fat thickness reduction, in sigficant score for weight 0.706 > α 0,05, for waist circumference 0.631 > α 0,05, and for fat thickness . 710 > α 0,05. Conclusion: The conclusion towards this reserach was: 1) there was a significant different effect between keto and mayo diet towards weight, waist circumference, and fat thickness reduction; 2) there was no different significant effect between diet (mayo and keto) and physical activity towards weight, waist circumference, and fat thickness reduction; and 3) there was no interaction between diet (keto and mayo) and activitiy (light, medium, high) towards weight, waist circumference, and fat thickness reduction. 38. Malunited extra-articular distal radius fractures: corrective osteotomies using volar locking plate.

Background: Multiple techniques for corrective osteotomy have been developed in recent years with the same aims: to improve the radiographic parameters and improve motion, pain and grip strength. Volar fixed-angle plates have added a new concept to the treatment of distal radius fractures thanks to the low morbidity of the surgical approach and the strength of the final construct, allowing early mobilization and return to function. Materials and methods: Between 2005 and 2012, 20 patients with symptomatic dorsally malunited extra-articular fractures of the distal radius underwent corrective osteotomy using a volar locking plate without additional bone graft. At a mean follow-up of 50 months, all the patients were clinically and functionally evaluated. Results: All measurements of pain, final range of motion and grip strength significantly improved compared with preoperative measurements. The mean preoperative DASH score reduced from 54 points preoperatively to 25 postoperatively. Based on the modified Mayo wrist score, we obtained 14 excellent and six good results. Palmar tilt improved from an average of 23° to 11°. Radial inclination improved from an average of 29° to 22°, and ulnar variance decreased from an average of 3.6 mm to 0.9 mm. There were two cases of transient median neuroapraxia that resolved before the 6-week follow-up appointment. No other major complications, including non-union and infection, were observed. Conclusion: The volar approach and locking plate, without necessarily the use of bone grafting, proved to be an effective approach for addressing symptomatic and even severe deformities of the distal radius. The budget was a fair budget. It was an honest Budget – it paid its way. It laid down the important and far-reaching principle that extra taxation ought to fall on those who can afford to pay. It removed the unjust privileges which landlords have possessed in the past. support the party which carried this democratic budget. Liberalism must re-unite itself with the Labour interest. Until that is done we cannot look for much success … The programme of the Liberal party must, therefore, be so altered as to include those items of legislation for which the industrial classes are striving. Radicals parting ways Although Chamberlain was rapidly marginalized within the radical left after 1892, his ‘materialist’ approach to politics – the priority of social reform – and emphasis on parliamentary centralism, in the conviction ‘that the day of Local Parliaments and of small nationalities is past’, were to have enormous impact on twentieth-century radical politics. If ‘modern’ radicalism was about ‘the social question’, and if poverty was to be reduced by government action, then the country needed the rational reconstruction and empowerment of the imperial executive at its centre, rather than legislative devolution. The Labourers won't pull with them and though these are a weak body in Ireland they may be enough to form a New party in alliance with Landlords of a Liberal type. When, said Mr Parnell, it was conceded to us as one of the principles of the Irish Party that it was the right of the Irish people to be governed by the people, for the people and in accordance with the will of the majority of the people, we gladly recognised that that was our principle, and ‘upon that principle we cordially shake hands with you, and we wish long life to the Liberal Party in their career of self-Government for Ireland, and justice to the English people’. The roots of Irish ‘popular liberalism’ ‘Legislative and administrative decentralization is one Irish idea’, Reynolds's commented in 1888, ‘and the abolition of landlordism is another. Introduction: The purpose of this retrospective study is to compare the clinical, functional outcome and complications occurrence between tension band wiring (TBW) and plate fixation both for simple and comminuted displaced olecranon factures. Materials and methods: Between January 2005 and June 2012 (minimum 1-year follow-up), 78 consecutive patients with Mayo type IIA and IIB fractures were treated with the following methods: tension band wire or plate and screws fixation. The primary outcome of this study was the functional outcome, assessed by the following self-administered evaluation scales: the disabilities of the arm, shoulder and hand, and the Mayo Elbow Performance Score. Secondary outcome measures included the assessment of pain level, analysis of passive range of motion, and the occurrence of any early or late complications. Results: Comparing the clinical results between the two groups, at mean 33 months follow-up, no significant differences in the functional and clinical outcome were observed. Complications were reported in 48 and 17 % of cases, following TBW and plate fixation in patients treated for type IIA fractures, and similarly in 40 and 23 % of cases in type IIB fractures, respectively. Hardware removal was more frequently performed in TBW group: 38 versus 17 % for type IIA fractures and 20 versus 6 % for type IIB fractures. Conclusions: The findings of this study indicate that both with the use of TBW and plate fixation excellent/good clinical outcomes with minimal loss of physical capacity, little pain and disability can be obtained in the majority of patients with simple and comminuted displaced olecranon fractures. In June 2013, the U.S. Supreme Court issued a unanimous decision upending more than three decades worth of established patent practice when it ruled that isolated gene sequences are no longer patentable subject matter under 35 U.S.C. Section 101. While many practitioners in the field believed that the USPTO would interpret the decision narrowly, the USPTO actually expanded the scope of the decision when it issued its guidelines for determining whether an invention satisfies Section 101. The guidelines were met with intense backlash with many arguing that they unnecessarily expanded the scope of the Supreme Court case in a way that could unduly restrict the scope of patentable subject matter, weaken the U.S. patent system, and create a disincentive to innovation. By undermining patentable subject matter in this way, the guidelines may end up harming not only the companies that patent medical innovations, but also the patients who need medical care. Background: The treatment of radial neck fractures in children varies according to the displacement, angulation, and skeletal maturity. There is a general agreement that displaced radial neck fractures with more than 30° angulations (Judet type III and IV fractures) should be surgically treated. There are several treatment possibilities for Judet type III and IV fractures including percutaneous pin reduction, elastic stable intramedullary nailing (ESIN), and open reduction with or without internal fixation. In this retrospective study we compared the clinical and radiographical outcomes, and complications following intramedullary versus percutaneous pinning in displaced radial neck fractures in children. Materials and methods: Between 2000 and 2011, 20 patients were treated using closed reduction: in 12 cases we used percutaneous pinning, and in 8 cases we used ESIN. According to Judet classification the two groups were composed as follows: 10 (77 %) type III and 3 (23 %) type IV fractures in the percutaneous pinning group; 4 (57 %) type III, and 3 (43 %) type IV fractures in the ESIN group. Results: After an average of 42 months, excellent results in Mayo elbow performance scores (MEPS) were obtained in 71 and 69 % of ESIN and percutaneous pinning groups respectively, with good results in the remaining cases apart from one fair case (8 %) in the percutaneous pinning group. After a radiological evaluation, all fractures healed in excellent or good alignment. When comparing the two groups, the subjects treated with the ESIN technique had higher range of motion (ROM) in flexion, extension and pronation. No patients developed complications, except three cases of asymptomatic enlargements of the radial head, reported only in the percutaneous pinning group. Conclusion: In this research the clinical outcome, assessed with the MEPS, and the radiological alignment, were comparable between the subjects that were treated with percutaneous pinning and those with ESIN techniques; whereas the ESIN technique demonstrated higher ROM in flexion, extension and pronation. The ESIN technique seems to be the ideal approach both for the higher ROM values and for the absence of complications. [ABSTRACT FROM AUTHOR]

How life would be for Sonorans without mescal? This question drives the general purpose of this work, focused on the identification of the importance of Agave angustifolia Haw in Sonorans' everyday lives. Most studies on this species in Sonora address almost only its use for the industrial production of an alcoholic beverage, popularly known as Bacanora. Thus, this research intends to contribute to some extent to the knowledge of the biological and cultural values of this plant. In doing so, ethnobotanics is considered as a fundamental basis in order to get a more comprehensive vision of the ways that men and women have related to this agave. The study region is divided into two mescal areas: the Sierra Oriente, where most residents are mestizos, and the southern region, where mestizos and the indigenous people Mayo and Quarijio cohabitate. In depth interviews to local residents were applied in these territories, including also interviews to researchers and other experts. Available historical sources were consulted as well. The results indicate the existence of regional differences in the uses and methods of preparation of this agave, defined by a number of factors closely related to the ethnicity and history of the local residents. Aim: Short-stemmed prostheses are increasingly regarded as implants of first choice in primary THA. As a result of the press-fit fixation in the femoral metaphysis, the occurrence of intraoperative fractures were reported. The aim of this study was to analyze the postoperative results of the Mayo™ short-stem prosthesis following treatment of an intrasurgical femur fracture with cerclage wiring. Patients and methods: From 1999 to 2005, in 38 patients (18 females, 20 males; mean age 56 years; mean BMI = 27) with the diagnosis of coxarthrosis in whom a Mayo™ short-stemmed prosthesis has been implanted, an intraoperative fracture was observed. The fractures were treated with cerclage wiring (1 cerclage, n = 32; 2 cerclages, n = 5; 3 cerclages, n = 1). Postoperatively, all patients were prescribed mobilization without weight-bearing (floor contact) on the treated leg for 6 weeks. Using the Wristing software, longitudinal stem migration and varus-valgus femoral stem alignment were examined digitally in anteroposterior X-rays taken immediately after surgery, after 6 weeks and on average after 5.7 years (Zeh et al., Z Orthop Unfall 149:200-205, ). Additionally, the incidence of periprosthetic radiolucent lines was captured in the anteroposterior X-rays and assigned to the Gruen zones. Additionally, a DEXA scan was performed. The X-rays of a matched control group after the implantation of a Mayo™ prosthesis without femur fracture were analyzed by the same method. Results: There was no significant migration of the Mayo™ prosthesis in the study or control groups during postoperative follow-up ( t test, P > 0.05). The cerclage group compared with the control group showed a statistically significant valgus tilt of 1.5° on average during the follow-up, which is regarded to be clinically not relevant. The frequency of occurrence of radiolucent lines was not statistically different (chi-square test, χ = 0.42, P = 0.51). DEXA scans showed no differences of the bone mineral density in the Gruen zones compared with a historical control group. Conclusion: After wiring of an intrasurgical fracture, no disadvantage could be proven for Mayo™ prosthesis regarding stem migration and varus-valgus alignment. Furthermore, due to the absence of differences in the occurrence of radiolucent lines and the same results in the DEXA scan, an unimpaired osseointegration is assumed. [ABSTRACT FROM AUTHOR]

Introduction This study has endeavoured to explore the ways that the Irish agricultural economy responded to the various changes in the internal and external economic circumstances which confronted it in the second half of the nineteenth century and the first decade or so of the twentieth century. In isolation from the international world, and solely in comparison with the rest of the UK, the Irish agricultural industry seemed to respond positively to economic stimuli, and looked in an increasingly healthier position than its larger neighbour across the Irish Sea. When the comparisons were widened to embrace countries on the European mainland some of that initial enthusiasm for the Irish performance could still be maintained, though in a diluted form, but in specific comparison with some of the leading Western European economies the light which appeared to emanate from the Irish industry dimmed considerably. The models which have been used in the central chapters above were related to considerations of gross agricultural income and physical output. In this final chapter of conclusions we pose what has become a fundamental question in nineteenth-century Irish economic history: to whose benefit did the structural changes in Irish agriculture accrue? Was it a question of a drain of benefits back to the colonial master? Of course not. Instead, after the Famine, slowly perhaps but inexorably, colonialism as a force of economic power (as distinct from a political reality) was turned on its head. Much of Ireland earned its economic independence by 1914, and soon afterwards full political independence followed. [ABSTRACT FROM AUTHOR]
