Introduction. Chronic mesenteric ischemia is a rare entity, with a prevalence of 0.03%, where more than 90% are due to arteriosclerotic disease that mainly affects the superior mesenteric artery. Its symptoms are chronic abdominal pain and weight loss, associated with imaging alterations that make the diagnosis. Treatment depends on the patient's clinical conditions and the number of vessels involved. It is clear that symptomatic multivessel disease requires revascularization. Clinical case. A 67-year-old woman, an active smoker with a history of high blood pressure and dyslipidemia, presented with an emergency-type hypertensive crisis that required management in the Intensive Care Unit. A diagnosis of Crawford IV thoracoabdominal aneurysm, aorto-iliac occlusion (TASC D), and chronic occlusion of the celiac trunk and superior mesenteric artery was made. Due to the symptoms of chronic mesenteric ischemia, she underwent surgical treatment with aorto-bifemoral bypass plus retrograde bypass to the superior mesenteric artery via an open approach. Result. The patient had improvement in symptoms and gained 13% weight at 3-month follow-up. Conclusion. Chronic mesenteric ischemia is an underdiagnosed condition, a marker of cardiovascular risk, with a high burden of morbidity and mortality, in which, with early identification, revascularization therapy can be offered, either endovascularly or open, in order to improve quality of life and weight gain, and avoiding intestinal necrosis. Introduction. Cystic pneumatosis intestinalis is described as the presence of gas within the intestinal wall. It is a rare entity, with an incidence of 0.03% in the global population. It appears with a predilection in the male gender after 45 years of age and is located mainly in the small intestine (42%) and the colon. It can be associated with several conditions that sometimes require surgical management. Clinical case. The case of a 75-year-old man with a history of high blood pressure is presented, who consulted for a 15-day history consisting of abdominal distention, pain and constipation. In the emergency room, a chest x-ray was requested, which showed pneumoperitoneum and several air-fluid levels. The surgeon on call considered a possible rupture of the hollow viscus. Results. The patient was taken to exploratory laparotomy, where intestinal cystic pneumatosis and a greatly enlarged stomach were identified, compatible with gastroparesis. As a result of the treatment provided, the patient had a satisfactory outcome, achieving medical discharge, supported with basic nursing care. Conclusions. Although cases of intestinal cystic pneumatosis have an unusual presentation, it can be found in patients with imaging findings of pneumoperitoneum. Therefore, a thorough analysis of each patient must be carried out and the case individualized for the correct diagnosis. [ABSTRACT FROM AUTHOR]

La isquemia mesentérica aguda es consecuencia de la interrupción repentina del suministro de sangre al intestino, de no tratarse, da lugar a complicaciones y daños irreversibles tales como la necrosis de la pared intestinal y la muerte. Las diversas causas y los síntomas clínicos inespecíficos dificultan el diagnóstico, lo que resulta en un diagnóstico tardío y una intervención terapéutica tardía. Constituye un gran desafío clínico ya que no están definidas las pautas para el manejo terapéutico de estos pacientes. Se presenta el caso clínico de un paciente joven con dolor abdominal secundario a isquemia mesentérica.

Introduction. Paradoxical embolism is a thrombotic event originating in the venous circulation, which manifests as arterial embolism through an anatomical cardiac or pulmonary defect. It is mainly associated with stroke, also presenting as acute myocardial infarction, renal infarction, and other ischemic events. Clinical case. A 47-year-old patient was admitted due to abdominal pain, which required emergency surgical management, finding significant intestinal ischemia. Additional studies found protein S deficiency and evidence of a patent foramen ovale. Discussion. Arterial thrombosis is known as unusual thrombosis; this situation requires to rule out associated thrombophilia or other prothrombotic diseases. Protein S deficiency is a rare thrombophilia, which in adults causes venous thrombosis. In the presence of anatomical defects, such as a patent foramen ovale, it can progress to arterial embolism, presenting a picture of paradoxical embolism. The study work of these patients requires imaging that demonstrates the aforementioned defect, as well as the possible origin of the emboli. Management is based on full anticoagulation, treatment of existing thrombotic manifestations, and management of the anatomical defect. Conclusions. Paradoxical embolism should be suspected in case of unusual thrombosis. It requires exhaustive studies based on imaging, and management should consist of anticoagulation and closure of the defect. The article titled "Multiple colonic phytobezoar as a cause of cecal volvulus in the critically ill adult" describes a clinical case of a 33-year-old exclusive vegan patient who was admitted to the Intensive Care Unit due to an acute cerebral hemorrhagic event. The patient presented symptoms of partial intestinal obstruction and a tomography was performed, revealing severe dilation of the cecum. An exploratory laparotomy was carried out, in which multiple phytobezoars were found in the transverse and left colon, resulting in a right hemicolectomy. Introduction. Hydatidosis or echinococcosis is a parasitic zoonosis that is acquired by ingesting eggs of cestodes of the genus Echinococcus. Clinical diagnosis is rarely made in non-endemic sites. Most patients are asymptomatic and incidental findings on imaging studies or surgical procedures allow for diagnostic suspicion. Clinical case. A 70-year-old male patient, resident in a rural area of the municipality of Puerto Libertador, department of Córdoba, Colombia, who consulted for an abdominal mass in the epigastrium and mesogastrium, partially mobile and painless. Results. In surgery, mesenteric and hepatic cystic lesions were identified. After surgery and through immunohistochemistry studies, the diagnosis of hydatid cyst was confirmed. The patient had a satisfactory evolution. Conclusion. Symptomatic mesenteric and hepatic cystic hydatidosis is a rare disease in non-endemic sites, where surgery constitutes a fundamental pillar in the diagnosis and treatment in addition to pharmacological medical management. Background: Adrenal gland cysts are a rare entity, with a reported incidence in post-mortem series of 0.06-0.18%. However, the incidence seems to be increasing in recent years. The presentation of adrenal gland cysts is usually asymptomatic, but those cases in which symptoms are present are usually non-specific, which makes adrenal cysts generally recognized as incidentalomas. The finding is mainly made by computed tomography. The main objective of this article was to describe the clinical course of a patient with an adrenal gland pseudocyst, which is accompanied by symptoms of compression and persistent pain of long evolution in the left flank. Clinical case: A 65-year-old female patient attended the emergency room of a second-level hospital due to an increase in volume of the abdominal region with a sensation of fullness, heartburn, vomiting and pain. Computed tomography was performed, which reported a cystic mass and, later, exploratory laparotomy plus adrenalectomy were carried out. The pathology analysis reported a diagnosis of a 10 x 15 x 14 cm solid, cystic, and adherent tumor, coinciding with a pseudocyst of the adrenal gland. Conclusions: Adrenal gland cysts are rare. Computed tomography is recommended for its diagnosis and the standard of treatment is surgical intervention in the presence of symptoms. [ABSTRACT FROM AUTHOR]

El vólvulo cecal es la torsión del ciego sobre su eje mesentérico, que conlleva a una obstrucción intestinal por causas mecánicas o anatómicas. Esta condición está relacionada con el estreñimiento crónico, dieta alta en fibra, uso frecuente de laxantes, cirugía pélvica anterior, gestación y estudios laparoendoscópicos1. Los fitobezoar son masas compuestas por material vegetal no digerible con altas concentraciones de fibras. Los vólvulos cecales causados por bezoares colónicos son eventos inusitados, representando menos del 0,4 – 4 % de todas estas afecciones2.

Aortoesophageal fistula (AEF) is a rare but life-threatening complication of esophageal stent placement characterized by the formation of a communication between the aorta and the esophagus, which can result in catastrophic hemorrhage and sepsis, making it a potentially fatal complication that requires prompt diagnosis and management. The surgical treatment strategy for each case should be designed based on the patient's clinical presentation and the availability of endovascular or open-surgical expertise and resources.The aim of this study is to show the first latinoamerican experience in the treatment of AEF secondary to self-expandable partially covered metal stents (PCSEMS) with a retrospective case series study between January 2018 and April 2023 aimed at evaluating the surgical approach and postoperative outcomes of patients with AEF secondary to PCSEMS placement in 2 centers in Bogota, Colombia. A study with a sample of 2 patients was performed. The two patients were treated using an open surgical approach with laparotomy and left anterolateral thoracotomy. Resumen: La fístula aortoesofágica es una complicación rara pero potencialmente mortal de la inserción de un stent esofágico. Está caracterizada por la formación de una comunicación entre la aorta y el esófago, lo cual puede resultar en hemorragia masiva y sepsis, convirtiéndola en una complicación potencialmente mortal que requiere un diagnóstico y manejo oportuno. La estrategia de tratamiento quirúrgico para cada caso debe diseñarse en función de la presentación clínica del paciente y la disponibilidad de recursos endovasculares o de cirugía abierta según la experticia.El objetivo de este estudio es mostrar la primera experiencia latinoamericana en el tratamiento de la fístula aortoesofágica secundaria a stents metálicos autoexpandibles parcialmente cubiertos (PCSEMS) con un estudio de serie de casos retrospectivo entre enero de 2018 y abril de 2023 con el objetivo de evaluar el abordaje quirúrgico y los resultados postoperatorios de pacientes con FAE secundaria a colocación de PCSEMS en 2 centros de Bogotá, Colombia. Se realizó un estudio con una muestra de 2 pacientes. Ambos pacientes fueron tratados mediante abordaje quirúrgico abierto con laparotomía y toracotomía anterolateral izquierda.

La tuberculosis es un problema común en el mundo, fundamentalmente en países en desarrollo; aproximadamente del 1 al 3 % del total de casos de tuberculosis son extrapulmonares, y de estos, del 11 al 16 % es tuberculosis abdominal, la sexta forma más frecuente. Es producida por diferentes tipos de mycobacterium, el agente causal más frecuente en el ser humano es el Mycobacterium tuberculosis. Se realizó una revisión sistemática a través de diversas bases de datos con artículos publicados entre el año 2010 y el año 2022. Se presenta este artículo con el objetivo de describir las características clínicas, métodos diagnósticos y tratamiento en un paciente con tuberculosis peritoneal, así como el análisis realizado a su historia clínica. En la laparotomía exploradora se evidenciaron lesiones granulomatosas y caseosas generalizadas en peritoneo parietal y visceral. El estudio histopatológico reportó la presencia de granulomas y bacilos ácido-alcohol resistentes, confirmando el diagnóstico de tuberculosis peritoneal. Se inició tratamiento antituberculoso durante seis meses con mejoría del cuadro. Frente a un paciente con abdomen agudo con signos de irritación peritoneal y evolución atípica, debe pensarse en la tuberculosis intestinal como diagnóstico diferencial, aunque no tenga antecedentes de afectación pulmonar previa.

Introducción. La hidatidosis o equinococosis es una zoonosis parasitaria que se adquiere al ingerir huevos de cestodos del género Echinococcus. El diagnóstico clínico raramente se hace en sitios no endémicos. La mayoría de los pacientes se encuentran asintomáticos y los hallazgos incidentales en los estudios de imágenes o en procedimientos quirúrgicos permiten la sospecha diagnóstica. Caso clínico. Paciente masculino de 70 años, residente en área rural del municipio de Puerto Libertador, departamento de Córdoba, Colombia, quien consultó por masa abdominal en epigastrio y mesogastrio, parcialmente móvil e indolora. Resultados. En cirugía se identificaron lesiones quísticas mesentérica y hepática. Después de la cirugía y mediante estudios de inmunohistoquímica, se confirmó el diagnóstico de quiste hidatídico. El paciente tuvo una evolución satisfactoria. Conclusión. La hidatidosis quística mesentérica y hepática sintomática es una enfermedad rara en sitios no endémicos, donde la cirugía constituye un pilar fundamental en el diagnóstico y tratamiento, sumado al manejo médico farmacológico.

La intususcepción intestinal es la invaginación intraluminal de un segmento proximal de intestino y su mesenterio dentro de otro adyacente o distal. Su presentación en la edad adulta es infrecuente1. Representa el 5 % de todos los casos de invaginación, que corresponde de dos a tres casos por 1.000.000 personas al año, con un pico de incidencia entre los 30 y 50 años, sin predominio de sexo 2.

Granulosa cell tumor (GCT) is the most common ovarian tumor in mares, promoting decreased ovarian activity or anestrus, in addition to virilization, possibly due to the production of testosterone and other hormones. [texto removido] This report describes the surgical treatment of a 10-year-old quarter-horse mare with GCT through right paralumbar fossa access for unilateral ovariectomy. The surgical procedure was performed with the animal in the standing position, sedated using detomidine hydrochloride in a continuous infusion in association with infiltrative anesthesia in the paralumbar fossa, perineural anesthesia and topical anesthesia of the mesovarium. The affected ovary had dimensions of 10 x 11cm, with a macroscopic appearance suggestive of a granulosa cell tumor, described as a "honeycomb", which was confirmed by histopathological exam. The animal was kept under absolute rest for four consecutive months, with ovarian activity in the following breeding season, with artificial insemination and positive pregnancy outcome. It is concluded that ovariectomy with access via the paralumbar fossa with the objective of excision of ovaries smaller than 11 cm is likely to be performed with sedation and access through the paralumbar fossa. This is the treatment of choice for GCTs, preserving the reproductive activity of mares with high zootechnical value. [ABSTRACT FROM AUTHOR]

The article titled "Intestinal obstruction in adults due to ileocolic intussusception" presents a clinical case of a 47-year-old patient with partial intestinal obstruction due to ileocolic intussusception. Intestinal intussusception is an intraluminal invagination of a segment of the intestine into an adjacent or distal segment. Although it is rare in adults, it represents 5% of all cases of intussusception. The diagnosis of this condition is challenging due to nonspecific clinical manifestations, but computed tomography is the imaging study of choice. Definitive treatment is based on surgical maneuvers. In this case, an exploratory laparotomy was performed and the invaginated segment was reduced, 70 cm of terminal ileum were resected, and a end-to-end anastomosis was performed. Introducción. La embolia paradójica es un evento trombótico originado en la circulación venosa, que se manifiesta como embolismo arterial por medio de un defecto anatómico a nivel cardíaco o pulmonar. Se asocia principalmente a eventos cerebrovasculares, aunque se han encontrado casos de infarto agudo de miocardio, infarto renal y otros eventos isquémicos. Caso clínico. Paciente de 47 años, quien consultó por cuadro de dolor abdominal, que requirió manejo quirúrgico de urgencia, donde identificaron isquemia intestinal importante. Los estudios adicionales hallaron déficit de proteína S y persistencia de foramen oval permeable. Resultados. La presencia de trombosis arterial se conoce como trombosis de sitios inusuales y requiere de estudios para descartar trombofilias asociadas u otros estados protrombóticos. El déficit de proteína S es una trombofilia infrecuente, la cual se asocia en la vida adulta a eventos trombóticos de origen venoso. En presencia de defectos anatómicos, como un foramen oval permeable, puede progresar a embolia arterial, configurando un cuadro de embolismo paradójico. La estratificación de estos pacientes requiere imágenes que demuestran el defecto mencionado, así como el posible origen de los émbolos. El manejo se basa en anticoagulación plena, manejo de soporte, resolver las manifestaciones trombóticas existentes y un cierre temprano del defecto anatómico. Conclusiones. El embolismo paradójico debe sospecharse en caso de trombosis de sitios inusuales. Requiere de un estudio exhaustivo con imágenes y su manejo debe basarse en anticoagulación y cierre del defecto.

Introducción. La isquemia mesentérica crónica es una entidad infrecuente, con una prevalencia de 0,03 %, donde más del 90 % son debidas a enfermedad arterioesclerótica que compromete principalmente la arteria mesentérica superior. Sus síntomas son dolor abdominal crónico y pérdida de peso, asociado a alteraciones imagenológicas que hacen el diagnóstico. El tratamiento depende de las condiciones clínicas del paciente y el número de vasos comprometidos. Es claro que la enfermedad multivaso sintomática requiere revascularización. Caso clínico. Mujer de 67 años, fumadora activa con antecedentes de hipertensión arterial y dislipidemia, con cuadro de crisis hipertensiva tipo urgencia que requirió manejo en Unidad de Cuidados Intensivos. Se hizo diagnóstico de aneurisma toracoabdominal Crawford IV, oclusión aorto-ilíaca (TASC D) y oclusión crónica del tronco celíaco y la arteria mesentérica superior. Por los síntomas de isquemia mesentérica crónica fue llevada a tratamiento quirúrgico con baipás aorto-bifemoral más baipás retrógrado a la arteria mesentérica superior por vía abierta. Resultado. La paciente tuvo mejoría de la sintomatología y aumentó 13 % del peso al seguimiento a los 3 meses. Conclusión. La isquemia mesentérica crónica es una condición subdiagnosticada, marcador de riesgo cardiovascular, con alta carga de morbilidad y mortalidad, en la cual, con una identificación temprana se puede ofrecer una terapia de revascularización, sea por vía endovascular o abierta, con el fin de mejorar la calidad de vida y la ganancia de peso, y evitar la necrosis intestinal.

Se presenta un caso de un paciente masculino de 33 años, que acudió al servicio de Emergencia con dolor abdominal agudo de moderada intensidad, de 16 horas de evolución, localizado en fosa iliaca izquierda sin otra sintomatología acompañante. Como antecedente tenía el haber sido intervenido quirúrgicamente hace pasado 7 días por un cuadro de apendicitis aguda e intervenida quirúrgicamente, realizándole una cirugía laparoscópica. El examen destacaba alza térmica de 39.4 grados, taquicardia de 120 latidos por minuto, abdomen doloroso a nivel de fosa iliaca derecha e izquierda, cicatrices de intervención quirúrgica sin signos de infección. La tomografía inicial demostró neumoperitoneo a nivel subdifragmatico en el espacio pararenal izquierdo, la laparoscopia mostro perforación de colon sigmoides. Se realiza sigmoidectomía laparoscópica y, sin embargo, el paciente continuó con evolución poco favorable, con persistencia de signos de SIRS.

Introducción. La neumatosis quística intestinal se describe como la presencia de gas dentro de la pared intestinal. Es una entidad poco frecuente, con una incidencia del 0,03 % en la población global. Aparece con predilección en el género masculino después de los 45 años y se localiza principalmente en el intestino delgado (42 %) y el colon. Se puede asociar a varias condiciones que en ocasiones requieren manejo quirúrgico. Caso clínico. Se presenta el caso de un hombre 75 años, con antecedente de hipertensión arterial, quien consultó por un cuadro de 15 días de evolución consistente en distensión abdominal, dolor y estreñimiento. En urgencias se solicitó una radiografía de tórax que mostró neumoperitoneo y varios niveles hidroaéreos, por lo que el cirujano de turno consideró una posible ruptura de víscera hueca. Resultados. Fue llevado a laparotomía exploratoria, donde se identificó neumatosis quística intestinal y estómago muy aumentado de tamaño, compatible con gastroparesia. Como resultado del tratamiento brindado, el paciente tuvo un desenlace satisfactorio logrando alta médica, apoyado con cuidados básicos de enfermería. Conclusiones. Si bien los casos de neumatosis quística intestinal son de presentación inusual, se puede encontrar en pacientes con hallazgos imagenológicos de neumoperitoneo. Por eso, se debe realizar un análisis concienzudo de cada paciente e individualizar el caso para el correcto diagnóstico.

Ovarian tumors represent one of the usual neoplasms of women and are responsible for almost half of the deaths from cancer of the female genital tract. Mucinous cystoadenocarcinoma is defined as a malignant neoplasm that originates in the ovarian epithelial tissue usually acquiring large dimensions. We present the case of a postmenopausal 79 years old woman with a history of hyperthyroidism and systemic hypertension controlled with medical treatment. The patient came to the General Surgery office suffering from significant increase in volume in the abdomen. The physical examination revealed the abdominal increase was due to a lump that occupied almost all of it. Exploratory laparotomy was performed finding a 30 cm right ovary tumor which was removed. Histological diagnosis reported mucinous ovarian cystadenoma. She passed the clinical-surgical evolution without complications and was referred to the Oncology service for treatment and follow-up without presenting any alteration. Mucinous ovarian cystadenoma can have large sizes, so it should be considered in women with history of giant pelvic or abdominal tumors. Currently, it does not represent a health problem in the territory. Technological advances in diagnostic devices foster an early detection. Surgical resection and chemotherapy are the choice for treating this entity.

Tuberculosis is a common problem in the world, mainly in developing countries; Approximately 1 to 3% of all tuberculosis cases are extrapulmonary, and of these, 11 to 16% are abdominal tuberculosis, the sixth most common form. It is produced by different types of mycobacterium, the most frequent causative agent in humans being Mycobacterium tuberculosis. The aim of this article is to describe the clinical characteristics, diagnostic methods and treatment in a patient with peritoneal tuberculosis. A systematic review was carried out through various databases from the 2010 to 2022 and the clinical history corresponding to the patient studied was analyzed. Exploratory laparotomy revealed generalized granulomatous and caseous lesions in the parietal and visceral peritoneum. The histopathological study reported the presence of granulomas and acid-fast bacilli, confirming the diagnosis of peritoneal tuberculosis. Antituberculous treatment was started for six months with improvement of the condition. When faced with a patient with an acute abdomen with signs of peritoneal irritation and atypical evolution, intestinal tuberculosis should be considered as a differential diagnosis, even if there is no history of previous pulmonary involvement. Introdução: Os lipossarcomas são tumores malignos raros de origem mesenquimal, a partir de precursores de adipócitos, podendo ocorrer no retroperitônio. Os lipossarcomas retroperitoneais representam de 0,3% a 0,6% das neoplasias malignas. Em virtude do crescimento lento e assintomático do tumor, tendo como principal manifestação clínica o aumento do volume abdominal, o quadro é comumente acompanhado de um diagnóstico tardio e curso indolente. Relato do caso: Homem, 70 anos de idade, com aumento progressivo abdominal há cerca de dois anos. A ressonância magnética demonstrou uma formação em topografia retroperitoneal à direita, promovendo importante desvio das estruturas da linha média, com presença de conteúdo sólido acima da estrutura supradescrita. Foi submetido à laparotomia xifopúbica, com apresentação de massa gigante retroperitoneal que ocupava todo abdome, com deslocamento das alças intestinais e rim direito. Foram realizadas ressecção do tumor em bloco, rafia diafragmática e colecistectomia. Após três meses do primeiro procedimento cirúrgico, o paciente apresentou novamente aumento do volume abdominal, com confirmação de recidiva tumoral após realização de exames de imagem. Um novo procedimento cirúrgico foi realizado, revelando tumor com cápsula rompida e múltiplos coágulos. Após alta hospitalar, foi submetido a sessões de radioterapia complementares à cirurgia. Conclusão: A rápida reapresentação do paciente após o surgimento da recidiva, comum nesses casos, foi essencial para a redução de resíduos na segunda cirurgia, evidenciando a importância de exames periódicos para o reconhecimento precoce da recorrência local. No presente caso, também foram realizadas sessões de radioterapia, com a finalidade de evitar a recidiva, sem sucesso.

Introducción. Los tricobezoares ocurren de forma frecuente en niñas y adolescentes, y se asocian a trastornos psicológicos como depresión, tricotilomanía o tricofagia. Caso clínico. Se presenta una paciente adolescente con síndrome de Rapunzel, con hallazgo adicional de perforación yeyunal debido al tricobezoar. Discusión. Dentro de las complicaciones de los tricobezoares se reporta invaginación intestinal (principalmente de yeyuno), apendicitis, obstrucción biliar, neumonía, pancreatitis secundaria y perforación, esta última como ocurrió en nuestra paciente. Conclusión. En pacientes mujeres adolescentes con dolor abdominal o abdomen agudo, se debe tener en cuenta el diagnóstico de síndrome de Rapunzel, así como sus probables complicaciones.

Objective: To describe the profile of patients treated in the "Red Wave", in the period of 2018-2019. Methods: It is a quantitative, cross-sectional, retrospective, descriptive and documentary research. Held at the first-aid post João XXIII Hospital, in Belo Horizonte. The universe of this study was 83 electronic medical records of patients who entered the "Red Wave" protocol. Simple frequency distributions, measures of central tendency (mean and median) and variability (standard deviation) have been performed. Results: Male, aged between 1 and 95 years old, being an average at 33,4 years old. The most frequent trauma mechanism was blunt, the means of transport was an ambulance from the Mobile Emergency Service, the causes of the trauma were an automobile accident, followed by trauma by a firearm projectile. From the performed surgeries, the most frequent ones were laparotomy, thoracotomy and craniectomy. Among the outcome, death was the most common one among patients. Conclusion: Patients arrive very seriously and death was the main outcome. Studies that allow comparative data analysis and standardization of the calculation of survival probability are suggested. It is recommended to update the red wave protocol, including other surgeries that are already performed and new inclusion criteria for patients. [ABSTRACT FROM AUTHOR]

Borderline or low malignant potential ovarian tumors are an intermediate entity of benign and malignant tumors whose histological alterations of the ovarian epithelial cells do not meet specific criteria for benignity or malignancy. They are tumors that generally appear during the 3rd - 4th decade of a woman's life and are diagnosed limited to the ovary in 80% of cases. In this way, the biological-oncological behavior is very good with a global survival to 10 years of 90% for initial stages and 60-70% for advanced stages. Clinically show few symptoms such as abdominal distension and oppressive pain. Imaging studies are important since it can help to consider your diagnosis, it is recommended to first perform an abdominopelvic ultrasound and then an abdominopelvic tomography. We present a 40-year-old patient case, nulliparous with a desire to preserve fertility, performing an early diagnosis and subsequent intervention with exploratory laparotomy plus left adnexectomy with a satisfactory evolution in 48 hours. The patient (Caramel) canine, mixed breed, approximately 5 years old, weighing 26kg was seen at the Ame Vet veterinary clinic, with severe abdominal pain, apathy, anorexia, prostration. Blood tests, abdominal ultrasound and abdominal x-ray were performed, according to the tests, a suspicion of partial torsion of the stomach and splenomegaly with right displacement, in addition to gastritis and enteritis. It was decided by the clinic and surgeon to perform laparotomy and prophylactic gastropexy. The animal was previously medicated by the clinic that treated the animal for analgesia with methadone, tramadol and dipyrone two hours before the beginning of the surgical procedure, therefore, pre-anesthetic medications, anesthetic induction with propofol, and ketamine were not used, aiming to perform analgesia. multimodal with continuous infusion of ketamine and fentanyl, that is, more than one different mechanism of action, acting synergistically and reducing dose-dependent adverse effects and monitoring hemostasis and patient parameters, presenting its efficacy and safety in intraoperative pain control. The parameters addressed were heart rate (HR), respiratory rate (RR), electrocardiographic tracing, Spo2, systolic blood pressure (SBP), mean arterial pressure (MAP ), temperature and mucosal color, evaluated every 5 minutes. The parameters evaluated indicated good hemodynamic stability, safety and analgesia, with no need to change the doses established during the procedure. A reduction in the requirement of isoflurane to maintain the surgical plan can also be observed. The patient woke up without signs of pain in the immediate postoperative period, thus avoiding systemic stress. which conferred the good result of the analgesic protocol instituted. A presença de fecalomas em equinos está relacionada a ingestão acidental de substâncias como pedaços de corda, cama, plástico e outros materiais, levando a uma compactação da ingesta com consequente obstrução e distensão de alças intestinais. É uma afecção comum em pôneis, mini horses e potros, causando uma obstrução, na maioria das vezes, no cólon menor. O presente trabalho relata um caso de síndrome cólica em um eqüino, Mini Horse, de 2 anos de idade, apresentando sinais de desconforto abdominal há dois dias. O exame físico revelou freqüência cardíaca e respiratória elevada, hipomotilidade à auscultação, distensão abdominal, mucosas de coloração pálida com presença de halo toxêmico e pirexia. A análise do líquido peritoneal revelou aumento de leucócitos. No hemograma foi identificada leucopenia. O animal foi submetido à laparotomia exploratória, sendo identificada a presença de um fecaloma em terço proximal do cólon menor, retroflexão da flexura pélvica e compactação de cólon maior.

Introducción. El embalaje y transporte de estupefacientes dentro del organismo, o body packing, es una práctica frecuente en Centroamérica y el Caribe. Además del riesgo de muerte por la exposición a las sustancias tóxicas, existe el riesgo de complicaciones mecánicas con indicación de manejo quirúrgico. El Hospital de Engativá, por su cercanía al aeropuerto de Bogotá, D.C., Colombia, es el centro de referencia para el tratamiento de estos pacientes. Caso clínico. Un hombre de 65 años traído al hospital por un episodio emético con expulsión de cuatro cápsulas para el transporte de estupefacientes. Al examen físico se encontraron masas palpables en el hemiabdomen superior, sin abdomen agudo. La tomografía de abdomen informó un síndrome pilórico secundario a retención gástrica de cuerpos extraños. Fue llevado a laparotomía y gastrotomía logrando la extracción de 97 objetos cilíndricos de látex que contenían sustancias ilícitas. Discusión. En los body packer asintomáticos, la administración de soluciones laxantes es una estrategia terapéutica segura. Los casos reportados de obstrucción gastrointestinal son infrecuentes y se relacionan con la ingesta de un gran número de cápsulas, por lo que es necesario el tratamiento quirúrgico. Conclusión. El síndrome pilórico es una presentación infrecuente en un body packer. Se debe tener un alto índice de sospecha para garantizar un manejo oportuno.

Introducción: la histerectomía radical con linfadenectomía pélvica es el tratamiento estándar para las pacientes con cáncer de cérvix en estadios tempranos que no desean preservar la fertilidad. El objetivo principal de este estudio fue comparar tasas de supervivencia global, periodo libre de enfermedad y recurrencia entre histerectomía radical por mínima invasión (CMI) versus cirugía abierta. Además analizar las características histopatológicas, complicaciones intraoperatorias, posoperatorias tempranas y tardías entre ambas técnicas. Metodología: estudio observacional de cohorte retrospectivo entre 2011 y 2017. Se incluyeron mayores de 18 años con diagnóstico de carcinoma de cuello uterino en estadios IA1 (invasión del espacio linfovascular) a IB1 (FIGO 2009) con seguimiento hasta diciembre 2020. Se hizo análisis bivariado utilizando chi cuadrado, prueba exacta de Fisher o U Mann Whitney para determinar las diferencias entre las técnicas quirúrgicas frente a características sociodemográficas, clínicas, complicaciones intraoperatorias, tempranas y tardías. Se calcularon tasas de supervivencia global y densidades de recurrencia con el tiempo aportado por las pacientes. Resultados: de 113 pacientes llevadas a histerectomía radical, 75 se excluyeron del análisis. La supervivencia global fue 100% para las del grupo de laparotomía versus 97.6% en CMI. La tasa de recurrencia global fue de 8 casos y la del grupo de CMI 14.1 por 1.000 personas/año de observación. El grupo de laparotomía no presentó recaídas. Hubo diferencia significativa en la mediana de sangrado intraoperatorio (600 cc laparotomía versus 100 cc laparoscopia, p= 0.002); 11.4% requirieron transfusión intraoperatoria (25% laparotomía versus 2.4% mínima invasión, p=0,006). Conclusiones: las pacientes llevadas a cirugía por laparoscopia tuvieron una mayor proporción de recurrencia y mortalidad en comparación con el abordaje por técnica abierta.

Background: Relaparotomies are associated with a considerable percentage of morbidity and mortality in patients who require intensive care; and around this procedure it is complex to decide when and how to do it, so that it is effective in every way. For this reason, the study of its results through the analysis of specific variables is of interest.Objective: to characterize the behavior of relaparotomies in a General Surgery service.Methods: descriptive, longitudinal and prospective observational study, which included patients who required relaparotomies, which were performed in the Surgery service of the Dr. Gustavo Aldereguía Lima General University Hospital General, in Cienfuegos, from January/2015 to December / 2017. The variables analyzed were: operative diagnosis at reintervention, surgical strategy, number of reintervention, and time elapsed from initial surgery to reintervention, among others.Results: a relaparotomy index of 2.7 was obtained. The group over 60 years old and the male sex predominated; peritonitis and postoperative eviscerations were the main causes. The clinical criteria were the most used for reintervention; On-demand relaparotomy was applied to a greater number of patients, compared to the other surgical strategies. Mortality increased in reoperated patients in the first 72 hours.Conclusion: In the series analyzed, the number of reoperations and mortality showed a direct proportionality relationship. Multiple organ dysfunctions were the cause of death most associated with the relaparotomy procedure.

The care of patients with enterocutaneous fistula constitutes a significant challenge owing to the alterations it usually brings about. For successful treatment, it is necessary to manage fluids and electrolytes adequately, provide practical nutritional support, and control sepsis until its eradication; thus, many fistulae close spontaneously. We present the case of a 36-year-old male patient with a four-month history of fecal-like umbilical secretion. When performing the fistulogram, we confirmed a fistulous tract of 9 cm, which ended at the level of the sigmoid colon, a rare location. In cases where the enterocutaneous fistula does not close, and surgical treatment is indicated, it is imperative to maximize perioperative care, decrease surgical time, choose the correct surgical technique, and prepare the patient for surgery to avoid complications with a fatal outcome. Se presenta el caso clínico de una paciente de sexo femenino de 36 años, atendida en el Hospital Japonés (HJ) de la ciudad de Santa Cruz, ingreso a nuestro hospital con cuadro clínico de abdomen agudo y cursando embarazo de 20 semanas, con antecedente de relevancia de aborto por embarazo ectópico hace tres años, se realizo laparotomía exploratoria en la que se evidencia embarazo abdominal, se realiza extracción del feto y de placenta, se coloca packing y se deja en laparotomía contenida, después de 72 hrs se realiza limpieza y cierre de pared, paciente con buena evolución clínica.

La atención de los pacientes con fístula enterocutánea constituye un gran reto, por las alteraciones con las que suelen acompañarse. Para lograr un tratamiento exitoso es necesario realizar un adecuado manejo de los líquidos y electrolitos, brindar un apoyo nutricional eficaz y controlar la sepsis hasta lograr su erradicación; de esta manera, muchas fístulas cierran espontáneamente. Se expone el caso de un paciente de 36 años de edad, con un cuadro de secreción umbilical de aspecto fecaloideo de 4 meses de evolución. Al realizar la fistulografía se constató un trayecto fistuloso de 9 cm, el cual terminaba a nivel del colon sigmoide, localización poco frecuente. En los casos en que la fístula enterocutánea no cierre y tenga indicación de tratamiento quirúrgico, es necesario extremar los cuidados perioperatorios, minimizar el tiempo quirúrgico, elegir la técnica quirúrgica correcta y preparar al paciente para la cirugía, de modo que se eviten complicaciones que pueden tener un desenlace fatal.

Background: Postoperative Nausea and Vomiting (PONV) is a multifactorial surgical complication with an unclear underlying cause. Anesthetic methods, patients' characteristics and the type of surgery are considered as factors affecting PONV. This study was designed to compare the effect of inhalational and intravenous anesthesia in abdominal surgery on the incidence and severity of PONV. Methods: A single-blinded prospective randomized clinical trial on 105 patients aged 18 − 65 years was carried out. Patients were divided into two groups of Total Intravenous Anesthesia (TIVA) and inhalational anesthesia. The incidence and the severity of PONV were examined at 0, 2, 6, 12 and 24 hours after the surgery. The use of a rescue antiemetic was also evaluated.
