Wajaul Mafasil is a compound Arabic term, where Waja literally means ‘pain’ and Mafasil means joints. Osteoarthritis is the most common form of the arthritis and is the leading cause of disability in older persons, affecting 10% of population >60 years of age, in which 80% of individual have limited mobility and 25% cannot perform major daily activities. The disease follows a more severe course in women with greater symptoms, more extensive involvement, and increased prevalence of knee and hand joint involvement. There are wide range of treatment options available in different system of medicines. Inspite of that patient are not fully satisfied. Unani scholars in their treatise have mentioned different formulations but their scientific validation have still not done. Hence a study was conducted to evaluate and compare the efficacy and safety of Habb-e-Hudar and Hijamah bila Shurt in Wajaul Mafasil of knee. The study was conducted as comparative clinical study. A total of 72 eligible patients fitting the inclusion criteria were selected from OPD/ IPD. However, only 60 patients (30 in each group) completed the trial. Group A patients was given 2 pills Habb-e-Hudar was given orally twice a day for 30 days and Group B patient received Hijamah bila Shurt on knee daily for period of 30 days. Patients were assessed on subjective and objective parameters at baseline and on every follow up till 30th day. Safety parameters were assessed at baseline and after the end of the trial. The results were analysed statistically using Student t test and Paired Proportion test. Significant improvement was observed in all the subjective parameters (pHijamah bila Shurt and Habbe-Hudar both are effective but Hijamah bila Shurt is comparatively more effective treatment for Wajaul Mafasil of knee. There were no clinically or statistically significant side effects or toxicity during or after the trial. However large sample sized controlled clinical trials of longer duration are required to generalize the results. It is known that Wet Cupping Therapy (WCT), one of thecomplementary and traditional therapy applications, has therapeutic effect onmany diseases that are nonresponsive to conventional treatment. In this casereport, a rhythmic and generalized convulsion seizure characterized withtonic-clonic movements after WCT was defined in the patient. The convulsionoccurred during scarification to DU14 point (C7 processus spinosus) just afterthe WCT started. Vital findings of the patient before the WCT were stable andblood parameters that were analysed were normal. He had no medical history. Somecomplications such as syncope are reported during the WCT. However, because thegeneralized convulsion was not within our knowledge this case is approved topresent., Islakkupa terapisi (IKT) geleneksel ve tamamlayıcı tedavi uygulamalarından biridir.Terapötik etkiye sahip olduğu bilinen IKT’nin konvansiyonel tedaviye cevapvermeyen birçok bozuklukta etkisi olduğu bilinmektedir. Bu olgu sunumunda, IKT sırasındatonik-klonik hareketlerle karakterize ritmik, jeneralize konvülziyon atağıgeçiren hasta sunulmuştur. IKT uygulaması yapılırken, DU14 bölgesine (C7-processusspinosus) skarifikasyon yapılırken jeneralize konvülziyon gerçekleşmiştir.Hastanın IKT öncesi vital bulguları stabil, kan parametreleri normaldir. Ayrıca,hastanın özgeçmişinde jeneralize konvülziyon ile uyumlu bulgu yoktur. Literatürde,IKT esnasında senkop gibi bazı komplikasyonlar rapor edilmiştir ancak jeneralizekonvülziyonun bilgimiz dahilinde bulunmaması sebebiyle bu vakanın sunulmasıuygun görülmüştür.

Bu çalışmada, hacamtın toplum üzerindeki algısı incelenmiştir.Kan bağışı ve donör kazanım programlarının önemi günümüzde artmış bulunmaktadır. According to their fields of application, they are applications that are highly believed to be significant, from past to today. Depending on their preference, applications that are reshaped in different places and methods and maintain their life reflects a culture.As one of the alternative medicine applications, Hacamat is the process of vacuum removal of toxic substances together with the discharged dirty blood, which obstructs the nerves that communicate with the related brain lobe and accumulates in the subcutaneous tissues for the purpose of treatment and healing.In this study, the perception of the volume on society was examined. The importance of blood donation and donor recovery programs has increased today. Turkish Red Crescent meets ninety-two percent of one million and eight hundred thousand units of blood that our country needs annually.In this study, it is mentioned that the individuals who want to donate blood after donating blood, manen, substance, biological and medical gains will be obtained. It isaimed to attract the attention of the society to the need of spreading all the layers of the society to the public health by the positive effect of the vaccination on public health in terms of preventive medicine and the positive support of the blood donation of the society. 95

Hacamat, tarihi milattan öncelere dayanan, pek çok medeniyet tarafından benimsenip uygulanan bir tedavi yöntemidir. Arap toplumunda da bu tedavi yönteminin uygulandığı bilinmektedir. Hadis kitaplarında, Hz. Peygamber döneminde öne çıkan tedavi yöntemlerinden biri olarak dikkatleri çekmektedir. Resûlullah'ın da bizzat tedavi amaçlı hacamat yaptırdığı ve çevresindeki kişilere tavsiye ettiği hususunda pek çok rivayet mevcuttur. Buna ilave olarak günümüzde de hacamat yoğun bir ilgi görmektedir. Bu yoğun ilgiye rağmen Hz. Peygamber dönemi hacamat uygulamasının mahiyetine dair ülkemizde bilimsel nitelikte yapılmış, yeterli ve tatmin edici bir çalışma bulunmamaktadır. Bu eksikliği gidermek amacıyla çalışmamızda, dönemin tıp kültürünün bir yönünü teşkil eden hacamat uygulamaları ele alınmıştır. Hacamat, medeniyet tarihi ve tıp tarihi açısından da irdelenerek bu tedavi yönteminin tarihî süreçte diğer toplumlarda nasıl bir yere sahip olduğu tespit edilmeye çalışılmıştır. Böylece Hz. Peygamber dönemindeki hacamat uygulamalarının tarihî süreç içerisindeki yerini görme imkânı elde edilmiştir. Araştırmamızın sonucunda, Hz. Peygamber döneminde Arap toplumunun medeniyetler arası etkileşim sayesinde elde ettiği tıbbî birikimin bir yansıması olarak hacamatı uyguladığı kanaatine varılmıştır. Aynı zamanda, Hz. Peygamber döneminde hacamatın vücudun hangi bölgelerinden ve hangi rahatsızlıklar sebebiyle yapıldığı hususlarıyla ilgili önemli bulgulara ulaşılmıştır. Bunun yanında, hacamatı uygulayan haccâmlarla ilgili tartışmalar ve rivayetlerde kimliği zikredilen haccâmlar hakkında bilgiler elde edilmiştir. Son olarak Hz. Peygamber döneminde hacamatı gerçekleştirmek için ihtiyaç duyulan hacamat aletleri hakkında bir takım bilgiler tespit edilmiştir. Hijamah is a treatment method dating back to before Christ, and adopted and implemented by many civilizations. It is known that this treatment was implemented also in Arab society. In the hadith books, it draws attention as a prominent treatment method during the era of Prophet Muhammad. There are many narrativies telling that Prophet himself got hijama for treatment and recommended it to people around him. Furthermore, hijamah attracts a great deal of attention even today. Despite this attention, there is no scientific, adequate and satisfying study in our country regarding the nature of hijamah during the Prophet Muhammad era.In our study, hijamah applications constituting a part of the era's medical culture were addressed to fulfill this deficiency. We tried to detect what kind of place this treatment method had in other societies during history by examining Hijamah in terms of civilization history and history of medicine. Therefore, we had the opportunity to see the place of hijamah applications in the era of Prophet Muhammad during the course of history.As a result of our study, we decided that the Arab society implemented hijamah as a reflection of medical accumulation which they gained thanks to interaction among civilizations in the era of Prophet Muhammad. Furthermore, important findings were accessed concerning which body parts hijamah is applied to and for what diseases during the era of Prophet Muhammad. Besides, information about the hijamah practitioners mentioned in discussions and rumors about practitioners applying hijamah. In conclusion, some information was acquired about the hijamah tools required to perform hijamah during the era of Prophet Muhammad. 107

Çalışmamızın amacı hacamat yaptıran farklı yaşlardaki kadınlardan, eş zamanda alınan kupa kanı ve venöz kanda oksidatif stres ile ilişkili olduğu düşünülen miR-21, miR-34a ve miR-200'ün ekspresyon seviyelerini ve oksidatif stresin biobelirteci olarak tanımlanan malondialdehid (MDA) ve glutatyon (GSH) düzeylerini tespit etmektir. Bu çalışma Konya Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Geleneksel ve Tamamlayıcı Tıp Merkezine başvuran hacamat yaptırmak isteyen kadın katılımcılar üzerinde gerçekleştirildi. Katılımcılar 20-45 yaş aralığında 30 kadın ve 45-75 yaş aralığında 30 kadın, toplamda 60 kadından oluşturuldu. Çalışmamızda gruplandırma şu şekilde yapıldı; grup 1, venöz kanında 25-45 yaş arası kadınlar, grup 2, venöz kanında 45-75 yaş arası kadınlar, grup 3, hacamat kanında 25-45 yaş arası kadınlar, grup 4, hacamat kanında 45-75 yaş arası kadınlar. Çalışmaya dahil edilen katılımcılardan kâhil noktası hacamat kanı ve rutin amaçla alınan venöz kanı alındı. Hem hacamat hem venöz kandan elde edilen serum örneklerinde MDA ve Glutatyon ölçümleri ELISA yöntemi ile yapıldı. Background: Cupping (Hijamah) therapy is very well documented as a result of several thousand years of clinical experiences in Unani medicine. In this procedure, suction is created by various means either with or without bloodletting. Though this therapy is being widely practiced across the globe for treating many chronic and intractable ailments but many reports reveal its unscientific and improper practices which results in many complications. Therefore to develop standard operative procedures and to propose protocols of cupping therapy in various diseases is the need of hour. Materials and methods: A thorough literature review of relevant journals and textbooks was performed to gather the maximum available data on cupping therapy. Conclusion: This paper seeks to introduce the general concepts of cupping therapy in Unani medicine and other traditional systems of medicine, shortcomings and limitations of the currently published studies and suggest ways to improve these technical/methodological flaws. In addition, the authors have also attempted to provide the cupping related materials, hypotheses, observations which will provide the researchers the base for evaluating their usefulness in future clinical trials., Uvod: Terapija puštanjem krvi (hijamah) vrlo je dobro dokumentirana kao posljedica nekoliko stotina godina kliničkih iskustava medicine Unani. U tom je postupku sukcija ostvarena različitim sredstvima s ispuštanjem krvi ili bez njega. Iako se ova terapija primjenjuje širom svijeta za liječenje mnogih kroničnih i teško lječivih bolesti, mnoga izvješća otkrivaju neznanstvene i nepravilne prakse koje rezultiraju mnogim komplikacijama. Stoga je nužno čim prije razviti standardnu proceduru te predložiti protokole za terapiju puštanjem krvi kod različitih bolesti. Materijali i metode: Kako bi se prikupila maksimalna količina podataka o terapiji puštanjem krvi, učinjen je pregled relevantnih časopisa i udžbenika. Zaključak: Ovaj rad nastoji predstaviti osnovni koncept terapije puštanjem krvi u medicini Unani kao i u drugim tradicijskim medicinama, nedostatke i ograničenja objavljenih aktualnih studija te predložiti na koje načine poboljšati ove tehničke/metodološke nedostatke. Osim toga autori su također pokušali odrediti materijale koji se koriste pri puštanju krvi, hipoteze i opažanja koja će istraživačima omogućiti temelj za evaluaciju njihove korisnosti u budućim kliničkim ispitivanjima.

Dünya Sağlık Örgütü'nün 2002 yılında yayınladığı `Geleneksel Tıp Strateji Planlaması` ile dünyada geleneksel ve tamamlayıcı tıp konularına akademik ve bilimsel ilgi artmıştır. Dünyanın birçok ülkesinde güvenli uygulama standartları getirilen GT Tıp uygulamaları hakkında ülkemizde de kupa, hacamat ve sülük tedavilerini içine alan birçok GT Tıp uygulaması yasallaşmıştır. Bu tedaviler Anadolu topraklarında bin yıllardır uygulanan tedavilerdir. Tez kapsamında geleneksel tedavi yöntemleri olarak kupa, hacamat ve sülükle tedavinin harf inkılabı sonrasında, Cumhuriyet dönemindeki yeri yazılı ve sözlü kaynaklar kullanılarak tespit edilmiştir. Following the publication of the `Traditional Medicine Strategy` by the World Health Organization in 2002, there has globally emerged an increasing academic and scientific interest in traditional and complementary medicine. Many traditional and complementary medicine practices including cupping, hijamah and leeches have been legalized in our country following the introduction of some safety standards in this field of medicine in many countries of the world. In fact, this field of medicine is one that has been practiced in Anatolia for thousands of years. This thesis handles the position of the medication through cupping, hijamah and leeches as methods of traditional medicine in the republican era beginning from the alphabet reform by delving into the written and oral sources. The study covers the historical development of the cupping, hijamah and leeches, their utilisation in Anatolian folkloric medicine, their position in the republican era written sources, the use of these types of medication in the folkloric medicine of today and the current practices in our country and some foreign countries.Keywords: Traditional Medicine, Traditional and Complementary Medicine, Cupping, Hijamah, Leeches, Folk Medicine. 66
