Background: Polycystic ovary syndrome (PCOS) is the most prevalent reproductive endocrinopathy and currently considered to be a part of the metabolic syndrome. Women suffering from PCOS present with a group of symptoms related with menstrual disorder and androgen excess. Women with PCOS cluster risk factors associated with risk of insulin resistance, dyslipidemia, hyperandrogenemia, obesity, cardiovascular disease (CVD), infertility, and psychological disorders. Objective :The study was designed to determine the levels of DPP4 in PCOS patients and compared with control and study the correlation with biomarkers in serum of PCOS patients. Subjects and Method: The study is case-control included 80 females and carried out from February 2019 till May 2019 for females in the reproductive age [18-45 years old], forty females were came from gynecological and obstetric hospital, in Karbala, and they all diagnosed by their physicians have polycystic ovarian syndrome and compared with forty control females {healthy females]. plasma activity of DPP4 and biochemical variables were performed. Results: Results obtained in this study showed that there are significant differences (p < 0.05) in serum of DPP4, SHBG, DHEA-S, LH, FSH, free testosterone and total testosterone and there is positive significant correlation between DPP4 with free testosterone, SHBG, DHEA-S, LH and total testosterone. Conclusion: Polycystic ovarian syndrome is a higher risk of type 2 diabetic mellitus, dyslipidemia and cardiovascular diseases, Dipeptidyl peptidase 4 (DPP4) was high in PCOS patient and had positive correlation with free testosterone, total testosterone, DHEA-S, SHBG and LH. Abstract Dipeptidyl peptidase-4 (DPP-4) inhibitors are oral antidiabetic drugs that safely reduce the blood glucose level over the long term. In Japan, DPP-4 inhibitors have become the oral antidiabetic drugs most frequently prescribed for patients with type 2 diabetes. However, the results of several cardiovascular outcomes studies have suggested that some DPP-4 inhibitors may increase the risk of hospitalization for heart failure. In patients with diabetes, heart failure is the most frequent cardiovascular condition, and it has a negative impact on the quality of life as well as being a potentially fatal complication. Therefore, it is important to determine whether an increased risk of heart failure is associated with certain DPP-4 inhibitors or is a class effect of these drugs. This review explores the mechanism by which DPP-4 inhibitors may increase the risk of heart failure and possible differences among these drugs. The available research suggests that DPP-4 inhibitors cause sympathetic activation as a class effect and this may increase the risk of heart failure. Unlike other DPP-4 inhibitors, sitagliptin and alogliptin are mainly excreted in the urine and suppress renal sodium-hydrogen exchanger 3 activity. Based on guidance from the National Institute for Health and Clinical Excellence (NICE), metformin is the first-line treatment for lowering glucose in patients with diabetes. Sulfonylureas can also be used as first line treatment in some patients, especially those who are not overweight and have overt hyperglycaemic symptoms such as polyurea and thirst.

Dipeptidyl peptidase IV/CD26 (DP IV) is a multifunctional serine protease cleaving off dipeptides from the N-terminus of peptides. The enzyme is expressed on the surface of epithelial and endothelial cells as a type II transmembrane protein. However, a soluble form of DPIV is also present in body fluids. Large scale expression of soluble human recombinant His(6)-37 - 766 DP IV, using the methylotrophic yeast Pichia pastoris, yielded 1.7 mg DP IV protein per litre of fermentation supernatant. The characterisation of recombinant DP IV confirmed proper folding and glycosylation similar to DP IV purified from porcine kidney. Kinetic comparison of both proteins using short synthetic substrates and inhibitors revealed similar characteristics. However, interaction analysis of both proteins with the gastrointestinal hormone GLP-1[sub7-36] resulted in significantly different binding constants for the human and the porcine enzyme (K[subd]=153.0 ± 17.0 &mirco;M and K[subd]=33.4 ± 2.2 µM, respectively). In contrast, the enzyme adenosine deaminase binds stronger to human than to porcine DP IV (K[subd]=2.15 ± 0.18 nM and K[subd]=7.38 ± 0.54 nM, respectively). Even though the sequence of porcine DP IV, amplified by RT-PCR, revealed 88% identity between both enzymes, the species-specific variations between amino acids 328 to 341 are likely to be responsible for the differences in ADA-binding.
