Introduction and aim: Drug-induced liver injury (DILI) is a diagnosis based on the ruling out of potential liver diseases and consolidated by establishing causality through the temporal relation between a potentially hepatotoxic substance and altered liver biochemistry. Incidence fluctuates greatly worldwide, with very few reports of causal agents of DILI in Colombia.A retrospective study on patients treated at the Centro de Estudios en Salud (CES), within the time frame of January 2015 and June 2020, was conducted to document the causal substances of DILI in patients with liver biopsy and to correlate the types of histologic patterns with the biochemical pattern of liver injury (R ratio). Results: Of the 254 adult patients with liver biopsy and no tumor etiology, 20 patients were identified as cases of DILI (7.87%). The two most frequently found causal substances were efavirenz, in three HIV-positive patients, and Moringa oleifera (moringa), in two patients. There was a statistically significant association between cholestatic patterns (p = 0.037) and mixed patterns (p = 0.031), in the comparison of the histopathologic categories and the R ratio. Conclusion: To the best of our knowledge, there are no reports on DILI secondary to Moringa oleifera (moringa). The R ratio could be a useful tool, in relation to the histologic pattern of injury, in cases of mixed and cholestatic patterns. Resumen: Introducción y objetivo: El daño hepático inducido por medicamentos (DILI, por sus siglas en inglés), es un diagnóstico basado en la exclusión de potenciales hepatopatías, y que se consolida, estableciendo la causalidad entre una sustancia potencialmente hepatotóxica y la alteración del perfil bioquímico hepático. La incidencia de este fenómeno fluctúa en un amplio rango a nivel mundial, con muy poca documentación de los agentes causantes de DILI en Colombia.Con el objetivo de documentar las sustancias causantes de DILI en pacientes con biopsia hepática y de relacionar los tipos de patrones histológicos con el perfil analítico de toxicidad (índice R), se realizó un análisis retrospectivo de los pacientes atendidos en la clínica CES (Centro de Estudios en Salud) entre enero de 2015 y junio de 2020. Resultados: De 254 pacientes adultos, con disponibilidad de biopsia hepática sin etiología tumoral, 20 pacientes fueron identificados como DILI (7.87%). Las dos sustancias más frecuentemente encontradas fueron efavirenz en tres pacientes HIV positivos, y en dos pacientes Moringa oleifera (Moringa). Se encontró asociación significativa entre los patrones colestásicos (p = 0.037) y mixtos (p = 0.031) en la comparación de las categorías histopatológicas y el índice R. Conclusión: En lo mejor de nuestro conocimiento, no hay reportes de casos de DILI secundarios a Moringa oleifera (Moringa). El índice R podría ser una herramienta útil de relación con el patrón histopatológico, para el caso de los patrones mixto y colestásico.

Resultados: de 222 artigos elegíveis utilizou-se 16 após critérios de exclusão. Discussão: em síntese, foi possível observar estudos com suplementação de Whey Proteins em que analisaram os biomarcadores alanina aminotransferase, aspartato aminotransferase, glicose, bilirrubina, fosfatase alcalina, gama glutamiltransferase, albumina e proteínas totais, detalhando os modelos dos estudos em procedimentos experimentais com ratos. Conclusão: Em conclusão, os dados deste estudo sugerem que as dosagens de suplementação variam entre 0,15g a 40g/kg de Whey proteins, sendo que de 0,15 a 20g/kg não ocorre alterações em parâmetros bioquímicos relacionados ao dano hepático.

Introducción: El síndrome de hiper-IgM es un error innato de la inmunidad, caracterizado por un defecto en el cambio de isotipo de inmunoglobulina, con valores disminuidos de IgG, IgA e IgE, y concentraciones normales o elevadas de IgM. Predispone a procesos infecciosos en el sistema respiratorio y aparato gastrointestinal, además de enfermedades autoinmunes y neoplasias. Reporte de caso: Paciente pediátrico de género masculino, de 5 años y 7 meses de edad, con antecedente de dos cuadros de neumonía (uno de estos grave) y diarrea crónica desde los 2 años. Neutropenia moderada persistente, disminución de la concentración de IgG y elevación de IgM. La citometría de flujo confirmó la ausencia de CD40L. Durante la evolución clínica tuvo afectación hepática temprana. Conclusión: El síndrome de hiper-IgM predispone a daño hepático, por lo que se requiere la evaluación completa y el diagnóstico oportuno. El tratamiento antiinfeccioso activo y el control de la respuesta inflamatoria son factores decisivos para establecer el tratamiento del daño hepático.

- Abstract
OBJECTIVE: To identify the different variables that can cause liver injury in a patient hospitalized for COVID-19. MATERIALS AND METHODS: A prospective cohort study that included patients with COVID-19 who were admitted to the Central University Hospital of the State of Chihuahua from April 2020 to November 2020. A univariate analysis was performed to find the frequencies of demographic characteristics and of the drugs prescribed, as well as a comparison of means of the biochemical parameters using Student's t test. RESULTS: One hundred thirty-four patients with a confirmed diagnosis of COVID-19 were included, who underwent liver function parameters and averages were obtained on the day of admission, on the fifth day and the last values recorded for improvement or death. Of the parameters, albumin levels showed a significant decrease on the 5th and last day of hospitalization compared to the first day of admission. On the other hand, the levels of alkaline phosphatase, gamma-glutamyl transferase and erythrocyte sedimentation rate increased significantly on the 5th and last day of stay. CONCLUSIONS: There are different mechanisms that can generate liver injury associated with COVID-19. Of these, the uncontrolled inflammatory response that occurs can alter liver function tests. Our results found a relationship between the alteration of different laboratory parameters and the days of hospital stay of patients with the disease. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: Hyper-IgM syndrome is an innate error of immunity in which there is a defect in change of isotype of immunoglobulins, with decreased values of IgG, IgA, and IgE, but normal or increased level of IgM. This predisposes to infectious processes at the respiratory and gastrointestinal levels, as well as autoimmune diseases and neoplasm. Case report: A 5 year 7-month-old boy with a history of 2 pneumonias, one of them severe, and chronic diarrhea since he was 2 years old. Persistent moderate neutropenia decreased IgG and elevated IgM. Cytometry flow confirmed absence of CD40L. Clinical evolution with early hepatic involvement. Discussion: Hyper-IgM syndrome predisposes to liver damage, so a complete evaluation is required as well as early diagnosis. Active anti-infective treatment and control of the inflammatory response are key to the treatment of liver damage. [ABSTRACT FROM AUTHOR]

High fructose (HF) ingestion is a common risk factor for the development of nonalcoholic fatty liver disease (NAFLD), which has become a serious health problem. The underlying mechanism of HF-induced NAFLD needs to be further understood and an effective prevention needs to be established urgently. Gas chromatography-mass spectrometer-based metabonomic method was employed to characterize the serum metabolic profile of HF-induced NAFLD in mice. β-Hydroxybutyric acid, elaidic acid and oleic acid that contributed to energy and lipid metabolism were confirmed as potential biomarkers for HF-induced NAFLD. With these changed metabolic pathways as possible drug targets, different administrations revealed that co-administration of stachyose and tea polyphenols was a more effective method than treatment of stachyose or tea polyphenols alone for preventing HF-induced negative changes of metabolic pathways in mice. These findings suggest that β-hydroxybutyric acid, elaidic acid and oleic acid are potential biomarkers in HF-induced NAFLD, and co-administration is a novel preventive strategy. Photoacoustic Spectroscopy (PAS) stand outs among Photothermal techniques due to its versatility for the characterization of different types of materials, including biological samples. In this study, were compared and analyzed by PAS, the differences between the optical absorption spectra of blood of male Fisher rats, in three cases, control (healthy rats), liver damage and liver damage treated with Curcuma longa. The experimental results show differences in the optical absorption spectra of each analysed case, therefore PAS would be proposed as a non conventional complementary technique, to study the effect of Curcuma longa in induced liver damage for an animal model. La obesidad es uno de los principales problemas de salud a nivel mundial, por lo que la investigación en todos los aspectos que involucran a esta condición resulta de gran valor. Una de las principales limitantes para el estudio de la obesidad es el acceso limitado a recursos económicos, por lo que desarrollamos un modelo de obesidad en ratas preparando una dieta alta en grasa a base de alimento comercial estándar y manteca de cerdo. El costo de esta formulación representa aproximadamente una décima parte de una formula comercial equivalente y las ratas desarrollaron obesidad central y acumulación de grasa en el hígado, sin cambios en el peso corporal, después de seis meses de consumo de la dieta. Restructured pork (RP) enriched in Seaweeds are potential functional foods. The antiapoptotic and hypocholesterolemic effects of consuming cholesterol enriched diets containing Wakame-RP (CW), Nori-RP (CN) and Sea Spaghetti (CS) were tested in a 1-wk study. Groups of six rats per group were fed a mix of 85% AIN-93M rodent-diet containing cholesterol and cholic acid as a cholesterol rising agent plus 15% RP containing alga. These diets were compared to control-RP diets enriched or not in cholesterol (CC and C, respectively). After 1-wk, cholesterol feeding significantly increased liver apoptosis markers which were significantly reduced by CS (cellular cycle DNA, caspase-3, and cytochrome c), CN (caspase-3 and cytochrome c) and CW (caspase-3) diets. CN and CS diets significantly blocked the cholesterolaemic rising effect observed in the CC group but no protective effect was observed in the CW group. Differences in seaweed composition added to RP appear responsible for blocking or not the proapoptotic and hypercholesterolemic effects of high cholesterol-RP consumption; thus, any generalization on seaweed effects or food containing seaweeds must be avoided. Although present results are worthy, future studies are demanded to ascertain the utility of consuming algal-RP as part of usual diets.

La radioembolización (RE), también conocida como radioterapia interna selectiva o SIRT (“Selective Internal Radiation Therapy”), consiste en la administración intraarterial de microesferas cargadas de isótopos radiactivos para el tratamiento locorregional de tumores hepáticos primarios o secundarios de diversos orígenes [1,2]. El radioisótopo de uso más común es el Ytrio-90 (Y90), un emisor β de alta energía con un periodo de semidesintegración de 64,1 horas y una escasa penetrancia en los tejidos (2,5 mm de media), características que hacen que el efecto de la radiación emitida se limite al entorno inmediato del vaso en el que queda embolizada cada esfera. La aplicabilidad de esta terapia se fundamenta en la vascularización arterial de los tumores hepáticos. Mientras que el parénquima hepático recibe la mayor parte del flujo sanguíneo a través de la vena porta (80 %), los tumores hepáticos (primarios y secundarios) se irrigan de manera preferente a través de la arteria hepática. Como consecuencia de esta doble vascularización, la administración intraarterial de esferas radiactivas permite alcanzar dosis elevadas de radiación en el tumor preservando el hígado sano [3,4]. Además, la administración puede ser más o menos selectiva según la ubicación del catéter. El tratamiento puede realizarse a través de la arteria hepática común (tratamiento total/bilobar), arteria hepática derecha o izquierda (tratamiento lobar) o a través de una rama segmentaria.

Las hepatopatías tóxicas son la consecuencia del daño hepático causado por sustancias extrañas al organismo, especialmente medicamentosas. Aunque se desconoce su verdadera incidencia, se sabe que originan 10 % de las hepatitis agudas en adultos y 15 a 30 % de las fulminantes, por lo cual continúan siendo un problema alarmante de salud, a pesar de haberse retirado algunos fármacos del mercado por esta razón. En el presente artículo se exponen algunas consideraciones generales sobre el tema, particularmente relacionadas con factores de riesgo, medicamentos causantes de intoxicación hepática, manifestaciones clínicas más importantes, diagnóstico de la hepatotoxicidad y tratamiento, con énfasis en el preventivo.Toxic hepatopathies are the consequence of hepatic damage caused by foreign substances, especially drugs. Although their true incidence is unknown, they cause 10% of the acute hepatitis in adults and 15 to 30% of fulminant hepatitis, thus continue to be an alarming health problem, despite the fact that some drugs have been withdrawn from the market for this reason. In this paper some general considerations on the topic are stated, particularly related to risk factors, drugs causing liver toxicity, most important clinical manifestations, diagnosis of hepatotoxicity and treatment, especially preventive one.

- Abstract
Background: Acute liver failure (ALF) is a severe and potentially lethal clinical syndrome. It has been demonstrated that micro ribonucleic acids (miRNAs) are crucial mediators of nearly all pathological processes, including liver disease., Objective: The present study investigates the role of miR-378 in ALF. An ALF mouse model was induced using intraperitoneal injections of d-galactosamine/lipopolysaccharide (d-GalN/LPS). A hepatocyte cell line and miR-378 analogue were used in vitro to investigate the possible roles of miR-378 in ALF., Methods: The expressions of miR-378 and predicted target genes were measured via reverse transcription-quantitative polymerase chain reaction and western blotting, and cell apoptosis was assayed using flow cytometry., Results: Compared with mice in the control group, the mice challenged with d-GalN/LPS showed higher levels of alanine aminotransferase, aspartate aminotransferase, tumour necrosis factor-alpha and interleukin-6, more severe liver damage and increased numbers of apoptotic hepatocytes. Hepatic miR-378 was distinctly downregulated, while messenger RNA and protein levels of cysteinyl aspartate specific proteinase 9 (caspase-9) were upregulated in the ALF model. Furthermore, miR-378 was downregulated in d-GalN/TNF-induced hepatocyte cells, and miR-378 was found to inhibit hepatocyte apoptosis by targeting caspase-9., Conclusion: Together, the present results indicate that miR-378 is a previously unrecognised post-ALF hepatocyte apoptosis regulator and may be a potential therapeutic target in the context of ALF., (Copyright © 2022 Elsevier España, S.L.U. Background: Species of mycotoxin-producing fungi are potentially dangerous to humans and animals. The liver is the best-known organ of action of these substances. The aim of this study was to isolate microscopic fungi from honey and investigate the cytotoxic effect of the extract of Penicillium sp. in an experimental model. Methods: Honey samples were cultured in Sabouraud agar. After isolated and identified microscopically, the colonies of the genus Penicillium sp. were transplanted to the Sabouraud dextrose agar culture medium. After its development, they were processed to obtain an extract. Eighteen Wistar mice were randomly assigned to experimental (GI) and control (GII) groups. The GI was subjected to an oral inoculation of the extract, while GII received a placebo. Procedures were performed every day for thirty days, after which the liver of each animal was removed for analysis. Results: Aspergillus sp. (86.2%), Geotrichum sp. (6.89%) and Penicillium sp. (6.89%) were isolated. The most frequent species was Aspergillus niger (46%). In relation to the cytotoxic effects of the extract of Penicillium sp., the gross findings in the liver of GI suggested mainly congestion. Light microscopy showed that the little hepatic lobules were preserved and there was vascular congestion of sinusoids. Light microscopy of specimens from the experimental group showed that 68.2% were abnormal, whereas 87.5% of the control group were within normal limits. Conclusions: The results suggest that there was contamination in honey samples. There was a predominance of macroscopic and microscopic changes in the liver of experimental rats, suggesting liver damage by Penicillium sp. Antecedentes: Las especies de hongos productores de micotoxinas son potencialmente peligrosas para humanos y animales. El hígado es el órgano de acción más conocido de estas sustancias. El objetivo de este estudio fue aislar hongos microscópicos de la miel e investigar el efecto citotóxico del extracto de Penicillium sp. en un modelo experimental. Métodos: Las muestras de miel se cultivaron en agar Sabouraud. Luego de aisladas e identificadas microscópicamente, las colonias del género Penicillium sp. fueron trasplantados al medio de cultivo agar dextrosa Sabouraud. Luego de su desarrollo, fueron procesados para obtener un extracto. Se asignaron aleatoriamente dieciocho ratones Wistar a grupos experimentales (GI) y de control (GII). El GI se sometió a una inoculación oral del extracto, mientras que el GII recibió un placebo. Los procedimientos se realizaron todos los días durante treinta días, después de lo cual se extrajo el hígado de cada animal para su análisis. Resultados: Aspergillus sp. (86,2%), Geotrichum sp. (6,89%) y Penicillium sp. (6,89%) fueron aislados. La especie más frecuente fue Aspergillus niger (46%). En relación con los efectos citotóxicos del extracto de Penicillium sp., los hallazgos macroscópicos en el hígado de GI sugirieron principalmente congestión. La microscopía óptica mostró que los pequeños lóbulos hepáticos estaban conservados y había congestión vascular de los sinusoides. La microscopía óptica de las muestras del grupo experimental mostró que el 68,2 % eran anormales, mientras que el 87,5 % del grupo de control estaba dentro de los límites normales. Conclusiones: Los resultados sugieren que hubo contaminación en las muestras de miel. Em relação aos efeitos citotóxicos do extrato de Penicillium sp., os achados macroscópicos no fígado do GI sugeriram principalmente congestão. A microscopia de luz mostrou que os pequenos lóbulos hepáticos estavam preservados e havia congestão vascular dos sinusóides. A microscopia de luz dos espécimes do grupo experimental mostrou que 68,2% estavam anormais, enquanto 87,5% do grupo controle estavam dentro dos limites normais. Conclusões: Os resultados sugerem que houve contaminação nas amostras de mel. Houve predomínio de alterações macroscópicas e microscópicas no fígado de ratos experimentais, sugerindo lesão hepática por Penicillium sp.

- Abstract
Introduction and Aim: Drug-induced liver injury (DILI) is a diagnosis based on the ruling out of potential liver diseases and consolidated by establishing causality through the temporal relation between a potentially hepatotoxic substance and altered liver biochemistry. Incidence fluctuates greatly worldwide, with very few reports of causal agents of DILI in Colombia. A retrospective study on patients treated at the Centro de Estudios en Salud (CES), within the time frame of January 2015 and June 2020, was conducted to document the causal substances of DILI in patients with liver biopsy and to correlate the types of histologic patterns with the biochemical pattern of liver injury (R ratio)., Results: Of the 254 adult patients with liver biopsy and no tumor etiology, 20 patients were identified as cases of DILI (7.87%). The two most frequently found causal substances were efavirenz, in three HIV-positive patients, and Moringa oleifera (moringa), in two patients. There was a statistically significant association between cholestatic patterns (p = 0.037) and mixed patterns (p = 0.031), in the comparison of the histopathologic categories and the R ratio., Conclusion: To the best of our knowledge, there are no reports on DILI secondary to Moringa oleifera (moringa). The R ratio could be a useful tool, in relation to the histologic pattern of injury, in cases of mixed and cholestatic patterns., (Copyright © 2021 Asociación Mexicana de Gastroenterología. 20. Papel de la inflamación periférica y la neuroinflamación en el deterioro cognitivo y motor en ratas con diferentes grados de daño hepático. Efectos del tratamiento con rifaximina

La encefalopatía hepática (EH) es un síndrome neuropsiquiátrico complejo consecuencia de un fallo hepático. Alrededor del 40% de los pacientes cirróticos sin síntomas clínicos evidentes de EH presenta encefalopatía hepática mínima (EHM), con déficits cognitivos y enlentecimiento psicomotor. La hiperamonemia y la inflamación periférica actúan sinérgicamente induciendo neuroinflamación, que altera la neurotransmisión conduciendo al deterioro de la función cognitiva y motora. Los pacientes con esteatohepatitis pueden presentar deterioro cognitivo leve si los niveles de hiperamonemia e inflamación son lo suficientemente elevados. Además, en cerebelo de estos pacientes se ha encontrado infiltración de linfocitos T y neuroinflamación. Estos datos sugieren que la aparición de la EHM puede ocurrir en etapas tempranas del daño hepático, antes de llegar a cirrosis. El objetivo de esta tesis fue analizar, en un modelo de rata con fallo hepático progresivo: a) las alteraciones cognitivas y motoras que se producen como consecuencia del daño hepático, cuándo aparecen y cómo evolucionan con el grado de fallo hepático, b) las alteraciones en la neurotransmisión en cerebelo e hipocampo asociadas a las diferentes alteraciones cognitivas y motoras, c) la neuroinflamación y los mecanismos por los que la inflamación periférica conduce a la neuroinflamación, d) los cambios en el sistema inmune asociados al daño hepático y e) los efectos del tratamiento con rifaximina y los mecanismos por los cuales puede mejorar las alteraciones cognitivas y motoras. Los resultados muestran que la incoordinación motora aparece ya en el estadio de esteatohepatitis, y está asociada a alteraciones de la neurotransmisión GABAérgica inducidas por la neuroinflamación en cerebelo. Las alteraciones cognitivas evolucionan con el grado de daño hepático: en esteatohepatitis se deterioran el aprendizaje y la memoria espaciales, mientras que la memoria de trabajo solo se deteriora en esteatohepatitis avanzada y la memoria de reconocimiento de un objeto nuevo en ratas con cirrosis. Estas alteraciones se asocian a neuroinflamación en hipocampo que induce alteraciones en la neurotransmisión glutamatérgica y GABAérgica. El daño hepático induce inflamación periférica y cambios en el inmunofenotipo, que evolucionan con el grado de daño hepático y promueven la infiltración de linfocitos T y macrófagos en cerebelo e hipocampo, lo que contribuye al mantenimiento de la neuroinflamación. El tratamiento con rifaximina de ratas con daño hepático crónico previene muchas de las alteraciones en el sistema inmune periférico y la infiltración de células del sistema inmune en el cerebro, lo cual disminuye la neuroinflamación y previene o revierte las alteraciones en la neurotransmisión. Esto conduce a la mejora de la función cognitiva y motora. Estos resultados indican que el tratamiento temprano con rifaximina podría prevenir el deterioro neurológico en pacientes con esteatohepatitis. Hepatic encephalopathy (HE) is a complex neuropsychiatric syndrome resulting from liver failure. About 40% of cirrhotic patients without evident clinical symptoms of HE show minimal hepatic encephalopathy (MHE), with cognitive deficits and psychomotor slowing. Hyperammonemia and peripheral inflammation act synergistically to induce neuroinflammation, which alters neurotransmission, impairing cognitive and motor function. Patients with steatohepatitis may show mild cognitive impairment if hyperammonemia and inflammation are high enough. Moreover, infiltration of T lymphocytes and neuroinflammation have been reported in cerebellum of these patients. These data suggest that the appearance of MHE may occur at early stages of liver damage, before reaching cirrhosis. The objective of this thesis was to analyze in a rat model of progressive liver failure: a) cognitive and motor alterations that occur as a consequence of liver damage, when they appear and how they progress with the progression of liver failure, b) alterations in neurotransmission in cerebellum and hippocampus associated with the different cognitive and motor alterations, c) neuroinflammation and the mechanisms by which peripheral inflammation leads to neuroinflammation, d) changes in the immune system associated with progression of liver damage and e) the effects of treatment with rifaximin and the mechanisms by which it can improve cognitive and motor function. The results show that motor incoordination appears already at steatohepatitis, and it is associated with impaired GABAergic neurotransmission induced by neuroinflammation in cerebellum. Cognitive impairments evolve with the grade of liver damage: spatial learning and memory are impaired in steatohepatitis, while working memory only deteriorates in advanced steatohepatitis and object recognition memory in rats with cirrhosis. These alterations are associated with neuroinflammation in hippocampus, which induces alterations in glutamatergic and GABAergic neurotransmission. Liver damage induces peripheral inflammation and changes in the immunophenotype, which evolve with the progression of liver damage and promote the infiltration of T lymphocytes and macrophages in cerebellum and hippocampus, which contributes to promotion and maintenance of neuroinflammation. Treatment with rifaximin of rats with chronic liver damage prevents many of the alterations in the peripheral immune system and the infiltration of immune system cells into the brain. This decreases neuroinflammation and prevents or restores changes in neurotransmission. This leads to improvement of cognitive and motor function. These results suggest that early treatment with rifaximin could prevent neurological impairment in patients with steatohepatitis.

Objective: to verify blood markers during a 12-week training protocol and after Sao Paulo Marathon. Methods: Blood samples of 9 male marathoners were collected before (C1) and after (C2) 12-week training protocol, before (C3) and after (C4) marathon. Muscle and liver damage markers (creatine kinase [CK-MM], aspartate aminotransferase [AST], alanine aminotransferase [ALT]), oxidative stress levels (thiobarbituric acid reactive substances [TBARS]) and serum iron concentration were measured. Results: changes were identified comparing moment C4 to other moments for CK-MM and iron. For AST, ALT, and TBARS no differences were identified. Conclusion: strenuous exercises might elicit changes on blood markers, needing follow up strategies to avoid impairments to athletes’ performance and health. RESUMO Objetivo: verificar marcadores sanguíneos durante 12 semanas de treinamento e após a Maratona de São Paulo. Metodologia: amostras de sangue de 9 maratonistas foram coletadas antes (C1) e após (C2) o treinamento de 12 semanas, antes (C3) e após (C4) a maratona. Marcadores sanguíneos e hepáticos (creatina quinase [CK-MM], aspartato aminotransf. [AST], alanina aminotransf. [ALT]), níveis de estresse oxidativo (subst. reativas ácido tiobarbitúrico [TBARS]) e ferro sérico foram analisados. Resultados: Verificaram-se alterações de CK-MM e ferro entre C4 e os outros momentos. Para AST, ALT e TBARS não se encontrou diferenças. Conclusão: exercícios exaustivos podem causar alterações em marcadores sanguíneos, requerendo estratégias de monitoramento para evitar danos ao desempenho e saúde do atleta. [ALT]), estrés oxidativo (sustancias reactivas del ácido tiobarbitúrico [TBARS]) y de hierro. Resultados: Se encontraron cambios comparando C4 y otros momentos para CK-MM y hierro. Para AST, ALT y TBARS no se encontraron diferencias. Conclusión: ejercicios extenuantes pueden causar cambios en marcadores sanguíneos, requiriendo estrategias de monitoreo para evitar daños al desempeño y salud del atleta.

Algumas pesquisas realizadas recentemente relatam que as células-tronco mesenquimais (CTM) podem ser induzidas à aquisição de marcadores hepatocíticos pelo transplante em modelos animais de dano hepático, ou pelo cultivo in vitro com fatores de crescimento e citocinas. O presente estudo teve por objetivo avaliar o comportamento das CTM frente à indução da diferenciação hepatocítica. As CTM foram isoladas da medula óssea de quatro doadores saudáveis, caracterizadas e submetidas ao protocolo de indução à diferenciação hepatocítica in vitro e in vivo. As células induzidas in vitro apresentaram mudanças na sua morfologia, mostrando a morfologia semelhante à do hepatócito, porém, o perfil imunofenotípico não foi modificado. As células induzidas também não apresentaram o aumento dos transcritos de albumina, citoqueratina 18 e citoqueratina 19 quando analisadas por RT-PCR em tempo real, e não alteraram a expressão de albumina, citoqueratina 18 e alfa-fetoproteína como demonstrado por imunofluorescência. Quando analisadas in vivo, as CTM demonstraram o potencial migratório para o tecido hepático danificado de camundongos imunodeficientes. Em conjunto, os resultados sugerem que as CTM da medula óssea não são capazes de se diferenciar em hepatócitos quando estimuladas in vitro pela metodologia utilizada neste estudo, mas são capazes de migrar para o tecido hepático danificado in vivo, o que sugere o seu papel no reparo do fígado. A contribuição para o reparo pode estar associada com o efeito parácrino dessas células.

Restructured pork (RP) enriched in Seaweeds are potential functional foods. The antiapoptotic and hypocholesterolemic effects of consuming cholesterol enriched diets containing Wakame-RP (CW), Nori-RP (CN) and Sea Spaghetti (CS) were tested in a 1-wk study. Groups of six rats per group were fed a mix of 85% AIN-93M rodent-diet containing cholesterol and cholic acid as a cholesterol rising agent plus 15% RP containing alga. These diets were compared to control-RP diets enriched or not in cholesterol (CC and C, respectively). After 1-wk, cholesterol feeding significantly increased liver apoptosis markers which were significantly reduced by CS (cellular cycle DNA, caspase-3, and cytochrome c), CN (caspase-3 and cytochrome c) and CW (caspase-3) diets. CN and CS diets significantly blocked the cholesterolaemic rising effect observed in the CC group but no protective effect was observed in the CW group. Differences in seaweed composition added to RP appear responsible for blocking or not the proapoptotic and hypercholesterolemic effects of high cholesterol-RP consumption; thus, any generalization on seaweed effects or food containing seaweeds must be avoided. Se compara el modelo estándar reducido de tres poblaciones (hepatocitos sanos y enfermos, y carga viral) para el estudio de la dinámica del virus de la hepatitis C (VHC), con el modelo de la dinámica viral con alanina aminotransferasa (ALT). En términos del parámetro umbral que es el mismo para ambos modelos, se determina la existencia y estabilidad del estado de equilibrio endémico, así como la estabilidad global del estado de equilibrio del individuo sano. [ES] La encefalopatía hepática (EH) es una alteración de la función cerebral consecuencia de un fallo en la función hepática previo. Existen dos tipos principales de fallo hepático: agudo y crónico, y ambos se producen por diferentes mecanismos y conllevan distintas consecuencias. Los pacientes con fallo hepático crónico (cirróticos en su mayor parte) sin síntomas evidentes de EH pueden presentar la llamada encefalopatía hepática mínima (EHM), la cual incluye ralentización psicomotora y déficits de atención entre otros síntomas que pueden ser detectados mediante tests psicométricos. La hiperamonemia y la inflamación periférica actúan sinérgicamente para producir estas alteraciones neurológicas. El propósito de este trabajo es estudiar en un modelo animal de la enfermedad, que se corresponde con ratas a las cuales se les administra tetracloruro de carbono para inducir el daño hepático, los mecanismos por los cuales esto ocurre. Se analizan cambios en el sistema inmune periférico, comprobando que hay un aumento en la expresión de CCL20 en el intestino de ratas con daño hepático, y si existe una relación con la neuroinflamación en el hipocampo. Para ello, se caracterizan las alteraciones en la expresión de IL-17 y TNF en el hipocampo debido al daño hepático, observándose una disminución de estas citoquinas en algunas regiones del hipocampo en ratas con daño hepático leve (esteatohepatitis). The standard model of three populations (healthy and unhealthy hepatocytes, and viral load) for the study of the dynamics of hepatitis C virus (HCV), is compared with viral dynamic model with alanine aminotransferase (ALT). In terms of the threshold parameter that it's the same for both models, the existence and local asymptotic stability of the endemically infected equilibrium steady state, and the global stability of the healthy individual equilibrium steady state are established. Furthermore, an experimental study is presented showing that it's possible to carry out the monitoring of hepatic damage biopsies free by numerical parameter estimation, considering only viral load and ALT levels measurements and a reasonable assessment of pretreatment hepatic damage. La resistencia a insulina es una característica típica de la diabetes tipo 2 y la obesidad, patologías vinculadas a un alto riesgo del desarrollo de enfermedades crónicas hepáticas y hepatocarcinoma. Sin embargo, se desconoce el papel que la resistencia a insulina pueda ejercer durante el daño hepático crónico y durante la hepatocarcinogénesis, por lo que se ha abordado esta cuestión a través del estudio del substrato receptor de insulina 2 (IRS2), principal promotor de la señalización de insulina en el hígado. La experimentación llevada a cabo durante este proyecto muestra que la deleción del gen Irs2 en el modelo murino Irs2-/- impide la inducción de Fgf7 en células estromales durante el daño crónico hepático y reduce la expresión de su receptor epitelial Fgfr2-IIIb en células progenitoras, dando lugar a fallos en la comunicación estromal-epitelial en el nicho progenitor. Además, retrasa la expansión de células de origen progenitor, da lugar a una mayor pérdida hepatocítica y frena la generación de hepatocitos 2c (carga genética diploide), reduciendo así la reepitelización hepática. Por otra parte, durante la hepatocarcinogénesis, la pérdida de expresión de IRS2 está asociada con un fenotipo más mesenquimal, migratorio y metastásico, mientras que su sobreexpresión reduce la migración tumoral e induce procesos de transición mesenquimal epitelial (MET) así como la expresión de FGFR2-IIIb. En conclusión, los resultados alcanzados en esta tesis indican que la señalización sistémica de insulina/IGF-1 actúa a nivel local a través de IRS2 en el nicho progenitor durante la reparación del daño crónico hepático. La expresión de la proteína IRS2 regula de forma positiva la comunicación entre células estromales y epiteliales además de promover procesos MET, favoreciendo la reparación epitelial y reduciendo la migración tumoral intrahepática.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES The objective of this study was to evaluate the effect of Baccharis trimera hydroethanolic crude extract (BtHE), popularly known as carqueja, on CCl4 induced hepatic injury in Wistar rats. The liver is the main organ of metabolism and excretion of the human body, and is continually exposed to xenobiotics because of its strategic anatomical function. Liver diseases represent a significant health problem due to their limited complications and treatment possibilities. The phytotherapeutic researches for the treatment of hepatic disorders generally use animal models for the induction of hepatotoxicity, and the hepatic damage promoted by the administration of carbon tetrachloride (CCl4) is one of the most accepted and widespread models. Several species of Baccharis are widely used in folk medicine, mainly in the form of infusions, to treat various disorders, such as liver and gastrointestinal problems. The BtHE presented high levels of polyphenols (0.152 g GAE/100 g DW), flavonoids (19.07 g CE/100 g DW) and tannins (20.165 g CE/100 g DW) and a high antioxidant capacity, mainly for lipophilic substrates (59.23% inhibition of linoleic acid peroxidation). The extract promoted a slight synthesis of collagen in the liver, in agreement with the control group, and CCl4 did not promote significant fibrosis. Poco se sabe sobre los cambios en la actividad de las enzimas séricas relacionadas con la función hepática durante la hipervitaminosis E. En el presente trabajo se estudió el efecto de la administración intraperitoneal de 50, 100, 200 y 400 mg de vitamina E/día, durante 20 días sobre la actividad enzimática sérica en 60 ratas Wistar machos, de 12 semanas de edad, con pesos entre 180 y 200 gramos. El grupo control estuvo integrado por 15 ratas Wistar sanas, con edad y peso similares a los animales tratados. Quarenta camundongos machos C57BL/6 foram divididos igualmente em quatro grupos: 1) grupo controle (C), que recebeu apenas água, 2) grupo tratado com DEC, que recebeu 50 mg/kg durante um período de 12 dias (DEC50), 3) o grupo alcoólico (EtOH), submetido ao consumo crônico de álcool e 4) o grupo alcoólico tratado com DEC (EtOH50), submetido ao consumo crônico de álcool e tratado com DEC. Fragmentos de fígado foram analisados por meio de microscopia de luz, imunohistoquímica e testes de western blot para avaliar vários mecanismos envolvidos na lesão hepática induzida pelo etanol, incluindo a peroxidação lipídica, marcadores inflamatórios e a ativação de fatores de transcrição. A análise histológica do grupo alcoólico mostrou dano hepatocelular evidente que foi reduzido no grupo alcóolico tratado com DEC. Os resultados da imunohistoquímica e do western blot mostraram expressão elevada de marcadores inflamatórios como MDA, TNF-, IL-1, COX-2 e iNOS nos hepatócitos do grupo EtOH. No entanto, pouca imunopositividade para estes marcadores foi detectada após tratamento com DEC. No grupo de EtOH a ativação do fator de transcrição NF-kB foi observada através de um aumento na expressão de ambos, NF-B e pNF-B, em hepatócitos. Esta expressão foi significativamente reduzida nos fígados do grupo EtOH50. A expressão da proteína IB foi medida para determinar se a ativação do NF-B seria resultado da degradação da IB. Observou-se que a expressão desta enzima era baixa no grupo EtOH, enquanto que os animais tratados com DEC tinha uma expressão elevada de IB. Os resultados do presente estudo indicam que a DEC atenua a lesão hepática alcoólica, em parte, pela inibição da ativação do NF-B e por suprimir a indução de genes dependentes do NF-B.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Acetaminophen (APAP) is the analgesic most used in world, in therapeutic doses does not show toxicity, but in elevated doses can cause hepatic damage due the formation of his metabolic toxic N-acetyl-p-benzoquinonimine (NAPQI). The brain damage can occurs because hepatic damage a condition called hepatic encephalopathy, since the liver already show his function altered, like transform ammonia in urea, causing accumulation of ammonia in the brain, which is toxic for this organ. Furthermore, the NAPQI can cause oxidative damage and mitochondrial dysfunction on brain tissue. The diphenyl diselenide [(PhSe)2] is an organoselenium compound that exhibit antioxidant activity and potential pharmacological. The aim of this study is investigated the ability of (PhSe)2 in reversing the oxidative brain damage and mitochondrial dysfunction induced by a toxic dose of APAP. Mice received a APAP (600mg/kg), followed by a dose of (PhSe)2 (15,6 mg/kg) 1 hour latter. Four hours after APAP administration, plasma was withdrawn from the mice and used for biochemical assays of aspastate aminotransferase (AST) and alanine aminotransferase (ALT) confirming the hepatic damage. The APAP administration increase lipid peroxidation, production of reactive oxygen species and decrease in activity of Na+, K+ - ATPase enzyme. Similary, APAP caused alteration on parameters of mitochondrial function. The treatment with (PhSe)2 revert the cerebral damage induced by a single dose of APAP. O paracetamol é o analgésico mais usado no mundo, em doses terapêuticas não apresenta nenhuma toxicidade, porém em doses elevadas pode causar dano hepático pela formação do seu metabólito tóxico N-acetil-p-benzoquinoneimina (NAPQI). O dano cerebral pode ocorrer em decorrência ao dano no fígado, uma condição chamada de encefalopatia hepática, já que o fígado pode apresentar um comprometimento das suas funções, como transformar amônia em uréia, causando um acúmulo de amônia no organismo, sendo esta tóxica para o cérebro. Além disso, o NAPQI pode causar estresse oxidativo e disfunção mitocondrial no cérebro. O disseleneto de difenila [(PhSe)2] é um composto orgânico de selênio que apresenta atividade antioxidante e potencial farmacológico. O objetivo desse estudo é verificar a capacidade do (PhSe)2 em reverter o dano cerebral e disfunção mitocondrial causada por uma dose tóxica de paracetamol. Os camundongos receberam paracetamol (600 mg/kg) e 1h após, disseleneto de difenila (15,6 mg/kg). Após 4h da administração do paracetamol, coletou-se o soro para as análises bioquímicas de aspartato aminotransferase (AST) e alanina aminotransferase (ALT) que confirmaram o dano hepático. A administração de APAP aumentou a peroxidação lipídica, a produção de espécies reativas de oxigênio e diminuiu a atividade da enzima Na+, K+ - ATPase. Da mesma forma, o APAP alterou os parâmetros de funcionalidade mitocondrial. O tratamento com (PhSe)2 reverteu o dano cerebral induzido por uma dose única de APAP.

De entre todas ellas (B1, B2, G1, G2, M1 y M2), destaca desde el punto de vista de la seguridad alimentaria la aflatoxina B1, tanto por ser la más prevalente en alimentos como la más tóxica para los seres humanos. La Unión Europea, debido a la toxicidad de estos compuestos y con el fin de garantizar una protección eficaz de la salud pública, ha establecido mediante el Reglamento (CE) Nº 1881/2006 contenidos máximos para la aflatoxina B1 y la suma de aflatoxinas B1, B2, G1 y G2 en diversos alimentos, entre los que se incluyen aquellos en los cuales la contaminación por este tipo de toxinas resulta más frecuente y puede resultar más peligrosa para la salud humana. En España, previamente a lo establecido por el Reglamento (CE) Nº 1881/2006, ya se dispone de una norma reguladora, el Real Decreto 475/1988, en el cual se establecen límites máximos permitidos de aflatoxinas B1, B2, G1 y G2 en alimentos para consumo humano de 10 μg/kg para la suma de dichas aflatoxinas y de 5 μg/kg para la aflatoxina B1. Si bien es cierto que el Reglamento (CE) Nº 1881/2006 cubre los alimentos que de manera más habitual pueden representar un riesgo para la salud humana originado por la presencia de aflatoxinas, existen trabajos científicos en los cuales se ha demostrado la presencia tanto de aflatoxinas totales como de aflatoxina B1 en alimentos no incluídos en el Reglamento (CE) Nº 1881/2006 en cantidades superiores a las establecidas como límites en el Real Decreto 475/1988. Por este motivo, y sin perjuicio de las medidas de gestión que sean pertinentes, el Comité Científico considera que, en este momento y hasta que se disponga de datos representativos de la presencia de aflatoxinas en algunos alimentos no incluídos en la legislación europea tales como la chufa, el Real Decreto 475/1988 que regula los límites máximos permitidos de aflatoxinas B1, B2, G1 y G2 ofrece un nivel de protección significativo para el consumidor respecto a determinados alimentos no regulados por el Reglamento (CE) Nº 1881/2006. The Spanish Royal Decree 475/1988, approved before the Commission Regulation (EC) No 1881/ 2006, sets MRL for the aflatoxins in food for human consumption; 5 μg/kg for aflatoxin B1 and 10 μg/ kg for the sum of aflatoxin B1, B2, G1 and G2. Despite the Commission Regulation (EC) No 1881/2006 includes food which most could usually pose a risk for human health, specific papers have demonstrated the presence of total aflatoxins and aflatoxin B1 in foods not included in the Commission Regulation (EC) No 1881/2006, even at higher concentrations than those set up by the Spanish Royal Decree 475/1988. For this reason, notwithstanding the management measures that are pertinent, the scientific committee considers that, while there are no more data about maximum limits for these substances other food samples different than those included in the previously mentioned legislations (as tiger nuts or other doubtful foods regarding their producing conditions) the Royal Decree 475/1988 offers a significant protection for consumers.

Se determinaron los cambios séricos de ALT, AST, FA, promovidos por veneno de ejemplares jóvenes Crotalus durissus cumanensis. Ratones Balb/C inoculados con 0,75 mg proteínas de veneno/kg vía intraperitoneal y controles con ssf. Se obtuvieron muestras a diferentes intervalos post-inyección (1, 3, 6, 12 y 24 h) determinándose los niveles séricos de: ALT, AST, y FA, posterior al sacrificio se tomaron muestras de hígado para la valoración histopatológica. Los valores de ATL, AST y FA se incrementaron significativamente, siendo más temprano el aumento de AST (1h) y más tardío el de la FA (24). El hallazgo histopatológico del tejido hepático revelo tumefacción celular severa, células de Kuffer activadas y necrosis coagulativa focal. El incremento de los marcadores enzimáticos ALT, AST, y FA, paralelo a los daños histológicos encontrados en este estudio, sugieren daño hepato-biliar, adicionalmente, los niveles séricos de AST podrían indicar daño al tejido muscular esquelético y/o cardíaco. To determinate the effect of the venom of young Crotalus durissus cumanensis on levels of ALT, AST and AP, Balb/C mice were inoculated intraperitoneal with 0,75 mg/Kg of purified venom. Serum samples were obtained at different post-injection intervals (1, 3, 6, 12, 24h) and serum levels of ALT, AST and AP were determined. A significative difference (P ‹0,01) was observed in serum levels of ALT at 6h and at 24h post-venom injection, with a maximum of 587,64 U/L at 24 h, when compared with controls. In addition, AST shows a significative elevation between 1h and 12h post venom injection, with a maximum level of 2327 U/L at 3 h, while AP only showed significant differences at 24 h post-venom injection. These results suggest that the venom of young Crotalus durissus cumanensis induced liver-biliar injury, with a possible effect on skeletal muscle and/ or cardiac tissue.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Due to the proximity of crop and fish culture areas, some agrichemicals that could be harmful for fish could enter into fishponds by different ways, such as by leaching through rain. The contamination of the aquatic environment by pesticides is a problem of global importance because of its toxic effects on non-target organisms. These products may affect toxicological endpoints in fish. Among the effects that pesticides can cause fish is the formation of reactive oxygen species (ROS) and changes in antioxidant enzymatic and nonenzyme, which may show a situation of oxidative stress. The formation of ROS may be associated with different pathological processes in fish exposed to pollutants such as pesticides. Several biochemical and physiological responses occur when a pesticide enters the body, resulting in the adaptation of the organism to the contaminant or if the body can not efficiently metabolize these products, they may induce toxicity. Rhamdia quelen (Teleostei) were exposed to sub-lethal concentrations of methyl parathion (MP), a glyphosate based herbicide (Gly), and tebuconazole (Teb). The liver of R. quelen exposed to MP and Teb showed enhanced levels of thiobarbituric acid reactive substances (TBARS), compared to the control fish (56% and 59%, respectively). In contrast, Gly did not alter the TBARS generation. The protein carbonyl content increased only in fish exposed to Teb. Fish exposed to the three agrichemicals showed a significant decrease of catalase activity (52%, 48%, and 67%, respectively) and increased glutathione-S-transferase (57%, 46%, and 160%, respectively) activity. Fish exposed to MP, Gly, and Teb showed higher reduced glutathione (151%, 472%, and 130%, respectively, when compared to the control levels) and ascorbic acid concentrations (121%,102%, and 184%, respectively),while the non-protein thiol content increased only in R. quelen exposed to tebuconazole. Fish exposed to MP and Teb showed several pathological changes in the liver, including hepatocyte degeneration and bile stagnation. The present work reports for the first time the toxicity of the pesticide MP and the fungicide Teb in R. quelen, and as in other works, suggests the relatively lower liver toxicity of Gly for fish. The data presented herein demonstrate that sublethal concentrations of MP and Teb cause changes in oxidative stress parameters as well as hepatic cell injuries in R. quelen, and that these parameters have the potential to be developed as bioindicators of exposure to these agrichemicals. A contaminação do ambiente aquático por agrotóxicos é um problema de importância mundial, devido ao seu efeito tóxico em organismos não-alvo. Estes produtos podem afetar parâmetros toxicológicos em peixes. Entre os efeitos que os agrotóxicos podem causar em peixes está a formação de espécies reativas de oxigênio (EROs) e alterações em antioxidantes enzimáticos e não-enzimáticos, o que pode evidenciar uma situação de estresse oxidativo. Várias respostas bioquímicas e fisiológicas ocorrem quando um pesticida entra no organismo, resultando na adaptação do organismo ao contaminante ou se o organismo não consegue metabolizar com eficiência estes produtos, os mesmos podem induzir toxicidade. Alevinos de Rhamdia quelen foram expostos a concentrações subletais de parationato metílico (MP), um herbicida a base de glifosato (GLI) e tebuconazole (TEB). O fígado de R. quelen exposto a MP e TEB mostrou um maior nível de substancias reativas do ácido tiobarbitúrico (TBARS), superior nos peixes no grupo controle (56% e 59%, respectivamente). Em contraste, GLI não alterou a produção de TBARS. Ocorreu aumento da carbonilação pretéica apenas nos peixes expostos a TEB. Os peixes expostos aos três agrotóxicos mostraram uma diminuição significativa da atividade da catalase (52%, 48%,e 67%, respectivamente) e aumento da atividade da glutationa-S-transferase (57%, 46% e 160%, respectivamente). Os peixes expostos a MP, GLI e TEB apresentaram um aumento de glutationa reduzida (151%, 472% e 130%, respectivamente, quando comparado com os níveis do grupo controle) e concentrações de ácido ascórbico (121%, 102% e 184%, respectivamente), enquanto que o conteúdo de tióis-não-proteicos aumentou apenas em R. quelen expostos a tebuconazole. Os peixes expostos a MP e TEB mostraram diversas alterações histopatológicas no fígado, incluindo a degeneração dos hepatócitos e estagnação biliar. O presente trabalho relata pela primeira vez a toxicidade dos pesticidas MP e do fungicida TEB em R. quelen, e sugere que a toxicidade de GLI é relativamente menor para os peixes. Os dados apresentados demonstram que concentrações sub-letais de MP, TEB podem causar alterações nos parâmetros de estresse oxidativo, bem como lesões de células hepáticas em R. quelen e que estes parâmetros têm o potencial de serem desenvolvidos como biomarcadores de exposição a esses agrotóxicos.

Drug induced liver injury (DILI) is a type of ADR that can be very severe and result in liver failure and death. The lack of specific biomarkers or diagnostic tests for DILI led to the development of numeric scales to assess the imputability of drug in DILI cases. The aim of this present work was to describe the types of DILI and assess the agreement of three scales used for its identification: RUCAM, Maria & Victorino and Naranjo. It is a sectional study was conducted at a university hospital in Rio de Janeiro. The target population was composed of 230 inpatients between June 2006 and November 2007, who had liver injury classified according to the criteria of CIOMS (Council for Internacional Organizations of Medical Science) that consider the results of liver tests and the enzyme levels. With the use of CIOMS criteria and after the exclusion of alternative causes of liver injury, 41 inpatients were identified as suspected DILI cases. Drug causality was assessed through the use of specific scales to study cases of hepatotoxicity, RUCAM and Maria & Victorino, and with Naranjo scale, used in the evaluation of any type of ADR. Kappa statistical test was used to assess the agreement between the scales. Twenty-three men and 18 were women were evaluated. The great majority of patients (87.8%) had cholestatic liver injury. All men had cholestatic liver injury, while for women, 72.2% of the cases were cholestatic, 22.2% were hepatocelular and 5.6% were mixed cases. For each suspected drug taken by the patients, the three scales were used, with a total of 166 ratings generated for each scale. The agreement between RUCAM and M & V was Кw = 0. 2492, CI 95% (0.1293 0.3617), as compared to RUCAM and Naranjo (Кw = 0.0006). The comparison between M&V and Naranjo (Кw= 0.013) showed low agreement. Results showed low agreement between the scales used for the diagnostic of DILI. As expected, the results were a little bit higher when assessment was made with specific scales for hepatotoxicity (RUCAM and M&V), as compared to the agreement made with these scales and the global scale Naranjo, which was designed to assess all types of RAM. The use of scales to assess hepatotoxicity are an alternative to diagnosis based only on clinical judgment. However, the low agreement between the scales shows that there are limitations in definitions and methods adopted by the scores that should be reconsidered. It is hoped that a consensus on common definitions, diagnostic criteria and terminology guide the construction of a new instrument to assess causality. Reação Adversa ao Medicamento (RAM) é definida como a reação a um medicamento que é nociva e não-intencional. O dano hepático induzido por medicamento (DILI) é um exemplo de RAM que pode ser muito severa e provocar casos de transplante hepático e morte. A falta de marcadores específicos ou testes para o diagnóstico de DILI conduziu ao desenvolvimento de escalas para avaliar a imputabilidade do medicamento na ocorrência do dano hepático. O presente estudo teve como objetivo caracterizar os tipos de danos hepáticos induzidos por medicamentos e investigar a concordância de três escalas utilizadas na sua identificação: RUCAM, Maria & Victorino (M&V) e Naranjo. Trata-se de um estudo seccional realizado em um hospital universitário no Rio de Janeiro em que a população fonte foi composta por 230 pacientes internados no período de junho de 2006 a novembro de 2007, que apresentaram dano hepático segundo os critérios do CIOMS (Council for Internacional Organizations of Medical Science), que consideram os resultados dos exames hepáticos e os níveis enzimáticos. Com a aplicação dos critérios CIOMS e após a exclusão das causas alternativas de dano hepático, foram identificados 41 indivíduos com suspeita de serem portadores de dano hepático induzido por medicamento. A imputabilidade do medicamento foi investigada mediante aplicação de escalas específicas para estudo da hepatotoxicidade, RUCAM e Maria & Victorino, e pela aplicação da escala Naranjo utilizada para qualquer tipo de RAM. O estimador kappa foi utilizado para interpretação da concordância entre as escalas. Foram avaliados 23 homens e 18 mulheres e a grande maioria dos pacientes (87,8%) apresentava dano hepático do tipo colestático. Todos os homens apresentavam este tipo de dano, enquanto que entre as mulheres, a distribuição foi: 72,2% para danos colestáticos, 22,2% para danos hepatocelulares e 5,6% para danos mistos. Para cada medicamento suspeito utilizado pelos pacientes foram aplicadas as escalas RUCAM, M&V e Naranjo, o que resultou em 166 avaliações. O cálculo para avaliar a concordância entre as escalas RUCAM e M&V resultou em Кw = 0, 2492, IC 95% (0,1293- 0,3617), valor superior ao encontrado na comparação entre RUCAM e Naranjo, Кw = 0,0006. A avaliação entre as escalas M&V e Naranjo com valor de Кw= 0,013 também apontou baixa concordância. Os resultados mostraram que a concordância no diagnóstico de DILI entre as escalas utilizadas é baixa, sendo, como esperado, um pouco maior quando da avaliação das escalas específicas para hepatotoxicidade (RUCAM e M&V), quando comparada às concordâncias entre estas e a escala global de Naranjo, que foi desenhada para a avaliação de todos os tipos de RAM. A aplicação de escalas para avaliação da hepatotoxicidade representa uma alternativa ao diagnóstico baseado apenas no julgamento clínico. Entretanto, a baixa concordância entre as escalas demonstra que existem limitações das definições e dos escores adotados pelos métodos que devem ser reavaliados. Espera-se que um consenso sobre definições em comum, critérios diagnósticos e terminologias oriente a construção de um novo instrumento para avaliar causalidade.

El ensayo tuvo como objetivo indagar eventuales cambios de los indicadores sanguíneos de daño hepático durante la fase subclínica (asintomática) de la fasciolosis. Se seleccionaron 56 muestras de sangre de novillos cruza cebú aparentemente sanos, en mataderos de zonas enzoóticas de la Provincia de Corrientes (Argentina). Posteriormente al sacrificio se estableció que 29 de ellos albergaba escasa a moderada cantidad de duelas en el hígado, permaneciendo libres del parásito los restantes. Con técnicas convencionales de laboratorio se efectuó eritrograma, leucograma, proteinograma, enzimograma (fosfatasa alcalina ALP, gammaglutamil transferasa GGT, aspartato aminotransferasa AST) y determinaciones de hierro, glucosa, bilirrubina y tiempo de protrombina. Los animales parasitados revelaron valores más elevados de leucocitos, eosinófilos, gamma globulinas y GGT que los controles (p < 0,05), signos inflamatorios que se imputan a una incipiente colangitis, propia de la fase subclínica de la distomatosis. No se verificaron cambios atribuibles a colestasis ni insuficiencia hepática. The objective of the trial was to investigate eventual changes of hepatic damage blood indicators, during the subclinical (asymptomatic) period of fasciolosis. Fifty six blood samples of apparently healthy half–bred Zebu steers were collected in slaughterhouses from enzootic areas of Corrientes Province (Argentina). After slaughtering it was verified that 29 of them harbored scarce to moderate amount of liver flukes, being the rest of the animals free of parasites. Erythrogram, leukogram, proteinogram, enzymogram (alkaline phosphatase ALP, gammaglutamyl transferase GGT, aspartate aminotransferase AST) and iron, glucose, bilirubin and prothrombin time were determined by conventional laboratory techniques. Leukocyte, eosinophil, gamma globulin, and GGT values were higher in parasitized animals than in controls (p < 0.05). Such inflammatory signs are imputed to an incipient cholangitis, which is characteristic of the subclinical period of liver fluke disease. Changes attributable to cholestasis and hepatic failure were not verified.

The human being is exposed occupationally and in the environment to a variety of hepatotoxic compounds, such as the use of paints and their derivatives (2-nitropropane, 2-NP), chemical reagents (carbon tetrachloride, CCl4) and exposure to cigarette (2-NP). Therefore, it is interesting the study of therapies to prevent or even reverse the poisoning caused by these compounds. Considering that reactive oxygen species (ROS) have an important role in various diseases, especially in liver diseases, the use of antioxidant therapies should be considered. In this context, the heterocyclic compounds containing selenium in their structures have attracted the attention of researchers. Thus, this study investigated the antioxidant activity of 3-alkynyl selenophenes in models of oxidative damage in vitro and ex vivo in rats (Wistar, male, weighing 200-300g). A class of 3-alkynyl selenophene compounds with different substitutions was tested, with the objective to assess their antioxidant profile and their possible toxic effect in vitro. As a result, 3-alkynyl selenophenes had antioxidant activity, but this activity was dependent on the presence of terminal alkynes in the molecule or easy conversion to it. The possible toxic effect of 3-alkynyl selenophenes was evaluated through the activity of the enzyme δ-aminolevulinate dehydratase (δ-ALA-D) in vitro. The results showed that none of 3-alkynyl selenophenes inhibited the activity of this enzyme, suggesting that this class of compound did not present toxicity on this enzyme. From these results, selenophene h (compound that had the best antioxidant activity in vitro) was selected for the evaluation of its protective effect against oxidative damage induced by 2-NP and CCl4 (ex vivo). Selenophene h (25 mg/kg) protected against the increase of markers of liver damage (aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities) and oxidative stress induced by administration of 2-NP in rats. 2-NP induced microscopic changes, evaluated by histopathological inspections, that were protected by this compound. Selenophene h showed a protective effect against the increase of lipid peroxidation and inhibition of activity of δ-ALA-D in animals treated with 2-NP. Selenophene h protected against oxidative damage induced by CCl4 in rats. A single dose of CCl4 caused significant hepatotoxicity, evidenced by elevated plasma enzyme activity of AST and ALT, increased incidence of histopathological lesions, increased lipid peroxidation levels and the activity of Glutathione-S-transferase (GST), decreased levels of ascorbic acid and the activity of catalase and δ-ALA-D. In conclusion, 3-alkynyl selenophene protected from all these changes, confirming its hepatoprotective effect. Considering the results, we suggest that 3-alkynyl selenophene, an antioxidant, may be a useful therapy for the oxidative damage induced by 2-NP or CCl4 . O fígado apresenta extraordinária pluralidade funcional, destacando-se no controle de produção de energia, defesa imunológica e reserva volêmica. No meio ambiente e ocupacionalmente, o ser humano está exposto a uma variedade de compostos hepatotóxicos, como por exemplo, no uso de tintas e seus derivados (2-nitropropano, 2- NP), reagentes químicos (tetracloreto de carbono, CCl4) e na exposição ao cigarro (2-NP). Portanto, é interessante o estudo de terapias que previnam ou até mesmo revertam a intoxicação causada por estes compostos. Considerando que as espécies reativas de oxigênio (EROs) apresentam importante papel sobre diversas patologias, em especial nas doenças hepáticas, o uso de terapias antioxidantes deve ser considerada. Neste contexto, destacam-se os compostos heterocíclicos contendo selênio em sua estrutura. Deste modo, neste estudo investigou-se a atividade antioxidante de 3-alquinil selenofenos em modelos de dano oxidativo in vitro e ex vivo em ratos (Wistar, machos, pesando entre 200 300 g). Para esse fim, testou-se uma classe de compostos 3-alquinil selenofeno, com diferentes substituições na estrutura química, com o objetivo de avaliar o perfil antioxidante e seu possível efeito tóxico in vitro em ratos. Como resultado, 3- alquinil selenofenos tiveram atividade antioxidante, porém esta atividade foi dependente da presença de um alquino terminal na molécula ou da fácil conversão da molécula a um alquino terminal. Além disso, o possível efeito tóxico dos 3-alquinil selenofenos foi avaliado através da atividade da enzima δ-aminolevulinato desidratase (δ-ALA-D) in vitro. Os resultados obtidos demonstraram que nenhum dos 3-alquinil selenofenos testados inibiu a atividade desta enzima, sugerindo que esta classe de compostos não apresentou toxicidade sobre a atividade da δ-ALA-D. A partir destes resultados, selecionou-se o selenofeno h (que obteve melhor atividade antioxidante in vitro) para a avaliação do seu efeito protetor contra o dano oxidativo induzido por 2-NP e CCl4 em ratos (ex vivo). O selenofeno h (25 mg/kg) protegeu contra o aumento dos marcadores de dano hepático (aspartato aminotranferase (AST) e alanina aminotransferase (ALT)) e de estresse oxidativo induzidos pela administração do 2-NP. O 2-NP induziu alterações microscópicas avaliadas por inspeções histopatológicas as quais foram protegidas pelo composto. O selenofeno h demonstrou efeito protetor contra o aumento da peroxidação lipídica e inibição da atividade da δ-ALA-D nos animais tratados com 2-NP. Além disso, o selenofeno h protegeu contra o dano oxidativo induzido pelo CCl4 em ratos. Uma única dose de CCl4 causou significante hepatotoxicidade, evidenciada por elevação da atividade plasmática das enzimas AST e ALT, aumento da incidência de lesões histopatológicas, aumento dos níveis de peroxidação lipídica e da atividade da enzima glutationa-S-transferase (GST), bem como diminuição dos níveis de ácido ascórbico e da atividade das enzimas catalase e δ-ALA-D. A partir dos resultados demonstrados, verificou-se que o selenofeno h protegeu contra todas estas alterações, confirmando o seu efeito hepatoprotetor. Considerando os resultados obtidos, pode-se sugerir que o selenofeno h, uma molécula com atividade antioxidante, pode ser uma útil terapia contra o dano oxidativo induzido pelos hepatotoxicantes: 2-NP e CCl4.

44. Effects of diphenyl diselenide administration on liver damage induced by 2-nitropropane, cadmium and carbon tetrachloride

The liver presented exceptional characteristics, like controlling energy production, immunological defenses, and blood reserve. In the environment like in the work place, the human is exposed to a different kind of hepatotoxic compounds, for example, on inks and derivatives (2-nitropropane), chemical reagents (carbon tetrachloride) and in tobacco smoke (2-nitropropane and cadmium). In fact, is interesting studies of therapies which protect or ameliorated the damage induced by these compounds. Considering the growing interesting around organochalcogens, in special interest, diphenyl diselenide (PhSe)2, which posses important pharmacological properties, such as: anti-ulcer, antiinflammatory, antinociceptive, anti-hyperglycemic, protected against orofacial diskinesia induced by reserpine and halopheridol and may act on memory facilitation in mice, the hepatoprotective properties of this compound induced by different models of liver damage (2-nitropropane, cadmium and carbon tetrachloride) were examined. The results demonstrated that (PhSe)2 (100 µmol/kg) significantly reduced hepatic markers levels when compared to 2-nitropropane (2-NP) group. Treatment with diphenyl diselenide, at all doses, effectively protects against the increase of lipid peroxidation when compared to 2-NP group. In addition, histological examination revealed that 2-NP treatment causes a moderate swelling and degenerative alterations on hepatocytes and (PhSe)2 protects against these alterations. This study evidences the protective effect of diphenyl diselenide by 2-NP-induced acute hepatic damage. In addition the effect of post-treatment with (PhSe)2 on liver damage induced by 2-NP was also examined. (PhSe)2 effectively restored the increase of aminotransferase activities and urea level when compared to the 2-NP group. At the highest dose (100 mol/kg), (PhSe)2 decreased -glutamyl transferase activity (GGT) and ameliorated the increase of hepatic and renal lipid peroxidation when compared to 2-NP group. 2-NP reduced catalase activity (CAT) and did not alter superoxide dismutase activity (SOD) nor ascorbic acid level. This study points out the involvement of CAT activity in 2-NP-induced acute liver damage and suggests that the post-treatment with diphenyl diselenide was effective in restoring the hepatic damage induced by 2-NP. Similar results were obtained with cadmium (Cd), an environmental toxic metal implicated in human diseases. Cadmium content determined in the tissue of rats exposed to cadmium chloride (CdCl2) provides evidence that the liver is the major cadmium target. The concentration of cadmium in liver was about three fold higher than that in kidney, and (PhSe)2 reduced about six fold the levels of this metal in liver of rats exposed. Rats exposed to CdCl2 showed histological alterations abolished by (PhSe)2 administration. In addition, (PhSe)2 administration ameliorated plasma malondialdehyde (MDA) levels, aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH) and GGT activities increased by CdCl2 exposure. In conclusion, this study demonstrated that co-treatment with (PhSe)2 ameliorated hepatotoxicity and cellular damage in rat liver after sub-chronic exposure with CdCl2. The proposed mechanisms by which (PhSe)2 acts in this experimental protocol are its antioxidant properties and its capacity to form a complex with Cd. On the contrary, the administration of (PhSe)2 potentiated acute hepatic damage induced by carbon tetrachloride (CCl4), as manifested by an increase in biochemical parameters (AST, ALT, ALP, GGT and BT) and severe alteration in histopathology. This study also demonstrated a potentiation of lipid peroxidation levels and a consequent depletion of important antioxidant defenses including catalase and ascorbic acid, suggesting that the oxidative damage is related to the potentiation effect induced by (PhSe)2. Considering the results obtained, could be suggested that (PhSe)2 present a hepatoprotective effect depending of experimental protocol. Portanto, é interessante o estudo de terapias que previnam ou até mesmo revertam à intoxicação causada por estes compostos. Considerando o crescente interesse por compostos orgânicos de selênio, em especial o disseleneto de difenila ((PhSe)2) que possui propriedades farmacológicas mais amplas como: efeitos anti-úlcera, antiinflamatório e antinociceptivo, anti- hiperglicemiante, protege contra a discinesia orofacial induzida por reserpina e haloperidol e pode atuar na facilitação da formação de memória em camundongos. Deste modo, os efeitos hepatoprotetores deste composto frente a diferentes modelos de dano hepático (2-nitropropano, cádmio e tetracloreto de carbono) foram examinados. Os resultados obtidos neste estudo demonstraram que a administração de (PhSe)2 (100 µmol/kg) reduziu os níveis de marcadores hepáticos e os níveis de peroxidação lipídica quando comparado ao grupo tratado com 2-nitropropano (2-NP). Além disso, os exames histológicos revelaram que o tratamento com 2-NP causou alterações degenerativas nos hepatócitos e que o (PhSe)2 foi capaz de proteger, evidenciando o efeito hepatoprotetor desse composto sobre o dano hepático induzido por 2-NP. O efeito do pós-tratamento com (PhSe)2 sobre o dano hepático induzido com 2-NP também foi investigado. Este composto restaurou a atividade plasmática das enzimas aminotransferases e os níveis de uréia quando comparado ao grupo tratado com 2-NP. Na maior dose (100 mol/kg), o (PhSe)2 causou uma diminuição na atividade da enzima -glutamil transferase (GGT) e restituiu o aumento nos níveis de peroxidação lipídica hepáticos e renais quando comparado ao grupo tratado com 2-NP. O tratamento com 2-NP reduziu a atividade hepática da catalase, entretanto não alterou a atividade da superóxido dismutase (SOD) e os níveis de ácido ascórbico, sugerindo que a inibição da CAT pode estar relacionada com o aumento nos níveis de peroxidação lipídica hepática nos ratos tratados com 2-NP. Resultados similares foram encontrados quando o dano hepático foi induzido por cádmio (Cd), um contaminante ambiental implicado em várias doenças. O conteúdo de Cd determinado nos ratos expostos ao cloreto de cádmio (CdCl2) provêm evidências de que o fígado é o maior alvo da toxicidade deste metal. A concentração de cádmio no fígado foi em torno de 3 vezes maiores que os níveis encontrados no rim. O (PhSe)2 reduziu em torno de 6 vezes os níveis deste metal no fígado dos ratos expostos ao CdCl2. Além disso, a administração de (PhSe)2 causou uma redução nos níveis de malondialdeído plasmáticos (MDA), na atividade das aminotransferases, na fosfatase alcalina (ALP), lactato desidrogenase (LDH) e GGT quando comparado ao grupo tratado com cádmio. Em conclusão, esse estudo demonstrou que o tratamento concomitante com (PhSe)2 reduziu a hepatotoxicidade e o dano celular em fígado de ratos expostos ao cádmio. O mecanismo proposto para ação do (PhSe)2 pode ser devido as suas propriedades antioxidantes ou pela sua capacidade de formar um complexo com Cd. Em contraste, a administração de (PhSe)2 potencializou o dano induzido por tetracloreto de carbono (CCl4), o que foi demonstrado pelo aumento dos níveis de marcadores bioquímicos (AST, ALT, ALP, GGT and BT) e pela severa alteração na histologia. Esses estudos também demonstraram que a administração de (PhSe)2 potencializou os níveis de peroxidação lipídica com consequente depleção das defesas antioxidantes, como a catalase e o ácido ascórbico, sugerindo que o dano oxidativo está relacionado com este efeito. Considerando os resultados obtidos, podemos sugerir que o disseleneto de difenila apresenta um efeito hepatoprotetor dependendo do modelo experimental.

No. of bitstreams: 0 Previous issue date: 2007-12-01 Algumas pesquisas realizadas recentemente relatam que as células-tronco mesenquimais (CTM) podem ser induzidas à aquisição de marcadores hepatocíticos pelo transplante em modelos animais de dano hepático, ou pelo cultivo in vitro com fatores de crescimento e citocinas. O presente estudo teve por objetivo avaliar o comportamento das CTM frente à indução da diferenciação hepatocítica. As CTM foram isoladas da medula óssea de quatro doadores saudáveis, caracterizadas e submetidas ao protocolo de indução à diferenciação hepatocítica in vitro e in vivo. As células induzidas in vitro apresentaram mudanças na sua morfologia, mostrando a morfologia semelhante à do hepatócito, porém, o perfil imunofenotípico não foi modificado. As células induzidas também não apresentaram o aumento dos transcritos de albumina, citoqueratina 18 e citoqueratina 19 quando analisadas por RT-PCR em tempo real, e não alteraram a expressão de albumina, citoqueratina 18 e alfa-fetoproteína como demonstrado por imunofluorescência. Quando analisadas in vivo, as CTM demonstraram o potencial migratório para o tecido hepático danificado de camundongos imunodeficientes. Em conjunto, os resultados sugerem que as CTM da medula óssea não são capazes de se diferenciar em hepatócitos quando estimuladas in vitro pela metodologia utilizada neste estudo, mas são capazes de migrar para o tecido hepático danificado in vivo, o que sugere o seu papel no reparo do fígado. A contribuição para o reparo pode estar associada com o efeito parácrino dessas células. Palavras-chave: célula-tronco mesenquimal; diferenciação hepatocítica; albumina; citoqueratina 18; dano hepático; camundongos imunodeficientes Some recent articles have reported that mesenchymal stem cells (MSCs) can be induced to express hepatocyte markers by transplanting them into animal models of liver damage, or by in vitro culture with growth factors and cytokines. In this study, the aim is to evaluate the behavior of MSCs subjected to induction of hepatocyte differentiation. The MSCs were isolated from the bone marrow of 4 normal donors, characterized and subjected to both in vitro and in vivo induction of hepatocyte differentiation. The in vitro induced cells showed morphological changes, acquiring hepatocyte-like features. However, the immunophenotype of these cells was not modified. The induced cells exhibited no increase in albumin, cytokeratin 18 or cytokeratin 19 transcripts, when analyzed by real-time RT-PCR. The expression of albumin, cytokeratin 18 and alpha fetoprotein was also unchanged, according to immunofluorescence tests. In vivo, the MSC demonstrated a potential to migrate to damaged liver tissue in immunodeficient mice. Taken together, the results suggest that bone marrow MSCs are incapable of in vitro differentiation into hepatocytes by the approach used here, but are capable of homing to damaged hepatic tissue in vivo, suggesting a role for them in the repair of the liver. This contribution to tissue repair could be associated with a paracrine effect exerted by these cells. la Verbena carolina (Verbenaceae), conocida como verbena está ampliamente distribuida en México y es utilizada tradicionalmente en enfermedades biliares. El objetivo de este estudio fue evaluar el efecto hepatoprotector de la verbena en la intoxicación aguda con CCl4, en ratas Wistar hembras. Las ratas se protegieron con verbena y colchicina (testigo positivo) durante 7 días y el 8o día se intoxicaron con CCl4 (2.5 mL kg-1). Se midieron el tiempo de sueño inducido con pentobarbital y la DL50. El CCl4 produjo un aumento de las actividades séricas de la gamma-glutamiltranspeptidasa, alaninoaminotransferasa y fosfatasa alcalina y de la concentración de bilirrubina así como del MDA pero disminuyó el glucógeno hepático. La verbena sólo protegió parcialmente la actividad de la #947;GT. La protección de la colchicina fue parcial para todos los parámetros evaluados. El tiempo de sueño inducido por el pentobarbital no cambió. La DL50 fue mayor a 30 g kg-1.

Las aflatoxinas son metabolitos tóxicos producidos por varias especies de hongos del género Aspergillus que crecen en plantas y alimentos de origen vegetal. De entre todas ellas (B1, B2, G1, G2, M1 y M2), destaca desde el punto de vista de la seguridad alimentaria la aflatoxina B1, tanto por ser la más prevalente en alimentos como la más tóxica para los seres humanos. La Unión Europea, debido a la toxicidad de estos compuestos y con el fin de garantizar una protección eficaz de la salud pública, ha establecido mediante el Reglamento (CE) Nº 1881/2006 contenidos máximos para la aflatoxina B1 y la suma de aflatoxinas B1, B2, G1 y G2 en diversos alimentos, entre los que se incluyen aquellos en los cuales la contaminación por este tipo de toxinas resulta más frecuente y puede resultar más peligrosa para la salud humana. En España, previamente a lo establecido por el Reglamento (CE) Nº 1881/2006, ya se dispone de una norma reguladora, el Real Decreto 475/1988, en el cual se establecen límites máximos permitidos de aflatoxinas B1, B2, G1 y G2 en alimentos para consumo humano de 10 μg/kg para la suma de dichas aflatoxinas y de 5 μg/kg para la aflatoxina B1. Si bien es cierto que el Reglamento (CE) Nº 1881/2006 cubre los alimentos que de manera más habitual pueden representar un riesgo para la salud humana originado por la presencia de aflatoxinas, existen trabajos científicos en los cuales se ha demostrado la presencia tanto de aflatoxinas totales como de aflatoxina B1 en alimentos no incluídos en el Reglamento (CE) Nº 1881/2006 en cantidades superiores a las establecidas como límites en el Real Decreto 475/1988. Por este motivo, y sin perjuicio de las medidas de gestión que sean pertinentes, el Comité Científico considera que, en este momento y hasta que se disponga de datos representativos de la presencia de aflatoxinas en algunos alimentos no incluídos en la legislación europea tales como la chufa, el Real Decreto 475/1988 que regula los límites máximos permitidos de af, Aflatoxins are toxic metabolites produced by some species of molds belonging to the genus Aspergillus which grow on plants and vegetable-origin foods. Among the aflatoxins that can be found (B1, B2, G1, G2, M1 and M2), from a food safety point of view the most remarkable is aflatoxin B1, because it is the most prevalent in foods and toxic for humans. Due to the toxicity of these substances and to protect consumers’ health, the European Union has stated maximum residue limits (MRL) for aflatoxins in foods, in the Commission Regulations (EC) No 1881/2006. MRL have been established for aflatoxin B1 and the sum of B1, B2, G1 and G2 in different foods, including those in which contamination with these kind of toxins is more frequent and can be more dangerous for human health. The Spanish Royal Decree 475/1988, approved before the Commission Regulation (EC) No 1881/ 2006, sets MRL for the aflatoxins in food for human consumption; 5 μg/kg for aflatoxin B1 and 10 μg/ kg for the sum of aflatoxin B1, B2, G1 and G2. Despite the Commission Regulation (EC) No 1881/2006 includes food which most could usually pose a risk for human health, specific papers have demonstrated the presence of total aflatoxins and aflatoxin B1 in foods not included in the Commission Regulation (EC) No 1881/2006, even at higher concentrations than those set up by the Spanish Royal Decree 475/1988. For this reason, notwithstanding the management measures that are pertinent, the scientific committee considers that, while there are no more data about maximum limits for these substances other food samples different than those included in the previously mentioned legislations (as tiger nuts or other doubtful foods regarding their producing conditions) the Royal Decree 475/1988 offers a significant protection for consumers.

La fibrosis hepática progresiva es la principal causa de insuficiencia del órgano en enfermedades hepáticas crónicas de cualquier etiología. La fibrosis se desarrolla con diferentes patrones espaciales y es una consecuencia de los diferentes mecanismos prevalentes de acuerdo con las diversas causas del daño al parénquima. La biopsia hepática es el estándar de oro para evaluar la actividad inflamatoria y el estado de fibrosis. Sin embargo, esta metodología se asocia con morbilidad y mortalidad, por lo que existe la necesidad de métodos no invasivos. Marcadores séricos se refiere a la determinación de una o más moléculas en una muestra de sangre o suero como un marcador de fibrosis en el hígado. Las posibles aplicaciones de marcadores no invasivos incluyen tanto la evaluación inicial como el monitoreo de la terapia antiviral y antifibrótica y, adicionalmente pueden dar nueva información acerca de la historia natural de la progresión y regresión de la fibrosis hepática.

The reduced standard model of three populations (healthy and diseased hepatocytes, and viral load) for the study of hepatitis C virus (HCV) dynamics is compared with the model of viral dynamics with alanine aminotransferase (ALT). In terms of the threshold parameter that is the same for both models, the existence and stability of the endemic equilibrium state is determined, as well as the overall stability of the healthy individual's equilibrium state. On the other hand, an experimental study is presented that shows that it is possible to monitor liver damage without biopsies, by numerical estimation of the model parameters, considering only measurements of viral load, ALT levels and a reasonable assessment of liver damage pretreatment., Se compara el modelo estándar reducido de tres poblaciones (hepatocitos sanos y enfermos, y carga viral) para el estudio de la dinámica del virus de la hepatitis C (VHC), con el modelo de la dinámica viral con alanina aminotransferasa (ALT). En términos del parámetro umbral que es el mismo para ambos modelos, se determina la existencia y estabilidad del estado de equilibrio endémico, así como la estabilidad global del estado de equilibrio del individuo sano. Por otro lado, se presenta un estudio experimental que muestra que es posible monitorear el daño hepático sin biopsias, mediante la estimación numérica de los parámetros de los modelos, considerando solamente mediciones de la carga viral, de los niveles de ALT y una valoración razonable del daño hepático pretratamiento.a

El tetraacetato de ditiotreitol (DTTAc), compuesto estable y menos tóxico que el ditiotreitol (DTT), se sintetizó y caracterizó por resonancia magnética nuclear y espectrometría de masas. El DTTAc es hidrolizado, in vivo, a DTT. Se estableció la actividad DTTAc-tiolacetato esterasa en sangre y en distintos órganos. El DTT, generado in vivo a partir del DTTAc, ejerció un efecto protector sobre la necrosis hepática producida por el CCl4. El tratamiento disminuyó significativamente la unión covalente (UC) de los metabolitos reactivos del CCl4 a componentes celulares. Además, el DTT fue capaz de inhibir la peroxidación de lípidos (PL) in vitro y reaccionó significativamente con el peróxido de benzoílo en un sistema modelo. En conclusión, el efecto protector del DTTAc podría deberse a la hidrólisis in vivo a DTT, el cual disminuirá la UC y la PL atrapando peróxidos, hidroperóxidos y radicales libres. Dithiothreitol tetraacetate (DTTAc), which is chemically stable and less toxic than dithiothreitol (DTT), was synthesized and characterized by proton nuclear magnetic resonance and mass spectrometry. DTTAc is hydrolyzed in vivo to give DTT. DTTAc-thiolacetate esterase activity was determined in blood and in different organs. DTT, generated in vivo from DTTAc, was able to significantly prevent the CCl4-induced liver necrosis. The treatment decreased the intensity of covalent binding (CB) of the CCl4 reactive metabolites to cellular components. Also, DTT was able to inhibit lipid peroxidation (LP) in vitro, and to react significantly with benzoyl peroxide in a model system. In summary, DTTAc protective effects might be due to its in vivo hydrolysis to DTT, which would be able to decrease the intensity of CB and LP by trapping peroxides, hydroperoxides and free radical species.
