Methods: In this prospectively designed ancillary study of the PICXEL trial, the effects of first-line combination perindopril/indapamide on ambulatory BP were compared with those of monotherapy with enalapril in 127 patients. Hypertensive patients with LVH received once daily either perindopril 2 mg/indapamide 0.625 mg (n = 65) or enalapril 10 mg (n = 62) for 52 weeks. Dose adjustments were allowed for uncontrolled BP. Twenty-four-hour ambulatory BP and echocardiographic parameters were measured at baseline, week 24, and week 52. Results: At study end, both treatments significantly improved ambulatory BP compared with baseline (p ≤ 0.01). Perindopril/indapamide treatment reduced 24-hour and daytime systolic BP (SBP) and pulse pressure (PP) significantly more than enalapril treatment (p < 0.01). No significant between-group differences were noted for diastolic BP (DBP) or for night-time measurements. Trough/peak ratios were higher with perindopril/indapamide than with enalapril (88.5 vs 65.8 for SBP and 86.7 vs 63.9 for DBP, respectively). The global smoothness index was higher with perindopril/indapamide than with enalapril (6.6 vs 5.2 for SBP and 5.6 vs 4.9 for DBP, respectively). Abstract

Main causes of secondary hypertrophic cardiomyopathy include acromegalic cardiomyopathy. Heart damage in patients with the acromegaly is mediated both by the direct action of growth hormone and insulin-like growth factor-1, and increased deposition of collagen and lymphomononuclear cells in the myocardium, which leads to architectural changes, disturbances in fluid and electrolyte balance, severe left ventricular myocardial hypertrophy, diastolic and systolic left ventricular dysfunction and chronic heart failure. This article presents the world's first described observation demonstrating the possibility of successful transfemoral aortic valve repair to a comorbid patient with severe aortic stenosis according to the potential risks caused by the active form of the acromegaly.

3. Type 2 diabetes mellitus and coronary artery disease: features of perfusion volume computed tomography of the heart in a pharmacological test with adenosine triphosphate

Abstract

Aim. To study myocardial perfusion in patients with coronary artery disease (CAD) with and without type 2 diabetes mellitus (DM) using volumetric computed tomography (VCT) of the heart with a pharmacological test with adenosine triphosphate (ATP). Materials and methods. The study included 93 patients, of which 18 had CAD with DM, and 50 had CAD without DM. All patients underwent one of the stress tests, cardiac VCT with ATP test, invasive coronary angiography, or CT coronary angiography. Left ventricle (LV) myocardial perfusion was evaluated for hypoperfusion zones and the calculation of semi-quantitative indices: decrease of LV myocardial density, LV myocardial perfusion index, transmural perfusion coefficient, and our proposed new indicator myocardial perfusion reserve (MPR). Results. The MPR index value in the hypoperfusion zones in patients with CAD and DM was 0.64 [0.620.66], in patients with CAD without diabetes 0.65 [0.630.66]; p=0.4; the value of the transmural perfusion coefficient in the areas of abnormal LV myocardial perfusion in patients with CAD and DM was 0.81 [0.800.86] versus 0.83 [0.800.85] in patients with CAD without DM (p=0.6). More hypoperfusion segments were observed in patients with CAD and DM (33.3%) compared to those without DM (14%; p=0.029). The MPR index in the hypoperfusion zones in patients with CAD with intact coronary arteries (CA) and DM was 0.56 [0.540.60] versus 0.55 [0.540.62] in patients with CAD with intact CA without DM; p=0.2. Conclusion. In patients with CAD and type 2 DM, according to the VCT with ATP test, more foci hypoperfusion areas were detected, regardless of the severity of coronary artery involvement, compared with patients with CAD without DM, which may be due to the microangiopathy in the myocardium. The similarity of the MPR parameters in the hypoperfusion zones associated with hemodynamic stenosis of the CA and with intact CAs indicates the ischemic genesis of these zones. Abstract

Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract

The Mendeleev Periodic Table of Chemical Elements delivered a strong impetus to the development of fundamental and applied chemistry, chemical technology, analytical chemistry, and material sciences. Each element under the Periodic Table is an idealized substance with a certain structure and properties as defined by existing theoretical frameworks. In the real world, we deal with substances that are close in composition to the element of Periodic Table under study but differ in the presence of different elements in them – impurities that distort (sometimes radically) the structure and properties of the target research object. For many centuries, humanity has sought to obtain pure substances in order to achieve desired properties. In the second half of the 20th century, a unique collection of high purity substances was created, which includes samples representing material artifacts, prototypes of elements of Periodic Table that contain record low contents of impurity elements. With ongoing scientific and technological progress, the achieved purity of substances continuously increases and, therefore, their approximation to idealized elements of Periodic Table. This is facilitated by: new technological processes for the production and storage of high purity substances with a constant decrease in the level of impurities; the creation of isotope-friendly substances; complexes of more highly sensitive multi-element analysis methods; identification of the unique properties of high purity substances, bringing them closer to the capabilities of analog elements of Periodic Table and much more. This article is devoted to progress in these areas. Special attention is also paid to the problems in modern analytical chemistry of high purity substances and the use of the latter in the metrology of chemical analysis as the standards of comparison.

Abstract

The article presents the application of the “one hundred minus amount of impurities” approach in the development of a reference material of ascorbic acid composition using state primary and secondary measurement standards. The method of high-performance liquid chromatography (HPLC) with a detector on a diode array is used for the quantitative content of impurities of similar structure, with the highest content compared to other impurities. Traceability of certified values was provided using the calibration method of the State primary standard of units of mass (molar) fraction and mass (molar) concentration of organic components in liquid and solid substances and materials based on liquid and gas chromatography-mass spectrometry with isotope dilution and gravimetry GET 208-2019. The paper presents the use of the reference material of ascorbic acid composition to test the qualifications of laboratories, satisfactory results of the participants confirmed the applicability of the reference material for various measurement methods.

Essas ferramentas, quando as crianças enfim as adquirem e as interiorizam, funcionam como mediadores de seus processos mentais superiores.

Abstract

A novel approach is developed for the determination of Co and Au dopants in advanced materials based on tin oxide using high-resolution continuum source graphite furnace atomic absorption spectrometry (HR CS GFAAS) with direct slurry sampling. Sodium carboxylmethylcellulose (Na-CMC) is an effective stabilizer for diluted suspensions. Use Na-CMC allows to transfer the analytes into graphite furnace completely and reproducibly. The relative standard deviation obtained by HR CS GFAAS was not higher than 4%. Accuracy was proven by means inductively coupled plasma mass spectrometry (ICP-MS) in solutions after decomposition as a comparative technique. To determine Au and Co in the volume of SnO2, the acid decomposition conditions (HCl, HF) of the samples were suggested by means of an autoclave in a microwave oven.

Abstract

This paper describes the potential of high-resolution continuum source graphite furnace atomic absorption spectrometry for determination of Pt, Pd and Rh after separation and concentration by original in-house developed heterochain polymer S, N-containing sorbent. The methods of sample preparation of spent ceramic-based autocatalysts were considered, two of which were used: autoclave decomposition in mixture of acids HCl:HNO3 (3:1) and high-temperature melting with K2S2O7. Both methods anyway limit the direct determination of analytes by HR CS GFAAS. Using the first method it is an incomplete digestion of spent autocatalysts samples, since the precipitate is Si, and the rhodium metal dissolves with difficulty and partially passes into solution. In contrast to the first method, the second method allow to completely transfer analytes into solution, however, the background signal produced by the chemical composition of the flux, overlaps the analytical zone. It was found, that Pt, Pd and Rh contained in the spent ceramic automotive catalysts could be effectively separated and concentrated by heterochain polymer S, N-containing sorbent, which has high sorption capacity, selectivity and resistant to dilute acids. The chosen HR CS GFAAS analysis conditions enable us to determine Pt, Pd and Rh with good metrological characteristics. The concentrations of Pt, Pd and Rh in two samples of automobile exhaust catalysts were found in range of 0.00015-0.00050; 0.170-0.189; 0.0180-0.0210wt%, respectively. The relative standard deviation obtained by HR CS GFAAS was not more than 5%. Limits of detection by HR CS GFAAS achieved were 6.2·10(-6)wt% for Pt, 1.8·10(-6)wt% for Pd, and 3.4·10(-6)wt% for Rh. Limits of determination achieved by HR CS GFAAS were 1.1·10(-5)wt% for Pt, 6.9·10(-5)wt% for Pd, and 8.3·10(-5)wt% for Rh. To control the accuracy of PGM in sorption concentrates by HR CS GFAAS method, it was appropriate to conduct an inter-method comparative experiment. The researches on the application of atomic-emission spectroscopy method with inductively coupled plasma as a comparative method were conducted. In addition, the trueness control of the obtained results is confirmed by added-found method.

Abstract

This work investigates the analytical possibilities of high-resolution continuum source graphite furnace atomic absorption spectrometry (HR CS GFAAS) for the determination of Pd, Pt and Rh (PGMs) in spent automobile catalysts without chemical separation. A novel approach for the microwave dissolution with the mixture of HCl and H2O2 (4:1) was developed for the further HR CS GFAAS. It was found that within 20 min at the temperature 210 °C of the PGMs quantitatively and simultaneously transferred to solution. It did not show any matrix interferences in determining Pd and Rh, when using HR CS GFAAS for determining the PGMs. When determining Pt, the matrix effect can be expressly minimized by diluting the sample or applying a corrective model. Limits of quantification (μg l−1) by HR CS GFAAS achieved were 1 (Pd), 2 (Pt), and 0.5 (Rh). Correlation coefficients were 0.9973 (Pd), 0.9995 (Pt), and 0.9983 (Rh). The relative standards deviation for PGM determination by the developed procedure was not higher than 5% in the concentration range 0.0005–0.5% weight. Moreover, validation of the developed multielement method demonstrated its robustness. The trueness of experimental results was assured by comparison with the atomic emission spectrometry with inductively coupled plasma method.

Abstract

The possibility of group concentration of As, Bi, Sb, Se, and Te using sulfur- and nitrogen-containing sorbents with the subsequent determination by ICP-AES and DCA-AES with MAES in analysis of metal-containing recyclable raw material is studied. Conditions of ICP-AES and DCA-AES with MAES analysis of sorption concentrate from solutions or from solid phase are selected. The developed method makes it possible to determine the listed toxic elements in the range of mass fraction of n × 105−n × 103% (RSD = 15-7 rel %). Accuracy of the results was controlled by varying weighed samples of analytes and by comparison of the results with the known values.

Abstract

Objective: To identify individuals at high cardiovascular risk (CVR) to check for an additional estimate of CVR with the use of the ESH/ESC Guidelines (2003, 2007) in patients earlier classified as being at low and moderate risk on «SCORE». Material and methods: The study included 600 people (155 men and 445 women) with low and moderate cardiovascular risk on the SCORE scale. All patients were examined with duplex scanning of the carotid arteries (DSCA) to the determined of the thickness of the intima – media (IMT), the presence of atherosclerotic plaques (ASP); it has also been performed sphygmographic computer (SC) with automatic estimation of brachial-ankle pulse wave velocity (baPWV), biochemical analysis of blood lipid spectrum. Results: The frequency of ASP was 59.5% (357 out of 600), and a thickening of thecomplex "intima-media" (IMT)> 0.9 mm was detected in only 5% of the cases (28 persons out of 600), that indicated a slight contribution to the magnitude of the risk of such parameters as the IMT. The total number of patients with signs of preclinicallesions of the arterial wall (the presence of ASP and/or increased baPWV) was 337 (56% of 600). Our results showed that the presence of subclinical atherosclerosis is in itself a risk factor. Conclusion: The usage of instrumental methods of research (DSCA, SC) allowed to detect 32% of individuals with high CVR from 600 previously classified as low and moderate risk on SCORE scale. In our opinion, the proposed algorithm is convenient and easy to use for transfer of the patients into high-risk group.

Abstract

This article depicts the complexities of everyday life, values, and psychology in diverse mountain villages of multiethnic, multilingual Dagestan. The author begins with a historical section that outlines the significance of extended kin groups and elder councils in traditional society. Various kinds of Islam are described, including customary law (adat) and Sufi practices. The author then balances his analysis of historical legacies with extensive data on post-Soviet adaptation of ethnic-based highland "communities" (jamaat). He contributes to crucial debates concerning the radicalization of Islam in the North Caucasus by arguing for a continuity of "folk Islam" in the Soviet period and a wide range of "political Islam" in the post-Soviet period.

Fixed-dose combinations (FDCs) of antihypertensive agents improve therapeutic efficacy, according to current guidelines and large clinical studies. This Russian study examined the effect on blood pressure (BP) of substituting current ineffective antihypertensive treatment with FDC perindopril/amlodipine in patients with uncontrolled hypertension. BP was measured in the doctor’s office at each visit, daily at home, and by ambulatory monitoring (ABPM) at inclusion and end-of-study. Abstract

A simple, fast and precise method for the electrothermal atomic absorption spectrometry (ETAAS) determination of Au(III), Ag(I), Pd(II) and Pt(IV) from a single aliquot at the level of 10 −4 % in complex samples was developed. The method is based on reducing the interfering effect of Al, Fe, Cu and Ni macrocomponents on determination of Au(III), Ag(I), Pd(II) and Pt(IV) by ETAAS by adjusting the acidic sample solution to pH 9–11 using aqueous ammonia. As a result, 98.4% of Al and 99.7% of Fe precipitated smoothly at room temperature and without additional reagents needed. Furthermore, Au(III), Ag(I), Pd(II) and Pt(IV) did not co-precipitate and remained in the soluble form as ammonia complexes. While soluble ammonia Ni and Cu complexes were also produced, they decomposed in a graphite furnace at 400 °C and did not interfere with ETAAS determination of noble metals. In addition, the benefit of the ETAAS determination of Au(III), Ag(I) in the ammonia complexes is that there were no losses of Ag and Au due to the formation of insoluble AgCl and vaporization of gold and silver chlorides before atomization or incomplete dissociation in the gas phase when conventional sample treatment methods were used The relative standard deviations (R.S.D.) were in the range of 8–10% for determination of Au and Pd in aluminium catalysts, Ag in pure gold samples, and Pt in nickel tailings.

Abstract

Our experience of executing proficiency testing programs on the analysis of precious metals and precious metal bearing materials is summarized. Six proficiency testing programs of the ”distributed sample testing” type were carried out using samples essentially distinct from each other: (I) certified samples of fine gold and fine silver, and (II) recoverable raw (scrap and catalyst waste) containing precious metals of unknown contents. The test results were assessed using the London Bullion Market Association (LBMA) rules and by Z-criteria using the ISO/IEC Guide 43 scale. Satisfactory results were presented by all participants for the analysis of certified samples of fine gold and fine silver. The results of catalyst waste analysis agreed rather well, whereas the scrap analysis revealed an essential discrepancy in the data for some samples. The reasons for such discrepancies are discussed.

Abstract

Regularities of extraction of rhenium (VII) melt tetraoctyl-ammonium bromide in a mixture of higher alcohols and paraffin were determined. Quantitative extraction of measurements obtained in the range of pH of the aqueous phase 1.5–2.0 was carried out. It was shown that the degree of extraction of rhenium is reduced during the transition from sulfuric to nitric acid. The introduction of extracting 20% of higher alcohols improved the solubility of the extracted complex in paraffin. The composition of the complex, calculated, was 3.37±0.4.

Abstract

An approach to testing artificial neural networks is described. The model of neural network is based on graph theory, and operations that are used in theoretical works devoted to graphs, trees, paths, cycles, and circuits. The methods are implemented in a C++ program in the form of a set of data structures and algorithms for their processing. C++ classes are used as data structures for implementing the processing of such objects as a graph vertex, edge, oriented and undirected graph, spanning tree, circuit. Lists of standard methods (constructors and destructors, different assigning operations) are given for all classes. Additional operations are represented in details, and among them – adding one graph to another graph, adding an edge to a graph, removing edges and vertices from graph, normalizing graph, and some more. Many different searching operations are offered. Variants of graph sorting operations are also included into graph model, some of them are similar to array sorting algorithms, and some are more specific. Above these low-level operations several more complex operations are considered as graph model components. These operations include building spanning tree for arbitrary graph, building cograph for spanning tree of a graph, discovering circuits in a graph, evaluating of circuit sign, and so on. Examples of the interfaces of the most important overloaded operations on the objects used are given. An example is given of implementation of one of testing procedures where overloaded operations of graph model objects are used.
