CASE We report a patient with hypereosinophilia-associated massive osteolytic lesion of the sacrum who was admitted to our hospital. Genetic analysis revealed that atypical eosinophilic cells were positive for FIP1-like-1-platelet-derived growth factor receptor-alpha (FIP1L1-PDGFRA) fusion gene. Treatment was initiated with oral administration of imatinib mesylate. The patient responded rapidly to this medication with a marked reduction in eosinophilia both from the peripheral blood and FIP1L1-PDGFRA fusion gene in the bone marrow within 2 weeks, followed by gradual osteosclerotic repair of the sacrum. CONCLUSIONS This case study found that the drug imatinib proved very effective in the treatment of this rare condition.

This tin-catalyzed reaction was highly atom-economical and required no other metal reagents.

はじめに）近年，予後不良といわれる肺癌の治療において化学療法・放射線治療・分子標的治療などの集学的治療の進歩により長期予後が得られるケースも出てきた．今回我々は肺癌の脊椎転移により脊髄麻痺を合併した手術した症例6例を経験したので評価し，報告する．対象と方法）2005年5月から2009年8月に肺癌の脊椎転移に対し手術を行った6症例（54歳から79歳まで 男性3例 女性3例)を対象とした．6症例中5例は当院で姑息的手術を施行し1例は他院に紹介して全脊椎切除・脊柱再建術を施行した．手術前後の状態をFrankel分類の改善・Performace Status（PS）の改善・術後生存中の歩行可能期間の3項目で評価した．結果）術前FranckelB：3例，C：1例，D：2例であったのに対して術後1カ月の時点でD: 5例，C：1例へ改善した．PSはGrade2：1例，Grade3：2例，術前Grade4：3例が術後はGrade1：3例，Grade2：1例，Grade3：2例に改善し5例が自宅退院可能となった．6例中4例で生存中の80％以上の期間で歩行可能であった．考察と結語）手術を行った6例中全例で麻痺の改善を認め，脊椎転移症例の手術（放射線治療，そのほかの薬物治療の併用）は患者の限られた生命予後期間におけるPSを高める点で極めて有効であると考えられた．

We address a decentralized control method for object transportation in coordination by a leader-follower type multiple robot system. The proposed system consists of a pushing leader, a robot without grasping mechanisms, and multiple follower robots. During the object transportation, a desired trajectory is given to the leader robot only, and follower robots estimate the trajectory of the leader based on force/moment from the object. In the proposed system, a variable internal force is introduced to each robot’s controller in decentralized style to let the follower’s estimator work on not only the pushing but also the “pulling” case that the leader needs to slow down or stop the object. Finally, a robot system including three omnidirectional mobile robots is presented and an experiment result is shown to illustrate the concept of the proposed control algorithm.
