Abstract

A study conducted on cloned ovine brain natriuretic peptide has yielded the first evidence for the presence of an amino-terminal form of brain natriuretic peptide called NT-proBNP in a species other than human and its enhanced secretion in ovine models of cardiac overload. Among other implications of the results, the confirmation of NT-proBNP as a circulating peptide calls for experiments addressing its possible bio-activity.

Abstract

Pathophysiological levels of atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) were co-infused into normal male subjects to determine if there are synergistic effects between ANP and BNP. Both peptides have similar physiological effects and are released into the plasma following myocardial infarction of congestive heart failure. The results show that co-infusion causes additive physiological effects, probably by causing the dissociation of pre-bound peptides from their clearance or biological receptors.

Abstract

Short term ouabain treatment does not change the plasma concentration of vasoactive hormones or pressor responsivesness in healthy males. Ouabain treatment decreases heart rate and enhances pulse pressure, but has no influence on pressor responsiveness to either norepinephrine or angiotensin II. Ouabain also does not influence the circulating quantities of renin, epinephrine norepinephrine, aldosterone, angiotensin II, brain natriuretic peptide and atrial natriuretic peptide.

Abstract

Atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP), heart secreted hormones in humans that circulate in the plasma, exhibit the same natriuretic endocrine and hemodynamic effects. A study of eight normal men receiving constant infusions of ANP with and without superimposed infusions of BNP, with a random order design revealed the interaction of human BNP and ANP. The important BNP or ANP interactions of metabolism at plasma levels occurred in heart failure.

Abstract

Objective: Flow abnormalities which occur in AF patient due to irregular heart rate which resultant turbulent flow both in left atrium and systemically may lead to endothelial dysfunction. The purpose is to detect endothelial dysfunction in non rheumatic AF patient. And test the hypothesis that endothelial dysfunction is reversible upon restoration of normal sinus rhythm and correction of the blood flow dynamics. Method: Endothelium-dependent (flow-mediated dilation) vasodilator function of brachial artery was measured using high resolution ultrasound in 30 patients with persistent non rheumatic AF who were scheduled for elective electrical cardioversion and in 10 control subjects. In patients who remained in sinus rhythm after cardioversion, these measurements were repeated after one month and two months. Result: Compared with control subjects, patients showed lower FMD during AF (6.66Â ± 1.62% vs 14.29Â ± 2.93%, p < 0.001). In patients who remained in sinus rhythm, FMD increased at both one month (6.66Â ± 1.62% vs 10.71Â ± 2.81%, p < 0.001) and two months (6.66Â ± 1.62% vs 14.28Â ± 3.48%, p < 0.001). Conclusion: There is endothelial dysfunction associated with non rheumatic persistent AF patients which is reversible upon restoration of normal sinus rhythm and correction of blood flow dynamics.

Abstract

Human brain natriuretic peptide-32 (BNP) (i.e. proBNP(77–108)), the mature form of BNP and secreted predominantly by the cardiac ventricle, is formed from a high molecular weight precursor, proBNP(1–108). We have recently identified the aminoterminal form proBNP(1–76) (NT-proBNP) in human plasma but its source, metabolism and production in circulatory disorders are unknown. We have investigated the relationship between immunoreactive (IR) NT-proBNP and BNP-32 in normal and hypertensive subjects and in patients with cardiac impairment, as well as the regional plasma concentrations in patients undergoing routine cardiac catheterization.

24. Neuroendocrine prediction of left ventricular function and heart failure after acute myocardial infarction

Abstract

Objective To determine the relations of plasma levels of brain natriuretic peptide (BNP), atrial natriuretic factor (ANF), N-terminal ANF (N-ANF), cyclic guanosine monophosphate (cGMP; the cardiac peptide second messenger), and plasma catecholamines to left ventricular function and to prognosis in patients admitted with acute myocardial infarction. Design Plasma hormones and ventricular function (radionuclide ventriculography) were measured 1-4 days after myocardial infarction in 220 patients admitted to a single coronary care unit. Radionuclide scanning was repeated 3-5 months after infarction. Clinical events were recorded over a mean period of 14 months. Results Both early and late left ventricular ejection fraction (LVEF) were most closely related to plasma BNP (r = -0.60, n = 220, p &lt; 0.001; and r = -0.53, n = 192, p &lt; 0.001, respectively), followed by ANF, N-ANF, cGMP, and the plasma catecholamines. Early plasma BNP concentrations less than twofold the upper limit of normal (20 pmol/l) had 100% negative predictive value for LVEF &lt; 40% at 3-5 months after infarction. In multivariate analysis incorporating all the neurohormonal factors, only BNP remained independently predictive of LVEF &lt; 40% (p &lt; 0.005). Survival analysis by median levels of candidate predictors identified BNP as the most powerful discriminator for death (p &lt; 0.0001). No early deaths (within 4 months) occurred in patients with plasma BNP concentrations below the group median (27 pmol/l), and over follow up only three of 26 deaths occurred in this subgroup. Of all episodes of left ventricular failure, 85% occurred in patients with plasma BNP above the median (p &lt; 0.001). In multivariate analyses, BNP alone gave additional predictive information beyond sex, age, clinical history, LVEF, and plasma noradrenaline for both subsequent onset of LVF and death. Conclusions Plasma BNP measured within 1-4 days of acute myocardial infarction is a powerful independent predictor of left ventricular function, heart failure, or death over the subsequent 14 months, and superior to ANF, N-ANF, cGMP, and plasma catecholamines.
