A 50-year-old female patient, presenting with a past history of Reynaud's syndrome, xerostomia and xerophthalmia, was admitted to Fujian Provincial Hospital because of coughing for 10 days and left anterior chest pain for 1 day. Chest imaging showed multiple nodules and masses, and diffuse cystic lesions in both lungs. Based on the differential diagnosis of multiple pulmonary masses and diffuse cystic lesions respectively, autoantibodies, radionuclide dynamic imaging of the parotid, positron emission tomography-CT, CT-guided percutaneous transthoracic needle biopsy, and other examinations were performed. The diagnosis of diffuse large B-cell lymphoma stage ⅣA (lung, parotid gland) and Sjögren's syndrome was confirmed. By analyzing the imaging features and pathogenesis in detail, the diffuse cystic lesions of both lungs were considered to be related to lymphocytic interstitial pneumonia caused by Sjögren's syndrome. The pulmonary space-occupying lesions in the lungs were significantly absorbed after RCHOP regimen for lymphoma.患者女，50岁，既往存在雷诺综合征、口干、眼干。此次因“咳嗽10 d，左前胸痛1 d”入院，胸部影像学提示双肺多发结节、团块，双肺弥漫性囊性病变。以“双肺多发占位、双肺弥漫性囊性病变”作为鉴别切入点，完善相关化验检查、经皮肺病灶穿刺活检，确诊弥漫大B细胞淋巴瘤ⅣA期（肺、腮腺）、干燥综合征。并通过详细分析影像特点及发病机制，考虑双肺弥漫性囊性病变与干燥综合征导致的肺部并发症，淋巴细胞间质性肺炎相关。而淋巴瘤经RCHOP方案治疗后肺部占位明显吸收。.

To understand the prevalence of soil-transmitted nematode infections in Yancheng City from 2016 to 2020, so as to provide insights into the development of the scheme for further soil-transmitted nematodiasis surveillance.Soil-borne nematode infections were detected using the modified Kato-Katz method (two slides for one sample) among the permanent residents living in Yancheng City from 2016 to 2020, andA total of 51 259 person-time residents were detected for soil-borne nematode infections in Yancheng City from 2016 to 2020, and 48 egg-positives were identified, with a 0.09% prevalence. The soil-borne nematodes infecting residents includedThe prevalence of soil-borne nematode infections appears a gradual decline in residents living in Yancheng City from 2016 to 2020, and is at an extremely low level. Further surveillance of soil-transmitted nematodiasis requires to be intensified targeting key regions and populations to consolidate the control achievements.

The notion of permeability is very important in understanding and modeling the flow behavior of fluids in a special type of porous medium (i.e., the underfill flow in flip-chip packaging). This paper presents a new concept regarding permeability in a porous medium, namely two types of permeability: superficial permeability (with consideration of both the pore cross-sectional area and the solid matrix cross-sectional area) and pore permeability (with consideration of the pore cross-sectional area only). Subsequently, the paper proposes an analytical model (i.e., equation) for the pore permeability and superficial permeability of an underfill porous medium in a flip-chip packaging, respectively. The proposed model along with several similar models in literature is compared with a reliable numerical model developed with the computational fluid dynamics (CFD) technique, and the result of the comparison shows that the proposed model for permeability is the most accurate one among all the analytical models in literature. The main contributions of the paper are as follows: (1) the provision of a more accurate analytical model for the permeability of an underfill porous medium in flip-chip packaging, (2) the finding of two types of permeability depending on how the cross-sectional area is taken, and (3) the correction of an error in the others' model in literature.

A series of 6-acetyl-coumarin derivatives (2a-n) were synthesized and evaluated for antiretroviral activity in C8166 T-cell line infected with HxBru-Gluc strain of human immunodeficiency virus-1. Michael Addition followed by re-aromatization and subsequent acid-induced elimination of water leads to the formation of coumarin intermediate, which undergoes Claisen-Schmidt condensation with substituted bezaldehydes using silica sulphuric acid as a catalyst to form 6-acetyl-coumarin derivatives, 2a-n. In silico absorption, distribution, metabolism, excretion and toxicity parameters of compounds 2a-n were found within their reference limits. All synthesized compounds were devoid of cytotoxicity as they have shown cell viability count more than 80 % in cytotoxicity assay. Compounds 2a, 2g and 2h showed potent inhibitory activity against human immunodeficiency virus infection with IC50 value of 4.7, 4.5 and 0.35 μM, respectively. It was found that electron-withdrawing group at phenyl ring, attached to the coumarin nucleus was crucial for activity against human immunodeficiency virus. The present study may be helpful in the development of some potent antiretroviral agents.

To explore the value of 64-section computed tomography (CT) perfusion imaging (CTPI) in the early diagnosis of acute radiation-induced lung injury (ARILI).Fifty-one patients with oesophageal cancers or malignant thymomas received postoperative radiation therapy with a 60-62 Gy dose and underwent CTPI at pre- and post-radiation therapy time points (week 0, week 4, week 8, and week 12 respectively). The CTPI values were prospectively compared and analysed in order to evaluate the diagnostic utility of CTPI in the early diagnosis of ARILI.Eighteen cases (18/51) of ARILI were diagnosed. The mean values of relative regional blood flow (rrBF), relative regional volume (rrBV), and relative regional permeability surface (rrPS) in the ARILI group were correspondingly higher than those of the non-ARILI group. At week 4, rrBF, rrBV, and rrPS in the ARILI group were significantly higher than those at pre-radiation (each p0.05). In the non-ARILI group, rrBF and rrBV were higher than those at pre-radiation (each p0.05); however, rrPS was not statistically different from that of pre-irradiation. Applying the diagnostic threshold value of rrPS = 1.22, the sensitivity, specificity, and positive and negative predictive values of CTPI for early diagnosis of ARILI were better than those of CT.CTPI metrics may reflect haemodynamic changes in the post-irradiation lung and can detect cases of early ARILI that appear normal at CT. CTPI is a promising technique for early diagnosis of ARILI.

To study brain networks of temporal lobe epilepsy (TLE) to investigate whether TLE brain dysfunction have an impact on depression, using resting state functional magnetic resonance (RS-fMRI) detection technology.A total of 18 patients with TLE were included in this study. According to Beck Depression Inventory (BDI) and Hamilton's Depression Scale (HAMD)-17 score, we divided them into two groups: depression group 9 cases, non-depression group 9 cases. All patients underwent 3.0T MRI , RS-fMRI and magnetic resonance spectroscopy (MRS) examinations and then the results were analyzed.Disease course of depression group was longer than non-depression group and the difference was statistically significant (P0.05). RS-fMRI examination showed that depression group had more active brain areas and more extral temporal active areas than non-depression group (P0.05). By compared with the non-depression group, we found more strong active brain areas including thalamus, and the default-mode network which involved in prefrontal cortex, precuneus, ventral anterior cingulate and hippocampus. We found the NAA and NAA/Cho+ Cr of the hippocampus which were ipsilateral to the advantage discharge side were decreased in 5/9 cases with depression in MRS and 3/5 cases had hippocampal atrophy, while the non-depression group had no obvious abnormalities.The brain default-mode network activity in TLE patients with depression is increased and there is more extral temporal activation than the non-depression group; furthermore abnormal hippocampus structure is more common in depression group, which suggests that epileptic brain dysfunction may affect the development of depression.

Degradation of scaffolds is an important problem in tissue regeneration management. This paper reports a comparative study on degradation of the printed 3D poly (lactic-co-glycolic acid) scaffold under three conditions, namely, micro-channel, incubator static, and incubator shaking in the phosphate buffer saline (PBS) solution. In the case of the micro-channel condition, the solution was circulated. The following attributes of the scaffold and the solution were measured, including the mass or weight loss, water uptake, morphological and structural changes, and porosity change of the scaffold and the pH value of the PBS solution. In addition, shear stress in the scaffold under the micro-channel condition at the initial time was calculated with Computational Fluid Dynamics (CFD) to see how the shear stress factor may affect the morphological change of the scaffold. The results showed that the aforementioned attributes in the condition of the micro-channel were significantly different from the other two conditions. The mechanisms that account for the results were proposed. The reasons behind the results were explored. The main contributions of the study were (1) new observations of the degradation behavior of the scaffold under the micro-channel condition compared with the conditions of incubator static and incubator shaking along with underlying reasons, (2) new understanding of the role of the shear stress in the scaffold under the condition of the micro-channel to the morphological change of the scaffold, and (3) new understanding of interactions among the attributes pertinent to scaffold degradation, such as weight loss, water uptake, pH value, porosity change, and morphological change. This study sheds important light on the scaffold degradation to be controlled more precisely.

Expression of the HIV Nef protein results in the down-regulation of cell surface expression of CD4, with a di-leucine motif in the cytoplasmic domain of CD4 being required for this effect. However, our results indicate that this motif is not sufficient to confer sensitivity to down-regulation by Nef. Using site-directed mutagenesis and a transient expression system, we demonstrate that an alpha-helical stretch of amino acids of the cytoplasmic tail of CD4 is also required for the down-regulation of CD4 induced by Nef. Some CD4 mutations allowed the discrimination between PMA- and Nef-induced down-regulation, suggesting the existence of multiple pathways. In addition, our results demonstrate that this motif is involved in the association of CD4 with the tyrosine kinase p56lck, thus defining a multifunctional domain of CD4. Although there is overlap between the sequence requirement for lck association and susceptibility to Nef, we fail to detect any preferential decrease in lck association with CD4 when Nef is expressed during acute HIV infection. Altogether, these results demonstrate that there is an overlapping, but noncompetitive, sequence requirement in the cytoplasmic domain of CD4 for lck association and down-regulation by Nef.

Aim To describe computed tomography (CT) imaging and histopathological manifestations of renal epithelioid angiomyolipomas (EAMLs) for better understanding and cognition in the diagnosis of this new category of renal tumours. Materials and methods Clinical data and CT images from 10 cases of EAML were retrospectively analysed. All patients underwent CT with and without contrast medium administration, with multiplanar reconstruction (MPR) when needed. Results Plain CT manifestations of EAMLs were a higher density of mass (10–25 HU) than renal parenchyma, bulging contour of the involved kidney, absence of fat, distinct edges without a lobulate appearance. Contrast-enhanced CT features were markedly heterogeneous enhancement (from rapid wash-in to slow wash-out), large tumour size without lobular appearance, complete capsule with distinct margins and frequent mild necrotic areas. Histopathological features were epithelioid cells with eosinophilic cytoplasm, large and deeply stained nuclei, and dense arrangement of tumour cells with patchy necrosis; diffuse sheets of epithelioid cells were positive for HMB-45 (melanoma-associated antigen) and negative for epithelial membrane antigen (EMA) staining. Conclusion Multiple specific CT features correlated well with the histopathology and may play an important role in the primary diagnosis of EAMLs.

The 96-amino acid Vpr protein is the only virion-associated regulatory protein encoded by the human immunodeficiency virus type 1 (HIV-1). Vpr incorporation into the viral particle is most likely due to an interaction with a viral structural protein. Recent data have shown that DNA encoding for the p55 Gag precursor protein (Pr55gag) is the minimal viral genetic information necessary for Vpr incorporation. Other studies have suggested that the p6 portion of Pr55gag, which is unique to lentiviruses, is involved in Vpr incorporation. To investigate the mechanism of incorporation of Vpr into HIV-1 virions, COS-7 cells were cotransfected with ptrENV, an expression vector that encodes all of the HIV-1 regulatory proteins including Rev and Vpr, and different constructs of pIIIgagCAR, a rev-dependent Gag expression plasmid that encodes Pr55gag and the viral protease. Virions produced from gag constructs containing a premature p6 termination codon at positions Leu-1, Ser-17, Tyr-36, or Leu-44 lacked detectable Vpr. In contrast, gag constructs with double Pro-10-Pro-11 substitutions for Leu-10-Leu-11 or a premature termination codon at position Pro-49 of p6 were still able to incorporate Vpr, however, with lower efficiency than wild type. The mutations described in this study affected directly two short regions within the p6 domain, which are highly conserved among primate immunodeficiency viruses. Our results suggest that the conserved (P-T/S-A-P-P) and (L-X-S-L-F-G) motifs located near the N-terminus and C-terminus, respectively, of the p6 domain of Gag are critical for Vpr incorporation into HIV-1 virions.

The human immunodeficiency virus type-1 (HIV-1) encoded Vpu protein facilitates the release of budding virions from the surface of infected cells and delays the rate of syncytium formation of the virus. Furthermore, Vpu induces rapid degradation of nascent CD4 molecules that are retained in the endoplasmic reticulum by the formation of gp160-CD4 complexes. Currently, little is known of the precise mechanism(s) by which Vpu function. Whether or not these different events are related remain unclear. In this report, we describe our recent structure/function studies on vpu suggesting that the protein may have independent biological activities during the HIV-1 infection.

This paper reports the synthesis of eleven N-(4-carbomethoxy-4-phthalimidobutanoyl)-N-substituted glycines (VII1-9), proline (VII10) and pyroglutamic acid (VII11) expected to have inhibitory activity on angiotensin converting enzyme. All of the compounds mentioned above and the corresponding t-butyl esters of VII1-9 were not reported in the literature previously. The structures were confirmed through their IR, 1HNMR, MS spectra and elemental analysis. In preliminary test in rats, compounds VII8, VII9 and VII10 showed marked hypotensive activity.

To investigate the role of vpu in the cytopathicity of human immunodeficiency type 1 (HIV-1), the MT4 CD4+ T-cell line was infected with viruses that were isogenic except for their ability to produce the vpu protein. The experiments described here demonstrate that expression of vpu reduces HIV-1 cytopathic effects by decreasing the rate of syncytia formation. By reducing the concentration of gp 120 at the cell surface, vpu limits cell killing by syncytia formation.

Mice were inoculated intracerebrally with a lethal dose of herpes simplex virus type 2. Three days later, the mice were treated intraperitoneally, twice daily for 4 days, with the following drugs alone or in combination: acyclovir (ACV), vidarabine (ara-A), 2'-fluoro-5-iodoaracytosine (FIAC), and 2'-fluoro-5-methylarauracil (FMAU). Despite delayed treatment, most of the animals receiving low doses of FMAU alone or in combination with ACV or ara-A survived. In contrast, significantly higher mortality rates were noted in mice receiving ara-A, ACV, or FIAC alone. The data were analyzed for quantitation of synergism, additivity, and antagonism of multiple drug effect by the median effect method. The median effective doses (in nanomoles per kilogram per day) calculated in this manner were: FMAU, 22.5; FIAC, 510; ara-A, 901; ACV, 7,587; ACV-ara-A (drug ratio, 1:1), 550; FIAC-ara-A (1:1), 376; FIAC-ACV (1:1), 133; FMAU-ACV (1:8), 60.3; and FMAU-ara-A (1:8), 65.2. Marked synergy was found throughout a wide range of effect levels with the five different combinations, with no increased toxicity over the single-drug treatments. Similar results were obtained when the data were analyzed by the isobologram method. Since many patients with severe herpetic infections, such as herpes encephalitis, have a poor prognosis despite single-drug therapy, the possible use of combinations including low doses of FMAU deserves further investigation.

We determined whether drug-resistant variants of HIV-1 could be isolated from the peripheral blood mononuclear cells of 20 individuals with HIV infection (Centers for Disease Control groups II and III) on long-term zidovudine (AZT) therapy. Toward this end, zidovudine (10 microM) has been included in the tissue culture medium used to isolate HIV-1. Under these circumstances, virus with a zidovudine-resistant phenotype was successfully obtained in five out of 20 cases. This property of drug resistance appeared to be stable, and did not disappear upon extended replication of such virus in the absence of drug pressure. Drug-resistant virus could also be isolated from these subjects on subsequent occasions, but was not present in samples obtained prior to therapy. Replication of these zidovudine-resistant isolates in tissue culture was inhibited by each of four other nucleoside analogues. Thus, other drugs may be useful in controlling selective zidovudine-resistant variants of HIV-1.
