1. Safety and Stability of Pulmonary Function in Patients with Decreased Respiratory Function Treated for Spasticity with OnabotulinumtoxinA

Abstract

Two randomized, placebo-controlled studies evaluated the pulmonary function safety of onabotulinumtoxinA (onabotA) for treatment of upper and/or lower limb spasticity. Patients with stable baseline respiratory status received one or two treatments with placebo, 240 U, or 360 U of onabotA. Pulmonary function tests, adverse events, and efficacy were measured at least every 6 weeks for 18 weeks (Study 1) or 30 weeks (Study 2). Study 1 enrolled 109 patients (n = 36–37/group) and Study 2 enrolled 155 patients (n = 48–54/group). Mean baseline forced vital capacity (FVC) was 76–78% of predicted per group in Study 1 and 71% of predicted per group in Study 2. In Study 1, change from baseline FVC values were significantly (p < 0.05) decreased vs. placebo at weeks 3 (240 U −57 mL vs. placebo +110 mL) and 12 (360 U −6 mL vs. +167 mL placebo). In Study 2, change from baseline FVC values were significantly decreased in the 360 U group vs. placebo at weeks 6 (−78 mL vs. +49 mL placebo), 13 (−60 mL vs. +119 mL placebo), 18 (−128 mL vs. +80 mL placebo), and 24 (−82 mL vs. +149 mL placebo). Individual pulmonary function-related adverse events were not correlated with PFT decreases. The most frequent pulmonary-related adverse events were nasopharyngitis (Study 1) and upper respiratory tract infection (Study 2). Ashworth scores were significantly improved at multiple time points in both studies. Injection of onabotA for spasticity in patients with decreased pulmonary function, at single and repeated doses of up to 360 U, was associated with small but statistically significant decreases in FVC or forced expiratory volume 1 s (FEV1) (>12% and 200 mL) that were subclinical and not correlated with any adverse clinical pulmonary events.

Two drops were dispensed every four hours four times daily.Results: Regardless of bottle orientation and shaking, all difluprednate emulsion concentrations at each time point analyzed were within 15% of declared concentration. Both branded and generic prednisolone acetate suspension concentrations varied substantially throughout the study. For the bottle stored upright and not shaken, 46% of the branded concentrations were not within 15% of declared concentration; for the bottle stored upright and shaken prior to use, 60% failed to meet this criterion. None of the branded concentrations from the inverted and not shaken bottle was within 15% of declared concentration. Generic prednisolone concentrations demonstrated the poorest dose uniformity, with 96% of the concentrations from the inverted and not shaken bottle not within 15% of declared concentration; 94% of the concentrations from the upright and shaken bottle and 87% from the upright and not shaken bottle similarly failed to meet this criterion.Conclusions: Dose uniformity of Durezol emulsion was predictable in all simulated patient usage conditions, whereas the drop concentrations of Pred Forte and generic prednisolone acetate suspensions were highly variable throughout the study. 3. A controlled investigation of optimal internal medicine ward team structure at a teaching hospital.

Abstract

BACKGROUND: The optimal structure of an internal medicine ward team at a teaching hospital is unknown. We hypothesized that increasing the ratio of attendings to housestaff would result in an enhanced perceived educational experience for residents. METHODS: Harbor-UCLA Medical Center (HUMC) is a tertiary care, public hospital in Los Angeles County. Standard ward teams at HUMC, with a housestaff∶attending ratio of 5:1, were split by adding one attending and then dividing the teams into two experimental teams containing ratios of 3:1 and 2:1. Web-based Likert satisfaction surveys were completed by housestaff and attending physicians on the experimental and control teams at the end of their rotations, and objective healthcare outcomes (e.g., length of stay, hospital readmission, mortality) were compared. RESULTS: Nine hundred and ninety patients were admitted to the standard control teams and 184 were admitted to the experimental teams (81 to the one-intern team and 103 to the two-intern team). Patients admitted to the experimental and control teams had similar age and disease severity. Residents and attending physicians consistently indicated that the quality of the educational experience, time spent teaching, time devoted to patient care, and quality of life were superior on the experimental teams. Objective healthcare outcomes did not differ between experimental and control teams. CONCLUSIONS: Altering internal medicine ward team structure to reduce the ratio of housestaff to attending physicians improved the perceived educational experience without altering objective healthcare outcomes.

Abstract

BACKGROUND: Smokers manifest varied phenotypes of pulmonary impairment. RESEARCH QUESTION: Which pulmonary phenotypes are associated with coronary artery disease (CAD) in smokers? STUDY DESIGN AND METHODS: We analyzed data from the University of Pittsburgh COPD Specialized Center for Clinically Oriented Research (SCCOR) cohort (n = 481) and the Genetic Epidemiology of COPD (COPDGene) cohort (n = 2,580). Participants were current and former smokers with > 10 pack-years of tobacco exposure. Data from the two cohorts were analyzed separately because of methodologic differences. Lung hyperinflation was assessed by plethysmography in the SCCOR cohort and by inspiratory and expiratory CT scan lung volumes in the COPDGene cohort. Subclinical CAD was assessed as the coronary artery calcium score, whereas clinical CAD was defined as a self-reported history of CAD or myocardial infarction (MI). Analyses were performed in all smokers and then repeated in those with airflow obstruction (FEV(1) to FVC ratio, < 0.70). RESULTS: Pulmonary phenotypes, including airflow limitation, emphysema, lung hyperinflation, diffusion capacity, and radiographic measures of airway remodeling, showed weak to moderate correlations (r < 0.7) with each other. In multivariate models adjusted for pulmonary phenotypes and CAD risk factors, lung hyperinflation was the only phenotype associated with calcium score, history of clinical CAD, or history of MI (per 0.2 higher expiratory and inspiratory CT scan lung volume; coronary calcium: OR, 1.2; 95% CI, 1.1-1.5; P = .02; clinical CAD: OR, 1.6; 95% CI, 1.1-2.3; P = .01; and MI in COPDGene: OR, 1.7; 95% CI, 1.0-2.8; P = .05). FEV(1) and emphysema were associated with increased risk of CAD (P < .05) in models adjusted for CAD risk factors; however, these associations were attenuated on adjusting for lung hyperinflation. Results were the same in those with airflow obstruction and were present in both cohorts. INTERPRETATION: Lung hyperinflation is associated strongly with clinical and subclinical CAD in smokers, including those with airflow obstruction. After lung hyperinflation was accounted for, FEV(1) and emphysema no longer were associated with CAD. Subsequent studies should consider measuring lung hyperinflation and examining its mechanistic role in CAD in current and former smokers.

Abstract

RATIONALE: Multiple studies have demonstrated an increased risk of chronic obstructive pulmonary disease (COPD) in heterozygous carriers of the AAT (alpha-1 antitrypsin) Z allele. However, it is not known if MZ subjects with COPD are phenotypically different from noncarriers (MM genotype) with COPD. OBJECTIVES: To assess if MZ subjects with COPD have different clinical features compared with MM subjects with COPD. METHODS: Genotypes of SERPINA1 were ascertained by using whole-genome sequencing data in three independent studies. We compared outcomes between MM subjects with COPD and MZ subjects with COPD in each study and combined the results in a meta-analysis. We performed longitudinal and survival analyses to compare outcomes in MM and MZ subjects with COPD over time. MEASUREMENTS AND MAIN RESULTS: We included 290 MZ subjects with COPD and 6,184 MM subjects with COPD across the three studies. MZ subjects had a lower FEV(1)% predicted and greater quantitative emphysema on chest computed tomography scans compared with MM subjects. Abstract

BackgroundBody composition measures, specifically low weight or reduced muscle mass, are associated with mortality in COPD, but the effect of longitudinal body composition changes is undefined.Research questionIs the longitudinal loss of fat-free mass (FFM) associated with increased mortality, including in those with initially normal or elevated body composition metrics?Study design and methodsParticipants with complete data for at least one visit in the COPDGene study (n= 9,268) and the ECLIPSE study (n= 1,760) were included and monitored for 12 and 8 years, respectively. Pectoralis muscle area (PMA) was derived from thoracic CT scans and used as a proxy for FFM. A longitudinal mixed submodel for PMA and a Cox proportional hazards submodel for survival were fitted on a joint distribution, using a shared random intercept parameter and Markov chain Monte Carlo parameter estimation.ResultsBoth cohorts demonstrated a left-shifted distribution of baseline FFM, not reflected in BMI, and an increase in all-cause mortality risk associated with longitudinal loss of PMA. For each 1-cm2 PMA loss, mortality increased 3.1%(95%CI, 2.4%-3.7%; P< .001) in COPDGene, and 2.4%(95%CI, 0.9%-4.0%; P< .001) in ECLIPSE. Increased mortality risk was independent of enrollment values for BMI and disease severity [BODE (body mass, airflow obstruction, dyspnea, and exercise capacity) index quartiles] and was significant even in participants with initially greater than average PMA.InterpretationLongitudinal loss of PMA is associated with increased all-cause mortality, regardless of BMI or initial muscle mass. Consideration of novel screening tests and further research into mechanisms contributing to muscle decline may improve risk stratification and identify novel therapeutic targets in ever smokers.

Abstract

Background Attenuation of transforming growth factor β by blocking angiotensin II has been shown to reduce emphysema in a murine model. General population studies have demonstrated that the use of angiotensin converting enzyme inhibitors (ACEis) and angiotensin-receptor blockers (ARBs) is associated with reduction of emphysema progression in former smokers and that the use of ACEis is associated with reduction of FEV1 progression in current smokers. Research Question Is use of ACEi and ARB associated with less progression of emphysema and FEV1 decline among individuals with COPD or baseline emphysema? Methods Former and current smokers from the Genetic Epidemiology of COPD Study who attended baseline and 5-year follow-up visits, did not change smoking status, and underwent chest CT imaging were included. Adjusted linear mixed models were used to evaluate progression of adjusted lung density (ALD), percent emphysema (%total lung volume Results Over 5 years of follow-up, compared with nonusers, ACEi and ARB users with COPD showed slower ALD progression (adjusted mean difference [aMD], 1.6; 95% CI, 0.34-2.9). Slowed lung function decline was not observed based on phase 1 medication (aMD of FEV1 % predicted, 0.83; 95% CI, –0.62 to 2.3), but was when analysis was limited to consistent ACEi and ARB users (aMD of FEV1 % predicted, 1.9; 95% CI, 0.14-3.6). No effect modification by smoking status was found for radiographic outcomes, and the lung function effect was more pronounced in former smokers. Results were similar among participants with baseline emphysema. Interpretation Among participants with spirometry-confirmed COPD or baseline emphysema, ACEi and ARB use was associated with slower progression of emphysema and lung function decline. Abstract

Rationale: Bronchoscopic lung volume reduction with Zephyr Valves improves lung function, exercise tolerance, and quality of life of patients with hyperinflated emphysema and little to no collateral ventilation. Objectives: Post hoc analysis of patient-reported outcomes (PROs), including multidimensional measures of dyspnea, activity, and quality of life, in the LIBERATE (Lung Function Improvement after Bronchoscopic Lung Volume Reduction with Pulmonx Endobronchial Valves used in Treatment of Emphysema) study are reported. Methods: A total of 190 patients with severe heterogeneous emphysema and little to no collateral ventilation in the target lobe were randomized 2:1 to the Zephyr Valve or standard of care. Changes in PROs at 12 months in the two groups were compared: dyspnea with the Transitional Dyspnea Index (TDI), focal score; the Chronic Obstructive Pulmonary Disease Assessment Test (CAT; breathlessness on hill/stairs); Borg; the EXAcerbations of Chronic pulmonary disease Tool–PRO, dyspnea domain; activity with the TDI, magnitude of task/effort/functional impairment, CAT (limited activities), and the St. George’s Respiratory Questionnaire (SGRQ), activity domain; and psychosocial status with the SGRQ, impacts domain, and CAT (confidence and energy). Abstract

Rationale: This is the first multicenter randomized controlled trial to evaluate the effectiveness and safety of Zephyr Endobronchial Valve (EBV) in patients with little to no collateral ventilation out to 12 months.Objectives: To evaluate the effectiveness and safety of Zephyr EBV in heterogeneous emphysema with little to no collateral ventilation in the treated lobe.Methods: Subjects were enrolled with a 2:1 randomization (EBV/standard of care [SoC]) at 24 sites. Primary outcome at 12 months was the Delta EBV-SoC of subjects with a post-bronchodilator FEV1 improvement from baseline of greater than or equal to 15%. Secondary endpoints included absolute changes in post-bronchodilator FEV1, 6-minute-walk distance, and St. George's Respiratory Questionnaire scores.Measurements and Main Results: A total of 190 subjects (128 EBV and 62 SoC) were randomized. At 12 months, 47.7% EBV and 16.8% SoC subjects had a Delta FEV1 greater than or equal to 15% (P Conclusions: Zephyr EBV provides clinically meaningful benefits in lung function, exercise tolerance, dyspnea, and quality of life out to at least 12 months, with an acceptable safety profile in patients with little or no collateral ventilation in the target lobe.

Abstract

Rationale: The American Thoracic Society (ATS)/European Respiratory Society defines a positive bronchodilator response (BDR) by a composite of BDR in either forced expiratory volume in 1 second (FEV(1)) and/or forced vital capacity (FVC) greater than or equal to 12% and 200 ml (ATS-BDR). We hypothesized that ATS-BDR components would be differentially associated with important chronic obstructive pulmonary disease (COPD) outcomes. Objectives: To examine whether ATS-BDR components are differentially associated with clinical, functional, and radiographic features in COPD. Methods: We included subjects with COPD enrolled in the COPDGene study. In the main analysis, we excluded subjects with self-reported asthma. We categorized BDR into the following: 1) No-BDR, no BDR in either FEV(1) or FVC; 2) FEV(1)-BDR, BDR in FEV(1) but no BDR in FVC; 3) FVC-BDR, BDR in FVC but no BDR in FEV(1); and 4) Combined-BDR, BDR in both FEV(1) and FVC. We constructed multivariable logistic, linear, zero-inflated negative binomial, and Cox hazards models to examine the association of BDR categories with symptoms, computed tomography findings, change in FEV(1) over time, respiratory exacerbations, and mortality. We also created models using the ATS BDR definition (ATS-BDR) as the main independent variable. Results: Of 3,340 COPD subjects included in the analysis, 1,083 (32.43%) had ATS-BDR, 182 (5.45%) had FEV(1)-BDR, 522 (15.63%) had FVC-BDR, and 379 (11.34%) had Combined-BDR. All BDR categories were associated with FEV(1) decline compared with No-BDR. Compared with No-BDR, both ATS-BDR and Combined-BDR were associated with higher functional residual capacity %predicted, greater internal perimeter of 10 mm, and greater 6-minute-walk distance. In contrast to ATS-BDR, Combined-BDR was independently associated with less emphysema (adjusted beta regression coefficient, −1.67; 95% confidence interval [CI], −2.68 to −0.65; P = 0.001), more frequent respiratory exacerbations (incidence rate ratio, 1.25; 95% CI, 1.03–1.50; P = 0.02) and severe exacerbations (incidence rate ratio, 1.34; 95% CI, 1.05–1.71; P = 0.02), and lower mortality (adjusted hazards ratio, 0.76; 95% CI, 0.58–0.99; P = 0.046). Sensitivity analysis that included subjects with self-reported history of asthma showed similar findings. Conclusions: BDR in both FEV(1) and FVC indicates a COPD phenotype with asthma-like characteristics, and provides clinically more meaningful information than current definitions of BDR.

Abstract

Background Chronic bronchitis (CB) increases risk of COPD exacerbations. We have shown that the St. George's Respiratory Questionnaire (SGRQ) CB definition identifies patients with a similar clinical phenotype as classically defined CB. Whether the SGRQ CB definition is a predictor of future COPD exacerbations is unknown. Methods We analyzed 7,557 smokers with normal spirometry and Global Initiative for Chronic Obstructive Lung Disease stage 1-4 COPD in the Genetic Epidemiology of COPD study with longitudinal follow-up data on exacerbations. Subjects were divided into classic CB+ or classic CB–, using the classic definition. In addition, subjects were divided into SGRQ CB+ or SGRQ CB–. Exacerbation frequency and severe exacerbation frequency were determined in each group. Multivariable linear regressions were performed for exacerbation frequency with either classic CB or SGRQ CB and relevant covariates. Results There were 1,434 classic CB+ subjects and 2,290 SGRQ CB+ subjects. The classic CB+ group had a greater exacerbation frequency compared with the classic CB– group (0.69 ± 1.26 vs 0.36 ± 0.90 exacerbations per patient per year; P Conclusions The SGRQ CB definition identified more subjects at risk for future exacerbations than the classic CB definition. SGRQ CB was at least a similar if not better predictor of future exacerbations than classic CB.

Abstract

BACKGROUND:Eosinophilic airway inflammation in patients with chronic obstructive pulmonary disease (COPD) is associated with exacerbations and responsivity to steroids, suggesting potential shared mechanisms with eosinophilic asthma. However, there is no consistent blood eosinophil count that has been used to define the increased exacerbation risk. OBJECTIVE:We sought to investigate blood eosinophil counts associated with exacerbation risk in patients with COPD. METHODS:Blood eosinophil counts and exacerbation risk were analyzed in patients with moderate-to-severe COPD by using 2 independent studies of former and current smokers with longitudinal data. The Genetic Epidemiology of COPD (COPDGene) study was analyzed for discovery (n=1,553), and the Evaluation of COPD Longitudinally to Identify Predictive Surrogate Endpoints (ECLIPSE) study was analyzed for validation (n=1,895). Asubset of the ECLIPSE study subjects were used to assess the stability of blood eosinophil counts over time. RESULTS:COPD exacerbation risk increased with higher eosinophil counts. An eosinophil count threshold of 300cells/μL or greater showed adjusted incidence rate ratios for exacerbations of 1.32 in the COPDGene study (95% CI, 1.10-1.63). The cutoff of 300cells/μL or greater was validated for prospective risk of exacerbation in the ECLIPSE study, with adjusted incidence rate ratios of 1.22 (95% CI, 1.06-1.41) using 3-year follow-up data. Stratified analysis confirmed that the increased exacerbation risk associated with an eosinophil count of 300cells/μL or greater was driven by subjects with a history of frequent exacerbations in both the COPDGene and ECLIPSE studies. CONCLUSIONS:Patients with moderate-to-severe COPD and blood eosinophil counts of 300cells/μL or greater had an increased risk exacerbations in the COPDGene study, which was prospectively validated in the ECLIPSE study.

Abstract

It has been observed that arctic coastal regions which receive river discharge show significant ice regression in advance of other coastal areas without this source of sensible heat. This phenomena has been particularly apparent on satellite imagery. The work reported here attempts (1) to quantify and assess the influence of springtime river discharge on the removal of nearshore ice and (2) to demonstrate the utility of satellite imagery as a source of data to “drive” a model assessing the influence of springtime river discharge on the removal of nearshore ice. Central to this work is a sequence of advanced very high resolution radiometer images of the Mackenzie Delta region, Northwest Territories, Canada. These images constitute a database establishing the temporal sequence of observable breakup events from which a thermodynamic model for the ice decay can be parameterized. The emphases of this model are the influence of the riverine sensible heat on the melting of sea ice and the ability of satellite imagery to monitor this process. For simplicity, climatological values rather than meteorological values for atmospheric and solar influences have been used. The results suggest that the river discharge supplies a significant amount of sensible heat to the base of the nearshore ice, accounting for nearly half of the energy required to melt the ice cover. As a result, the coastal ice is removed 7–14 days in advance of areas without significant river discharge.

Abstract

Geophysical models are usually derived from the idealistic viewpoint that all required external parameters are, in principle, measurable. The models are then driven with the best available data for those parameters. In some cases, there are few measurements available, because of factors such as the location of the phenomena modeled. Satellite imagery provides a synoptic overview of a particular environment, supplying spatial and temporal variability as well as spectral data, making this an ideal source of data for some models. In other cases, although frequent satellite-image observations are available, they are of little use to the modeler, because they do not provide values for the parameters demanded by the model. This paper contains two examples of geophysical models that were derived expressly to utilize measurements and qualitative observations taken from satellite images as the major driving elements of the model. The methodology consists of designing a model such that it can be ‘run’ by numerical data extracted from image data sets, and using the image data for verification of the model or adjustment of parameters. The first example is a thermody namic model of springtime removal of nearshore ice from an Arctic river delta area, using the Mackenzie River as a study site. In this example, a multi-date sequence of AVHRR images is used to provide the spatial and temporal patterns of melt, allowing the required physical observations in the model to be parameterized and tested. The second example is a dynamic model simulating thq evolution of a volcanic ash cloud under the influence of atmospheric winds. In this case, AVHRR images are used to determine the position and size of the ash cloud as a function of time, allowing tuning of parameters and verification of the model.

Abstract

Multi-temporal satellite images, field observations and field measurements were used to investigate the mechanisms by which sea ice melts offshore from the Mackenzie River Delta. Satellite data recorded between April and August 1986 were corrected to a map projection and calibrated such that albedo and temperature values could be compared. Three stages in the melting of sea ice were identified: flooding (overflows), insolation and melting by warm river water. The albedo values of overflows were as much as 1/7 that of ice values while the albedo of ice decreased by 1/3 over the summer. Approximately two weeks after the overflows develop, sea surface temperatures rise as the river-discharge peaks and becomes the dominant source of energy. By this process, ice removal in the delta regime is initiated two months earlier than adjacent coasts with minimal runoff. However, the net result is only a two-week acceleration of ice removal in the delta region.

Two drops were dispensed every four hours four times daily.Results: Regardless of bottle orientation and shaking, all difluprednate emulsion concentrations at each time point analyzed were within 15% of declared concentration. Both branded and generic prednisolone acetate suspension concentrations varied substantially throughout the study. For the bottle stored upright and not shaken, 46% of the branded concentrations were not within 15% of declared concentration; for the bottle stored upright and shaken prior to use, 60% failed to meet this criterion. None of the branded concentrations from the inverted and not shaken bottle was within 15% of declared concentration. Generic prednisolone concentrations demonstrated the poorest dose uniformity, with 96% of the concentrations from the inverted and not shaken bottle not within 15% of declared concentration; 94% of the concentrations from the upright and shaken bottle and 87% from the upright and not shaken bottle similarly failed to meet this criterion.Conclusions: Dose uniformity of Durezol emulsion was predictable in all simulated patient usage conditions, whereas the drop concentrations of Pred Forte and generic prednisolone acetate suspensions were highly variable throughout the study. 27. Observations of double arch formation in the Bering Strait

Abstract

During the time that ice is present in the Bering Strait, it is constantly moving, both northward and southward, subject to wind and current stresses. There were, however, several times in the last decade when the ice was prevented from travelling through the Strait. The blockage of ice was due to the formation of fairly stable double arches that extended across the Strait. Six episodes of double arch formation, resulting in ice blockage in the Bering Strait, were recorded on NOAA satellite imagery spanning the last eleven years; one additional episode was seen on Landsat imagery. All of the incidences of double arching occurred between February and May. In the most extreme case the arches prevented the southward flow of ice through the Strait for at least 27 days during April and May 1980.

Surface atmospheric pressure data from a triangular station network surrounding the Bering Strait are used to calculate hypothetical geostrophic wind velocities. Net daily Strait sea ice movement is derived from visible and infrared NOAA satellite imagery for November through May, 1974 to 1984. These historical ice-motion data and network wind-velocity data are used to develop an empirical 12-h advance forecast (nowcast) sea ice movement model with all-weather capabilities. A necessary outgrowth of this study has been the identification and classification of three modes of ice movement and two modes of ice immobilization according to their major driving forces. The first ice-movement mode is from the Chukchi to the Bering Sea requiring a minimum northeasterly geostrophic wind of 12 m s−1. Abstract

The availability and the choice of appropriate comparison groups are essential for valid occupational epidemiologic studies. Too often, however, adequate comparison groups cannot easily be found within a workplace environment or extracted from the general population. An evaluation of the efficacy of using a pool of comparison subjects from the health and Nutrition Examination Survey (HANES) was performed on data gathered by the National Institute for Occupational Safety and Health in 1979. Comparison groups from the HANES pool were derived for 246 workers at four different commercial/industrial facilities in the Niagara Falls, New York, area and the comparability between the groups was assessed for several demographic, behavioural, and biomedical variables. The HANES groups exhibited a high degree of comparability with regard to most variables, excluding ancestry. The HANES pool may serve as a useful source of subjects to allow for the comparison of disease rates where occupational exposure is the key distinguishing feature between groups.

Occupational and nonoccupational risk factors for bladder cancer were analyzed in a cohort of 1385 workers with known exposure to a potent bladder carcinogen, beta-naphthylamine. Bladder cancer was approximately seven times (95% confidence interval [CI] = 3.9, 12.4) more likely in exposed rather than nonexposed individuals, yet, otherwise, the groups were generally similar in other exogenous or hereditary risk factors. A total of 13 cases of bladder cancer were identified. After the first year of a screening program involving 380 members of the cohort, 9 of the 13 cases of bladder cancer and 36 persons with atypical bladder cytology, histology, or pathology were compared with 335 noncases for distributions of different variables. Occupational variables were significant in a multivariate model that controlled for age, cigarette smoking history, and source of drinking water. The estimated odds ratio for the association for bladder cancer and the duration of employment, when controlling of these other variables, is 4.3 (95% CI = 1.8, 10.3). In addition to the occupational factors, age was significant in the multivariate analysis. Other potential risk factors, such as consumption of coffee or artificial sweeteners, use of phenacetin, or decreased use of vitamin A were not found to be significantly different in cases and noncases.

Abstract

In an evaluation of chronic occupational exposure to arsine (AsH3), an epidemiologic survey was conducted at a lead-acid battery manufacturing plant. Personal (breathing zone) air samples were obtained for the measurement of exposure to arsine and particulate arsenic (As), and area air samples were also collected for the determination of arsenic trioxide (As2O3) vapor concentrations. For the quantification of arsenic absorption, total arsenic content was determined in duplicate 24-h urine samples. Arsine in 177 breathing-zone air samples ranged from nondetectable to 49 micron/m3. The highest levels were found in the battery formation area, where arsine is generated by the reaction of battery acid with lead-arsenic alloy. Exposures to particulate arsenic (maximum 5.1 micron/m3) and to As2O3 (maximum (20.5%) of 39 production workers had urinary arsenic concentrations (corrected to a specific gravity of 1.024) of 50 micron/1 (0.67 mumol/1) or above, indicating increased arsenic absorption. None of eight office staff had elevated urinary arsenic levels. A close correlation was found between urinary arsenic concentration and arsine exposure (N = 47; r = 0.84; p = 0.0001). Arsine levels above 15.6 micron/m3 were associated with urinary arsenic concentrations in excess of 50 micron/1 (0.67 mumol/1). No correlation was found between urinary arsenic content and exposures to particulate arsenic or to As2O3. Consumption of neither seafood, red wine, tobacco, nor contaminated drinking water accounted for urinary arsenic excretion. It was concluded that the current arsine exposure standard, 200 micron/m3, fails to prevent chronic increased absorption of trivalent arsenic from the inhalation of arsine.

Abstract

Geostrophic wind velocities were calculated using atmospheric pressure data from Bering Strait stations at Uelen (Siberia), Bukhta Provideniya (Siberia) and Nome (Alaska). These velocities were matched to Strait sea ice displacements derived from satellite imagery (1974–1984), resulting in an all-weather ice movement and now-casting model. Also, five ice displacement modes were identified. The first mode is Chukchi to Bering Sea movement when northeasterly winds exceed 11.5 m/s. The second and third modes are Bering to Chukchi Sea movement. Mode two is driven by a pre-existing north-flowing ocean current during weak opposing winds. Mode three is due to winds and currents acting in concert. The first immobilization mode (maximum duration one week) is an apparent balance between northerly wind stress, water stress from the south, and internal ice stresses. The second immobilization mode is due to large, double, solid ice arches jamming the Strait up to four weeks.

As early as 1930 the first adverse health effects of vinyl chloride (VC) were reported.' Since then, numerous observations and studies have indicated a wide range of toxicity attributable to the effects of VC on the central nervous system, the liver, the bones of the fingers, and the Lately, the spectrum of known toxic effects of VC broadened to include malignant neoplasms. Viola et al.," in 1971, reported the induction of tumors of the skin, lungs, and bones in rats exposed by inhalation to 30,000 ppm of VC for twelve months. However, widespread acceptance of the carcinogenic activity of VC did not occur until early in 1974 after the announcement of three cases of liver angiosarcoma among workers at a single VC polymerization facility in the U.S.A." About the same time, Maltoni reported the same cancerous hepatic effect in animals following exposure by inhalation to VC.13 Shortly thereafter, Maltoni and Lefemine l 4 and Keplinger et al.15 reported the induction of tumors of the brain, kidney, liver, lung, and lymphatic system in mice, rats, and hamsters exposed to VC by inhalation. Consequently, an epidemiological evaluation was undertaken by the National Institute for Occupational Safety and Health (NIOSH) to assess the magnitude and spectrum of neoplastic effects in workers exposed to VC. This study included industrial hygiene surveys, retrospective cohort analyses of mortality patterns, and histopathologic evaluation of tumors.

Abstract

A marker for biological response to bladder carcinogen exposure was evaluated in a cross-sectional study of 504 workers at high risk due to a range of exposures to various carcinogenic aromatic amines, primarily 2-naphthylamine. A quantitative fluorescence cytology method using the DNA-binding dye, acridine orange, was employed to measure DNA in exfoliated urothelial cells. DNA hyperploidy (greater than 5 C) was observed in 16 (21.6%) of 74 workers who had been exposed compared with 15 (3.5%) of 430 workers who had not (p less than 0.001). The prevalence of DNA hyperploidy increased in a dose-response manner from 3.5% to 60% with increasing duration of exposure. The association between DNA hyperploidy and exposure persisted when adjustment was made for age and cigarette smoking (p = 0.0001). The prevalence of the marker was greatest for exposed workers who smoked (23%), and lowest for those who had no exposure and who had not smoked (2%). This study indicates that DNA hyperploidy can serve as a marker for identifying workers who are at increased risk in occupational groups exposed to bladder carcinogens.

Abstract

Objective: To find out the relationship between the duration and amplitude of oscillatory breathing (OB) and their exercise capacity in patients with chronic heart failure (CHF) we did this study. Methods: Two hundred and thirty-seven CHF patients performed a maximum incremental upright cycle ergometry cardiopulmonary exercise testing (CPET). Respiratory gas exchange was measured on a breath-by-breath basis throughout the test. OB was defined as 3 or more continuous cycle fluctuations of ventilation during CPET, and the amplitude of VE oscillations exceed 25% of concurrent mean value. The CHF patients with OB (OB+) were divided into 3 sub-groups according to their Peak VO 2 . Group1 (mild OB+) Peak VO 2 of ≥16 ml/min/kg, group 2 (moderate OB+) Peak VO 2 is between 12~16 ml/min/kg, group 3 (severe OB+) Peak VO 2 ≤ 12 ml/(min·kg). Results: There were 78(32.6%) patients detected as OB+ in 237 CHF patients. Among OB+ patients, OB duration in s related negatively to Peak VO 2 in mL/min/kg (r=-0.82), Peak VO 2 in %pred (r=-0.65), VO 2 at AT (r=-0.78), and related positively to VE/VCO 2 at AT (r=0.61). Conclusion: OB duration is related negatively to exercise capacity of CHF patients.

37. Popular Ethnic Foods in the United States: A Historical and Safety Perspective.

Abstract

In recent years, a dramatic increase in the demand for ethnic foods in the United States has been observed. Interestingly, with their rise in popularity, more foodborne illness outbreaks associated with ethnic foods have also been reported. Despite a more than 200-y history of ethnic foods in the United States, there is a paucity of information about them. Furthermore, there is also a lack of research on food safety issues involving ethnic foods. Therefore, this paper provides a comprehensive overview of ethnic foods, including the history, types, popularity, characteristics, ingredients, and consumer attitudes toward them. Importantly, this review provides an analysis of the statistics of foodborne illness outbreaks associated with ethnic foods based on data from the Centers for Disease Control and Prevention (CDC). The breakdown of etiology of ethnic foods identified the number of total outbreaks, the causative microorganisms, the food vectors, and the locations where foodborne disease outbreaks have occurred. Also covered is a review of the Hazard Analysis Critical Control Point (HACCP) system implementation, food safety training, and inspection score systems related to ethnic foods and how these can serve as effective tools for the prevention of foodborne illness outbreaks. Abstract

The aim of this work is to investigate the effect of pectin on in vitro digestion of whey protein. Digestion of heated whey protein isolate (WPI) and pectin solutions (WPI-pectin) as influenced by pectin concentration and pH was studied under simulated gastric conditions. Electrophoresis, dynamic light scattering, colorimetric measurements, and gel microstructures were used to study the digestion pattern. At low pectin concentration (0.25% w/w), pectin did not significantly influence the degradation of whey protein. Increasing the pectin concentration to 1% led to extensive intragastric gelation immediately after mixing with simulated gastric fluid. The microstructure of intragastric gel from WPI-pectin at pH 6.0 showed a more interconnected and denser gel network than that at pH 7.0. More protein and pectin were involved in the gelation at pH 6.0 than pH 7.0. The digesta of samples at pH 6.0 was mainly composed of peptides, while that at pH 7.0 mostly consisted of aggregates and crosslinked peptides. This study suggests that WPI-pectin at high biopolymer ratio formed intragastric gel in simulated gastric models, which could delay protein digestion and potentially slow gastric emptying and promote satiety.

Abstract

Expansion of intra-abdominal adipose tissue and the accompanying inflammatory response has been put forward as a unifying link between obesity and the development of chronic diseases. However, an apparent sexual dimorphism exists between obesity and chronic disease risk due to differences in the distribution and abundance of adipose tissue. A range of experimental protocols have been employed to demonstrate the role of estrogen in regulating health benefits; however, most studies are confounded by significant differences in body weight and adiposity. Therefore, the purpose of this study was to compare weight-matched obese male and female mice to determine if the sex-dependent health benefits remain when body weight is similar. The development of obesity in female mice receiving a high-fat diet was delayed; however, subsequent comparisons of weight-matched obese mice revealed greater adiposity in obese female mice. Despite excess adiposity and enlarged adipocyte size, obese females remained more glucose tolerant than weight-matched male mice, and this benefit was associated with increased expression of adiponectin and reductions in immune cell infiltration and oxidative stress in adipose tissue. Therefore, the protective benefits of estrogen persist in the obese state and appear to improve the metabolic phenotype of adipose tissue and the individual.

40. Conjugated linoleic acid content of beef differs by feeding regime and muscle.

Abstract

The project objective was to determine the CLA content of three muscles (Longissimus lumborum, LD; Semimembranosus, SM; Triceps brachii, TB), in both raw and cooked states, in cattle finished on pasture or with grain supplements. Cattle were randomly assigned to one of four finishing regimens; pasture (n=11), pasture with grain supplement (n=11), pasture with grain supplement containing soyoil (n=12), and feedlot (n=12). In the raw state, TB had higher (P<0.05) CLA than LD or SM on a mg/g sample basis. Total CLA was higher (P<0.05) in the soyoil diet when compared to the other three feeding regimes on a mg/g sample basis and when expressed as mg/g fat in both raw and cooked analyses. Pasture inclusion produced higher levels (P<0.05) of total CLA than the feedlot diet on a mg/g fat basis for cooked samples while maintaining acceptable eating quality.

41. Prediction of color and pH measurement throughout boneless center-cut pork loins.

Abstract

Boneless pork loins (N=95) divided into three groupings based on National Pork Producers Council color standards were utilized to characterize changes in color and pH throughout the loin. CIE L(∗), CIE a(∗), CIE b(∗), saturation, and hue angle differed (P<0.05) for color category and chop position. A color category by position interaction (P<0.05) existed only for pH values. These differences in color and pH measures throughout the loin indicate that one measurement may not characterize the entire boneless loin. For all color and pH measures significant orthogonal and polynomial contrasts existed, therefore, separate regression equations were developed for each color category to predict position response. Multiple regression equations were significant for most objective measures of color and pH, but the low R(2) values indicate that these models provide little information as to changes in color and pH throughout the boneless pork loin.

Abstract

Inhibition of Escherichia coli O157:H7, Salmonella Typhimurium, and Listeria monocytogenes by grape seed extract (ActiVin) and pine bark extract (Pycnogenol) and the effect of these natural extracts on the oxidative stability of raw ground beef were studied. In an agar dilution test, the MICs of ActiVin and Pycnogenol were determined to be 4.0 mg/ml for 4.43 log CFU per plate of E. coli O157:H7 and 4.0 mg/ml for 4.38 log CFU per plate of L. monocytogenes. In an inhibition curve test, populations of E. coli O157:H7, Salmonella Typhimurium, and L. monocytogenes fell to below the detection limit (10 CFU/ml) after 16 h of incubation. The numbers of E. coli O157:H7, L. monocytogenes, and Salmonella Typhimurium declined by 1.08, 1.24, and 1.33 log CFU/g, respectively, in raw ground beef treated with 1% Pycnogenol after 9 days of refrigerated storage. ActiVin (1%) and oleoresin rosemary (1%) resulted in an approximately 1-log CFU/g reduction in the populations of all three pathogens after 9 days. The addition of 1% ActiVin and Pycnogenol contributed to the maintenance of an acidic pH of 5.80 and 5.58, respectively, in raw ground beef. Compared to the control, all treatments increased in L* (lightness), with the exception of ActiVin. ActiVin and oleoresin rosemary had the highest a* (redness) and b* (yellowness) values, respectively. ActiVin most effectively retarded lipid oxidation, followed by Pycnogenol. The results suggest that these natural extracts have potential to be used with other preservative methods to reduce pathogenic numbers, lipid oxidation, and color degradation in ground beef.
