Dans cet article, nous racontons une histoire relative au lancement et à l’installation d’une politique belge de santé mentale lancée en 2010. L’histoire s’articule autour de trois réunions partiellement fictives et re- constituées, dans une logique d’idéaltype, à partir de l’observation directe de plus de septante réunions dont elles résument les caractéristiques principales. Ces trois réunions se déroulent dans le monde politique, le monde professionnel et organisationnel, et le monde sensible représen- té par les « lieux du lien ». L’histoire vise à reconnecter ces mondes qui, dans la réalité, sont séparés par des divisions fonctionnelles et culturelles.

The difficulty of implementing mental healthcare reforms owes much to the influence of stakeholders. So far, the endorsement of mental health policy reforms by stakeholder coalitions has received little attention. This study describes stakeholder coalitions formed around common mental health policy goals and highlights their central goals and oppositions. Data were collected on the policy priorities of 469 stakeholders (policymakers, service managers, clinicians, and user representatives)involved in the Belgian mental healthcare reform. Four coalitions of stakeholders endorsing different mental health policy goals were identified using a hierarchical cluster analysis on stakeholders’ policy priorities. A belief network analysis was performed to identify the central and peripheral policy goals within coalitions. Coalitions brought together stakeholders with similar professional functions. Disagreements were observed between service managers and policymakers around policy goals. The two coalitions composed of policymakers supported a comprehensive approach that combines the different goals and also supported the shortening of hospital stays, whereas the two coalitions composed of service managers emphasised the personal recovery of users and continuity of care. Regardless of the coalitions’ differing policy priorities, strengthening community care was a central goal while patient-centred goals were peripheral. The competing policy positions of the coalitions identified may explain the slow and inconsistent pace of the Belgian mental healthcare reform. Strengthening community care may be an essential part of reaching consensus across coalitions. Finally, special care must be taken to ensurethat patient-centred policy goals, such as social integration, are not set aside in favour of other goals

This article examines social inclusion in the context of the deinstitutionalisation of mental health care. It draws on a scientific evaluation of the Belgian reform of mental health care (2010), designed to assess the influence of organisational mechanisms on the social and care trajectories of service users. The findings highlight the ongoing challenge for mental health systems to support the inclusion of service users within the community, and the increasingly difficult access to mental health care for people with complex and chronic mental health problems. Drawing from Systems Theory (Luhmann, 2013) and the analysis of subjective experiences, this article delves into the complex processes of social inclusion using the empirically-grounded concepts of the patient role and the impatient role. By acknowledging the relational dimensions of social inclusion, this article argues that complementarities between two faces of the mental health system are key to achieving inclusion beyond the walls of institutions and within society at large.

This paper examines social inclusion in the context of the deinstitutionalisation of mental health care. It aims to disentangle the concept of social inclusion through considering people with mental health problems’ subjective experiences of inclusion. It draws on two qualitative research evaluations of an ongoing Belgian reform of mental health care that was launched in 2010. The results emphasise that access to contemporary mental health systems has been increasingly difficult for socially vulnerable populations experiencing complex and long-term mental health problems. In reference to Luhmann’s theory of social systems, this paper connects difficult access to mental health care with the functional differentiation of modern mental health systems. It then discusses how mental health and alternative services respectively place themselves as mediators between people with mental health problems and society: while the former are based on specification, which implies endorsing the role of patient in order to eventually achieve social inclusion; the latter are based on de-specification and create heterogeneous communities, where people with mental health problems can explore the boundaries of various functionally differentiated social systems. Finally, this paper suggests that alternative services complement modern mental health systems by offering a processual understanding of social inclusion, which moves beyond the limits of modern mental health systems and contributes to social change., Cet article propose une analyse de l'inclusion sociale dans le contexte de la désinstitutionnalisation des soins de santé mentale. Il vise à approfondir le concept de l'inclusion sociale, souvent réduit à ses aspects économiques ou distributionnels, par la prise en considération des expériences subjectives de personnes vivant avec des problèmes de santé mentale. Pour ce faire, cet article s'appuie sur deux recherches d'évaluation portant sur la réforme 107 lancée en 2010, une réforme des soins de santé mentale pour la population belge adulte. Les résultats de ces recherches mettent en évidence une difficulté d'accès aux soins de santé mentale pour les publics socialement vulnérables, présentant des problèmes de santé mentale chroniques et complexes. Cet article emprunte la théorie des systèmes sociaux de Luhmann pour fournir une analyse articulant d'une part, les difficultés d'accessibilité pour ces publics et d'autre part, la différentiation fonctionnelle du système de la santé mentale de la modernité. Il discute ensuite de la manière dont les services de la santé mentale et des services alternatifs se positionnent respectivement comme médiateurs entre les personnes vivant avec des problèmes de santé mentale et la société. Tandis que les premiers se basent sur la spécification, impliquant la performance du rôle de patient pour espérer atteindre l'objectif d'inclusion sociale; les seconds s'ancrent dans la dé-spécification par la création de communautés hétérogènes au sein desquelles les personnes vivant avec des problèmes de santé mentale peuvent explorer les frontières de différents systèmes sociaux fonctionnellement différenciés. Lancée en 2010 et généralisée en 2016, la " réforme 107 " a pour objectif de réorganiser l'offre de soins de santé mentale autour de l'usager en favorisant sa prise en charge dans son environnement de vie. La présente étude a pour objectif d'évaluer la mise en œuvre de cette réforme en région bruxelloise et de formuler des recommandations en conséquence. Le projet Parcours.Bruxelles, commandité par l'Observatoire de la Santé et du Social, a été mené par l'Institut de Recherche Santé et Société de l'Université catholique de Louvain. Il se distingue par la priorité donnée aux parcours des usagers des soins de santé mentale, afin d'y identifier les bifurcations principales. Les bifurcations identifiées ont ensuite guidé l'analyse de l'organisation, des collaborations et de la coordination du système de la santé mentale, afin de comprendre comment ces dimensions impactent le parcours des usagers. En vue de saisir les liens complexes entre les parcours des usagers et les aspects organisationnels du système, des méthodes qualitatives ont été utilisées, combinant des entretiens semi-directifs et des focus groups avec des professionnels et des usagers inclus et exclus des soins. Les 27 professionnels rencontrés représentent une diversité de secteurs, de services, de disciplines et de fonctions professionnelles, parmi lesquelles la coordination de réseaux, la gestion de services, et le travail de terrain. En vue d'inclure les expériences d'usagers présentant des difficultés d'accès aux soins souvent liées à la complexité et à la chronicité de leur situation, des entretiens ethnographiques ont également été réalisés dans des services à bas seuil d'accès. Au total, 29 rencontres auprès d'usagers issus de profils diversifiés ont été inclues dans cette recherche. Le rapport de recherche comprend deux parties principales. Het project Parcours.Brussel onderzoekt hoe het geestelijke gezondheidssysteem in het Brussels Hoofdstedelijk Gewest is georganiseerd aan de hand van de zorgtrajecten van personen die in contact staan of stonden met diensten voor geestelijke gezondheidszorg uit die regio. Wat het anders maakt dan andere projecten is de bewuste keuze om te vertrekken vanuit de ervaringen van personen die zorg krijgen of van zorg uitgesloten zijn en om bijzondere aandacht te schenken aan de sociale- en verzorgingsdimensie van de zorgtrajecten. De geestelijke gezondheid en de trajecten die erin worden afgelegd slagen of falen altijd binnen welbepaalde sociale omstandigheden die gekarakteriseerd worden door onder andere toegang of geen toegang tot adequate huisvesting, en doorheen interacties, met name op de werkvloer. Het project Parcours.Brussel conceptualiseert daarom de geestelijke gezondheid als een collectieve actie/onderneming die het domein van de zorg overstijgt. Dit vertaalt zich in het gebruik van de term “geestelijke gezondheidssysteem” in plaats van “geestelijke gezondheidszorgsysteem”.

Introduction: During the last decades, reforms in the organisation of mental health care (MHC) have been implemented in many countries. However, reforms are often partially implemented because of the resistance of stakeholders, especially in bottom-up and fragmented societies. So far, coalitions of stakeholders in the organisation of MHC has never been described. Objectives: The objective was to identify coalitions of stakeholders in relation to dimensions of the organisation of MHC. Methods: Using a national online survey in Belgium, data were collected on 469 stakeholders (policymakers, clinicians, managers, professional associations, user representatives). Seven dimensions of the MHC organisation were proposed to stakeholders: target group, geographical organisation of care, type of service provided, type of coordination, formalization of practice, provider payment mechanisms, and resource-pooling level. Classification was performed on stakeholders’ organisational choices with a non-metric cluster analysis. Results: Organisational choices of stakeholders in five clusters accounted for 50% of the variance. Coalitions have emerged around the following dimensions: geographical organisation of care, type of service provided, resource-pooling level and formalization of practices. Clusters composed of clinicians and professional associations favoured the autonomy of individual, specialised services while those composed of policymakers favoured generic services, the formalization of practice and integration at the network level in terms of provision, coordination, and funding of services. Conclusions: Coalitions that are composed of stakeholders with different backgrounds differ on the organisational dimensions that underlie issues of autonomy and identity of services. The definition of MHC policies and interventions should consider the impact of stakeholder coalitions.

Aujourd’hui, les nouvelles formes d’habitat se présentent comme une innovation sociale et offrent une alternative intéressante aux modes d’hébergement traditionnels. L’innovation majeure réside dans l’accent mis sur l’implication des habitants dans la vie sociale et sur la co-construction des modes de fonctionnement de leur habitation. Parmi ces nouvelles formes d’habitat, l’habitat groupé autogéré de type Abbeyfield offre une combinaison entre espace privé et espace partagé, autonomie et vie sociale. En effet, la préservation de l’autonomie et le développement personnel des habitants des maisons sont des éléments-clés du projet Abbeyfield. L’entrée dans une maison Abbeyfield met en tension la préservation du projet personnel et le projet des autres cohabitants, voire le projet de la maison partagée. Cette tension, observée dans la plupart des initiatives comparables au niveau international, est à l’origine du souhait d’identifier des « outils pour mieux vivre ensemble ». Ce souhait a conduit à faire appel à une équipe de recherche pour appuyer et documenter ce processus de réflexion portant sur de tels outils. Ce processus a duré un an, de juin 2018 à juin 2019. L’étude s’est déroulée dans le respect des valeurs-clé du projet Abbeyfield. Elle a pris la forme d’une recherche-action incluant les habitants à toutes les étapes, en ce compris l’élaboration du protocole de recherche ; le recrutement des habitants pour la partie empirique ; la discussion des résultats et la rédaction des recommandations. Le déroulement de l’étude incluait des réunions avec le comité de suivi, des entretiens collectifs auxquels tous les habitants des maisons étaient invités, et un processus itératif permettant la discussion des résultats émanant de l’analyse réalisées par l’équipe de recherche et la rédaction collégiale des résultats. Mental health is a fundamental component of good health. The World Health Organization (WHO) defines mental health as ‘a state of well-being in which every individual realizes his or her own potential, can cope with the normal stresses of life, can work productively and fruitfully, and is able to make a contribution to her or his community’. Consequently, mental health problems range from the worries we all experience as part of everyday life to serious long-term conditions. The WHO pyramid Framework for mental health1 pleads for a comprehensive care offer and continuity of care. From a policy viewpoint every government needs to evaluate whether its mental health care system is effective and efficient and provides easy access for every civilian. Also for Belgium this exercise is important, especially in the light of recent social and political phenomena. In past decades the organisation of mental health care in Belgium underwent several reform waves with the main aim to further orient mental health care towards a reduction of residential hospital care in favour of recovery and reintegration treatment in the community. In order to attaint this objective, policies to promote five core foundations (i.e. deinstitutionalisation, inclusion, decategorisation, intensification, and consolidation)2 were developed. In 2016, the inter-cabinet working group (IKW – GTI) ‘task force on Mental Health care’ asked the KCE to look at the provision of mental health care services and the needs of the population. The current study focused on an in-depth analysis of the organisation of mental health services in Belgium with a need to clearly visualize the present care offer and to evaluate the organisation and continuity of care for the future decade. This contribution offers an empirically grounded argument in which meetings are considered as events linking differences [1] and thus contributing to the ongoing displacement of boundaries between self and system, and between system and environment. It draws on a research that aims to evaluate a mental health reform in the Brussels Region, through professional and organizational practices subsequent to a shift from institutionalized to community-based mental health care. A key objective underlying this reform is the inclusion of service users within wider society. The research methods were designed to gather a diversity of mental health service users and professionals through meeting events [1, 2, 3], which combined the use of focus groups and ethnographic meetings. Circulating through meetings enables participants, from contrasting professional status and with diverse experiences with mental health services, to meet different ways of viewing the world and of understanding mental health. As a result, participant interactions in a given setting create meetings events, or interrelated systems of communication, allowing for opportunities to observe and make sense of individual narratives and the social context in which they unfold in a new light. Processing newly observed differences entails the continuous displacement of boundaries within and surrounding meetings. This contribution discusses how the research process focusing on meetings contributes to displacing boundaries between self, system and environment. Through the incremental changes this entails, the research process offers new perspectives on the very meaning of mental health, on the care system being assessed, and on the political objective of social inclusion. Cette contribution s’ancre dans une conception de l’éducation thérapeutique comme activité sociale mobilisant des chercheurs, des professionnels et des « usagers », c’est-à-dire des personnes ayant ou ayant eu des contacts avec la psychiatrie et les services de santé mentale. Elle examine la contribution d’une recherche ethnographique à l’émergence d’un sens personnel et collectif de la santé mentale et de ses interactions avec la société globale. Elle questionne spécifiquement l’apport de réunions ethnographiques et des groupes de discussion rassemblant chercheurs, professionnels et usagers, à l’évolution de la réflexion des usagers sur leur rôle personnel, social et politique. « La réunion » est ici considérée comme événement social communicatif, et sa contribution à l’évolution des rôles mis en scène par les usagers est analysée à la lumière de la théorie des systèmes sociaux, telle que définie par Niklas Luhmann. Cette contribution est empiriquement ancrée dans une recherche qualitative réalisée entre janvier et septembre 2018 dans la Région de Bruxelles-Capitale. Cette recherche a combiné des entretiens semi-directifs, des groupes de discussion et des réunions ethnographiques qui ont impliqué 27 professionnels et 29 usagers. L’analyse des données d’interaction, à l’aune des théories sociologiques des systèmes et de la communication, permet de formuler trois observations. Premièrement, la réunion constitue une interruption. Il s’agit d’une situation sociale irrégulière, peu ou mal déterminée pour les usagers participants. Comme interruption, la réunion implique de faire sens d’un rôle social, dans et à l’extérieur de cette situation. Cette activité de « sensemaking » s’actualise en communication verbale et non verbale. La communication d’un usager est pour les autres participants l’occasion d’observer une série de différences – différents registres d’interprétation de la place et du rôle de la personne malade dans la sphère personnelle et sociale. This paper draws on a conception of “meetings as brackets in time and space”, that is, distinct social events capable of reflection on their environment (reference to the authors’ work). It provides an empirically grounded argument on a processual collective reflection, unfolding through interrelated meeting events, about the evolving social role of mental health service users (MSU). Specifically, it claims that meetings derive their agency as they connect different types of meetings which differ according to their level of separateness from and connectedness to their environment. Thus, a heterogeneous chain of meetings would guide the collective reflection examined throughout this paper. This paper analyses this chain of meetings by relying on ethnographic and focus group material collected for a research designed to evaluate the organisation of mental health services in the Brussels area from September 2017 to September 2018. This project, called Parcours.Brussels, gave priority to the perspectives of former or current MSUs. These people (n=27) were recruited through “alternative spaces” which are not formerly associated with traditional mental health services. They are inclusive spaces which voluntarily dissolve or at least de-emphasize both social and diagnostic categories in every day interactions. They are mostly centrally situated and accessible to anyone passing by, regardless of their mental health history. The research participants were deliberately encountered in different contexts including (1) planned but informal meetings taking place within alternative spaces (natural context); and (2) planned and formal meetings held for the research purpose in neutral settings (experimental meetings). The research process thus embodied an original history of meetings bringing together, on a regular basis, people from very different worlds in different contexts.
