Abstract: The first search for the lepton number violating decay K+→π0π−μ+e+ and lepton flavour violating decays K+→π0π+μ−e+, K+→π0π+μ+e− has been performed using a dataset collected by the NA62 experiment at CERN in 2016–2018. Upper limits of 2.9×10−10, 3.1×10−10 and 5.0×10−10, respectively, are obtained at 90% CL for the branching ratios of the three decays on the assumption of uniform phase-space distributions.

2. Hydroclimatic processes as the primary drivers of the Early Khvalynian transgression of the Caspian Sea: new developments

Abstract: It has been well established that during the late Quaternary, the Khvalynian transgression of the Caspian Sea occurred, when the sea level rose tens of meters above the present level. Here, we evaluate the physical feasibility of the hypothesis that the maximum phase of this extraordinary event (known as the “Early Khvalynian transgression”) could be initiated and maintained for several thousand years solely by hydroclimatic factors. The hypothesis is based on recent studies dating the highest sea level stage (well above +10 m a.s.l.) to the final period of deglaciation, 17–13 kyr BP, and studies estimating the contribution of the glacial waters in the sea level rise for this period as negligible. To evaluate the hypothesis put forward, we first applied the coupled ocean and sea-ice general circulation model driven by the climate model and estimated the equilibrium water inflow (irrespective of its origin) sufficient to maintain the sea level at the well-dated marks of the Early Khvalynian transgression as 400–470 km3 yr−1. Secondly, we conducted an extensive radiocarbon dating of the large paleochannels (signs of high flow of atmospheric origin) located in the Volga basin and found that the period of their origin (17.5–14 ka BP) is almost identical to the recent dating of the main phase of the Early Khvalynian transgression. Water flow that could form these paleochannels was earlier estimated for the ancient Volga River as 420 km3 yr−1, i.e., close to the equilibrium runoff we determined. Thirdly, we applied a hydrological model forced by paleoclimate data to reveal physically consistent mechanisms of an extraordinarily high water inflow into the Caspian Sea in the absence of a visible glacial meltwater effect. We found that the inflow could be caused by the spread of post-glacial permafrost in the Volga paleocatchment. The numerical experiments demonstrated that the permafrost resulted in a sharp drop in infiltration into the frozen ground and reduced evaporation, which all together generated the Volga runoff during the Oldest Dryas, 17–14.8 kyr BP, up to 360 km3 yr−1 (i.e., the total inflow into the Caspian Sea could reach 450 km3 yr−1). The closeness of the estimates of river inflow into the sea, obtained by three independent methods, in combination with the previously obtained results, gave us reason to conclude that the hypothesis put forward is physically consistent.

Abstract: A sample of 3984 candidates of the K+→π+γγ decay, with an estimated background of 291±14 events, was collected by the NA62 experiment at CERN during 2017–2018. In order to describe the observed di-photon mass spectrum, the next-to-leading order contribution in chiral perturbation theory was found to be necessary. The decay branching ratio in the full kinematic range is measured to be (9.61±0.17)×10−7. The first search for production and prompt decay of an axion-like particle with gluon coupling in the process K+→π+a, a→γγ is also reported.

Abstract: Abstract Measurement of the ultra-rare K + → π + ν ν ¯ $$ {K}^{+}\to {\pi}^{+}\nu \overline{\nu} $$ decay at the NA62 experiment at CERN requires high-performance particle identification to distinguish muons from pions. Calorimetric identification currently in use, based on a boosted decision tree algorithm, achieves a muon misidentification probability of 1.2 × 10 −5 for a pion identification efficiency of 75% in the momentum range of 15–40 GeV/c. In this work, calorimetric identification performance is improved by developing an algorithm based on a convolutional neural network classifier augmented by a filter. Muon misidentification probability is reduced by a factor of six with respect to the current value for a fixed pion-identification efficiency of 75%. Alternatively, pion identification efficiency is improved from 72% to 91% for a fixed muon misidentification probability of 10 −5.

Abstract: Abstract A sample of 1.3 × 105 K + → π 0 e + νγ candidates with less than 1% background was collected by the NA62 experiment at the CERN SPS in 2017–2018. Branching fraction measurements are obtained at percent relative precision in three restricted kinematic regions, improving on existing results by a factor larger than two. An asymmetry, possibly related to T-violation, is investigated with no evidence observed within the achieved precision.

Abstract: Abstract The NA62 experiment at CERN targets the measurement of the ultra-rare K + → π + ν ν ¯ $$ {K}^{+}\to {\pi}^{+}\nu \overline{\nu} $$ decay, and carries out a broad physics programme that includes probes for symmetry violations and searches for exotic particles. Data were collected in 2016–2018 using a multi-level trigger system, which is described highlighting performance studies based on 2018 data.

Abstract: The first search for ultra-rare K+ decays into the π+e+e−e+e− final state is reported, using a dataset collected by the NA62 experiment at CERN in 2017–2018. An upper limit of 1.4×10−8 at 90% CL is obtained for the branching ratio of the K+→π+e+e−e+e− decay, predicted in the Standard Model to be (7.2±0.7)×10−11. Upper limits at 90% CL are obtained at the level of 10−9 for the branching ratios of two prompt decay chains involving pair-production of hidden-sector mediators: K+→π+aa, a→e+e− and K+→π+S, S→A′A′, A′→e+e−.

Abstract: Abstract A sample of 2.8 × 104 K + → π + μ + μ − candidates with negligible background was collected by the NA62 experiment at the CERN SPS in 2017–2018. The model-independent branching fraction is measured to be (9.15 ± 0.08) × 10 −8, a factor three more precise than previous measurements. The decay form factor is presented as a function of the squared dimuon mass. A measurement of the form factor parameters and their uncertainties is performed using a description based on Chiral Perturbation Theory at O $$ \mathcal{O} $$ (p 6).

Abstract: A search for the K+→μ−νe+e+ decay, forbidden within the Standard Model by either lepton number or lepton flavour conservation depending on the flavour of the emitted neutrino, has been performed using the dataset collected by the NA62 experiment at CERN in 2016–2018. An upper limit of 8.1×10−11 is obtained for the decay branching fraction at 90% CL, improving by a factor of 250 over the previous search.

Abstract: Abstract The NA62 experiment reports the branching ratio measurement BR K + → π + ν ν ¯ = 10.6 − 3.4 + 4.0 stat ± 0.9 syst × 10 − 11 $$ \mathrm{BR}\left({K}^{+}\to {\pi}^{+}\nu \overline{\nu}\right)=\left({10.6}_{-3.4}^{+4.0}\left|{}_{\mathrm{stat}}\right.\pm {0.9}_{\mathrm{syst}}\right)\times {10}^{-11} $$ at 68% CL, based on the observation of 20 signal candidates with an expected background of 7.0 events from the total data sample collected at the CERN SPS during 2016–2018. This provides evidence for the very rare K + → π + ν ν ¯ $$ {\pi}^{+}\nu \overline{\nu} $$ decay, observed with a significance of 3.4σ. The experiment achieves a single event sensitivity of (0.839 ± 0.054) × 10 −11, corresponding to 10.0 events assuming the Standard Model branching ratio of (8.4 ± 1.0) × 10 −11. This measurement is also used to set limits on BR(K + → π + X), where X is a scalar or pseudo-scalar particle. Details are given of the analysis of the 2018 data sample, which corresponds to about 80% of the total data sample.

Abstract: Searches for lepton number violating K+→π−e+e+ and K+→π−π0e+e+ decays have been performed using the complete dataset collected by the NA62 experiment at CERN in 2016–2018. Upper limits of 5.3×10−11 and 8.5×10−10 are obtained on the decay branching fractions at 90% confidence level. The former result improves by a factor of four over the previous best limit, while the latter result represents the first limit on the K+→π−π0e+e+ decay rate.

Abstract: Abstract A search for the K + → π + X decay, where X is a long-lived feebly interacting particle, is performed through an interpretation of the K + → π + ν ν ¯ $$ {\pi}^{+}\nu \overline{\nu} $$ analysis of data collected in 2017 by the NA62 experiment at CERN. Two ranges of X masses, 0–110 MeV/c 2 and 154–260 MeV/c 2, and lifetimes above 100 ps are considered. The limits set on the branching ratio, BR(K + → π + X), are competitive with previously reported searches in the first mass range, and improve on current limits in the second mass range by more than an order of magnitude.

Abstract: The 30-year simulations of seasonal snow cover in 22 physically based models driven with bias-corrected meteorological reanalyses are examined at four sites with long records of snow observations. Annual snow cover durations differ widely between models, but interannual variations are strongly correlated because of the common driving data. No significant trends are observed in starting dates for seasonal snow cover, but there are significant trends towards snow cover ending earlier at two of the sites in observations and most of the models. A simplified model with just two parameters controlling solar radiation and sensible heat contributions to snowmelt spans the ranges of snow cover durations and trends. This model predicts that sites where snow persists beyond annual peaks in solar radiation and air temperature will experience rapid decreases in snow cover duration with warming as snow begins to melt earlier and at times of year with more energy available for melting.

Abstract: Abstract The NA62 experiment reports an investigation of the K + → π + ν ν ¯ $$ {K}^{+}\to {\pi}^{+}\nu \overline{\nu} $$ mode from a sample of K + decays collected in 2017 at the CERN SPS. The experiment has achieved a single event sensitivity of (0.389 ± 0.024) × 10 −10, corresponding to 2.2 events assuming the Standard Model branching ratio of (8.4 ± 1.0) × 10 −11. Two signal candidates are observed with an expected background of 1.5 events. Combined with the result of a similar analysis conducted by NA62 on a smaller data set recorded in 2016, the collaboration now reports an upper limit of 1.78 × 10 −10 for the K + → π + ν ν ¯ $$ {K}^{+}\to {\pi}^{+}\nu \overline{\nu} $$ branching ratio at 90% CL. This, together with the corresponding 68% CL measurement of ( 0.48 − 0.48 + 0.72 $$ {0.48}_{-0.48}^{+0.72} $$ ) × 10 −10, are currently the most precise results worldwide, and are able to constrain some New Physics models that predict large enhancements still allowed by previous measurements.

16. Cardiovascular and nephrological risk in patients with chronic kidney disease in ambulatory care.

Abstract: Patients with chronic kidney disease (CKD) have higher than in general population all-cause and cardiovascular mortality. Arterial hypertension (HTN) is a powerful potentially modifiable risk factor that affects the majority of patients with chronic kidney disease and one of the main causes of end stage renal disease worldwide. Existing tools for assessment of risk of CKD progression do not take into account arterial hypertension. The aim – to investigate the association between cardiovascular and nephrological risk factors in patients with CKD in ambulatory practice. The study was carried out in the Center of Nephrology Care in Mechnikov Dnipropetrovsk Regional Hospital, Dnipro, Ukraine. 278 patients (114 males and 164 women, aged 41 [31;61] years) with CKD (stages 1-3) who were followed-up in ambulatory care, but required diagnosis or treatment revision were enrolled to the study. All patients were examined and followed-up according to local and European standards. Females slightly prevailed in our study, gender distribution varied insufficiently in groups by CKD progression risk. Elevation of risk of CKD progression was accompanied by rise of prevalence of diabetes mellitus, left ventricle hypertrophy, proteinuria and HTN. Risk of CKD progression correlated with age, systolic and diastolic blood pressure, erythrocyte sedimentation rate, total cholesterol, glomerular filtration rate, albumin excretion rate, duration of HTN and body mass index. Rise of cardiovascular risk was accompanied by rise of proportion of patients with high risk of CKD progression. Increase in risk of CKD progression is associated with rise of burden of cardiovascular risk factors. HTN and blood pressure values should be accounted for assessment of risk of CKD progression.

Abstract: Abstract The results of a search for π 0 decays to a photon and an invisible massive dark photon at the NA62 experiment at the CERN SPS are reported. From a total of 4.12 × 108 tagged π 0 mesons, no signal is observed. Assuming a kinetic-mixing interaction, limits are set on the dark photon coupling to the ordinary photon as a function of the dark photon mass, improving on previous searches in the mass range 60–110 MeV/c 2. The present results are interpreted in terms of an upper limit of the branching ratio of the electro-weak decay π 0 → γ ν ν ¯ $$ {\pi}^0\to \gamma \nu \overline{\nu} $$ , improving the current limit by more than three orders of magnitude.

Abstract: The NA62 experiment at the CERN SPS reports the first search for K+→π+νν¯ using the decay-in-flight technique, based on a sample of 1.21×1011 K+ decays collected in 2016. The single event sensitivity is 3.15×10−10, corresponding to 0.267 Standard Model events. One signal candidate is observed while the expected background is 0.152 events. This leads to an upper limit of 14×10−10 on the K+→π+νν¯ branching ratio at 95% CL.

Abstract: This paper describes ESM-SnowMIP, an international coordinated modelling effort to evaluate current snow schemes, including snow schemes that are included in Earth system models, in a wide variety of settings against local and global observations. The project aims to identify crucial processes and characteristics that need to be improved in snow models in the context of local- and global-scale modelling. A further objective of ESM-SnowMIP is to better quantify snow-related feedbacks in the Earth system. Although it is not part of the sixth phase of the Coupled Model Intercomparison Project (CMIP6), ESM-SnowMIP is tightly linked to the CMIP6-endorsed Land Surface, Snow and Soil Moisture Model Intercomparison (LS3MIP).

Abstract: The NA62 experiment at CERN reports searches for K+→μ+N and K+→μ+νX decays, where N and X are massive invisible particles, using the 2016–2018 data set. The N particle is assumed to be a heavy neutral lepton, and the results are expressed as upper limits of O(10−8) of the neutrino mixing parameter |Uμ4|2 for N masses in the range 200–384 MeV/c2 and lifetime exceeding 50 ns. The X particle is considered a scalar or vector hidden sector mediator decaying to an invisible final state, and upper limits of the decay branching fraction for X masses in the range 10–370 MeV/c2 are reported for the first time, ranging from O(10−5) to O(10−7). An improved upper limit of 1.0×10−6 is established at 90% CL on the K+→μ+ννν¯ branching fraction.

Abstract: A search for heavy neutral lepton (N) production in K+→e+N decays using the data sample collected by the NA62 experiment at CERN in 2017–2018 is reported. Upper limits of the extended neutrino mixing matrix element |Ue4|2 are established at the level of 10−9 over most of the accessible heavy neutral lepton mass range 144–462 MeV/c2, with the assumption that the lifetime exceeds 50 ns. These limits improve significantly upon those of previous production and decay searches. The |Ue4|2 range favoured by Big Bang Nucleosynthesis is excluded up to a mass of about 340 MeV/c2.

23. The solid solution Sr3Ti2-xFexO7-δ (x ≤ 0.5): characterization of Fe (III) – Fe (IV) mixed valences

Abstract: The results of a magneto chemical and Mössbauer characterization are reported for the solid solution Sr3Ti2-xFexO7-δ (x ≤ 0.5), the intergrowth of a double perovskite block and one rock-salt layer type. The charge compensation mechanism induced by the introduction of iron atoms in the matrix of Sr3Ti2O7 is sensitive to the conditions of synthesis, namely an oxidation process triggers the formation of mixed Fe(III)-Fe(IV) valences. The crystallographic characterization - variation of the cell parameters and structure calculations – brings evidence for the respective occurrence of mixed valences and oxygen vacancies which form in the middle plane of the double perovskite block. Ferromagnetic exchange interactions which are absent in the Fe(III) containing compositions, appear and progressively strengthen depending on the oxidizing treatment. They are ascribed to the presence of an increasing amount of Fe(IV) species. Remarkably, a mixed valence state of iron forms during annealing in air with an increasing contribution of the Fe(IV) species for the larger iron contents, as deduced from Mössbauer data.

24. Inverse reconstruction technique based on time-dependent Petschek-type reconnection model: first application to THEMIS magnetotail observations

Abstract: We apply the inverse reconstruction technique based on the two-dimensional time-dependent Petschek-type reconnection model to a dual bipolar magnetic structure observed by THEMIS B probe in the Earth's magnetotail during a substorm on 22 February 2008 around 04:35 UT. The technique exploits the recorded bipolar magnetic field variation as an input and provides the reconnection electric field and the location of the X-line as an output. As a result of the technique application, we get (1) the electric field, reaching ~1.1 mV/m at the maximum and consisting of two successive pulses with total duration of ~6 min, and (2) the approximate X-line position located in the magnetotail between 18 and 20 RE.

25. Collisionless magnetic reconnection: analytical model and PIC simulation comparison

Abstract: Magnetic reconnection is believed to be responsible for various explosive processes in the space plasma including magnetospheric substorms. The Hall effect is proved to play a key role in the reconnection process. An analytical model of steady-state magnetic reconnection in a collisionless incompressible plasma is developed using the electron Hall MHD approximation. It is shown that the initial complicated system of equations may split into a system of independent equations, and the solution of the problem is based on the Grad-Shafranov equation for the magnetic potential. The results of the analytical study are further compared with a two-dimensional particle-in-cell simulation of reconnection. It is shown that both methods demonstrate a close agreement in the electron current and the magnetic and electric field structures obtained. The spatial scales of the acceleration region in the simulation and the analytical study are of the same order. Such features like particles trajectories and the in-plane electric field structure appear essentially similar in both models.

Abstract: An upgraded version of the OGRAN -- combined optical-acoustic gravitational wave detector -- has been investigated in a long-term operation mode. This installation, located at the Baksan Neutrino Observatory (BNO) INR RAS, is designed to work under the program for detecting collapsing stars in parallel with the neutrino detector: Baksan Underground Scintillation Telescope (BUST). Such joint search corresponds to the modern trend for a development of "multi-messenger astronomy". In this work the effects of thermal relaxation OGRAN are experimentally investigated using passive and active thermal stabilization systems in the underground laboratory BNO PK-14.

Abstract: A high peak brilliance, laser-based Compton-scattering γ-ray source, capable of producing quasimonoenergetic photons with energies ranging from 0.1 to 0.9 MeV has been recently developed and used to perform nuclear resonance fluorescence (NRF) experiments. Techniques for characterization of γ-ray beam parameters are presented. The key source parameters are the size (0.01 mm^{2}), horizontal and vertical divergence (6×10 mrad^{2}), duration (16 ps), and spectrum and intensity (10^{5} photons/shot). These parameters are summarized by the peak brilliance, 1.5×10^{15} photons/mm^{2}/mrad^{2}/s/0.1% bandwidth, measured at 478 keV. Additional measurements of the flux as a function of the timing difference between the drive laser pulse and the relativistic photoelectron bunch, γ-ray beam profile, and background evaluations are presented. These results are systematically compared to theoretical models and computer simulations. NRF measurements performed on ^{7}Li in LiH demonstrate the potential of Compton-scattering photon sources to accurately detect isotopes in situ.

33. Analysis of outcomes of the first line systemic therapy in patients with solitary, single and multiple metastases of renal cell carcinoma

Abstract: BACKGROUND: Currently there are practically no works in the literature to assess the outcomes of systemic therapy in patients with solitary, single and multiple metastases of renal cell carcinoma. AIM: The aim of the study was to analyze the outcomes of systemic drug therapy of the first line in patients with solitary, single and multiple metastases of renal cell carcinoma. MATERIALS AND METHODS: The data of 981 patients with metastatic renal cell carcinoma who underwent systemic therapy of the first line at the City Oncological Hospital No. 62 in Moscow and the City Oncological Dispensary in St. Petersburg from 2006 to 2022 were retrospectively analyzed. All patients underwent clinical, laboratory and pathomorphological examination. 90 (9.2%) patients had solitary metastases, 252 (25.7%) single metastases and 639 (65.1%) multiple metastases. An analysis was made of the outcomes of 1st line therapy, which were conditionally divided into favorable, including all cases of complete response, partial response and stabilization, and unfavorable progression during treatment, death or deregistration. Subsequently patients who had previously received chemotherapy or cytokine treatment were excluded from the analysis. RESULTS: Complete response (3.3%) and deregistration (5.56%) were more often observed in patients with solitary metastases, stabilization more often occurred in patients with single metastases (51.1%), partial response (9.4%) and death (6.2%) in patients with multiple metastases. In patients with multiple metastases treated with immune checkpoint inhibitors a partial response was observed in almost half of the cases. Stabilization and progression were observed in almost the same percentage of cases (about a quarter of cases), and only two patients had a fatal outcome, which is slightly lower than in patients receiving tyrosine kinase inhibitors. Frequent outcomes when using tyrosine kinase inhibitors were stabilization of the process (40.72% of cases) or progression (38.72%), a complete and partial response was rarely recorded. Significant differences in the occurrence of favorable and unfavorable outcomes were revealed in patients with multiple metastases, depending on the number of affected organs and the prescribed drug. When comparing the results of systemic therapy of the first line, a higher efficiency of tyrosine kinase inhibitors was observed in solitary metastases of tumors with a high and moderate degree of differentiation. Systemic therapy of G3 tumors with solitary metastases had low efficacy in the appointment of tyrosine kinase inhibitors in 27.27% of patients. Higher efficiency was noted in single and multiple metastases. The effectiveness of immune checkpoint inhibitors was revealed in 70.6% of patients with single and multiple metastases. CONCLUSIONS: When choosing systemic therapy for metastatic renal cell carcinoma in clinical practice, it is necessary to take into account such prognostic factors as histological variants, the degree of differentiation of the tumor and the number of affected organs.

Abstract: The treatment of localized oncological process requires a reconstructive intervention in the vast majority of cases. Thus, the problem of reconstructive plastic material is acute. There is no standard material for reconstruction, due to the variability of defects in length, composition and localization of the tumor process. Both cover tissues and fragments of the gastrointestinal tract can be used as the autologous graft. The presented clinical case describes the esophageal reconstruction with the radial forearm flap. The radial flap is easy to cut out, survives well, and its use excludes the presence of complications from the donor area, in comparison with the techniques of using fragments of the gastrointestinal tract. The ability to perform simultaneous tumor removal and reconstruction allows for full restoration of vital functions eating, breathing, speech, achievement of good aesthetic and functional results, including long-term ones, and a satisfactory quality of life.

Abstract: Professor Vladimir Ivanovich Burakovsky was born on August 20, 1922 in Tiflis (Tbilisi). In 1940, Vladimir entered the Tbilisi Medical Institute. In 1947, V. I. Burakovsky was enrolled in a clinical residency at the Department of Faculty Surgery № 2 (since 1955 – Department of Surgery for Advanced Doctors № 1) Military Medical Academy named after S. M. Kirov in Leningrad. In the spring of 1960, Vladimir Ivanovich was accepted as a senior researcher at this Institute and appointed the head of the Department of Surgery of the heart and blood vessels in children. Vladimir Ivanovich was the author and co-author of more than 500 scientific papers, including 15 monographs: on the issues of pathophysiology, clinic, diagnosis and surgical treatment of congenital and acquired heart defects and major vessels, heart diseases in young children, complications after open heart surgery, surgery of complex congenital defects and coronary heart disease, heart defects with pulmonary hypertension, the use of hyperbaric oxygenation in cardiac surgery, surgical treatment of coronary disease, heart transplantation in experiment, history and philosophy of cardiac surgery. In 1970, «for the best scientific work on thoracic surgery», Vladimir Ivanovich was awarded the title of laureate of the S. I. Spasokukotsky Prize of the USSR Academy of Medical Sciences (together with B. A. Konstantinov). Academician Vladimir Ivanovich Burakovsky died on September 22, 1994 in Moscow and was buried at the Kuntsevo cemetery. The Institute of cardiac Surgery of the Federal State Budgetary Institution «A.N. Bakulev National Medical Research center of cardiovascular Surgery» (A. N. Bakulev NMRcVS) of the Ministry of Health of the Russian Federation bears his name, in which a bust of his creator is installed.

Abstract: Introduction: One of the key elements of the infectious disease surveillance system is microbiological monitoring of contamination of environmental objects and health care personnel hands. In the context of the COVID-19 pandemic, virology testing of swabs from hospital objects and personal protective equipment of workers of infectious disease hospitals for patients with COVID-19 has acquired special importance. According to the current regulatory documents, however, greater priority in microbiological monitoring is given to determination and identification of bacterial pathogens, thus necessitating the development and implementation of an advanced technique of a simultaneous assessment of viral and bacterial contamination. Objective: To compare different environmental surface sampling techniques used to assess viral and bacterial contamination. Materials and methods: Samples for environmental swab testing were collected in accordance with the “Scheme for sampling environmental swabs for simultaneous assessment of viral and bacterial contamination” patented by the authors (Industrial Design Patent No. 132971 of September 5, 2022). We applied bacteriological, molecular genetic, and statistical methods in the study. Results: Overall, 343 wipe samples were tested, of which 68 were atypical (two 38-swab portions, 11.1 % each, contained SARS-CoV-2 RNA and opportunistic microorganisms). Among the opportunistic microorganisms, 42 bacterial strains were identified, including 16 strains of Enterococcus faecalis (38.1 %), 9 strains of Klebsiella pneumoniae (21.4 %), 7 strains of Escherichia coli (16.7 %), 3 strains of Enterococcus faecium (7.1 %), 3 strains of Staphylococcus aureus (7.1 %), 2 strains of Pseudomonas aeruginosa (4.9 %), and 2 strains of Pantoea agglomerans (4.9 %). Eleven variants of viral and bacterial associations were identified. The comparison of environmental swabbing performed by the technique under study with that performed by the standard method, based on test results, indicated a significant 11.1 and 12.3-fold difference in the proportion of non-standard findings for SARS-CoV-2 and opportunistic microorganisms, respectively. Conclusions: The study results prove that our technique of environmental swabbing meets all modern requirements and facilitates an objective assessment of the level of viral and bacterial contamination of the study objects. This approach can be used for laboratory testing within state and industrial control at institutions of various specialties (healthcare, food industry, childcare facilities, etc.).

42. Separationless method of determination of volume fractions of gas, condensate and water in products extracted from gas condensate wells

Abstract: The problem of the lack of instruments for the operational measurement of the components of the production of gas condensate wells – multiphase flow meters – is considered. Actual problems solved with the use of multiphase flowmeters are described, and the main requirement for these devices is the measurement of the components of a gas condensate flow without separating it into gas and liquid phases using a separator. A method is proposed for determining the volumetric contents of the components of the products of gas condensate and oil and gas condensate wells using a non-separation technique for registering components. The probing of a gas-liquid flow by decimeter radio waves at the moment of flow passing through a microwave resonator is described, and a variant of the design of a non-separation analyzer of volume fractions of gas, condensate and water in the production products of gas condensate wells is proposed. The volume fractions of gas, water, and condensate were determined from the results of measurements of the frequency shift and the quality factor of the resonator. At the same time, an acceptable accuracy (less than 5 %) of determining the amount of condensate is provided only at high condensategas ratios (300–1000 cm3/ m3) and low water-gas ratios (30–100 cm3/m3). With an increase in the proportion of water or a decrease in the proportion of condensate, the error in determining the gas-condensate factor becomes unacceptably high (more than 10 %). It is proposed to reduce this error by introducing a bypass line into the flow meter containing a filter for separating the liquid phase and a reference resonator. The reference resonator registers the frequency shift due to the gas phase. The filter is probed with an eight-millimeter radio beam, which reacts to the content of only the water component. The data obtained from the resonator and filter significantly expand the range of recorded condensate-gas and water-gas factors. Possibilities of using the proposed technique: determination of volume fractions in the production products of gas condensate wells; application in multiphase flow meters for gas condensate fields; quality control of separators that separate liquid before further gas supply (when equipment is installed both before the separator and after it). Large bore resonators could be used to control gas-liquid flow through pipelines of large diameters, such as collector pipelines.

44. Technology of the Reinforced Concrete Structures Post-Tensioning

Abstract: Introduction. A large number of building materials is used for construction of buildings and structures, the reinforced concrete is being one of them. It has the leading position when choosing the material for construction of buildings and structures. However, like any other material, the reinforced concrete has its disadvantages: apart from considerable weight of the reinforced concrete structures, the impossibility to cover with them the large spans can also be referred to disadvantages, as the increase of structures’ spans impairs the economic efficiency of using the reinforced concrete. To handle this, engineers have developed the technology of steel reinforcement prestressing. Thanks to prestressed reinforcement, it has become possible to increase the spans and reduce the cross section of the reinforced concrete structures. This scientific paper investigates the technology of steel reinforcement post-tensioning.Materials and Methods. The materials for the research were obtained from the available scientific editions, articles, journals and Internet blogs. The documents’ content analysis method was used to write the article. In the course of the research, it is planned to find the information about the post-tensioning technology. The object of the study is the steel wire-rope post-tensioning.Results. Thanks to the research, the data on the history of origin of the reinforcement prestressing technology was obtained and the advantages and disadvantages of post-tensioning were analysed.Discussion and Conclusions. After the research, the data on the technology of steel reinforcement post-tensioning was obtained, the application of the technology in Russia in the construction of industrial and civil facilities was considered. This technology allows increasing the spans and reducing the cross section of the reinforced concrete structures.

45. Proton beam therapy in repeat irradiation of recurrent head and neck tumors: analysis of short-term results

Abstract: Introduction. Recurrence of head and neck tumors occurs in 50 % of cases and usually has locoregional character. Due to the characteristics of dose distribution, proton beam therapy is a promising treatment option for patients with recurrences of tumors in this location who previously underwent radiation therapy.Aim. To evaluate the effectiveness and tolerability of repeat irradiation using active scanning proton beam therapy in patients with recurrent head and neck tumors who previously underwent radiation therapy.Materials and methods. Between November of 2015 and December of 2020, 40 patients with locoregional recurrence of head and neck tumors underwent treatment using active scanning proton beam therapy at the A. F . Tsyb Medical Radiological Research Center – branch of the National Medical Research Radiological Center. Median cumulative dose of primary irradiation was 64.5 Gy. Median time between primary and repeat irradiation was 35.7 months, mean irradiated volume of the repeat course was 94.5 cm3. Proton beam therapy was performed using standard mode (2 isoGy) and accelerated hypofractionation (2.4 isoGy / 3 isoGy) with mean equivalent cumulative dose of 56.4 Gy (α / β = 10). Radiation toxicity was evaluated using the Radiation Therapy Oncology Group European (RTOG) / Organization for Research and Treatment of Cancer (EORTC) scale.Results. Treatment response was achieved in 34 (85 %) patients: in 17 (42.5 %) patients, stable disease was observed; in 10 (25 %) patients, partial response was observed; and in 7 (17.5 %) patients, complete response was observed. In 6 (15 %) cases, disease progression was diagnosed at first follow-up examination. One- and two-year locoregional control, progression-free survival and overall survival were 58.4 / 19.8; 44.5 / 19.8 and 82.3 / 38.8 % respectively with median follow-up duration of 14.2 months. Median survival was 19.5 months. Grade III and above early radiation toxicity was observed in 3 (7.5 %) patients. In total, 6 (15 %) cases of grade III complications and 2 (5 %) episodes of carotid artery rupture leading to death were observed. Overall frequency of complications of grade III and higher was 20 %.Conclusion. Repeat irradiation using proton beam therapy can be considered an effective and safe treatment method for patients with recurrent head and neck tumors. Dosimetric and radiobiological benefits of proton beams allow to achieve balance between high doses and radiation exposure in previously irradiated tissues.

Abstract: The objective was to evaluate the effectiveness of the erector spinae plane block (ESP-block) as a component of perioperative analgesia in endovideosurgical hernioplasty of inguinal hernias.Materials and methods. The study included 70 patients who underwent endovideosurgical plastic surgery of inguinal hernias. All patients randomly divided into the main (n=32) and control (n=38) groups, were given general combined anesthesia. In the main group, it was supplemented with the ESP-block, which was performed with ropivacaine at the L1 level. Intraoperatively, fentanyl was used for analgesia. The indication for its introduction was an increase in the Surgical Pleth Index (SPI) of more than 50. In the early postoperative period, ketoprofen and tramadol were used for analgesia, which were administered with the development of moderate and high intensity of the pain. The effectiveness of the ESP-block was evaluated by comparing the total doses of analgesic drugs that were required for analgesia during surgery and in the early postoperative period in patients of both groups.Results. In the main group, the total doses of fentanyl and ketoprofen were statistically significantly lower compared to the control group (pn=15), the pain syndrome developed only 12 hours after the end of the surgery. Their intraoperative need for fentanyl did not differ from other patients of the main group (p>0.05).Conclusion. In endovideosurgical hernioplasty of inguinal hernias, the use of the ESP-block as part of multimodal analgesia contributes to achieving a good level of analgesia both intra- and in the early postoperative period.
