1. Pandemic lockdowns: who feels coerced and why? - a study on perceived coercion, perceived pressures and procedural justice during the UK COVID-19 lockdowns

Abstract: Abstract Background This study examined perceptions of coercion, pressures and procedural injustice and how such perceptions influenced psychological well-being in those who experienced a UK COVID-19 lockdown, with a view to preparing for the possibility of future lockdowns. Methods 40 individuals categorised as perceiving the lockdown(s) as either highly or lowly coercive took part in one of six asynchronous virtual focus groups (AVFGs). Results Using thematic analysis, the following key themes were identified in participants’ discussions: (1) Choice, control and freedom; (2) threats; (3) fairness; (4) circumstantial factors; and (5) psychological factors. Conclusions As the first qualitative study to investigate the psychological construct of perceived coercion in relation to COVID-19 lockdowns, its findings suggest that the extent to which individuals perceived pandemic-related lockdowns as coercive may have been linked to their acceptance of restrictions. Preparing for future pandemics should include consideration of perceptions of coercion and efforts to combat this, particularly in relation to differences in equity, in addition to clarity of public health messaging and public engagement.

Those wishing to set up a multisite study must undergo local Research & Development review, in addition to ethical review by a Research Ethics Committee. However, both the documentation required for multisite review and the length of time it takes to complete such a review can cause significant difficulty to researchers and add to research delays. This short article will examine the process of confirming multisite studies and the implications of current practice.

Abstract: BACKGROUND: In patients receiving venovenous (VV) extracorporeal membrane oxygenation (ECMO) packed red blood cell (PRBC) transfusion thresholds are usually higher than in other patients who are critically ill. Available guidelines suggest a restrictive approach, but do not provide specific recommendations on the topic. The main aim of this study was, in a short timeframe, to describe the actual values of haemoglobin and the rate and the thresholds for transfusion of PRBC during VV ECMO.METHODS: PROTECMO was a multicentre, prospective, cohort study done in 41 ECMO centres in Europe, North America, Asia, and Australia. Consecutive adult patients with acute respiratory distress syndrome (ARDS) who were receiving VV ECMO were eligible for inclusion. Patients younger than 18 years, those who were not able to provide informed consent when required, and patients with an ECMO stay of less than 24 h were excluded. Our main aim was to monitor the daily haemoglobin concentration and the value at the point of PRBC transfusion, as well as the rate of transfusions. The practice in different centres was stratified by continent location and case volume per year. Adjusted estimates were calculated using marginal structural models with inverse probability weighting, accounting for baseline and time varying confounding.FINDINGS: Between Dec 1, 2018, and Feb 22, 2021, 604 patients were enrolled (431 [71%] men, 173 [29%] women; mean age 50 years [SD 13·6]; and mean haemoglobin concentration at cannulation 10·9 g/dL [2·4]). Over 7944 ECMO days, mean haemoglobin concentration was 9·1 g/dL (1·2), with lower concentrations in North America and high-volume centres. PRBC were transfused on 2432 (31%) of days on ECMO, and 504 (83%) patients received at least one PRBC unit. Overall, mean pretransfusion haemoglobin concentration was 8·1 g/dL (1·1), but varied according to the clinical rationale for transfusion. In a time-dependent Cox model, haemoglobin concentration of less than 7 g/dL was consistently associated with higher risk of death in the intensive care unit compared with other higher haemoglobin concentrations (hazard ratio [HR] 2·99 [95% CI 1·95-4·60]); PRBC transfusion was associated with lower risk of death only when transfused when haemoglobin concentration was less than 7 g/dL (HR 0·15 [0·03-0·74]), although no significant effect in reducing mortality was reported for transfusions for other haemoglobin classes (7·0-7·9 g/dL, 8·0-9·9 g/dL, or higher than 10 g/dL).INTERPRETATION: During VV ECMO, there was no universally accepted threshold for transfusion, but PRBC transfusion was invariably associated with lower mortality only when done with haemoglobin concentration of less than 7 g/dL.FUNDING: Extracorporeal Life Support Organization.

Abstract: Catheter dislocation and fracture with migration of central venous lines have been reported in the International literature. Catheter fracture with consequent migration has been observed in 0.5-3.0% and may either be consequent to catheter removal or it can occur spontane-ously. Our case report concerns the migration of a Hickman catheter connected to a venous port to the right atrium in a 61-year old patient. A literature up-to-date has been performed to assess the risk of port-a-cath positioning. The position of catheter tip is considered critical for the risk of migration, that is greater as higher the tip localization respect to the carina. The aim of our study is to underline the critical role of X-ray to visualize the exact location of the catheter tip, regard-less of the approach used for catheter positioning.

Abstract: International audience; Single crystals and polycrystalline samples of barbosalite Fe3(PO4)2(OH)2 iron phosphate were successfully synthesized by hydrothermal method at low pressure and temperature. Careful investigations of the crystal structure by high resolution X-ray diffraction and transmission electron microscopy evidence a complex barbosalite framework where trimers of face sharing FeO6 octahedra along the direction are connected by an additional Fe site, partly occupied. Despite this structural disorder, the spins order in a rather simple magnetic structure below 160 K. The corresponding magnetic point group of P21 makes the barbosalite a promising candidate for the design of new multiferroics.

Abstract: Water molecules play a fundamental role in the physics of slow crack propagation in glasses. It is commonly understood that, during stress-corrosion, water molecules that move in the crack cavity effectively reduce the bond strength at the strained crack tip and, thus, support crack propagation. Yet the details of the environmental condition at the crack tip in moist air are not well determined. In a previous work, we reported direct evidence of the presence of a 100 nm long liquid condensate at the crack tip in fused silica glass during very slow crack propagation (10^-9 to 10^-10 m/s). These observations are based on in-situ AFM phase imaging techniques applied on DCDC glass specimens in controlled atmosphere. Here, we discuss the physical origin of the AFM phase contrast between the liquid condensate and the glass surface in relation to tip-sample adhesion induced by capillary bridges. We then report new experimental data on the water condensation length increase with relative humidity in the atmosphere. Abstract: Chemical physical analysis, photo-oxidative stability and lipid oxidation of innovative polymeric films based on blends of nylon 6 and ethylene-co-vinyl alcohol for use in food packaging have been investigated. Thermal mechanical analysis showed that the presence of an interfacial agent in the blend stabilized the films towards the action of permeants. Synergistic effects of the interfacial agent are reported with respect to UV photostability. Peroxide value (PV) was used to follow the oxidation of the olive oil, and for this parameter also the influence of the interfacial agent was clearly detected. Abstract: The dissolution of α-FePO(4) and the α-Ga(0.75)Fe(0.25)PO(4) solid solution with α-quartz-type structures under hydrothermal conditions in 1 M HNO(3) aqueous solution was investigated by in situ X-ray absorption spectroscopy (XAS) at the Fe K-edge. The solubility of α-FePO(4) increases with temperature and is higher at 25 MPa than at 50 MPa. The Fe(3+) cation in solution is 6-fold coordinated with an average Fe-O distance close to 2.0 Å. A similar experiment was performed with a solid solution of α-quartz-type Ga(0.75)Fe(0.25)PO(4) as the starting phase under a pressure of 25 MPa. By varying the temperature from 303 K up to 573 K a single crystal was grown with 23% Fe(3+) with the α-quartz-type structure. These results show that the crystallization of pure α-quartz-type FePO(4) by the hydrothermal method is not possible due to the formation of very stable Fe(3+) hexa-aquo complexes [Fe(H(2)O)(6)](3+) and to the absence of FeO(4) tetrahedra in solution. Ga(3+) cations in solution induce the formation of gallophosphate complexes at the solid-liquid interface, which are at the origin of the nuclei for crystallization. We propose a crystallization mechanism in which the Fe(3+) substitutes Ga(3+) with a 4-fold coordination in mixed (iron/gallo)-phosphate complexes that leads to the growth of an α-quartz-type Ga(0.77)Fe(0.23)PO(4) single crystal.

Abstract: International audience; A Li-rich layered oxide with the formula Li[Li0.2Mn0.61Ni0.18Mg0.01]O2 was successfully synthesised and characterised using several in situ characterisation techniques. The electronic state and structural evolution of the material upon cycling were investigated using in situ XRD, EXAFS and XANES measurements. XANES and SQUID magnetic measurements showed that the initial material contains a certain amount of Mn3+ in a low spin configuration (average Mn oxidation state: +3.75). In situ measurements showed that the first part of the charge (up to 4.4 V vs. Li+/Li) corresponds to oxidation of the Mn3+ fraction, and that the oxidation of nickel occurs only later, on the main charge plateau at 4.5 V. Electrochemical and structural results tend to show that the main first-charge plateau is a two- phase process where a new phase is created. This new phase is structurally very close to the starting one, and could be an oxygen-deficient spinel with a 1⁄4 8.25 A#. This process is non-reversible, and further cycling occurs in the new phase formed in situ.

Abstract: The dissolution of Si(1-x)Ge(x)O(2) solid solutions under hydrothermal conditions was studied by in situ X-ray absorption spectroscopy. Experiments were performed at the Ge K-edge using a high-pressure cell mounted on the FAME beamline of the European Synchrotron Radiation Facility. Spectra in both transmission and fluorescence mode were collected in isobaric conditions (100 and 150 MPa) up to 475 °C. The local atomic structure around the Ge atom was investigated as a function of the temperature and in pure water and sodium hydroxide solutions. In pure water, the solubility of the cristobalite-type Si(0.8)Ge(0.2)O(2) increases with the temperature and the Ge atom is in 4-fold coordination. In a sodium hydroxide aqueous solution, a complex between Ge and Na atoms forms and gives rise to precipitation of sodium germanates. Under these conditions, the Ge content in the solution decreases with increasing temperature. These results show that a sodium hydroxide aqueous solution, usually used for quartz crystal growth, is not suitable for Ge-containing crystals. The dissolution kinetics and phase transformation of the solid solution were studied as a function of the atomic fraction of Ge. Ge-rich solid solutions dissolve and transform to stable phases faster than Ge-poorer composition, giving rise to important variations of the Ge content in solution.

22. Can baseball improve balance in blind subjects?

Abstract: Baseball, one of the most popular sports in the world, is a fast-moving sport that requires various motor abilities. Baseball is played also by blind subjects that participate in many other sports. In this study, we evaluated the role of the Italian modified version of baseball for blind subjects on balance.This modified version of baseball maintains the fast-moving characteristic ensuring the athlete safety. Forty total blind subjects were enrolled: 20 baseball athletes and 20 sedentary participants, as control. The balance was evaluated using the Fukuda Test and Tinetti Test, both in silence and in noise.This baseball game may help to improve the balance ability in blind subjects. The balance was significantly improved in blind athletes as compared with blind sedentary subjects.Given the peculiar characteristics of play, this modified version of baseball seems effective in improving various motor skills that, once transferred into daily activities, may significantly ameliorate the quality of life of blind subjects.

Abstract: International audience; The substitution of germanium in the α-quartz structure is a method investigated to improve the piezoelectric properties and the thermal stability of α-quartz. Growth of α-quartz type Si(1-x)Ge(x)O(2) single crystals was performed using a temperature gradient hydrothermal method under different experimental conditions (pressure, temperature, nature of the solvent, and the nutrient). To avoid the difference of dissolution kinetics between pure SiO(2) and pure GeO(2), single phases Si(1-x)Ge(x)O(2) solid solutions were prepared and used as nutrients. The influence of the nature (cristobalite-type, glass) and the composition of this nutrient were also studied. Single crystals were grown in aqueous NaOH (0.2-1 M) solutions and in pure water. A wide range of pressures (95-280 MPa) and temperatures (315-505 °C) was investigated. Structures of single crystals with x = 0.07, 0.1, and 0.13 were refined, and it was shown that the structural distortion (i.e., θ and δ) increases with the atomic fraction of Ge in an almost linear way. Thus, the piezoelectric properties of Si(1-x)Ge(x)O(2) solid solution should increase with x, and this material could be a good candidate for technological applications requiring a high piezoelectric coupling factor or high thermal stability.

Abstract: International audience; The structure of glasses in the binary system SiO2-GeO2 has been studied by Raman spectroscopy. Our results are consistent with mixing of SiO2 and GeO2 tetrahedra. The changes induced by temperature and by pressure on the structure are monitored by in situ measurements on the same mixed glasses. Anomalous temperature dependences are observed not only for SiO2 glass and GeO2 glass but also for mixed glasses. Particular attention is focused on the pressure densification mechanism in mixed glasses. Via the pressure dependence of the width of the main Raman band, we show that the compression mechanism in mixed glasses is intermediate between that of the end members.

Abstract: alpha-quartz-type Si1−xGexO2 solid solutions (x=0.06, 0.11, and 0.24) and pure SiO2 were investigated by Raman spectroscopy. Coupled and decoupled vibrational modes were identified at room temperature as a function of composition. Tetrahedral tilting librational modes involved in the displacive alpha beta-quartz phase transition are decoupled. The wave number of the coupled A1 mode located at 464 cm−1 for pure alpha-quartz was found to vary quite linearly with germanium content. Raman spectra were recorded up to 1473 K. Substitution of germanium in the quartz lattice clearly improves the thermal stability of the alpha phase. The alpha beta-quartz phase transition temperature increases from 846±1 K for x=0 to about 1300±50 K for x=0.24. At the same time, the dynamic disorder observed in quartz well below the transition is reduced in the solid solutions.

27. Is there an association between antiphospholipid antibodies and psoriasis?

Abstract: Ever increasing evidence supports the association between psoriasis and cardiovascular risk. Antiphospholipid antibodies (APAs), which can occur in many autoimmune diseases, are considered prothrombotic and have been associated with atherosclerosis. The aim of this study is to evaluate the prevalence and levels of APAs in psoriasis patients. Fifty patients with moderate to severe plaque psoriasis and 48 healthy subjects were investigated for lupus anticoagulant (LAC) by screening and confirmatory coagulation tests, as well as for antibodies against cardiolipin or beta2-glycoprotein I. Levels of APAs and LAC-related parameters were similar for patients with psoriasis and normal controls (p0.05). APAs were found in only one psoriatic patient (2%) and in none of the controls. LAC was detected in 2 patients (4%) and in one subject of the control group (2.1%). These results suggest that the prevalence of APAs is not increased in plaque psoriasis as compared to the control group. The increased cardiovascular risk observed in psoriatic patients is therefore likely to be correlated to factors different from APAs.

Abstract: In Optometry and in Audiology, the routine tests to prescribe correction lenses and headsets are respectively the visual acuity test (the first chart with letters was developed by Snellen in 1862) and conventional pure tone audiometry (the first audiometer with electrical current was devised by Hartmann in 1878). At present there are psychophysical non invasive tests that, besides evaluating visual and auditory performance globally and even in cases catalogued as normal according to routine tests, supply early information regarding diseases such as diabetes, hypertension, renal failure, cardiovascular problems, etc. Concerning Optometry, one of these tests is the achromatic luminance contrast sensitivity test (introduced by Schade in 1956). Concerning Audiology, one of these tests is high frequency pure tone audiometry (introduced a few decades ago) which yields information relative to pathologies affecting the basal cochlea and complements data resulting from conventional audiometry. These utilities of the contrast sensitivity test and of pure tone audiometry derive from the facts that Fourier components constitute the basis to synthesize stimuli present at the entrance of the visual and auditory systems; that these systems responses depend on frequencies and that the patient's psychophysical state affects frequency processing. The frequency of interest in the former test is the effective spatial frequency (inverse of the angle subtended at the eye by a cycle of a sinusoidal grating and measured in cycles/degree) and, in the latter, the temporal frequency (measured in cycles/sec). Both tests have similar duration and consist in determining the patient's threshold (corresponding to the inverse multiplicative of the contrast or to the inverse additive of the sound intensity level) for each harmonic stimulus present at the system entrance (sinusoidal grating or pure tone sound). In this article the frequencies, standard normality curves and abnormal threshold shifts inherent to the contrast sensitivity test (which for simplicity could be termed “visionmetry”) and to pure tone audiometry (also termed auditory sensitivity test) are analyzed with the purpose of contributing to divulge their ability to supply early information associated to pathologies not solely related to the visual and auditory systems respectively.

Abstract: CMS is a general purpose experiment, designed to study the physics of pp collisions at 14 TeV at the Large Hadron Collider (LHC). It currently involves more than 2000 physicists from more than 150 institutes and 37 countries. The LHC will provide extraordinary opportunities for particle physics based on its unprecedented collision energy and luminosity when it begins operation in 2007. The principal aim of this report is to present the strategy of CMS to explore the rich physics programme offered by the LHC. This volume demonstrates the physics capability of the CMS experiment. The prime goals of CMS are to explore physics at the TeV scale and to study the mechanism of electroweak symmetry breaking - through the discovery of the Higgs particle or otherwise. To carry out this task, CMS must be prepared to search for new particles, such as the Higgs boson or supersymmetric partners of the Standard Model particles, from the start-up of the LHC since new physics at the TeV scale may manifest itself with modest data samples of the order of a few fb-1 or less. The analysis tools that have been developed are applied to study in great detail and with all the methodology of performing an analysis on CMS data specific benchmark processes upon which to gauge the performance of CMS. These processes cover several Higgs boson decay channels, the production and decay of new particles such as Z′ and supersymmetric particles, Bs production and processes in heavy ion collisions. The simulation of these benchmark processes includes subtle effects such as possible detector miscalibration and misalignment. Besides these benchmark processes, the physics reach of CMS is studied for a large number of signatures arising in the Standard Model and also in theories beyond the Standard Model for integrated luminosities ranging from 1 fb-1 to 30 fb-1. The Standard Model processes include QCD, B-physics, diffraction, detailed studies of the top quark properties, and electroweak physics topics such as the W and Z0 boson properties. The production and decay of the Higgs particle is studied for many observable decays, and the precision with which the Higgs boson properties can be derived is determined. About ten different supersymmetry benchmark points are analysed using full simulation. The CMS discovery reach is evaluated in the SUSY parameter space covering a large variety of decay signatures. Furthermore, the discovery reach for a plethora of alternative models for new physics is explored, notably extra dimensions, new vector boson high mass states, little Higgs models, technicolour and others. Methods to discriminate between models have been investigated. This report is organized as follows. Chapter 1, the Introduction, describes the context of this document. Chapters 2-6 describe examples of full analyses, with photons, electrons, muons, jets, missing E T, B-mesons and τ's, and for quarkonia in heavy ion collisions. Chapters 7-15 describe the physics reach for Standard Model processes, Higgs discovery and searches for new physics beyond the Standard Model. © 2007 IOP Publishing Ltd.

Abstract: Planar waveguides were obtained by deposition of Te 2 As 3 Se 5 films on polished As 2 S 3 glass substrates by RF-sputtering or thermal evaporation. The RF-sputtered films were porous and had a column-like structure. By contrast, the thermal evaporated films were homogeneous and dense. They were adhesive, transparent up to 18 μm and characterized by a refractive index of 2.821 ± 0.005 at 10.6 μm. M-lines measurements highlighted the existence of several guided mode lines, proving that the manufactured structures behaved as waveguides. The geometry of the film was modified by physical or reactive ion etching. Some quasi vertical ribs up to 2.11 μm in depth were etched by reactive ion etching under a CF 4 /O 2 atmosphere.

31. Follow-up short and long-term mortalities of tracheostomized critically ill patients in an Italian multi-center observational study

Abstract: Abstract The effects of tracheostomy on outcome as well as on intra or post-operative complications is yet to be defined. Admission of patients with tracheostomy to rehabilitation facility is at higher risk of suboptimal care and increased mortality. The aim of the study was to investigate ICU mortality, clinical outcome and quality of life up to 12 months after ICU discharge in tracheostomized critically ill patients. This is a prospective, multi-center, cohort study endorsed by Italian Society of Anesthesia, Analgesia, Reanimation, and Intensive Care (SIAARTI Prot. n° 643/13) registered in Clinicaltrial.gov (NCT01899352). Patients admitted to intensive care unit (ICU) and requiring elective tracheostomy according to physician in charge decision were included in the study. The primary outcome was ICU mortality. Secondary outcomes included risk factors for ICU mortality, prevalence of mortality at follow-up, rate of discharge from the hospital and rehabilitation, quality of life, performance status, and management of tracheostomy cannula at 3-, 6, 12-months from the day of tracheostomy. 694 critically ill patients who were tracheostomized in the ICU were included. ICU mortality was 15.8%. Age, SOFA score at the day of the tracheostomy, and days of endotracheal intubation before tracheostomy were risk factors for ICU mortality. The regression tree analysis showed that SOFA score at the day of tracheostomy and age had a preeminent role for the choice to perform the tracheostomy. Of the 694 ICU patients with tracheostomy, 469 completed the 12-months follow-up. Mortality was 33.51% at 3-months, 45.30% at 6-months, and 55.86% at 12-months. Patients with tracheostomy were less likely discharged at home but at hospital facilities or rehabilitative structures; and quality of life of patients with tracheostomy was severely compromised at 3–6 and 12 months when compared with patients without tracheostomy. In patients admitted to ICU, tracheostomy is associated with high mortality, difficult rehabilitation, and decreased quality of life. The choice to perform a tracheostomy should be carefully weighed on family burden and health-related quality of life. Clinical trial registration: Clinicaltrial.gov (NCT01899352).

Abstract: Crack propagation in oxide glasses at low crack velocities is controlled by stress corrosion. Proper knowledge of the crack tip chemical environment is thus crucial to understand the slow fracture process of these materials. The formation of a liquid condensate in the confined area of the crack tip is theoretically expected and is classically mentioned to explain specific behavior during the crack propagation. Since this condensate is of the nanometer scale and cannot be traced post mortem, it is extremely difficult to observe. This paper reports the experimental evidences of the presence of a liquid condensate at the tip of a crack propagating by stress corrosion in silica glass. The observation has been performed in situ under carefully controlled atmosphere through phase imaging by atomic force microscopy (AFM). The evolution of the condensate has been followed as a function of the relative humidity. The impact of those evidences of the liquid condensate will be discussed and information on its size or evolution kinetic will be reported.

Abstract: The Confidentiality Advisory Group (CAG) is a specialised body that advises the Health Research Authority (HRA) and the Secretary of State for Health on requests for access to confidential information, in the absence of informed consent from its owners. Its primary role is to oversee the safe use of such information and to counsel the governing bodies mentioned above as to whether such use is appropriate or inappropriate. Researchers who seek access to England or Wales-based confidential data, for medical purposes that are in the interest of the public, are typically required to submit an application to this body. However, it is not always clear to researchers whether requests for access to patient data fit within the remit of the CAG or a Trust’s local information governance team. This commentary will, therefore, explore the role of the CAG and reflect on how best to support researchers with this question.

Abstract: Una delle tematiche legate alla tutela dell'ambiente che oggi richiede maggiore attenzione è quella dei terreni contaminati per la quale è sentita la necessità di un aggiornamento del quadro normativo. Il Tavolo di Roma, un think tank costituitosi alcuni anni fa per una gestione sostenibile dei rifiuti e delle risorse, composto da figure professionali non portatrici di interessi, in rappresentanza di diversi ambiti disciplinari e funzionali, si è fatto interprete di questa esigenza, promuovendo la costituzione del Comitato Tecnico Terreni Contaminati (CTTC). L'apporto di diversi ambiti disciplinari ha determinato, grazie anche a un intenso e franco confronto sempre condotto con logiche tecnico-scientifiche, un fermento di idee e una varietà di osservazioni che costituiranno il migliore viatico per una revisione della normativa fondata saldamente su un elevato grado di accettazione e di condivisione da parte della comunità coinvolta nella sua applicazione.

Abstract: BACKGROUND: In the acute respiratory distress syndrome (ARDS), positive end-expiratory pressure (PEEP) may decrease ventilator-induced lung injury by keeping lung regions open that otherwise would be collapsed. Since the effects of PEEP probably depend on the recruitability of lung tissue, we conducted a study to examine the relationship between the percentage of potentially recruitable lung, as indicated by computed tomography (CT), and the clinical and physiological effects of PEEP. METHODS: Sixty-eight patients with acute lung injury or ARDS underwent whole-lung CT during breath-holding sessions at airway pressures of 5, 15, and 45 cm of water. The percentage of potentially recruitable lung was defined as the proportion of lung tissue in which aeration was restored at airway pressures between 5 and 45 cm of water. Abstract: Purpose of review Acute lung injury/acute respiratory distress syndrome is a common, serious condition affecting a heterogeneous population of critically ill patients. Other than low tidal volume ventilation, no specific therapy has improved survival. Understanding the epidemiology, pathogenesis, and lessons to be learned from previous clinical trials is necessary for the development of new therapies and the rational design of studies assessing their efficacy. Recent findings Acute lung injury/acute respiratory distress syndrome occurs in 6-8% of the general intensive care unit population, with a mortality of 32-45%. A recent epidemiologic study found that multi-organ dysfunction, use of tidal volumes higher than 6 ml/kg, and high mean fluid balance were independent risks for mortality. Although high levels of inflammatory mediators are also markers for acute respiratory distress syndrome development and death, short courses of high-dose steroids are not effective in acute cases. The latest theory of biotrauma proposes cellular mechanisms by which mechanical ventilation incites a local and systemic inflammatory response; protective lung ventilation with low tidal volumes can attenuate this inflammation and injury to distal organs. Endogenous surfactant function is clearly impaired, but no commercially available surfactant preparation has been shown to reduce mortality. Results of trials to determine efficacy of steroids in late cases and optimal fluid management are pending. Summary The results of recent clinical trials have raised more questions. Further study of the inflammatory response, surfactant regulation, and the cellular impact of mechanical ventilation should help to develop new therapies, target patients most likely to benefit, and identify appropriate timing of intervention.

Abstract: Background: Covid 19 was declared as a public health emergency by the World Health Organisation (WHO) due to its rapid spread and catastrophic effects on health. It affected around 119 M people with mortality rate of 0.27% worldwide, including South-Asians. This review aims to understand the risk perceptions, cultural religious beliefs and the coping mechanisms of South Asians during the Covid 19 pandemic., Methods: We conducted a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The following search engines were used: Medline, Cochrane Library, PsycINFO, CINAHL, and Web of Science. Included studies investigated perceptions and opinions of individuals on knowledge, risk and protective factors, native faith based practices, and attitudes towards the COVID-19 pandemic., Results: The database search produced 282 articles to screen. The final narrative synthesis included five studies comprising of 13,476 participants from Pakistan, India, Nepal, and Bangladesh. Ten studies, comprising 7,893 participants, were eligible and included for meta-analysis. The overall pooled prevalence with maximum heterogeneity for correct knowledge of symptoms, hand washing or use of sanitizers, face masking use of herbal or traditional remedies and physical distancing or avoidance of contact was reported through meta-analysis., Conclusion: The review brings forth a useful comparison of individual and cultural differences in KAP, risk perceptions and coping strategies. This review highlights the need for and importance of tailored information dissemination, culturally sensitive risk communication, targeted educational interventions, community engagement and empowerment, policy, and infrastructure improvements, as well as continued research and data collection. 48. Identifying the facilitators and barriers to implementation of renal tumour biopsy in the diagnostic pathway for small renal masses.

Abstract: Objectives: To understand the facilitators and barriers to the implementation of renal tumour biopsy (RTB) in the diagnostic pathway for renal tumours in England., Patients and Methods: Participants consisted of patients who had a renal tumour diagnosed and/or treated at one of five tertiary centres in England, healthcare professionals involved in the direct care of patients diagnosed with renal tumours, and clinical service managers and commissioners. The study employed a mixed-methods research methodology consisting of individual interviews and an on-line survey that explored the types of facilitators and barriers individuals perceived and experienced and the frequency in which these were reported. A public dissemination event took place following the completion of data collection; to facilitate discussion of potential solutions to implementing RTB., Results: There were 50 participant interviews (23 patients, 22 clinicians, and five health service commissioners/operations managers). The patient on-line survey received 52 responses, and the clinician survey received 22 responses. Patients most frequently reported influences in choosing whether to undergo RTB pertained to wanting to know the diagnosis of their kidney mass (40%), the advice or information provided by healthcare professionals (40%), and not wishing to delay treatment (23%). Clinicians most frequently reported barriers to recommending RTB related to their uncertainty of diagnostic accuracy (56%), availability of appointments or hospital beds (52%), concerns of risk of bleeding (44%), risk of seeding (41%), and delays in meeting national cancer pathway targets (41%). The dissemination event was attended by 18 participants (seven patients and 11 clinicians). Suggestions to improve implementation included reducing variation and promotion of standardisation of practice by a consensus statement, increasing the evidence base (clinicians) and improved communication by developing better patient aids such as videos and diagrams (patients and clinicians)., Conclusion: Implementation of RTB may be dependent on the quality of information provided, its format and perceived reliability of the information. Abstract: Introduction: Crash data analyses based on accident datasets often do not include human-related variables because they can be hard to reconstruct from crash data. However, records of crash circumstances can help for this purpose since crashes can be classified considering aberrant behavior and misconduct of the drivers involved., Method: In this case, urban crash data from the 10 largest Italian cities were used to develop four logistic regression models having the driver-related crash circumstance (aberrant behaviors: inattentive driving, illegal maneuvering, wrong interaction with pedestrian and speeding) as dependent variables and the other crash-related factors as predictors (information about the users and the vehicles involved and about road geometry and conditions). Two other models were built to study the influence of the same factors on the injury severity of the occupants of vehicles for which crash circumstances related to driver aberrant behaviors were observed and of the involved pedestrians. The variability between the 10 different cities was considered through a multilevel approach, which revealed a significant variability only for the inattention-related crash circumstance. In the other models, the variability between cities was not significant, indicating quite homogeneous results within the same country., Results: The results show several relationships between crash factors (driver, vehicle or road-related) and human-related crash circumstances and severity. Unsignalized intersections were particularly related to the illegal maneuvering crash circumstance, while the night period was clearly related to the speeding-related crash circumstance and to injuries/casualties of vehicle occupants. Cyclists and motorcyclists were shown to suffer more injuries/casualties than car occupants, while the latter were generally those exhibiting more aberrant behaviors. Abstract: Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA., Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up., Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections., Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception., (© 2024. The Author(s).)
