Abstract

The objective: to analyze efficacy of ascorbic acid or its combination with glucocorticoids and thiamine on clinical outcomes in patients with sepsis and septic shock.Subjects and Methods. The literature search was carried out in the database of MEDLINE, Embase, and the Cochrane Library systems. It is limited to published articles from January 1, 2008 to March 1, 2022. The selection criteria were observational studies in patients over 18 years of age with septic shock, if they provide data on the use of ascorbic acid in the form of monotherapy or in combination with hydrocortisone and thiamine.Conclusion. The review includes 18 publications, including 3 meta-analyses on the use of ascorbic acid or its combination with hydrocortisone and thiamine in patients in the adjuvant therapy program for sepsis and septic shock. Clinical efficacy has been proven in 11 studies. However, 7 publications did not show global changes in the outcomes of sepsis and septic shock therapy compared to basic therapy. Currently, there is a need for further research.

Abstract

Many historians believe that Russia became a great power either as a result of the Poltava victory in 1709, or after the Nystadt Peace of 1721. It is difficult to agree with this. Peter the Great’s rule indeed produced a combat-ready regular army, a guard, an officer corps, a navy with shipyards, military bases, and coastal artillery. There was an upsurge in the metallurgical industry and mining. Schools with high-quality military and secular education, the Academy of Sciences, the Senate, and the Synod were established. St. Petersburg was founded. Talented and enterprising individuals were promoted to military, diplomatic and administrative posts. The main factor in the rise of the state was military modernization. The main geopolitical achievement of Peter I was the conquest of full access to the Baltic Sea. However, a limited resource base, military and diplomatic defeats and setbacks did not allow Russia to rise to the rank of a great power. The disasters of Narva in 1700 and on the Prut River in 1711 were painful. Russia lost access to the Sea of Azov, the city of Azov, city of Taganrog, the Azov squadron, shipyards and shipbuilding in the Voronezh Territory were lost. The damage from three treatises with the Ottomans in 1711-1713 was great. Russia has lost all of Zaporozhye. The demarcation of the borders of 1714 threw Russia back several hundred kilometers from the Black Sea region. In 1719, the Russian military force was squeezed out of Central Europe – from Mecklenburg. The sphere of influence of Russia after the victorious Peace of Nystad in 1721 was established only in Northern and Eastern Europe – in the Polish-Lithuanian Commonwealth, the Swedish and Danish-Norwegian kingdom, partly in Prussia. The tsar had no claims to hegemony in Europe and no claims to join the circle of the then great powers. Russia was not a great power like the Habsburg monarchy, France, Great Britain, and the Eurasian-African Ottoman Empire. Russia could not compare with the great powers of that time neither in terms of economic (industrial, financial) power, nor in terms of the intensity of expansionism. The entry of the Russian Empire into the system of international relations as one of the five great powers – France, Great Britain, Austria, Prussia, and Russia – occurred during the Seven Years War of 1756-1763. Another rise to great power took place during the reign of Catherine II. The apogee of greatness and the culmination of Russia's influence on European affairs was the Congress of Vienna in 1814-1815.

Abstract

The paper deals with the issues of cybersecurity in the conditions of digital transformation and the development of society. The authors define cybersecurity and cyberspace by following international practice and IT standardization. According to the ISO/IEC 27032 standard, the cybersecurity position concerning other security domains is defined. Based on the best practices of IT international standards, the authors have considered the current aspects of cybersecurity threats, both external and internal. Considering the results of global statistical research, the authors have identified the current threats to digital transformation in organizations. The paper deals with guidelines to reducing the risks from losses of dangerous cyberattacks.

Abstract

The article deals with the development of human society under the conditions of change of the final technological pattern existing before the 4th industrial revolution. The change is associated with the development of information and communication technologies (ICT) as one of the catalysts of sustainable progress in the development of social and industrial relations in the modern society. It is stated that progress in the field of industrial relations and technologies developing conformity with the exponential law. At the same time, according to the law, the so-called point of technological singularity appears on the time scale of the development when the development path of what is referred to as machine intelligence becomes unpredictable. Various behavior scenarios and the use of artificial intelligence (AI), including its machine learning tools employed for the adverse purposes of enslaving human society, are considered. However, recent studies of mathematicians prove that the possibilities of AI are not unlimited. Like the human mind, AI is limited by the paradoxes of the theory of sets. In this regard, in the development and application of ICT, it is necessary to implement the integrated level of ICT fusion with the evolutionary development of personality and society that would not lead to the suppression of the human thought process by the services of technocratic means.

5. Success of early antiviral monotherapy with pegylated interferon α-2a for posttransplantation fibrosing cholestatic hepatitis C (a clinical case)

Abstract

The paper describes a case of posttransplantation fibrosing cholestatic hepatitis C (PFCHC) in a female patient whose transplant was preserved due to timely antiviral therapy (AVT). Its differential diagnosis primarily required that chronic graft rejection and biliary anastomotic stricture be excluded as a cause of dysfunction. PFCHC was proposed to be a possible cause of graft dysfunction on the basis of the clinical course of the disease, the presence of a very high viral load, intervals after orthotopic liver transplantation, the results of histological studies, and no signs of biliary hypertension. Antiviral monotherapy was initiated using pegylated interferon α-2a (Pegasys), which resulted in a considerable improvement and yielded complete early virological and biochemical responses to AVT. This allows one to hope to obtain a sustained virological response and to eliminate hepatitis C infection in the patient after termination of a 48-week course of therapy.

Abstract

One of the principal problem in contemporary macroeconomics is concerned with factors increasing or decreasing economic dynamics. The mainstream approach is based on neoclassical assumptions, but recently new approaches appear mostly based on new Keynesian concepts. In present time the influence of monetary market and credit instruments become more and more significant. Credit resources of banking and financial structures can affect and distort to reallocation of resources for national and even for global economic. In present paper an empiric and econometric analysis for some macroeconometric and monetary indices for Russian Federation is done. An econometrical models describing the influence of credit variables onto real GDP is estimated. It is shown that in short-term periods changes in credit variables do influence significantly onto GDP. It is shown that on short-term periods changes in money aggregate M2 brings influence (through credit variables) onto national output. As well it is shown that changes in short-term interest rate brings significant negative influence onto real output. Impulse response functions for GDP on shocks of credit variables, monetary base and short-term interest rate are evaluated. For the present study of credit cycles and their impact to real business cycles statistical data (quarterly time series) on the following factors for Russian Federation are collected: nominal and real GDP, monetary base M2, short-term interest rate, long-term interest rate (10-year treasuries bill rate), total debt outstanding. All time series are seasonally adjusted and collected for the period 2004 Q1 - 2013 Q2. All interest rates are adjusted for inflation (i.e. we deal with real interest rates). The investigation of long-term relationship for the factors under consideration are based on integration. It is important to note that in the present paper all econometric models are estimated on "pure" statistical data, while in many research papers on business and credit cycles all evaluations and inferences are based on "filtered" time series (mostly filtered by Hodrick-Prescott's method). In present paper "causality" always means "Granger causality". All estimations are made in gretl, an open-source multiplatform econometric software.

The results of technical diagnosis of the destruction of renewed rotor blades of a stationary gas turbine plant are described. Visual, fractographical, and metallographical inspection methods were used to determine the causes of the destruction. The causes of the operational destruction were determined. Practical recommendations for preventing similar destructions were formulated.

Results of experimental studies of the cementation recovery of tin in a dispersed form from sulfuric solutions are presented. The dependence of the steady potential of aluminum on the concentration of chlorine ions in a sulfuric solution of divalent tin is shown. Polarization measurements of electrode processes of the Al-Sn galvanic pair are performed. The dependence of dispersity and bulk density of the tin powder on cementation conditions is established.

Abstract

The results of applied investigations on the optimization of existing methods and the development of new methods for testing the microstructure of a metal that is used under creep conditions at the steam-power generating facility of a thermal power plant are generalized. The process of obtaining metallographic polymer replicas and metal replicas fabricated by electrostatic precipitation and plastic deformation of an aluminum foil is studied. The technique of carbide analysis is optimized. An electrochemical method for diagnosing heating surfaces is proposed. The possibility of diagnosing superheaters based on the concentration of chromium in cinders is extended to boilers that run on Ekibastuz coal. Examples of the application of metallographic analysis results for revealing the causes of operational fractures in power generating facilities are given.

The possibility of obtaining metallographic replicas made of aluminum foil by knurling is considered. Results are presented from determining the force that has to be applied to the knurling tool to ensure that the aluminum foil’s contacting surface will adequately reflect a steel metallographic section’s microstructure. A device for obtaining aluminum replicas that was manufactured and tested under field conditions is described.

Effect of kinetic parameters of electrode processes on the growth dynamics and structural characteristics of the friable metal deposit, which forms via its contact deposition from an aqueous solution, is studied. A model concept is based on the electrochemical processes proceeding at the metal/solution interface. Dependences of the deposition dynamics and deposit characteristics on the exchange currents of depositing and dissolving metals and on the hydrogen exchange currents at the surfaces of both metals are studied.

Abstract

A model is proposed for describing dynamics of contact exchange of metals, which takes into account the side process of hydrogen evolution. The contact exchange dynamics and structural characteristics of the deposit are considered as functions of the concentration of ions that undergo reduction in solution and the cementation EMF. The consideration makes allowance for different densities of microelements and the effect of the solution stirring by hydrogen evolving concurrently.

The paper deals with one of the technological properties of metal powders, such as tap density. The objective is to analyze the state standard in force for determining the density of metal powders after tapping, taking into account operational experience in the field of powder metallurgy. It is established that the methods identified in the standard for determining the density of metal powders have contradictions and discrepancies. A simple and reliable device is proposed and manufactured for mechanical tapping of powders.

The opportunity to make metallographic replicas from aluminum foil by means of knurling is investigated. The force to be applied to a roller for the contact surface of foil to adequately replicate a steel metallographic specimen microstructure is determined. The device for making aluminum replicas for non-destructive testing of a steam pipeline microstructure is made and tested under the thermal power plant conditions.

Abstract

Results of laboratory studies on depositing metallic replicas by chemical reduction from aqueous solutions of copper and a copper-nickel alloy are presented. Data of quantitative measurements of the strength of cohesion with steel metallographic specimens and of the reflectivity of the replicas are discussed. The method for controlling microstructure with the help of replicas deposited by chemical reduction has been tested under the conditions of thermal power plants.

It is shown experimentally that a compact tin layer is formed on aluminum during the cementation of tin from the concentrated solution of tin sulfate in the initial period of the process. A method for the metallographic identification of the tin layer on the aluminum surface is suggested.

A girl aged 14 years had a severe electric injury after contact with an earth electrode of a high voltage power transformer. Two months after the injury, bilateral electric cataract developed in the presence of posttraumatic cerebral angiodystonia and polyneuropathy. Both eyes were operated on and intraocular lenses were implanted with good results.

Abstract

Results of crystallographic analysis of the lacrimal fluid in patients with glaucoma combined with cataract are analyzed. The characteristic features of lacrimal crystallograms of such patients consist in changed ratio of the crystallization zones with disappearance of the intermediate zone and appearance of stellate structures on crystallograms of patients with progressing cataracts. The method of lacrimal crystallography is suggested to be used in examinations of patients with glaucoma.

Abstract

The development of glaucoma involves changes in the chamber humor caused by hydrodynamic disorders. Primary glaucoma is often combined with lens opacities. Comparison of the content of protein and protein fractions in aqueous humor and blood serum of patients with glaucoma and different degree of lens opacity showed that, in contrast to senile cataract, cataract in a glaucomatous eye forms when the permeability of the blood-eye barrier is increased, which justifies the administration of drugs stabilizing it.

Abstract

Biochemical changes in the chamber humor play an important role in the pathogenesis of primary open-angle glaucoma and are largely determined by the permeability of the blood-eye barrier. Comparative study of total protein content in the chamber humor in mature and far advanced glaucoma showed that the disease involves a progressive increase in the permeability of the blood-aqueous humor barrier, which may be due to dystrophic changes in ocular tissues in the course of glaucoma progress and accumulation of metabolites damaging the biological membranes in the chamber fluid or to antiglaucoma drugs increasing the permeability of the blood-humor barrier.

Abstract

1. On the basis of an investigation into the properties of various solutions, the following optimum electrolyte composition (moles/liter) was chosen: 0.2 nickel and iron (jointly) in the form of sulfates, 0.38 (NH4)2SO4, 0.07 NaF, and 0.2 KCl; pH 5. 2. It is shown that the composition of the iron-nickel alloy powder is governed by the concentration of its components in the electrolyte, but is not affected by changes in current density or electrolyte temperature. These characteristics are attributed to the fact that the limiting discharge currents of nickel and iron are similar and change in an equivalent manner under the influence of electrolysis conditions. 3. A study was made of the phase composition of the alloy as a function of nickel and iron concentrations and current density. It was established that the alloys represent a continuous series of solid solutions. 4. The influence of electrolyte composition and electrolysis parameters on the current efficiency was investigated. It was concluded that the optimum electrolysis parameters are a current density of 3000 A/m2 and a temperature of 30°C. 5. The physical properties of the alloy powders were investigated as a function of electrolysis conditions. 1. Adsorption of SAA on the electrodes of contact electrolytic cells strongly affects the cementation emf and current in the deposition copper in finely divided form and hence the deposition rate and properties of the resultant powder. 2. Use of the SAA thiourea, gelatin, and chlorine ions offers an effective means of regulating the physical properties of cementation copper powder. 3. The addition of chlorine ions, unlike that of thiourea and gelatin, promotes the formation of a looser cementation deposit composed of particles with a more dendritic structure and hence increases the rate of powder deposition.

Abstract

A copper powder produced by cementing copper with iron from a sulfate solution can be purified from iron by treatment with a sulfuric acid solution. Reverse dissolution of copper in this process can be reduced by employing a protective atmosphere generated by the addition of sodium carbonate to the H2SO4, solution. A necessary condition for the effective purification of finely divided copper from the cementing metal is deposition of copper in loose form.
