Abstract

Summary Shallow stratigraphy in Southern Oman is characterized by the presence of an anhydrite layer causing a strong velocity inversion which makes seismic imaging particularly difficult. This known shallow sharp velocity inversion cannot be easily captured with reflected wave-based techniques or even acoustic full waveform inversion. We propose to recover it by applying multi-wave inversion, an approach combining information from P wave first breaks and ground-roll dispersion curves. In addition, an unsupervised machine learning technique is used to improve the quality of surface wave dispersion curve picks, crucial for the reliability of the multi-wave inversion results. With this innovative approach, the joint inversion of first breaks and surface waves leads to a better high resolution P-wave velocity model of the near surface which enables improved deep imaging.

Abstract

Introduction Les complications les plus frequentes et les mieux connues de l’embolie pulmonaire sont la recidive et l’hypertension arterielle pulmonaire. L’embolie pulmonaire a egalement un impact psychologique, et plusieurs etudes ont montre que les patients ayant ete atteints d’une embolie pulmonaire avaient des scores de depression, d’anxiete et de qualite de vie moins bons que les groupes temoins. Aucune etude a notre connaissance ne s’est attachee a analyser le vecu des patients de moins de 50 ans, tranche d’âge particulierement active et porteuse de projets. L’objectif de cette etude qualitative transversale est d’analyser le vecu des patients ayant ete atteints d’une embolie pulmonaire alors qu’ils avaient moins de 50 ans. Patients et methodes Les patients etaient inclus s’ils avaient moins de 50 ans au moment du diagnostic d’embolie pulmonaire, et en l’absence de neoplasie sous jacente. Un entretien semidirige base sur un guide de 11 questions a ete realise pour chaque patient au CHU de Rennes. Ces entretiens ont ete enregistres, puis retranscrits sur ordinateur. Une analyse phenomenologique interpretative a ete realisee pour l’ensemble des entretiens afin de degager les themes principaux. Resultats Dix-neuf patients ont ete inclus, 1 mois a 19 ans apres le diagnostic d’ embolie pulmonaire. L’analyse phenomenologique interpretative permet de degager 3 themes principaux. Premierement, l’embolie pulmonaire est une maladie peu connue, les symptomes initiaux sont souvent banalises par les patients et la gravite sous estimee. La diversite des symptomes peut entrainer des erreurs diagnostiques et un retard au diagnostic, source d’anxiete pour le patient. La recherche etiologique est parfois negative, aggravant l’anxiete ressentie par les patients. Les anticoagulants, malgre leurs contraintes et leurs risques apparaissent essentiels. Deuxiemement, l’embolie pulmonaire est un evenement traumatisant, de survenue brutale chez des patients en bonne sante, qui prennent souvent conscience de la gravite de la situation a la vue de leurs proches ou suite a l’annonce diagnostique par les medecins. Cette peur retrospective est associee a l’evocation de la mort chez de nombreux patients. Une rupture identitaire peut survenir au decours avec une accumulation de pertes : perte de l’image d’invincibilite, des capacites physiques, de l’image des autres. Les proches sont egalement touches par la survenue de l’embolie pulmonaire. Enfin, les reactions au decours de l’embolie pulmonaire sont variables, un sentiment d’injustice est frequent chez ces patients jeunes qui sont parfois initialement dans le deni. Le temps favorise l’acceptation de la maladie, bien que certains patients developpent une symptomatologie depressive. Neanmoins, cet evenement aigu permet une prise de conscience et de nombreux patients declarent profiter plus de la vie qu’auparavant. Les strategies d’adaptation deployees varient selon les patients, mais passent souvent par la recherche de soutien. Leur vie peut etre bouleversee par cet episode, entrainant l’abandon de certaines activites ou le renoncement a certains projets. Certains patients sont obliges de s’adapter aux limitations physiques imposees par la maladie, alors que d’autres ne changent ni leurs habitudes ni leurs projets de vie, et considerent l’embolie comme une maladie aigue. Quelle que soit l’appreciation qu’ont les patients de cette maladie, ils vivent tous avec la peur de la recidive. Conclusion L’embolie pulmonaire survient brutalement chez des patients jeunes, il s’agit d’un evenement traumatisant pouvant entrainer une rupture identitaire et des modifications des projets de vie. Il est essentiel de s’interesser au vecu des patients ayant ete atteints d’une embolie pulmonaire, afin de leur proposer une prise en charge optimale et pluridisciplinaire.

Abstract

Described for the first time by Bradshaw in 1846, actinomycosis is a rare, slowly progressing disease associated with the development of anaerobic bacteria of the Actinomyces genus. It predominates in males and is actively encouraged by poor buccal and dental hygiene and by overall decline of defence mechanisms. Thoracic lesions constitute 15 to 20% of the cases and seem to be relatively increasing. Apart from fistulization to the skin, physical examination usually does not show much; only the presence of yellow grains suggests the diagnosis. Radiography is often misleading, suggesting tuberculosis or cancer. Ultrasonography and, chiefly, computerized tomography are the best methods to evaluate the extension of the disease to the pleura, the chest wall and the mediastinum. The final diagnosis is more often supplied by pathological examination than by bacteriology, which is frequently negative. Treatment is facilitated by the fact that Actinomyces is sensitive to antibiotics, notably to penicillin which still is the first choice drug. A well-conducted treatment will give satisfactory results, but sequelae of retractile fibrosis may be disabling.

7. What Is the Gravitational Field of a Mass in a Spatially Nonlocal Quantum Superposition?

Abstract

The study of the gravitational field produced by a spatially nonlocal, superposed quantum state of a massive particle is an interesting and active area of research. One outstanding issue is whether the gravitational field behaves like the classical superposition of the gravitational field of two particles separated by a spatial distance with half the mass at each position. Alternatively, does the gravitational field behave as a quantum superposition with a far more interesting and subtle behavior than a simple classical superposition? Quantum field theory is ideally suited to probe exactly this kind of question. We study the scattering of a massless scalar on a spatially nonlocal quantum superposition of a massive particle. We compute the differential scattering cross section corresponding to one-graviton exchange. We find that the scattering cross section disagrees with the Newton-Schrödinger picture of potential scattering from two localized sources with half the mass at each source. This suggests that experimental observation of gravitational scattering could inform the viability of the semiclassical treatment of the gravitational field, as in the Newton-Schrödinger description, vs the fully quantum mechanical treatment adopted here. We comment on the experimental feasibility of observing such effects in systems with many particles such as Bose-Einstein condensates.

Abstract

Introduction: Focusing on the monthly prevalence of health problems and recourse to different levels of care of the population is an interesting approach to demonstrate the respective roles of different levels of health care. In the present study, the ecology of health care was studied in the region of Liège, Belgium., Method: A survey questioning people about their health problems was conducted in 2009 in two communes of the province of Liège. For each health problem, 'health care' was defined as contact with any qualified care provider. For each consultation, three elements were recorded: the profession of the health care provider; the place where the care was provided and the kind of health care received., Results: A total of 537 people were interviewed. The monthly prevalence of people who experienced a health problem during the previous month was 85.1%. The monthly prevalence of people who turned to a health care provider at least once during the month was 62.2%. The proportion of people turning to doctors, primarily local doctors, for a simple consultation was important (49.2%)., Discussion: Our results are highly comparable with those of other studies. Recourse to a doctor is high (49%), which probably reflects the broad accessibility of health care in Belgium and maybe its overuse. Additional questions on the current and future organisation of the Belgian health care system are debated.

Abstract

Background: In order to address the challenges of an ageing population the Belgian government decided to allocate resources to the creation of geriatric day hospitals (GDHs). Although GDHs are meant to be a strategy to support general practitioners (GPs) caring for the frail elderly, few Belgian GPs seem to refer to a GDH. This study aims to explore the barriers and facilitating factors of GPs' referral to GDHs., Methods: A qualitative study using focus group discussions (FGDs) was conducted. Fifteen FGDs were organized in the different Belgian regions (Flanders, Wallonia, Brussels)., Results: Contextual factors such as the unsatisfactory cooperation between hospital and GPs and organizational barriers such as the lack of communication on referral procedures between hospital and primary health care (PHC) were identified. Lack of basic knowledge about the concept or the local organization of GDH seemed to be a problem. Unclear task descriptions, responsibilities and activities of a GDH formed prominent points of discussion in all FGDs. Nevertheless a lot of possible advantages and disadvantages of GDHs for the patient and for the GP were mentioned., Conclusions: In the case of poor referral to GDHs, focusing on improving overall collaboration between primary and secondary health care is essential. This can be achieved by actively delivering adequate information, permanent communication and more involvement of PHC in the organization and functioning of GDHs. The absence of a transparent health care system with delineated role definitions, seems to hinder the integration of new initiatives like GDHs in the care process. Strategies to enhance referral to GDHs should use a comprehensive approach.
