First of all, I would like to thank my colleagues for their interest in this article [1] and note the relevance of the problem of surgical treatment of moyamoya disease, which provoked this discussion. Moyamoya disease is a rare, atypical pathology for our country, the diagnosis and treatment of which is controversial among specialists, and the lack of agreed protocols is the cause of differences in the interpretation of the results of surgical treatment of such patients.В первую очередь хотелось бы поблагодарить коллег за интерес к представленной статье [1] и отметить актуальность проблемы хирургического лечения болезни моямоя, вызвавшей данную дискуссию. Болезнь моямоя является редкой, нетипичной для нашей страны патологией, диагностика и лечение которой вызывает споры среди специалистов, а отсутствие согласованных протоколов является причиной различий в трактовке результатов хирургического лечения таких пациентов.

An adult patient with progressive chronic cerebral ischemia associated with moyamoya disease who underwent combined revascularization of both cerebral hemispheres in step-by-step fashion is reported in the article. The feature of this case is a large volume of revascularization procedures with double-barrel extra-intracranial anastomoses combined with indirect synangioses. This surgical approach ensured early postoperative development of extensive collateral network and complete compensation of impaired cerebral circulation. Particular attention is paid to description and discussion of preoperative diagnosis, the choice of surgical treatment, as well as the features of surgical technique. The advantages and disadvantages of this approach are discussed in comparison with literature data.В работе представлено редкое клиническое наблюдение: взрослый пациент с прогрессирующей хронической ишемией головного мозга, обусловленной болезнью моямоя, которому проведена этапная комбинированная реваскуляризация обоих полушарий мозга. Особенностью этого случая является большой объем выполненных реваскуляризирующих вмешательств в виде «двуствольных» экстраинтракраниальных анастомозов в сочетании c комбинацией непрямых синангиозов. Подобная тактика хирургического лечения позволила уже в короткие сроки после операции добиться развития обширной сети коллатералей и полностью компенсировать имевшийся дефицит мозгового кровообращения. Особое внимание уделено описанию и обсуждению предоперационной диагностики, выбору тактики хирургического лечения, а также особенностям техники оперативного вмешательства. Обсуждаются преимущества и недостатки использованного подхода по сравнению с данными, представленными в литературе.

Combined cerebral revascularization with direct and indirect components is recognized as the most appropriate method for treating patients with Moyamoya disease all over the world, however, large studies on its effectiveness in Russia have not yet been conducted.Was to evaluate the results of combined cerebral revascularization in patients with Moyamoya disease with an analysis of the clinical state and perfusion and angiographic features of neoangiogenesis.For the period from 2013 to 2020 in N.N. Burdenko National Medical Research Center of Neurosurgery 79 combined revascularizations were performed in 55 patients with Moyamoya disease. The average age was 13.9±10.11 years. All patients were examined according to a single protocol, including a comprehensive assessment of the clinical symptoms, the state of the brain tissue, the cerebral vascular system, and cerebral blood flow. The results of surgical treatment were evaluated in the early postoperative and long-term periods. The average follow-up period was 14.66±13.17 months. In the postoperative period, the dynamics of neurological status and cerebral blood flow were assessed and the features of neoangiogenesis from various components of revascularization were studied.In the early postoperative period, in 77.2% of cases, a good treatment result was observed, with no negative dynamics of the neurological status. Ischemic stroke in the operated hemisphere developed in 4 cases (5.1%). In the follow-up period, an improvement in neurological symptoms was observed in 76.6%. Improvement of cerebral blood flow was noted in 92.4% of cases, direct anastomoses functioned in 94.3%, and signs of neovascularization in the area of indirect synangiosis were observed in 80.0%. At the same time, a complementary influence of direct and indirect components of revascularization was revealed in ensuring good general angiographic and perfusion results.Combined revascularization is a highly effective method of surgical treatment of all patients with Moyamoya disease.Комбинированная реваскуляризация головного мозга с использованием прямых и непрямых компонентов признана наиболее целесообразным методом лечения пациентов с болезнью мойя-мойя во всем мире, однако крупных исследований по изучению ее эффективности в России до настоящего времени не было.Провести оценку результатов комбинированной реваскуляризации головного мозга у пациентов с болезнью мойя-мойя с анализом клинической картины, перфузионных и ангиографических особенностей неоангиогенеза.За период с 2013 по 2020 г. во ФГАУ «НМИЦ нейрохирургии им. акад. Н.Н. Бурденко» Минздрава России выполнено 79 комбинированных реваскуляризаций у 55 пациентов с болезнью мойя-мойя. Средний возраст больных составил 13,9±10,11 года. Все пациенты обследованы по единому диагностическому протоколу, включающему анализ клинического течения, комплексную оценку состояния мозговой ткани, сосудистой системы головного мозга и мозгового кровотока. Результаты хирургического лечения оценивали в раннем послеоперационном и отдаленном периодах. Средний срок катамнеза составил 14,66±13,17 мес. В послеоперационном периоде проведена оценка динамики неврологического статуса, мозгового кровотока, исследованы особенности неоангиогенеза при различных типах реваскуляризации.В раннем послеоперационном периоде в 77,2% случаев наблюдался хороший результат лечения с отсутствием отрицательной динамики неврологического статуса. Ишемический инсульт в прооперированном полушарии развился в 4 (5,1%) случаях. В отдаленном послеоперационном периоде улучшение очаговой и общемозговой неврологической симптоматики наблюдалось у 76,6% пациентов. Прямые анастомозы функционировали в 94,3% случаев, а признаки неоваскуляризации в области непрямых синангиозов наблюдались в 80,0%. Улучшение мозгового кровотока отмечалось в 92,4% случаев, при этом выявлено взаимодополняющее влияние прямых и непрямых компонентов реваскуляризации в обеспечении хорошего общего ангиографического и перфузионного результатов оперативного лечения.Комбинированная реваскуляризация является высокоэффективным методом хирургического лечения всех пациентов с болезнью мойя-мойя.

The main aim of the study was to investigate the effect of carotid endarterectomy on the prognosis of chronic cerebral ischemia in patients with symptomatic occlusions of the contralateral internal carotid artery, assess risks of surgical complications, and substantiate the staged surgical approach for treatment of patients with this pathology. The article analyzes the experience in surgical treatment of 83 patients with symptomatic ICA occlusions who underwent surgery for contralateral carotid artery stenosis. In 40 patients, only carotid endarterectomy (CEA) was performed on the side of hemodynamically significant stenosis (group 1). In 43 (52%) cases, apart from CEA, extracranial-intracranial (EC-IC) bypass was performed at the second stage (23 cases, group 2) or the first stage (19 cases, group 3). A surgical treatment approach was chosen based on clinical symptoms, severity of contralateral carotid artery stenosis, and the magnitude of perfusion deficiency in the territory of carotid occlusion. The conducted analysis revealed that patients with symptomatic occlusions and contralateral carotid artery stenoses represented a heterogeneous group with a different efficacy of carotid endarterectomy and with risks of perioperative complications. As cerebrovascular insufficiency in the territory of carotid occlusion and a related neurological deficit worsen, the risks of complications of contralateral carotid endarterectomy increase, and its clinical efficacy decreases. In these cases, cerebral revascularization on the ICA occlusion side should be performed at the first stage.Цель исследования - изучение влияния каротидной эндартерэктомии на прогноз клинического течения хронической церебральной ишемии у больных с симптоматическими окклюзиями противоположной внутренней сонной артерии, оценка рисков осложнений хирургических вмешательств и обоснование тактики этапного хирургического лечения больных с данной патологией. Материал и методы. Анализируется опыт хирургического лечения 83 больных с симптоматическими окклюзиями ВСА, оперированных по поводу стеноза сонной артерии с противоположной стороны. У 40 пациентов выполнялась только каротидная эндартерэктомия (КЭА) на стороне гемодинамически значимого стеноза (1-я группа). В 43 (52%) случаях, помимо КЭА, выполнялась операция по созданию экстра-интракраниального анастомоза, вторым этапом - в 23 случаях (2-я группа) и первым этапом - в 19 случаях (3-я группа). Выбор тактики хирургического лечения определялся клинической симптоматикой, степенью стеноза сонных артерий с противоположной стороны и величиной перфузионного дефицита в бассейне каротидной окклюзии. Результаты и заключение. Установлено, что пациенты с симптоматическими окклюзиями и контралатеральными стенозами сонных артерий представляют собой неоднородную группу, с разной эффективностью каротидной эндартерэктомии и рисками развития периоперационных осложнений. По мере нарастания цереброваскулярной недостаточности в бассейне каротидной окклюзии и связанного с ним неврологического дефицита повышаются риски осложнений контралатеральной КЭА и снижается ее клиническая эффективность. В подобных случаях первым этапом целесообразно выполнение реваскуляризации головного мозга на стороне окклюзии ВСА.

The article describes a case of one-stage surgical treatment of a patient with progressive chronic cerebral ischemia caused by combined steno-occlusive lesions of the carotid and vertebral arteries. The disease was complicated by intolerance to temporary occlusion of the carotid artery due to an incomplete circle of Willis. We performed extra-anatomic carotid-vertebral artery bypass with subsequent ipsilateral carotid endarterectomy. A temporary intraluminal shunt was used at the main stage of reconstructive surgery. We use this clinical case to analyze the issues of surgical treatment for combined lesions of the carotid and vertebral arteries and the techniques for prevention of associated ischemic complications.Рассматривается случай одномоментного хирургического лечения больного с клинической картиной прогрессирующей хронической церебральной ишемии, обусловленной сочетанным стенозирующим поражением каротидного и вертебрального артериальных бассейнов на фоне непереносимости временного пережатия сонной артерии. Пациенту была выполнена экстраанатомическая реконструкция позвоночной артерии в сочетании с каротидной эндартерэктомией. Основной этап реконструкции выполнялся с использованием временного внутрипросветного шунта. На примере данного клинического наблюдения анализируются проблемы хирургического лечения, сочетанных поражений сонных и позвоночных артерий и методы профилактики, связанных с ним ишемических осложнений.

This review is devoted to moyamoya disease. It is a rare chronic steno-occlusive cerebrovascular disease. However, moyamoya disease is increasingly diagnosed by neurosurgeons in our country. Unlike atherosclerotic lesions of cerebral arteries, pathogenesis and course of this disease are much more complex and variable. Therefore, specialists often have certain difficulties in diagnosis, management and treatment of these patients. To date, a large number of surgical interventions have been proposed for the treatment of moyamoya disease. Revascularization approaches include direct procedures (extra-intracranial microanastomoses), indirect methods (synangioses) and combined revascularization. The purpose of the review is to systematize current literature data on the pathogenesis, diagnosis, clinical patterns and surgical treatment of patients with moyamoya disease. results Outcomes of surgical revascularization and the role of its various components in combined approach are under particular attention.Представленный обзор литературы посвящен болезни мойямойя — редкому хроническому стено-окклюзирующему цереброваскулярному заболеванию, которое все чаще выявляется нейрохирургами в России. В отличие от атеросклеротического поражения магистральных артерий головного мозга патогенез и клиническое течение этого заболевания гораздо сложнее и разнообразнее, поэтому у специалистов нередко возникают трудности в диагностике, ведении и лечении таких больных. К настоящему времени для лечения заболевания предложено много разнообразных оперативных вмешательств, направленных на реваскуляризацию мозга и включающих методы прямой (создание экстраинтракраниального микроанастомоза), непрямой (создание синангиозов) и комбинированной реваскуляризации. Целью обзора является освещение и систематизация накопленных данных мировой литературы, касающихся патогенеза, диагностики, клинической картины и тактики хирургического лечения пациентов с болезнью мойямойя. Особое внимание уделено результатам хирургической реваскуляризации и роли ее различных компонентов при комбинированных вмешательствах.

Surgical treatment of cerebral ischemia at the Burdenko Neurosurgical Center for the period from 1999 to 2019 is analyzed in the paper. The details of the treatment strategy in patients with steno-occlusive lesion of craniocervical arteries followed by cerebral ischemia developed over 20 years are discussed in the article. We have analyzed the features of surgical interventions on the major craniocervical arteries in a neurosurgical clinic and the results of this treatment.To demonstrate management of various lesions of major cerebral arteries in modern neurosurgical vascular hospital.In total, there were 3098 interventions on the major cerebral arteries in 2527 patients for this period. Mean age of patients ranged from 1.5 to 91 years (58±14 years). Interventions included open reconstructions of the carotid arteries (2031 surgeries), reconstructions of the vertebrobasilar arteries (135 surgeries), brain revascularization (658 surgeries), excision of the tumors of neurovascular bundle on the neck compressing carotid arteries (51 interventions). Endovascular interventions were performed in 223 cases and consisted of angioplasty and stenting of the extracranial segments of craniocervical arteries (185 surgeries), stenting of the intracranial arteries (30 surgeries) and endovascular thrombextraction (8 cases). Staged surgeries were performed in 541 patients (22.3%).Favorable outcomes were obtained in 87.6% of cases, satisfactory results - in 9% of patients. Clinical deterioration due to long-term postoperative complications and recurrent strokes occurred in 2.9% of cases. Postoperative morbidity rate was 4.6%, persistent neurological deficit developed in 2.6% of cases. Mortality rate was 0.5%.Surgical treatment of stenotic and occlusive lesion of the major cerebral arteries is an interdisciplinary problem. Solution of this issue is closely associated with technological progress, new discoveries in normal and pathological physiology, as well as clinical researches. Individualized choice of surgical approach is one the main modern trends of neurosurgical approach to this problem. At the same time, own surgical experience is the most important factor determining the results of arterial reconstructions.Представлен опыт хирургического лечения сосудистой патологии, приводящей к церебральной ишемии, в ФГАУ «Национальный медицинский исследовательский центр нейрохирургии им. акад. Н.Н. Бурденко» в период 1999—2019 гг. Обсуждаются разработанные за 20 лет алгоритмы выбора тактики лечения пациентов с клиническими проявлениями церебральной ишемии, обусловленной стенозирующей и окклюзирующей патологией брахиоцефальных артерий, особенности хирургических вмешательств на магистральных артериях в условиях нейрохирургической клиники и результаты этого лечения.Продемонстрировать возможности лечения разнообразной патологии магистральных артерий головного мозга в условиях современной нейрохирургической сосудистой клиники.За исследуемый период 2527 пациентам выполнено 3098 оперативных вмешательств на магистральных артериях головного мозга. Возраст больных варьировал от 1,5 лет до 91 года, составив в среднем 58±14 лет. Спектр оперативных вмешательств включал открытые реконструктивные операции на сонных артериях (2031 операция) и артериях вертебробазилярного бассейна (135 операций), реваскуляризацию головного мозга (658 операций), иссечение опухолей сосудисто-нервного пучка шеи, компримирующих сонные артерии (51 операция). Эндоваскулярные вмешательства выполнены в 223 случаях и представлены ангиопластикой и стентированием экстракраниальных сегментов брахиоцефальных артерий (185 операций), стентированием интракраниальных артерий (30 операций) и эндоваскулярной тромбэкстракцией (8 случаев). Этапные хирургические вмешательства выполнены у 541 (22,3%) пациента.В 87,6% случаев получены хорошие результаты, в 9% — удовлетворительные. В 2,9% случаев отмечено ухудшение клинической симптоматики вследствие развившихся осложнений и повторных нарушений мозгового кровообращения в отдаленном послеоперационном периоде. Осложнения хирургического лечения регистрировались в 4,6% случаев, при этом стойкие неврологические дефекты развились в 2,6% случаев, а летальный исход — в 0,5% случаев.Хирургическое лечение стенозирующей и окклюзирующей патологии магистральных артерий головного мозга представляет собой многогранную междисциплинарную проблему, решение которой тесно связано с технологическим прогрессом, новыми открытиями в области нормальной и патологической физиологии, а также с результатами клинических исследований. К основным современным тенденциям нейрохирургического подхода к решению этой проблемы можно отнести индивидуальный выбор тактики хирургического лечения. При этом личный хирургический опыт является важнейшим фактором, определяющим результаты артериальных реконструкций.

The article presents a case of successful surgical treatment of a patient with progressive chronic cerebral ischemia due to occlusions of both internal carotid arteries combined with occlusion of the vertebral artery in the first and second segments. We describe a surgical technique that includes an auto arterial carotid-subclavian bypass in the third segment of the vertebral artery, with an extracranial portion of the occluded internal carotid artery (after preliminary thromboendarterectomy) being used as a shunt. Previously, the patient had undergone surgery for creating bilateral EICMA. We analyzed the indications for each phase of the surgery with allowance for the peculiarities of compensatory collateral circulation and possible complications of the surgical treatment.В статье приводится пример успешного хирургического лечения больного с прогрессирующей хронической церебральной ишемией, обусловленной окклюзиями обеих внутренних сонных артерий в сочетании с окклюзией позвоночной артерии в первом и втором сегментах. Описан метод хирургического лечения, представленный аутоартериальным сонно-подключичным шунтированием в третьем сегменте позвоночной артерии, с использованием в качестве шунта экстракраниальной порции окклюзированной внутренней сонной артерии после предварительной тромбэндартерэктомии. Ранее больному были выполнены операции по созданию ЭИКМА с двух сторон. Проведен анализ показаний к каждому из этапов операции с учетом особенностей компенсаторного коллатерального кровообращения и возможных осложнений хирургического лечения.

Doppler ultrasound (DUS) has been widely used in neurosurgical practice to diagnose various cerebrovascular diseases. This technique is used in transsphenoidal surgery to identify the localization of intracranial arteries when making an approach or during tumor resection.To identify the cavernous segment of the internal carotid artery (ICA) and/or basilar artery during endoscopic transsphenoidal surgery, we used a combined device on the basis of a click line curette («Karl Storz») and a 16 MHz Doppler probe (Lassamed). The technique was used in 51 patients during both standard transsphenoidal surgery (23 cases) and transsphenoidal tumor resection through an extended approach (28 cases).Doppler ultrasound was used in different situations: to determine a trajectory of the endonasal transsphenoidal approach in the absence of the normal anatomical landmarks (16 cases), to define the limits of safe resection of a tumor located in the laterosellar region (7), and to implement an extended transsphenoidal endoscopic approach (28). Intraoperative Doppler ultrasound enabled identification of the cavernous segment of the internal carotid artery in 45 cases and the basilar artery in 2 cases; a blood vessel was not found in 4 cases. Injury to the cavernous segment of the internal carotid artery was observed only in 1 case.The use of the described combined device in transsphenoidal surgery turned Doppler ultrasound into an important and useful technique for visualization of the ICA within the tumor stroma as well as in the case of the changed skull base anatomy. Its use facilitates manipulations in a deep and narrow wound and enables inspection of the entire surface of the operative field in various planes, thereby surgery becomes safer due to the possibility of maximum investigation of the operative field.Ультразвуковая допплерография (УЗДГ) нашла широкое применение в нейрохирургической практике для диагностики различных цереброваскулярных заболеваний. Использование этой методики в транссфеноидальной хирургии позволяет определить местоположение интракраниальных артерий на этапе доступа или в процессе удаления опухоли. Материал и методы. При проведении эндоскопических транссфеноидальных операций для обнаружения кавернозного сегмента внутренней сонной артерии (ВСА) и/или базилярной артерии использовалось комбинированное устройство, основу которого представляет кюретка click line фирмы 'Karl Storz' и допплеровский датчик 16 МГц фирмы 'Lassamed'. Описываемая методика была использована у 51 пациента в процессе осуществления как стандартных транссфеноидальных операций (23 случая), так и транссфеноидальных резекций опухоли из расширенных доступов (28). Результаты и обсуждение. Ультразвуковая допплерография применялась в разных ситуациях: при отсутствии нормальных анатомических ориентиров, используемых для определения траектории эндоназального транссфеноидального доступа (16 случаев), при удалении латероселлярно расположенной опухоли с целью определения границ безопасной резекции новообразования (7), при осуществлении расширенных транссфеноидальных эндоскопических доступов (28). Использование интраоперационной ультразвуковой допплерографии позволило определить местоположение кавернозного сегмента ВСА в 45 наблюдениях, базилярной артерии в 2 случаях; кровеносный сосуд не был обнаружен в 4. При этом повреждение кавернозного сегмента ВСА наблюдалось только в 1 случае. Заключение. Использование упомянутого выше комбинированного устройства в транссфеноидальной хирургии сделало допплерографию важным и полезным методом визуализации ВСА в строме опухоли, а также при измененной анатомии основания черепа. Его применение облегчает манипулирование в глубокой и узкой ране и позволяет лоцировать всю поверхность операционного поля в разных плоскостях, что делает операцию более безопасной в связи с возможностью максимального исследования операционного поля.

Objective: to study higher mental functions (HMF) in patients with stenotic lesion of the brachiocephalic arteries (BCA) before and after carotid endarterectomy (CE). Patients and methods. Prior to and following CE, 97 patients underwent neurological examination, neuropsychological syndrome analysis described by A.R. Luria, evaluation using the Wexler verbal working memory scale, Stroop word-digital test, and Schulte test. BCA ultrasound duplex scanning and cerebral perfusion were made over time. Results. Preoperative neuropsychological study revealed abnormalities in 98% of the patients. There was most common bilateral frontal dysfunction and deep structural pathology, which were more pronounced in persons above 50 years of age and in the presence of mixed BCA stenosis. Stenosis-associated unilateral parietal and temporal dysfunctions were more frequently registered. Within 30 days after CE, the maximum HMF changes were found in patients having unilateral stenosis. There was ameliorated dysfunction of the structures blood is supplied to which by an operated vessel and the vertebrobasilar system. Poststroke patients were found to have a significant intraoperative HMF reduction in the early rehabilitation period of stroke. The syndromes having common and local mechanisms of development and different topic sites were identified among the short-term and reversible HMF impairments. Conclusion. Despite heterodirectional HMF changes in the early period after BCA reconstructive operations, the positive cognitive functional changes in all the patients at 30 days of CE confirm the rationale for and efficiency of surgical intervention.

To assess changes in local hemodynamic parameters in patients with symptomatic ICA occlusions and moyamoya disease after placement of extracranial-intracranial bypass (EC-IC bypass).The study included 112 patients who underwent surgical treatment at the National Scientific and Practical Center for Neurosurgery in the period between 1999 and 2015. Of these, 105 patients had ICA occlusions, and 7 patients had moyamoya disease. During the main stage of EC-IC bypass placement, all patients were monitored for local hemodynamic parameters using intraoperative contact Doppler ultrasonography - 89 (72%) patients (72%) and flowmetry - 56 (50%)). In 33 (29%) cases, both techniques were used. Forty two patients underwent preoperative SCT perfusion to assess the degree of perfusion deficit. Grade 1 cerebrovascular insufficiency (acute oligemia) was detected in 6 patients; grade 2 perfusion deficit (persistent oligemia) was found in 25 patients; grade 3 perfusion deficit (chronic oligemia) was present in 11 patients. Measurements were performed before bypass placement: the blood flow direction and hemodynamic parameters in the cortical arteries were evaluated; and after bypass placement: blood flow values and directions in the cortical artery, proximal and distal to the bypass area, were assessed.A total of 112 EC-IC bypasses were placed without perioperative complications and deaths. Bypass functioning was confirmed in 108 (96.3%) cases; bypass thrombosis occurred in 4 (3.7%) cases. The distal blood flow direction was observed in patients with ICA occlusions (105 patients) in all cases before EC-IC bypass placement. Patients with moyamoya disease had more often the proximal blood flow direction - 5 (71%) out of 7 cases. The cerebral blood flow parameters obtained in this study differed significantly, depending on the baseline degree of perfusion deficit. The blood flow rate was minimal in patients with grade 1 cerebrovascular insufficiency. After revascularization, local hemodynamics in the cortical arteries was significantly dependent on the ability of EC-IC bypass to reverse blood flow in the proximal acceptor artery. A change in the blood flow direction was observed in 86 (77%) cases. The mean volumetric blood flow in EC-IC bypass was 34.2±5.7 mL/min.The knowledge of baseline hemodynamic parameters and their changes after revascularization plays an important role in choosing the correct surgical technique, further bypass functioning, and, as a result, improvement of the clinical outcome after surgery.Цель исследования - оценка изменений параметров локальной гемодинамики у пациентов с симптоматическими окклюзиями внутренней сонной артерии (ВСА) и болезнью моя-моя после создания экстра-интракраниального микроанастомоза (ЭИКМА). Материал и методы. В исследование вошли 112 пациентов, которым проведено хирургическое лечение в НМИЦ нейрохирургии им. Н.Н. Бурденко с 1999 по 2015 г. Среди них были 105 пациентов с окклюзией ВСА и 7 - с болезнью моя-моя. У всех пациентов во время основного этапа создания ЭИКМА были изучены локальные показатели гемодинамики с использованием методов интраоперационной контактной допплерографии (89, 72%) и флоуметрии (56, 50%). В 33 (29%) случаях использовались оба метода. У 42 пациентов имелись данные о степени перфузионного дефицита на основании предоперационного СКТ-перфузионного исследования. У 6 пациентов выявлен I тип цереброваскулярной недостаточности (острая олигемия), у 25 - II тип перфузионного дефицита (сохраняющаяся олигемия), у 11 - перфузионный дефицит III типа (хроническая олигемия). До создания анастомоза оценивались направление кровотока и значения гемодинамических параметров в корковых артериях, после создания анастомоза оценивались значения и направление кровотока в корковой артерии проксимальнее и дистальнее области анастомоза. Результаты. Всего выполнено 112 операций по созданию ЭИКМА без периоперационных осложнений и летальности. Функционирование анастомоза было подтверждено в 108 (96,3%) случаях, тромбоз анастомоза наблюдался в 4 (3,7%). У пациентов с окклюзиями ВСА во всех случаях до ЭИКМА наблюдалось дистальное направление кровотока. У 5 (71%) из 7 пациентов с болезнью моя-моя чаще отмечалось проксимальное направление кровотока. Полученные в ходе исследования параметры мозгового кровотока достоверно отличались в зависимости от исходной степени перфузионного дефицита. У больных с цереброваскулярной недостаточностью I типа скорость кровотока имела наименьшие значения. Локальная гемодинамика в корковых артериях после реваскуляризации существенно зависела от способности ЭИКМА развернуть кровоток в проксимальных отделах акцепторной артерии. Изменение направления кровотока наблюдалось в 86 (77%) случаях. Средний объемный кровоток по ЭИКМА составил 34,2±5,7 мл/мин. Выводы. Знание исходных параметров гемодинамики и их изменений после реваскуляризации играет важную роль в обосновании правильной техники операции, контроле дальнейшего функционирования анастомоза и, как следствие, клинического состояния после операции.

The article reports a clinical case of endovascular treatment of a female patient with a giant aneurysm of the cavernous internal carotid artery complicated by gross ipsilateral bending of the proximal third of the common carotid artery. To ensure an adequate endovascular approach to the aneurysm, open repair of the common carotid artery and elimination of its bending were performed at the first stage. This enabled successful placement of a flow-diverting stent into the internal carotid artery at the aneurysm level at the second stage, resulting in good technical and clinical outcomes. We discuss the technical aspects of performed interventions and the opportunity of using this approach in patients who need endovascular interventions that are complicated by anatomical-morphological changes in the brachiocephalic arteries.Рассматривается клинический случай эндоваскулярного лечения больной с гигантской аневризмой кавернозного сегмента внутренней сонной артерии, осложненный наличием грубой ипсилатеральной деформации проксимальной трети общей сонной артерии. Для обеспечения адекватного эндоваскулярного доступа к аневризме первым этапом была выполнена открытая реконструкция общей сонной артерии с устранением ее изгиба. Это позволило вторым этапом успешно выполнить установку потокоперенаправляющего стента во внутреннюю сонную артерию на уровне аневризмы с хорошим техническим и клиническим результатом. Обсуждаются технические аспекты выполненных вмешательств и возможности использования данного подхода у больных, нуждающихся в эндоваскулярных вмешательствах, проведение которых осложнено анатомо-морфологическими изменениями брахиоцефальных артерий.

Surgical management of pathological deformities of the internal carotid arteries, a cause of chronic brain ischemia, is discussed. This pathology is very common and is found in 25% of all individuals who underwent preventive medical examination according to the ultrasonography data. Most deformities do not pose any threat to patients, while some of them may cause ischemic stroke and chronic brain ischemia. The study included 165 patients with the known follow-up history who had been operated on at the N.N. Burdenko Neurosurgical Institute since 2001. A total of 196 reconstructive interventions of carotid arteries were analyzed. The indications for surgical management of pathological deformities based on clinical symptoms and identification of the signs of vascular wall dysplasia are thoroughly discussed. The local and cerebral hemodynamics during pre- and postoperative period are analyzed. The results of pathomorphological examination of the resected fragments of the deformed arteries are presented; they show that the changes are identical to those in patients with fibromuscular dysplasia. The follow-up history of the patients was recorded; it showed a sustained regression of transitory ischemic strokes and cerebral symptoms in most cases (69%). For proper indications for surgical management, reconstructive surgical interventions are a reliable and effective method for treating chronic brain ischemia and preventing recurrent ischemic strokes in patients with deformities of carotid arteries.

The article provides an example of successful surgical treatment of progressive chronic cerebral ischemia due to occlusion of the brachiocephalic trunk and critical stenosis of the right internal carotid artery (more than 85%). We describe a hybrid method of surgical treatment, which include direct access to the neurovascular bundle, retrograde endovascular recanalization following angioplasty and stenting of the brachiocephalic trunk during temporary occlusion of the distal internal carotid artery and carotid endarterectomy. We provide an analysis of indications for each phase of operation considering the features of compensatory collateral circulation in the basin of the occluded brachiocephalic trunk and the possible complications of surgery.

46 year old man appealed to the Cancer Research Center of RAMS in October 2012 with unverified anterior superior mediastinal tumor, which was diagnosed in 2010. Progressive compartment syndrome of the superior vena cava was observed. On examination: CT, MRI, angiography, histological and cytological examination of biopsy material did not allow to confirm the morphological structure of the tumor. Removal of the tumor with bifurcation of the brachiocephalic trunk prosthetics was performed. Immunohistochemical (IHC) study verified malignant hemangioendothelioma.

The paper contains information about surgical occlusion of the common carotid arteries--one of the causes of chronic cerebrovascular ischemia. For the period from 2003 to the present time have been treated 25 patients with a clinical picture of cerebral ischemia due to occlusion of the common carotid artery. Performed a total of 48 surgical procedures among which subclavian-external carotid and common carotid bypasses, retrograde carotid thromboendarterectomy. In 7 cases revascularization of the brain was performed as a second stage. Choice of reconstruction type based on preoperative investigation results and intraoperative data. No persistent complications or mortality were observed. Positive clinical dynamics was noted in 80% of cases. Crossability of reconstruction area at the annual catamnesis was 84%. Prosthetic vascular graft thrombosis occurred most often (up to 40%) in patients with type 2 occlusion and in the presence of severe collateral perfusion. In patients with 1st type of occlusion of the common carotid artery reconstructive surgery have shown to be a reliable and effective treatment for chronic cerebral ischemia and prevention of recurrent ischemic stroke.

The article describes a case of successful surgical treatment of an 81-year-old male patient with symptomatic occlusion of the internal carotid artery that led to severe impairment of cerebral hemodynamics and was accompanied by severe gait and equilibrium disorders. Given the patient's age and his positive medical history, he underwent surgery for the creation of an extracranial-intracranial microvascular anastomosis under regional anesthesia, with spontaneous breathing and a baseline level of consciousness being maintained. In the postoperative period, the patient achieved a marked clinical effect in the form of complete regression of syncopal conditions and gait disorders associated with recovery of cerebral perfusion in the right MCA territory. The article discusses the indications for surgical treatment and the technical details of surgery for the creation of an extracranial-intracranial microvascular anastomosis under regional anesthesia.Приводится пример успешного хирургического лечения пациента 81 года с симптоматической окклюзией внутренней сонной артерии, приводившей к выраженным нарушениям церебральной гемодинамики и сопровождавшейся синкопальными состояниями в виде нарушения походки и статики. Учитывая возраст больного и отягощенный соматический анамнез, выполнена операция по созданию экстра-интракраниального микрососудистого анастомоза в условиях регионарной анестезии при сохранении самостоятельного дыхания и исходного уровня сознания. В послеоперационном периоде был получен положительный клинический эффект в виде полного регресса синкопальных состояний и нарушений походки на фоне восстановления церебральной перфузии в бассейне правой средней мозговой артерии. Обсуждаются показания к хирургическому лечению и технические детали операции по созданию экстра-интракраниального микрососудистого анастомоза в условиях регионарной анестезии.

Authors studied cognitive functions in 97 patients with different clinical presentations, lateralization and number of stenotic brachio-cephalic vessels. The Luria's syndrome neuropsychological analysis distinctly revealed the topics of cognitive deficits. Wechsler verbal memory tests, the Stroop test (words-figures), the Schulte test were administered as well. Disturbances of high psychic functions were found in 97.9% of patients. The bilateral pathology of lobe regions was noted most often. The dysfunction of parietal and temporal lobes was mainly unilateral and was linked with the side of stenosing process. Disturbances of neurodynamic parameters of mental activity were correlated with the age (higher frequency in patients older than 50 years), a side of stenosis and/or lesion of stroke, bilateral lesions of carotid arteries. The results allow a more precise assessment of clinical significance of carotid stenosis in the development of cognitive dysfunction that may assist in recommendation of surgical treatment.

Authors conducted a clinical-instrumental examination of 97 patients in the post-operative phase of reconstructive surgery of carotid arteries. Evaluation of neurological status, Luria's syndrome neuropsychological analysis for assessment of higher mental functions dynamics (HMF) were carried out. Pre-operative ultrasound duplex scanning of brachio-cephalic arteries and investigation of cerebral perfusion using spiral computed tomography (SCT-perfusion) were done before and 1 day after the surgery. The frequency of intrasurgery stroke in the area of the operated carotid artery was 2.1%. Pre-operative SCT-perfusion revealed the syndrome of cerebral hyperperfusion in 3.1% of patients with higher (1 s) difference between the average time of blood transition in temporal areas. Dynamics of HMF was the key clinical indicator that allowed to evaluate the course of nearest post-operative phase and to compare it with an area of operated vessel. Changes of HMF were characterized by the decrease in dysfunction of the hemisphere ipsilateral to the side of operation or had differently directed character. The aggravation of HMF disturbances was found in 40% of patients and was represented by syndromes with different topical location. Causes and mechanisms of the development of these changes are discussed.

The aim of this study was to assess the role of different modalities in early diagnosis of cerebral hyperperfusion syndrome (CHPS) following carotid endarterectomy, as well as to develop effective treatment protocol for abovementioned complication.Study group consisted of 61 patient who underwent uni- or bilateral carotid endarterectomy (CEA). Diagnostic modalities included transcranial Doppler study (TCD), cerebral oximetry, perfusion CT scanning and neuropsychological evaluation.We demonstrated that both bilateral carotid stenosis and stroke were independent predictive factors for CHPS at early postoperative period. "Classical" two-fold increase of cerebral blood flow in ipsilateral CMA was seen in 15% patients only. According to our data, predictive TCD-value was 1.48 increase of CBF. The most informative tool for early CHPS diagnosis was perfusion CT scans: preoperative mean transit time asymmetry in temporal areas (in the group of patients with unilateral carotid stenosis) correlated with CHPS development. Effective medical treatment for CHPS was achieved with Ca2+ channel antagonists use, that enabled us to prevent evolution of "mild" clinical appearance of CHPS into status epilepticus or intracerebral hemorrhage.Modern diagnostic modalities that assess brain perfusion can (with certain rate of accuracy) predict CHPS development even preoperatively. Early therapy of this complication with Ca2+ antagonists can usually control it.

Range of methods for diagnostics of extracranial arterial stenosis includes both untrasonic techniques (transcranial Doppler study, duplex sonography, which are not only screening methods, but also used in surgery as well) and high-tech imaging methods--spiral CT scans and MR angiography with visualization of structural changes in vascular wall, quantitative and functional assessment of blood flow in vessels. Aim of this study was to determine the effective of non-invasive diagnostic techniques--MR angiography (MRA), including phase-contrast MRA (PCMRA), and spiral CT angiography (SCTA) in identification and comprehensive assessment of stenosis of internal carotid artery and comparison of capabilities of these methods with gold standard--direct angiography.

The paper contains the protocol of examination and surgical management of patients with stenosing lesions of mail cerebral arteries. Diagnostic algorithm including evaluation of clinical and instrumental data is presented. Indications for ultrasonic (duplex scanning, transcranial Doppler study) and neuroradiological (CT, spiral CT angiography, MRI) methods of assessment of stenosing lesions are described. Indications for digital subtraction angiography as well as CT- and MR-angiographic studies in postoperative period are to be specified. Variants of reconstructive surgical procedures on carotid, vertebral and subclavial arteries, surgical revascularization of the brain and endovascular procedures on brachiocephalic arteries are discussed. Indications and contraindication to different kinds of surgical treatment and selection of anesthesia are listed. Methods of intraoperative multimodal and neurodynamic monitoring and criteria of application of temporary intraluminal shunt on the main stage of carotid reconstructions are described. Tactics of step-by-step surgical management of multiple stenosing lesions of brachiocephalic arteries is presented. The paper also contains recommendations for preoperative preparation and early and delayed postoperative management of patients.

The paper analyzes the efficiency of multimodal neuromonitoring (transcranial ultrasound Doppler study, cerebral oximetry, and electroencephalography) for the early intraoperative diagnosis of cerebral ischemia in patients during carotid reparative interventions. On the basis of the findings, the authors have developed a diagnostic algorithm of cerebral ischemia in this group of patients at surgery. Its basis is Doppler monitoring; the data of cerebral oximetry and EEG are taken into account with the borderline values of linear blood flow velocity. The diagnosis of cerebral ischemia during operation on the carotids is a direct indication for the usage of a temporary intraluminal shunt. During carotid reconstructive interventions made under general anesthesia, the application of multimodal neuromonitoring is a valid approach and it should make the most use.

The article analyzes results of surgical revascularization in patients with symptoms of chronic cerebral ischemia caused by occlusion of the carotid arteries.We analyzed 404 surgeries for placement of extra-intracranial microvascular anastomoses (EICMAs) performed in 376 patients between 2000 and 2015. All patients underwent detailed neurological and neuropsychological examinations before surgery and throughout the follow-up period using the neurological deficit scale (NIHSS). Additionally, the medical history data, technical features of surgery, and results of instrumental tests were recorded. For a more detailed study of the cerebral circulation, a SCT perfusion examination was conducted in 58 patients before and after placement of EICMA.All patients were divided into 3 groups, depending on the surgical treatment outcomes: improvement (53%), without significant changes (43%), and worsening of clinical symptoms (4%). A statistical analysis revealed that the efficacy of EICMA surgery ranged from 22 to 79% and was reliably confirmed by hemodynamic and anamnestic factors as well as by technical details of surgery.When determining the indications for surgical revascularization in patients with ischemic stroke consequences, the patient's age, occlusion duration, location and size of ischemic lesions should be considered. Also, the choice of the acceptor artery and blood flow through the created anastomosis are of great importance.Цель исследования - анализ результатов хирургической реваскуляризации у больных с признаками хронической церебральной ишемии, вызванной окклюзией артерий каротидного бассейна. Материал и методы. Проанализированы 404 операции по созданию экстра-интракраниальных микроанастомозов (ЭИКМА), выполненных 376 пациентам в период с 2000 по 2015 г. Все пациенты проходили детальное неврологическое и нейропсихологическое обследование до операции и на протяжении всего периода катамнестического наблюдения, с использованием шкалы оценки неврологического дефицита (NIHSS). Дополнительно фиксировались анамнестические данные, технические особенности операции, результаты инструментальных методов исследования. С целью более детального изучения мозгового кровообращения до и после создания ЭИКМА у 58 пациентов выполнялись СКТ-перфузионные исследования. Результаты. В зависимости от исходов оперативного лечения все пациенты были разделены на три группы: с улучшением (53%), без существенной динамики (43%) и с ухудшением клинической симптоматики (4%). По данным статистического анализа, выявлено, что эффективность операции ЭИКМА варьировала от 22 до 79% и достоверно зависела от гемодинамических, анамнестических факторов, а также технических особенностей оперативного вмешательства. Заключение. При определении показаний к хирургической реваскуляризации у больных с последствиями ишемического инсульта следует учитывать возраст больного, длительность окклюзии, локализацию и размер очагов ишемии. Результаты операции также зависят от выбора акцепторной артерии и объемного кровотока по созданному анастомозу.

To investigate changes in cerebral perfusion in patients with unilateral internal carotid artery occlusion before and after surgical revascularization of the brain, depending on the clinical efficacy of surgical treatment.The study included 60 patients with unilateral ICA occlusions who underwent placement of an extra-intracranial microvascular anastomosis (EICMA). All patients underwent a CT perfusion study before and after cerebral revascularization. In addition, the degree of neurological deficit was evaluated before surgery and during follow-up (3 and 8-10 months) using the NIHSS score.All patients were divided into 3 groups, depending on the results of surgical treatment: objective improvement (43 patients), no changes (14 patients), and worsening of clinical symptoms (3 patients). In each group, the absolute and relative perfusion parameters (MTT, CBV, and CBF) were analyzed to identify the perfusion criteria for the EICMA efficacy. A significant relationship between the clinical efficacy of EICMA and a baseline perfusion deficit and its change after anastomosis placement was found.Placement of EICMA is effective treatment for patients with symptomatic ICA occlusions and an increase in the blood transit time in the hemisphere ipsilateral to occlusion by more than 40% compared to that in the opposite side provided that perfusion is recovered in more than one area of the MCA territory (in accordance with the ASPECTS scale).Цель исследования - определить критерии эффективности хирургической реваскуляризации головного мозга на основании исследований церебральной перфузии. Материал и методы. В исследование включены 60 пациентов с односторонней окклюзией ВСА, которым выполнено наложение экстра-интракраниального микрососудистого анастомоза (ЭИКМА). Всем больным проводились КТ-перфузионные исследования до и после реваскуляризации головного мозга. Оценивалась степень неврологического дефицита до операции и в течение катамнестического наблюдения (3, 8-10 мес) в соответствии со шкалой неврологического дефицита NIHSS (шкала инсульта Национального института здоровья США). Результаты. В зависимости от исхода хирургического лечения пациенты были разделены на три группы: с объективным улучшением (43 пациента), без динамики (14) и с ухудшением клинической симптоматики (3). The study was conducted at the Academician N. N. Burdenko Institute of Neurosurgery, Moscow, in 1999 to 2006. The paper presents the results of stepwise surgical treatment in 84 patients with chronic cerebral ischemia, caused by multiple stenotic and occlusive lesions of the brachiocephalic arteries, who underwent 183 reparative operations. Six major groups of patients with various combinations of lesions of major cerebral arteries were identified. An attempt was made to create an algorithm of the optimum stepwise surgical treatment policy in the identified groups of patients on the basis of the site, degree, and nature of stenotic lesions, by taking into account the cerebral hemodynamics and collateral circulation at all stages of surgical treatment. At the same time, changes in the degree of neurological deficit as one of the major determinants of the results of surgical treatment could be evaluated. The study has demonstrated that the chosen tactics of employing series arterial reconstructions in multiple stenotic and occlusive lesions of major cerebral arteries is an effective treatment option in patients with chronic cerebral ischemia.

The paper describes a relatively rare clinical case of stepwise surgical treatment in a patient with concomitant cerebrovascular pathology: thrombosis of the left internal carotid artery + critical stenosis of the right internal carotid artery + arteriovenous malformation of the right occipital lobe. The patient underwent open right carotid endarterectomy left extra-intracranial microvascular anastomotic application, and histoacryl endovascular embolization of the major afferents and arteriovenous malformation stroma in the stepwise fashion. Management policy, possible complications, and ways of their prevention in these patients are discussed.

The paper gives a clinical example of embolization of the vascular network of carotid paraganglioma with subsequent en-block tumor removal with bifurcation of the common carotid artery and with recovery of great blood flow along the internal carotid artery through alloprosthesis of the latter. The positive effects of a combination of endovascular embolization of the vascular network of the tumor with its subsequent removal are noted. Indications for endovascular embolization and its procedure, intraoperative evaluation of cerebral collateral circulation are discussed. Those for carotid repair during en-block removal of carotid paraganglioma with bifurcation of the common carotid artery are considered.

The results of surgical treatment of patients with occlusive and stenosing lesions of brachiocephalic arteries implemented at Burdenko's Research Institute of Neurosurgery of the Russian Academy of Medical Sciences (RAMS) during 1999-2002 are described. A total of 164 operations were made in 128 patients. Different diagnostic methods and indications for various types of reconstructive and revascularization surgeries are under discussion. A variety of the carotid endarterectomy techniques and the conditions needed for their implementation (microsurgical equipment and intraoperative multi-modality neuromonitoring) are defined. Finally, the main complications and the methods of their prevention, correction and treatment are also described.

The paper describes a clinical case of traumatic right carotid-jugular anastomosis, a rare abnormality of the great arteries in the neck. It presents data on its clinical and instrumental diagnosis, a procedure for a direct surgical intervention repairing the common carotid artery and the right internal jugular vein, which has yielded a good clinical result.

Theoretical and practical aspects of the complex treatment of brain and spinal vascular diseases using microsurgical, endovascular and radiosurgical methods are considered. Authors present the data demonstrating that, due to the implementation of the program of development of vascular centers in the Russian Federation, considerable progress was made in the treatment of cerebral aneurisms and hemorrhagic stroke. In author's opinion, wide introduction of surgical methods in the treatment of occlusive lesions of the blood vessels supplying the brain is needed.В статье рассматриваются теоретические и практические аспекты комплексного лечения заболеваний сосудистой системы головного и спинного мозга с использованием микрохирургических, эндоваскулярных и радиохирургических методов. Приведены данные, показывающие, что благодаря реализации программы по созданию сосудистых центров в стране, в последние годы достигнуты существенные успехи в лечении мозговых аневризм и геморрагического инсульта. Авторы считают, что необходимо более широкое внедрение хирургических методов лечения при окклюзирующих поражениях сосудов, снабжающих кровью мозг.
