The purpose of this study was to assess the effects of a weekend of swimming competition on various physiological parameters in a group of elite female swimmers. Eight female swimmers (age, 16.6 +/- 0.5 years) participated in this study. Resting blood lactate (Bla) and heart rate (HR) were taken at the beginning of each testing session. Testing involved a discontinuous incremental peak VO2 treadmill test during which on-line, measures of VO2 were obtained. HR and Bla measurements were taken at the end of each exercise increment. A 30-s leg Wingate test (WAnT) was used to measure anaerobic power. Paired t-tests were carried out on all data. Resting HR was significantly higher and submaximal and maximal HR were significantly lower comparing pre- and postcompetition (p < .005). Resting Bla and submaximal VO2 were significantly higher postcompetition (p < .005). The results suggest that swimming competition causes a number of the recognized symptoms related to excitatory (acute) overtraining. [ABSTRACT FROM AUTHOR]

The aims of this study were twofold: 1. to compare the strength of four muscle groups ot the lower limb (quadriceps [Q], hamstrings [H], plantar flexors [PE], and dorsiHexors [DF]) between female adolescent ballet dancers (BALs) and basketball players (BBs) over three angular velocities (30°, 60° and 90°/sec) and cwo types of contraction (eccentric and concentric); and 2. to compare peak and mean power between the BALs and BBs. Eleven BALs (age: 15.8 ± 1.2 years; stature: 163.9 ± 6.2 cm; body mass: 56.3 ± 5.7 kg; Tanner stage 4: N = 10, stage 5: N = 1) and ten BBs (age: 15.8 ± 1.0 years; stature: 172.5 ± 5.8 cm; body mass: 63.2 ± 10.0 kg; Tanner stage 4: N = 7, stage 5: N = 3) volunteered to participate. Each participant performed one habituation session and one testing session on a Blodex isokinetic dynamometer to determine peak torque. Three angular velocities (30°, 60° and 90°/sec) were used for each muscle group. To determine absolute and relative peak and mean power, participants performed a 30 second anaerobic Wingate test on a cycle ergometer. There were no significant differences in peak torque for Q, H, PF, and DF between the groups. Basketball players produced greater absolute peak power [569.7 ± 82.2 vs. 454.6 ± 79.3 W (p < .05)1, relative peak power [9.1 ± 1.3 vs. 8.1 ± 1.0 W/kg body mass (p < .05)] absolute mean power [428.4 ± 53.9 vs. 333.7 ± 68.2 W (p < .05)! and relative mean power [6.83 ± 0.7 vs. 5.9 ± 0.7 W/kg (p < .05)1 than BALs. Ballet dancers and BBs had similar isokinetically measured lower extremity muscular strength, but BALs generated lower levels of peak power and mean power compared to BBs. [ABSTRACT FROM AUTHOR]

There is an increasing tendency for young children to participate in training and competitive running. The impact long-term training has upon stimulating functional physiological adaptation has yet to be fully understood. In this study cardio-respiratory and kinematic differences were assessed at submaximal and maximal exercise intensities in run-trained and non-run-trained boys. Thirty three pre-pubertal boys volunteered to take part in the study. The subjects were in two groups: 15 run-trained subjects [age 11.7+/-1.06 yrs, mean+/-SD] and 18 non-run-trained (control) subjects [age 11.3+/-0.90 yrs]. Two separate (4x3 min) submaximal protocols were used for the trained and non-run-trained groups, with two of the speeds overlapping for comparison purposes. In addition, all boys also performed a maximal oxygen consumption test. Mean VO2max value for the run trained group was 60.5+/-3.3 ml/kg/min and for the control group 51.1+/-4.3 ml/kg/min, (p<0.001). No significant differences were found for submaximal running economy at either comparison speed. In addition, no significant (p>0.05) differences were noted between the groups for any of the kinematic variables at the two comparison speeds. However, selected physiological differences did exist at the submaximal running speeds. The source of the differences that did exist between the two groups may be the result of training, genetic pre-selection or developmental differences between the groups. [ABSTRACT FROM AUTHOR]

The question of how best to identify and develop youth soccer players has received considerable attention from the scientific community. Existing literature has, however, largely focused on male players, with comparatively little exploration of the specific approaches employed within women's soccer. Accordingly, we sought to investigate the key factors deemed important by elite women's soccer clubs concerning the: 1) identification of potential talent; 2) development of players within the player pathway; and 3) selection of players for the next age group or senior team. Data were generated through semi-structured interviews with 11 key representatives from seven elite women's soccer clubs. Clubs were purposefully sampled to include the highest performing teams (38 domestic titles and 10 UEFA Women's Champions League titles) from five European nations (Spain, France, Sweden, Germany, and Italy). Data were analysed using thematic content analysis, resulting in six higher-order themes: 1) prioritising local talent; 2) recruitment from mixed grassroots leagues; 3) creating challenging developmental environments; 4) ensuring player wellbeing; 5) patience in decision-making; and 6) facilitating the youth-to-senior transition through a top-down approach. A total of 17 lower-order themes were subsequently identified. The present study offers novel insights of key strategies deemed important by some of the most successful women's clubs in top-performing European nations. Future research examining the efficacy of such approaches could help inform the development of evidence-based practices for nurturing the next generation of elite female players.

This study examined the relationships between the decision-making performances of soccer referees and markers of physiological load. Following baseline measurements and habituation procedures, 13 national-level male referees completed a novel Soccer Referee Simulation whilst simultaneously adjudicating on a series of video-based decision-making clips. The correctness of each decision was assessed in relation to the mean heart rate (HR), respiratory rate (RR), minute ventilation (VE), perceptions of breathlessness (RPE-B) and local muscular (RPE-M) exertion and running speeds recorded in the 10-s and 60-s preceding decisions. There was a significant association between decision-making accuracy and the mean HR (p = 0.042; V C  = 0.272) and RR (p = 0.024, V C  = 0.239) in the 10-s preceding decisions, with significantly more errors observed when HR ≥ 90% of HR max (OR, 5.39) and RR ≥ 80% of RR peak (OR, 3.34). Decision-making accuracy was also significantly associated with the mean running speeds performed in the 10-s (p = 0.003; V C  = 0.320) and 60-s (p = 0.016; V C  = 0.253) preceding decisions, with workloads of ≥250 m·min -1 associated with an increased occurrence of decisional errors (OR, 3.84). Finally, there was a significant association between decision-making accuracy and RPE-B (p = 0.021; V C  = 0.287), with a disproportionate number of errors occurring when RPE-B was rated as "very strong" to "maximal" (OR, 7.19). Collectively, the current data offer novel insights into the detrimental effects that high workloads may have upon the decision-making performances of soccer referees. Such information may be useful in designing combined physical and decision-making training programmes that prepare soccer referees for the periods of match play that prove most problematic to their decision-making., (© 2024 The Authors. Introduction: This study evaluates feasibility of telemedicine to deliver diabetic care among different regions of the country., Materials and Methods: Medical interns affiliated with Rotaract Club of Medicrew (RCM) organized a Free Diabetes Screening Camp called " Diab-at-ease" at multiple sites across the country. Of all beneficiaries of the camp >18 years of age, patients previously diagnosed with diabetes and undiagnosed patients with a random blood sugar level of more than 200 mg/dL were interviewed regarding their knowledge, attitude, and practice regarding diabetes care and preparedness and vigilance to receiving care through telemedicine. Random blood sugar, height, weight, and waist circumference were also documented., Results: About 51.1% ( N = 223) of female patients aged 57.57 ± 13.84 years (>18 years) with body mass index (BMI) =26.11 ± 4.63 were the beneficiaries of the health camps. About 75.3% ( n = 168) of them were on oral hypoglycemic agents (OHAs), 15.7% ( n = 35) were on insulin preparations, and 59.6% ( n = 156) and 88.5% ( n = 31) of which were highly compliant with treatment, respectively. About 35% ( n = 78) and 43.9% ( n = 98) of them were unaware of their frequency of hypoglycemic and hyperglycemic episodes, respectively. About 64.6% ( n = 144) of the patients were equipped for receiving teleconsultation. Glucometer was only possessed by 51.6% (115) of which only 46.95% ( n = 54) can operate it independently. Only 80 patients (35.9%) were aware of the correct value of blood glucose levels., Conclusion: While a majority of the population is compliant with treatment and aware about diabetes self-care, they lack adequate knowledge and resource equipment for the same leading to very limited utilization., Competing Interests: There are no conflicts of interest., (Copyright: © 2024 Journal of Family Medicine and Primary Care.)

Aims: Cardiac adaptations in elite, male adolescent youth soccer players have been demonstrated in relation to training status. The time course of these adaptations and the delineation of the influence of volatile growth phases from the training effect on these adaptations remain unclear. Consequently, the aims of the study were to evaluate the impact of 3 years of elite-level soccer training on changes in left ventricular (LV) structure and function in a group of highly trained elite youth male soccer players (SP) as they transitioned through the pre-to-adolescent phase of their growth., Methods: Twenty-two male youth SP from the highest Level of English Premier League Academy U-12 teams were evaluated once a year for three soccer seasons as the players progressed from the U-12 to U-14 teams. Fifteen recreationally active control participants (CON) were also evaluated over the same 3-year period. Two-dimensional transthoracic echocardiography was used to quantify LV structure and function., Results: After adjusting for the influence of growth and maturation, training-induced increases in Years 2 and 3 were noted for: LV end diastolic volume (LVEDV; p = 0.02) and LV end systolic volume (LVESV; p = 0.02) in the SP compared to CON. Training-induced decrements were noted for LV ejection fraction (LVEF; p = 0.006) and TDI-S' (p < 0.001)., Conclusions: An increase in training volume (Years 2 and 3) were aligned with LV volumetric adaptations and decrements in systolic function in the SP that were independent from the influence of rapid somatic growth. Background: Pregnancy is associated with a number of physical, emotional, and biological changes that can exacerbate maternal psychological disturbances, such as body image concerns and depression. Sleep disturbances during pregnancy can also have adverse impacts. This study aimed to determine the prevalence of depression, sleep disturbances, and body image concerns among pregnant women. The study also examined the relationship between these factors and pregnancy-related variables, such as bad obstetric history and whether the pregnancies were unplanned., Methods: A cross-sectional study of 146 pregnant patients was conducted at a tertiary care center over 15 months. The patients were administered the Beck Depression Inventory, Pittsburgh Sleep Quality Index, and Body Image Concern Inventory questionnaires. Contingency tables, Fisher exact test, and Spearman correlation were used to identify underlying relationships., Results: The prevalence of depression was 22.6%. Although body image disturbance was noted in only 2.7% of patients, 46.6% had poor sleep quality. Poor sleep was associated with primigravida status. Bad obstetric history and unplanned pregnancy were associated with depression. Depression was found to be significantly correlated with body image disturbances and poor sleep quality., Conclusion: Psychiatric disorders were prevalent during pregnancy. This study highlights the importance of screening for depression in pregnant patients. Counselling and caregiver education can be useful for mitigating psychological disturbances. Management of pregnancies by multidisciplinary teams that include psychiatrists could be immensely useful in improving the pregnancy experiences of patients.

Purpose: This study examined the interday reliability of a predetermined and a self-determined isometric-squat test among youth soccer players. Familiarization effects were evaluated to determine the minimum number of trials necessary to obtain consistent outputs. Finally, differences between protocols were evaluated., Methods: Thirty-one youth soccer players (mean [SD] age: 13.2 [1.0] y; body mass: 54.1 [3.4] kg; stature: 166.3 [11.2] cm; percentage of estimated adult height: 92.6% [3.6%]) from a top-tier professional academy completed 4 experimental sessions for each protocol: familiarization 1, familiarization 2, test, and retest sessions. Peak force; relative peak force; impulse from 0 to 50 milliseconds, 0 to 100 milliseconds, 0 to 150 milliseconds, and 0 to 200 milliseconds; and rate of force development from 0 to 50 milliseconds, 0 to 100 milliseconds, 0 to 150 milliseconds, and 0 to 200 milliseconds were measured., Results: Both protocols displayed acceptable (intraclass correlation coefficient ≥.75 and coefficient of variation ≤10%) reliability statistics for all metrics apart from rate of force development of any time epoch. Differences were found between familiarization 2 and both test and retest sessions for peak force (P = .034 and .021, respectively) and relative peak force (P = .035 and .005, respectively) across both protocols., Conclusions: The isometric-squat test is a reliable test among youth soccer players. Two familiarization sessions seem to be sufficient to ensure data stabilization. Outputs between the self-determined and predetermined are comparable; however, the latter seems preferable due to improved testing time efficiency.

Purpose: To gain knowledge on the beliefs and practices of football practitioners applying high-speed and sprint running exposure programming strategies., Methods: One hundred two football practitioners from 22 different countries participated in a study consisting of a survey including 5 domains: demographic and professional characteristics (Who); importance of high-speed and sprint running exposure for physical-capability development, preparation for competition, and injury-prevention strategies (Why); exposure timing (When); methodological procedures for exposure monitoring and training scheduling (What); and effectiveness of common training practices (How). Data were analyzed using a combination of descriptive statistics, generalized mixed effects, and multinomial logistic regression models., Results: Data revealed 5 main findings: (1) overall agreement on the importance of exposure for physical-capability development, preparation for competition, and injury-prevention strategies; (2) different exposure timing and selective training scheduling for starting and nonstarting players across typical and congested weeks; (3) lack of consensus on the conceptual constructs defining high-speed and sprint running metrics and the methodological procedures used for monitoring; (4) a probable association between match-related outcomes and exposure strategies used in training; and (5) a broad range of training methods considered effective to elicit exposure., Conclusions: This study provides actionable insights into the planning, implementing, and monitoring strategies for high-speed and sprint running exposure in football. While some conform with the evidence on high-speed and sprint running training in football, further research and professional debate are warranted to develop empirical knowledge and provide pragmatic recommendations to help practitioners in adopting evidence-informed decisions.

Evidence suggests a limited contribution to the total research output in leading obstetrics and gynaecology journals by researchers from the developing world. Editorial bias, quality of scientific research produced and language barriers have been attributed as possible causes for this phenomenon. The aim of this study was to understand the prevalence of editorial board members based out of low and lower-middle income countries in leading journals in the field of obstetrics and gynaecology. The top 21 journals in the field of obstetrics and gynaecology were selected based on their impact factor, SCImago ranking and literature search. The composition of the editorial boards of these journals was studied based on World Bank Income Criteria to understand the representation status of researchers from low and lower-middle income countries. A total of 1315 board members make up the editorial composition of leading obstetrics and gynaecology journals. The majority of these editors belong to high-income countries ( n  = 1148; 87.3%). Low ( n  = 6; 0.45%) and lower-middle income ( n  = 55; 4.18%) countries make up for a very minuscule proportion of editorial board members. Only a meagre 9 out of 21 journals have editorial board members from these countries (42.85%). Low and low-middle countries have poor representation in the editorial boards of leading obstetrics and gynaecology journals. Background: The SARS-CoV-2 agent initiated a global pandemic. The initial response to the pandemic was severe disruption to the public and private sector including sports. The resultant was that soccer clubs had to prescribe that the players trained in isolation for a prolonged period of time in an attempt to maintain fitness. The aim of the present study was to evaluate the impact of a 10-week period of training in isolation on aerobic fitness, body composition and injury incidence on the return to preseason team-training in a group of elite, male soccer players., Methods: Twenty-two professional soccer players (age: 25.2±4.4 years) who played for an English Championship first team participated in this study. A weekly training program was sent to each player at the start of each week. Prior to the start of the isolated training period, all players underwent a maximal aerobic speed test (MAS), and Body Mass Index data (BMI) were obtained. These measurements were repeated on the return to team training., Results: There was a significant (P<0.05) increase in MAS pre-post isolated training (pre: 4.71±0.15 vs. post: 4.92±0.17 m/s), no change in BMI (pre: 24.3±1.3 vs. post: 24.1±1.1 kg/m 2 ) and a low non-contact, soft-tissue injury incidence on the return to team training., Conclusions: The evidence from this study suggests that a more prolonged preseason schedule can enhance aerobic conditioning and mitigate the injury risk on the return to competitive match-play in elite soccer players.

There is an increase in the prevalence of elite youth sports academies, whose sole aim is to develop future elite athletes. This involves the exposure of the child and adolescent athlete to high-volume training during a period of volatile growth. The large amount of data in this area has been garnered from the resting echocardiographic left ventricular (LV) evaluation of the youth athlete; while this can provide some insight on the functional adaptations to training, it is unable to elucidate a comprehensive overview of the function of the youth athletes' LV during exercise. Consequently, there is a need to interrogate the LV responses in-exercise. This review outlines the feasibility and functional insight of capturing global indices of LV function (Stroke Index-SVIndex and Cardiac Index-QIndex), systolic and diastolic markers, and cardiac strain during submaximal and maximal exercise. Larger SVI and QI were noted in these highly trained young athletes compared to recreationally active peers during submaximal and maximal exercise. The mechanistic insights suggest that there are minimal functional systolic adaptions during exercise compared to their recreationally active peers. Diastolic function was superior during exercise in these young athletes, and this appears to be underpinned by enhanced determinants of pre-load.

It is unclear what the effect of long-term, high-volume soccer training has on left ventricular (LV) function during exercise in youth soccer players. This study evaluated changes in LV function during submaximal exercise in a group of highly trained male soccer players (SP) as they transitioned over a three-year period from pre-adolescent to adolescent athletes. Data were compared to age- and sex-matched recreationally active controls (CON) over the same time period. Twenty-two SP from two professional English Premier League youth soccer academies (age: 12.0 ± 0.3 years at start of the study) and 15 CON (age: 11.7 ± 0.3 years) were recruited. Two-dimensional echocardiography was used to quantify LV function during exercise at the same submaximal metabolic load (approx. 45%VO 2peak ) across the 3 years. After controlling for growth and maturation, there were training-induced changes and superiority (p < 0.001) in cardiac index (QIndex) from year 1 in the SP compared to CON. SP (year 1: 6.13 ± 0.76; year 2: 6.94 ± 1.31; and year 3: 7.20 ± 1.81 L/min/m 2 ) compared to CON (year 1: 5.15 ± 1.12; year 2: 4.67 ± 1.04; and year 3: 5.49 ± 1.06 L/min/m 2 ). Similar training-induced increases were noted for mitral inflow velocity (E): SP (year 1: 129 ± 12; year 2: 143 ± 16; and year 3: 135 ± 18 cm/s) compared to CON (year 1: 113 ± 10; year 2: 111 ± 12; and year 3: 121 ± 9 cm/s). Background: Patients of thalassemia major require frequent hospitalization. Caregivers are more affected than the patient themselves as they better appreciate the magnitude of illness and treatment, resulting in increased risks for psychiatric illness., Aims and Objectives: The purpose of the study was to assess the prevalence of psychiatric morbidity in the caregivers of patients with thalassemia major. The study also examined the quality of life of the caregivers, their coping strategies, and its association with sociodemographic variables., Methodology: A cross-sectional study with 100 caregivers, recruited by convenience sampling technique, attending the thalassemia daycare center, was carried out over 12 months in a tertiary care hospital. They were administered a semistructured proforma along with General Health Questionnaire 12 (GHQ 12), WHO-Quality of Life-BREF (WHO-QOL-BREF), and Coping Inventory for Stressful Situations 21 scale. The GHQ 12 was used for screening and those scoring three or more underwent a clinical psychiatric interview. Those who were diagnosed with psychopathology were ascribed diagnosis as per ICD-10. Descriptive analysis was done. Associations were studied using Fischer's exact test. Comparison of quality of life with blood transfusion variables was done using Mann-Whitney U test., Results: The prevalence of psychiatric morbidity amongst the caregivers was found to be 35% with depressive episode (22%) being the most common. Psychiatric morbidity was found to have a significant association with both, the frequency (P = 0.037) and total number of blood transfusions (P = 0.012). Coping was found to have a strong association with psychiatric morbidity (P = 0.001) and employment (P = 0.009)., Conclusions: Caregivers of children with thalassemia major face psychological burden like depression or anxiety, for which treatment is not sought. Improved psychological health of the caregivers will ensure better care of the child and guarantee better adherence to the treatment.

We quantified on pitch external loading of English Premier League (EPL) academy soccer players (n=76; U12-U18 age groups) over an entire competitive season. Mean accumulative weekly duration and total distance, respectively, was similar in the U12 (329±29 min; 19.9±2.2 km), U13 (323±29 min; 20.0±2.0 km) and U14 (339±25 min; 21.7±2.0 km; P>0.05 for all comparisons) age-groups, though all teams were less than U15 (421±15 min; 26.2±2.1 km), U16 (427±20 min; 25.9±2.5 km) and U18 (398±30 min; 26.1±2.6 km) players (P<0.05 for all comparisons). Mean weekly high-speed running and sprint distance was not different between U12 (220±95 m and 6±9 m respectively), U13 (331±212 m and 6±27 m) and U14 (448±193 m and 21±29 m) age-groups (P>0.05 for all pairwise comparisons) though all squads were less than U15 (657±242 m and 49±98 m), U16 (749±152 m and 95±55 m) and U18 (979±254 m and 123±56 m) age-groups (P<0.05 for all pairwise comparisons). Data demonstrate that absolute weekly training volume in EPL academy soccer players increases throughout the academy pathway. Furthermore, although U16-U18 players are capable of achieving similar training and match volumes as previously reported in adult EPL players, they do not yet achieve the absolute intensities of adult EPL players.

Female soccer has seen a substantial rise in participation, as well as increased financial support from governing bodies over the last decade. Thus, there is an onus on researchers and medical departments to develop a better understanding of the physical characteristics and demands, and the health and performance needs of female soccer players. In this review, we discuss the current research, as well as the knowledge gaps, of six major topics: physical demands, talent identification, body composition, injury risk and prevention, health and nutrition. Data on female talent identification are scarce, and future studies need to elucidate the influence of relative age and maturation selection across age groups. Regarding the physical demands, more research is needed on the pattern of high-intensity sprinting during matches and the contribution of soccer-specific movements. Injuries are not uncommon in female soccer players, but targeting intrinsically modifiable factors with injury prevention programmes can reduce injury rates. The anthropometric and physical characteristics of female players are heterogeneous and setting specific targets should be discouraged in youth and sub-elite players. Menstrual cycle phase may influence performance and injury risk; however, there are few studies in soccer players. Nutrition plays a critical role in health and performance and ensuring adequate energy intake remains a priority. Despite recent progress, there is considerably less research in female than male soccer players. Many gaps in our understanding of how best to develop and manage the health and performance of female soccer players remain.

Cardiac modifications to training are a product of the genetic pre-disposition for adaptation and the repetitive haemodynamic loads that are placed on the myocardium. Elite pre-adolescent athletes are exposed to high-intensity training at a young age with little understanding of the physiological and clinical consequences. It is unclear how right ventricular (RV) structure and function may respond to this type of stimulus. The aim of this study was to compare RV structure and strain across the cardiac cycle and within individual segments in elite soccer players (SP) and controls (CON)., Methods: Twenty-two highly trained, male pre-adolescent SP and 22 age-and sex-matched recreationally active individuals CON were investigated using 2D echocardiography, including myocardial speckle tracking to assess basal, mid-wall, apical and global longitudinal strain and strain rate during systole (SRS) and diastole (SRE and SRA)., Results: greater RV cavity size was identified in the SP compared to CON (RVD 1 SP: 32.3 ± 3.1 vs. CON: 29.6 ± 2.8 (mm/m 2 ) 0.5 ; p = 0.005). No inter-group differences were noted for peak global RV strain (SP: - 28.6 ± 4.9 vs CON: - 30.3 ± 4.0%, p = 0.11). Lower mid-wall strain was demonstrated in the SP compared to CON (SP: - 27.9 ± 5.8 vs. CON: - 32.2 ± 4.4%, p = 0.007)., Conclusion: Soccer training has the potential to increase RV size in pre-adolescent players. The unique segmental analyses used in this study have identified inter-group differences that were masked by global strain evaluations. The clinical and physiological implications of these findings warrant further investigation.

Isokinetic strength screening is utilized in professional soccer. However, there has been little research on the interaction between seasonal changes in players' peak torque (PT) and injury incidence. Twenty-five (age 16.5 ± 0.68 years) professional youth soccer players participated in the study. Bilateral isokinetic concentric (CON) and eccentric (ECC) testing of Quadriceps (Q) and Hamstrings (H) were conducted at three time-points across the season. Absolute CON and ECC PT were measured at 60 degree/sec and in a supine 170-degree position. Testing data was normalized to body mass. A mixed design (2 by 3) repeated measures ANOVA with injury as a co-variate was conducted to evaluate the effect of season and/or limb dominance on PT and injury incidence. With regard to the seasonal variation and injury incidence, an interaction was identified with respect to non-dominant limb (NDL) QCON (p = 0.01) and to a lesser extent the dominant limb (DL) QCON (p = 0.05). The seasonal variation of the PT of the NDL QCON was different between the injured and non-injured individuals. Non-injured individuals, QCON strength increased over the course of the season. While for the Injured players, QCON declined from pre-season to mid-season then increased but never recovered to starting pre-season values.

Considering the perceived benefit of early recruitment and the time and resources spent developing youth players, individuals released from talent development programmes are often re-recruited by rival academies. However, due to the contractual nature of many talent development programmes, limited empirical data exists on players deselected from (or reselected to) youth soccer academies. Adopting a novel case study approach, differences in skill, psychological, and physical attributes associated with reselection following closure of a junior-elite soccer academy were explored. Overall subjective coach ratings for skill, psychological, and physical abilities; subjective coach ratings for skill and psychological attributes; and physical fitness test performance of 79 junior-elite soccer players (U11-U17) were assessed as part of regular scheduled testing and monitoring practices prior to the academy closure. Reselection status was monitored and recorded for all players in the 6 months following the academy closure and was classified as a persistence/progression ("Reselected") or attrition ("Deselected") in playing level. Of the 79 released players, a total of 60 players (76%) were re-signed to a junior-elite academy within 6 months. Differences were observed for overall ratings of skill, psychological, and physical abilities in favor of the "Reselected" player group. "Reselected" players were also rated higher by coaches for all attributes categorized as skill and psychological, as well as performing better at all physical fitness tests. However, "Reselected" players were lesser in stature and body mass and less mature than "Deselected" players. Our findings suggest that reselection is not a product of anthropometric criteria and, therefore, a pathway for selection remains open for later maturing players. Significant structural, developmental, and financial constraints exist in Scottish soccer that may predicate a different approach to talent identification and development. To our knowledge, no published reports exist evaluating the prevalence of the relative age effect (RAE) in Scottish soccer players. Consequently, the aim of this study was to investigate the prevalence of the RAE among varied playing levels and ages of male Scottish youth soccer players. Birthdates of male youth players ( n = 1,230) from U10 to U17 age groups and from playing levels: "Amateur" ( n = 482), "Development" ( n = 214), and "Performance" ( n = 534), alongside a group of male Scottish senior professional players ( n = 261) were recorded and categorized into quartiles (Q1 = January-March; Q2 = April-June; Q3 = July-September; and Q4 = October-December) and semesters (S1 = January-June and S2 = July-December) from the start of the selection year. Birthdates were analyzed for: (a) each playing level and (b) each age group irrespective of playing level. For the varied playing levels examined, an RAE was evident in "Development" and "Performance" playing levels only at youth level. When examining each age group, an RAE was observed in U12-U17 players only. While there was a slight asymmetry favoring Q1 born senior professional players, the RAE was not present within this group of our sample. Results from our study suggest that a bias in selecting individuals born earlier in the selection year may exist within male soccer academy structures, but not at amateur level. Introduction: Burns is one of the foremost causes of worldwide morbidity. Changes in appearance and functional impairment causes stigmatisation, impacting socio-occupational engagement and causing discomfort., Aims and Objectives: The study examined prevalence of psychiatric morbidity in post-burn patients, their perceived social support and self-esteem. It further assessed quality of life amongst post-burn patients., Materials and Methods: A cross-sectional study was performed in a tertiary care centre where 100 patients were interviewed using purposive sampling technique over six months. Patients were administered a semi-structured questionnaire along with Mini International Neuropsychiatric Interview (MINI), Rosenberg's Self-Esteem Scale (RSES), Multidimensional Scale of Perceived Social Support (MSPSS) and WHO Quality Of Life - BREF (WHOQOL- BREF) scales. Contingency tables and Spearman's correlation helped to examine associations and correlations. Fischer's exact test, Mann Whitney test and ANOVA test were also used for statistical analysis., Results and Discussion: There is high prevalence of psychiatric morbidity among patients with burns. The most common disorder was major depressive episode. Low self-esteem was found in one-third of patients with burns. Patients having low self-esteem had eleven times higher prevalence of suicidality. Patients with burns had high perceived social support. Quality of life in patients with burns depends on self-esteem, perceived social support and presence of psychiatric illnesses., Conclusion: This study illustrates the need for thorough evaluation and screening in patients with burns for psychopathology and self-esteem issues by primary physicians. Behaviour therapy, supportive psychotherapy, counselling and adequate socio-occupational rehabilitation of the patient should be done. Purpose: This study aimed to inform the energy requirements of highly trained adolescent soccer players. Total energy expenditure (TEE) was quantified in academy soccer players from the English Premier League (EPL)., Methods: Twenty-four male adolescent soccer players from an EPL academy (under (U) 12/13 (U12/13), n = 8; U15, n = 8; U18, n = 8) were assessed for baseline maturity (maturity offset), body composition (dual-energy x-ray absorptiometry), and resting metabolic rate (indirect calorimetry). Subsequently, TEE, energy intake (EI) and physical loading patterns were assessed over a 14-d in-season period using doubly labeled water, the remote food photographic method, and global positioning system technology, respectively., Results: U18 players presented with greater resting metabolic rate (2236 ± 93 kcal·d) and TEE (3586 ± 487 kcal·d; range, 2542-5172 kcal·d) than did both U15 (2023 ± 162 and 3029 ± 262 kcal·d, respectively; TEE range, 2738-3726 kcal·d) and U12/13 players (1892 ± 211 and 2859 ± 265 kcal·d, respectively; TEE range, 2275-3903 kcal·d; all, P < 0.01), although no difference in TEE was apparent between the U12/13 and U15 age groups. Fat-free mass was significantly different between all comparisons in a hierarchal manner (U18: 57.2 ± 6.1 kg > U15: 42.9 ± 5.8 kg > U12/13: 31.1 ± 3.5 kg; all, P < 0.01). Within age groups, no differences were apparent between EI and TEE (U12/13: -29 ± 277 kcal·d, P = 0.78; U15: -134 ± 327 kcal·d, P = 0.28; U18: -243 ± 724 kcal·d, P = 0.37), whereas U18 players (3180 ± 279 kcal·d) reported higher EI than did both U15 (2821 ± 338 kcal·d; P = 0.05) and U12/13 players (2659 ± 187 kcal·d; P < 0.01)., Conclusions: The TEE of male academy soccer players progressively increase as players progress through the academy age groups. In some individuals (evident in all age groups), TEE was greater than that previously observed in adult EPL soccer players.

Aims: This study aimed to evaluate left ventricular structure, function and mechanics, in highly-trained, pre-adolescent soccer players compared with age- and sex-matched controls., Design: The study design was a prospective, cross-sectional comparison of left ventricular structure, function and mechanics., Methods: Twenty-two male soccer players from two professional youth soccer academies (age: 12.0 ± 0.3 years) and 22 recreationally active controls (age: 11.7 ± 0.3 years) were recruited. Two-dimensional conventional and speckle tracking echocardiography were used to quantify left ventricular structure, function and peak/temporal values for left ventricular strain and twist, respectively., Results: End-diastolic volume index was larger in soccer players (51 ± 8 mm/(m 2 ) 1.5 vs . 45 ± 6 mm/(m 2 ) 1.5 ; p  = 0.007) and concentricity was lower in soccer players (4.3 ± 0.7 g/(mL) 0.667 vs . 4.9 ± 1.0 g/(mL) 0.667 ; p  = 0.017), without differences in mean wall thickness between groups (6.0 ± 0.4 mm vs . 6.1 ± 0.5 mm; p  = 0.754). Peak circumferential strain at the base (-22.2% ± 2.5% vs . -20.5% ± 2.5%; p  = 0.029) and papillary muscle levels (-20.1% ± 1.5% vs . -18.3% ± 2.5%; p  = 0.007) were greater in soccer players. Peak left ventricular twist was larger in soccer players (16.92° ± 7.55° vs . 12.34° ± 4.99°; p  = 0.035) and longitudinal early diastolic strain rate was greater in soccer players (2.22 ± 0.40 s -1 vs . 2.02 ± 0.46 s -1 ; p  = 0.025)., Conclusions: Highly-trained soccer players demonstrated augmented cardiac mechanics with greater circumferential strains, twist and faster diastolic lengthening in the absence of differences in wall thickness between soccer players and controls.

For the first time we aimed to: (1) assess fat-free mass (FFM) and RMR in youth soccer players, (2) compare measured RMR to estimated RMR using previously published prediction equations, and (3) develop a novel population-specific prediction equation. In a cross-sectional design, 99 males from a Premier League academy underwent assessments of body composition (DXA) and RMR (indirect-calorimetry). Measured RMR was compared to estimated values from five prediction equations. A novel RMR prediction equation was developed using stepwise multiple regression. FFM increased (P<0.05) between U12 (31.6±4.2 kg) and U16 (56.3±5.3 kg) after which no further increases occurred (P>0.05). RMR in the U12s (1655±195 kcal.day -1 ), U13s (1720±205 kcal.day -1 ) and U14s (1846±218kcal.day -1 ) was significantly lower than the U15s (1957±128 kcal.day -1 ), U16s (2042±155 kcal.day -1 ), U18s (1875±180 kcal.day -1 ) and U23s (1941±197 kcal.day -1 ) squads (P>0.05). FFM was the single best predictor of RMR (r 2 =0.43; P<0.01) and was subsequently included in the novel prediction equation: RMR (kcal.day -1 ) = 1315 + (11.1 x FFM in kg). Both FFM and RMR increase from 12-16 years old, thus highlighting the requirement to adjust daily energy intake to support growth and maturation. The novel prediction RMR equation developed may help to inform daily energy requirements.

It is unclear, what the underlying cardiovascular mechanisms are that give rise to the high level of aerobic fitness seen in youth soccer players. The aim of the study was to evaluate global and regional markers of systolic and diastolic function in a group of pre-adolescent soccer players during an incremental exercise test. Twenty-two, male soccer players (SP) from two professional soccer clubs (age: 12.0 ± 0.3 years) volunteered for the study. Fifteen recreationally active boys (CON), of similar age (age: 11.7 ± 0.2 years) were also recruited. All boys underwent a cycle ergometer test to exhaustion. Cardiac dimensions were determined using M-mode echocardiography. During submaximal and maximal exercise, continuous-wave Doppler ultrasound techniques were used to derive stroke volume (SVIndex). Tissue-Doppler imaging was used to quantify systolic (S'adj) and diastolic function (E; E'adj and E/E') at rest and both submaximal and maximal exercise intensities. Speckle tracking echocardiography was used to determine peak longitudinal ε at submaximal exercise intensities. SP demonstrated significantly (P ≤ 0.05) greater peak VO 2 values than CON (SP: 48.0 ± 5.0 vs CON: 40.1 ± 7.5 mL/kg/min). Allometrically scaled to body surface area left ventricular end-diastolic volume (LVEDV) was larger (P ≤ 0.05) in the SP (51.3 ± 9.0) compared to CON (44.6 ± 5.8 mL·BSA 1.5 ). At the same relative, submaximal exercise intensities, the SP demonstrated greater SVIndex, cardiac output (QIndex), and E. No differences were noted for peak longitudinal ε during submaximal exercise. Little evidence exists with regard to changes in cardiac strain that occur during submaximal exercise in young males. The aims of the study were to evaluate the changes that occur in longitudinal (L), radial (R), and endocardial circumferential (EC) strain during submaximal upright cycle ergometry and to examine the test-retest reproducibility of these measurements. Fourteen recreationally active, adolescent (age: 17.9 ± 0.7 years) males volunteered for the study. All subjects underwent an incremental (40 W) submaximal cycle ergometer test. L, R, and EC strain values were obtained using speckle tracking, from two-dimensional B-mode images of the left ventricle (LV) during rest and the initial stages of submaximal exercise (40 and 80 W). The average of 6 LV segments was used to determine both peak wall deformation (%) and the time to peak deformation (ms). There was a statistically (P < 0.05) significant increase from rest to submaximal exercise for peak deformation for L, R, and EC strain. There was a statistically significant (P < 0.05) decrease from rest to submaximal exercise for time to peak for L and R and EC strain and between submaximal workloads for time to peak for L strain and EC strain. Coefficients of variation demonstrated reproducibility for upright strain and strain rate measurements similar to published supine measurements. This study has demonstrated that changes in left ventricular wall deformation (L, R and EC strain) that occur during the transition from rest to submaximal exercise can be reliably measured and confirm that a healthy LV has a hyperdynamic response to exercise., (© 2014, Wiley Periodicals, Inc.)

The effects of a short, high-intensity bout of exercise on cardiac systolic and diastolic function are not well understood in adolescent athletes. Consequently, the aims of the study were to evaluate global left ventricular (LV) systolic and diastolic function, as well as segmental wall motion responses (cardiac strain), prior to as well as 45 and 225 min following a simulated 5 km cross-country race. Twenty trained, adolescent males (age: 15.2 ± 0.7 years) volunteered for exercise testing. LV fractional shortening and the ratio of early (E) and late (A) peak flow velocities reflected global systolic and diastolic function, respectively. Peak longitudinal mitral annular septal tissue velocities were also determined during systole and diastole. Longitudinal strain (ε) and strain rates were determined across the LV. LV fractional shortening was significantly (P < 0.05) higher at 225 min post-race (37.6 ± 5.8%) compared to pre-race (34.5 ± 4.7%) and 45 min post-race (34.9 ± 5.4 %). This difference was abolished after adjusting for post-race heart rates. There was a significant (P < 0.05) decrease in the E:A ratio at both 45 min (2.04 ± 0.57) and 225 min post-race (2.20 ± 0.66) compared to the pre-race value (2.80 ± 0.68). When these data were adjusted for post-race heart rates, these pre-post-race differences in E:A ratio were abolished. There were no significant alterations in either tissue Doppler velocities or longitudinal ε. The evidence suggests that a 5 km race does not lead to any significant post-exercise attenuation in global or regional LV systolic and diastolic function in trained adolescents.

Unlabelled: Little evidence exists with regard to the effect that exercise training has upon oxygen uptake kinetics in adolescent females., Purpose: The aim of the study was to compare [Formula: see text] and muscle deoxygenation kinetics in a group of trained (Tr) and untrained (Utr) female adolescents., Method: Twelve trained (6.4 ± 0.9 years training, 10.3 ± 1.4 months per year training, 5.2 ± 2.0 h per week) adolescent female soccer players (age 14.6 ± 0.7 years) were compared to a group (n = 8) of recreationally active adolescent girls (age 15.1 ± 0.6 years) of similar maturity status. Subjects underwent two, 6-min exercise transitions at a workload equivalent to 80 % of lactate threshold from a 3-min baseline of 10 W. All subjects had a passive rest period of 1 h between each square-wave transition. Breath-by-breath oxygen uptake and muscle deoxygenation were measured throughout and were modelled via a mono-exponential decay with a delay relative to the start of exercise., Result: Peak [Formula: see text] was significantly (p < 0.05) greater in the Tr compared to the Utr (Tr: 43.2 ± 3.2 mL kg(-1 )min(-1) vs. Utr: 34.6 ± 4.0 mL kg(-1 )min(-1)). The [Formula: see text] time constant was significantly (p < 0.05) faster in the Tr compared to the Utr (Tr: 26.3 ± 6.9 s vs. Utr: 35.1 ± 11.5 s). There was no inter-group difference in the time constant for muscle deoxygenation kinetics (Tr: 8.5 ± 3.0 s vs. Utr: 12.4 ± 8.3 s); a large effect size, however, was demonstrated (-0.804)., Conclusion: Exercise training and/or genetic self-selection results in faster kinetics in trained adolescent females. The faster [Formula: see text] kinetics seen in the trained group may result from enhanced muscle oxygen utilisation.

Background and Aim: Physical frailty has been regarded as a precursor state to disability and dependence on others for daily activities. Frail individuals are considered to be the group of patients that presents the most complex and challenging problems to health care professionals. The purpose of this study was to evaluate whether a 12-week functional circuit training program (FCT) could reduce self-reported fear of falling and improve health status in a group of physically frail community-dwelling older individuals., Methods: Fifty-one individuals (31 F, 20 M), mean age (SD) 84 (2.9) years old, were considered frail and tested at week 0 and randomly assigned into two groups (intervention group, FCT = 26; control group, CG = 25). FCT underwent a structured 12-week FCT program, which focused on a combination of functional balance and lower-body strength-based exercises. CG met once a week for health education meetings. Measures of self-reported fear of falling (Activities-specific Balance Confidence scale) and health status (SF-12) were assessed at week 0, 12 and 36. All longitudinal analyses for continuous variables that were measured at the three time points (week 0, 12 and 36) were performed using linear mixed modeling., Results: FCT participants had greater improvements than those in the CG in fear of falling and self-reported physical function, physical composite score and mental composite score; these improvements were maintained in the week 36 follow-up, with significant group-by-time interactions by the end of the study., Conclusions: These data indicate that a FCT program is effective in improving self-reported measures of fear of falling and health status in a group of physically frail individuals.

Objective: To compare the phase II oxygen uptake time constant (τV'O(2)) and V'O(2) mean response time (V'O(2)MRT) in overweight (OW) and non-OW (NO) children during moderate intensity exercise., Design: Between subjects where participants completed a maximal ramp exercise test on an electromagnetically braked cycle ergometer to determine peak V'O(2) (V'O(2peak)) and gas exchange threshold (GET). Gas exchange was measured breath-by-breath using a mass spectrometer. On subsequent visits, 6 square-wave transitions (2 per day) from 0 W to 90% GET were completed. Individual phase II τV'O(2) and V'O(2)MRTs were estimated from time aligned average V'O(2) traces., Subjects: Eleven OW (11.8±0.4 years) and 12 NO (11.9±0.4 years) children were recruited to the study. The OW group was significantly heavier (62.9±9.7 vs 39.4±5.8 kg, P<0.001), taller (1.58±0.05 vs 1.47±0.07 m, P<0.001) and had a higher body mass index (25.8±3.4 vs 18.3±1.8 kg m(-2), P<0.001)., Results: Both τV'O(2) (30.2±9.6 vs 22.8±7.1 s, P<0.05) and V'O(2)MRT (43.5±10.7 vs 36.3±5.3 s, P<0.05) were significantly slower in OW compared with NO children; absolute V'O(2peak) was higher in the OW compared with NO group (2.23±0.04 vs 1.74±0.04 l min(-1), P<0.05); mass relative V'O(2peak) was lower in OW compared with NO children (35.9±8.3 vs 43.8±6.2 ml kg(-1) min(-1), P<0.05); allometrically scaled V'O(2peak) was similar between OW and NO groups whether relative to body mass(0.67) (139.8±29.1 vs 147.2±23.9 ml kg(-67) min(-1)) or stature(3) (576.0±87.2 vs 544.9±84.9 ml m(-3) min(-1)) (P>0.05); absolute V'O(2) at GET was similar between OW and NO groups (0.94±0.24 vs 0.78±0.27 l min(-1), P>0.05); GET expressed as percentage of V'O(2peak) was similar between the groups (42.0±0.1 vs 44.8±0.1%, P>0.05)., Conclusion: These findings demonstrate impairment in the factors determining V'O(2) kinetics in OW children at a relatively young age. Furthermore, assessment of cardiorespiratory fitness using peak exercise values is likely to be misleading and not useful when designing exercise programmes for OW children.

Previous studies have demonstrated faster pulmonary oxygen uptake (VO2) kinetics in the trained state during the transition to and from moderate-intensity exercise in adults. Whilst a similar effect of training status has previously been observed during the on-transition in adolescents, whether this is also observed during recovery from exercise is presently unknown. The aim of the present study was therefore to examine VO2 kinetics in trained and untrained male adolescents during recovery from moderate-intensity exercise. 15 trained (15 ± 0.8 years, VO2max 54.9 ± 6.4 mL kg(-1) min(-1)) and 8 untrained (15 ± 0.5 years, VO2max 44.0 ± 4.6 mL kg(-1) min(-1)) male adolescents performed two 6-min exercise off-transitions to 10 W from a preceding "baseline" of exercise at a workload equivalent to 80% lactate threshold; VO2 (breath-by-breath) and muscle deoxyhaemoglobin (near-infrared spectroscopy) were measured continuously. The time constant of the fundamental phase of VO2 off-kinetics was not different between trained and untrained (trained 27.8 ± 5.9 s vs. untrained 28.9 ± 7.6 s, P = 0.71). However, the time constant (trained 17.0 ± 7.5 s vs. untrained 32 ± 11 s, P < 0.01) and mean response time (trained 24.2 ± 9.2 s vs. untrained 34 ± 13 s, P = 0.05) of muscle deoxyhaemoglobin off-kinetics was faster in the trained subjects compared to the untrained subjects. VO2 kinetics was unaffected by training status; the faster muscle deoxyhaemoglobin kinetics in the trained subjects thus indicates slower blood flow kinetics during recovery from exercise compared to the untrained subjects.

Background: Inspiratory muscle training (IMT) has been shown to improve inspiratory muscle function, lung volumes (vital capacity [VC] and total lung capacity [TLC]), work capacity, and power output in people who are healthy; however, no data exist that demonstrate the effect of varying intensities of IMT to produce these outcomes., Objectives: The purpose of this study was to evaluate the impact of IMT at varying intensities on inspiratory muscle function, VC, TLC, work capacity, and power output in people who are healthy., Design: This was a randomized controlled trial., Setting: The study was conducted in a clinical laboratory., Participants: Forty people who were healthy (mean age=21.7 years) were randomly assigned to 4 groups of 10 individuals., Interventions: Three of the groups completed an 8-week program of IMT set at 80%, 60%, and 40% of sustained maximum inspiratory effort. Training was performed 3 days per week, with 24 hours separating training sessions. A control group did not participate in any form of training., Measurements: Baseline and posttraining measurements of body composition, VC, TLC, inspiratory muscle function (including maximum inspiratory pressure [MIP] and sustained maximum inspiratory pressure [SMIP]), work capacity (minutes of exercise), and power output were obtained., Results: The participants in the 80%, 60%, and 40% training groups demonstrated significant increases in MIP and SMIP, whereas those in the 80% and 60% training groups had increased work capacity and power output. Only the 80% group improved their VC and TLC. The control group demonstrated no change in any outcome measures., Limitations: This study may have been underpowered to demonstrate improved work capacity and power output in individuals who trained at 40% of sustained maximum inspiratory effort., Conclusion: High-intensity IMT set at 80% of maximal effort resulted in increased MIP and SMIP, lung volumes, work capacity, and power output in individuals who were healthy, whereas IMT at 60% of maximal effort increased work capacity and power output only. Inspiratory muscle training intensities lower than 40% of maximal effort do not translate into quantitative functional outcomes.

The purpose of this study was to evaluate whether a 12-wk functional circuit-training program (FCT) could alter markers of physical frailty in a group of frail community-dwelling adults. Fifty-one individuals (31 women, 20 men), mean age (± SD) 84 (± 2.9) yr, met frailty criteria and were randomly assigned into groups (FCT = 26, control group [CG] = 25). FCT underwent a 12-wk exercise program. CG met once a week for health education meetings. Measures of physical frailty, function, strength, balance, and gait speed were assessed at Weeks 0, 12, and 36. Physical-frailty measures in FCT showed significant (p < .05) improvements relative to those in CG (Barthel Index at Weeks 0 and 36: 73.41 (± 2.35) and 77.0 (± 2.38) for the FCT and 70.79 (± 2.53) and 66.73 (± 2.73) for the CG. These data indicate that an FCT program is effective in improving measures of function and reducing physical frailty among frail older adults.

The purpose of the study was to determine the degree to which scores of a modified version of the 'Timed Get Up and Go' test (TGUG) were associated with other measures of functional performance. Thirty-seven community-dwelling older women (72.3+/-5.5 years) volunteered to participate. Subjects were assessed when performing the modified TGUG test. Correlations between the performance-oriented mobility assessment (POMA), single-leg balance, five chair rises, fast and normal gait speed, knee extension and flexion strength, and the modified TGUG were conducted. Total time to perform the modified TGUG test was significantly correlated with normal and fast gait speed (p<0.05). The Pearson correlation coefficients were -0.841 and -0.748, respectively. The time needed to perform several tasks of the modified TGUG test significantly correlated with five chair rises, and with right knee extensor strength (p<0.05). Points obtained in the assessment questionnaire correlated significantly to points obtained in the POMA scale (p<0.05). The Pearson correlation coefficient was 0.795. Based on the strength of the correlations obtained between components of the modified TGUG and the comparison tests, concurrent, criterion validity of the modified TGUG has been established., (Copyright 2009 Elsevier Ireland Ltd. Whilst endothelial dysfunction is associated with a sedentary lifestyle, enhanced endothelial function has been documented in the skin of trained individuals. The purpose of this study was to investigate whether highly trained adolescent males possess enhanced skin microvascular endothelial function compared to their untrained peers. Seventeen highly and predominantly soccer trained boys (V(O)(2)(peak): 55 +/- 6 mL kg(-1) min(-1)) and nine age- and maturation-matched untrained controls (V(O)(2)(peak): 43 +/- 5 mL kg(-1) min(-1)) aged 13-15 years had skin microvascular endothelial function assessed using laser Doppler flowmetry. Baseline and maximal thermally stimulated skin blood flow (SkBF) responses were higher in forearms of trained subjects compared to untrained participants [baseline SkBF: 11 +/- 4 vs. 9 +/- 3 perfusion units (PU), p < 0.05; SkBF(max): 282 +/- 120 vs. 204 +/- 68 PU, p < 0.05]. Similarly, cutaneous vascular conductance (CVC) during local heating was superior in the forearm skin of trained versus untrained individuals (CVC(max): 3 +/- 1 vs. 2 +/- 1 PU mmHg(-1), p < 0.05). Peak hyperaemia following arterial occlusion and area under the reactive hyperaemia curve were also greater in forearm skin of the trained group (peak hyperaemia: 51 +/- 21 vs. 35 +/- 15 PU, p < 0.05; area under curve: 1596 +/- 739 vs. 962 +/- 796 PUs, p < 0.05). These results suggest that chronic exercise training in adolescents is associated with enhanced microvascular endothelial vasodilation in non-glabrous skin.

Unlabelled: Exercise training results in a speeding of pulmonary oxygen uptake (VO2) kinetics at the onset of exercise in adults; however, only limited research has been conducted with children and adolescents., Purpose: The aim of the present study was to examine VO2 and muscle deoxygenation kinetics in trained and untrained male adolescents., Methods: Sixteen trained (15 +/- 0.8 yr, VO2peak = 54.7 +/- 6.2 mL x kg-1 x min-1, self-assessed Tanner stage range 2-4) and nine untrained (15 +/- 0.6 yr, VO2peak = 43.1 +/- 5.2 mL x kg-1 x min-1, Tanner stage range 2-4) male adolescents performed two 6-min exercise transitions from a 3-min baseline of 10 W to a workload equivalent to 80% lactate threshold separated by a minimum of 1 h of passive rest. Oxygen uptake (breath-by-breath) and muscle deoxygenation (deoxyhemoglobin signal from near-infrared spectroscopy) were measured continuously throughout baseline and exercise transition., Results: The time constant of the fundamental phase of VO2 kinetics was significantly faster in trained versus untrained subjects (trained: 22.3 +/- 7.2 s vs untrained: 29.8 +/- 8.4 s, P = 0.03). In contrast, neither the time constant (trained: 9.7 +/- 2.9 s vs untrained: 10.1 +/- 3.4 s, P = 0.78) nor the mean response time (trained: 17.4 +/- 2.5 s vs untrained: 18.3 +/- 2.3 s, P = 0.39) of muscle deoxygenation kinetics differed with training status., Conclusions: The present data suggest that exercise training results in faster VO2 kinetics in male adolescents, although inherent capabilities cannot be ruled out. Because muscle deoxygenation kinetics were unchanged, it is likely that faster VO2 kinetics were due to adaptations to both the cardiovascular system and the peripheral musculature.
