魚骨穿通による肝膿瘍の1例を経験したので報告する. 症例は53歳の女性で, 右上腹部痛, 発熱を主訴に紹介入院となった. 肝膿瘍 (S6) の診断にて経皮的膿瘍ドレナージを行ったが, 腹部CT, 腹部超音波検査にて線状陰影を認め異物刺入が疑われた. 手術にて魚骨が十二指腸より肝臓へ穿通したことによる肝膿瘍と診断した. 魚骨穿通による肝膿瘍はまれな疾患であるが, 腹部CT, 超音波検査を行い線状石灰化像を確認することが重要であると考えられた.

急性期虚血性小腸炎の手術症例を経験したので報告する.症例は61歳,男性.腹痛,嘔吐を主訴に当科紹介入院となった.腹部CT上,絞扼性イレウスが疑われたため緊急手術を行った.開腹時,絞扼の所見はなかったが,小腸の虚血性変化を認めたため小腸部分切除術を行った.一般的に急性期虚血性小腸炎は保存的加療が行われるが,特徴的所見に乏しいため診断が難しく,特に自験例のような絞扼が疑われる症例に対しては緊急手術の適応であると考えられた.

症例は77歳,男性で肛門痛を主訴に来院.肛門機能は廃絶しており二次口からの便排出を認めた.CT検査で痔瘻病変と広範な石灰化を認めた.生検で悪性所見無く,結核菌検査は陰性.副甲状腺機能の異常は認めなかった.疼痛軽減目的でS状結腸人工肛門を造設した.現在まで痔瘻の瘻管内結石形成の2例が報告されているにすぎず,広範な石灰化をきたした痔瘻の報告はない.

症例は33歳,女性, 21歳の時,他施設でCrohn病と診断され,緩解と再燃を繰り返していた. 31歳時, Crohn病の増悪に伴う下行結腸穿孔にて手術施行.これまで,痔瘻の既往は認めていない.今回,再度腸穿孔を認め,回腸に人工肛門を造設した.入院中に肛門痛が出現し, Crohn病の肛門病変を疑ったが明らかな膿瘍,痔瘻は認めなかった.その後も肛門痛持続するため,肛門狭窄部の生検を行ったところ,粘液癌と診断され直腸切断術を施行した.肉眼的には肛門管主体に5型の腫瘍を認め,組織学的には粘液癌や高分化腺癌が混在していた.発症から13年目に肛門管癌を合併した小腸大腸型Crohn病の1例を報告した.また, Crohn病に合併した大腸癌の本邦報告49例についての集計し検討を加えた.

当センターでこの12年間に経験したpositive selectionのCAPD症例59例中, 死亡15例, 血液透析 (HD) への移行23例の計38例 (64.4%) が, CAPD脱落症例であった.これらの脱落原因の50%がCAPDに関連したもので, 中でもHD移行の主な理由は, 限外濾過不全, 難治性・頻回のCAPD腹膜炎, 硬化性被嚢性腹膜炎 (SEP) であった. また, 開腹手術を要した6例中5例がHDへ移行した. 脱落関連因子として, CAPD施行期間の長さ, CAPD腹膜炎罹患が有意に相関があると考えられた.SEP症例は, 59例中3例, 5.1%であった. 限外濾過不全のためHD移行後, この前段階と考えられる著明な腹水貯留4症例に, 発症早期からのステロイド治療が有効であった.CAPD排液中のamylase値は, 平常時およびCAPD腹膜炎時には, それぞれ, 5.6±2.7, 19.3±22.6mU/mlで, 後者で有意に高値を呈した. 穿孔性腹膜炎2例 (S状結腸, 回腸) と急性膵炎1例に緊急開腹手術を施行したが, 術前のこれらの値はさらに著明な高値を示し, 一般のCAPD腹膜炎との鑑別に極めて有用であった. 回腸穿孔例では, 術後に縫合不全を合併し手術死亡となったが, このような後期の腹膜硬化症を呈する症例においては, 腸管吻合に重大な危険性が存在すると考えられた.脱落症例38例中16例に, カテーテル抜去時や開腹手術時に腹膜のサンプリングが行われ, 病理組織学的検索がなされた. CAPD施行期間が8-18か月の3例では, いずれも腹膜線維症の所見であった. しかし, 60か月以上の13例中12例, 92.3%では, 腹膜機能障害が非可逆性といわれる中期以後の腹膜硬化症の所見 (中期5例, 後期7例) が認められ, 極めて注目すべき結果が得られた. この事実は, かかる長期CAPD症例においては, SEPとの関連から, その中止時期も含めて, 注意深い経過観察が必要であると考えられた.

症例は60歳, 女性. 1986年12月9日慢性糸球体腎炎による慢性腎不全と診断され, 持続腹膜透析 (CAPD) 導入. その直後の1987年2月12日両側手根管症候群にて開放術施行. 以後9年間で腹膜炎を2回発症. 徐々に除水能の低下をきたし, 1995年12月より血液濾過透析に移行. 透析低血圧および, 凝固能異常の合併によると思われる内シャントの閉塞を反復した. 1996年11月より悪心, 嘔吐, 腹部膨満等の腸閉塞症状出現, 保存的療法にて-時軽快. 消化管透視腹部超音波検査, 腹部CT検査で, 腸管の拡張, 腸蠕動の著明な低下が認められ, 硬化性被嚢性腹膜炎と診断された. 同年12月再び腸閉塞出現, 保存的加療施行するも症状遷延. 内シャント再建術中, 突然ショック状態を呈し, 急性心不全状態持続したのち, 重篤な心筋伝導障害で12月16日死亡した. 病理剖検所見では, 化膿性腹膜炎および腹膜の線維性硬化像に加えて, 全身骨・関節, 消化管, 心, 肺, その他諸臓器にアミロイド沈着を認め, 硬化性被嚢性腹膜炎およびアミロイドーシスの合併が判明した. 本例のアミロイドは, β2-microglobulin由来であったが, AA抗体陽性, 過マンガン酸カリ抵抗性で, 従来の文献とは異なっていた.透析に伴う重篤な合併症を重複した, 貴重な剖検例を経験した. 透析療法の進歩で, 慢性腎不全患者の長期生存が可能となった現在, 同様の症例の出現が今後も予想され, 本例を長期透析患者の全身管理の指針について, 貴重な布石としたい.

症例は63歳の男性で, 主訴は下血. 血清CA19-9異常高値 (830U/ml), CT, Gaシンチなどから, 悪性腸間膜腫瘍を疑い開腹した. 回盲部より80cmの部位にMeckel憩室を認め, 同部に腫瘍および腸間膜リンパ節の腫脹を認めたため, リンパ節郭清を含めた回腸切除を行った. 病理組織学的にはMeckel憩室から発生した中分化型腺癌で, 憩室内には迷入組織を認めなかった. 術後血清CA19-9は速やかに低下し, また抗CA19-9抗体を用いた免疫染色で陽性であり, CA19-9産生腫瘍と診断した. Meckel憩室癌は本邦22例目と極めてまれである. 本症は予後不良であるが, 自験例は1年6か月経過した現在も再発の兆候を認めず, CA19-9の上昇も認めていない.

We experienced a case of secondary amyloidosis causing intestinal obstruction. An 81-year-old woman, having an underlying disease of rheumatoid arthritis, was admitted to the hospiatal because of bloody diarrhea, vomiting, abdominal pain and fever elevation. An emergency laparotomy was carried out under a diagnosis of intestinal obstruction due to ischemic enteritis, and extended dilatation and multiple ulcers of the ileum were found. Partial resection of the ileum was performed. Histopathological examination revealed AA amyloidosis. Emergency surgery for amyloidosis of the gastrointestinal tract is rare, with only 18 reported cases in Japan so far. At operation, most patients with amyloidosis already have deposited amyloid proteins to multiple organs, so that their prognoses are very poor.

A case of intussusception caused by a cancer of the sigmoid colon is reported with a review of the literature. An 86-year-old woman was admitted to the hospital because of tenesmus and diarrhea. A tumor was palpated by digital examination. The patient was diagnosed as intussusception due to a sigmoid colon cancer by abdominal computed tomography, colon fiber, pelvic magnetic resonance imaging (MRI), and gastrographin enema. Upon laparotomy the sigmoid colon headed by the tumor invaginated into the rectum by about 10cm in length causing intussusception. The tumor of the sigmoid colon was MP, N0, P0, H0, M(-), and in Stage I. Hartmann operation with D, lymph nodes dissection was performed. Microscopically it was well differentiated adenocarcinoma, ss, n(-), ly1, v1. Adult intussusception is a compartively rare disease. But it matters whether intussusception is associated with cancer or not and diagnostic imaging, especially MRI is useful for its diagnosis. Pelvic MRI can show any cross-section and is very useful for visualizing intussusception caused by a tumor and for estimating invasive depth of the tumor like in our case.

In rectal cancer, lateral lymphatic spread is considered to worsen the prognosis. Therefore, accurate determination of lymph node involvement preoperatively is helpful in assessing the malignant character of the tumor and in determining resectability. The present study was performed to evaluate the accuracy, limitations, and usefulness of endoluminal ultrasonography in preoperatively diagnosing the localization of lymph node metastasis in rectal cancer. Seventy-six patients with rectal cancer underwent preoperative endoscopic ultrasonography. Three of these patients had lateral lymphatic spread and all could be distinguished on endoscopic ultrasonography by the detection of involvement of lymph nodes in the anterior rectal space. These findings suggest that endoscopic ultrasonography plays a potentially useful role in the strategy for dealing with rectal cancer prior to surgery.

1980年から1991年の間に切除された全周性大腸癌は107例であり, このうちA群 (イレウスを伴った症例 : 16例), B群 (軽度の閉塞症状を伴った症例 : 37例), C群 (閉塞症状をまったく有しなかった症例 : 54例) に分類し, A群の臨床病理学的特徴および予後に関して他の2群と比較検討した. 年齢, 性別, 肉眼型, リンパ節転移度, 脈管侵襲および組織学的進行度では3群間に有意差は認められなかった. 腫瘍径ではA群はC群に比べ縦径は有意に短く (P

Recently colorectal carcinomas have been increasingly reported, multiple carcinomas of the colon and rectum are not rare, and the hereditary factor seems to be involved in the etiology of such multiple occurrence. Thirty-seven cases of multiple colorectal cancers experienced at the department in a recent 14-year period were clinicopathologically studied. Of these 37 patients, seven patients (15 lesions) had a family history of colorectal cancer within the first degree relatives (positive group) and the remaining 30 patients (65 lesions) had no family history (negative group). Patients in the positive group were younger than those in the negative group. Carcinomas of the positive group were located in more prpximal colon than those of the negative group. Histological examination revealed that most of the first lesions in the positive group were moderately differentiated adenocarcinomas while the second lesions were commonly well differentiated adenocarcinomas. Presence of associated adenomatous polyps was more common in the negative group than in the single carcinomas. The cumulative 5-year survival rate was 62.5% in the positive group and 42.0% in the negative group. All patients in the positive group satisfied the diagnosis criteria of HNPCC (hereditary nonpolyposis colorectal cancer), but the negative group may include initiator of HNPCC.

過去16年間に当科で手術を施行した75歳以上の高齢者大腸癌は60例で,50歳以上70歳未満の対照者群319例,39歳以下の若年者群18例の内,主に単発癌を比較の対象とし臨床的特徴と遠隔成績を検討した.高齢者群は,(1)占拠部位では右側結腸に有意に多かった.(2)壁深達度は,漿膜内にとどまる症例が多く比較的浅い傾向.(3)リンパ節転移陽性率は低い傾向.(4)手術直接死亡例はないが他病死例が多く,他病死を除いたら5年生存率は対照者群と比べて差はなかった.高齢者大腸癌は,壁深達度は浅く,リンパ節転移率も低い傾向にあり,D2もしくはD3の適切なリンパ節郭清にて根治度AまたはBの切除が可能であり,高齢といえども患者の状態の許す限り積極的に対照者群と同様の郭清を伴った根治切除術を心がけるべきである.高齢者では術前より全身状態の悪い症例が多く,合併疾患の術前後の管理を充分に行えば対照者群と同等の成績が期待できる.

Benign papillary mesothelioma of the peritoneum is an uncommon lesion that is usually discovered by chance during a surgical procedure. This lesion resembles metastatic carcinoma in gross appearance; therefore, intraoperative diagnosis can be difficult. This report presents a case of benign papillary mesothelioma concurrent with gastric cancer. The tumor was located on the hepatogastric ligament and resembled a metastatic peritoneal implant. A pathological review confirmed the diagnosis to be benign papillary mesothelioma.

大腸癌症例中痔核を合併または過去に指摘された患者は肛門出血が生じた際に症状を癒核によると考えて癌の病悩期間が延長する可能性がある.今回肛門出血を主訴とする大腸癌症例で癒核の既往がある症例とない症例の病悩期間と予後を比較した.病悩期間は痔核の既往がある症例で平均7.9か月, ない症例では平均5.2か月と既往がある症例で長くなる傾向を認めた.病悩期間を3か月ごとに区切ると痔核の既往がない症例では3か未満が52.9%, ある症例で29.6%を占めた.Stage I, II症例では病悩期間が3か月未満で3年生存率が85.7%3か月以上で49.8%, Stage III, IV, V症例では3か月未満で75.9%3か月以上で32.4%であり有意差を認めた.病悩期間と生存率に関連性はないという意見もあるが, 今回の検討では肛門出血を主訴とする症例に関して病悩期間の短縮は予後を改善する可能性があり痔核患者の経験観察は慎重に行う必要がある.

Tailgut cysts are rare congenital lesions. To date, only four cases have been reported in Japan, and the occurrence of a tailgut cyst with rectal cancer has never been documented. We describe here the case of a patient in whom a tailgut cyst in the retrorectal space was associated with rectal cancer. Preoperative computed tomography scans and endorectal ultrasonography failed to identify the lesion as cystic, instead suggesting an involved lymph node. This case emphasizes the necessity for careful diagnosis of masses in the retrorectal space in patients with rectal cancer.

A small cell carcinoma of the extrahepatic bile duct in a 75-year-old Japanese man is reported. The patient suffered from obstructive jaundice, and percutaneous transhepatic cholangiography-drainage (PTCD) revealed a massive lesion in the lower common bile duct. Because it was diagnosed as a malignant tumor, pancreaticoduodenectomy was performed. A nodular infiltrating tumor measuring 4.5 x 3.0 x 2.0 cm was located in the intrapancreatic portion of the extrahepatic bile duct. Histologically, the tumor was composed of a dense proliferation of small atypical cells with a little region of high-grade dysplasia in the adjacent epithelium of the common bile duct. Tumor cells were immunoreactive to neuroendocrine markers such as chromogranin A, synaptophysin, CD56, and Leu7. Although carcinoma cells invaded into pancreas and duodenum, there were no histological findings that indicated the carcinoma arose from the mucosa of either the pancreatic duct or duodenum. These results indicated that the tumor was a small cell carcinoma derived from the epithelium of the extrahepatic bile duct; a rare neoplasm with only a few cases reported. A few neuroendocrine cells were recognized in the adjacent epithelium of the extrahepatic bile duct, suggesting that the tumor cells might be derived from them. Using immunohistochemical examination, no p53 abnormality was found. Tumor cells showed positive nuclear staining for p16, while negative for cyclin D1, suggesting that functional retinoblastoma protein (pRB) might be lost in the p16/pRB pathway, as in small cell lung cancer.

Inflammatory fibroid polyps (IFPs) are rarely found in the gastrointestinal tract. The majority of IFPs are sessile-pedunculated or pedunculated polypoid lesions, whereas a polyp presenting like a Borrmann type II lesion is extremely unusual. This report describes the case of a 74-year-old man with a history of intussusception, in whom a preoperative diagnosis of a cecal tumor of the ileocecal valve was made. A laparotomy subsequently revealed a lesion similar to a Borrmann type II tumor located 15 cm above the ileocecal valve, but not at the valve. The lesion was diagnosed as an IFP which had been caused by repeated colostomy irrigation. The aim of the present report is to draw attention to this entity, which should be included in the differential diagnosis of intussusception and small bowel obstruction.

週3回・1回3時間以内の透析を短時間透析と定義し, 短時間透析希望者全員に対して短時間透析を行い検討した.ダイアライザーは男性にRENAK A-23 H (2.3m2), 女性にRENAK A-15 H (1.5m2) を用い, 血流を200ml/min以下で短時間透析を行った. 1年以上経過した症例の透析前血液生化学検査値, 死亡した症例の死因について検討した. 血液透析患者58例中56例が短時間透析を希望し, 54例 (希望者の96.4%) に施行した. 短時間透析施行1年後の透析前血液生化学検査はBUN 79.2±15.6mg/dl, Cr 13.0±3.20mg/dl, K 5.02±0.94mEq/l, Ht 20.6±3.70%, time-averaged BUN (TACurea) 58.9±11.3mg/dlで, 普通透析 (5時間透析) 時に比較し1%以内の危険率で有意差を認めなかった. 短時間透析施行1年後の透析前心胸比 (CTR) は55.9±6.30%で, 普通透析時に比較し1%以内の危険率で有意の増加を示した. 死亡15例中3例が透析との因果関係が推測されたが, 短時間透析に起因するものではなかった. 短時間透析に移行できなかった症例は大量水分摂取1例, 血流不足1例であった.我々の血液生化学的データからは, 血液生化学的に短時間透析がほぼ可能であると思われた. ただCTRの有意の増加が見られたことは今後の検討が必要と思われる.
