A 1996 project examined transition from "special schools" to postsecondary education in the United Kingdom. Students were interviewed concerning their feelings about their change of circumstance and environment, and parents were surveyed about their optimal scenario for the change. A mentoring model to facilitate the transition and a process to enhance student progression and provide educational continuity were designed. (Author/MSE)

The food safety issues of the high pressure pasteurized fruit/vegetable juices are the focus of this paper. The development of Hazard analysis and critical control points system for production of carrot, red beet and apple/broccoli/orange juice is given together with results of analysis of samples, challenge testing and storage tests. The anti-mutagenic activity in vitro tests are also presented. Procedures have been established for the start-up of experimental production of mentioned juices for the Czech food market. The development of other types of juices based on cruciferous vegetables is mentioned. The bias stability of zinc-tin-oxide (ZTO) thin-film transistors (TFTs) with either Al2O3 gate dielectrics deposited via atomic layer deposition (ALD) or SiO2 gate dielectrics deposited via plasma-enhanced chemical vapor deposition (PECVD) was compared. Both device types showed incremental mobility ≥11 cm2/V s, subthreshold slopes <0.4 V/dec, and ION/IOFF ratios of ∼107. During repeated ID-VGS sweeping, both device types showed positive parallel shift of the turn-on voltage (VON) without significant degradation of subthreshold slope or mobility, consistent with electron trapping without creation of new traps. A smaller VON shift was observed in the SiO2/ZTO devices. In an effort to improve the bias stress stability of the Al2O3/ZTO devices, the impact of ALD temperature, plasma exposure of the Al2O3, and the addition of an interfacial PECVD SiO2 capping layer were investigated. The positive bias stress stability of the Al2O3/ZTO TFTs was found to be relatively unaffected by the Al2O3 ALD temperature, degraded with plasma exposure, and improved by the addition of a thin (∼3 nm) PECVD SiO2 interfacial layer between the Al2O3 dielectric and the ZTO channel. These results point to the vicinity of the Al2O3/ZTO interface as the dominant source of charge trapping. [ABSTRACT FROM AUTHOR]

Polycyclic aromatic hydrocarbons (PAHs) were analysed in mosses (Hypnum cupressiforme) and pine needles (Pinus sylvestris) collected in the Czech Republic between 1988/94 at a regional background site in Kosetice, south Bohemia (1988/94) and two industrial sources. One industrial site (sampled 1989/91) in middle Moravia, was near a factory producing PAHs, carbon black and phthalates, the other (sampled 1991/93) near a coal and gas fuel production plant in western Bohemia. Selected chlorinated pesticides and polychlorinated biphenyl congeners were also analysed in samples at the regional background site. This study clearly shows that vegetation sampling can be used to show spatial differences in the atmospheric burden of a range of persistent organic pollutants with differences in the mixtures of compounds reflectingdifferences in their regional or local use/atmospheric emission. We studied an effect of elevated atmospheric CO2 on rhizosphere microorganisms in a hydroponics system where young wheat plants provided the only source of C for microorganisms. Plants were cultivated in mineral solution in sterile silica sand and exposed to control (ambient) and elevated (double) CO2 concentrations for periods of 13, 20, 25 and 34 days. Microbial biomass C (C content in fraction of size 0.3--2.7 mu m) was not affected by the elevated CO2 concentration during the first 25 days of plant growth andwas increased after 34 days of plant growth. A content of poly-beta-hydroxybutyrate (PHB) reserve compounds (measured as derivatized product of 3-hydroxy-butyric acid and N-tert-butyldimethylsilyl-N-methyltrifluoracetamid e using GC-MS) was lowered significantly (p <0.001) in the elevated CO2 after 25 and 34 days. It was accompanied with a shift of bacterial distribution towards the nutritional groups utilising more complex organic material (number of CFUs on media with different sources of C and N). A coincidence of several events connected with plant and microbial carbon economy (decrease of an assimilation rate and relative growth rate of plants, small increase of microbial biomass, PHB decrease and suppression within the bacterial nutritional group requiring the most readily available source of C and energy) was observed in the system under elevated CO2 on the 25th day. A modification of the GC-MS method for the detection of low levels of PHB compounds in natural samples was developed. We excluded the lipids fractionation step and we used EI MS/MS detection of the main fragment ions of the derivatized compound. This guarantees that the ion profiles have high signal-to-noise ratio at correct retention time. The detection limit is then about 30 pg g-1 of sand or soil. Aims: This review summarizes the findings of preclinical studies evaluating the pleiotropic effects of ticagrelor. These include attenuation of ischemia-reperfusion injury (IRI), inflammation, adverse cardiac remodeling, and atherosclerosis. In doing so, it aims to provide novel insights into ticagrelor's mechanisms and benefits over other P2Y 12 inhibitors. It also generates viable hypotheses for the results of seminal clinical trials assessing ticagrelor use in acute and chronic coronary syndromes., Methods and Results: A comprehensive review of the preclinical literature demonstrates that ticagrelor protects against IRI in the setting of both an acute myocardial infarction (MI), and when MI occurs while on chronic treatment. Maintenance therapy with ticagrelor also likely mitigates adverse inflammation, cardiac remodeling, and atherosclerosis, while improving stem cell recruitment. These effects are probably mediated by ticagrelor's ability to increase local interstitial adenosine levels which activate downstream cardio-protective molecules. Attenuation and augmentation of these pleiotropic effects by high-dose aspirin and caffeine, and statins respectively may help explain variable outcomes in PLATO and subsequent randomized controlled trials (RCTs)., Conclusion: Most RCTs and meta-analyses have not evaluated the pleiotropic effects of ticagrelor. Purpose: Outcomes from randomized controlled trials (RCTs) inform the latest recommendations on percutaneous coronary intervention (PCI) management of a short period of oral anticoagulation (OAC), a P2Y 12 receptor inhibitor, and aspirin for 1 week or until hospital discharge in patients with atrial fibrillation (AF) undergoing PCI, and up to 4 weeks in individuals considered to be at high-risk for ischemic events, followed by discontinuation of aspirin and continuation of OAC and a P2Y 12 inhibitor for up to 12 months., Methods: We examined and summarized the outcomes of bleeding and major adverse cardiac events (MACEs) from RCTs and meta-analyses, published between 2013 and 2022, comparing therapy with OAC and a P2Y 12 inhibitor with and without aspirin in AF patients undergoing PCI with stenting., Results: Data comparing dual therapy with OAC and a P2Y 12 inhibitor alone to triple therapy with OAC, a P2Y 12 inhibitor, and aspirin with respect to the risks of MACEs, including stent thrombosis within the first 30 days, are underpowered and inconclusive. The addition of aspirin does not appear to be associated with a decreased risk of ischemic events, even in patients with high-risk CHA 2 DS 2 -VASc scores, but does significantly increase bleeding hazards. The increased safety of newer generation drug-eluting stents may have further minimized any theoretical anti-ischemic benefits of aspirin. The possible attenuation of the pleiotropic effects of concomitant cardiovascular medications by aspirin may also have been a contributing factor., Conclusion: The addition of aspirin to OAC and a P2Y 12 inhibitor is likely associated with a net clinical harm in patients with AF who undergo PCI with stenting, even within the first 1-4 weeks after PCI. Background: Diagnosis of left circumflex artery (LCx) myocardial infarctions via 12‑lead electrocardiogram (ECG) has posed a challenge to healthcare professionals for many years., Methods and Results: A retrospective observational study was performed to analyze patients admitted with myocardial infarction. The study used electronic medical records and specific ICD-10 codes to identify eligible patients, resulting in 2032 encounters. After independent adjudication of cardiac biomarkers, coronary angiography, and electrocardiographic changes, a final patient population of 58 encounters with acute occlusion myocardial infarction (OMI) with a culprit LCx lesion was established. OMI was defined as a lesion with either thrombolysis in myocardial infarction flow (TIMI) 0-2 or TIMI 3 with Troponin I > 1 ng/mL (Reference range 0.00-0.03 ng/mL). ECGs of these patients were then independently evaluated and grouped into 8 different classifications based on the presence or absence of ST elevation and/or depression in corresponding leads. ECG patterns and anatomical characteristics (proximal or distal to the first obtuse marginal artery) of the LCx lesions were then correlated. The appropriateness of triage and delay in reperfusion therapy were also assessed. Those with a left dominant or codominant circulation, and with LCx lesions proximal to the first obtuse marginal artery, were more likely to present with no or subtle ST-segment changes that led to delays in reperfusion therapy., Conclusions: Patients with left or codominant coronary artery circulation, with OMI proximal to the first obtuse marginal artery, may be less likely to have "classic" findings of ST-segment elevation on ECG due to cancellation forces in the limb leads., Competing Interests: Declaration of Competing Interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (Published by Elsevier Inc.)

Purpose: The 2021 European Society of Cardiology guidelines on acute and chronic heart failure (HF) recommend that non-dihydropyridine calcium channel blockers (NDCC) should be avoided in patients with HF with reduced ejection fraction. It also emphasizes that beta-blockers only be initiated in clinically stable, euvolemic patients. Despite these recommendations, NDCC and beta-blockers are often still employed in patients with AF with rapid ventricular response and acute decompensated HF. The relative safety and efficacy of these therapies in this setting is unclear., Methods: To address the question of the safety and efficacy of NDCC and beta-blockers for acute rate control in decompensated HF, we provide a perspective on the literature of NDCC and beta-blockers in chronic HF with reduced and preserved ejection fraction and AF, including trials on the management of AF with rapid ventricular response with and without HF., Results: Robust data demonstrates mortality benefits when beta-blockers are used in patients with chronic HF with reduced ejection fraction. The data that inform the contraindication of NDCC in HF with reduced ejection fraction are outdated and were not primarily designed to address the efficacy and safety of rate control of AF in patients with HF. Several studies indicate that for acute rate control, NDCC and beta-blockers are both efficacious therapies, especially in the setting of tachycardia-induced cardiomyopathy., Conclusion: Future studies are needed to assess the safety and efficacy of beta-blockers and NDCC in both acute and chronic AF with HF with reduced and preserved ejection fraction., (© 2022. A patient with right bundle branch block (RBBB) presented with chest pain. An ECG showed ST-elevation in leads V1, V2, and aVR, with widespread ST-depression in leads II, aVF, I, aVL, and V4-6. The initial ECG interpretation missed ST-elevation myocardial infarction (STEMI), as ST-elevation thresholds were not reached. Non-urgent angiography showed severe left anterior descending artery stenosis requiring percutaneous coronary intervention. The course was complicated by cardiac arrest necessitating resuscitation and dual chamber pacemaker placement with left bundle branch pacing. Background: Hepatocellular carcinoma (HCC) is a common cancer worldwide. Extrahepatic spread is not unusual during HCC disease, but bone metastases at initial presentation are rare., Case Description: We describe a case of HCC presenting with a clavicular head mass and spinal metastases with normal α-fetoprotein (AFP) level and hepatitis C virus infection without cirrhosis. After undergoing bone and liver biopsies, the patient started a 12-week course of sofosbuvir/velpatasvir and bevacizumab/atezolizumab for lifelong therapy with palliative intent. Since 2021, the patient has been receiving a combination of bevacizumab and atezolizumab every 21 days. On this regimen as of March 2023, his osseous metastases were stable and his liver lesions had not enlarged., Conclusions: This case demonstrates a very unusual HCC presentation, the importance of a thorough workup of bone metastasis, and the limited value of AFP for HCC screening, even in disseminated disease.

Much attention has been paid lately to harnessing the diagnostic and therapeutic potential of non-coding circular ribonucleic acids (circRNAs) and micro-RNAs (miRNAs) for the prevention and treatment of cardiovascular diseases. The genetic environment that contributes to atherosclerosis pathophysiology is immensely complex. Any potential therapeutic application of circRNAs must be assessed for risks, benefits, and off-target effects in both the short and long term. A search of the online PubMed database for publications related to circRNA and atherosclerosis from 2016 to 2022 was conducted. These studies were reviewed for their design, including methods for developing atherosclerosis and the effects of the corresponding atherosclerotic environment on circRNA expression. Investigated mechanisms were recorded, including associated miRNA, genes, and ultimate effects on cell mechanics, and inflammatory markers. The most investigated circRNAs were then further analyzed for redundant, disparate, and/or contradictory findings. Many disparate, opposing, and contradictory effects were observed across experiments. These include levels of the expression of a particular circRNA in atherosclerotic environments, attempted ascertainment of the in toto effects of circRNA or miRNA silencing on atherosclerosis progression, and off-target, cell-specific, and disease-specific effects. The high potential for detrimental and unpredictable off-target effects downstream of circRNA manipulation will likely render the practice of therapeutic targeting of circRNA or miRNA molecules not only complicated but perilous.

The clinical significance of right bundle branch block (RBBB) or bifascicular block (BFB) in the setting of acute myocardial infarction (AMI) is uncertain. RBBB was found in 211 of 7,626 patients (2.8%), presenting to the ED (emergency department) with chest pain, of which 18 (8.5%) also had acute coronary syndrome (ACS). Incidences of ACS were not significantly different between new or presumed new RBBB and prior known RBBB or new or presumed new BFB and prior known BFB. In 2 patients, baseline ST-segment depression in leads V1-3 masked anterior ST-elevation detected on electrocardiogram (ECG). In opposition to the guidelines, the presence of RBBB or BFB does not appear to offer any clinical utility when evaluating patients with suspected AMI. Patients with suspected AMI who present with RBBB and any ST-elevation in leads V1-3 should be considered for emergent coronary angiography rather than RBBB alone., (Copyright © 2022 Elsevier Inc. The blood-brain barrier prevents the passage of many drugs that target the central nervous system. This paper presents the preparation and characterization of silica-based nanocarriers loaded with piracetam, pentoxifylline, and pyridoxine (drugs from the class of nootropics), which are designed to enhance the permeation of the drugs from the circulatory system through the blood-brain barrier. Their permeation was compared with non-nanoparticle drug substances (bulk materials) by means of an in vivo model of rat brain perfusion. The size and morphology of the nanoparticles were characterized by transmission electron microscopy. The content of the drug substances in silica-based nanocarriers was analysed by elemental analysis and UV spectrometry. Microscopic analysis of visualized silica nanocarriers in the perfused brain tissue was performed. The concentration of the drug substances in the tissue was determined by means of UHPLC-DAD/HRMS LTQ Orbitrap XL. It was found that the drug substances in silica-based nanocarriers permeated through the blood brain barrier to the brain tissue, whereas bulk materials were not detected in the brain.

The quality of medical plants used for the production of galenics or pharmacologically useful compounds is usually assessed by the content of biologically active compounds. Because most of these plants are grown in fields, this study focused on stimulation of active compounds by in vivo elicitation. Foliar application of elicitors on the immunostimulating medical plant purple coneflower ( Echinacea purpurea L. Moench.) grown on soil was used to increase the content of biologically active phenolics. Natural plant stress mediators and their derivatives (acetylsalicylic acid, salicylic acid, and methyl salicylate) as well as newly introduced biocompatible metal elicitor [titanium(IV) ascorbate] were chosen as active components of foliar sprays. A tremendous increase of phenolics (up to 10 times compared to control) and stimulation of the biomass yield were achieved. Tuning of organ specificity by modulation of the concentration of elicitor was also observed. This methodology represents a convenient alternative to cell suspension or hydroponic cultures being applicable in wide agricultural practice.

While the metabolism of polychlorinated biphenyls (PCBs) in plant cells is a rarely studied field, hydroxy-PCBs have been detected in several studies involving the use of various plant species. The ability of the tobacco (Nicotiana tabacum) callus culture WSC-38 to metabolize six dichlorobiphenyls under aseptic conditions was studied, and the resulting PCB metabolites were analyzed. WSC-38 cultures were cultivated with individual dichlorinated PCB congeners. The metabolites were identified based on mass spectra characteristics after gas chromatography separation. In addition, metabolites of PCB 9 (2,5-dichlorobiphenyl) were identified by comparing their retention characteristics with the available standards. In most cases at least two hydroxy-PCBs were produced from each parent PCB. Methoxy-PCBs and hydroxy-methoxy-PCBs were other groups of metabolites produced. To the best of our knowledge, ours is the first report to determine the presence of methoxy- and hydroxy-methoxy-metabolites of PCBs in plants. The role of the O-methyltransferases (OMTs) in the methylation of hydroxy-PCBs is discussed. As methoxy-metabolites of acetophenone were found among our samples, we posit that the OMTs responsible for the methylation of these compounds are also involved in the metabolism of PCBs in cultures of WSC-38.

The present study is intended to determine metabolites of 12 dichlorinated, seven trichlorinated, five tetrachlorinated and one pentachlorinated PCB congener transformed by black nightshade (Solanum nigrum) hairy root culture SNC-9O. Free hydroxylated PCB metabolites were identified based on the mass spectra characteristics after gas chromatography separation. The number of metabolites decreases with an increasing number of chlorine atoms per molecule of PCB. Dichlorinated PCBs lead always to at least two metabolites. In the case of PCB 9 some metabolites could be identified by comparing their RF values due to available standards. The 2',5'-dichloro-2-biphenylol, 2',5'-dichloro-3-biphenylol and 2',5'-dichloro-4-biphenylol, present as the main metabolite, were found in biomass of SNC-9O hairy root culture. Two monochlorinated biphenylols were found in biomass of SNC-9O degrading PCB 9 congener. It was the only case when metabolites with decreased number of chlorine atoms compared to parent PCB were found. Trichlorinated PCBs mostly lead to a lower number of metabolites but tetrachlorinated and pentachlorinated PCBs mostly did not give any metabolites. In the media, only traces of metabolites were found in sporadic cases, so exudation of unbound biphenylols from the cells is not expected.

Root exudates of the common buckwheat, especially phenolic compounds, were studied. Their contents, both in the soil during the growing season and in agar medium during germination, were determined by HPLC and GC-MS. The allelopathic activity of the soil from a buckwheat stand was evaluated, as well. Palmitic acid, squalene, epicatechin, vitexin, a gallic acid derivative, and a quercetin derivative were the main compounds of the agar medium. In the soil, palmitic acid methyl ester, vanillic acid, rutin, a gallic acid derivative, and a 4-hydroxyacetophenone derivative were identified. The effects of vitexin, squalene, epicatechin, 4-hydroxyacetophenone, and vanillic and gallic acids were tested on eight plant species. Inhibitive effects were observed in the cases of 4-hydroxyacetophenone and vanillic and gallic acids. Comparisons of the identified compounds and inhibitive effects of soil extracts indicated that palmitic acid and the gallic acid derivative probably have an important function in the allelopathic root response of buckwheat.

Buckwheat leaves and young parts of the plant are consumed in some countries as a vegetable. Green flour, obtained by milling of the dried plants, is used as a natural food colorant. The distribution of vitamin E, squalene, epicatechin, and rutin (as the most important antioxidants) within buckwheat plants, as well as changes of their content within leaves during the growing season, were determined by GC-MS and HPLC analyses. alpha-Tocopherol was found as the main component of vitamin E in all parts of the plant; epicatechin and squalene were also detected. For the use of buckwheat as an antioxidant source in the human diet, the most suitable part of the plants seems to be the leaves and the flowers at the stage of full flowering due to the considerable amounts of rutin and epicatechin. alpha-Tocopherol content correlates positively with temperature, drought, and duration of solar radiation. Certain differences appear among varieties of buckwheat, especially in their squalene and rutin contents.
