La sarcoïdose est une granulomatose multisystémique rare touchant les adultes jeunes. L'atteinte cardiaque de la sarcoïdose, parfois inaugurale, est associée à un pronostic défavorable. Malgré une amélioration des connaissances sur la prise en charge de la sarcoïdose cardiaque, les recommandations récentes montrent que les données, notamment concernant les traitements, restent limitées, notamment en raison de la rareté de la maladie. L'objectif de ce travail était de décrire la présentation clinique, biologique et radiologique de l'atteinte cardiaque de sarcoïdose, et de décrire les traitements proposés dans les années récentes. Il s'agissait d'une étude rétrospective menée entre Janvier 2014 et Septembre 2024. Les critères d'inclusion étaient la présence d'une sarcoïdose définie selon les critères du WASOG avec une documentation histologique et une atteinte cardiaque retenue selon les recommandations (ref 1). Les données cliniques, biologiques, radiologiques, et de suivi ont été collectées. Un total de 48 dossiers de patients remplissant les critères d'inclusion ont été analysés : 30 hommes (63 %) d'âge moyen au diagnostic de sarcoïdose de 44,8 ans (écart type : 14,4 ans). Neuf patients (19 %) avaient une cardiopathie ischémique associée, et 14 (25 %) une intoxication tabagique sevrée ou active. Près de trois quarts des patients avaient une atteinte médiastinale ou parenchymateuse pulmonaire associée (n = 34, 72 %), suivies par une atteinte ganglionnaire (n = 23, 49 %) et hépatique ou splénique (n = 11, 23 %). Dix patients (21 %) avaient une hypercalcémie. L'atteinte cardiaque était diagnostiquée à un âge moyen de 50,5 ans (écart type : 12,9 ans). Elle était inaugurale de la maladie chez 20 patients (42 %), ou survenait avec un délai moyen de 68 mois (écart type : 165 mois) après le diagnostic. Les signes cliniques étaient dominés par la dyspnée (n = 21, 46 %) et les palpitations (n = 16, 35 %). L'électrocardiogramme était anormal chez 40 patients (89 %) avec principalement des troubles de conduction. Le traitement de l'atteinte cardiaque a consisté en une corticothérapie pour 79 % des patients (chez les 21 % restants, l'absence de corticothérapie était liée à une contre-indication, ou à une atteinte cardiaque mise en évidence à un stade tardif, sans signe d'activité), combinée à un traitement immunosuppresseur comportant pour la moitié des patients du méthotrexate et pour 16 patients de l'infliximab. Un défibrillateur implantable était indiqué pour 21 patients (48 %) et 2 patients ont bénéficié d'une transplantation cardiaque. Les biomarqueurs cardiaques étaient élevés (troponinémie moyenne : 75,5 ng/L, taux de NT-proBNP moyen : 713 ng/L). L'échocardiographie transthoracique objectivait une fraction d'éjection ventriculaire gauche moyenne à 47 % à la faveur d'une cardiomyopathie hypertrophique (n = 12, 48 %) ou dilatée (n = 10, 40 %). L'IRM cardiaque confirmait la discrète altération de la fonction ventriculaire gauche, mais également droite (fraction d'éjection ventriculaire droite moyenne à 36 %). Il existait un rehaussement tardif chez 80 % des patients. La tomographie par émissions de positons montrait un hypermétabolisme cardiaque dans 55 % des cas qui, quand il était présent, était associé à une fixation extracardiaque dans trois quarts des cas. La biopsie endomyocardique, réalisée chez 10 patients, offrait une rentabilité diagnostique de 70 %. Deux patients sont décédés subitement, alors qu'il étaient traités, sans autopsie réalisée. L'atteinte cardiaque de sarcoïdose est une atteinte souvent inaugurale, et sévère, avec une dysfonction ventriculaire associée fréquente. Les anticorps anti-tumor necrosis factor-α (anti-TNF-α), en particulier l'infliximab, sont actuellement proposés comme médicaments de troisième ligne dans le traitement de la sarcoïdose. Malgré cela, certains patients ne s'améliorent pas ou rechutent au cours du suivi. Une explication pourrait être le développement d'anticorps anti-infliximab. Nous avons évalué l'incidence et l'impact des anticorps anti-infliximab dans la sarcoïdose avec au moins une atteinte extra-thoracique, chez les patients traités par infliximab. Il s'agissait d'une étude rétrospective menée dans un centre entre janvier 2015 et décembre 2023. Les critères d'inclusion étaient (1) la présence d'une sarcoïdose extrathoracique documentée histologiquement (critères WASOG) ; (2) un traitement par inflixmab ; (3) au moins une détermination d'anticors anti-infliximab au cours du suivi. La détermination des anticorps antiinfliximab était réalisée par le même laboratoire, en même temps que le dosage résiduel d'infliximab, après au moins 2 perfusions de traitement, et juste avant une injection. Parmi 91 patients ayant une sarcoïdose extrathoracique traitée par infliximab, 51 dossiers de patients remplissant les critères d'inclusion ont été inclus : 35 hommes (69 %), d'âge median au diagnostic de sarcoïdose 39 ans interquartile [33 ; 47], avec en médiane 4 organes atteints [2-4] et une forte prévalence des atteintes neurologiques (n = 35, 69 %) et cardiaque (n = 10, 20 %). Cinquante patients sur 51 (98 %) avaient reçu des corticostéroïdes avant l'initiation de l'infliximab et 48 (94 %) avaient été traités avec un agent immunosuppresseur avant l'introduction d'infliximab. Le temps de suivi médian entre l'initiation de l'infliximab et la détermination de la présence d'anticorps anti-infliximab était de 1,6 ans [0,67 ; 3,75]. La durée médiane (IQR) de traitement par infliximab était de 3,8 ans [3,04 ; 7,33]. L'évolution clinique sous infliximab, évaluée par l' "extrapulmonary physician organ severity tool score" (ePOST) et le "clinical outcome status" (COS) était significativement favorable pour l'ensemble de la cohorte (p < 0,001). Vingt-cinq patients (49 %) avaient une diminution de la charge lésionnelle supérieure à 50 %, évaluée par le Physician global assessment (PGA) en fin de suivi. Parmi les 26 patients qui ont été évalués par 18F-fluoro-desoxyglucose tomographie par émission de positrons (18FDG-TEP-scanner), à l'introduction de l'infliximab et en fin de suivi, 25 (96 %) se sont améliorés. Neuf patients (18 %) ont développé des anticorps anti-infliximab au cours du suivi. Il n'y avait pas de différence comparativement au patients sans anticorps anti-infliximab sur les caractéristiques initiales, les traitements reçus avant ou pendant l'infliximab (en particulier le pourcentage de patients ayant un immunosuppresseur et une corticothérapie associée à l'infliximab), ni les scores d'activité au cours du suivi. On notait toutefois dans le groupe avec anticorps anti-infliximab une utilisation du methotrexate dans 67 % des cas (et 83 % dans le groupe sans anticorps anti-infliximab). La concentration résiduelle d'infliximab était par ailleurs significativement plus basse chez les patients ayant des anticorps anti-infliximab positifs que chez ceux n'en ayant pas (0 μg/mL [0 ;0] et 13,6 μg/mL [5,4 ; 20,0]) restpectivement (p < 0,001). Vingt-trois patients (45 %) ont eu au moins une infection au cours du suivi. Neuf patients (18 %) ont développé des anticorps anti-infliximab au cours du suivi. Ces derniers ne semblent pas avoir d'impact sur l'évolution sous traitement par anti-TNF- α. Néanmoins, l'échantillon de petite taille et le suivi limité dans le temps nécessitent d'obtenir des éléments de suivi à plus long terme, d'autant que la concentration résiduelle d'infliximab était plus basse chez les patients ayant des anticorps anti-infliximab. Objective: To examine the course of interstitial lung disease associated with rheumatoid arthritis (RA-ILD) in France on treatment with Janus kinase inhibitors (JAKis) using the MAJIK-SFR registry., Methods: Prospective national multicentre observational study identifying patients with RA-ILD from the MAJIK-SFR registry. Pulmonary assessment data were collected at JAKi initiation and follow-up visits (6 months, 12 months and a median of 21 months postinclusion), including chest high-resolution CT (HRCT), pulmonary function tests (forced vital capacity (FVC) and diffusing capacity of the lungs for carbon monoxide (DLCO)), acute exacerbations of ILD, respiratory infections and lung cancers., Results: We enrolled 42 patients (26 women, 62%) with RA-ILD with a mean age of 61±13 years and a mean disease duration of 16±10 years. Compared with the 778 RA patients without ILD from the MAJIK registry, RA-ILD patients were older, displayed more severe and active disease and had more prevalent comorbidities. Non-specific interstitial pneumonia and usual interstitial pneumonia accounted for 46% and 43% of the chest HRCT ILD patterns, respectively. No significant changes in FVC and DLCO were observed during the follow-up period. Chest HRCT lesions remained stable in 69% of patients. Progressive ILD was identified in 8 patients (19%). 16 (38%) respiratory tract infections were observed. Only one acute regressive exacerbation of ILD was noted, and no lung cancer was diagnosed. No deaths occurred. JAKi was discontinued in 17 patients including 8 for inefficacy on joint involvement and 5 for intolerance., Conclusion: The analysis indicates stability of RA-ILD in patients treated with JAKi. Purpose: The outcomes of immunocompromised patients with cardiogenic shock treated with venoarterial extracorporeal membrane oxygenation (VA-ECMO) are seldom documented, making ECMO candidacy decisions challenging. This study aims (1) to report outcomes of immunocompromised patients treated with VA-ECMO, (2) to identify pre-ECMO predictors of 90-day mortality, (3) to assess the impact of immunodepression on 90-day mortality, and (4) to describe the main ECMO-related complications., Methods: This is a retrospective, propensity-weighted study conducted in two French experienced ECMO centers., Results: From January 2006 to January 2022, 177 critically ill immunocompromised patients (median (interquartile range, IQR) age 49 (32-60) years) received VA-ECMO. The main causes of immunosuppression were long-term corticosteroids/immunosuppressant treatment (29%), hematological malignancy (26%), solid organ transplant (20%), and solid tumor (13%). Overall 90-day and 1-year mortality were 70% (95% confidence interval (CI) 63-77%) and 75% (95% CI 65-79%), respectively. Older age and higher pre-ECMO lactate were independently associated with 90-day mortality. Across immunodepression causes, 1-year mortality ranged from 58% for patients with infection by human immunodeficiency virus (HIV) or asplenia, to 89% for solid organ transplant recipients. Hemorrhagic and infectious complications affected 39% and 54% of patients, while more than half the stay in intensive care unit (ICU) was spent on antibiotics. In a propensity score-weighted model comparing the 177 patients with 942 non-immunocompromised patients experiencing cardiogenic shock on VA-ECMO, immunocompromised status was independently associated with a higher 90-day mortality (odds ratio 2.53, 95% CI 1.72-3.79)., Conclusion: Immunocompromised patients undergoing VA-ECMO treatment face an unfavorable prognosis, with higher 90-day mortality compared to non-immunocompromised patients. Introduction: Systemic lupus erythematosus (SLE) is non-organ specific autoimmune disease with mainly skin, joint, and kidney involvement. SLE-related acute lung disease (ALD) is rare, poorly investigated and can lead to acute respiratory failure. We conducted a retrospective study aiming to describe clinical features, treatments and outcome of SLE-related APD., Methods: We retrospectively included all patients with SLE and ALD admitted from November 1996 and September 2018 to La Pitié-Salpêtrière Hospital, after exclusion of viral or bacterial lung infection, cardiac failure or any other alternate diagnosis., Results: During the time of the study, 14 patients with 16 episodes were admitted to our center: female 79%, mean age ± SD at admission 24 ± 11 years. ALD was inaugural of the SLE in 70% cases. SLE main organ involvement were: arthritis 93%, skin 79%, serositis 79%, hematological 79%, kidney 64%, neuropsychiatric 36% and cardiac 21%. 11 episodes required ICU admission for a median time of 8 days. Chest CT-scan revealed mostly basal consolidation and ground-glass opacities. When available, bronchoalveolar lavage mostly revealed a neutrophilic alveolitis with alveolar hemorrhage in 67% cases. Symptomatic respiratory treatments were: oxygen 81%, high-flow nasal canula oxygen 27%, non-invasive ventilation 36%, mechanical ventilation 64% and venovenous extracorporeal membrane oxygenation 18%. SLE-specific treatments were: corticosteroids 100%, cyclophosphamide 56% and plasma exchange 25%. All patients but one survived to ICU and hospital discharge. Two patients had a relapse of SLE-related ALD but none had interstitial lung disease during follow-up., Conclusion: Systemic lupus erythematosus-related acute respiratory failure is a severe event, mostly occurring at SLE onset, typical harboring a basal consolidation pattern on chest CT-scan and alveolar hemorrhage on BAL pathological examination. Mortality in our cohort is lower than previously reported but these results needs to be confirmed in further larger studies.

The mini mental state examination (MMSE) has become a benchmark for the screening and follow-up of cognitive impairment. The numerous translations of the MMS into other languages attest to its popularity. Clinical practice suggests that the consensual French version from the Greco (Groupe de réflexion sur les évaluations cognitives - Research working group for cognitive assessment) is not adapted to the West-Indies population because of the low socio-economic level and the widespread use of the Creole language among the elderly population. Modification of certain items by a multidisciplinary committee made it possible to adapt the instrument to the Creole culture. This procedure increases comprehension of the instrument, and should lead to improved detection of cognitive impairment in the West-Indies.

Considering 36 observations, the authors show the relative infrequency of pleural aspergillosis localizations by comparison with pulmonary aspergillomas. Twenty four pleural aspergilloses developed on post-operative residual cavities, particularly after pulmonary exeresis and not only for aspergilloma; 11 forms were apparently primitive after therapeutical pneumothorax; 3 anatomo-clinical forms were found, aspergillomas and pyothoraxes were the most frequent. Pleural involvement can be associated to pulmonary, even bronchial involvement. Discovery circulstances vary: 4 times symptomatology was noisy, other times diagnosis was founded on radiological changes or hemoptysis or on the systematic fungal detection. A major intervention with a radical aim (pulmonary decortication or pleuro-pneumonectomy) is rarely envisaged (7 times out of 35). Parietopleurectomy and rooting out with thoracoplasty are often the only possible ways or even a simple pleurotomy or a medical treatment with is modalities. Post-operative complications are often serious, particularly acute mycotic outbursts. Out of 35 patients, one was never seen again, 13 died, rather late of cardio-respiratory insufficiency, 21 are still alive but only 13 can truly be considered cured.
